本発明の方法には、免疫力が低下した非ヒト動物を用いる。非ヒト動物であって、免疫力を低下させることができ、癌細胞の移植のために全身麻酔が可能であれば、限定されず、マウス、ラット、モルモット等の種々の非ヒト動物を用いることができる。多数個体で実験できることから、マウスを用いることが好ましい。以下では、マウスを用いる場合を例に本発明やその実施態様を説明することがあるが、当業者であれば、その説明を他の非ヒト動物を用いる場合にも適宜当てはめて理解することができる。
非ヒト動物に移植される胃癌細胞は、移植される非ヒト動物と同種の動物由来のものであっても、異種由来であってもよい。ヒトの胃癌のモデルとするためには、ヒトの胃癌由来のものを用いることが好ましい。用いることのできる胃癌細胞の例は、一般に入手可能な、例えば理化学研究所バイオリソースセンター(BRC)等から入手できる、MKN1(日本人胃癌細胞株で腺上皮、扁平上皮の両方向へ分化することがある株。Regan型アルカリフォスファターゼを産生するという。)、MKN45(日本人胃癌細胞株で低分化型充実型腺癌由来。CEA高産生株。)、MKN7(日本人胃癌細胞株でc-erbB-2高発現株。Regan型アルカリフォスファターゼを産生するという。高分化型管状腺癌由来。)、MKN74(日本人胃癌細胞株で中分化型管状腺癌由来。Hypodiploid株だったもの)、NCC-StC-K140(ヒト胃癌由来細胞株。)、NUGC-4(ヒト胃がん由来細胞株。Signet ring cell carcinoma由来。)、ECC10(胃癌由来の小細胞癌。Creatine kinase-BB活性が高い。肺由来の小細胞癌とはやや性質が異なる。この株はc-mycの発現がある。)、ECC12(胃癌由来の小細胞癌。Creatine kinase-BB活性が高い。この株はaromatic L-amino-acid decarboxilase活性も高い。)、GCIY(胃癌(Borrmann IV型)。ムチン産生性。CA19-9, CEA, alphafetoprotein陽性。)、GSS(ヒト胃癌由来細胞株。肝臓転移巣から樹立。)、GSU(ヒト胃癌由来細胞株。腹水中の細胞より樹立。)、H-111-TC(ヒト胃癌由来細胞株。 TKG0411 (東北大学医用細胞資源センターからの寄託)。)、HGC-27(ムチン産生性胃癌細胞で、ヌードマウス可移植性である。)、HuG1-N(胃癌だがNagao型アルカリフォスファターゼを産生。CEA、CA19-9も検出されるという。増殖は浮遊性。HuG1-PIとは同一患者。)、HuG1-PI(胃癌だが、アルカリファオスファターゼ(腸型と胎盤型のヘテロダイマー)産生。HuG1-Nとは同一患者。)、KE-39(ヒト胃癌由来細胞株)、KE-97(ヒト胃癌の腹腔内(腸間膜)転移巣に由来する細胞株)、Kato III(ヒト胃癌由来細胞株。Signet ring carcinoma. TKG0213 (東北大学医用細胞資源センターからの寄託)。)、LMSU(ヒト胃癌由来細胞株。リンパ節転移巣由来。)、TKG0449 (東北大学医用細胞資源センターからの寄託)等が挙げられる。用いる胃癌細胞は、株化されたものに限られず、動物の腫瘍から採取したものであっても、それを継代培養したものであってもよい。あるいはまた、継代培養した胃癌細胞を皮下移植した動物から腫瘍を採取し、それを再度継代培養したものでもよい。
本発明の方法においては、非ヒト動物の胃の粘膜下層に、胃癌細胞の細胞懸濁液が注入される。従来、ヒト胃癌細胞株を免疫不全マウスに投与し生体内での癌細胞の動態を観察することは古くから行われているが、その際には、手技的簡便性からマウス皮下への投与が多く行われてきた。しかしながら、血流やリンパ流に依存する転移の状態や、ドラッグデリバリーを想定した場合、由来組織に準じた部位に癌細胞を移植することが有用である。
麻酔した仰臥位のマウスに対し、適切な位置で水平切開を行い、胃を静かに引き出す。次に、適切な細胞密度で必要量準備した胃癌細胞の懸濁液を粘膜下層に注入する。粘膜下層に注入されているか否かは、粘膜下層が比較的疎な組織であるため、注入する際に、プランジャ圧力の放出を感じることによって同定することができる。注入後、胃壁の注入部位に圧力を加えて、注入された懸濁液の漏れがないことを確認することができる。
このようにして得られたモデル動物は、胃癌の組織学的位置を正確に模倣するものである。従来の多くの研究では、胃癌細胞又は移植片は、部位を詳細には特定しない胃壁か、又は腹膜播種を達成するために漿膜層の近くに移植されてきた。漿膜下層(固有筋層と漿膜との間の層)への移植(前掲非特許文献2)では、粘膜への生着又は進展は起きにくい。固有筋層を超えて内向きには浸潤しにくいからである。また、漿膜下層には粘膜下層のような脈管がほとんどないため、転移が起こるとすれば、脈管を介して生じているものと考えられる。また漿膜下層への移植では、癌性腹膜炎が早期に起こり、転移成立の前に死亡する割合が上がると考えられる。本発明の方法により得られたモデル動物において腫瘍転移が生じやすいのは、粘膜下層の血管が移植された腫瘍細胞に曝される可能性が高いためであると考えられる。このような機能的な差異があることから、本発明の方法により得られたモデル動物と、従来の漿膜層付近に胃癌細胞を移植することにより得られたモデル動物は、作製方法が異なるのみならず、得られたモデル動物自体に差異があるといえる。
本発明の方法により作製された、胃癌細胞が生着している非ヒト動物を用いることにより、薬物のスクリーニングを行うことができる。好ましい態様においては、スクリーニングは、胃癌の転移を抑制する薬物のスクリーニングであり、より特定すると、胃癌の発生・進展過程におけるリンパ節・多臓器への転移を選択的に抑制する薬物のスクリーニングである。スクリーニングは通常、次の工程を含む:
作製された非ヒト動物を準備する工程;及び
作製された非ヒト動物に、候補薬物を投与する工程。
[細胞株及び薬物]
ヒト胃癌細胞株MKN45及びMKN74は理化学研究所細胞バンクから入手した。細胞を、10%ウシ胎仔血清及び5%CO2、湿度80%を補充したRPMI1640培地中、37℃で培養した。続いて、連続(3-5日)5-FU曝露の存在下で細胞を培養した後、約1年間、5-FU濃度を増加させた。経路のタイミング及び薬物濃度は、成長速度に基づいて調整された。ここで、MKN45及びMKN74細胞は、それぞれ5-FU耐性株MKN45/5FU及びMKN74/5FUとして確立された12。下記のコロニープロファイリング及びウェスタンブロットを除いて、我々はその後のすべての研究にMKN45/5FU細胞を使用した。増殖抑制及びコロニー形成アッセイを行って、以前に記載されているように細胞株の特性を確認した9。5-FUに加えて、CIS、DTX、ソラフェニブ及びゲフィチニブを、交差耐性を排除するためにアッセイに使用した。
増殖阻害アッセイのために、細胞を96ウェルマイクロタイタープレート中、10,000細胞/ウェルの密度で培養した。24時間インキュベートした後、薬物を希釈系列の各ウェルに添加した。4時間後、WST溶液を先に記載したように各ウェルに加えた(Dojindo)16,49,50。コロニー形成は、10倍段階希釈で薬物の非存在下又は存在下のいずれかで、6ウェルプレートの100細胞/ウェルの密度で行った。次いで、細胞を10-14日間インキュベートして、コロニー形成を可能にした。薬物を含まないウェル中のコロニー形成を確認した後、-20℃でメタノールでコロニーを固定し、続いてクリスタルバイオレット染色した。染色されたコロニーを300dpiでCanonフラットベッド光学スキャナー(Canon)でスキャンし、得られた画像を目視検査のために印刷した。1ミリメートル以上の直径を有するコロニーを計数した。すべての実験を少なくとも3回独立して繰り返した。
各々の薬物条件下で細胞を100細胞/ウェルの密度で6ウェルプレートに散布した。大きさが1ミリメートルを超えるコロニーを手で採取し、PBSで洗浄し、200μLの微量遠心チューブ中のPinkBuffer51,5210μlで溶解した。細胞破片を遠心分離によって除去し、上清(細胞溶解物)を384ウェルマイクロタイタープレートの各々のウェルに移した。各ライセートをニトロセルローススライド(GraceBioLabs)が搭載されたガラススライド上に転写することができるように、プレートをRPPAマイクロアレイヤー(AushonBioSystems)に装填した。ライセートの量は他のRPPAアプリケーションの場合よりもはるかに低く、したがってライセートよりも希釈されているため、膜中のより多くのタンパク質種を捕捉するために5回のプリンタ「ピンヒット」を使用した9。その後の免疫染色及びスキャニングは、以前に記載されたプロトコル9,18に従って実施した。
MKN45及びMKN45/5FU細胞懸濁液を、1.0×106細胞/100μL PBSの密度で個別に調製した。マウスを仰臥位に2%イソフルランガスで麻酔した。次いで、中央の線の約5mmから左側の肋骨弓の尾側の3mmから12mmの水平切開を行った。胃を静かに引き出し、左の人差し指で支えた。次に、癌細胞懸濁液を粘膜下層に注入した。粘膜層は、懸濁液を粗い組織結合層(すなわち、粘膜下層)に注入する際に、プランジャ圧力の放出を感じることによって同定された。注射後、胃壁の注射部位に圧力を加えて、注射された懸濁液の漏れがないことを確認した。開腹術後の癒着を防ぐために、各切開部を層々に縫合した。
LY294002(Sigma-Aldrich)53,54、ケルセチン(Abcam)55、ウォートマンニン(CellSignalingTechnology)56、GNE493(SYNキナーゼ)29及びGDC-0941(Abcam)33を含むPI3K活性を阻害することが知られている5つの化合物を、コロニー形成アッセイにおける最初のin vitroスクリーニング。GDC-0941を経口投与したOXモデルにおける腫瘍抑制アッセイのために選択した。
薬物をOXモデルマウスに静脈内(5-FU、Kyowa-HakkoBio)又は経口(GDC-0941、LCLaboratories)のいずれかで投与した。尾静脈から5-FU溶液(200μl、25mg/kg)を注入した。0.5%(w/v)ヒドロキシプロピルメチルセルロース(信越化学工業)及び0.2%(v/v)Tween-80(Sigma-Aldrich)に溶解したGDC-0941を150mg/kgの濃度で、28日間1日1回経口投与した57。
各腫瘍由来のDNAをQiagenDNAミニキット(Qiagen)を用いて抽出した。ゲノムDNAライブラリーは、IonPGM(ThermoFisherScientific)上のIonAmpliSeqCancerPanel(190個の領域について49個の遺伝子)から製造業者のプロトコルに従って構築した。標的遺伝子のリストは、http://www.lifetechnologies.com/order/catalog/prodct/4471262で入手できる。ゲノムビューア及びigvソフトウェアを備えたTorrentVariantCaller(バージョン4)を用いて、単一ヌクレオチド変異体を評価した。
全ての実験は、岩手医科大学動物実験学会倫理委員会(24週齢)のガイドライン承認に従って行われた。この実験では、6週齢-8週齢のヌードマウス(BALB/cAjcl-nu/nu)029)。
MKN45/5FU細胞をVECTOR-Luc4でトランスフェクトし、生物学的特性を親MKN45細胞と比較した。in vivo腫瘍成長は、IVISイメージングシステム(PerkinElmer)を用いてMKN45/5FU-Luc4でOXの画像を撮ることによって測定した。マウスに150mg/kgのD-ルシフェリンを注射し、次いで2%イソフルランガスで麻酔した。D-ルシフェリン注射の15分後、マウスをIVISイメージングシステムの撮影台に置いた。特に複数の病変が存在する場合に信号飽和を防止するために、実験に応じてスキャン時間を5-10分とした。
一次抗体を指示された希釈比率でインキュベートした:Phospho-PI3キナーゼp85(Tyr458)/p55(Tyr199)、1:100;ホスホ-AKT(Ser473)、1:75;ホスホ-mTOR(Ser2448)(49F9)、1:75;及びPTEN(138G6)、1:450(CellSignalingTechnology、Danvers、MA)。抗原回収(95℃で30分間のEDTA緩衝液pH9)の後、サンプルを一次抗体と共に室温で60分間インキュベートした。次いで、ペルオキシダーゼ標識抗ウサギ二次抗体(Histofine(登録商標)SimpleStainMAXPO、NichireiBiosciences、Inc.)を室温で30分間適用した。比色検出にはジアミノベンジジン(DAB)を用いた。抗p-AKT抗体が一次抗体である場合、抗原回収後の試料を4℃で一晩インキュベートし、続いて室温でペルオキシダーゼ標識抗ウサギ二次抗体との15分間のインキュベーションを行った。比色検出は、DAKOの触媒シグナル増幅(CSA)IIビオチンフリーチラミドシグナル増幅システム(AgilentTechnologies)を用いて行った。癌細胞の5%以上が染色された場合、試料を陽性染色と判定した。
MKN45及びMKN45/5FU細胞の試料ならびに周囲のマウス組織(>5mm)と巨視的かつ明確に区別される肝臓及び転移性OX腫瘍の原発性OX腫瘍を分析のために選択した。細胞ペレット又はWAXFREE(TrimGen)処理エタノール固定パラフィン包埋OX腫瘍材料からの鋳型DNAをQIADNAMiniキット(Qiagen)で抽出した。マウス組織による汚染を最小限に抑えるために、OX腫瘍を顕微鏡下で細切した。ゲノムコピー数を各サンプルについて約20,000に調整したQuantStudio3Dシステム(ThermoFisherScientific)でDNAをデジタルPCR反応に付した。特定の点突然変異は、コドン707突然変異(G>A)に特異的なTaqManMGBプローブで検出された。全ての手順は製造業者のプロトコルに厳密に従った。

ゲノムDNA(1μg)をQiagenQiampDNAマイクロキットを用いてMKN45及びMKN45/5FU細胞から抽出した。AgilentGenomicDNAEnzymaticLabelingKit(Cy3-dUTP)を用いてMKN45から抽出したDNAを標識し、MKN45/5FUのDNAをCy5-dUTPで標識した。標識されたDNAを、AgilentHumanGenomeCGHマイクロアレイキット244Kにハイブリダイズさせ、製造者のプロトコルに従って処理した。蛍光アレイ画像は、AgilentDNAマイクロアレイスキャナー(AgilentTechnologies)を用いて取得した。log2比は、ゲノムGC含量について1Mbp距離 GC含量波効果について補正した。異常値を平滑化し、正規化されたlog2比を、PartekGenomicsSuitev6.4(PartekInc.)における循環バイナリセグメンテーション分析を用いてセグメント化した。3つの基準を満たすセグメントを見つけるために用いられるゲノムセグメンテーションアルゴリズムは以下の通り:1)最適な統計的有意性を与えるためにブレークポイント(領域境界)を選択(P<0.001)。2)領域は最低10個のプローブをにより検出。3)「Gain」や「Loss」などのCNステータスを生成するために、相対コピー数が2.3以上1.7未満のセグメント。マイクロアレイデータセット全体は、データシリーズ受託番号GSE89191の下でhttp://www.ncbi.nlm.nih.gov/geo/info/linking.htmlから入手可能である。
JMP(SASInstitute、Inc.)又はGraphPadPrism(GraphPadSoftware、Inc.)ソフトウェアのいずれかを用いて統計解析を行った。群平均比較のために、スチューデントt検定又はマンホイットニーU検定のいずれかを使用した。0.05未満のp値は統計的に有意であると考えられた。
細胞を、5-FU及びGDC-0941の存在下で、単独又は一緒に、又はPI3K/mTOR阻害剤の存在下で、通常のインキュベーターにて、37℃で24時間、70%コンフルエンスまで増殖させた。薬物濃度は、増殖抑制アッセイから得られた48時間のGI50値に基づいて最適化した。本研究では、24時間薬物曝露に、各薬物の親細胞48時間でのおよそのGI75値を使用した。タンパク質を分子量に従って分離し、セミドライフォーマット装置(iBlot、Invitrogen)を用いてニトロセルロース膜に転写した。次いで、ニトロセルロース膜を特定の一次抗体とインキュベートし、続いてルシフェラーゼと結合した種特異的二次抗体をインキュベートした。その後、バンドを、製造者のプロトコルに従って、ルシフェリン基質を用いて可視化した。
[5-FU耐性癌細胞株の細胞増殖]
培養培地中の濃度が連続的に上昇する5-FUの存在下で親胃癌細胞株MKN45を1年間培養した後、いくつかの細胞は薬物の存在下で増殖を続けた12。この観察は、親細胞が薬物耐性細胞特性を誘導し得ること、又は集団が異なる薬物感受性を有し得ることを示唆する。得られた5-FU耐性細胞株MKN45/5FUはMKN45細胞と同様の形態を有し、MKN45とMKN45/5FUとは(GI50)とコロニー形成(CoI50)との間の50%阻害濃度において同様の傾向を示した(図1a)。5-FUに対するMKN45/5FUの特異的かつ高い耐性は、GI50(図1b)及びCoI50(図1c)値の差によって示された。シスプラチン(CIS)及びドセタキセル(DTX)に対するMKN45/5FU交差耐性の試験は、5-FUに対する選択的耐性の存在を示した(図1b及びc)。MKN45及びMKN45/5FU異種移植片の皮下移植は、腫瘍原性に有意差を示さなかった(図1d)。
次に、MKN45/5FU細胞が薬物耐性細胞集団の選択中に遺伝子異常を獲得したかどうかを調べた。46種の癌関連遺伝子からの191個の標的領域における遺伝子異常を、半導体型次世代シーケンサー(NGS、IonPGM、LifeTechnologies、この研究で使用されたIonAmpliSeqCancerPanelのaccession番号はDRA005227である)を用いて配列を決定した。これらの46個の遺伝子のうち、7個がMKN45及びMKN45/5FU細胞の両方で異常が確認された。標的遺伝子変異は、しばしば薬物耐性を促進するが13、我々の遺伝子型解析は、MKN45/5FU細胞の薬物耐性表現型が遺伝子突然変異に起因するのではなく、代わりに5-FUによって選択された圧力に起因することを示唆しているようである。「選択された」集団は、本質的に内因性薬物耐性集団14から生じると考えられるが、関連する分子経路を活性化する適応応答もまた、癌細胞が薬物の存在下で生存し、増殖することを可能にする7, 11。実際、遺伝子異常が限定された効果を有するならば、再発性腫瘍の急速な出現は容易に説明することができる。化学療法後の再発は、遺伝子突然変異の発生の可能性に比べてしばしば短時間で起こる5,15。実際、肝転移又は腹膜播種は化学療法によって一時的に抑制することができるが、高度進行胃癌患者のほとんどは数カ月以内に再発するため、薬物耐性集団が化学療法開始時に既に存在していて、化学療法期間中に生存し続けていることを示唆する16。これらの所見により、我々は、化学療法薬の存在下で出現するコロニーとして単離することができる薬物耐性細胞亜集団のプロテオームプロファイルを検討することとした。
この研究では、異なる薬物投与条件、すなわちDTPに曝露した後、直径1mm未満の二次元の円形コロニーにおいて接着して増殖する細胞の集団を明らかにした(図2a)。各々のDTPのタンパク質発現を定量的に測定するために、我々は、逆相タンパク質アレイ(RPPA)技術に基づいて様々な薬物条件に曝露された細胞から各々のコロニーを選択することにより、コロニー溶解産物アレイ(CoLA)を開発した17, 18。これらの1mmコロニーは一般に3,000-10,000個の細胞しか含まないので、セルソーターのような技術ではタンパク質種の定量的タンパク質測定には適さない。CoLAを使用すると、単一のコロニーからのライセートを含む数百から数千の各々のドットをニトロセルロース表面に集積することができ、抗体セットを用いた同時及び定量検出が可能になる(図2b)。
MKN45及びMKN45 / 5FU細胞の皮下異種移植細胞は、同様の程度の腫瘍原性を示したが(図1d)、CoLAアッセイは、MKN45 / 5FU細胞におけるPI3K/AKT/ラパマイシン経路活性化の機構的な標的(mTOR)が増殖上の利点を与えることを示唆している(図2d)。この仮説を確認するために、MKN45及びMKN45/5FU細胞を、組織の連結性が緩やかな粘膜下層と適切な筋層との間の「液体注入による層剥離」によって注入した(実験手順を参照)。移植から6週間後、MKN45 OXマウスは検出可能な腫瘍を示さなかったが、MKN45/5FU OXマウスは胃で著しく大きな腫瘍(25-100mm3)を有していた(図3a)。胃のMKN45/5FU細胞由来のOX腫瘍は、リンパ節及び肝臓に転移するだけでなく、腹膜腫瘍を形成し得る(図3b及びc)。α平滑筋抗原(α-SMA)によるH&E染色及び免疫組織化学は、胃のOX腫瘍が主に粘膜下層と筋肉層との間で増殖することを示した(図3d)。「組織レベル」及び「器官レベル」OXモデルは、前者が微環境に関連する組織学的位置を正確に模倣する点で異なる。これまでの多くの研究では、腹膜播種を達成するために異種移植片を漿膜層の近く又は胃の壁に注入した20。したがって、この研究における腫瘍転移の頻度は、粘膜下層の血管が移植された腫瘍細胞に曝される可能性が高いためであろう。本明細書で使用される層特異的注入法により、進行した胃癌発達をより正確に観察することができる。総合すると、MKN45/5FU細胞によって示される攻撃的腫瘍原性は、5-FUによる選択がin vivoで高度に悪性の可能性を獲得することを示唆している。
次に、薬物選択と環境選択プロセスに従って、癌細胞と腫瘍免疫表現型を調べた。MKN45/5FUOXマウスに、術後1日目(POD)から5日間毎日5-FUを投与した(図4a)。5-FU処置は、サイズ及び転移能をある程度まで減少させ、薬物感受性細胞の間に薬物感受性集団が残っていることを示唆した(図4b)。5-FUの存在下で形成されたコロニーに由来する「薬物耐性」集団、培養細胞及びOX組織がp-PI3Kに対する抗体で染色されたという仮説に基づいて、CoLAアッセイが薬物耐性マーカーとして同定された(図4c)。予想どおり、免疫組織化学染色は、p-AKT、p-mTOR、及びホスファターゼ及びテンシン相同体(PTEN)タンパク質を含むPI3K経路成分に対して陽性であった細胞の比率は、腫瘍発生の間に実質的に変化したことを示した(図4d)。さらに、MKN45/5FU細胞では、p-PI3K陽性細胞の比率はMKN45と比較して有意に増加し、ほとんどすべての各々のMKN45/5FU細胞は、その後のOX及び転移部位において陽性染色傾向を示した。さらに、p-AKT及びp-mTORは、p-PI3Kに対して逆パターンを示し、PI3K/Akt/mTORフィードバックループの活性化がAKT及びmTORの活性化21を低下させる可能性があることを示唆している21。PTENの発現はOXにおいてほとんど検出されなかった。これは、減少したPTENレベルが微小環境因子によってさらに調節されることを示している22。全体的に、これらの知見は、PI3K/AKT/mTOR経路を介した増殖シグナルが、原発性5-FU化学療法後に強く生存する腫瘍細胞の治療標的であり得ることを示唆する。
PIK3CAホットスポット突然変異は、強い発癌性を引き起こすために酵素活性を増加させると考えられている19。事実、異所性のホットスポット変異は、アポトーシス活性を低下させ、腫瘍浸潤の可能性を高めることができる23。PIK3CAをコードするエクソンにおける突然変異は、幅広い頻度の様々な腫瘍において報告されている19。増大した配列決定の深さ及び長さの適用範囲は、潜在的に発癌性の非ホットスポット突然変異の同定を可能にした24。ここで、PIK3CA突然変異は、キナーゼ様ドメインをコードする領域にあるコドン707(すなわち、E707K)で検出された。E707K突然変異は、耳下腺25及び乳房26から生じる癌腫において報告されており、PIK3CA突然変異を用いて、薬物耐性癌細胞及び悪性表現型を有する細胞集団の選択中の集団濃縮を追跡することができる。in vitroでのコドン707 G>A変異の変異対立遺伝子頻度(VAF)のNGS評価は、それぞれMKN45及びMKN45/5FU細胞について31.2及び30.8%であった。これらの配列決定結果を検証し、細胞培養及びOX中のVAFの濃縮を調べるために、rs3729687を標的とする特定のプローブを有するデジタルPCR(dPCR)のための一対のプライマーを設計した(図5a)。MKN45、MKN45/5FU、MKN45/5FUの腹部腫瘍及びMKN45/5FU肝転移では、変異型アレルの平均VAFはそれぞれ39.7,40.5,32.5及び32.2%であった(図5b)。
遺伝子異常が腫瘍悪性腫瘍の獲得に限定された効果を有するという知見から、次にタンパク質レベルで活性化されたPI3K経路の阻害効果を調べた。PI3K阻害剤が5-FU処置後に持続性増殖を伴う腫瘍を抑制できるかどうかを決定するために、PI3K阻害剤LY294002、ケルセチン、ウォートマンニン、GNE493、及びGDC0941についてコロニー形成アッセイを行った29-32。5-FU又はPI3K阻害剤単独の投与は、適度なコロニー抑制と関連していたが、5-FU及び試験したPI3K阻害剤の同時投与は併用効果を有していた(図6a)。5-FUとGDC-0941の同時投与はコロニー形成を有意に抑制したので、OXモデルにおけるGDC-0941の腫瘍抑制効果を調べた33。MKN45/5FUOXを有するマウスを、静脈内5-FUで処置し、続いて経口投与したGDC-0941で処置した(図6b)。治療無しで、OXマウスの100%(9/9)が胃で局所腫瘍増殖を示し、9匹のマウスのうち5及び4匹が肝転移及び腹膜播種を示した(図3d及び図4b)。5-FU処置では、82%(9/11)のOXマウスが局所腫瘍増殖を有し、27%(3/11)は転移を有した(図4b及び6c)。OXマウスの33%(2/6)及び17%(1/6)が肝転移及び腹膜炎を有していたのに対して、PI3K阻害剤GDC-0941単独を与えたマウスのうち、OXマウスの67%(4/6)(図6c)。5-FUとGDC-0941の組み合わせは、顕著な腫瘍抑制を示し、OXマウスのわずか17%(1/6)が目に見える腫瘍を有し、転移の証拠はなかった(図6b及び6c)。興味深いことに、増殖抑制アッセイは、5-FU耐性細胞株MKN45/5FU及びMKN74/5FUが、GDC-0941に対する感受性が、それぞれの親細胞のものよりも3.2倍及び2.4倍高かったことを示した。さらに、処置後に残った腫瘍の免疫組織化学的結果は、GDC-0941が5-FUによって活性化されたものとは異なる経路活性化を誘導したことを示した。ここで、p-PI3K陽性細胞の有意な減少が明らかであったが、PTEN陽性細胞は、胃のGDC-0941処置腫瘍において時折見出された(図6d)。PTEN陽性細胞の数は、レベルが親MKN45細胞株のレベルにほぼ等しい肝臓転移でさらに増加した(図4c)。p-PI3K陽性細胞は5-FU耐性と関連するようであるので、GDC-0941処置後に見られるp-PI3K陽性細胞の数の減少は、5-FUの有効性の増加を示し得る。全体的に、これらの結果は、PI3K阻害活性化処置が、PI3K経路活性化を伴う5-FU処置濃縮細胞でより有効であることを示唆する。
CoLAプロファイリングは、5-FU寛容性の派生者が5-FU濃度で増加したp-PI3Kレベルを有する傾向があることを示唆した。OX腫瘍免疫組織化学は、p-PI3K発現5-FU耐性細胞が胃微小環境において濃縮されるという仮説を支持した。ウェスタンブロットでのバルク細胞溶解物レベルでは、同時の5-FU/GDC-0941処置は、5-FU耐性細胞におけるp-PI3Kレベルの低下を示した(図7)。薬物処理によって誘発されたPTENレベルの低下は、5-FU耐性細胞においてのみ生じた。しかし、PTEN削減のレベルは、OXで見られたほど顕著ではなかった。したがって、完全なPTEN発現の喪失は、胃の微小環境に起因する可能性がある。親株及び耐性細胞株における同時の5-FU/GDC-0941処置において、p-AKTの阻害が見られた。重要なことに、GDC-0941によるPI3K経路活性化の阻害は、特に5-FU耐性細胞におけるリボソームp70S6キナーゼ(S6キナーゼ)リン酸化(p-S6キナーゼ、Ser235/236及びSer240/244)の減少により明らかに示された。これらの結果は、mTORサロゲートマーカーとしてのS6キナーゼが、5-FU耐性サブ集団の増殖抑制のためのGDC-0941の重要な標的でもあることを示唆している34。しかしながら、薬物耐性の胞子とは対照的に、培養細胞は依然として相当量の薬物感受性細胞を含有している可能性がある。ウェスタンブロットは、S6キナーゼを除いて評価されたPI3K経路タンパク質の大部分において穏やかな変化を示した。これは、p-S6キナーゼ阻害が培養細胞の大部分において比較的迅速に起こり得ることを示唆する。事実、mTORの減少は、GDC-0941処置マウスのOX腫瘍において明らかではなかった。この結果は、ウェスタンブロッティングが培養細胞による分子レベルでの実際の薬物応答を示すのに対し、OX腫瘍の免疫組織化学は、治療後も生存可能な腫瘍細胞のみを表すためであり得る。
PI3K経路は5-FU耐性の残基において活性化されるようであるが、Akt及びS6キナーゼ活性化は必ずしも付随的に誘発されない34。しかしながら、S6キナーゼのリン酸化は、PI3K経路阻害の主要な標的であると考えられている33,35。我々がDTPで観察したように、5-FU処置では、持続者におけるp-PI3Kレベルが徐々に増加した。GDC-0941は、p-S6キナーゼを抑制することによって5-FU耐性p-PI3K陽性細胞を減少させた。これらの結果は、GDC-0941処置が5-FUにより富化された薬物耐性サブ集団に対して有効であることを示唆している。
ここでは、一次化学療法剤5-FUに対する耐性を獲得した癌細胞の増殖を抑制する効果的なアプローチを実証した。各々のDTPによって表されるこれらの5-FU耐性サブ集団は、5-FU濃度が増加するにつれてp-PI3Kのレベルが増加する。高いp-PI3Kレベルを有するより多くのDTPが、MKN45/5FU細胞においてMKN45細胞よりも観察され、MKN45/5FU細胞が親株MKN45よりも5-FUの存在下でより高い生存率を有することを示唆している。興味深いことに、MKN45細胞は、5-FU耐性だけでなく、ヌードマウスの胃における非常に悪性の表現型も示した。p-PI3K陽性細胞のパーセンテージは5-FU耐性及び悪性表現型と並行して増加したが、これらの細胞は明らかな遺伝子異常を有さず、細胞シグナル伝達レベルでの適応が表現型の獲得にとって重要であることを示唆した。従って、5-FU処置後に再発を予防することは、PI3Kシグナル伝達を阻害して、5-FU処置に濃縮されたp-PI3K陽性細胞亜集団を減少させることによって達成することができた。この仮説と一致して、PI3K阻害剤GDC-0941はin vivoでの5-FU耐性及び悪性細胞の増殖を効果的に抑制した。化学療法薬の中で、5-FUは長い間、広範な種類の癌の選択肢として好まれていた36。したがって、化学療法後の再発を経験した患者集団に対する本研究の影響は、他の化学療法薬よりも大きいはずである。

Claims (3)
- 胃癌細胞が生着している非ヒト動物の作製方法であって、
免疫力が低下した非ヒト動物の胃の粘膜下層と筋層との間に細胞懸濁液を注入する工程
を含み、
胃癌細胞が薬物耐性株であり、胃癌細胞がヒトの胃癌由来であり、薬物がフルオロウラシルであり、
非ヒト動物がヌードマウスであり、
細胞懸濁液を注入する工程が、1匹当たり1.0×105~1.0×107個の胃癌細胞を注入する工程である、方法。 - 胃癌細胞がMKN45である、請求項1に記載の方法。
- 請求項1又は2に記載の方法で作製された非ヒト動物を準備する工程;及び
作製された非ヒト動物に、候補薬物を投与する工程
を含む、胃癌の転移を抑制する薬物のスクリーニング方法。
