本発明の方法に従って対象に投与されるIL−33アンタゴニスト(例えば、抗IL−33抗体)の量は、一般的に治療有効量である。本明細書で使用される場合、「治療上有効な量」という用語は、IL−33媒介疾患または障害の1以上の症状または徴候の検出可能な改善をもたらすIL−33アンタゴニストの量を意味する。「治療に有効な量」はまた、対象におけるそのような病気または障害の進行を阻害、予防、軽減、または遅延させるIL−33アンタゴニストの量を含む。

ヒト免疫グロブリン重鎖およびκ軽鎖可変領域をコードするDNAを含むVELOCIMMUNE(登録商標)マウスに、免疫応答を刺激するためのアジュバントと共にヒトIL−33を含む免疫原を直接投与した。抗体の免疫応答を、IL−33特異的イムノアッセイによってモニターした。所望の免疫応答が達成されると、脾細胞を採取し、マウス骨髄腫細胞と融合させて、それらの生存率を維持し、ハイブリドーマ細胞株を形成させた。ハイブリドーマ細胞株をスクリーニングし、IL−33特異的抗体を産生する細胞株を同定するために選択した。この技術を用いて、いくつかの抗IL−33キメラ抗体(すなわち、ヒト可変ドメインおよびマウス定常ドメインを有する抗体)が得られ、このようにして生成された例示的な抗体を、H1M9559N、H1M9566N、H1M9568NおよびH1M9565Nと命名した。その後、キメラ抗体由来のヒト可変ドメインをヒト定常ドメイン上にクローニングして、本明細書に記載の完全ヒト抗IL−33抗体を作製した。

本発明の5つの異なる例示的なIL−33アンタゴニスト(IL−33トラップ)を、標準的な分子生物学的技術を用いて構築した。第1のIL−33アンタゴニスト(hST2−hFc、配列番号323)は、そのC末端がヒトIgG1 Fc領域(配列番号331)のN末端に融合したヒトST2の可溶性細胞外領域(配列番号327)からなる。第2のIL−33アンタゴニスト(hST2−mFc、配列番号324)は、そのC末端がマウスIgG2a Fc領域(配列番号332)のN末端に融合したヒトST2の可溶性細胞外領域(配列番号327)からなる。第3のIL−33アンタゴニスト(hST2−hIL1RAcP−mFc、配列番号325)は、そのN末端にヒトST2(配列番号327)を有し、続いてヒトIL−1RAcP(配列番号329)の細胞外領域、続いてそのC末端にマウスIgG2a Fc(配列番号332)が続くインライン融合からなる。第4のIL−33アンタゴニスト(mST2−mIL1RAcP−mFc、配列番号326)は、そのN末端にマウスST2(配列番号328)、続いてマウスIL−1RAcPの細胞外領域(配列番号330)、続いてそのC末端にマウスIgG2a Fc(配列番号332)を有するインライン融合物からなる。第5のIL−33アンタゴニスト(hST2−hIL1RAcP−hFc、配列番号335)は、配列番号327のヒトST2をそのN末端に有し、続いてヒトIL−1RAcPの細胞外領域(配列番号329)、続いてそのC末端にヒトIgG1 Fc(配列番号331)を有するインライン融合物からなる。表2aは、異なるIL−33アンタゴニストおよびそれらの成分部分の概要を記載する。表2bは、IL−33アンタゴニストおよびそれらの成分部分のアミノ酸配列を示す。
IL−33を過剰発現するマウスにおいてどの遺伝子が上昇しているかを調べるための研究が行われた。以下に記載する研究は、どの遺伝子がインビボでIL−33発現と相関し、どの遺伝子がIL−33アンタゴニストで動物を治療した後に調節されたかを決定するために使用される方法および動物モデルを要約する。
マウスIL−33は、流体力学的DNA送達(HDD)によって野生型(WT)マウスで過剰発現した。HDD実験のために、WTマウスに、全長マウスIL−33(GenBank受入番号NM_001164724;mIL−33参照)を発現するプラスミド50μgもしくは25μgで、または、50μgもしくは25μgのコード配列を欠いている同じプラスミド(空ベクター)を注入した。HDD注入の7日後にマウスを屠殺し、血液、後根神経節(DRG)、心臓、腎臓、肝臓、肺および脾臓を採取した。これらの組織で発現される上位10個の遺伝子を以下の表4に示す。
QuickAmp RNA Amplification Kit(Agilent)を用いて500ngの全RNAから増幅したcRNAにCy3−CTPを組み込んだ。次いで、各サンプルからのCy3標識cRNAを、マウス転写物をカバーする43538個の60merオリゴを含むカスタムAgilentアレイにハイブリダイズさせた。アレイのハイブリダイゼーションおよび洗浄は、Agilentプロトコルに従って行い、アレイは、Agilent Microarrayスキャナーでスキャンした。データは、Agilent Feature Extraction Software 9.5を使用してスキャンしたアレイ画像から抽出した。
研究終了時に心臓穿刺により全血をMicrotainer(登録商標)チューブに集めた。血液を室温で少なくとも30分間放置して凝固させた。凝固した血液および細胞を、18,000×g、10分間、4℃で遠心分離することによってペレット化した。得られた上清(指定の血清)を清浄なポリプロピレンプレートに移し、適切に処理または保存した。
R&DシステムからのELISAキットを使用して、製造業者の指示に従って、ヒト(DY3625)およびマウス(DY3626)IL−33のIL−33濃度を決定した。
マウスカルシトニンを、CusaBio(Through ARP)Cat#CSB−E05133mのマウスカルシトニン(CT)ELISAキットを用いてマウスの血清中で測定した。アッセイ手順は、キットと共に提供される製造者の指示に従って実施した。
肺タンパク質抽出物中のサイトカイン濃度を、V-Plexカスタムマウスマルチプレックスイムノアッセイキット(MesoScale Discovery、#K152A41)を使用して、製造者の指示に従って測定した。
IL−33 HumInマウスに20μLの1Xリン酸緩衝生理食塩水(PBS)または20μLの1XPBSで希釈したハウスダストダニ抽出物(HDM;Greer、#XPB70D3A2.5)50μgを1週間に3日間15週間鼻腔内投与した。IL33 HumInマウスの第2のコントロールグループには、1週間に20μLの1XPBSで希釈した50μgのHDM抽出物を4週間または11週間のいずれかに3日間投与して、抗体治療の開始時の病気の重篤度を評価した。2グループのHDMチャレンジマウスに、HDMチャレンジの4週間後または11週間後のいずれかで開始し、その後、HDMチャレンジの終了まで(抗体治療の8または4週間)週2回、抗IL−33抗体、H4H9675P、またはアイソタイプコントロール抗体のいずれかの25mg/kgを皮下注射した。実験の85日目に、全てのマウスを屠殺し、それらの肺を採取した。マウスグループに関する実験的投薬および治療プロトコルを表3に示す。
放血後、肺を取り出し、400μLのRNA Later(Ambion、Cat#AM720)を含むチューブに入れ、処理するまで−20℃で保存した。TRIzol中で組織をホモジナイズし、相分離のためにクロロホルムを使用した。全RNAを含有する水相を、製造業者の仕様書にしたがって、MagMAX(商標)-96 for Microarrays Total RNA Isolation Kitを用いて精製した。MagMAX(商標)Turbo(商標)DNase BufferおよびTURBO DNaseを使用して、上記のMagMAXキットからゲノムDNAを除去した。SuperScript(登録商標)VILO(商標)Master Mixを使用して、mRNA(最大2.5μg)をcDNAに逆転写した。cDNAを2ng/μLに希釈し、ABI 7900HT Sequence Detection System(Applied Biosystems)を用いてTaqMan(登録商標)Gene Expression Master Mixおよび関連プローブ(マウスB2m、マウスCalca、ヒトIL33)で10ngのcDNAを増幅した。cDNA入力の差異を標準化するために、B2mプローブを用いて内部コントロールとしてβ−2ミクロブロブリン(B2m)遺伝子を増幅した。データ分析は、Microsoft ExcelおよびGraphPad Prism(商標)ソフトウェアを用いて行った。各遺伝子の発現は、同じサンプル内でB2m発現に対して正規化された。
放血後、各マウスの右肺の中葉および頭蓋を取り出し、1×停止プロテアーゼ阻害剤カクテル(Pierce、#78430)を補充した組織タンパク質抽出試薬(1×T-PER試薬;Pierce、#78510)の溶液を含むチューブに入れた。全てのさらなる工程は氷上で行った。1:8(w/v)組織対T−PER比に一致するように、各試料についてT−PER試薬(プロテアーゼ阻害剤カクテルを含む)の容量を調節した。肺サンプルを、Tissue Lyser II(Qiagen、Cat#85300)を用いてチューブ中でホモジナイズした。得られた溶解物を遠心分離して破片をペレット化した。可溶性タンパク質抽出物を含有する上清を新しいチューブに移し、さらなる分析まで4℃で保存した。
続希釈物を抽出物のタンパク質濃度を決定するための標準として使用した。室温で5分間インキュベートした後、595nmでの吸光度をMolecular Devices SpectraMax M5プレートリーダーで測定した。GraphPad Prism(商標)ソフトウェアを使用して、BSA標準に基づいて総肺抽出物タンパク質含量を決定するためのデータ分析を行った。
放血後、各マウスの右肺の尾葉を取り出し、Hank's Balanced Salt Solution(HBSS)で希釈した20μg/mL DNaseおよび0.7U/mL Liberase THの溶液を入れたチューブに入れ、約2〜3mmの大きさに切断した。次いで、細断した肺を有するチューブを37℃の水浴中で20分間インキュベートした。エチレンジアミン四酢酸(EDTA)を終濃度10mMで添加することにより反応を停止させた。次いで、サンプルをgentleMACS Cチューブに移した。Cチューブの容量をMACS緩衝液で3mLにし、その後サンプルを解離させて、gentleMACS dissociator(登録商標)(Miltenyi Biotec)を用いて単一細胞懸濁液を形成した。次いで、チューブを遠心分離し、得られたペレットを1X赤血球溶解緩衝液4mLに再懸濁して、赤血球を溶解させた。室温で3分間インキュベートした後、10mLの1XDPBSを加えて赤血球溶解バッファーを失活させた。次いで、細胞懸濁液を遠心分離し、得られた細胞ペレットを1mLの1XDPBSに再懸濁した。再懸濁したサンプルを100μmフィルタープレートを通して遠心分離濾過し(400×gで1分間)、ウェル当たり約1.5×106個の細胞を96ウェルU底プレートに播種した。次いで、細胞を遠心分離し、細胞ペレットを1XDPBSで1:500に希釈した100μLのNear-IR LIVE/DEAD(登録商標)Fixable Dead Cell Stain(Invitrogenカタログ番号:L34976、Lot#:1647137)に再懸濁し、細胞生存率を測定した。細胞は、光から保護されている間、室温で20分間、生存性色素とともにインキュベートされた。1XDPBSで1回洗浄した後、10μg/mLの精製ラット抗マウスCD16/CD32 Fcブロックを含む50μLのMACS緩衝液中で細胞を4℃で10分間インキュベートした。光から保護しながら、4℃で30分間、ブリリアント染色緩衝液(表5に記載)で希釈した適切な2x抗体混合物中で細胞をインキュベートした。抗体インキュベーション後、細胞をMACS緩衝液で2回洗浄し、1XDPBSで1:4に希釈したBD CytoFixに再懸濁し、光から保護しながら4℃で15分間インキュベートした。その後、細胞を洗浄し、MACS緩衝液中に再懸濁した。次いで、細胞懸濁液を、AcroPrep Advance 96フィルタープレート30〜40μmを介して新しいU底プレートに濾過した。サンプルデータは、HTSアタッチメント(BD Biosciences)を使用するLSR Fortessa X-20細胞分析装置で取得した。FlowJo X Software(Tree Star、OR)を用いてデータ解析を行い、GraphPad Prism(商標)(GraphPad Software、CA)を用いて統計解析を行った。好酸球は、生存(細胞生存性色素陰性)、一重項、CD45+、F4/80+、Ly6G-、CD11clo-Int、SiglecFhiとして定義された。好中球は、生存、一重項、CD45+、F4/80-、Ly6G+と定義された。好酸球および好中球のデータを生存細胞の頻度として表した。
統計分析は、GraphPad Prism(商標)バージョン6.0(GraphPad Software、CA)を用いて行った。
結果は、IL−33を過剰発現する動物において乱された上位10の遺伝子がカルシトニン(Calca)レジスチン様アルファ(RETNA)、ケモカイン(C−Cモチーフ)リガンド8(Ccl8)、血清アミロイドA3(Saa3)、Gm1975(BC117090)、キラー細胞レクチン様受容体(Kirg1)、ステフィンA1(Csta)、膜貫通4ドメイン(Ms4a8a)、ケモカイン(C−Cモチーフ)リガンド11(Ccl11)、およびセリン(またはシステイン)ペプチド(Serpina3f)(表4を参照されたい。)であることを実証した。さらに、データはまた、このマウスモデルにおいて、血清IL−33および血清カルシトニンの両方が有意に上昇したことを示した(血清IL−33レベルについては図1A、血清カルシトニンレベルについては図1Bを参照されたい。)。血清IL−33の増加は、カルシトニンの血清レベルの増加と相関していた。
実際、本明細書に記載されたものに加えて、本発明の様々な改変が、前述の説明および添付の図面から当業者に明らかになるであろう。そのような改変は、添付の特許請求の範囲の範囲内に入ることが意図される。
Claims (17)
- 参照カルシトニンレベルに対してカルシトニンレベルの上昇を示すことに基づいてインターロイキン33アンタゴニストによる治療について選択した対象における、気道または肺のインターロイキン33(IL−33)媒介炎症性疾患または障害の治療において使用するための、治療上有効な量のIL−33アンタゴニストを含む医薬組成物であって、前記IL−33アンタゴニストは抗IL−33抗体またはその抗原結合性フラグメントである、医薬組成物。
- 前記カルシトニンレベルはIL−33媒介疾患または障害を有する対象の血清で増加し、血清カルシトニンにおける前記増加は、対象の肺における、増加したレベルのカルシトニンおよびIL−33と相互に関連があり、場合により、前記IL−33媒介疾患または障害の治療は、正常なレベルへと前記対象の血清におけるカルシトニンのレベルを低減させることを含む、請求項1に記載の医薬組成物。

- 前記IL−33アンタゴニストは、抗IL−33抗体である、請求項1〜3のいずれかの医薬組成物。
- 前記医薬組成物は、IL−33媒介疾患または障害の少なくとも1つの症状を低減するための、第2の治療薬と組み合わせて使用するための組成物であり、場合により、前記第2の治療薬は、非ステロイド系抗炎症薬(NSAID)、ステロイド、副腎皮質ステロイド(吸入または局所)、免疫抑制剤(例えば、シクロホスファミド)、抗コリン剤(例えば、チオトロピウム)、ムスカリン性薬物(例えば、グリコピロニウム)、ホスホジエステラーゼ阻害物質(例えば、テオフィリン、ロフルミラスト、シロミラスト)、βブロッカー、シクロスポリン、タクロリムス、ピメクロリムス、アザチオプリン、メトトレキサート、クロモグリク酸ナトリウム、プロテイナーゼ阻害物質、呼吸器作用薬、β2アゴニスト、抗ヒスタミン剤、エピネフリン、充血緩和剤、ロイコトリエン阻害物質、脂肪細胞阻害物質、胸腺間質性リンパ球新生因子(TSLP)アンタゴニスト、TNFアンタゴニスト、IgEアンタゴニスト、IL−1アンタゴニスト、IL−4またはIL−4Rアンタゴニスト、IL−13またはIL−13Rアンタゴニスト、IL−4/IL−13二重アンタゴニスト、IL−5アンタゴニスト、IL−6またはIL−6Rアンタゴニスト、IL−8のアンタゴニスト、IL−9アンタゴニスト、IL−12/23アンタゴニスト、IL−22アンタゴニスト、IL−17アンタゴニスト、IL−31アンタゴニスト、IL−33アンタゴニスト、経口PDE4阻害物質、およびIL−25に対する異なる抗体、からなる群から選択される、請求項1〜6のいずれかの医薬組成物。
- 前記気道または肺のIL−33媒介炎症性疾患または障害は、喘息、アレルギー、アレルギー性鼻炎、アレルギー性気道炎症、および慢性閉塞性肺疾患(COPD)からなる群から選択される、請求項1〜7のいずれかの医薬組成物。
- 前記IL−33媒介炎症性疾患または障害は、喘息である、請求項8に記載の医薬組成物。
- 前記喘息は、喘息増悪を含む、請求項9に記載の医薬組成物。
- 前記IL−33媒介炎症性疾患または障害は、COPDである、請求項8に記載の医薬組成物。
- 前記IL−33媒介炎症性疾患または障害は、アレルギー性気道炎症である、請求項8に記載の医薬組成物。
- 前記抗IL−33抗体は、配列番号276−278−280−284−286−288のアミノ酸配列を含むHCDR1−HCDR2−HCDR3−LCDR1−LCDR2−LCDR3ドメインをそれぞれ含む、請求項1〜12のいずれかの医薬組成物。
- 前記抗IL−33抗体は、配列番号274のアミノ酸配列を含むHCVR、および配列番号282のアミノ酸配列を含むLCVRを含む、請求項13に記載の医薬組成物。
- 前記抗IL−33抗体は、ヒトIgG4重鎖定常領域を含む、請求項14に記載の医薬組成物。
- 前記対象は、前記対象から得られたサンプルにおいてカルシトニンのレベルを測定することによって選択され、前記サンプルは、固形組織サンプル、細胞サンプル、または血液サンプルである、請求項1に記載の医薬組成物。
- (a)前記固形組織サンプルは、肺サンプルであり、(b)前記細胞サンプルは、痰細胞サンプル、気管支肺胞洗浄細胞サンプル、鼻ポリープ細胞サンプル、または肺生検細胞サンプルであり、(c)前記血液サンプルは、全血、血漿、または血清である、請求項16に記載の医薬組成物。
