Description
Claims (14)
- 無線周波数(RF)エネルギ及びマイクロウェーブエネルギを肺組織に搬送するための電気外科装置であって、前記電気外科装置が、
RFエネルギとマイクロウェーブエネルギとを別々または同時に生成するためのジェネレータと、
患者の肺内への非経皮的挿入のための、導入可能な機器コードを有し、前記機器コードが、その長さに沿って通る機器チャネルを有する、電磁ナビゲーション気管支鏡と、
電気外科機器であって、
前記ジェネレータに接続され、RFエネルギ及びマイクロウェーブエネルギを搬送するように構成された同軸ケーブルであって、前記同軸ケーブルが、内側導電体、外側導電体、及び、前記内側導電体と外側導電体とを分離する誘電材料を有する、前記同軸ケーブル、ならびに、
前記RFエネルギ及びマイクロウェーブエネルギを前記同軸ケーブルから受領するように前記同軸ケーブルの遠位端に配置された放射チップ部分を備えている、
前記電気外科機器と、を備え、
前記放射チップ部分が、前記内側導電体に電気的に接続された第1の導電要素、及び、前記内側導電体から電気的に絶縁された第2の導電要素を備え、
前記第1の導電要素が、その長さに沿って前記誘電材料によって囲まれており、
複数の前記第2の導電要素が、前記放射チップ部分において前記誘電材料の周囲を被覆し、
前記放射チップ部分はさらに、前記放射チップ部分の長さに沿って前記第2の導電要素間に配置され、前記誘電材料を露出させる複数の放射スロットを備え、
前記第1の導電要素と前記第2の導電要素とが、
前記RFエネルギを、前記放射チップ部分を囲む組織内に搬送するための作動電極及びリターン電極、ならびに、
前記マイクロウェーブエネルギを、局所化されたマイクロウェーブ場として放射するためのアンテナとして作用するように構成されており、
前記電気外科機器が、前記電磁ナビゲーション気管支鏡内の前記導入可能な機器コードの前記機器チャネルを通して挿入可能である、前記電気外科装置。 - 前記同軸ケーブル及び前記放射チップ部分が、1.9mm以下の最大外径を有する、請求項1に記載の電気外科装置。
- 前記マイクロウェーブエネルギを搬送している場合、前記同軸ケーブルが、2dB/m以下の損失を示すように構成されている、請求項1または請求項2のいずれかに記載の電気外科装置。
- 前記ジェネレータが、RFエネルギ及びマイクロウェーブエネルギを搬送して、エネルギ搬送プロファイルに従って、前記放射チップ部分における組織の切除を生じさせるように構成されている、請求項1から請求項3のいずれかに記載の電気外科装置。
- 前記エネルギ搬送プロファイルが、もっぱらRFエネルギで構成された最初の部分を含んでいる、請求項4に記載の電気外科装置。
- 前記エネルギ搬送プロファイルが、パルス状のマイクロウェーブエネルギを含むマイクロウェーブ切除部分を含んでいる、請求項4または請求項5に記載の電気外科装置。
- 前記パルス状のマイクロウェーブエネルギが、前記マイクロウェーブエネルギが搬送されない一連のOFF部分によって分離されて前記マイクロウェーブエネルギが搬送される一連のON部分を含み、RFエネルギが、前記OFF部分の1つまたは複数で搬送される、請求項6に記載の電気外科装置。
- 前記ジェネレータが、前記放射チップ部分において、組織のインピーダンスを検出するように構成されており、前記エネルギ搬送プロファイルが、前記検出されたインピーダンスに基づいて調整可能である、請求項4から請求項7のいずれか一項に記載の電気外科装置。
- 前記エネルギ搬送プロファイルが、もっぱらRFエネルギで構成された第1の部分と、それに次ぐ、マイクロウェーブエネルギを含む第2の部分とを備え、前記ジェネレータが、前記組織のインピーダンスが所定の閾値を超えていると判定された際に、前記第2の部分に切替えるように構成されている、請求項8に記載の電気外科装置。
- エネルギ搬送プロファイルが、前記マイクロウェーブエネルギが搬送される部分を含み、前記ジェネレータが、前記RFエネルギに対応する周波数において前記マイクロウェーブエネルギを調整するように構成されている、請求項4から請求項9のいずれか一項に記載の電気外科装置。
- 無線周波数(RF)エネルギ及びマイクロウェーブエネルギを肺組織に搬送するための電気外科装置であって、前記電気外科装置が、
RFエネルギとマイクロウェーブエネルギとを別々または同時に生成するためのジェネレータと、
患者の肺内への非経皮的挿入のための、導入可能な機器コードを有し、前記機器コードが、その長さに沿って通る機器チャネルを有する、電磁ナビゲーション気管支鏡と、
電気外科機器であって、
前記ジェネレータに接続され、RFエネルギ及びマイクロウェーブエネルギを搬送するように構成された同軸ケーブルであって、前記同軸ケーブルが、内側導電体、外側導電体、及び、前記内側導電体と外側導電体とを分離する誘電材料を有する、前記同軸ケーブル、ならびに、
前記RFエネルギ及びマイクロウェーブエネルギを前記同軸ケーブルから受領するように前記同軸ケーブルの遠位端に配置された放射チップ部分を備えている、
前記電気外科機器と、を備え、
前記放射チップ部分が、前記内側導電体に電気的に接続された第1の導電要素、及び、前記内側導電体から電気的に絶縁された第2の導電要素を備え、
前記第1の導電要素が、前記外側導電体の遠位端よりも遠位に延在する前記内側導電体の長さをさらに含み、前記第1の導電要素が、その長さに沿って前記誘電材料によって囲まれており、
前記第2の導電要素が、前記放射チップ部分において前記誘電材料の周囲を被覆し、
前記放射チップ部分はさらに、前記放射チップ部分の長さに沿って前記第2の導電要素間に配置され、前記誘電材料を露出させる複数の放射スロットを備え、
前記第1の導電要素と前記第2の導電要素とが、
前記RFエネルギを、前記放射チップ部分を囲む組織内に搬送するための作動電極及びリターン電極、ならびに、
前記マイクロウェーブエネルギを、局所化されたマイクロウェーブ場として放射するためのアンテナとして作用するように構成されており、
前記電気外科機器が、前記電磁ナビゲーション気管支鏡内の前記導入可能な機器コードの前記機器チャネルを通して挿入可能である、前記電気外科装置。 - 前記複数の放射スロットが、前記誘電材料内の前記マイクロウェーブエネルギの波長の約10分の1の長さを有する近位のスロットと、前記誘電材料内の前記マイクロウェーブエネルギの波長の約10分の1の長さを有する遠位のスロットと、前記近位のスロットと前記遠位のスロットとの間の中間のスロットと、を含み、前記中間のスロットが、前記誘電材料内の前記マイクロウェーブエネルギの波長の約4分の1の長さを有する、請求項1から請求項11のいずれか一項に記載の電気外科装置。
- 前記第1の導電要素と前記第2の導電要素との両方が、前記放射チップ部分の遠位面に延びる、請求項1から請求項10のいずれか一項に記載の電気外科装置。
- 前記同軸ケーブルの前記内側導電体が、前記放射チップ部分の遠位面において露出している、請求項1から請求項13のいずれか1項に記載の電気外科装置。
