本願は、2015年8月26日に出願された「Methods of detecting cancer recurrence」を発明の名称とするオーストラリア仮出願第2015903456号からの優先権を主張し、その内容の全体を参照により本明細書に組み入れる。
本発明は、概して、癌の再発の可能性を検出する方法に関する。より具体的に、本発明は、癌の再発の可能性と相関する核酸指標を同定する方法およびそのような指標を使用する方法に関する。

本発明は、一部、TSMに関連する1つまたは複数の特定の変異が対象において癌が再発する可能と相関するという予想外の発見に基づいている。本発明は、そのような変異を同定する方法およびそれらの変異を検出することによって対象において癌が再発する可能性を判定する方法を包含する。
活性化誘導型シチジンデアミナーゼ(AID)は、適応免疫において重要な酵素であり、B細胞において免疫グロブリン遺伝子の体細胞超変異(SHM)およびクラススイッチ組み換えに関与する。AIDは、シチジンからウラシル(CからU)に脱アミノさせることによってSHMを誘発し、免疫グロブリン可変領域遺伝子(VDJ)を多様化させ、新しい抗原結合部位を生成する。通常、Cの(例えば、CからTへの)変異は、非転写鎖(NTS)上で起こるシトシンの脱アミノの結果である。逆に、Gの(例えば、GからAへの)変異は、転写鎖(TS)上で起こるシトシンの脱アミノの結果である。
AIDに加えて、ヒトゲノムは、自然免疫およびRNA編集に関与することが公知のいくつかの相同なAPOBECシチジンデアミナーゼをコードしている(Smith et al. (2012), Semin. Cell. Dev. Biol. 23: 258-268)。ヒトにおいて、少なくともAPOBEC1、APOBEC3A、APOBEC3B、APOBEC3C、APOBEC3D、APOBEC3F、APOBEC3GおよびAPOBEC3Hが、自然免疫の提供および/または細胞性のmRNA編集に関与する。
RNA編集は、ゲノムDNAによってコードされているものとは異なるRNA配列を生成し、それによって遺伝子産物および機能を多様化させる転写後プロセシング機構である。高等真核生物において最もよく見られるRNA編集のタイプは、二本鎖RNA特異的アデノシンデアミナーゼまたはADAR酵素の作用を通じて二本鎖RNA(dsRNA)内のアデノシン残基をイノシンに変換する(AからIへの編集)。
本明細書で実証されているように、一部の変異誘発物質は、1つまたは複数の特定モチーフにおいてヌクレオチドの変異誘発を引き起こすだけでなく、そのモチーフおよび変異するヌクレオチドは、コドン文脈の中で認識される、すなわち、変異するヌクレオチドは、コドン構造内の特定位置にある、例えば、変異コドン内の(5’から3’に向かって)第1、第2または第3のヌクレオチドである。置換(replacement)または置換(substituting)ヌクレオチドに関しても明確な選択性が見られる。変異誘発物質によるこのモチーフ特異的およびコドン文脈特異的な標的指向の組み合わせは、本明細書でTSMと称される。非限定的な例として、表5に示されるように、核酸分子の非転写鎖のWRCGモチーフにおけるCの変異は、変異コドンの第1の位置(MC-1)において優先的に発生し得、それはTへの(すなわち、CからTへの)変異であり得る。したがって、核酸分子の標的指向された体細胞変異が起こるかどうかの可能性は、核酸分子の配列を分析し、1つまたは複数の特定モチーフ(例えば、WRCGSSモチーフ)における変異タイプ(例えば、CからT)の変異のコドン文脈を決定することによって判定され得る。そのモチーフにおけるその変異タイプの変異の位置にコドンのバイアスがない場合(すなわち、変異が本質的に、コドン内の各位置に均等に分布している場合)、その変異が偶発的に起こったものであり、変異誘発物質によるTSMの結果として起こったものではない可能性が非常に高い。しかし、核酸分子内のコドン内の1つの特定位置(例えば、MC-1、MC-2またはMC-3部位)においてその変異タイプの変異の比率または数が高い場合、これは、TSMが起こったことまたは起こった可能性があることを示している。
上記のおよび本明細書の他箇所に記載されるように、卵巣癌において、AIDは、モチーフWRCG/CGYWを特異的に標的指向することが見出され、下線が付されたヌクレオチドが変異する。本明細書で実証されているように、MC-2部位でGが標的化され、GからAへの変異が起こることに関して有意な選択性が見られる。したがって、核酸分子のMC-2部位でのCGYWモチーフにおけるGからAへの変異の予想以上に高い数または比率は、AIDがその核酸のTSMの原因である可能性が高いこと、ならびにAIDが、生物学的サンプルを取得した卵巣細胞および/または組織において活性を有することを示している。これもまた本明細書で実証されているように、ここでも非限定的な例として卵巣癌を用いると、MC-1部位でCが標的とされ、CからTへの変異が起こることに関して有意な選択性が見られる。したがって、核酸分子のMC-1部位でのWRCGモチーフにおけるCからTへの変異の予想以上に高い数または比率は、その核酸でTSMがおそらく起こったこと、ならびにAIDが、核酸分子を取得した細胞および/または組織において活性を有する可能性があることを示している。
本明細書に明確に示されているように、変異誘発物質は、特定のコドン文脈の中で、あるモチーフのあるヌクレオチドを標的化する。したがって、そのような物質による標的指向された体細胞変異は通常、コドン構造の中の1つの位置(例えば、MC-1であって、MC-2またはMC-3でない)かつ1つのモチーフ(例えば、CCG)において1つの変異タイプ(例えば、CからTであって、CからGまたはCからAでない)をもたらす。上記のように、そのモチーフにおけるおよび特定のコドン文脈の中での特定の変異タイプに関して核酸配列を分析することによって、そのモチーフにおいて単に変異が発生した以上のTSMが起こったかどうかのより正確な指標が試験された。
実施例に示されているように、個々のデアミナーゼアイソフォームについて特徴づけられるDBDは、特定の組織タイプに特異的である。したがって、1つの特定の組織タイプから得られた生物学的サンプルにおけるTSMのレベルの予測は、その組織タイプにのみ特異的である。したがって、好ましい態様において、TSMモチーフは、癌に冒されていることが分かっている組織由来の生物学的サンプルにおいて同定され、対応する健常組織(例えば、癌が存在しないことが分かっている、および癌を発症するリスクがない組織)と比較される。
核酸分子の配列を取得および評価するための当技術分野で公知の任意の方法が、本発明の方法において使用され得る。本発明の方法を用いて分析される核酸分子は、任意の核酸分子であり得るが、通常はDNA(cDNAを含む)である。典型的に、核酸分子は、哺乳動物核酸分子、例えばヒト核酸分子である。上記のように、核酸分子は、任意の生物学的サンプルから取得され得る。
本発明はまた、TSMのサブセットが癌の再発の可能性に関連することを開示する(本明細書で「癌進行関連指標」(C-PAS)と称する)。C-PASは、変異誘発物質(例えば、AID、APOBEC、ADAR等)によって特異的に標的指向される個々の核酸配列を検出することによって同定され得、これらは癌の再発の指標となる。
いくつかの態様において、本発明は、癌を有するまたは癌を有していた対象由来の生物学的サンプル中に存在する核酸におけるC-PASの同定を含む。C-PASであるとみなされるために、2つのルールが適用され得る。第1に、C-PASにおいて同定される変異誘発物質による単一ヌクレオチド置換は、有意な(すなわち、単一のタイプおよびコドン文脈の)標的指向選択性を示さなければならない。これにより、その核酸モチーフが単一のデアミナーゼの変異誘発活性に関連すること、および生じるTSMパターン(すなわち、C-PAS)が偶然によってのみ発生するものでないことが確実になる。C-PASとみなされる核酸モチーフの第2の基準は、再発する癌由来のサンプルと再発しない癌由来のサンプルの間で、C-PASにおいてサンプルあたりの平均標的指向変異数に増加がなければならないことである。
本明細書に記載される、対象においてTSMを検出して癌または腫瘍が再発し得る可能性をさらに同定するために必要とされる必須の材料および試薬ならびに関連方法はすべて、キットとしてひとまとめにされ得る。例えば、本発明の方法が最初に分析される核酸を単離および/または配列決定する工程を含む場合、その単離および/または配列決定を容易にするための試薬を含むキットが想定される。そのような試薬は、例えば、DNAの増幅のためのプライマー、ポリメラーゼ、dNTP(標識されたdNTPを含む)、陽性および陰性対照、ならびに緩衝液および溶液を含み得る。そのようなキットはまた、通常、適切な手段で、個々の試薬の各々のための別個の容器を含む。キットはまた、そのキットを使用するための様々なデバイスおよび/または印刷された説明書を特徴とする。
標的指向された体細胞変異誘発が起こったかどうかを検出するためならびにC-PASの同定および特徴づけのための本明細書に記載される方法は、多くの有用な診断および治療用途を有する。体細胞変異誘発は、多くの癌の存在に関連することが公知である。同様に、一部の変異誘発物質は、多くの癌の発生および進行に関連することが公知である。本明細書に記載される方法(例えば、1つまたは複数の変異誘発物質により引き起こされたTSMの存在および/または程度)を用いて、対象における癌の再発の可能性が、判定され得る。そのような判定は、癌を有しているまたは癌を有していた対象のための処置計画の策定を容易にし得る。例えば、判定は、現行の処置計画が継続されるべきか、縮小されるべきかまたは中止されるべきかを判定することを支援し得る。加えて、1つまたは複数の変異誘発物質に起因する標的指向された体細胞変異の継続評価は、癌が進行しているかもしくは退行しているかおよび/または処置計画の成功もしくは失敗を評価するために使用され得る。例えば、同一対象においてサンプル、例えば生検由来の核酸内で検出される標的指向された体細胞変異の数の経時的な増加は、癌の悪化または処置計画に対する無応答を示し得、変異の数の安定化または減少は、その状態の緩和または処置計画に対する望ましい応答を示し得る。
(i)腫瘍医学において使用される、抗増殖/抗新生物薬およびそれらの組み合わせ、例えばアルキル化剤(例えば、シスプラチン、カルボプラチン、シクロホスファミド、ナイトロジェンマスタード、メルファラン、クロラムブシル、ブスルファンおよびニトロソ尿素);代謝拮抗物質(例えば、抗葉酸剤、例えば、フルオロピリジン、例えば5-フルオロウラシルおよびテガフール、ラルチトレキセド、メトトレキサート、シトシンアラビノシドおよびヒドロキシ尿素;抗腫瘍抗生物質(例えば、アントラサイクリン、例えばアドリアマイシン、ブレオマイシン、ドキソルビシン、ダウノマイシン、エピルビシン、イダルビシン、マイトマイシン-C、ダクチノマイシンおよびミトラマイシン);抗有糸分裂剤(例えば、ビンカ・アルカロイド、例えばビンクリスチン、ビンブラスチン、ビンデシンおよびビノレルビン、ならびにタキソイド、例えばパクリタキセルおよびドセタキセル;ならびにトポイソメラーゼ阻害剤(例えば、エピポドフィロトキシン、例えばエトポシドおよびテニポシド、アムサクリン、トポテカンおよびカンプトテシン);
(ii)細胞増殖抑制剤、例えば、抗エストロゲン(例えば、タモキシフェン、トレミフェン、ラロキシフェン、ドロロキシフェンおよびヨードキシフェン)、エストロゲン受容体下方調節剤(例えば、フルベストラント)、抗アンドロゲン(例えば、ビカルタミド、フルタミド、ニルタミドおよび酢酸シプロテロン)、UHアンタゴニストまたはLHRHアゴニスト(例えば、ゴセレリン、ロイプロレリンおよびブセレリン)、プロゲストゲン(例えば、酢酸メゲストロール)、アロマターゼ阻害剤(例えば、アナストロゾール、レトロゾール、ボラゾールおよびエキセメスタン)ならびに5α-レダクターゼの阻害剤、例えばフィナステリド;
(iii)癌細胞浸潤を阻害する薬剤(例えば、メタロプロテイナーゼ阻害剤、例えばマリマスタットおよびウロキナーゼプラスミノーゲン活性化因子受容体機能の阻害剤);
(iv)成長因子機能の阻害剤、例えば、そのような阻害剤は、成長因子抗体、成長因子受容体抗体(例えば、抗erbb2抗体トラツズマブ[ハーセプチン(商標)]および抗erbb1抗体セツキシマブ[C225])、ファルネシルトランスフェラーゼ阻害剤、MEK阻害剤、チロシンキナーゼ阻害剤およびセリン/スレオニンキナーゼ阻害剤、例えば上皮成長因子ファミリーの他の阻害剤(例えば、他のEGFRファミリーチロシンキナーゼ阻害剤、例えば、N-(3-クロロ-4-フルオロフェニル)-7-メトキシ-6-(3-モルホリノプロポキシ)キナゾリン-4-アミン(ゲフィチニブ、AZD1839)、N-(3-エチニルフェニル)-6,7-ビス(2-メトキシエトキシ)キナゾリン-4-アミン(エルロチニブ、OSI-774)および6-アクリルアミド-N-(3-クロロ-4-フルオロフェニル)-7-(3-モルホリノプロポキシ)キナゾリ-n-4-アミン(CI 1033))、例えば血小板由来成長因子ファミリーの阻害剤および例えば肝細胞成長因子ファミリーの阻害剤;
(v)抗血管新生剤、例えば、血管内皮成長因子の効果を阻害するもの(例えば、抗血管内皮細胞成長因子抗体ベバシズマブ[アバスチン(商標)]、国際特許出願WO 97/22596、WO 97/30035、WO 97/32856およびWO 98/13354に開示されるような化合物)ならびに他のメカニズムによって作用する化合物(例えば、リノマイド、インテグリンαvβ3機能の阻害剤およびアンギオスタチン);
(vi)血管損傷剤、例えば、コンブレスタチンA4ならびに国際特許出願WO 99/02166、WO00/40529、WO 00/41669、WO01/92224、WO02/04434およびWO02/08213に開示される化合物;
(vii)アンチセンス療法、例えば、上で列挙されている標的に指向するもの、例えば、ISIS 2503、抗rasアンチセンス;ならびに
(viii)例えば、異常な遺伝子、例えば異常なp53または異常なGDEPT(遺伝子指向性酵素プロドラッグ療法)アプローチに取って代わるアプローチ、例えばシトシンデアミナーゼ、チミジンキナーゼまたは細菌ニトロレダクターゼ酵素を用いるもの、ならびに化学療法または放射線療法に対する患者の耐性を高めるアプローチ、例えば、多剤耐性遺伝子療法を含む、遺伝子療法アプローチ。
変異のストランドバイアスパターンは他の癌と共通する
以前の研究は、再構成された免疫グロブリン可変領域遺伝子座において観察される体細胞超変異(SHM)参照パターンが、すべてのワトソン・クリック相補体で見られる有意なストランドバイアスによって特徴づけられることを示した(例えば、Steele and Lindley, 2010を参照のこと)。本研究は、この顕著なSHM様のストランドバイアスのかかったパターンが肺腺癌、肺小細胞癌、乳管癌、扁平上皮癌および悪性黒色腫において起こる体細胞変異においても観察されることの最初の証拠を提供する。そのために、TCGA漿液性卵巣腺癌変異データセットにおけるストランドバイアスパターンもまたこのストランドバイアスを示すかどうかを判定した。その結果は、多くの癌において、異常なIg SHMと体細胞点変異の発生の間に因果関係が存在することを強く示唆している。これはすなわち、SHMに関与する潜在的標的、例えばAIDおよびADAR1、ならびに転写共役修復(TCR)経路を調節するプロセスが関与することを示唆している。
データソース
体細胞変異データのソースは、The Cancer Genome Atlas(TCGA 2013)に保管されている429個の臨床的に注釈を付けられたステージII〜IVの漿液性卵巣腺癌サンプルのコホートである。Kanchi et al(2014)によるこれらのサンプルの以前の分析は、一致する生殖系列および腫瘍組織サンプルのエクソームワイド配列決定を用いる最初の大規模研究であった。
DNA変異データを、Educational Research In Codon-context(ERIC)バージョン1.6を用いて分析した。このアプリケーションは、研究支援ツールとして開発された。それは、分析用のDNA変異データの分析およびコンパイルのプロセスを自動化するEXCEL VBAスクリプトで記述されている。このプログラムのルーチンは、各変異の周囲のヌクレオチド配列文脈を同定するために、および変異コドン(MC)のヌクレオチド構造内の各変異の位置を決定するために、ENSEMBL遺伝子転写物(Cunningham et al., 2015)を使用する。
最初の工程として、蓄積された変異データセットにおいてストランドバイアス変異分析を行った。次いで、そのストランドバイアスパターンを、他の組織型における様々な癌のストランドバイアスパターンと比較した。蓄積されたデータのストランド変異バイアスパターンを、(Steele and Lindley, 2010;およびLindley and Steele, 2013に記載されるように)各変異タイプの相対比率を4x4表形式で表にすることによって作成した。
TSMプロフィールはAID/APOBEC3G/APOBEC3BおよびADAR1デアミナーゼ活性を示す
AID(WRCG/CGYW、ここでW=A/T、Y=T/CおよびR=A/G)、APOBEC3G(CCG/CGG)、APOBEC3B(TCG/CGA)およびADAR1(WAY、ここでW=A/TおよびY=T/C)について選択されたモチーフのTSMプロフィールが、表5に示されている。3x3データセットの各モチーフの変異について、予想からの偏差の統計学的有意性のカイ二乗レベルが示されている。すべての3x3データセットは、有意性レベルp<0.001(8df)で、特定タイプの、および好ましい標的部位での変異に対する強い選択性を示している。各デアミナーゼについてDBDのアイソフォームが存在することが知られているが、選択されたデアミナーゼ間で重複がないことを確実にするために、各デアミナーゼの支配的なDBDに対するモチーフが選択されたことに留意されることが重要である。
DBDの異なるアイソフォームは標的部位が相違する
ADAR DBDの異なるアイソフォームが標的指向選択性を変化させ得ることを検証するため、WAまたはAWの塩基組成を含む様々なADARモチーフの標的指向選択性を特定し比較した。我々はまた、3ヌクレオチド塩基モチーフWRC/GYWを用いて、AIDデアミナーゼドメインの可能性のあるアイソフォームにおけるMCヌクレオチド構造内の好ましい標的ヌクレオチド位置の場所の変化を同定した。その結果が表6に示されている。予想からの偏差の統計学的有意性のカイ二乗レベルが、3x3データセットの各モチーフの変異について、有意性レベルp<0.001(8 df)で示されている。
C-PASの同定および再発/予後の予測におけるそれらの使用
このTSM法を用いて、我々は、漿液性卵巣腺癌サンプルにおいて疾患進行指標に関連する様々なDBDを同定した。これらのDBDは、上記の鍵となる選択基準を満たし(実施例1を参照のこと)、これらは以降、本明細書で癌進行関連指標(C-PAS)と呼ぶ。本明細書で同定されたC-PASは、それらのそれぞれのモチーフのヌクレオチド配列によって定義され、したがってデノボ変異の導入を担う特定のデアミナーゼの結合部位を特定する。臨床情報が利用可能な疾患進行指標は、病期および無疾患生存時間を含む。表7は、これらの癌進行指標に関連することが見出されているAID、APOBEC3G、APOBEC3BおよびADARデアミナーゼ活性に関連するデアミナーゼ標的モチーフを示している。これらのデアミナーゼの各々は、腫瘍形成において役割を果たすことが知られている点およびすべての(またはほぼすべての)体細胞組織タイプにおいて見出される点で普遍的である。
急性骨髄性白血病(AML)におけるC-PASの同定
本発明の適用のコンセプトの例のさらなる証明を提供するため、異なる組織タイプを冒している癌サンプル(すなわち、急性骨髄性白血病(AML))由来のサンプルを、臨床的に関連するC-PASについて評価した。表8は、臨床記録および一致する配列データが公衆に提供されているサンプルの数の要約を提供する。
Claims (4)
- 対象において癌が再発する可能性を判定するための方法であって、該方法が、
対象から得られた生物学的サンプル中の核酸分子の配列を分析し、変異誘発物質によって認識または標的指向される1つまたは複数のモチーフにおけるある変異タイプの複数の変異について該複数の変異のコドン文脈を決定し、それによって、該核酸分子内の複数の変異コドンの各々について変異の位置および変異タイプを同定する工程であって、対応する変異コドンの3つの位置のいずれにおいて個々の変異が生じているかを決定することによって、個々の変異のコドン文脈が決定される、工程、および
複数の変異コドンの3つの位置のうちの1つにおけるある変異タイプの変異のレベルが、癌の再発と相関する既定のしきい値を上回る場合に、対象において癌が再発する可能性があると判定する工程
を含み、
(a)癌が卵巣癌であり、かつ
癌が再発する可能性があるという判定が、該核酸分子の変異コドンの第1の位置(MC-1部位)における
モチーフにおけるCからTへの変異のレベルが既定のしきい値を上回る場合になされる;
癌が再発する可能性があるという判定が、該核酸分子のMC-1部位における
モチーフにおけるCからTへの変異のレベルが既定のしきい値を上回る場合になされる;
癌が再発する可能性があるという判定が、該核酸分子の変異コドンの第3の位置(MC-3部位)における
