Description
〔1〕ヒトから採取した生体試料中のMAIT細胞について、(1)全T細胞に対するMAIT細胞の存在割合、又は(2)CD4、CD8、CCR4、CCR7、CXCR1、NKp80、CD150、CD107a、CD8β、CD44、及びCXCR4からなる群より選ばれる1以上の表面抗原の発現量を測定することを含む、線維筋痛症の判定のための方法。
〔2〕線維筋痛症患者と、健常人、脊椎関節炎患者、及び/又は関節リウマチ患者とを鑑別するための方法である、〔1〕の方法。
〔3〕前記表面抗原が、CD4、CD8、CCR4、CCR7、CXCR1、NKp80、CD150、CD107a及びCD8βからなる群より選ばれる、〔2〕の方法。
〔4〕前記MAIT細胞のうち、CD8陽性(CD8+)細胞について、CCR7、NKp80、CD150及びCD8βからなる群より選ばれる1以上の表面抗原の発現量が測定される、〔2〕の方法。
〔5〕前記MAIT細胞のうち、CD4及びCD8二重陰性(CD4−CD8−すなわちDN)細胞について、CCR4、CXCR1及びCD107aからなる群より選ばれる1以上の表面抗原の発現量が測定される、〔2〕の方法。
〔6〕前記表面抗原が、CD44若しくはCXCR1、又はその両方である、〔2〕の方法。
〔7〕前記MAIT細胞のうち、DN MAIT細胞について、CXCR1の発現量が測定される、〔2〕の方法。
〔8〕前記表面抗原がCXCR4である、〔2〕の方法。
〔9〕以下(a)又は(b)の方法である、〔2〕の方法:
(a)全T細胞に対するMAIT細胞の存在割合を測定することによる、線維筋痛症患者の疼痛の程度を判定するための方法;又は
(b)全T細胞に対するCD4陽性(CD4+)MAIT細胞の存在割合を測定することによる、線維筋痛症患者を健常人から鑑別するための方法。
〔10〕生体試料が末梢血である、〔1〕〜〔9〕のいずれかの方法。
〔11〕前記発現量がタンパク質量である、〔1〕〜〔10〕のいずれかの方法。
〔12〕表面抗原タンパク質に対する抗体を用いて、前記表面抗原のタンパク質量が測定される、〔11〕の方法。
〔13〕ヒトから採取した生体試料中のMAIT細胞について、(1)全T細胞に対するMAIT細胞の存在割合、又は(2)CD4、CD8、CCR4、CCR7、CXCR1、NKp80、CD150、CD107a、CD8β、CD44、及びCXCR4からなる群より選ばれる1以上の表面抗原の測定手段を含む、線維筋痛症の診断キット。
〔14〕前記測定手段が抗体である、〔13〕の診断キット。
例えば、全MAIT細胞(CD4+, CD8+,DN細胞)について、FMS患者は、健常人に比し、CCR4、CCR7、CXCR1、NKp80、CD150、及びCD8βからなる群より選ばれる1以上の表面抗原の発現量が有意に増加又は減少する。したがって、本発明の方法によれば、上記表面抗原の発現量が基準値以上又は基準値以下のときに被験体は健常である可能性があり、上記表面抗原の発現量が基準値未満又は基準値を超えるときに被験体はFMSに罹患している可能性があると判定することができる。
また、CD8+ MAIT細胞について、FMS患者は、健常人に比し、CCR7、NKp80、CD150及びCD8βからなる群より選ばれる1以上の表面抗原の発現量が有意に増加又は減少する。したがって、本発明の方法によれば、上記表面抗原の発現量が基準値以上又は基準値以下のときに被験体は健常である可能性があり、上記表面抗原の発現量が基準値未満又は基準値を超えるときに被験体はFMSに罹患している可能性があると判定することができる。
さらに、CD4−CD8− MAIT細胞について、FMS患者は、健常人に比し、CCR4、CXCR1及びCD107aからなる群より選ばれる1以上の表面抗原の発現量が有意に減少する。したがって、本発明の方法によれば、上記表面抗原の発現量が基準値以上のときに被験体は健常である可能性があり、上記表面抗原の発現量が基準値未満のときに被験体はFMSに罹患している可能性があると判定することができる。
このような基準値としては、FMS患者及び健常人を鑑別し得るように適宜設定されたカットオフ値を利用することができる。
例えば、全MAIT細胞(CD4+, CD8+,DN細胞)について、FMS患者は、RA患者に比し、CD44の発現量が有意に減少する。したがって、本発明の方法によれば、CD44の発現量が基準値以上のときに被験体はFMSではなくRAに罹患している可能性があり、CD44の発現量が基準値未満のときに被験体はRAではなくFMSに罹患している可能性があると判定することができる。
また、DN(CD4−CD8−) MAIT細胞について、FMS患者は、RA患者に比し、CXCR1の発現量が有意に減少する。したがって、本発明の方法によれば、CXCR1の発現量が基準値以上のときに被験体はFMSではなくRAに罹患している可能性があり、CXCR1の発現量が基準値未満のときに被験体はRAではなくFMSに罹患している可能性があると判定することができる。
このような基準値としては、FMS患者及びRA患者を鑑別し得るように適宜設定されたカットオフ値を利用することができる。
例えば、全MAIT細胞、及びDN(CD4−CD8−) MAIT細胞について、FMS患者は、SpA患者に比し、CXCR4の発現量が有意に減少する。したがって、本発明の方法によれば、CXCR4の発現量が基準値以上のときに被験体はFMSではなくSpAに罹患している可能性があり、CXCR4の発現量が基準値未満のときに被験体はSpAではなくFMSに罹患している可能性があると判定することができる。このような基準値としては、FMS患者及びSpA患者を鑑別し得るように適宜設定されたカットオフ値を利用することができる。
例えば、MAIT細胞(CD4+/−CD8+/−細胞)について、FMS患者は、健常人に比し、CCR4、CCR7、CXCR1、NKp80、CD150、及びCD8βからなる群より選ばれる1以上の表面抗原の発現量が有意に増加又は減少する。したがって、本発明の診断キットは、MAIT細胞マーカー(例、Vα7.2、Jα33、CD161)の測定手段、及びCCR4、CCR7、CXCR1、NKp80、CD150、及びCD8βからなる群より選ばれる1以上の表面抗原の測定手段を含んでいてもよい。
また、CD8+ MAIT細胞について、FMS患者は、健常人に比し、CCR7、NKp80、CD150及びCD8βからなる群より選ばれる1以上の表面抗原の発現量が有意に増加又は減少する。したがって、本発明の診断キットは、MAIT細胞マーカー(例、Vα7.2、Jα33、CD161)の測定手段、CD8の測定手段、並びにCCR7、NKp80、CD150及びCD8βからなる群より選ばれる1以上の表面抗原の測定手段を含んでいてもよい。
さらに、CD4−CD8− MAIT細胞について、FMS患者は、健常人に比し、CCR4、CXCR1及びCD107aからなる群より選ばれる1以上の表面抗原の発現量が有意に減少する。したがって、本発明の診断キットは、MAIT細胞マーカー(例、Vα7.2、Jα33、CD161)の測定手段、CD4及びCD8の測定手段、並びにCCR4、CXCR1及びCD107aからなる群より選ばれる1以上の表面抗原の測定手段を含んでいてもよい。

例えば、DN MAIT細胞(CD4−CD8−)について、FMS患者は、SpA患者に比し、CXCR4の発現量が有意に低下している。したがって、本発明の診断キットは、MAIT細胞マーカー(例、Vα7.2、Jα33、CD161)の測定手段、及びCXCR4の測定手段を含んでいてもよい。

1−1)患者
被験体は、26人のFMS患者、21人のRA患者、及び37人のSpA患者、並びに16人のHDから構成されていた。患者の特徴を表1に要約する。全てのFMS患者は、ACR基準[Hauser Wら(2012) Reumatismo 64:194〜205;及びScott DLら(2010) Lancet 376:1094〜1108]を満たしていた。HIV、糖尿病、末梢神経障害、脱髄障害(例、多発性硬化症)、及び炎症性リウマチ疾患(例、RA、SpA、及びリウマチ性多発筋痛)等の疾患を併発している患者は、FMS群から除いた。RAは、1987年のACR基準[Scott DLら(2010) Lancet 376:1094〜1108]にしたがい診断した。全てのSpA患者は、欧州脊椎関節炎スタディ・グループによる標準セット及び/又はニューヨーク基準により修正された標準セット[Dougados Mら(2011) Lancet 377: 2127〜2137]を満たした。全てのRA、SpA、及びFMS患者は、一次線維筋痛症でありかつ如何なる生物学的治療(例、抗TNF−α又は抗IL−6モノクローナル抗体)も受けていなかった。いずれのSpA患者も、HLA−B27を保有していなかった。
研究機関審査委員会又は倫理委員会の承認(北海道大学大学院医学研究科及び時計台記念病院)並びに書面による患者の同意書を、ヘルシンキ宣言にしたがい研究参加前に得た。
FMS、RA、SpA、及びHDの末梢血単核細胞(PBMC)を、フィコール勾配を用いて調製し、605nmフィルターを備えたMACSQuant(Miltenyi,ドイツ)を用いて、既報[Wakao Hら(2013) Cell Stem Cell 12:546〜558]で報告したとおり、8色蛍光活性化細胞選別(FACS)解析に供した。細胞表面抗原の発現については、ブリリアントバイオレット421標識CD3+、アロフィコシアニン(APC)標識CD161+、及びPE/Cy7又はフルオレセインイソチオシアネート標識(FITC)抗Vα7.2(3C10)+のサブセットにおいて、示したフィコエリスリン(PE)標識抗ヒト抗体を用いて解析した。反応混合物は、ブリリアントバイオレット605標識CD4、APC/Cy7標識CD8、及びFITC又はPE/Cy7標識CD45ROを含んでいた。用いたPE標識細胞表面抗原の完全なリストを、表2A及びBに提示する。9人のFMS患者においてサンプル調製前に、薬物服用を最低48時間中断した。
GraphPad Prism(ver.6)を用いて、FACSデータの統計学的解析を行い、細胞表面抗原の発現強度の有意差を、ノンパラメトリック・マン−ホイットニーU検定、クラスカル・ワリス検定及びウィルコクソン一致対符号順位(Wilcoxon matched−pairs signed rank)検定で評価した。必要に応じて、ダンの補正(Dunn’s multicomponent)により、P値を補正した。P値<0.05である場合、有意差を示すものとした。
FMSが広範な疼痛、疲労、及び心的苦痛を特徴とすることを考慮すると、炎症性サイトカインは、神経内分泌異常を誘発し、最終的にこれらの症状につながる役割を果たすと考えられる。幾つかの報告により、FMSにおける炎症性サイトカインの異常な発現が観察されている[Pernambuco APら(2013) Clin Exp Rheumatol 31:S60〜S63;Kadetoff Dら(2012) J Neuroimmunol 242:33〜38;及びSturgill Jら(2014) J Immunol Res 2014:938576]。しかし、サイトカインの供給源は同定されておらず、バイオマーカーとしてのそれらの潜在的有用性は疑問である。そこで、本発明者らは、多くの炎症性サイトカイン及びケモカインを産生するMAIT細胞[Wakao Hら(2013) Cell Stem Cell 12:546〜558]を解析した。FMS患者由来のMAIT細胞の代表的なFACSプロフィール及び付随する細胞表面抗原発現(NKG2D,NK受容体)を図1Aに示す。次いで、本発明者らは、本疾患における全MAIT細胞(CD3+細胞の範囲内においてVα7.2+CD161high細胞として規定される)の百分率を比較したところ、それらはHD、FMS、RA、及びSpAにおいてそれぞれ中央値(25パーセンタイル;及び75パーセンタイル):2.9%(0.9;4.7)、1.5%(0.8;2.8)、0.9%(0.4;2.4)、1.6%(0.6;2.9)を示すことが見出された(図1B及び表3)。しかしながら、これらの疾患間で如何なるMAIT細胞集団において統計学的有意差は見出されなかった。MAIT細胞は主にCD8+細胞及び二重陰性(DN)細胞、並びに僅かなCD4+細胞から構成される[Le Bourhis Lら(2011) Trends Immunol 32:212〜218]ことから、各サブセットをさらに解析した。クラスカル・ワリス検定後、CD4+ MAIT細胞並びにDN MAIT細胞において、MAIT細胞頻度の差異が認められた(表4)。P値の補正によってHDとSpAとの間でのDN MAIT細胞の頻度差異の存在、及びHDとFMSとの間でのCD4+ MAIT細胞の頻度の差異の存在が明らかになった(図1D〜E及び表3及び4)。全MAIT細胞(Vα7.2+CD161high細胞)におけるCD8+ MAIT細胞、DN MAIT細胞、及びCD4+ MAIT細胞の存在割合を解析したところ、SpAではHDに比較して、CD8+ MAIT細胞の有意な増加と共にDN MAIT細胞の減少が観察された(図1F〜G)。これは、全MAIT細胞中のCD8+ MAIT細胞の存在割合の増加(DN MAIT細胞の減少との均衡作用である可能性が最も高い)がSpAを特徴付けるものであり、SpA鑑別診断のマーカーとして利用できる。CD4+ MAIT細胞が僅かにしか存在しないことから、以下の実験では全MAIT細胞、CD8+ MAIT細胞及びDN MAIT細胞を使用して研究を行った。

- 前記発現量がタンパク質量である、請求項1〜10のいずれか一項記載の方法。
- 表面抗原タンパク質に対する抗体を用いて、前記表面抗原のタンパク質量が測定される、請求項11記載の方法。
- ヒトから採取した生体試料中のMAIT細胞について、(1)全T細胞に対するMAIT細胞の存在割合、又は(2)CD4、CD8、CCR4、CCR7、CXCR1、NKp80、CD150、CD107a、CD8β、CD44、及びCXCR4からなる群より選ばれる1以上の表面抗原の測定手段を含む、線維筋痛症の診断キット。
- 前記測定手段が抗体である、請求項13記載の診断キット。
