Description
本出願は、2015年4月20日に出願された「血小板様タンパク質微粒子およびそれらを薬剤送達に用いる方法」という表題の米国仮特許出願第62/149,849号の出願日の利益を主張するものであり、その内容の全体が、参照により本明細書に組み込まれている。
本明細書に記載のタンパク質微粒子として、1種類以上の血小板膜タンパク質を含む任意の微粒子が可能であり、その血小板膜タンパク質は、その微粒子の表面に提示することができる。いくつかの実施態様では、本明細書に記載のタンパク質微粒子は血小板様プロテオリポソーム(PLP)である。PLPとは、リポソームの膜の中に通常は人工的手段で挿入された1種類以上の血小板膜タンパク質を有するリポソーム様ビヒクルを意味する。PLPは、中に1種類以上の血小板膜タンパク質が挿入されたリポソームを含むことができる。血小板膜タンパク質の少なくとも一部をPLPの表面に露出させ、そのタンパク質が結合パートナー(例えば単球などの循環血液細胞の表面上の受容体)と相互作用できるようにすることができる。いくつかの実施態様では、リポソームの中にある脂質と血小板膜タンパク質の比は、1,000,000:1〜30:1(w/w)の範囲である。いくつかの例では、この比は、1,000:1、30:1〜50:1(w/w)であり、例えば30:1〜40:1、または40:1〜50:1である。
本明細書で用いる「リポソーム」という用語は、内部水性空間を取り囲む外側脂質層膜(例えば、単膜リポソームとして知られる単一脂質二重層、または多重膜リポソームとして知られる多重脂質二重層)を含む組成物を意味する。例えばCullis他、Biochim. Biophys Acta、第559巻:399〜420ページ(1987年)を参照のこと。単膜リポソームは、一般に直径が約20〜約400ナノメートル(nm)、または約50〜約300 nm、または約300〜約400 nm、または約100〜約200 nmの範囲である。多重膜リポソームは、通常は直径が約1〜約10マイクロメートルの範囲であり、2〜数百の間の任意の数の同軸になった脂質二重層を水相の層と交互に含むことができる。
本明細書に記載のプロテオリポソーム(PLP)などのタンパク質微粒子は、1種類以上の血小板膜タンパク質を含み、その血小板膜タンパク質はPLPの表面に提示されることが好ましい。いくつかの実施態様では、その1種類以上の血小板膜タンパク質は、活動していない血小板および/または一部が活性化された血小板の表面にだけ存在し、活性化された血小板の表面には存在しない。血小板膜タンパク質は、p-セレクチン(CD62p)、CD40L、CD18、またはこれらの組み合わせを含むことができる。あるいは血小板膜タンパク質は、CD40L、またはCD18、またはその両方を実質的に含んでいない(例えばCD40Lを含まない、またはCD18を含まない、またはその両方を含まない)。いくつかの実施態様では、血小板膜タンパク質は、GPIIb、および/またはCD42c、および/または下記の表2に掲載した1種類以上のタンパク質、例えば循環血液細胞(例えば単球)との相互作用に関与するタンパク質を含むことができる。いくつかの例では、本明細書に記載のPLPは、活動していない血小板および/または一部が活性化された血小板から従来の技術(例えば下記の実施例に記載した方法)によって単離した膜タンパク質の混合物を含んでいる。
本明細書に記載のどのタンパク質微粒子も、治療剤、例えば心臓保護剤(例えば抗炎症剤、抗アポトーシス剤、抗線維形成剤、免疫調節剤、血管新生促進剤)を封入することができる。
本開示により、本明細書に記載の1種類以上の治療剤を封入することのできる本明細書に記載のプロテオリポソームなどの任意のタンパク質微粒子と、医薬として許容可能な基剤または賦形剤を含む医薬組成物も提供される。この医薬組成物に含まれる基剤は、その組成物の活性成分と適合するという意味で「許容可能」でなければならず、しかもその活性成分を安定化し、治療する被験者にとって有害でないことが好ましい。
本開示により、治療剤を標的部位に送達するのに用いるキット、または抗IHD剤(例えば抗炎症剤)を心臓の梗塞領域に送達することによってIHDを治療するのに用いるキットも提供される。このようなキットは、本明細書に記載の任意の医薬組成物を収容した1つ以上の容器を含むことができる。その医薬組成物には、タンパク質微粒子(例えば、治療剤を封入したプロテオリポソームやナノ粒子など)と、医薬として許容可能な基剤が含まれている。

有効な心臓保護治療戦略の開発は難しい状態が続いている。というのも、可能性のある多くの心臓保護薬が基礎医学での結果を臨床での結果に結びつけるのに失敗しているからである。鍵となる問題の1つは、梗塞を起こした心臓を標的とした薬剤送達の最適化である。封入された薬剤を梗塞領域へと能動的に送達するための薬剤送達系がいくつか報告されてきたが、そうした薬剤送達系の機能化された表面は、薬剤を標的部位により長く保持すること、または薬剤の循環半減期をより長くすることを可能にしているだけである。したがって、これら送達系のための薬剤送達の主要経路として、増大した透過性と保持(EPR)の効果が相変わらず必要とされている。
動物実験
すべての研究で使用する8週齢のオスのBALB/cマウスは国研院動物中心(台湾国)から購入し、餌と水に自由にアクセスできるようにして12時間夜/昼のサイクルを維持した。
I/RとMI両方のマウスモデルにおける心臓機能を、Chenら((2015年)、Stem Cells Trans Med 第4巻:269〜275ページ)に公開されている方法に従い、30-MHzプローブ(Vevo770;Visual Sonics社、トロント、オンタリオ州、カナダ国)を用いて評価した。マウスを最初に左側面位の状態にした。胸骨傍長軸画像をMモード画像と2次元心エコー画像の両方について取得した。乳頭筋挿入部位の位置で心室の長軸に垂直に左心室拡張終期直径(LVEDD)と左心室収縮終期直径(LVESD)を測定した。LVEVは、心エコーシステムが(LVEDV - LVESV)/LVEDV×100%によって自動的に計算した。この式でLVEDVは左心室拡張終期体積であり、7.0×LVEDD3/(2.4 + LVEDD)として計算され、LVESVは左心室収縮終期体積であり、7.0×LVESD3/(2.4 + LVESD)として計算される。
不活性化された血小板または一部が活性化された血小板を、中央研究院の生物医学科学研究所からの倫理的承認を得た上で、ヒトドナーから回収した。血液を、酸性クエン酸デキストロース(ACD)で抗凝固処理した真空容器(BD Sciences社、カタログ番号366450)の中に回収した。血液を室温にて350×gで20分間遠心分離することにより、血小板が豊富になった血漿(PRP)を用意した。上層(PRP)を新しい試験管に移し、下層を廃棄した。次にPRPを室温にて1,200×gで遠心分離すると、血小板が少ない血漿を含む血小板ペレットが上清として得られた。この血小板ペレットをタイロードの緩衝液(1.8 mM CaCl2、1 mM MgCl2、2.7 mM KCl、136.9 mM NaCl、0.4 mM NaH2PO4、11.9 mM NaHCO3、5.6 mM D-グルコース、0.1U/mlアピラーゼ)に再懸濁させ、再び室温にて1,200×gで10分間遠心分離した。上清を廃棄し、血小板を1 mlのタイロードの緩衝液に再懸濁させた。
精製したヒト血小板膜タンパク質溶液に含まれる膜タンパク質の同定は、IBMS 蛋白質体核心設施(中央研究院、台湾国)で実施した。
マウス内皮細胞SVEC(CRL-2181、ATCC)と単球RAW264.7細胞(TIB-71、ATCC)を、2 mMのグルタミンと10%ウシ胎仔血清を含むダルベッコ改変イーグル培地(DMEM)の中で培養した。Zhangら((2008年)、Curr Protoc Immunol第14巻:14.1ページ)に従ってマウス腹腔マクロファージ(MΦ)を成体マウスから単離した。簡単に述べると、マウスを2 mlの3%チオグリコール酸塩に少なくとも3日間曝露した。冷たいPBS(10 ml)を用いて腹腔滲出細胞を回収した。その細胞を37℃で2時間放置して組織培養プレートに接着させた後、新鮮な培地(DMEM/F-12+10%FBS)と交換した。
サンプルを4〜12%SDS-ポリアクリルアミドゲル(BioRad社、アメリカ合衆国)上で分離した。PVDF膜に移した後、一次抗体と二次抗体を適用し、信号をECL-プラス試薬で検出した。ヒトGPIIbに対する一次抗体(GTX113758)、ヒトCD42cに対する一次抗体(GTX113355)、β-アクチンに対する一次抗体(GTX109639)をGeneTex社(アメリカ合衆国)から購入した。抗ヒトCD62P(sc-19672)はSanta Cruz社(アメリカ合衆国)からのものであった。ウサギポリクローナル抗HO-1抗体は、Linら((2013年)、Arterioscler Thromb Vasc Biol 第33巻:785〜794ページ)に従って自作した。
サンプルをスクロース溶液(15%w/v)の中で6時間脱水し、次いで濃(30%w/v)スクロース溶液に一晩入れた後、-20℃で組織凍結媒体の中に埋め込み、低温で切片にした。厚さ5μmの各切片をスライドガラスの上に装着した後、マッソンの三色染色プロトコルに従ってヘマトキシリン-エオシンで染色した。染色した切片を、顕微鏡(Axiovert200M;Zeiss社、ドイツ国)に取り付けたディジタルカメラ(モデルD30、日立製作所、日本国)で撮影した。
空リポソームとPLPをDiIC18(Life Technologies社、アメリカ合衆国)で標識した。その後、DiIで標識した両方の材料とさまざまなタイプの細胞の相互作用を調べた。細胞を最初にスライドガラスの上に播種し、どちらかの材料に37℃で6時間曝露した。結合しなかったあらゆる材料を温かいPBSで洗い流した。その後、スライドガラスの上に播種した細胞をカバースリップで覆い、Zeiss社のAxioscope顕微鏡で画像を取得し、AxioVisionソフトウエアで処理した。凍結させた心臓切片に含まれるDiIで標識した空リポソームとPLPを免疫検出するため、マウス心筋細胞を染色するための抗マウストロポニンI(DSHB社、アメリカ合衆国)でその心臓切片を標識し、核をDAPI(0.1 mg/ml)で対比染色した。画像をやはりZeiss社のAxioscope顕微鏡で取得し、AxioVisionソフトウエアで処理した。
成体マウスから心臓を切除した後、その組織を冷たいハンクス平衡塩溶液(HBSS)+1%FBSで3回洗浄した。心臓を灌流して過剰な血液を除去した後、200μlのディスパーゼII溶液(5 U/mlディスパーゼII、5 mM CaCl2、0.1 U/mlコラゲナーゼB)の中に入れた。次に、カミソリの刃で心臓を切断して細かいメッシュにした後、5 mlのディスパーゼII溶液の中で37℃にて30分間インキュベートした。その後、5 mlのDMEM+10%FBSを溶液に添加し、氷の上で40μmのフィルタを用いて濾過した。濾液を保管し、4℃にて1,000 rpmで5分間遠心分離した。次に、ペレットでフローサイトメトリーを実施した。
細胞を、DiIで標識した空リポソームまたはDiIで標識したPLPに曝露した後、温かいPBSで洗浄した。室温にて1,000 rpmで5分間遠心分離することにより、結合しなかった空リポソームまたはPLPを除去した。その後、細胞ペレットを5%ヤギ血清の混合物とともに氷の上で30分間インキュベートした。サンプルを4℃にて1,000 rpmで5分間遠心分離した後、それぞれの一次抗体、すなわち抗マウスF4/80 APC(MCA497APC、AbDSerotec社、アメリカ合衆国)、または抗マウスCD11b(14-0112、eBioscience社、アメリカ合衆国)、または抗CD144(555289、BD Science社、アメリカ合衆国)とともに4℃にて暗所で1時間インキュベートした。過剰な一次抗体を4℃にて1,000 rpmで5分間遠心分離することによって除去した。その後、サンプルを蛍光標識したそれぞれの二次抗体とともに4℃にて暗所で1時間インキュベートした。過剰な二次抗体を遠心分離によって除去した後、サンプルに対してフローサイトメトリー分析(BD Science社、アメリカ合衆国)を実施した。
TRI試薬(1 ml/心臓全体)を用いて全RNAを心臓全体から単離した。その後、ランダムプライマーミキサー(ProtoScript M-MuLV First Strand cDNA合成キット、New England BioLabs社)を用いて1μgのRNAをcDNAに転写し、特異的プライマー(表1)を用いてPCRにより35サイクルかけて増幅した。反応物に対して95℃で1分間、58℃で1分間、72℃で1.5分間のサイクルを実施し、変性、アニーリング、伸長がそれぞれ可能になるようにした。次に、TBE緩衝液(80 mMトリス塩基、80 mMホウ酸、2 mM EDTA、pH 8)の中の1%(w/v)寒天ゲル上で60 Vにて1時間にわたってPCR産物を分離した。HealthView Nucleic Acid Stain(Genomics社、台北、台湾国)を用いてゲルを30分間染色した後、UV光のもとで可視化した。
血小板様プロテオリポソームの調製は、薄膜水和法に基づいていた。Jang他、(2012年)、PNAS第109巻:1679〜1684ページ。方法の一例を図1Aに示す。1,2-ジオレオイル-sn-グリセロ-3-ホスホコリン(DOPC)の10 mg/ml貯蔵溶液と3.28 mgのコレステロール(Avanti Polar Lipids社)の10 mg/ml貯蔵溶液を1 mlのクロロホルムとメタノール(9:1 v/v)に溶かし、体積にて6:4のモル比で混合した。薄膜をロータリーエバポレータによって形成した。脂質にDiIC18(Life Technologies社、アメリカ合衆国)の標識があらかじめ付けられている場合、染料を原初の混合物に添加した後、薄膜形成工程(10 mgのDOPCにつき20μl)を実施した。次に、薄膜を1 mlのHEPES-PBS緩衝液に再懸濁させると、脂質の最終濃度は約10 mg/mlであった。凍結-解凍を何回か実施した後、脂質溶液を100 nmのポリカーボネート膜(Whatman社、アメリカ合衆国)を通過させて押し出した。
その後のHPLC分析のため、Chenら((2015年)、Nanoscale 第7巻:15863〜15872ページ)に従ってサンプルを調製した。DiIで標識した空リポソームまたはPLPを注射してから4時間後、すべてのマウスにPBSを灌流して血管内のDiIで標識したすべての材料を洗い流した。その後マウスを安楽死させ、主要な臓器(脳、肺、心臓、肝臓、腎臓、脾臓)を手早く回収した。回収した組織を約100 mgのいくつかの断片に切断して計量した。IPA緩衝液(0.5 ml、0.075 M HCl と混合した10%イソプロパノール、9:1 v/v)を各サンプルに添加した後、ジルコニアビーズを備えるMagNALyser装置(Roche社、マンハイム、ドイツ国)を用いて完全なホモジェネートにした。次に、ホモジェネートになったサンプルを4℃にて3,000 rpmで20秒間遠心分離した後、0.5 mlのIPA緩衝液をさらに添加した。サンプルを4℃で一晩放置した後、4℃にて14,000 rpmで15分間遠心分離した。抽出された蛍光染料を含む上清を取り出して希釈し、HPLC分析を実施した。HPLCは、Waters社のe2695 Separation ModuleとWaters社の2475 FLR Detector(アメリカ合衆国)を用いて実施した。X-Bridge C18カラム(250×4.6 mm、5μm, Waters社、アメリカ合衆国)を40℃で使用し、蛍光検出器を励起波長505 nm、発光波長515 nmに設定した。移動相はメタノールと脱イオン水(77:23、v/v)からなり、流速は1 ml/分であった。DiIで標識した既知の濃度の空リポソームまたはPLPの段階希釈によってHPLC標準を測定した。
クライオEM画像を得るためのサンプル調製と撮影は、独立したスタッフが中央研究院(台湾国)の学術及儀器事務所の低温電顕核心実験室で実施した。簡単に述べると、Tecnai F20電子顕微鏡(FEI)を200 keVで用いて空リポソームまたはPLPの画像を取得した。各曝露に関する低用量条件は、約20e-/Å2であった。焦点を2〜3μm外して画像を取得し、4k×4k CCDカメラ(Gatan社、アメリカ合衆国)に記録した。
生体多光子イメージング実験のためのイメージング手続きは、以前にLeeら((2012年)、Nat Commun 第3巻:1054ページ)が記載しているようにして実施した。実験の前に、耳を脱毛し、75%エタノールと水で拭った。多光子レーザービームを耳の真皮の限られた領域に約30秒間フォーカスさせることによってレーザー損傷を誘導した。すべてのマウスを2.5%イソフラン(Minrad社)で麻酔した後、実験中は1.5%イソフランの状態を維持した。FITC-デキストランを尾の静脈から静脈内注射してマウスの血管を標識し、マウスを空リポソーム処理群またはPLP処理群に分けた(n=3)。4時間安定化させた後、FVMPE-RSマルチモード多光子走査顕微鏡(Olympus社)を用いて組織損傷領域を撮影した。その後、DiIで標識した5 mg/mlの空リポソームまたはPLPを尾の静脈から100μl静脈内注射し、その直後から30分後まで耳の組織損傷領域を観察した。その間、画像を5分ごとに撮影した。
CoPPに曝露した細胞内で誘導されたHO-1の比活性を求めるため、分析した細胞抽出物に含まれるビリルビンの量を測定した。Lam他、(2005年)、J Immunol第174巻:2297〜2304ページ。マグネシウムを補足したリン酸カリウム溶液(0.1M KPO4 と2 mM MgCl2;pH 7.4)に細胞ペレットを再懸濁し、凍結-解凍サイクルを3回実施し、超音波処理によって細胞質のHO-1タンパク質を放出させた。HO-1酵素アッセイでは、100 mM PBS、2 mM MgCl2、3 mgのラット肝臓サイトゾル、0.8 mM NADPH、2 mMグルコース-6-リン酸塩、0.2 Uグルコース-6-リン酸デヒドロゲナーゼ、20μM酵素基質ヘミンを含む反応混合物と、1mgのサンプルを用いた。反応を各サンプルの最終体積が1 mlになるまで行なわせ、暗所で37℃にて1時間インキュベートした。クロロホルムを添加して反応を終了させ、ビリルビンを遠心分離によって抽出し、分光測光によって464 nmと530 nmでの吸光度の差として測定した(ビリルビンの消光係数は40 mM-1cm-1)。各サンプル中のタンパク質の濃度をBradfordタンパク質アッセイで求め、HO-1活性を、1時間にタンパク質1 mgにつき形成されたビリルビンのマイクロモル数として表わした。
血液検査はすべて、台湾小鼠診所で独立したスタッフが実施した。約56週齢のオスのBALB/cマウスをすべての分析で用いた(各群でn=5)。
一元ANOVA(分散の分析)検定の後、チュキー事後検定を利用して試験条件下での処理の間の統計的有意性を比較した。p値が0.05未満を有意であると見なした。
ヒト血小板からの血小板膜タンパク質の精製
精製した血小板膜タンパク質溶液が細胞質タンパク質をまったく含んでいないことを確認するため、SDS-PAGE分析とウエスタンブロッティング分析を実施した(それぞれ図1Dと図1E)。SDS-PAGEにより、精製したPMPを装填したレーンで多数のバンドが明らかになった。これらのバンドは、さまざまなPMPを表わしている可能性が大きい(白い長方形、図1D)。約42 kDaの位置にある共通のバンドはβ-アクチンを表わしており(Moebius他、(2005年)、Mol Cell Proteomics 第4巻:1754〜1761ページ)、精製したヒトPMPを除く全サンプルで検出された(黒い長方形、図1D)。β-アクチンは血小板の中の主要な細胞質タンパク質である(Lewandrowski他、(2009年)、Blood第114巻:E10〜E19ページ)ため、その不在は、精製したPMP溶液が細胞質タンパク質によって実質的に汚染されていないことを示唆していた。次に、検出されたバンドのいくつかが何であるかをウエスタンブロッティングによって確認した。
薄膜水和法(Jang他、(2012年)、PNAS 第109巻:1679〜1684ページ)によってPLPのバッチを調製した。このPLP は、DOPCとコレステロールの混合物(9:1、w/w)と、精製したヒトPMPを30:1の比にしたものからなる。クライオEM画像では、空リポソームは不規則な形と凝集を示した(図1F)。それに対してPLPでは一様な円形が見られた。しかしクライオEM画像から、PLPのサイズが揃っておらず、すべてのPLPが約100 nmのサイズになっているわけではないことがわかった。動的光散乱(DLS)測定から同様の結果が明らかになり、平均すると大半のPLPはサイズが100 nmに近い(PDI=0.077)のに対し、空リポソームは平均サイズが約130 nmであった(PDI=0.12、表3)。重要なことだが、PLPの全表面電荷は空リポソームと比べてより大きな負の値であることが示された。血小板膜タンパク質は負に帯電していることが知られているため、これは、DOPC系リポソームへのヒトPMPの共役が成功したことを示唆していた。Lewandrowski他、(2009年)、Blood 第214巻:E10〜E19ページ。さらに、PLPの表面にヒトPMPが存在することが、ウエスタンブロッティングによって確認された(図1G)。なぜなら抗ヒトGPIIb抗体と抗ヒトCD42c抗体の両方ともPLPに対してプラスの反応性を持っていたが、空リポソームに対してはプラスの反応性を持たなかったからである。
ヒトPMPが機能的役割を持つことを証明するため、DiIで標識した空リポソームとDiIで標識したPLPを、マウス内皮細胞(SVEC)、マウス単球(RAW264.7)、マウス腹腔マクロファージ(MΦ)とともに37℃で4時間インキュベートした。その後、DiIで標識した空リポソームまたはDiIで標識したPLPで過剰なもの、または結合しなかったものを除去し、次いでフローサイトメトリー分析を実施した(図3A)。空リポソームとは異なり、PLPはRAW264.7に対して強い結合親和性を示したが、SVECに対しては結合親和性を示さなかった。MΦのファゴサイトーシス活性が原因で、空リポソームとPLPの両方とも、MΦでは他の2つのタイプの細胞におけるよりもDiI信号が小さかった。それに加え、PLPをMΦに曝露すると多数の小胞が形成され(白い矢印、図3B)、いくつかの小胞の中にPLPを見ることができた(白い矢印、図3B)。
傷の治癒における炎症反応はCHD患者で見られるのと同様であるため、レーザーで誘導したマウスの耳組織の傷をモデルとして、空リポソームとPLPの標的プロファイルを生体内で調べた。損傷を作り出した後、マウスを約48時間放置し、次いでDiIで標識した空リポソームまたはPLPをマウスの尾の静脈を通じて静脈内注射した。注射するとき、2光子顕微鏡のレンズの焦点を損傷領域に合わせ、2種類のリポソームの画像を30分後まで5分ごとに取得した。PLPを注射したマウス(図4B)と比較すると、注射した空リポソームのうちのごく少量(白い矢印の先頭、図4A)が損傷領域に見られた。30分後の時点で撮影した画像から3D回転画像を構成すると、DiIで標識したPLPは大量に組織損傷部位に浸潤した一方で、ごく少量の空リポソームしか損傷組織では見られないことがわかった。損傷領域に存在していなかった血管を30分後の時点で可視化すると、DiIで標識した空リポソームが大量に存在していることがわかった(図4C)。逆に、損傷部位の外側の領域ではDiIで標識したPLPがごく少量しか検出されなかった(図4D)。PLPを注射したマウスは、PBSまたは空リポソームで処理したマウスと比べて損傷部位で有意に大きいDiI信号を示した(図9)。結論として、生体多光子イメージングデータから、PLPは空リポソームよりも損傷部位のほうを標的とすることが証明された。これは、リクルートされた単球に背負われたことによる可能性が大きい。
PLPを臨床で冠動脈心疾患(CHD)患者に応用できることを証明するため、空リポソームとPLPの組織分布をI/R損傷のマウスモデルで調べた。マウスに外科的に誘導した虚血を45分間起こした後、再灌流させた。ヒトCHD患者では、梗塞を起こした心臓にリクルートされる単球の数が梗塞の72時間後にピークに達すること(van der Laan他、(2014年)、Eur Heart J 第35巻:376〜385ページ)が明らかにされているため、再灌流の24時間後または72時間後に、DiIで標識した5 mg/kgの空リポソームまたはPLPをマウスに100μl注射した。その後、空リポソームとPLPの両方を4時間循環させてからマウスを安楽死させ、臓器中のDiI信号のレベルをHPLC分析で調べた(図5Aと図5B)。
コバルトプロトポルフィリンIX(CoPP)は、ヘムオキシゲナーゼ-1(HO-1)の発現を通じてマクロファージの炎症活性を抑えることが知られている小分子である(図10A)。HO-1は、ヘムが壊れてビリベルジンと一酸化炭素(CO)と鉄になる触媒として機能した。COおよびビリベルジンと、ヘム最終分解代謝産物であるビリルビンは、強い抗酸化活性と抗炎症活性を持つことが知られていて(Zao他、(2013年)、PLoS One第8巻:e75927ページ)、副生成物である鉄は、抗アポトーシス活性を持つフェリチンの合成に関与することが明らかにされている(Zao他、(2013年)、PLoS One第8巻:e75927ページ)。CoPPを用いた治療によって心臓の梗塞領域が有意に小さくなることが示されている。Sodhi他、(2015年)、J Cardiol Ther第2巻:291〜301ページ;Cao他、(2012年)、Front Physiol第3巻:160ページ;Chen他、(2013年)、Int J Mol Sci第14巻:2684〜2706ページ。そこで、I/R損傷のマウスモデルにおいてPLPに封入したCoPP(PLP-CoPP)を静脈内注射すると同様の治療効果が見られるかどうかを調べた。
MIの動物モデルにおけるCoPPの治療上の利点が文献によく記載されているが、その望まない標的外効果も報告されている。Schmidt、(2007年)、FASEB J 第21巻:2639ページ;Ryter他、(2006年)、Physiol Rev 第86巻:583〜650ページ。PLP-CoPPの全体的効果を評価するため、非再灌流MIのマウスモデルにおいてPLP-CoPP処理後の心臓機能を最初に調べた。虚血を外科手術によって誘導した後、マウスに5 mg/kgの単独のCoPP、リポ-CoPP、PLP-CoPPを梗塞の3日後に100μl静脈内注射した。その後、マウスに同じ用量を梗塞の28日後まで5日ごとに投与した(図7A)。MIの28日後に全処理群でマウスの心臓機能を心エコー検査によって評価した(図7B)。血液が心臓と1回接触するごとに心臓を離れる血液の量を測定する左心室駆出率(LVEF、%)は、MI+PLP-CoPPの群において、MI+生理食塩水の群、MI+リポ-CoPPの群、2種類のビヒクルだけの群よりも有意に大きかった。MI+CoPPの群とMI+PLP-CoPPの群の間に有意差はなかった。同様の結果が、心筋の厚さの影響を受ける短縮率(FS%)の測定でも見られ、PLP-CoPPで処理した群は、MI+CoPPの群を除く他の処理群と比べてFS%の有意な改善を示した。左心室拡張終期体積(LVEDV)と左心室収縮終期体積(LVESV)における血液の体積もすべての群で評価した。単独のCoPPまたはPLP-CoPPでの処理は、シャム群で見られたのと同じレベルまでLVEDVとLVESVを下げることはなかったが、それでも結果は、他の処理群と比べて有意に優れていた。同様に、拡張期における心室間隔膜厚(IVSd)または収縮期における心室間隔膜厚(IVSs)の測定値はさらに、PLP-CoPPが、単独のCoPPで処理した場合と同様にマウスの心臓機能全体を改善させることを示していた。
潜在的な心臓保護薬の基礎医学における有望な結果を臨床に移すことは困難な仕事に留まっている。過去数十年にわたって投資が続けられているにもかからわず、有効な心臓保護薬は相変わらず市販されていない。Altamirano他、(2015年)、J Physiol 第593巻:3773〜3788ページ;Perricone他、(2014年)、Pharmacol Res 第89巻:36〜45ページ;Sluijter他、(2014年)、Pharmacol Ther 第144巻:60〜70ページ。臨床でうまくいっていないいくつかの結果は、不適切な動物モデルとヒト患者の個々のライフスタイルに帰されてきた。しかし全身に送達される薬剤の標的特異性を最大にすることもまだ実現されないままである。例えばアメリカ合衆国のFDAがII型糖尿病を治療するための抗糖尿病薬として認可したメトフォルミンは、いくつかの動物モデルで心臓保護効果を持つことが示されている。Whittington他、(2013年)、Cardiovasc Drug Ther 第27巻:5〜16ページ。しかし冠動脈バイパス手術の間にCHD患者をメトフォルミンであらかじめ治療した最近の臨床試験は、心筋の損傷を有意に減らすことに失敗した。El Messaoudi他、(2015年)、Lancet Diabetes Endocrinol 第3巻:615-623ページ。さらに、メトフォルミンで治療した群の患者は、プラセボ群と比べて下痢やそれ以外の胃腸の不快感の発生が有意に多かった。したがって臨床の結果は、メトフォルミンが、治療した患者の心臓保護機能を持たないだけでなく、不利な効果も誘導することを示していた。報告されているいくつかの薬剤送達系は、梗塞を起こした心臓に能動的に送達されると主張されているが、これら送達系の機能化された表面は、標的部位にそれをよりよく保持すること、および/またはその循環半減期を長くすることを可能にするだけである。Dvir他、(2011年)、Nano Lett 第11巻:4411〜4414ページ;Yan他、(2014年)、Biomaterials 第35巻:1063〜1073ページ;Chang他、(2013年)、J Control Release 第170巻:287〜294ページ;Nguyen他、(2015年)、Adv Mater 第27巻:5547〜5552ページ。これら送達系は、標的部位に到達する主要経路として相変わらずEPR効果に頼っている。いくつかの報告と臨床研究が、今や、EPR効果は、がんの治療を含め、もはや依存できる薬剤送達戦略ではないことを示している。Nichols他、(2014年)、J Control Release第190巻:451〜464ページ。したがって本明細書に開示した単球を媒介とする送達戦略は、薬剤送達の真に能動的な形態である。

- タンパク質微粒子であって、該タンパク質微粒子は、活動していないかまたは一部が活性化された血小板の膜タンパク質の混合物とリポソームとを含むプロテオリポソームであり、ここで、該膜タンパク質の混合物は、CD62、GPIIbおよびCD42cを含み、そして該タンパク質微粒子は、損傷部位に移動することができる循環血液細胞に結合する、タンパク質微粒子。
