Description
本発明は、国立衛生研究所(National Institutes of Health)から付与された認可番号R01 HL069929、R01−AI080455、R01−AI100288、R01−AI101406、P01−CA023766、及びP01−CA023766、ならびに国立アレルギー感染病研究所(U.S.National Institute of Allergy and Infectious Disease)から付与された契約HHSN272200900059Cの下、米国政府支援によりなされた。政府は本発明における特定の権利を有する。
特定の実施形態では、例えば以下の項目が提供される。

クロストリジウム目の細菌の1つ以上の精製された集団を含む、骨髄移植(BMT)または造血幹細胞移植(HSCT)後の移植片対宿主病(GVHD)の予防及び/または治療のための治療用組成物。

前記細菌が、配列番号1、3、4、5、7、8、9、12、及び15のうちの1つのヌクレオチド配列または前記配列と約98%〜100%同一のヌクレオチド配列を有する16Sr DNAを含む、項目1に記載の治療用組成物。
(項目3)
前記細菌が、ブラウチア(Blautia)属、ルミノコッカス属、ユーバクテリウム属、ホールディマニア属、及びクロストリジウム属、またはブラウチア様種より選択される、項目1に記載の治療用組成物。
(項目4)
前記細菌が、ルミノコッカスオベウム、クロストリジウムハセワイ、ユーバクテリウムデスモランス、ドレアロンギカテナ、ルミノコッカスラクタリス(ブラウチアプロダクタ(Blautia producta))、ユーバクテリウムコントルム(Eubacterium contorum)、ルミノコッカスファエシス、ホールディマニアフィリフォルミス、クロストリジウムソルデリイ、及びそれらの組み合わせまたは混合物からなる群より選択される、項目3に記載の治療用組成物。

経口投与用に製剤化された項目1〜9のいずれかに記載の治療用組成物。
(項目11)
結腸/直腸投与用に製剤化された項目1〜9のいずれかに記載の治療用組成物。
(項目12)
骨髄または造血幹細胞移植を受ける対象における、移植片対宿主病(GVHD)を発症するリスクを減少させる及び/またはGVHDを治療する方法であって、クロストリジウム目からの1つ以上の細菌を含む治療有効量の治療用組成物を前記対象に投与することを含む、方法。

骨髄移植(BMT)または造血幹細胞移植(HSCT)後の対象において移植片対宿主病(GVHD)を発症するリスクを減少させるための方法であって、
(a)前記対象からの糞便物質の試料中のアッカーマンシアムシニフィラの存在量を決定することと、
(b)アッカーマンシアムシニフィラの存在量が1%〜10%を上回るときに、アンピシリン及び経口バンコマイシンより選択される治療有効量の抗生物質を前記対象に投与することと
を含み、前記抗生物質の投与が、アッカーマンシアムシニフィラの存在量を減少させ、GVHDの前記リスクが減少されるか、または排除される、方法。

前記試料中のアッカーマンシアムシニフィラの存在量が2%を上回る、項目25に記載の方法。

(項目40)
骨髄移植(BMT)または造血幹細胞移植(HSCT)を受ける対象において移植片対宿主病(GVHD)のリスク、発生率、または重症度を減少させるための方法であって、静脈内バンコマイシン、セフトリアキソン、セフタジジム、セフェピム、アズトレオナム、トリメトプリム−スルファメトキサゾール、シプロフロキサシン、レボフロキサシン、及びアトバコンからなる群より選択される、嫌気性細菌に対する活性が低下した抗生物質を前記対象に投与することを含む、方法。
本明細書に記載される方法での使用のための種は、ブラウチアプロダクタ(ATCCR27340−DSM2950、American Type Culture Collection、Manassas、VA)、または下の表1でアスタリスクで示されるものを含んでもよい。新しいブラウチア分離株が毎年同定されており、いくつかの例では、他の属の分離株がブラウチアとして再分類されている。つまり、例えば、ブラウチア様またはブラウチア関連種は、ルミノコッカスオベウム、ルミノコッカスファエシス、ルミノコッカスラクタリスなどを含んでもよい(下の表1を参照されたい)。
1つ以上の異なる細菌接種剤が、同時にまたは連続して投与され得る(異なる時間に投与することを含む)。本発明の方法に従って投与される細菌株は、生細菌、凍結細菌、細菌胞子、またはそれらの組み合わせを含み得る。そのような細菌は、適切な微生物叢源から単離するか、または細胞バンク(American Type Culture Collection/ATCCなど)から得て、既知の技術を使用して培地で増殖させることができる。
1つの研究では、臨床転帰に対する抗生物質の影響を後方視的に分析される対象は、1994年〜2013年にMemorial Sloan Kettering Cancer Center(MSKCC)でall−HSCTを受けた283人の成人患者で構成された。従来の移植片を受けた(非T細胞除去)患者はこの研究に含まれ、エキソビボT細胞除去移植片またはペリ移植アレツムバブを受けた患者は除外された。移植入院の間毎週、大便検体を採取して保管した。本研究は、MSKCCの治験審査委員会によって承認された。すべての研究患者は、生体試料採取及び分析ついて書面によるインフォームドコンセントを提供した。
患者からの大便試料は、−80℃での凍結前に4℃で24時間未満保管した。マウスからの回腸及び大腸試料は、−80℃で凍結した。DNAは、同様の結果を生ずる2つの方法のうちの1つを使用して抽出した。
各大便検体について、DNAを抽出し精製した。試料は、細菌16Sr RNA遺伝子のV1〜V3領域を配列決定するために、454 GS FLXチタンプラットフォームを使用して分析されたか、または16Sr RNA遺伝子のV4〜V5領域を配列決定するためにIllumina MiSeqプラットフォームを使用して代替的に分析された。配列データは、mothurバージョン1.34及びQIIMEバージョン1.8.0を使用してコンパイル及び処理し、品質についてスクリーニング及びフィルタ処理した。操作的分類単位(OTU)は、Greengenes参照データベースの変形形態を使用して種レベルに分類した。主成分分析は、RソフトウェアにおいてOUT存在量の重み付け及び正規化されたUnifrac距離行列を基に実施した。この研究からのデータは、NCBI Sequence Read Archiveに保管されている(url:ncbi.nlm.nih.bov/sra)。
すべての抗体はBD Biosciences−Pharmingenから得た。表面マーカーの細胞分析のため、細胞を、Fcブロック後、20分間4℃で0.5%BSA(PBS/BSA)を有するPBS中で染色し、洗浄し、PBS/BSA中のDAPI中に再懸濁した。細胞表面染色の後に、製造業者の指示を通じて、eBioscienceキットでの細胞内染色を行った。死細胞をLIVE/DEAD Fixable Dead Cell Stainキット(Invitrogen)で除外した。すべてのフローサイトメトリーは、LSR II(BD Biosciences)上で実施し、FlowJo(TreeStar Software)で分析した。
移植入院中に使用される抗生物質を、嫌気性細菌に対する著しい活性を含むもの(pip/tazo、チカルシリンクラブラン酸、イミペネム、メロペネム、メトロニダゾール、経口バンコマイシン、及びクリンダマイシン)、及び減少した活性を有するもの(静脈内バンコマイシン、セフトリアキソン、セフタジジム、セフェピム、アズトレオナム、トリメトプリム−スルファメトキサゾール、シプロフロキサシン、レボフロキサシン、アトバコンに分けた(19)。
本発明者らの制度的慣習の通り、シプロフロキサシン予防療法を受けた患者、及び骨髄非破壊的レジメンよりも激しい前処置を受ける患者は、2日目に開始して7日目まで静脈内バンコマイシン予防療法も受けた(59)。ニューモシスチスイロベジイに対する抗生物質予防療法(トリメトプリムスルファメトキサゾール、吸入ペンタマジン(pentamadine)、またはアトバコン)を、移植医の指示で与えた。

マウスを、合計3回の抗生物質治療を受けてから16日後に屠殺した。遠位結腸を除去して、プールした(n=4、アズトレオナム及びイミペネム治療群の両方で)後、TrizolLSを使用してRNA単離を行った。RiboMinus(LifeTechnologies)を使用してRNAを調製した。Ion Proton System(LifeTechnologies)を使用してライブラリを配列決定した。整列したRNAを倍数変化について分析した。遺伝子発現差をイミペネム治療マウス対アズトレオナム治療マウスで分析した。
ショットガン配列決定からのペアエンド生リードを、最大2箇所の不一致、最小30の末端塩基スコア、及びIllumina TruSeqアダプター配列を使用したTrimmomatic 0.32(69)を使用してトリムした。残りの切り取られたリードをKraken(70)を使用して分類学的に割り当てた。簡単に言うと、トリムされフィルタ処理されたリードを、NCBIゲノム及び染色体コレクション(2014年11月12日評価)から生成される細菌、ウイルス、及び真菌k−merプロファイルとのk−mer類似度により分類学的に分類した。未分類リードをBLAST(ntデータベース、2015年3月24日)でさらに照合し、非細菌リードを廃棄した。所与のメタゲノム群集内の遺伝子の存在量及び経路を決定する、HUMAnN v0.99(71)を使用した、品質フィルタ処理されたリードに対する機能分析を実行した。アズトレオナム治療した対象試料とイミペネム治療した対象試料との間で異なって表されたそれらの機能分類を同定するために、本発明者らは、微生物群衆間で遺伝子、経路、または、有機体などの豊富なバイオマーカーを特異的に同定する有効なツールであるLEfSe(21)を用いた。
レシピエントからのホルマリン固定した結腸を、イソタイプ対照と対比して、抗マウスCD3抗体A0452(DAKO)、pSTAT3抗体9135(Cell Signaling)、CD11b抗体ab133357(Abcam)、B220抗体550286(BD Pharmingen)で染色した。免疫蛍光二次染色を、pSTAT3及びB220の場合はAF488、ならびにCD3及びCD11bの場合はAF594で実施した。糞便物質を有する結腸の片をカルノア中に固定し、細菌FISH(EUB338)(35)及び免疫染色を、先に記載されるようなHoechst 34580(Life technologies)によりMUC2C3抗血清及びDNAを用いて行った(74、75)。
メスのC57BL/6、C57BL/6/Ly5.1、及び129S1/SvlmJマウスをJackson Laboratory(Bar Harbor、Maine、USA)より得た。実験に使用したマウスは生後6〜9週間であった。マウスを腸除染抗生物質混合液(アンピシリン及びバンコマイシン)で処置して、allo BMT患者で発生する微生物叢を模倣した。次いでマウスを骨髄破壊的な1回量の全身照射に曝露し(TBI、投与間が3時間間隔の分割線量として137Cs源から11Gy)、次いで静脈内注射により完全にMHC不適合のB10.BRマウスからの骨髄及び精製した脾臓T細胞を移植した(H2kをH2bへ)。二酸化炭素を使用した窒息によりドナーマウスを安楽死させ、脾臓、大腿骨、及び脛骨を無菌で摘出した。冷たい組織培地を用いて脛骨及び大腿骨を洗い流すことによりドナーBMを得た。T細胞除去されたBM及び実験マウス中のドナー脾臓T細胞の同時注射によりGVHDを再現可能に誘発することができるように、ドナーBMを、106BM細胞あたり2.5μgの抗Thy−1.2と共に30分間4℃でインキュベートし、続いて106BM細胞あたり10μLの低TOX−Mウサギ補体と共に40分間37℃でインキュベートすることによりT細胞除去(TCD)した。脾臓T細胞を抗CD5 MACSビーズ(Miltenyi)で精製した。BM細胞(レシピエントあたり5×106)及び脾臓T細胞(レシピエントあたり1×106)を尾静脈注射により移植した。
大便検体すべてを集め、1〜3体積のペプトンと共に無菌ステンレス鋼ブレンダーを使用して1〜3体積の0.05%ペプトン中で均質化する。およそ1グラムの検体を前還元の嫌気的滅菌(PRAS)希釈ブランク(Anaerobe Systems)中で系列希釈する(10回)。別個の約1グラムのアリコートを秤量し、真空中で一晩乾燥させ、乾燥重量基準での量を計算するために再び秤量する。ブラウチア種を含むクロストリジウム目細菌を選択するには、100μLの均質化された大便試料希釈系列を、4μg/mLトリメトプリム(Sigma Chemical)及び1μg/mLスルファメトキサゾール(Sigma)で補足されたブレインハートインフュージョン血液寒天培地(SBA、Becton Dickinson)、ブルセラ血液寒天培地(BAP、Anaeobe Systems)、CDC ANA血液寒天培地、(BBL Microbiology Systems)、ならびに卵黄寒天培地(EYA、Anaerobe Systems)上にプレーティングする(Finegold SM,Molitoris D,Song Y,Liu C,Vaisanen ML,Bolte E,McTeague M,Sandler R,Wexler H,Marlowe EM,Collins MD,Lawson PA,Summanen P,Baysallar M,Tomzynski TJ,Read E,Johnson E,Rolfe R,Nasir P,Shah H,Haake DA,Manning P,Kaul A,2002.Gastrointestinal microflora studies in late−onset autism.Clin Infect Dis 1:35)。胞子菌を選択するには、希釈物を70〜80℃で10〜20分間加熱し、非加熱均質化大便試料と同じ様式でプレーティングしてもよい。嫌気性チャンバ内で37℃での5日間の増殖後、単コロニーを選択する。選択単コロニーをリストリークし、上記のように増殖させ、単コロニーを再び選択することによりコロニー精製プロセスを繰り返す。単コロニーを、クライオチューブ内15%〜25%グリセロール中で凍結させ、−80℃で保管する。
The Jackson Laboratory(Bar Harbor、Maine)より購入したC57BL/6マウスを経口バンコマイシン及びアンピシリンで処置した。除染に続いて、マウスをオートクレーブ条件(ケージング、床敷、水、及び食料)に安置して、マウスのフローラ内に存在するほぼすべての内在性クロストリジウムを排除した。次いで、マウスを、対照として培養されたエンテロコッカスフェカーリスの液体懸濁液またはブラウチア分離株のいずれかでの経管栄養で処置した。次いでマウスを骨髄破壊的な1回量の全身照射(TBI、11Gy)に曝露し、次いで静脈内注射により完全にMHC不適合のB10.BRマウスからの骨髄及び精製したT細胞を移植した(H2kをH2bへ)。腸の病理及び全生存に対する影響を記載のように評価した。ブラウチアによってコロニー化されたマウスは、エンテロコッカスを持っているものと比較して、GVHDから保護され、改善された生存率を有した(図9)。
マウスを、生存については毎日、及びGVHD臨床スコアについては毎週監視した(Cooke,K.R.,L.Kobzik,T.R.Martin,J.Brewer,J.Delmonte Jr.,J.M.Crawford,and J.L.Ferrara.1996.An experimental model of idiopathic pneumonia syndrome after bone marrow transplantation:I.The roles of minor H antigens and endotoxin.Blood.88:3230−3239を参照されたい)。小腸、大腸、及び肝臓試料を、GVHDの証拠について組織学的に評価し、先に記載されるようにスコア付けした(Hill,G.R.,J.M.Crawford,K.R.Cooke,Y.S.Brinson,L.Pan,and J.L.Ferrara.1997.Total body irradiation and acute graft−versus−host disease:the role of gastrointestinal damage and inflammatory cytokines.Blood.90:3204−3213を参照されたい)。
成体マウスの小腸内腔を氷冷水で洗い流し、分割する。陰窩は、まず切片を裏返しにして次にそれらを30mM EDTAを含みかつCa2+及びMg2+を欠いたPBS中で振ることにより溶出する。溶出された絨毛及び陰窩を、700xgでペレット化し、PBS中に再懸濁し、キャピラリーピペットを使用してシリコン処理したミクロチューブに移した。分泌刺激への曝露に備えて、陰窩をiPIPES緩衝液(10mM PIPES(pH7.4)及び137mM NaCl)中に再懸濁する。
DyNAmo SYBR Green qPCRキット(Finnzymes)ならびに0.2μMのユニバーサル細菌プライマー8F(5’−AGAGTTTGATCCTGGCTCAG−3’SEQ ID NO:1)及び広範な細菌プライマー338R(5’−TGCTGCCTCCCGTAGGAGT−3’SEQ ID NO:2)を使用して、組織試料において細菌16Sr RNA遺伝子の定量的PCR(qPCR)を実施した。16Sr RNA遺伝子の1コピーを含むPCR bluntベクター(Invitrogen)の系列希釈により標準曲線を作成した。
短鎖脂肪酸(SCFA)は、多くの細菌によって炭水化物発酵の副産物として産生される。SCFAは免疫システムの重要な調節因子であることが分かっている。それらは、ブラウチア及びクロストリジウム綱からの関連細菌によって豊富に産生される。どのようにブラウチア及び関連細菌がGVHDの抑制を緩和するかを評価するために、糞便ペレットを採取して、SCFAレベル、特にアセテート、プロピオナート、またはブチレートを定量化した。大便試料をアルカリ化し、Bruker Avance−600 MHz分光計で1D 1H NMRを使用して試料の代謝組成物のフィンガープリントを得、Chenomx NMR Suiteソフトウェアを使用して監視下の多変量統計法で分析することによって、SCFA、クレアチン、及びヒドロキシ−SCFAを定量化した。
マウスGVHDに対するSCFAの投与の影響を試験するために予備実験を行った。飲料水を介して与えられるプロピオナート(データは示されない)、または飲料水もしくは浣腸を介して与えられるブチレート(データは示されない)の著しい有効性は観察されなかったが、飲料水を介したアセテートの投与では目立った有効性が観察された。酢酸ナトリウム(150mM)は、BMTの2週間前からマウスの飲料水を介して送達される。次いで、マウスは、放射線で治療され、酢酸ナトリウムの継続した補給と共に移植される。マウスにおいて評価される予後は、GVHD臨床スコア、生存率、ならびに14日目及び28日目の組織病理を含む。カプラン−マイヤー曲線は、2つの群の全生存を示す。加えて、曲線下面積(AUC)は、各マウスの注入時から13週目までの週ごとの総GVHDスコアをまとめたものである。マウスを安楽死させて、GVHDの病理上の証拠について評価し、ならびに14日目及び28日目にフローサイトメトリーによって大腸Treg及びアロ反応性エフェクターT細胞を定量化及び特徴付けする。
以前に記載されるような腸上皮陰窩培養系(Toshiro Sato,Robert G.Vries,Hugo J.Snippert,Marc van de Wetering,Nick Barker,Daniel E.Stange,Johan H.van Es,Arie Abo,Pekka Kujala,Peter J.Peters & Hans Clevers,2009)を使用した。Single Lgr5 stem cells build crypt−villus structures in vitro without a mesenchymal niche,Nature,459:262−265)。ウェルあたり400の陰窩を液化した増殖因子減少マトリゲル(Corning)(25% Advanced DMEM/F12培地(Gibco)、75%増殖因子減少マトリゲル)中に4℃で懸濁させた。次いで、それらを小腸には50μL滴、大腸には30ul滴で(それぞれおよそ100〜500の陰窩を含む)、余熱したデルタ表面Nunc24ウェルプレート内にプレーティングした。マトリゲル滴が重合した後、500ulの完全な陰窩培地を、小腸陰窩培地(ENR−培地:advanced DMEM/F12(Sigma)、2mM L−グルタミン(Sigma)、10mM HEPES(Sigma)、100U/mlペニシリン/100μg/mlストレプトマイシン(Sigma)、1mM N−アセチルステイン(Sigma)、1×B27補足物(Invitrogen、)、1×N2補足物(Invitrogen)、50ng/ml mEGF(Peprotech)、100ng/ml mNoggin(Peprotech)、及びhR−spondin−1をトランスフェクトされたHEK 293T細胞の10%ヒトR−spondin−1条件培地に添加した。簇出(budding)を評価するいくつかの実験においては、hR−spondin−1を1.25〜5%に下げた。大腸陰窩を、先述のタンパク質及び1%ウシ血清アルブミン(Sigma)に加えて、50%のWnt3a条件培地を含むWENR培地で培養した。大腸培地の場合、10uM SB202190(Sigma、Cat.nr.S7067)及びALK5阻害剤(A83−01、Tocris)をWENRに添加した。すべてのプレートを37℃/5%CO2でインキュベートし、培地は2〜3日ごとに置き換えた。対照ウェルは未処置のままにし、適切な場合、処置ウェルには培地変更と共に異なる濃度の細菌代謝産物を入れた。陰窩は、それらを血清ピペットで機械的に破壊し、過剰培地中の陰窩を遠心沈殿することによりマトリゲルを洗い流し、それらを液化したマトリゲル中のペレットの再構成後に再プレーティングすることによって、7日目に継代培養した。
C57BL/6由来のブラウチア分離株、ならびにC57BL/6由来のラクトバチルスジョンソニイを利用して、これらの細菌が様々な糖を発酵させる能力を、グルコースを欠いた培地での細胞増殖を評価するためにpH及び光学濃度を使用して評価した。ラクトバチルスジョンソニイを評価したのは、この細菌がカロリー制限の状況でクロストリジウムを犠牲にして拡大し、故に、恐らくはマウス腸内の栄養物の直接のコンペティターであるためである。ラクトバチルスではなくブラウチアによって発酵される2つの糖、ラムノース及びキシロースをこの分析から同定した。
C57BL/6マウスに、キシロースを発酵させることで知られるマウスブラウチア分離株を経口接種した。次いで、一部のマウスに、B10.BR BM及びT細胞を用いたBMTの7日前から飲料水(10g/L)中のキシロースを投与した。キシロースを受けるこれらのマウスは、BMTの14日後にGVHDの存在にもかかわらず腸内フローラにおいてブラウチアの増加を呈した(16Sディープシーケンシングによって測定された)(図9)。興味深いことに、キシロースの長期的投与もGVHDを有するマウスの生存率の改善をもたらした。
Bruker Avance−600 MHz分光計で1D 1H NMRを使用して試料の代謝組成物のフィンガープリントを得、Chenomx NMR Suiteソフトウェアを使用して監視下の多変量統計法で分析することによって、SCFA、クレアチン、及びヒドロキシ−SCFAを含む細菌代謝産物を定量化する。
腸内微生物叢組成に対するカロリー制限の影響を研究することにより、偏性嫌気性菌(バクテロイデス門、クロストリジウム菌)の存在量が減少する一方、通性嫌気性菌(プロテオバクテリア門、ラクトバシラス目)が拡大することが観察された。飢餓の状況における大腸菌によるフリーラジカルの産生が以前に記載されている(Saby S,et al.Appl Environ Microbiol.1999;5600−5603.)。実験は、ラクトバチルスジョンソニイが飢餓条件下で本発明者らのマウスブラウチア分離株の増殖を抑制できるどうかをインビトロで検査するように、及びフリーラジカルの産生が寄与因子であり得るかどうかをさらに調査するように設計された。具体的には、ブラウチアを、単独またはL.ジョンソニイと一緒のいずれかで培養したところ、L.ジョンソニイは実際にブラウチアの増殖を抑制するが、追加培地が飢餓を防ぐために追加された場合にはそれを行うことができないということが観察された(図11)。L.ジョンソニイの定常期までの増殖をサポートした培地の影響を除菌後にさらに調査した。ラクト条件培地は、1:1比で新規培地と混合されたときにはラクトバチルスの増殖に影響を与えなかったが、ブラウチアの増殖を抑制することができ、飢餓条件下で、ラクトバチルスは、ラクトバチルス自体ではなく、ブラウチアの増殖を抑制する物質を放出することを示した。次いで、還元剤がラクト条件培地媒介(Lacto−conditioned−media−mediated)抑制からブラウチアを救出することができるかどうか検査した。L−システイン、アスコルビン酸、及びチオグリコール酸ナトリウムはすべて、ラクト条件培地の存在下でブラウチア増殖をサポートすることができることが分かった。まとめると、これらの結果は、ラクトバチルスが、グルタチオントランスフェラーゼ、カタラーゼ、及びスーパーオキシドジスムターゼを含む防御酵素の欠如に起因して偏性嫌気性細菌に選択的に害を与えているフリーラジカルの産生に起因する限られた栄養の状況において、ブラウチアに対する競争優位性を築き得ることを示唆する。
ブラウチアを、液体PY培地単独またはグルコースまたはキシロース(10g/L)で補足されたもので48時間培養した。培地を遠心分離し、上清はSCFAのレベルについて評価した。
移植入院中に使用した抗生物質を、嫌気性細菌に対して著しい活性を含むもの(ピペラシリン−タゾバクタム、チカルシリン−クラブラン酸、イミペネム−シラスタチン、メロペネム、メトロニダゾール、経口バンコマイシン、及びクリンダマイシン)と、減少した活性を有するもの(静脈内バンコマイシン、セフトリアキソン、セフタジジム、セフェピム、アズトレオナム、トリメトプリム−スルファメトキサゾール)19とに分けた。

急性GVHD及びGVHD関連死亡の発生率は、再発及びGVHDに関連のない死を競合イベントとして処理して、累積発生関数を用いて推定し、グレイ検定(Gray’s test)を使用して因子間で比較した。全生存可能性は、カプラン−マイヤー方法論を使用して推定し、ログランク検定を使用して比較した。細菌存在量の比較は、対応のない検定のためのマン・ホイットニーUを使用して実施した。マウス実験では、データは、平均±SEMとして提示した。生存曲線は、Mantel−Cox log−rank検定で分析した。他の比較には、ノンパラメトリックなマン・ホイットニーU検定を使用した。すべての分析統計有意性は、両側検定に基づいてP<0.05と定義した。統計分析は、Rバージョン3.1.0(The R Foundation for Statistical Computing、Vienna、Austria)及びGraphPad Prismバージョン6.00 for Machintosh(GraphPad Software、San Diego、California、USA)を使用して実施した。
本発明者らのグループは、近年、生着時の細菌多様性の増大がallo BMT後の全生存の改善及び移植関連死亡と関連付けられることを報告したが、その異質の患者母集団においては、本発明者らは、多様性がGVHDの減少と関連付けられるかどうかを決定することができなかった13。現在の研究では、本発明者らは、細菌フローラ多様性がGVHDを発症する高リスクにある患者のより均一な母集団において致死性GVHDについて予測することができるかどうかを問うことから開始した。本発明者らは、本発明者らの施設でallo BMTを受けた患者から採取した保存大便試料を利用した。本発明者らは、T細胞除去なしの従来のallo BMTの後に、BMT注入後及び退院前に大便試料を提供した(採取日8〜16日目、中央値12日目、臨床特徴は表2にまとめた)64人の患者のコホートを同定した。本発明者らは、454プラットフォームを使用して16Sr RNA遺伝子の領域V1〜V3を配列決定することによってこれらの大便試料のフローラ組成を分析し、GVHD関連死亡の発生について患者を臨床的に追跡した。
ブラウチア存在量がさらなる予後情報を提供するかどうかを判定するために、本発明者らは、ブラウチア存在量と急性GVHDの既知のリスク因子との潜在的な可能性を調査した31〜33。本発明者らは、前処置強度、患者の年齢、パフォーマンスステータス、ドナー/患者の性別、CMVステータス、及び疾病リスクがブラウチア存在量とは関連付けられないことを発見した(表4)。少ない数に制限されるが、アジア系またはラテンアメリカ系の患者は、より低いブラウチアの存在量を有するように思われた。最後に、ブラウチア存在量及び移植片源を評価することは関連性を示さなかった。要するに、本発明者らの分析は、ブラウチア存在量が急性GVHDの既知のリスク因子とは関連付けられないように思われることを示す。
本発明者らの患者母集団におけるブラウチア存在量の不均一性をより良く理解するために、本発明者らは、ブラウチア存在量の決定因子を同定することを試みた。両方のフローラコホートからのすべての大便試料の分析は、大部分の患者が、移植入院の申込時に比較的大量のブラウチアを有することを示した(>0.1(10%)の存在量中央値)(図4A)。しかしながら、多くの患者において、ブラウチアレベルは、入院期間中に急速に低下した。予測されるように、本発明者らは、嫌気性カバーを有する抗生物質へ曝露されなかった患者が増大したレベルのブラウチアを有する傾向があることを発見した(図4B)。
第2の研究では、本発明者らは、嫌気性細菌を標的とする抗生物質での治療がGVHD関連死亡における臨床的な差異と関連付けられるかどうかを問うことから開始した。本発明者らの施設においてAllo−HSCT患者は、ニューモシスチスイロベジイ肺炎を防ぐための短期間のトリメトプリム−スルファメトキサゾール、ならびに好中球減少の期間にわたる静脈内バンコマイシン及びシプロフロキサシンを含め、抗生物質の予防的レジメンを受ける。とりわけ、本発明者らは、このレジメンが通常は、腸内微生物叢の組成に軽度の攪乱のみをもたらすことを発見した(14)。allo−HSCTの期間の後半に、好中球減少性発熱を発症する患者は、実験抗生物質で治療され、その選択は、薬アレルギー歴または患者固有の検討事項に起因して様々であり得る。持続性発熱、腹部症状を発症するか、または耐性菌での感染症の微生物証拠を有する一部の患者は、多くの場合嫌気性菌に対してより活性である第二抗生物質を受け得る。最後に、allo−HSCT患者はまた、共通して、嫌気性腸内共生の損失を急速に引き起こすクロストリジウムディフィシル大腸炎とallo−HSCT入院中に診断され、かつその治療を受ける(17、18)。
嫌気性活性を有する抗生物質のGVHDに対する影響の因果関係及び機序をさらに探求するため、本発明者らは、マウスでの実験に切り替えた。発明者らはまず、健常なC57BL/6マウスを、嫌気性細菌に対する活性が増大した抗生物質(pip/tazo、イミペネム、及びメトロニダゾール)または活性の減少した抗生物質(アズトレオナム及びセフェピム)のいずれかで処置した。マウスは、各抗生物質皮下(SC)注射により2日間にわたり1日2回(pip/tazoは500mg/kg、及び他は100mg/kg)処置され、大便試料を採集し、続いて16Sr RNA遺伝子増殖及び配列分析を行った。発明者らは、イミペネムまたはメトロニダゾールでの全身治療はクロストリジウム目の存在量を著しく減少させ、エンテロコッカスの存在量を増加させる一方、アズトレオナムまたはセフェピムでの治療はクロストリジウム目に危害を加えないことを発見した(図16A)。興味深いことに、pip/tazoでの治療は治療の2日後に増殖可能な細菌DNAを生じず、マウスにおいてほぼ完全な除染を示した(データは示されない)。
興味深いことに、アッカーマンシアムシニフィラの増加は、これらの動物における6つの実験において一貫して観察された(図16G及びH)。本発明者らは、GVHDを有するマウスの結腸におけるT細胞侵入及びSTAT3リン酸化に対するイミペネム治療の影響を評価し、フローサイトメトリー及び組織診断の両方により結腸に侵入するT細胞の数の増加を、蛍光顕微鏡検査法によりインシチュでT細胞において見られる著しく高いレベルのリン酸化されたSTAT3と共に発見し、IL−23レベルの上昇が、特に結腸内で悪化したGVHDに寄与した可能性の高い、ドナーT細胞の動員及び活性化に関与したという考えを支持する。GVHDを有するマウスにおいて、イミペネム治療は、ヒト、マウス、及び他の動物の腸管で見つかる一般的な共生細菌であるアッカーマンシアムシニフィラの拡大をもたらした。とりわけ、この細菌は、炭素及び窒素の源としてムチンを利用するその能力という点で異例である(33)。結腸粘液層の崩壊が、アッカーマンシアムシニフィラでの無菌マウスの単一コロニー形成後に観察されており、アッカーマンシアムシニフィラがムチンをインビボならびにインビトロで低下させ得ることを示唆している(51)。しかしながら、腸の恒常性に対するアッカーマンシアムシニフィラの影響が何であるかは、不明確であり、環境依存性である可能性が高い。マウス肥満モデルでは、アッカーマンシアムシニフィラでの治療は、代謝障害の改善及び全身的な内毒素のレベルの減少をもたらし、この状況において、アッカーマンシアムシニフィラが腸の障壁機能を改善したことを示唆している(52)。しかしながら、サルモネラチフィムリウム感染症のノトバイオートマウスモデルは、アッカーマンシアムシニフィラの存在が、結腸粘液破壊の原因となる腸内炎症の悪化をもたらすことを示した(53)。イミペネム治療した動物における結腸の発明者らによる検査は、粘液層のほぼ完全な消滅を示した。本発明者らはまた、腸粘膜の侵害に対する防護の重大な第一線を提供する粘液層と一致している破壊された粘液層を超えた結腸粘膜固有層内の細菌の存在を検出した(54)。なぜアッカーマンシアがイミペネムで治療された移植マウスの結腸内で拡大するのかは不明瞭であり、発明者らが知るところでは、アッカーマンシア分離株がイミペネムまたは他の関連抗生物質に対して耐性があるとはこれまでに記されていない。アッカーマンシアとクロストリジウム目との競合的相互作用もまた、発明者らの知るところでは、これまでに説明されていないが、複数の研究が、クリンダマイシンなどのクロストリジウム目を抑制する他の抗生物質でのマウスの治療後のアッカーマンシアの同様の増加を目にしている(55)。これらの結果は、より限られた活性のスペクトルを有する(特に嫌気性菌に対する)抗生物質を選択することが、微生物叢損傷を防ぎ、GVHDを減少させ得ることを示唆する。

Claims (16)
- クロストリジウム目の細菌の1つ以上の精製された集団を含む、骨髄移植(BMT)または造血幹細胞移植(HSCT)後の移植片対宿主病(GVHD)の予防及び/または治療のための治療用組成物であって、前記細菌の少なくとも1つ以上が、ルミノコッカスオベウム、ユーバクテリウムデスモランス、ルミノコッカスラクタリス(ブラウチアプロダクタ(Blautia producta))、ルミノコッカスファエシス、クロストリジウムソルデリイ、及びそれらの組み合わせまたは混合物からなる群より選択される、治療用組成物。
- 前記細菌が、配列番号1、4、7、9、及び15のうちの1つのヌクレオチド配列または前記配列と約98%〜100%同一のヌクレオチド配列を有する16S rDNAを含む、請求項1に記載の用途のための治療用組成物。
- 前記組成物中の前記細菌が、生細菌、凍結細菌、発芽可能な胞子、またはそれらの組み合わせである、請求項1または2に記載の用途のための治療用組成物。
- 前記細菌が、104〜1010CFUの1回量で存在する、請求項1〜3のいずれか一項に記載の用途のための治療用組成物。
- 前記細菌が、105〜109CFUの1回量で存在する、請求項1〜3のいずれか一項に記載の用途のための治療用組成物。
- 前記細菌が、106〜108CFUの1回量で存在する、請求項1〜3のいずれか一項に記載の用途のための治療用組成物。
- 前記細菌が、キシロース、ラフィノース、セロビオース、メリチトース(melizitose)およびこれらの組み合わせより選択されるオリゴ糖を発酵させる、請求項1〜6のいずれか一項に記載の用途のための治療用組成物。
- 経口投与用に製剤化された請求項1〜7のいずれか一項に記載の用途のための治療用組成物。
- 結腸/直腸投与用に製剤化された請求項1〜7のいずれか一項に記載の用途のための治療用組成物。
- 骨髄または造血幹細胞移植を受ける対象における、移植片対宿主病(GVHD)を発症するリスクを減少させる及び/またはGVHDを治療するための、クロストリジウム目からの1つ以上の細菌を含む治療用組成物であって、前記細菌の少なくとも1つ以上が、ルミノコッカスオベウム、ユーバクテリウムデスモランス、ルミノコッカスラクタリス(ブラウチアプロダクタ(Blautia producta))、ルミノコッカスファエシス、クロストリジウムソルデリイ、及びそれらの組み合わせまたは混合物からなる群より選択される、治療用組成物。
- 前記組成物が、
(i)移植前もしくは移植後のいずれかに対象の微生物叢中で圧倒的に少ない1つ以上の細菌分類群の増殖もしくは活性を促進する、または
(ii)前記対象の微生物叢中で圧倒的多数の1つ以上の細菌分類群の増殖もしくは活性を抑制する、請求項10に記載の用途のための治療用組成物。 - 前記対象に投与されることを特徴とする、請求項10または11に記載の用途のための治療用組成物。
- 前記組成物が、嫌気性菌に対して高活性を有する抗生物質を用いた前記対象の治療の中断から約1日〜約2週間後に前記対象に投与されることを特徴とする、請求項12に記載の用途のための治療用組成物。
- 前記組成物が、allo−BMTまたはallo−HSCTの約7〜10日前に前記対象に投与されることを特徴とする、請求項12または13に記載の用途のための治療用組成物。
- 前記組成物が、allo−BMTまたはallo−HSCTの約1日〜約1週間前に前記対象に投与されることを特徴とする、請求項12または13に記載の用途のための治療用組成物。
- allo−BMTもしくはallo−HSCTを受ける個人におけるGVHDの予防、リスクの減少、及び/または治療での使用のための、クロストリジウム目からの1つ以上の細菌を含む治療用組成物であって、前記細菌の少なくとも1つ以上が、ルミノコッカスオベウム、ユーバクテリウムデスモランス、ルミノコッカスラクタリス(ブラウチアプロダクタ(Blautia producta))、ルミノコッカスファエシス、クロストリジウムソルデリイ、及びそれらの組み合わせまたは混合物からなる群より選択される、治療用組成物。
