本発明は一般に、分泌障害の処置のための治療薬に関する。特に本発明は、タンパク質合成を有害に誘導しない分泌促進物質の提供、およびこれらの化合物を利用した処置方法に関する。
線維芽細胞増殖因子21(FGF21)はホルモンであり、その合成および分泌は多様な代謝ストレスおよび細胞ストレス(これは飢餓からミトコンドリアストレスおよびERストレスに及ぶ)によって引き起こされる。FGF21は、1回膜貫通タンパク質であるβクロトーと複合体を形成するチロシンキナーゼ活性を有する通常のFGF受容体(FGFR)から構成される細胞表面受容体を介して作用する。FGF21、βクロトーおよびFGFRは膵外分泌部に共発現しており、これは膵腺房細胞がインビボ(in vivo)におけるFGF21の作用部位であることを示唆している。したがって、FGF21 mRNAレベルは、絶食状態から摂食状態への移行中に膵臓全体において、およびERストレスを誘導するタプシガルジン処理の後に単離された腺房細胞において誘導される。FGF21の薬理学的投与は遺伝的および薬理学的に誘導されたマウスモデルにおいて腺房細胞障害を減弱するが、膵外分泌におけるFGF21の生物学的機能は不明である。
FGF21療法での処置によって修正可能であることが本開示において示されている分泌障害は、(例えば嚢胞性線維症、アルコール依存症、膵炎、膵がん、胆石症、セリアック病、高トリグリセリドもしくはループスに起因し、またはこれらとの併存症である)膵外分泌機能不全を含む。FGF21処置によって修正可能な他の分泌障害は、唾液分泌障害(シェーグレン症候群を含む)、および外科手術に起因する分泌障害(内視鏡的逆行性胆道膵管造影(ERCP)に起因するEPIを含む)を含む。これらの障害に罹患する対象は、本明細書において提供される方法に従ってFGF21またはFGF21の誘導体で処理され得、これらの障害の症状を軽減する。
本明細書に記載されるFGF21製剤はさらに、(例えば、嚢胞性線維症、アルコール依存症、膵炎、膵がん、胆石症、セリアック病、高トリグリセリドもしくはループスに起因し、またはこれらとの併存症である)EPIの処置に有用である。EPIは、膵外分泌部が(消化酵素が栄養分の消化および吸収を促進する場所である)小腸に消化酵素を分泌および送達できないことである。したがって、EPIに罹患している患者は栄養失調を経験する。EPIは他の膵臓疾患(膵炎および嚢胞性線維症など)から生じ得る。これらの疾患のための既存の治療法は症状を改善することに限定されており、これらの障害に直接対処し、またはEPIの進行を妨げる公知の処置は存在しない。本明細書において提供されるFGF21組成物および誘導体は消化酵素の分泌および膵液の流動を刺激し、EPI患者における消化酵素の分泌および流動を復元するために治療的に使用され得る。本発明のFGF21製剤およびFGF21療法は単独で、またはEPIを処置もしくは予防する他の物質と組み合わせて使用され得る。
本明細書に記述されるように、FGF21またはその誘導体は分泌促進物質として作用し、タンパク質毒性を増加させることなく分泌を増加させる。したがって、FGF21およびその誘導体はシェーグレン症候群(口内乾燥疾患)を含む唾液分泌障害の処置に有用である。本発明のFGF21製剤およびFGF21療法は単独で、または唾液分泌障害を処置または予防する他の物質と組み合わせて使用され得る。
内視鏡的逆行性胆道膵管造影(ERCP)は、胆管、膵管および胆嚢を調べるために臨床において使用される技術である。この技術は、内視鏡を患者の口を通して導入し、これらの管が排出する場所である十二指腸にアクセスするために食道および胃を通過することからなる。内視鏡は、これらの管の可視化を促進する放射線造影剤を送達するために使用される。この手技後の一般的な合併症の1つは、この手技を経験している患者の約10%における膵炎である。
100,000人のうち約50人が膵炎を経験している米国において、急性膵炎および慢性膵炎の両方の発生率が著しく増加している。慢性膵炎および急性膵炎の両方のための現在の標準的な治療法は、患者がこの疾患由来の数種の合併症(腹痛、嘔吐および吐き気)のうちいくつかを経験している場合は常に、入院に限定される。本明細書において提供されるFGF21製剤および誘導体は、急性膵炎および慢性膵炎の予防および処置に有用である。FGF21およびその誘導体は膵炎のげっ歯類モデルにおいて膵臓への炎症および損傷の両方を低減させるのに有効であることが本明細書において示される。
本明細書において記述されるFGF21製剤はさらに、がんまたは前がん状態の障害(例えばEPIの進行に起因する膵がん)の処置に有用である。既存の治療法は症状を対象としており、EPIの根底にある原因を処置せず、または膵がんの進行を妨げない。本発明のFGF21製剤およびFGF21療法は単独で、またはがんもしくは前がん状態の障害を処置もしくは予防する他の物質と組み合わせて使用され得る。
EPIは、膵外分泌部が(消化酵素が栄養分の吸収のために消化中に栄養分を分解する場所である)小腸に消化酵素を分泌および送達できないことである。結果として、EPIに罹患している患者は処置をしなければ栄養失調状態となる。数種類の疾患(アルコール依存症および嚢胞性線維症など)がEPIを引き起こし得る。本明細書において提供されるFGF21製剤またはFGF21誘導体製剤は、膵臓の分泌および流動を誘導することによってEPIを予防または処置するように作用する。FGF21は標準的な食前の分泌促進物質(CCKなど)とは異なる受容体を介して作用し、この受容体は慢性膵炎または嚢胞性線維症において影響を受けない。これは正常な膵臓の分泌の復元を可能にする。
本明細書に記載されるように、本発明のFGF21またはFGF21誘導体は分泌障害(EPI、唾液分泌障害、または外科手術に起因するEPIを含む)を処置するために投与され得る。これらは、個々の治療効果のある成分または治療効果のある成分の組合せのいずれかとしての医薬品と組み合わせた使用のために利用できる従来のあらゆる方法によって投与され得る。これらは単独で投与され得るが、一般的には選択された投与経路および標準的な医薬品の実務に基づいて選択された薬学的に許容され得る担体と共に投与される。
ある実施態様において、本発明の有効な化合物は経口投与され得る。これは一般的に消化酵素によるタンパク質分解に対して抵抗性である、または抵抗性を与えられた物質において想定される。そのような化合物は、製造者から錠剤の形状で入手できるあらゆるそれらの化合物または薬剤、ならびにそれらの誘導体および類似体を含むことが想定される。
本発明の有効な化合物はまた、非経口投与のために処方され得る(例えば静脈内経路、筋肉内経路、皮下経路、または腹腔内経路を介した注射のために処方され得る)。本発明のFGF21またはFGF21誘導体を活性成分として含む水溶性組成物の調製は、本開示を考慮すると当業者に公知である。通常、このような組成物は注射剤(溶液または懸濁液のいずれか)として調製され得る;注射の前に液体を添加することによって溶液または懸濁液を調製するための使用に適した固体形状もまた調製され得る;この調製物はまた、乳化していてもよい。
これを達成する1つの方法は、新規な薬剤を従来の治療法と組み合わせることである。本発明の文脈において、本発明によって提供されるFGF21またはその誘導体は、新規または既存の医薬品、手術、化学療法、放射線療法および/または遺伝子療法と組み合わせて使用され得ることが想定される。
マウス研究
Fgf21ノックアウト(KO)マウス(Potthoff, et al., Proc Natl Acad Sci USA 106, 10853-10858, 2009)、Fgf21トランスジェニック(Tg)マウス(Inagaki, et al., Cell Metab 5, 415-425, 2007)、KLB-TおよびWTの同腹仔のうちの1匹をC57Bl/6Jバックグラウンド上で維持した。βクロトーKOおよびヘテロ接合体の同腹仔のうちの1匹を、混合したC57BL/6J;129/Svバックグラウンド上で保持した(Ding, et al., Cell Metab 16, 387-393, 2012)。全てのマウスを無菌設備において飼育し、標準的な固形飼料の食事を与えた(Harlan Teklad 2916)。絶食とリフィーディング実験のために、マウスを一晩絶食させ、次に標準的な固形飼料の食事を2時間再供給した。急性膵炎を、50μg/kgセルレイン(Sigma)の7時間毎の腹腔内注射による投与によって誘導し、対照群のマウスに生理食塩水を注射した。血漿中のFGF21を、マウスFGF21 ELISAキット(Biovendor)を用いて測定した。全ての実験において雄性マウスを使用した。
マウスに賦形剤(10mM Na2HPO4、2%[w/v]グリセロール、pH7.6)または1mg/kgヒト組換えFGF21(Novo Nordisk)を、2日間連続で1日に2回皮下注射した。マウスを6時間絶食させ、膵液を15分間回収した。膵液を回収する1時間前に、賦形剤またはFGF21の最後の注射を行った。
膵臓のタンパク質合成を、3H-フェニルアラニンを用いた大量投与法によって測定した(Sans, et al., American journal of physiology 287, G667-675, 2004)。
膵臓由来のRNAをRNA-Stat60(IsoTex Diagnostics)を用いて抽出した。cDNAをRNA(2μg)からHigh-Capacity cDNA Reverse Transcription Kit (Life Technologies)を用いて作製した。qPCRを、(Bookout, et al. Current protocols in molecular biology / ed. Ausubel, Chapter 15, Unit 15-18, 2006)に記載されるように、SYBR green法によって行った。
マウスの膵臓を10%中性緩衝ホルマリンで一晩固定し、次にパラフィン包理し、薄片を作製し、ヘマトキシリンおよびエオシン染色した。新鮮凍結した組織学のために、膵臓をすぐに切断し、冷却イソペンタン中で瞬間凍結させ、14~16μmの薄片を作製した。次に薄片を10%中性緩衝ホルマリン中において4℃で15分間固定し、1X PBSで3回洗浄し、DAPIを含むVectashield mounting medium(Vector Laboratories)を用いてマウントした。画像をZeiss Axioscan Z1スライドスキャナーを用いて20倍の拡大率で取得した。好酸球の面積の定量化をImage Jを用いて行った。
膵臓を、プロテアーゼ阻害剤およびホスファターゼ阻害剤の混合物(Roche)を添加した1X RIPA緩衝液(Cell Signaling Technology)中において、Fastprep-24用lysing matrixビーズチューブ(MP Biomedicals)を用いてホモジナイズした。タンパク質溶解物の濃度をDCプロテインアッセイ(Biorad)を用いて測定した。等量のタンパク質をロードし、4~20%SDS-ポリアクリルアミドゲル(Biorad)中で電気泳動し、ニトロセルロース膜(Biorad)に移行した。膜をTBST中において5%BSAで1時間ブロックした。FGF21、アミラーゼおよびリパーゼ(Santa Cruz);βアクチン(Abcam);p-EIF2α、p-ERK1/2およびp-eIF4E-BP1(Cell Signaling Technology)に対する抗体を用いた膜の探索を4℃で一晩行った。次に膜を、一次抗体の宿主種(host species)に対するHRP標識二次抗体(Cell Signaling Technology)で1時間インキュベートした。膜を、Super Signal West化学発光キット(Thermo)を用いて展開し、信号をImageQuant LAS4000発光イメージャー(General Electric)を用いて検出した。定量化をImage Jを用いて行った。
二群分析のためにスチューデントのT検定を行った。多群分析のために、ニューマン・コイルス事後補正を伴う一元配置分散分析および二元配置分散分析(Graphpad Prism)を用いた。データを平均値±SEMで表す;p<0.05を有意であると見なした。
Fgf21ノックアウト(KO)マウスを調べ、膵臓の表現型を示すか否かを決定した。H&E染色された膵臓の薄片の組織学的分析は、絶食していたマウスおよび摂食したマウスの両方に由来するFgf21-KOの膵臓における好酸球染色の有意な増加を明らかにし、これはチモーゲン顆粒密度の増加を示していた(図2A)。この観察を支持するように、Fgf21-KOの膵臓におけるアミラーゼレベルおよびリパーゼレベルは絶食中および摂食中において増加していた(図2B)。外来性FGF21の投与は、Fgf21-KOマウスにおける膵臓の好酸球の面積の割合を野生型(WT)のレベルまで減少させた(図2C)。免疫ブロット解析は、FGF21処理後のFgf21-KOマウスにおける膵臓の代謝酵素レベルの対応する減少を確認した(図2D)。膵臓におけるFGF21 mRNAおよびタンパク質のレベルがリフィーディング中に誘導された(図2Eおよび図2F)。しかしながら、注目すべきことに、リフィーディングは血漿中のFGF21濃度を減少させ(図2G)、これはFGF21が膵外分泌部から血中に分泌されていないことを示す。対照的に、自由摂食のWTマウスの血漿と比較して200倍高いFGF21レベルが膵液において観察された(図2H)。さらに、膵臓の酵素合成を著しく増加させることが知られているCCKでの処理は、血中においていかなる変化も生じることなく、膵液中のFGF21濃度をさらに3倍増加させた(図2H)。したがって、生理的ストレスおよび薬理的ストレスに応じて、FGF21は自己分泌またはパラ分泌の様式で作用し、膵消化酵素のレベルを制御している。
FGF21はタンパク質合成を阻害するか、または消化酵素の分泌を増加させるかのいずれかによって、膵腺房細胞中の消化酵素濃度を減少させ得る。CCKと比較した、膵臓のタンパク質合成に対するFGF21の効果を調べた。予想されるように、CCKの投与は3H-フェニルアラニンの膵臓のタンパク質への取り込みを著しく増加させた(図3A)。対照的に、FGF21の投与は膵臓のタンパク質合成に対して有意な効果を持たなかった(図3A)。このタンパク質合成のプロファイルと一貫して、CCKは真核生物翻訳開始因子4E結合タンパク質1(eIF4E-BP1)のリン酸化を増加させたが、FGF21は効果を持たなかった(図3B)。FGF21とCCKの両方は、膵臓中のERK1/2のリン酸化を誘導し(図3B)、これはこれらが共に膵臓中でシグナル伝達することを示した。したがって、FGF21はタンパク質合成を抑制することによって膵臓中の消化酵素レベルを減少させるのではない。さらに、CCKとは異なり、FGF21は膵臓のタンパク質合成を刺激しない。
マウスの一次腺房細胞の調製
一次腺房細胞を、公開されたプロトコル(Williams, et al., The American journal of physiology 235, 517-524, 1978)の改変によって調製した。簡潔に述べると、マウスを断頭によって屠殺し、次に切開し、膵臓を1X PBSで洗浄した。膵臓の消化培地(DMEM中、0.75mg/ml XI-S型コラゲナーゼ[Sigma]、0.1mg/mlダイズトリプシンインヒビター[Sigma]、1%BSA)を膵臓実質に30gのシリンジで注入した。次に、膵臓を直径が減少していくセロロジカルピペットを用いて一定に混合しながら、消化培地中で20~40分間インキュベートした。細胞を100μmの濾過器に通して濾過し、洗浄毎に50 x gで3分間遠心分離することによってインキュベーション培地(DMEM中、0.1mg/mlダイズトリプシンインヒビター、1%BSA)で3回洗浄した。細胞を播種し、インキュベーション培地中で2時間、37℃で回復させた。
一次腺房細胞およびAR42J細胞を賦形剤、FGF21(一次腺房細胞およびA42Jの両方について1μg/ml)またはCCK(一次腺房細胞について10pM、およびAR42J細胞について100pM[Phoenix Pharmaceuticals])で30分間、37℃で処理した。細胞培地および細胞の両方におけるアミラーゼレベルをアミラーゼアッセイキット(Amylase Assay Kit)を用いて分析した。
AR42J細胞を1μMのカルシウム指示薬fluo4-AM(Life Technologies)で30分間、37℃で前処理した。指示されているように、1μM PD173074(Sigma)、10μM L-364,718(Tocris Bioscience)、10μM U73122(Tocris Bioscience)または100μM 2-APB(Tocris Bioscience)をfluo4-AMの前処理中にイメージング培地(imaging medium)(フェノールレッドを含まないDMEM)に添加した。細胞をBD Pathway 855 bioimager上で10倍の対物レンズを用いて画像化した。賦形剤、FGF21(1μg/ml)およびCCK(100pM)を、自動点滴機構によってベースライン蛍光取得の10秒後に送達し、合計2分間画像化した。カルシウム放出を、Image Jを用いてΔF/F0として計算し、ここでΔFは細胞からの未加工の蛍光信号(F)マイナス(細胞が存在せず、蛍光信号が存在しないと予想された領域から計算された)それらのバックグラウンド蛍光であり、F0は化合物を添加する前の最初の6フレームの平均であった。試料サイズ(n)は、5~20細胞を含む各細胞集団と共に測定された細胞集団の数に対応する。
CCKは、ERからの細胞内カルシウム放出を誘導するホスホリパーゼC(PLC)-イノシトール三リン酸受容体(IP3R)シグナル伝達カスケードを活性化することによって、膵外分泌部からの分泌を刺激する。膵外分泌部に発現するFGF21補助受容体(co-receptor)であるFGFR21もまたPLCを活性化し、これはFGF21が類似の機構を介して作用してい得ることを示唆している。これを支持するように、AR42J細胞のFGF21またはCCKのいずれかでの処理は、細胞内カルシウム放出をもたらした(図4A)。興味深いことに、CCKと比較して、FGF21での処理によるカルシウム放出のプロファイルは動的および時間的に異なっていた:カルシウムレベルはCCK処理の後30秒以内にピークに達したが、FGF21の効果は処理の1~2分後にピークに達したカルシウムレベルを伴ってより持続していた(図4A)。FGF21によって誘導されたカルシウム放出は、FGF受容体アンタゴニストであるPD173074によって遮断されたが、CCK-A受容体アンタゴニストであるL-364,718によっては遮断されなかった;逆に、CCKを介するカルシウム放出はL-364,718によって遮断されたが、PD173074によっては遮断されなかった(図4Bおよび図4C)。AR42J細胞のPLC阻害剤U73122またはIP3R阻害剤2-APBのいずれかでの処理は、FGF21およびCCKの両方によって誘導されたカルシウム放出を遮断した(図4Dおよび図4E)。単独でカルシウム放出に影響を与える阻害剤は存在しなかった(図5)。まとめると、これらのデータは、FGF21およびCCKが異なる細胞表面受容体を介して作用して、下流のPLC-IP3R経路に関与し、膵腺房細胞においてカルシウム放出を誘導していることを示す。
FGF21は、強力なCCK類似体であるセルレインによって誘導された膵炎からマウスを保護する。しかしながら、この有益な効果の根底にある機構は不明である。FGF21が膵臓の分泌を刺激し、これによりERストレスおよび組織損傷を減弱することによってセルレイン誘導性膵炎(CIP)を防いでいるのか否かを調べた。CIPをWTマウスおよびFgf21-KOマウスにおいて誘導し、ERストレスのマーカー(真核生物翻訳開始因子2(eIF2)のサブユニットのリン酸化、ならびに活性化転写因子4(Atf4)、CCAAT/エンハンサー結合タンパク質相同タンパク質(Chop)および免疫グロブリン重鎖結合タンパク質(Bip)の誘導を含む)を測定した。CIPは、WTマウスおよびFgf21-KOマウスの両方の膵臓において強いERストレス応答を誘導した(図6A)。しかしながら、ERストレスは、リン酸化eIF2ならびにAtf4、ChopおよびBipのmRNAの著しく高いレベルによって明らかなように、Fgf21-KOマウスにおいて悪化した(図6A)。逆に、循環FGF21を超生理学的なレベルに増加させる肝臓特異的な導入遺伝子を用いたFGF21の慢性投与は、CIPに応答するこれらのERストレスのパラメータを抑制した(図6B)。これらのデータは、FGF21が薬理学的に誘導された膵臓のERストレスを軽減し、それにより腺房細胞におけるタンパク質恒常性(proteostasis)を回復することを示唆している。
FGF21がERCP誘導性膵炎に対して有益な効果を与えるか否かを評価するために、ERCPの24時間後に膵炎が発生するERCPの確立されたげっ歯類モデルにおいて前臨床試験を行った。雌性Swiss Websterマウスを、以下の方法で賦形剤または1mg/kgのFGF21のいずれかの4回の皮下注射を用いて処理した:ERCPの前日における1回の注射、ERCP処置の日における2回の注射、およびERCPの次の日における1回の注射。マウスにERCP処置を行い、これは膵管を介する放射線造影剤の高圧注入からなっていた。マウスの膵臓の組織学的グレード分類は、FGF21がERCP後の浮腫および炎症の両方を減少させたことを明らかにした。
本発明におけるFGF21またはその誘導体を膵炎に罹患している対象に投与し、これはタンパク質合成の増加を伴わずに分泌の増加をもたらし、したがって膵炎の根底にある原因を処置し、ERストレスを解消するように作用する。

Claims (18)
- 治療に十分な量の線維芽細胞増殖因子(FGF21)またはFGF21の有効な誘導体を対象に投与することを含む、前記対象の膵外分泌機能不全(EPI)を処置する方法における使用のための、線維芽細胞増殖因子21(FGF21)またはFGF21の有効な誘導体を含む組成物。
- 膵外分泌機能不全が、嚢胞性線維症、アルコール依存症、膵炎、膵がん、胆石症、セリアック病、高トリグリセリドおよびループスからなる群から選択される障害との併存症である、請求項1記載の組成物。
- 膵外分泌機能不全が外科手術に起因している、請求項1または2に記載の組成物。
- 外科手術が内視鏡的逆行性胆道膵管造影(ERCP)である、請求項3記載の組成物。
- 局所投与、局部投与、全身投与または継続投与のために製剤化されている、請求項1~4のいずれかに記載の組成物。
- 単回投与のために製剤化されている、請求項1~5のいずれかに記載の組成物。
- 経口投与、静脈内投与または筋肉内投与のために製剤化されている、請求項1~6のいずれかに記載の組成物。
- FGF21またはFGF21の有効な誘導体が0.1mg~300mgの用量で投与される、請求項1~7のいずれかに記載の組成物。
- FGF21またはFGF21の有効な誘導体が1mg~200mgの用量で投与される、請求項8記載の組成物。
- FGF21の有効な誘導体がFGF21の長時間作用型誘導体である、請求項1~9のいずれかに記載の組成物。
- FGF21の有効な誘導体が、FGFR/βクロトー受容体複合体を標的とするFGF21類似体またはFGF21模倣物である、請求項1~10のいずれかに記載の組成物。
- FGF21の有効な誘導体が、LY2405319、FGF21模倣物モノクローナル抗体mimAb1、PF-05231023、またはFGF21模倣物アゴニスト抗FGFR1(FGF受容体1)抗体R1MAbである、請求項1~11のいずれかに記載の組成物。
- FGF21またはFGF21の有効な誘導体が、薬学的に許容され得る担体、緩衝液または希釈剤中に分散している、請求項1~12のいずれかに記載の組成物。
- 第2の治療法と組み合わせて使用され、前記第2の治療法が医薬品、手術、化学療法、放射線療法および遺伝子療法からなる群から選択される、請求項1~13のいずれかに記載の組成物。
- 第2の治療法がFGF21またはFGF21の有効な誘導体を投与する前に提供される、請求項14記載の組成物。
