Description

1つまたはそれ以上の前立腺がん関連疾患状態が、工程(b)にて測定した1個またはそれ以上のバイオマーカーの試験試料中の発現が、工程(f)にて測定した1個またはそれ以上のバイオマーカーの対照試料中の発現に相当するという事象において同定される。
(i)プロリンは、その側鎖がα炭素ならびに窒素の両方に結合するので、ペプチド構造を安定化させて良い;
(ii)フェニルアラニン、チロシンおよびトリプロファンは、芳香族側鎖を有し、非常に疎水性である、一方でロイシンおよびイソロイシンは、脂肪族側鎖を有し、また疎水性である;
(iii)リシン、アルギニンおよびヒスチジンは塩基性側鎖を有し、中性pHにて正に帯電し、一方でアスパラギン酸およびグルタミン酸は、酸性側鎖を有し、中性pHにて負に帯電する;
(iv)アスパラギン酸およびグルタミン酸は中性pHでニュートラルであるが、水素結合に参加してよいアミド基を含む;
(v)セリン、スレオニンおよびチロシン側鎖は、ヒドロキシル基を含み、これは水素結合に参加してよい。
(i)試料中に存在するバイオマーカーをビオチンで標識する工程;
(ii)ビオチン標識タンパク質を、その表面上の個別の位置で固定化された複数のscFvを含むアレイと接触させる工程であって、scFvが、表1中のタンパク質の1つまたはそれ以上に対して特異性を有する、工程;
(iii)固定化scFvを、蛍光色素を含むストレプトアビジンコンジュゲートと接触させる工程;および
(iv)アレイ表面上の別個の位置での色素の存在を検出する工程
を含んでもよく、アレイ表面上の色素の発現が、試料中の表1からのバイオマーカーの発現の指標である。
A)本発明の第1の態様、もしくはその任意の実施形態もしくは実施形態の組合せにおいて定義した1つまたはそれ以上の第1の結合剤、または本発明の第2の態様、もしくは任意の実施形態またはその実施形態の組合せによるアレイ;

を含む、前立腺がんの存在を決定するためのキットを提供する。
血中の前立腺特異的抗原(PSA)を使用した前立腺がん(PC)の早期検出が、スクリーンされていない男性間で、PC−死を減少させる。しかしながら、一般に使用されるカットオフでの、PSAの特異性が適度であるため、穏やかに上昇するPSAを有する男性間で、増強したリスク分類に寄与しているさらなるバイオマーカーに対する差し迫った必要性が存在する。本研究では、組換え抗体マイクロアレイを、ルーチンのPSA−測定からの80個の血漿試料のタンパク質発現プロファイリングのために適用し、総および%遊離PSAのレベルに基づいて、推測的に4つのリスク群に分けた。結果は、血漿タンパク質プロファイルが、PC(悪性バイオマーカーサイン)を正確に示すこと、および最も重要なことに、生化学的に定義されたPCリスク群と、中〜高相関を示したことを、同定可能であることを実証した。とりわけ、データはまた、推測的に異質であると公知の、中程度範囲PSAと低%遊離PSAを有するリスク群がさらに、それぞれ最も低いおよび最も高いリスク群とより共通点のある、2つの亜群に層別化可能であったことを暗示した。結論として、この概念実証研究において、本発明者らはしたがって、PCのリスク群に関連した、血漿タンパク質バイオマーカーサインが、アフィニティプロテオミクスを使用して、未精製血漿試料から同定可能であったことを示した。本アプローチは、より長期の視点において、PC患者の改善されたリスク分類のための新規の機会を提供可能であった。
臨床試料
本発明者らは、Dept.of Clinical Chemistry、Skane University Hospital,Malmo,Swedenでの、日常的に実施されたPSA試験を参照して、50〜70歳の80人の男性からの、非特定化EDTA抗凝固血液試料を使用した。臨床情報または患者同定子は、これらの試料に対して保持されておらず、試料を使用まで−80℃にて保存した(表3)。遊離およびtPSAのレベルを、WHO96/670(PSA−WHO)およびWHO68/668(遊離PSA−WHO)較正器に対して較正した、二重標識DELFIA Prostatus(C)総/遊離PSA−アッセイ(Perkin−Elmer、Turku、Finland)を使用して決定した。検出可能範囲は、tPSAに対して0.10〜250ng/ml、遊離PSAに対して0.04〜250ng/mlであった。tPSAを測定することに対する変化係数(CV)は、tPSAに対して≦10.6%、遊離PSAに対して≦7.3%であった。試料を、%遊離および総PSAのレベルに基づいて4つの群に分けた。A(n=20)はtPSA≦0.70ng/mlを有した;B(n=20)は、%fPSA≧27.9%で2.1〜8.0ng/mlのtPSAを有した;C(n=20)は、%fPSA≦12.6%で5.0〜10−3ng/mlのtPSAを有した;およびD(n=20)は、24.6〜724ng/mlのtPSAを有した。非特定化試料のこれらの4群は、非常に異なったPC診断または結果のリスクのカテゴリーを反映し、群Aは、意義のあるPCの非常に低い長期リスクを有し、群Bは、前立腺疾患の中程度増加リスクを有するが、臨床的に意義のあるPCの可能性は低く、群Cは、PCの重度の増加リスクを有し、群Dは、臨床的に意義のあるまたは進行ステージのPCの非常に高リスクを有する[38〜41]。従った手順は、1975のHelsinki Declarationによった。
EDTA抗凝固血漿試料を、1つのマイナーな調整を加えて、血清プロテオームに対して、先に最適化されたプロトコル[28、29]にしたがって標識した。血漿試料を、凝固することからの防ぐために、EDTAを、プロトコルを通して使用したPBS緩衝液に、4mMの最終濃度まで加えた。簡単には、試料を、16000×gにて20分間、4℃にて遠心し、5μlの試料をついで、45回、4mM EDTA PBS中に希釈し、約2mg/mlの総タンパク質濃度を得た。希釈試料を、0.6mM EZ−Link Sulfo−NHS−LC−Biotin(Pierce、Rockford、IL、USA)とともに、2時間氷上でインキュベートし、その後未反応ビオチンを、4mM EDTA−PBSに対して、72時間4℃にて透析することによって除去した。最後に、試料を分液し、マイクロアレイ実験での使用の前に−20℃で保存した。
主に免疫制御に関与する62つの異なる解析物に対して指向する162個のヒト組換えscFv抗体断片を、n−CoDeRライブラリー[42]から選択し、Bioinvent International AB(157個のscFvクローン;Lund、Sweden)およびM.Ohlin博士(5つのムチン−1特異的クローン、Dept.of Immunotechnology、Lund University、Lund、Sweden)より好意に提供された(サポート情報表3)。これらのファージディスプレイ誘導scFv抗体の特異性、アフィニティ(1〜10nM範囲内)およびオンチップ機能性を、(i)厳重ファージディスプレイ選別プロトコル[42]、(ii)多重クローン(≦4)/標的解析物および(iii)マイクロアレイ適用に対して適合した分子デザイン[26、27]を使用して確かにした。さらに、種々の抗体の特異性/反応性パターンは、十分に特徴付けられた血漿試料と、ELISA、Meso Scale Discovery(MSD)、サイトメトリビーズアレイ(CBA)および質量分析(MS)のような直交法を用いて、ならびにスパイキングおよびブロッキング実験を使用しすでに確認されてきた(サポート情報表3)[31、33、34、36]。全てのscFvを、100ml大腸菌培養液中で生成し、Ni2+−NTAアガロース(Qiagen、Hilden、Germany)上のアフィニティクロマトグラフィーを使用して、発現上清から精製した。結合した分子を、250mMイミダゾール(Saveen Werner、Malmo,Sweden)にて溶出し、PBSに対して透析し、ついで、マイクロアレイ実験にて、使用の前に4℃にて保存した。scFvの完全性と純度を、10% SDS−PAGE(Invitrogen、Carlsbad、CA、USA)によって評価した。タンパク質濃度を、280nmでの吸収を測定することによって決定した。
簡単に、scFvマイクロアレイを、被接触ディスペンサ(SciFlexarrayer S11、Scienion、Berlin、Germany)を使用して生成し、先に最適化されたセットアップにしたがって処理した[28、29]。合計において、180個のscFvと対照を、8複製にて、Blank Polymer Maxisorpスライド(NUNC A/S、Roskide、Denmark)上に、0.05〜0.4mg/mlのscFv濃度にて、各位置に一滴(300pL)で、アレイした。180個のプローブを、それぞれ60列を含む3つのカラム内にアレイした(図1A)。AlexaFlour−647標識ストレプトアビジン(10ug/ml)を陽性対照としてプリントし、プリンティング緩衝液(PBS)を陰性対照として含んだ。プリンティングの後、スライドを乾燥させ、PBS中5%(w/v)無脂肪牛乳(Semper AB、Sundbyberg、Sweden)中で、o/nでブロックした。スライドをついで、Protein Array Workstation(Perkin Elmer Life & Analytical Sciences、Wellesley、MA、USA)中に配置し、PBS中0.5%(w/v)Tween−20(PBS−T)で、4分間洗浄した。次に、1%(w/v)無脂肪牛乳粉末と1%(v/v)Tween−20(PBS−MT)中1:2希釈した70μlの標識試料を注射し、60分間撹拌しながらインキュベートした。第2の洗浄後、アレイを、PBS−MT中350μlの1μg/ml Alexa−647コンジュゲートストレプトアビジンとともに60分間インキュベートした。洗浄後、アレイを、窒素気体流下乾燥させ、5つの異なるスキャナ設定(50%PMTゲインおよび70%レーザー出力(50/70)、70/70、80/80、80/90および90/90)を使用して、共焦点マイクロアレイスキャナ(ScanArray Express、Perkin Elmer Life & Analytical Sciences)にて、5μm解析度でスキャンした。シグナル強度を、ScanArray Expressソフトウェア・バージョン4.0(Perkin Elmer Life & Analytical Sciences)を使用して定量化した。局所バックグラウンドを差し引き、任意の潜在的局所欠陥に対して補償するために、2つの最も高い、および最も低い複製を自動的に除外した。提示したシグナル強度は、残りの4つの複製スポットに対する平均値を提示している。不飽和スポットのみが、解析のために考慮された。
データ組のチップ間標準化を、DNAマイクロアレイに対して開発された標準化と類似の、半グローバル標準化アプローチ[32、33]を使用して実施した。変化係数(CV)をまず、各解析物に対して計算し、ランク付けした。全ての試料にわたり、最も低いCV−値を表示した15パーセントの解析物を同定し、24個の解析物が相当し、各試料に対して、チップ間標準化因子を計算するために使用した。標準化因子Niを、式Ni=Si/μによって計算し、式中Siは各試料に対して、24個の解析物に対するシグナル強度の合計であり、iは、全ての試料にわたり平均化した、24個の解析物に対するシグナル強度の合計である。1つの試料から生成した各データ組を、標準化因子Niで割った。シグナル強度のLog2値をさらなる解析のために使用した。
80個の患者試料を、tPSAと%fPSAの値に基づいて、4つの群(n=20)に分けた。初期分類解析のために、各4つのリスク群を、トレーニング組(n=15)および試験組(n=5)に分けた。試料を分類するために、本発明者らは、サポートベクターマシン(SVM)、Rにおける教師あり学習法を使用した[43]。教師付き分類を、直線カーネルを使用して実施し、制約のコストを1に設定し、R機能SVMにおけるデフォルト値であり、オーバーフィッティングを避けるために、それを調節するための試みは実施しなかった。SVMモデルを、トレーニング組を使用してトレーニングし、ついで凍結し、試験組に適用した。データのフィルター処理は、SVMのトレーニングの前に実施せず、すなわちアレイ上の全ての抗体からのデータを、解析に含めた。さらに、受信者動作特性(ROC)曲線を、SVM決定値を使用して構築し、曲線下面積(AUC)を計算した。Cluster and Treeveiw[44]中の教師なし階層的クラスタリングを使用して、C群を、C1(n=10)とC2(n=10)と記される2つの亜群に分けることができた。より小さな試料数のために、トレーニングと試験組への分離は、この場合不可能であり、SVMをしたがって、リーブ・ワン・アウト交差検証手順を用いてトレーニングした。有意にアップまたはダウンレギュレートされた血漿タンパク質(p<0.05)を、相対タンパク質レベルに基づいて定義し、ウイルコクソンの符号付き検定を使用して同定した。試料を、主要成分解析(PC)ソフトウェアプログラム(Qlucore Omics Explorer、Lund、Sweden)および/またはCluster and Treeviewを使用して可視化した。
抗体マイクロアレイ結果を検証することの試みにおいて、ヒトTh1/Th2 10−plex MSA(Meso Scale Discovery、Gaithersburg、MD、USA)アッセイを、全ての80個のEDTA−血漿試料に対して実行した。MSD 96−プレートの各ウェルを、空間的に異なる電極スポットにて、IFN−a、IL−1a、IL−2、IL−4、IL−5、IL−8、IL−10、IL−12p70、IL−13およびTNF−aに対する抗体で、前機能化した。アッセイを、製造業者によって提供されたプロトコルにしたがって実行し、電気化学発光に基づくリードアウトを、MSD SECTOR(登録商標)器具にて実施した。検出の制限を、標準曲線中のゼロポイントにわたる、標準偏差の2.5倍シグナルとして定義した。
本研究において、本発明者らは、本発明者らの組織内で開発した組換え抗体マイクロアッセイを使用して、PCを有する種々のレベルのリスクでの、80人の患者からの非画分化、ビオチン化EDTA−血漿試料の、タンパク質発現プロファイリングを実施した。試料を推論的に、異なるカテゴリーのPC診断または結果のリスクを反映している、tPSAおよび%fPSAに基づいた、4つのリスク群に分けた(表3)。
代表的マイクロアレイイメージを、図1Aにて示し、これは、同質スポット形態学、高シグナル対ノイズ比、および動的シグナル強度を得たことを示している。全ての80個の試料を首尾良くプロファイルし、したがって、全ての80個の試料からのアレイデータを、続く統計学的解析に使用可能であった。アッセイ内再現性を、スポット対スポット検証を解析することによって査定し、0.95の決定の平均係数(R2)をもたらした(図1B)。アッセイ内再現性、すなわちアレイ対アレイ変化を、独立したアレイ上の単一試料を解析することによって評価し、0.96のR2−値が得られた(図1C)。
まず、本発明者らは、A〜Dと記した4つの前立腺がんリスク群の、焦点をあてた(62個の解析物)血漿プロテオームプロファイルを決定し(表3)、群Aは最も低いリスクを提示し、群Dは最も高いリスクを提示する。ウイルコクソンの符号付き検定を使用して、3〜44個の示差的に発現した(p<0.05)血漿解析物を同定し、図2A中のヒートマップとして示す。データは、3つの解析物のみが、2つの最も低いリスク群AおよびBの間で示差的に発現したことを示した。したがって、臨床的観点から推定可能であるように、データは、2つの最も低いリスク群間で小さな差違のみを暗示した。対照的に、37個、44個および30個の脱制御解析物が、最も高いリスク群Dに対して、群A、BまたはCに関して観察され、これは、大きい(より大きい)差にて示唆している。より詳細には、種々の補体タンパク質(例えばC3、C4、C1q、ファクターBおよびプロペルジン)が、高リスク群Dにてダウンレギュレートされることが見い出され、一方でアップレギュレートされた解析物は、TH1(例えばIL−2、IL−3およびINF−a)とTH2(例えばIL−4、IL−10)サイトカイン両方の複雑なパターンを提示した。最も重要なことに、群A(非常に低いPC−リスク)対群D(PCの臨床的に意義のあるまたは進行したステージの非常に高いリスク)を区別しているバイオマーカーサインが、PCを特定している悪性バイオマーカーサインとして見ることができた。
Cリスク群をさらに層別化可能であったかどうか調査するために、本発明者らは、フィルター処理なしのデータに基づいて、すなわち、アレイ上に含まれたすべての抗体からのデータを使用して、教師なし階層的クラスタリングを実施した。結果は、Cリスク群が実際に、C1およびC2と記す、2つの異なる亜群に層別化可能であったことを示した(図3A)。同様に、主要要素解析(PCA)を使用して、Cコホートの明確な亜分割も観察された(図3B)。さらに、49個の有意に示差的に(p<0.05)発現した血漿解析物を、C1対C2に対して観察した(図3C)。より詳細には、3つの補体タンパク質が、C1にてアップレギュレートされ(C1q、ファクターBおよびC4)、一方でC1において、示差的に発現したタンパク質がダウンレギュレートされたままである。解析物の後者群には、多数のサイトカイン(例えばIL−6およびIL−4)、補体タンパク質(例えばC1−INHおよびC1s)、ならびに細胞表面タンパク質(例えばICAM、HLA−DR、ムチン−1およびCD−40)が含まれた。合わせると、データは、異質リスク群Cが、多数の脱制御解析物にて、2つの異なる亜群、C1およびC2に層別化可能であったことを示した。
リスク群Cの層別化された再分割の生物学的インパクトを査定するために、亜群C1およびC2のタンパク質発現プロファイルを、他の3つの本来のリスク群A、BおよびDのものと比較した。C1の場合、結果は、C1が、リスク群Dとよく区別される(AUC=0.82)が、リスク群AおよびBからは区別されない(両方の場合でAUC=0.5)ことを示した(図4A)。さらに、47個の示差的に発現した解析物が、C1対Dに対して観察されたが、C1対AおよびBではそれぞれ、16個および15個のみであった(図4B)。前者の場合、アップレギュレートされた補体タンパク質(例えばC1q、C3およびファクターB)と、ダウンレギュレートされたサイトカイン(例えばTGF−a1、IL−1ra、IL−6およびMCP−1)および細胞表面マーカー(例えばICAM、CD40およびルイスX)のパターンが、C1対Dにて観察された。
血液血漿は最小限に侵襲的であり、PCの早期検出とリスク分離のためのバイオマーカーの理想的な供給源であろう臨床的によく確立された試料フォーマットである。その結果として、多数の大規模プロテオミクス効果において、本目的のために査定されてもきた[19、20]。しかしながら、タンパク質数およびダイナミックレンジに関して、血漿試料の固有の複雑性により、非画分化試料の古典的プロテオミクス解析は、PCを分類するために有用でありそうにないということが議論されてきた[21]。前画分化が、試料の複雑性を減少させるけれども、同時に、湾曲されたタンパク質収率/回収、ならびに再現性および感度に関する重大な問題に関連する[22、23]。この文脈において、本発明者らは最近、組換え抗体マイクロアレイによって表される、アフィニティプロテオミクスが、未精製プロテオーム中の、高くおよび低く不足している解析物をプロファイルするために使用可能であることを示した[26、27]。先に、単一血清マーカートロンボスポンジン−1(TSP−1)が、抗体マイクロアレイを使用して、良性および悪性前立腺疾患間を区別することが可能であることを示した[45]。本概念実証試験において、本発明者らは、PCのリスク群に関連した、第1の多重化候補血漿タンパク質バイオマーカーサインを解釈するために、未精製血漿試料を標的としているアフィニティプロテオミクスを使用した。したがって、PCの組換え抗体マイクロアレイに基づく解析が、次世代のPC−関連バイオマーカーを定義することに対する、潜在的経路を実証した。

- 工程(b)は、表1に定義したバイオマーカーの全ての試験試料中の発現を測定することを含むか、またはからなる、請求項1〜5のいずれか1項に記載の方法。
- 工程(b)は、表1に定義したバイオマーカーの全ての、および、表2に定義したバイオマーカーの全ての試験試料中の発現を測定することを含むか、またはからなる、請求項1〜6のいずれか1項に記載の方法。
- 工程(b)は、抗体またはその抗原結合断片を含むか、またはからなる、結合剤を使用して実行される、請求項1〜7のいずれか1項に記載の方法。
- 工程(b)、(d)および/または工程(f)は、アレイを使用して実行される、請求項1〜8のいずれか1項に記載の方法。
- (i)試料中に存在するバイオマーカーをビオチンで標識する工程;
(ii)ビオチン標識タンパク質を、その表面上の個別の位置で固定化された複数のscFvを含むアレイと接触させる工程であって、該scFvが、表1中のタンパク質の1つまたはそれ以上に対して特異性を有する、工程;
(iii)固定化scFvを、蛍光色素を含むストレプトアビジンコンジュゲートと接触させる工程;および
(iv)アレイ表面上の別個の位置での色素の存在を検出する工程
を含み、アレイ表面上の色素の発現が、試料中の表1からのバイオマーカーの発現の指標である、請求項1〜9のいずれか1項に記載の方法。 - 工程(b)は、4個またはそれ以上のバイオマーカーをコードしている核酸分子の発現を測定することを含む、請求項1〜10のいずれか1項に記載の方法。
- 工程(b)は、それぞれ、核酸分子を含むか、またはからなる、4つまたはそれ以上の結合部分を使用して実行される、請求項11に記載の方法。
- 工程(b)において用意される試料は、非画分化血液、血漿、血清、組織液、前立腺組織、前立腺液、胆汁および尿からなる群から選択される、請求項1〜12のいずれか1項に記載の方法。
- A)請求項8において定義された4つまたはそれ以上の結合剤、および/または、請求項12において定義された4つまたはそれ以上の結合部分;
B)請求項1〜13のいずれか1項に記載の方法を実行するための説明書
を含む、前立腺がんの存在を決定するためのキット。
