Description

子供の血漿中の自閉症スペクトラム障害のバイオマーカー
可能なかぎり若い年齢で自閉症スペクトラム障害(ASD)を診断することが、最適な有効介入の開始に重要である。患者は、行動テストを介して約2齢で確実に診断可能である。しかしながら、米国では、診断の平均年齢は約4齢である。ASDは、多くの原因およびさまざまな遺伝的リスク因子を有することが、増大する証拠から示唆される。血液サンプルからASDの代謝バイオマーカーシグネチャーを同定することにより、早期の診断テストを開発する機会が提供される。
被験体サンプル
最初に、カリフォルニア大学デービス校神経発達障害医学研究所クリニック地域センター(UC Davis M.I.N.D.Institute Clinic,Regional Centers)、臨床医からの紹介、地域学校区、およびコミュニティー支援グループ、たとえば、早期自閉症治療を求める家族の会(Families for Early Autism Treatment)(FEAT)を介して、実験被験体を募集し、4齢〜6齢の狭い年齢範囲に限定した(表1参照)。定型発達参加者(N=30)は、地域学校区およびコミュニティーセンターから募集した。最初の研究の側面はすべて、カリフォルニア大学デービス校施設内審査委員会(University of California at Davis Institutional Review Board(IRB)により承認された。各参加者の親または保護者から書面によるインフォームドコンセントを取得し、個人情報識別子なしでデータを解析した。インフォームドコンセント後、被験体の診断測定および心理測定を終了した。経験を積んだ神経心理学者(BAC)により決定されたDSM−IV基準に基づく自閉症スペクトラム障害の診断からなる選択基準の下で、ASDの研究参加者(N=52)を登録し、研究に信頼性のある臨床医による次の測定によりさらに確証した。自閉症診断観察スケジュール−包括版(Autism Diagnostic Observation Schedule−Generic)(ADOS−G)は、自閉症および自閉症スペクトラム障害に対するカットオフを有するアルゴリズムを含めて、子供のコミュニケーション、社会的相互作用、および常同行動についての知見を提供する。
血漿サンプルを50μlアリコートに分割し、代謝物抽出前、−80℃で貯蔵した。これらの手順の間、サンプルを氷上に保持した。診断、IQ、年齢、および民族性が各バッチ内で均等に分布するように、LC−HRMS分析のためにサンプルを3つのバッチにランダム化した。−20℃で450μLの8:1メタノール:水の溶液を用いて、50μL血漿アリコートから低分子を抽出した(Jiye et al.,2005,Anal Chem;77:8086−8094)。抽出溶液はまた、内部標準を含有していた。サンプルを2〜8℃で10分間撹拌し、次いで、4℃、18,400×gで20分間遠心分離し、沈殿物を除去した。上清を新鮮なチューブに移し、遠心分離工程を繰り返し、いずれの残留沈殿物も除去した。最終遠心分離後、450μLの上清を新鮮なチューブに移し、次いで、SpeedVac内で蒸発乾固させ、次いで、内部標準をも含有するアセトニトリル:0.1%ギ酸中の0.1%ギ酸の45μLの50:50混合物に再溶解させた。次いで、この溶液をLC−HRMS分析のための高性能液体クロマトグラフ(HPLC)オートサンプラー注入バイアルに移した。
より良好なメタボローム対象範囲を得るために、標的化GC−MSと、さらには非標的化LC−HRMSとの両方を利用した。正イオン極性および負イオン極性の両方でエレクトロスプレーイオン化と組み合わせたC8またはHILICクロマトグラフィーを含めて、4つの非標的化LC−HRMS法を用いて、1個のサンプルあたり4つの個別のデータ取得を行った。低分子代謝物の対象範囲の幅を最適化するために、臨床患者サンプルの評価前に、LC−HRMS法の開発およびテストを行った。
高分解能(QTOF)質量分析計と組み合わせたAgilent 1290HPLCからなるAgilent G6540四重極飛行時間(QTOF)LC−HRMSシステムを用いて、LC−HRMSを行った。高分解能精密質量条件下で二重ESIイオン源を用いて、正イオンおよび負イオンモードの両方でエレクトロスプレーイオン化(ESI)を利用した。親水性相互作用液体クロマトグラフィー(HILIC)では、2.1×150mmの寸法、1.7μMの粒子サイズを有するWaters Acquity超高性能液体クロマトグラフィー(UPLC)BEHアミドカラムを使用し、40℃に維持した。水中0.1%ギ酸およびアセトニトリル中0.1%ギ酸を有する溶媒グラジエントを用いて、0.5mL/分の流量で、各サンプルについてデータを29分間取得した。サンプルの2μLアリコートを注入した。C8クロマトグラフィーでは、水中0.1%ギ酸およびアセトニトリル中0.1%ギ酸を有するグラジエントを用いて、0.5mL/分の流量で、各サンプルについてデータを50分間取得した。Agilent Zorbax Eclipse Plus C8 2.1×100mm、1.8μM粒子サイズのカラムを使用し、40℃に維持した。サンプルの2μLアリコートを注入した。
Fiehn et al.(Fiehn et al.,2008,Plant J;53:691−704)に記載されるように、GC−MS分析を行った。LECO Pegasus IV TOF質量分析計と組み合わせたAgilent6890ガスクロマトグラフを用いて、GC−MSデータを取得した。サンプルデータと、質量スペクトル、保持指数、構造、外部代謝データベースとのリンクを含むGC−MSにより同定された1,000種超の化合物のデータベースと、を比較することにより、代謝物の同定を行った。
主要な代謝物の化学構造を、それらの発見に用いたのと同一のクロマトグラフィー条件でタンデム質量分析(LC−HRMS−MS)法により、さらに確認した。最高品質の生成物イオンスペクトルが得られるように最適化された衝突のエネルギー条件を用いて、患者サンプルおよび/または参照血液サンプルについて、Agilent QTOF質量分析計によりLC−HRMS−MS分析を行った。次いで、得られた生成物イオンスペクトルと、公共スペクトルデータベース、たとえば、METLIN(Smith et al.,2005 Ther Drug Monit;27:747−751)、MassBank(Horai et al.,2010,J Mass Spectrom;45:703−714)、Steminaの自社SteminaMetDBデータベースで利用可能なMS−MSスペクトルと、を比較した。
LC−HRMSデータの前処理
最初に、方法開発時に確立された品質基準、たとえば、存在量閾値、保持時間ピーク形状整合性に関して、生の質量スペクトルデータ全イオンクロマトグラムおよび内部標準抽出イオンクロマトグラムを調べた。外れ値基準を満たしたクロマトグラムを呈するデータファイルをさらなる分析から除去した。生データをオープンソースmzDataファイル(Orchard et al.,2007,Proteomics;7:3436−3440)に変換した。オープンソースソフトウェアライブラリーXCMS(Smith et al.,2006,Anal Chem;78:779−787)を用いてピーク選択および特徴生成を行い、次いで、非線形クラスタリング法に基づくobiwarpアルゴリズム(Prince and Marcotte,2006,Anal Chem;78:6140−6152)を用いて保持時間の偏差を補正し、LC−HRMSデータを整合させた。XCMS密度ベースのグループ化アルゴリズムを用いて質量特徴を生成し、次いで、繰返しピーク充填を用いて特徴ビンの保持時間および質量範囲に基づいて欠測特徴を組み込んだ。「質量特徴」(本明細書では「特徴」としても略記される)とは、1)検出された質量電荷比(m/z)および2)クロマトグラフィー保持時間の2つの性質により定義された質量分析計により検出された部分のことである。
82個の患者サンプル(52個のASDサンプルおよび30個のTDサンプル)を、2つのセット、すなわち、(1)統計的に有意な特徴の同定および分類モデリングのための61個のサンプル(39個のASDおよび22個のTD)のトレーニングセットと、(2)分類モデルの性能を評価するために使用される21個のサンプルの独立検証セット(13個のASDおよび8個のTD)と、に分割した。これは、各セットがランダム化に使用される因子を類似の比率で含有するように、これらのトレーニングセットおよび検証セットで診断、患者IQ、および性別を用いてサンプルをランダム化することにより達成された。検証サンプルセットを単変量フィルタリングプロセスおよびモデル開発プロセスから差し引いて、モデル性能を評価する独立外部サンプルセットとして機能させた。
全特徴セット、過適合可能性を低減し、かつ予測シグネチャーの生物学的解釈可能性を増加させるために、t検定を使用した(Haury et al.,2011,PLoS One;6:e28210)。GC−MSおよびLC−HRMSのプラットフォームからの内部標準(IS)に規格化された質量特徴の積分面積を単一のデータセットに組み合わせた。質量特徴の平均積分面積の差が実験クラス間に存在しないという帰無仮説と、示差的特徴を同定するASDトレーニングセットサンプルとTDトレーニングセットサンプルとの間に平均積分面積の差が存在しないという対立仮説と、の下でウェルチのt検定を用いて、サンプルのトレーニングセットで前処理フィルターを通過した4572個の特徴をさらにフィルタリングした。未補正p値<0.05を有して統計的に有意な変化を示した各特徴について、その抽出イオンクロマトグラム(EIC)を、サンプル間の積分の整合性、ピーク形状、および>3000の最小ピーク高さ要件に関して、レビューした。次いで、このEIC品質レビュープロセスを通過した特徴を分類モデリングに利用した。p値補正のベンジャミニ・ホッホバーグ法を用いて、偽発見率(FDR)を計算した(Benjamini and Hochberg,1995,J R Stat Soc Ser B;57:289−300)。
2つの主目的で、すなわち、VIP(投影における変数重要性)スコア指数を用いてASDを識別する際に代謝物の重要性をロバストにランク付けするために、およびASDおよびTDの実験クラスの最高レベルの識別を達成するのに必要とされる予測代謝物の最小セットを同定するために、モデル開発を行った。全体で61個のサンプルのトレーニングセットを用いて、部分最小二乗判別分析(PLS−DA)またはサポートベクターマシン(SVM)の分類器をトレーニングすることにより、モデルを生成した。複数の手法を用いて分子シグネチャーが予測性でありうることを実証するために、モデリング技法PLS−DAと、さらには線形カーネルを用いたSVMと、の両方を利用した(Wold,1985,“Partial least squares,”In:Kotz S,Johnson NL,editors.Encyclopedia of statistical sciences.New York:Wiley,Vol.6.pp.581−591、およびCortes and Vapnik,1995,Mach Learn;20:273−297)。Rパッケージの分類および回帰トレーニング「caret」バージョン5.17−7(Kuhn,2008,J Stat Softw;28:1−26)を用いて、部分最小二乗(PLS)およびSVMの分類モデルを生成した。RパッケージROCRバージョン1.0−5(Sing et al.,2005,Bioinformatics;21:3940−3941)を用いて、レシーバーオペレーター曲線(ROC)分析を行った。
複数の分析方法の使用は、メタボロームの広範な対象範囲を提供し、各方法は、ASD児とTD対照との分類モデルに質量特徴を付与した。各方法は、提供された特有の特徴に関して評価された。最初に、5つの分析プラットフォーム全体で10187個の質量特徴が検出された。HILIC LC−HRMS法は、モデル中で最も多くの個別の質量特徴をもたらし、その次は、C8 LC−HRMS、次いで、GC−MSであった。前のフィルターを通過した4572個の特徴について、単変量解析フィルタリングを行った。LC−HRMS特徴の約60%は、化学構造が推定上アノテートされ、アノテートされた特徴の8%(503個)は、FBF手順の基準を通過した。標的化GC−MS特徴の約36%(142個)は、代謝物であることが確認された。これらの結果の内訳は、表2に含有されている。
179個の選択された特徴を変数として用いてSVMおよびPLSの分類方法によりASD児のサンプルとTD児のサンプルとを識別し、将来的なバイオマーカー開発への取組みのために分類への各特徴の寄与を評価した。各モデリング法(CVトレーニングセット)で行われたモデリングの100回の繰返しのすべてからの最適スコアを用いて、トレーニングセットおよび独立検証テストセットの両方からROCプロットを作成し、モデル性能を理解した。生成された100個のモデルを平均し、真応答率対偽陽性率の関数としてプロットした。ASD児とTD個体とを分類可能な代謝シグネチャーを検出可能であることが、SVMモデリング法およびPLSモデリング法の両方から示唆された。SVMモデルは、0.95のAUC値(95%信頼区間(CI)0.94〜0.96)を提供し、PLSモデルは、0.92のAUC値(95%CI 0.91〜0.94)を与えた。モデルの分類確度がランダムな結果ではないことを確認するために、グループ診断クラスラベルのランダム置換を用いて特徴をモデリングした。これらの結果は、SVMおよびPLSに対してそれぞれ0.52(95%CI 0.48〜0.57)および0.52(95%CI 0.49〜0.56)のAUC値を有して、ほぼランダムな分類を示したことから、特徴は、ランダム化データセットを用いてクラスを識別できないことが示唆される(図2)。
21個のサンプルの独立検証セットを用いて、加重VIPスコアに基づいて生成されたさまざまな特徴サブセットを外側交差検証ループに依存せずに評価した。以上に記載の80個の特徴のSVMモデルは、81%の分類予測確度、85%の感度、75%の特異度および0.84のAUCを有していた(図2、黒色細線)。140個の変数を含む最良性能PLSモデルは、81%の確度、85%の感度、75%の特異度、および0.79のAUCを有していた(図2、灰色細線、表4)。70%の確度を達成するために少なくとも40個の特徴が必要であり、かつ80〜160個の範囲内の特徴が、この独立検証データセットで良好な性能を発揮することが、結果から示唆される。
LC−HRMS−MSにより確認された7個のLC−MS質量特徴の化学的同一性を表5に示す。LC−HRMS−MSまたは標的化GC−MSにより確認された代謝物に含まれるのは、ホモシトルリンであり、これは、この研究で最大の統計的有意性ならびにSVMおよびPLSの分類モデルの両方ですべての特徴の中で最高のランクを有していた。有意なアップレギュレーションまたはダウンレギュレーションを示す他の代謝物は、アスパルテート、グルタメート、デヒドロエピアンドロステロン硫酸(DHEAS)、クエン酸、コハク酸、メチルヘキサデカン酸、テトラデカン酸、ヘプタデカン酸、イソロイシン、グルタル酸、3アミノイソ酪酸、およびクレアチニンを含む。これらは、表5に列挙されており、アミノ酸、有機酸、ステロール、および脂肪酸を含めて、さまざまな分子クラスを表す。
この実施例に記載の非標的化メタボロミック法は、使用した分析方法の分離限界および検出限界以外で可能な経路への偏りを有していなかった。この手法は、ASDのリスクがある個体を識別するのに役立ちうる生化学的に多様な代謝物セットの発見をもたらした。
自閉症に既に関連付けられてきた本発明者らの研究で有意なアップレギュレーションまたはダウンレギュレーションを示す代謝物の例は、以下のものを含む。
クエン酸(減少)およびコハク酸(増加)を含むトリカルボン酸回路関連分子は、ASD参加者で有意に変化することが見いだされた。自閉症児での尿スクシネートの上昇(Yap et al.,2010,J Proteome Res;9:2996−3004、およびMing et al.,2012,J Proteome Res;11:5856−5862)および尿シトレートの減少(Frye et al.,2013,Transl Psychiatry;3:e220)が、他のものにより報告されてきた。
メチルヘキサ−、テトラ−、およびヘプタ−デカン酸での本発明者らの観測に類似して、ASD児の血漿で脂肪酸が減少することが、既に観測されてきた(El−Ansary et al.,2011,Lipids Health Dis;10:62)。飽和脂肪酸代謝と酸化ストレスとの関連は、ASD児の赤血球で報告されてきた(Ghezzo et al.,2013,PLoS One;8:e66418)。
3アミノイソ酪酸は、ASDの参加者のサンプルで増加した。これはまた、以前の知見に一致する(Adams et al.,2011,Nutr Metab(Lond);8:34)。
また、クレアチニンは、ASD児で減少した。これは、PDDと診断された子供の尿クレアチニンで類似の変化を観測しているWhitelyらの知見と一致している(Whiteley et al.,2006,Pediatr Int;48:292−297)。
確認された代謝物の多くは、ASDおよびミトコンドリア生物学の側面の両方に直接関連付けられる。ミトコンドリア疾患または機能不全は、潜在的に自閉症に関与すると提案されてきた(Marazziti et al.,2012,Eur Rev Med Pharmacol Sci;16:270−275)。それに加えて、いくつかの代謝物は、酸化ストレスを含めてASDに関与すると既に提案されてきた他のプロセスに関連付けられる(Rossignol and Frye,2012,Mol Psychiatry;17:389−401)およびエネルギー生産(Blaylock,2009,Altern Ther Health Med;15:60−67)。
消化管マイクロバイオームとASDとの間のこの潜在的関係はまた、かなりの関心を受けている(Mulle et al.,2013,Curr Psychiatry Rep;15:337)。ASD個体の尿に関するメタボロミック研究では、マイクロバイオームに関連付けられてきたジメチルアミン、ヒプレート、フェニルアセチルグルタミンなどの分子が同定されてきた(Yap et al.,2010,J Proteome Res;9:2996−3004、およびMing et al.,2012,J Proteome Res;11:5856−5862)。この研究では、p−ヒドロキシフェニルラクテートの血漿中レベルの減少が観測された。p−ヒドロキシフェニルラクテートは、血流および組織の両方で抗酸化剤として機能することが知られているビフィズス菌および乳酸桿菌に関連付けられる代謝物である(Beloborodova et al.,2012,J Biomed Sci;19:89)。
この研究ではまた、ホモシトルリンなどの新しいこれまで記載されてこなかった潜在的ASDバイオマーカーを同定した。これは、SVMおよびPLSの両方の分類モデルですべての特徴のうちで最大の統計的有意性および最高のランクを有していた。ホモシトルリンは、ミトコンドリア内でリシンおよびカルバモイルリン酸から生成されることが知られているよく理解されていない分子である。オルニチントランスロカーゼ(SLC25A15)欠損に関連付けられる尿素回路欠損を有するホモシトルリン尿症(HHH)症候群患者は、より高い尿ホモシトルリンレベルを有し、発達遅延、運動失調症、痙縮、学習障害、認知障害、および/または原因不明発作などのASDに類似した行動異常を呈しうる(Palmieri,2004,Pflugers Arch;447:689−709)。これらのデータから、尿素回路機能の変化は、本発明者らが血漿で観測したホモシトルリンの減少に関連付けられうることが示唆されうる。
この実施例では、いくつかのMSベースのメタボロミック分析の組合せにより子供の血漿中の代謝物の変化のプロファイルを検出可能であることを明らかにする。導出された代謝データから開発された統計モデルは、80%よりも良好な確度でASD児とTD個体とを識別した。研究では、臨床診断された4〜6歳のASD児および定型発達個体の良好に管理されたサンプルセットを使用した。発見された代謝物のより多くの化学構造を確認するために、およびより多くのかつより若い患者集団で評価することにより、ASDのリスクを決定するのにどれが最もロバストであるかを決定するために、さらなる研究を行っている。
確認された追加の代謝物
実施例1でより詳細に記載された手順を用いて、第2のASDサンプルセット(MIND2研究)をアッセイした。この研究集団は、180名の定型被験体(男性69%、平均年齢3.1歳、発達状態106)および93名の自閉症被験体(男性83%、平均年齢3歳、発達状態62)に由来するサンプルを含んでいた。サンプルが採取された時、すべての被験体の食事状態は、摂食状態であった。抗凝固剤としてシトレートを使用した。
高機能自閉症および低機能自閉症の代謝シグネチャー
実施例1でより詳細に記載されるように、多数の教師ありおよび教師なしの統計的方法を用いて、ASDの予測性の高い代謝シグネチャーを同定した。70名のASD患者および年齢の一致する30名の定型発達対照の集団からのサンプルで、サンプルモデル確度感度特異度を4〜6歳の年齢範囲(平均5.4歳)の高機能自閉症(HFA)児(IQ>70、n=33)、低機能自閉症(LFA)児(IQ<70、n=36)、および定型発達(TD)児(n=34)に分割した。

追加のコホート
この実施例では、80%超の確度を有してASDの患者を同定可能な血液中の179種の代謝物(または代謝産物グルーブ)を成功裏に発見した前の実施例の研究が継続されよう。患者の血液で測定可能なバイオマーカーは、障害の代謝面での理解、および現在の主要の診断方法である行動分析よりも早期の診断を可能にしうる。この実施例では、ASDの個体の血漿中の何百〜何千もの代謝物が直接測定され、これらの測定結果と類似の年齢の非自閉症個体から得られたものとが比較されよう。非標的化メタボロミック解析法では、カリフォルニア大学デービス校(UC−Davis)の神経発達障害医学研究所(MIND Institute)で非常に良好に特徴付けられたサンプルセットからの研究保存血サンプルが使用されよう。最終的に、この実施例では、定型患者と比較してASDの個体の血漿中に異常レベルのいくつかの代謝物が存在することが通知されよう。代謝物が同定され、代謝経路にマッピングされよう。このことは、ASDの機序の理解を深めるのに役立つと同時に、将来的な療法開発の潜在的目標を提供するのに役立つであろう。最終的に、同定された代謝物は、臨床診断キットなどの他のタイプのプラットフォームへ移行可能である。
Claims (11)
- 自閉症に対する被験体のリスクを評価する方法であって、
表6に列挙された179種の低分子代謝物のうちの1種以上に関して前記被験体のバイオサンプルをアッセイする工程であって、ここで、前記アッセイは、複数のクロマトグラフィー質量分析技術を利用する工程と、および
表6に列挙された179種の低分子代謝物のうちの1種以上の量を定量する工程と、

ここで、表6に列挙された前記代謝物のうちの少なくとも40種の、非自閉症対照と比較して統計的に有意な存在量差が、自閉症のリスク増加の指標となり、または
