Description
本願は、2015年9月8日に出願された米国仮特許出願第62/215,651号の利益を主張するものであり、該米国仮特許出願は、参照によりその全体が本明細書に組み込まれる。
本開示のある態様は、サイトカインおよびカルボキシルエステラーゼ酵素を発現する腫瘍溶解性ワクシニアウイルスであって、好ましくは、活性型のチミジンキナーゼを発現しない腫瘍溶解性ワクシニアウイルスに関する。ワクシニアウイルスは、およそ250の遺伝子をコードしている線状二本鎖DNAゲノム(約190Kbp)を有する、大きく複雑なエンベロープウイルスである。ワクシニアウイルスは、大きさがおよそ360nm×250nmの、大きなウイルスである。ワクシニアは、天然痘を撲滅したワクチンとしての役割でよく知られている。天然痘の撲滅後、科学者たちは、生体組織へ遺伝子を送達する道具として、あるいは、生体組織に対するワクチンとして、ワクシニアウイルスの利用を研究してきた(遺伝子療法および遺伝子工学)。
上記組み合わせの腫瘍溶解性ワクシニアウイルス成分は、カルボキシルエステラーゼを発現するよう設計され得る。カルボキシルエステラーゼは、主に肝臓(カルボキシルエステラーゼ1)および腸(カルボキシルエステラーゼ2)に存在するセリンエステラーゼ酵素であり、様々な生体異物の解毒に関与すると考えられている。本明細書において、カルボキシルエステラーゼには、イリノテカンから活性代謝物SN−38への変換を触媒する酵素(ブチリルコリンエステラーゼなど)を含む。SN−38はイリノテカンと同じようにトポイソメラーゼI阻害剤であるが、元々の薬剤よりも100倍から1000倍、活性が高い。イリノテカンを用いた標準的な治療を受けているヒト癌患者において、イリノテカンのごく一部のみがSN−38に変換される。カルボキシルエステラーゼ1のカルボキシル末端にある4つのアミノ酸(HIEL)、およびカルボキシルエステラーゼ2のカルボキシル末端にある4つのアミノ酸(HTEL)によって、酵素が細胞に残留する。これらの末端の4つのアミノ酸を取り除くことにより、酵素が分泌される。
ある実施形態では、本開示の組成物、組み合わせ、および方法に用いられる腫瘍溶解性ウイルスは、サイトカインを発現するよう設計される。
特に好ましい実施形態では、腫瘍溶解性ワクシニアウイルスは、SJ−815である。SJ−815は、Western Reserveワクシニア親株のWR A34R K151Eに由来する、弱毒化形質転換腫瘍溶解性ワクシニアウイルスである。WR A34R K151Eは、EEV産生を増加させるため、K151E変異を有するA34R遺伝子を含む(Blasco 1993)。SJ−815は、ヒトCES2遺伝子およびヒトIFNbeta1遺伝子を(それぞれ、合成初後期プロモーターとp7.5初後期プロモーターの制御下で)、親ウイルスのチミジンキナーゼ(TK)遺伝子に挿入し、これにより、TK遺伝子を不活性化して作出された。TK遺伝子の不活性化は、ワクシニアウイルスの毒性を低減し、腫瘍特異的な複製を増加させることが示されている。
本開示に係る腫瘍溶解性ワクシニアウイルスは、局所投与、局部投与、または全身投与され得る。例えば、限定はされないが、本開示に係る腫瘍溶解性ワクシニアウイルスは、血管内投与(動脈内投与または静脈内投与)、腫瘍内投与、筋肉内投与、皮内投与、腹腔内投与、皮下投与、経口投与、非経口投与、鼻腔内投与、気管内投与、経皮投与、髄腔内投与、眼内投与、または頭蓋内投与することができる。好ましくは、ワクシニアウイルスは、血管内投与および/または腫瘍内投与される。腫瘍内投与は、一般的に、腫瘍塊への注射または腫瘍関連血管系への注射を伴う。ある態様では、ウイルスの投与前に、または、投与中に、腫瘍を画像化する。
腫瘍溶解性ワクシニアウイルスと併用可能な癌用コドラッグとしては、トポイソメラーゼI阻害剤およびトポイソメラーゼII阻害剤が挙げられる。なお、トポイソメラーゼ阻害剤との表記は、それらの薬学的に許容される塩を含むことを意図していると理解されたい。トポイソメラーゼ阻害剤は、少なくとも1つの塩基性基を有していると、酸付加塩を形成することができる。また、酸性基を有するトポイソメラーゼ阻害剤も、塩基と共に塩を形成することができる。トポイソメラーゼ阻害剤とそれらの塩は、水和物の形態で用いられてもよく、他の溶媒(例えば、結晶化に用いられる溶媒)を含んでいてもよい。
本開示のいくつかの態様は、カルボキシルエステラーゼ酵素およびサイトカインを発現する腫瘍溶解性合成ワクシニアウイルスであって、好ましくは活性型のチミジンキナーゼを発現しない腫瘍溶解性合成ワクシニアウイルスを有効量、哺乳動物に投与することにより、当該哺乳動物の癌を治療する方法に関し、理想的には、癌用コドラッグを併用する方法に関する。本開示のいくつかの態様は、(a)サイトカインおよびカルボキシルエステラーゼ酵素を発現する腫瘍溶解性合成ワクシニアウイルスであって、好ましくは活性型のチミジンキナーゼを発現しない腫瘍溶解性合成ワクシニアウイルスを含む組成物と、(b)癌用コドラッグと、を含む組み合わせを有効量投与することにより、哺乳動物の癌を治療する方法に関する。

本開示のいくつかの態様は、(i)サイトカインおよびカルボキシルエステラーゼ酵素を発現する腫瘍溶解性合成ワクシニアウイルスであって、好ましくは活性型のチミジンキナーゼを発現しない腫瘍溶解性合成ワクシニアウイルスと、(ii)能動的塞栓術に適した生体適合性微粒子物質または親水性高分子ゲル物質と、を含む組成物を哺乳動物の血管系に導入することによって、当該哺乳動物の癌を治療する方法に関する。いくつかの実施形態では、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグの組み合わせが血管系に導入される。本開示のいくつかの態様は、(a)サイトカインおよびカルボキシルエステラーゼ酵素を発現する腫瘍溶解性合成ワクシニアウイルスであって、好ましくは活性型のチミジンキナーゼを発現しない腫瘍溶解性合成ワクシニアウイルスと、能動的塞栓術に適した生体適合性微粒子物質または親水性高分子ゲル物質と、を含む組成物を哺乳動物の血管系に導入することによって投与する工程と、(b)有効量の癌用コドラッグを含む組成物を当該哺乳動物に投与する工程、によって当該哺乳動物の癌を治療する方法に関する。上記の通り、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグは、同時に、逐次的に、または、個別に投与され得る。同時投与は、例えば、これら薬剤を含む1つの固定された組み合わせの形態で行われてもよいし、各薬剤を独立した製剤として同時に投与することにより行われてもよい。逐次使用(投与)は、好ましくは、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグを、異なる時点で、すなわち、時間をずらして投与することを意味する。好ましくは、逐次使用(投与)は、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグを独立して投与するよりも、これらの組み合わせがより有効となるように、投与することを意味する。個別使用(投与)は、好ましくは、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグをそれぞれ独立して異なる時点で投与することを意味する。好ましくは、個別使用(投与)は、腫瘍溶解性ワクシニアウイルスおよび癌用コドラッグを、両薬剤の血中レベルが測定可能な状態で(同時に)重複して存在しないように投与することを意味する。独立して投与される場合、能動的塞栓術に適した生体適合性微粒子物質または親水性高分子ゲル物質を用いた能動的塞栓術によって一方または両方が投与され得る。したがって、本開示はすべてのそのような同時治療レジメンまたは交互治療レジメンを含むと理解され、「投与(administering)」との用語はそれに応じて解釈されたい。
一態様において、本開示は、(i)本開示の腫瘍溶解性ウイルスと、(ii)能動的塞栓術に適している生体適合性微粒子物質または親水性高分子ゲルと、を任意構成で癌用コドラッグと併用して哺乳動物の血管系に導入することによって、当該哺乳動物の血管上の部位に能動的塞栓術を施す方法を提供する。
本開示の組成物および組み合わせと共に投与され得る1つ以上の追加の化学療法薬には、限定されないが、5−フルオロウラシル(FU)、フォリン酸(FA)(すなわちleucovorin)、メトトレキサート、カペシタビン(Xeloda;5−FUの経口プロドラッグ)、オキサリプラチン(Eloxatin)、ベバシズマブ(Avastin)、セツキシマブ(Erbitux)、およびパニツムマブ(Vectibix)およびその任意の組み合わせが含まれる。これらの薬剤は、既知の治療プロトコルにしたがって投与され得る。一般的に、追加の化学療法薬は、静脈内に投与される(経口製剤であるカペシタビンを除く)。
癌または腫瘍細胞へ腫瘍溶解性ワクシニアウイルスを送達する好ましい方法は、腫瘍内注入または血管内注入を介したものである。しかし、本明細書に開示される薬学的組成物は、その代わりに、非経口投与、皮内投与、筋肉内投与、経皮投与されてもよいし、または、米国特許第5,543,158号、米国特許第5,641,515号、および米国特許第5,399,363号(それぞれ、参照によりその全体が具体的に本明細書に組み込まれる)に記載されているように、腹腔内投与によって投与されてもよい。

<導入>
ヒトカルボキシルエステラーゼ2およびヒトインターフェロンベータ1を発現する組換えワクシニアウイルスSJ−815を、これら2つの遺伝子をWestern Reserve株ワクシニアウイルスのチミジンキナーゼ領域に挿入することにより作製した。親であるワクシニアウイルスWestern Reserveは、ワクシニアウイルスの細胞外エンベロープ型(EEV)を増加させることで知られているA34R遺伝子に変異を有している。
(ウイルス)
A34RにK151E変異を有しているワクシニアウイルスWestern Reserve(WR A34R K151E)を、SJ−815のバックボーンとして選出した。これは、Western Reserve(WR)株がWyeth株よりも強力であると立証されており、腫瘍溶解性ワクシニアウイルスの治療指数を向上させると予想されるからである。A34R領域のコドン151におけるリジン(K)からグルタミン酸(E)の変異によって、A34Rタンパク質の能力であるEEV粒子を細胞膜に保持する能力が低下する。この変異は、in situの固形腫瘍だけでなく転移腫瘍にも影響を与えるという、組換えワクシニアウイルスSJ−815の能力を増強することを意図して含められている。
相同組換え用のプラスミドベクターは、2つの形質転換インサートを有するpSC65プラスミドから構成されていた。当該2つのインサートのために、ヒトカルボキシルエステラーゼ2(hCE2)およびヒトインターフェロンベータ1(hIFNβ1)遺伝子を新規に合成した。
相同組換えの原理に従って、組換えウイルスを作製した。その詳細な手順としては、公知の方法を利用する。
組換えワクシニアウイルスによって発現されているインターフェロン−βは、ヒトインターフェロンベータセンサー細胞株におけるJAK/STAT/ISGF3経路の、IFN−βに媒介された活性化を監視することによって測定することができる。このセンサー細胞株において、JAK/STAT/ISGF3の刺激は、続けて分泌型アルカリホスファターゼ(SEAP)の産生を誘発する。懸濁液中のSEAPレベルは、QUANTI-Blue(商標)システムを用いて測定することができる。このシステムでは、QUANTI-Blue(商標)培地がSEAPの存在下で紫/青に変色し、SEAPの量に比例する比色変化を、620nm〜655nmの分光光度計を用いたOD測定によって評価することができる。
インターフェロンの存在を、PCR生成物によって確認した。インターフェロン導入遺伝子の両側を特異的に標的とするプライマーを設計し、その後、BIONEER(Daejeon, Korea)に合成を委託した(フォワード:GCCTAGATCTGTCGACTTCGAGC(配列番号2)、リバース:AACGTATATGTGTACCGGGAGCAG(配列番号3))(図2)。反応混合物を、100ngのテンプレートDNAを含む500μLのチューブ内で、製造者の取扱説明書(TaKaRa Ex Taq(商標))に従って調製した。当該チューブを、あらかじめ通常のパラメータにプログラムしたサーモサイクラー(SureCycler 8800、G8800A, Agilent Technologies)内に設置した。
(導入)
SJ−815ウイルスを、in vitro細胞培養アッセイを用いて、プラークサイズ、ウイルス産生量、感染粒子量、ウイルスタンパク質の産生、感染細胞の形態形成、および細胞外エンベロープビリオン(EEV)の形成について評価した。
プラークアッセイのために、6ウェル組織培養プレート中のU−2 OS細胞単層またはBS−C−1細胞単層を、WR、WR.A34R、SJ−815、またはmSJ−815ウイルスの10倍希釈液で感染させた。2時間の吸収後、ウイルス接種材料を取り除き、2.5%のFBSを含むD−MEMまたはE−MEMを用いて細胞を洗浄し、3%のメチルセルロースを含む培地を添加した。感染を、37℃で72時間または48時間進行させた。その後、上記単層を0.1%のクリスタルバイオレットで1時間着色し、洗浄し、プラークの数を数えた。観察されたSJ−815のプラークサイズは、U−2 OS細胞(図3Aおよび図3B)およびBS−C−1細胞(図3B)における他のウイルスのものよりも小さかった。

マウス癌細胞株TIB−75、CT−26、B16−F10、MC−38、RENCA、および4T1を、96ウェルプレート中の体積100μLの増殖培地に播種した。プレートを、37℃のインキュベーターで24時間インキュベートした。10倍単位の様々な感染希釈系列(100PFU/細胞から0.0001PFU/細胞の範囲)を用いて、細胞をmSJ−815およびWR.mGMCSF.TK−に感染させた。感染から48時間後、製造者の取扱説明書に従い、Cyto Tox-Gloアッセイ(Promega)を用いて、細胞生存率を判定した。いくつかのマウス癌細胞株(TIB−75肝細胞癌(図9A)、CT−26結腸癌(図9B)、B16−F10皮膚黒色腫(図9C)、MC−38結腸癌(図9D)、RENCA腎細胞癌(図9E)、および4T1乳癌(図9F)など)において、mSJ−815ウイルスの濃度の上昇が細胞溶解を誘発した。
(導入)
マウスSJ−815ウイルスの投与の影響を測定することによって、SJ−815ウイルスをin vivoで評価した。当該評価では、マウスSJ−815ウイルスを単独投与した場合とマウスSJ−815ウイルスとイリノテカン処置とを併用した場合との両方について、腫瘍増殖、動物の生存、および臓器重量への影響を測定した。
C57BL/6マウスにMC−38マウス結腸癌細胞を皮下移植した(2×106細胞/マウス)。腫瘍が形成されたとき(100mm3〜200mm3)、マウスを無作為に4つの処置群に分けた(n=5匹のマウス/群):(1)リン酸緩衝食塩水(PBS)のみ、(2)WR.TK−.mGMCSF(3日毎に静脈内投与(1×108pfu)を4回)、(3)WR.TK−.mGMCSF(2と同じスケジュールで腫瘍内投与)、および(4)上記と処置スケジュールで、mSJ−815を用いて腫瘍内投与による処置。その後、ノギスでの測定により腫瘍組織量を追跡した。腫瘍が1,500mm3に達した時点で、そのマウスを致死させた。1週間あたり2日毎にマウスを計量した。graphpad prism version 5を用いてデータ解析を行った。21日目、24日目、および27日目における腫瘍の大きさの減少に示される通り(図10C)、MC−38マウス結腸癌の増殖は、mSJ−815を用いた処置によって遅らされた。生存または体重に関しては有意な変化は観察されなかった(図10Aおよび図10B)。
雌のヌードマウスの皮下にMIA PaCa−2ヒト膵臓癌細胞を注入し、腫瘍を生じさせた。腫瘍の体積が100mm3〜200mm3に達したとき、マウスを無作為に8つの処置群に分けた(n=3匹のマウス/群):(1)リン酸緩衝食塩水(PBS)、(2)SJ−815(1×105)、(3)SJ−815(1×106)、(4)SJ−815(1×107)を週1回腫瘍内投与(0日目、7日目、および14日目)、(5)CPT−11(0.25mg/kg)、(6)CPT−11(2.5mg/kg)、(7)CPT−11(25mg/kg)を3日目、10日目、および17日目に静脈内投与、(8)SJ−815(1×106)+CPT−11(25mg/kg)。ノギスを用いて腫瘍の測定を週に2回、飼育終了まで行った。平均腫瘍体積の減少が示す通り(図11)、MIA PaCa−2ヒト膵臓癌の増殖は、mSJ−815とCPT−11(イリノテカン)との併用処置によって遅らされた。この実験は、以下の事を示す:すなわち、ヌードマウスはT細胞をほとんど欠失しているので免疫系が阻害されており、そのため、トポイソメラーゼ阻害剤とカルボキシルエステラーゼ酵素の発現を組み合わせることによって、サイトカイン(hIFN−b)とは独立に、所望の効果をもたらしていた。仮にヒトIFN−bがマウスの免疫系に影響し得たとしても、ヌードマウスのIFN−bに対する応答性は依然として低いはずである。

動物が死亡した日に、腫瘍、筋肉、卵巣、および肝臓を取り出し、0.1%のウシ血清アルブミンを含む2mLの平衡塩類溶液に入れ、次の使用まで直ちに−80℃で保管した。臓器を解凍し、1.4mmサイズのセラミックビーズを入れた1.5mLチューブを用いて、Bead Ruptor 24 Homogenizer(OMNI International)によって均質化した。氷水に浸したチューブ内において、均質化された組織を45秒間隔で超音波処理した。その後、マイクロ遠心分離機Sorvall Legend Micro 17Rによって、400×gで10秒間遠心分離した。上清を分取し、U−2 OS細胞でプラークアッセイを行うことによってウイルス価を判定した。筋肉内、卵巣内、および肝臓内におけるウイルスレベルと比較して、腫瘍内におけるウイルスレベルはより高かった。処置からある程度の日数が経過すると、ウイルスは主に腫瘍内に見られるが、臓器内には見られなかった(図19)。
Claims (15)
- サイトカインおよびカルボキシルエステラーゼ酵素を発現する腫瘍溶解性ワクシニアウイルスの組換えWestern Reserve株であって、活性型のチミジンキナーゼを発現しない腫瘍溶解性ワクシニアウイルスの組換えWestern Reserve株を含み、
上記カルボキシルエステラーゼは、ヒトCES2(hCES2)であり、
上記サイトカインは、インターフェロン−ベータ−1であり、
上記腫瘍溶解性ワクシニアウイルスの組換えWestern Reserve株は、機能性のB18R遺伝子を有しており、
哺乳動物の癌を治療する方法に用いるための、組成物。 - 上記カルボキシルエステラーゼ酵素の発現は、後初期VACVp7.5プロモーター、ワクシニア改変H5(mH5)プロモーター、ワクシニア短縮合成初後期pSプロモーター、pC11Rプロモーター、pF11Lプロモーター、psFJ1−10合成初期プロモーター、pHyb合成初期プロモーター、任意の未改変ワクシニア初期プロモーター、および、後初期最適化(LEO)プロモーターの制御下にある、請求項1に記載の組成物。
- 上記サイトカインの発現は、後初期VACVp7.5プロモーター、ワクシニア改変H5(mH5)プロモーター、ワクシニア短縮合成初後期pSプロモーター、pC11Rプロモーター、pF11Lプロモーター、psFJ1−10合成初期プロモーター、pHyb合成初期プロモーター、任意の未改変ワクシニア初期プロモーター、および、後初期最適化(LEO)プロモーターの制御下にある、請求項1または2に記載の組成物。
- 上記ワクシニアウイルスは、顆粒球マクロファージコロニー刺激因子(GM−CSF)、シトシンデアミナーゼタンパク質、および、ソマトスタチン受容体2型タンパク質のうち1つ以上を発現する、請求項1〜3のいずれか1項に記載の組成物。
- 上記ワクシニアウイルスは、活性型のワクシニア成長因子(VGF)遺伝子を発現しない、請求項1〜4のいずれか1項に記載の組成物。
- 上記ワクシニアウイルスはWestern Reserve株であり、上記カルボキシルエステラーゼはC末端保持配列を有するヒトCES2酵素であり、上記サイトカインはヒトインターフェロン−ベータ−1である、請求項1に記載の組成物。
- 上記組成物は、1×106〜1×1012プラーク形成単位(pfu)を含む、請求項1〜6のいずれか1項に記載の組成物。
- 上記癌は、結腸直腸癌、肺癌、黒色腫、膵臓癌、卵巣癌、子宮頸癌、または肝臓癌である、請求項1〜7のいずれか1項に記載の組成物。
- 上記哺乳動物はヒトである、請求項1〜8のいずれか1項に記載の組成物。
- 上記癌は、下記(i)〜(v)のうち1つ以上を満たす、請求項1〜9のいずれか1項に記載の組成物:
(i)1つ以上の化学療法薬を用いた治療に対して難治性、
(ii)1つ以上の抗体を用いた治療に対して難治性、
(iii)トポイソメラーゼ阻害剤を用いた治療に対して難治性、
(iv)フルオロピリミジンおよびオキサリプラチンを含む治療に対して難治性、
(v)セツキシマブおよび/またはパニツムマブを含む治療に対して難治性。 - 上記癌は結腸直腸癌である、請求項1〜10のいずれか1項に記載の組成物。
- 上記組成物を、1回以上の投与量で、1×106〜1×1012プラーク形成単位(pfu)、腫瘍内投与または静脈内投与する、請求項1〜11のいずれか1項に記載の組成物。
- (a)上記組成物は、(b)癌用コドラッグを更に含む有効量の組み合わせの一部である、請求項1〜12のいずれか1項に記載の組成物。
- 上記癌用コドラッグは、イリノテカンである、請求項13に記載の組成物。
- 下記(i)〜(iii)の1つ以上を満たす、請求項13または14のいずれか1項に記載の組成物。
(i)相乗作用をもたらす有効量の(a)および(b)を投与する、
(ii)逐次的に、同時に、または個別に(a)および(b)を投与する、
(iii)(a)および(b)を、同じ製剤または異なる製剤で、上記哺乳動物に同時投与する。
