Description
姑息的前方十字靭帯再建術は、解剖学的位置ではない等尺点(isometric point)部位に新たな靭帯を再建する方法であり、成功率は80%ほどと知られているが、回転不安定性に対して、制限(constraint)役割を行えないという問題点が存在すると知られている。従って、最近では、本来の前方十字靭帯(native ACL(anterior cruciate ligament)を再建するために、解剖学的位置(すなわち、前方十字靭帯が、正常の人体において、骨に付着される位置)に、前方十字靭帯を再建する概念に関心が高まっている。
さらに詳細には、図3に図示されているように、大腿トンネル誘導機100が、硬骨トンネル311を通過した状態で、図4に図示されているように、大腿トンネル誘導機100が矢印方向に一定程度回転されることにより、誘導ピン140を前方十字靭帯の解剖学的位置に位置させるのである。
このとき、前方十字靭帯の解剖学的位置の一般的な場合を考慮すれば、オフセット部130の舌状部131が、誘導管120の延長線上から、およそ10mm離隔されて形成されることが最も望ましいのである。
Claims (5)
- 取っ手と、
前記取っ手と結合し、一方向に延設される誘導管と、
前記誘導管の一端部に形成され、前記一方向において、前記誘導管の延長線上からの離隔距離が変更可能であるように形成されるオフセット部と、
前記オフセット部と連結され、前記オフセット部と前記誘導管との離隔距離を制御するオフセット制御部と、を含み、
前記オフセット部は、
前記誘導管から延設された連結部と、
前記連結部の一端部に形成される舌状部ベースと、
前記誘導管に対して引き入れられたり引き出されたりするガイド部と、
前記舌状部ベースと前記ガイド部とを連結する1以上のリンクと、を含み、
前記連結部、前記ガイド部、前記1以上のリンク、及び前記舌状部ベースは、4節リンクを形成することを特徴とする、
大腿トンネル誘導機。 - 前記オフセット部と前記誘導管との離隔距離は、8mm以上15mm以下に制御されることを特徴とする請求項1に記載の大腿トンネル誘導機。
- 前記大腿トンネル誘導機は、
前記大腿トンネル誘導機が、硬骨トンネルを介して大腿骨に挿入された状態で、前記誘導管を貫通した誘導ピンが、前方十字靭帯の解剖学的位置に位置するように形成されることを特徴とする請求項1または2に記載の大腿トンネル誘導機。 - 前記大腿トンネル誘導機は、
前記大腿トンネル誘導機が、前記硬骨トンネルを通過した状態で、前記大腿トンネル誘導機を一定程度回転させることにより、前記誘導ピンが、前方十字靭帯の解剖学的位置に位置するように形成されることを特徴とする請求項3に記載の大腿トンネル誘導機。 - 前記オフセット制御部を調節すれば、前記ガイド部が、前記誘導管に対して引き入れられたり引き出されたりしながら、前記誘導管と前記オフセット部との離隔距離が変更されることを特徴とする請求項1に記載の大腿トンネル誘導機。
