Start of the New Millennium.The burden of musculoskeletal conditions at the start of the new millennium:Report of a scientific group.Geneva,Switzerland:World Health Organization technical report series 919;2003;27頁]定義された骨粗鬆症または骨減少症などの状態に関連することが理解される。運営上は、骨粗鬆症は、これら当局によって、若年成人女性の平均BMDよりも2.5標準偏差未満だけ低い骨塩密度(BMD)(BMD T−スコア<−2.5)として定義される。骨減少症は、運営上は、若年成人女性の平均BMDより下のBMD1から2.5標準偏差(−2.5<BMD T−スコア>−1.0)として定義される。臨床的には、骨粗鬆症は通常は、手首、椎骨、および股関節部への特徴的な低度の外傷性骨折によって認識される。加えて、ここでは「骨量減少障害」はまた、BMD以外の物理的な骨の特徴が有意な骨量減少が生じたことを示す状態のことも指す。例えば、骨塩量の減少、骨体積率の減少、骨梁幅および骨梁数の減少、骨梁間隔の増加、皮質髄面積の増加、または皮質骨面積の減少はすべて、骨減少症または骨粗鬆症への進行を示す。また、ここで用語「骨量減少障害」を用いる場合は、これら骨量減少の指標のいずれかによって骨質の低下をもたらすいかなる病理をも指すことは知られている。

24匹の若い雌BALB/cマウスをJackson Laboratories(The Jackson Laboratory,Bar Harbor,ME)から入手した。マウスを2つの同等のコホートに分け、一方のコホートのマウスを10週齢で卵巣切除した。各コホートをさらに、6匹の卵巣切除(OVX)マウスよりなる2つの群と、6匹の繁殖可能マウスよりなる2つの群とに細分した。OVXの1つの群と繁殖可能マウスの1つの群には胃管栄養法によりPEGを与えた(2g/kg体重のHMW PEGを週に3x)。ここで報告した作業の過程を通して、HMW PEG製剤はMDY−1001であり、これは平均15K MWの10%のHMW PEG、および2%までのより高分子量のPEG、残りの8K MWのPEGよりなる。
マイクロコンピュータ断層撮影(μCT)を介して骨構造を分析した。大腿(10%の緩衝ホルマリンに24時間固定し次に70%のエタノール中に保持した)を、GE Explore Locus μCTシステムを用いて、720個のビューから得られるボクセル解像度20μmでスキャンした。増分のビーム角は0.5、ビーム強度は80ピークkVおよび450μAに設定した。各スキャンの積分時間は2000msであった。各試行は、各群からの骨および較正用ファントムより構成して、グレースケール値を標準化して一貫性を維持した。多数の骨試料の自己閾値(autothreshold)および等密度面分析(isosurface analysis)に基づいて、固定閾値(835)を用いて骨髄から骨を分離した。オリジナルおよびセグメント化した画像スライスの比較によって精度を検証した。骨分析は骨の状態に対して盲検的に行った。骨梁骨分析は、外部皮質骨を除く骨幹方向に1.5mm(骨長の10%)延びる大腿近位部の成長板より〜0.15mm(全長の〜1%)遠位で画定される骨梁骨(trabecular bone)の領域で行った。骨梁骨塩量、骨塩密度、骨体積率、厚さ、間隔、および数の値を、容積画像データの可視化および分析のためのGE Healthcare MicroViewソフトウェアアプリケーションによって計算した。皮質測定を、1430の閾値を用いて骨を髄質から分離して骨の長さの中間を中心として2x2x2mmの立方体で行った。
骨梁骨塩量(BMC)は、骨梁骨のある領域にわたって存在する骨塩の量を表す。BMCは、両卵巣を除去した女性で減少することが知られており、これは図2に示すOVXマウスモデルに反映されている。OVX試料群の対照群との比較により、BMCにほぼ62%の統計的に有意な(p=0.0265)平均減少が示される。これは、IBDのDSS誘発マウスモデルにおいてHarrisらにより報告されたBMCの低下の程度(57%)に酷似する[Harrisら、(2008)、表2、欄1、1行参照]。
骨梁骨塩密度(BMD)含量は、所定の体積の骨梁骨に存在する骨塩の量を表す。両卵巣を除去した女性はBMDが低いことが知られており、これはまた図3のOVXマウスモデルで示される。OVX試料群の対照群との比較により、ちょうど62%を超える統計的に有意な(p=0.0012)平均減少が示される。これは、IBSのDSS誘発マウスモデルにおいてHarrisらにより報告されたBMDの低下の程度(59%)と一致する[Harrisら、(2008)、表2、欄1、2行参照]。
PEGによる処置の結果、未処置対照の約115%の骨塩密度となった。OVXマウスの向上は統計的に有意(p=0.0005)であるだけではなく、より高くもあり、HMW PEG処置の結果、未処置OVX群に対してBMDで131%の向上であった。HMW PEG処置OVXマウスのBMDは、平均で、未処置繁殖可能マウスの約82%であった。従って、HMW PEGによる処置は、閉経後骨粗鬆症を患う動物においてBMDを有意に増大させる。
皮質骨塩量(CBMC)は、皮質骨の所定の領域にわたって存在する骨塩の量の尺度である。図4は、CMBCに関して対照群平均とOVX群平均との間に統計的なまたは明白な差異がほとんどないことを示すが、どちらの群のHMW PEG処置の結果もCBMCが増加している。対照群および対照+PEG群の場合には、HMW PEG処置の結果、対照に対して約115%のCBMCをもたらした。しかしこの値は信頼性の統計的閾値をちょうど越えた値である(p=0.0593)。CBMCの類似のおよび統計的にもっと堅調な向上が、HMW PEG処置したOVX群をOVXマウスと比較すると見られる。この場合は、HMW PEG処置の結果、OVX群に対して約118%のCBMCをもたらした(p=0.0022)。従って、HMW PEGによる処置はCBMCを向上させる。
皮質骨塩密度(CBMD)は、皮質骨の所定の領域にわたって存在する骨塩の量の尺度である。
骨状態の変化の最も信頼できる指標は恐らく骨体積率(BV/TV)である。これは試料の単位体積当たりの鉱化した骨の量である。
骨梁骨内の個々の柱(trab)は、比較的狭い境界内で大きさおよび長さが変動する。一般に、個々の柱が厚いほど、骨は強い。従って、平均骨梁幅(Tb.Th.)の増加は骨状態の向上を表す。
骨梁骨の健康の別の尺度は骨梁数Tb.N.であり、これは所定の面積内の柱の数を反映する。個々の柱の数が多いほど、骨は強い。従って、Tb.N.の増加は骨状態の向上を表す。
骨梁間隔(Tb.Sp)は、骨梁骨内の空隙の尺度である。一般に、Tb.Sp.が大きいほど骨は弱い。従って、ここで議論される他の骨梁骨状態の尺度とは異なり、Tp.Sp.の減少が骨状態の向上を表す。
皮質髄面積(CMA)は、髄質を含む皮質骨内の面積の尺度である。Tb.Sp.と同様に、CMAが大きいほど骨は弱い。従って、CMAの減少が骨状態の向上を表す。
皮質面積(CA)は、皮質骨の断面内の骨面積の尺度である。一般に、CAが大きいほど骨は強い。従って、CAの増加が骨状態の向上を表す。
DSSおよびOVX+DSS処置マウスの血清オステオカルシンに対するHMW PEGの効果
HMW PEG処置の有益な効果が骨吸収の阻止によるものか骨形成の誘発によるものかどうかを決定するために、異なる基礎状態から生じる骨粗鬆症を患う処置および未処置マウスにおいて血清オステオカルシンのレベルを決定した。
Jackson Laboratory,Bar Harbor,ME)から入手した。マウスを2つの同等のコホートに分け、一方のコホートのマウスを10週齢で卵巣切除した。各コホートをさらに4匹の卵巣切除(OVX)マウスよりなる2つの群と、4匹の繁殖可能マウスよりなる2つの群とに細分した。OVXの1つの群と繁殖可能マウスの1つの群には、胃管栄養法でHMW PEGを与えた(2g/kg体重のHMW PEGを週に3x)。これら動物にはまた、D5W中の1%のHMW PEGを、胃管栄養処置の間に有効量のHMW PEGを維持するために随意に与えた。他の2群には同じスケジュールで同等量の水を胃管栄養供給し、またD5Wを与えた。4群すべてに、実験の終了15日前から開始して飲料水を介して1%のDSSを与えた。この結果、4群、PEGにさらされなかった繁殖可能マウスの対照群(対照+DSS)、PEGにさらされた繁殖可能マウスの群(対照+DSS+PEG)、PEGにさらされなかったOVXマウスの群(OVX+DSS)、およびPEGにさらされたOVXマウスの群(OVX+DSS+PEG)を得た。実験を通して、動物には標準的な食餌および水を自由に与え、12時間の明/暗サイクルで保持した。すべての動物手順はMichigan State University Institutional Animal Care and Use Committeeによって承認された。
PEGによる処置により、血清オステオカルシンはHMW PEG処置した対照+DSSマウスと同じレベルまで回復した。対照+DSS群の血清オステオカルシンレベルは、PEG処置群より低いが、OVX+DSS群より高い。血清オステオカルシンがHMW PEG処置に応じて増大するという事実により、HMW PEGにより処置した被験体がより高い同化性骨形成の能力を有することが示唆される。
対照、DSS、およびOVX+DSS処置マウスの血清オステオカルシンに対するHMW PEGの効果
HMW PEG処置の血清オステオカルシンに与える有益な効果が健常なマウスにも拡張したか否かを調べるために、およびDSSおよびOVX+DSS処置マウスにおける血清オステオカルシンのHMW PEGによる増加の程度の尺度を提供するために、実施例1および実施例2の被験体からのデータを互いに対してプロットした。
健常鶏および感染鶏の飼料転換率に与えるHMW PEGの効果 典型的な生産条件の下で飼育したブロイラー鶏の成長能力に与えるHMW PEG処置の効果を試験した。480羽の鶏を無作為に、各檻に15羽で、4つの実験群に配置した。全トリを孵化の日から標準的な飼料、水、光、空気、および温度条件の下で20日間飼育した。陰性対照群のトリには特別な処置は行わなかった。0.33%MDY(1−21)群のトリには、0.33%飼料添加物として随意にHMW PEG MDY−1001を与えた。SE対照群のトリは、腸炎菌に感染し胃腸障害を引き起こしたが、他の特別な処置は行わなかった。0.33%MDY(1−21)+SE群のトリは、腸炎菌に感染し、0.33%飼料添加物として随意にHMW PEG(MDY−1001)を与えた。各群の飼料消費および体重増加を20日目で評価した。
感染鶏からの細菌の回収に対するHMW PEGの効果
腸炎菌が感染鶏内で持続する能力に対するHMW PEG処置の効果を調べた。鶏を4つの群に配し、実施例4で述べた基本条件の下で飼育した鶏の成長能力の胃腸炎症の下で飼育した。しかし、この実験では、すべてのトリを最初に腸炎菌に感染させ、トリから回収した細菌の量を処置期間後に測定した。対照群のトリには特別な処置は行わなかった。0.1%−T群のトリには0.1%飼料添加物として随意にHMW PEGを与えた。0.1%―P群のトリには0.1%飼料添加物として随意に予防薬を与えた。処置期間の終わりに各鶏の盲腸から回収したコロニー形成単位数を決定した。
非特定糞便攻撃にさらされた離乳仔ブタの累積成長能力に対するHMW PEGの効果
離乳仔ブタの成長能力に対するHMW PEG処置の効果を、2〜4および7日目(離乳後)に、各檻で飼料皿の内側に生産型養育場(1,100頭のブタ)から回収した糞便スラリーを塗りつけることによって非特定糞便攻撃にさらすことで、試験した。160頭の仔ブタを離乳時に体重を測り、各実験群で20個の複製檻に各檻に2頭ずつ配し、20日間実験食を与えその後再び体重を測った。陽性対照群は非特定糞便攻撃には供さなかったが、陰性対照群および2つの処置群は糞便攻撃に供した。処置群は、1つの群には35g/トンのDenagard(登録商標)(12.5%チアムリン水素フマレート抗生物質)を与え、1つの群には2%HMW PEGを直接飼料に混合して与えた。各群での下痢(scouring)(下痢)の頻度もまた下痢スコアを決定することによって評価した。下痢スコアとは、下痢事象の数を各実験群の全檻数で割りこれに100を掛けたものである。
Claims (14)
- 患者における閉経期、男性更年期、性腺機能低下症、骨減少症、骨粗鬆症、慢性感染症または免疫学的ストレスの処置において骨形成を増大させる、かつ/または骨吸収を減少させるための組成物であって、前記組成物は、15,000ダルトン〜20,000ダルトンの間の平均分子量を有する高分子量ポリエチレングリコール(HMW−PEG)を含む化合物を含み、前記HMW−PEGは、疎水性コアに付着した少なくとも2本の炭化水素鎖を備え、各炭化水素鎖は、前記HMW−PEG化合物の少なくとも40パーセントの平均分子量を有し、前記疎水性コアは環構造を備える、組成物。
- 患者において骨形成を増大させるための、請求項1に記載の組成物。
- 患者において骨吸収を減少させるための、請求項1に記載の組成物。
- 患者において同時に骨形成を増大させ、かつ骨吸収を減少させるための、請求項1に記載の組成物。
- 患者において外傷性骨折の治癒を促進するための、請求項1に記載の組成物。
- 患者において外傷性骨折の発生を予防するための、請求項1に記載の組成物。
- 患者における閉経期、男性更年期、性腺機能低下症、骨減少症、骨粗鬆症、慢性感染症または免疫学的ストレスの処置において骨形成を増大させる、かつ/または骨吸収を減少させるための、医薬を含むキットであって、前記医薬は、治療上有効用量の15,000ダルトン〜20,000ダルトンの間の平均分子量を有するHMW−PEGを含み、前記HMW−PEGは、疎水性コアに付着した少なくとも2本の炭化水素鎖を備え、各炭化水素鎖は、前記HMW−PEG化合物の少なくとも40パーセントの平均分子量を有し、前記疎水性コアは環構造を備える、キット。
- 患者における閉経期、男性更年期、性腺機能低下症、骨減少症、骨粗鬆症、慢性感染症または免疫学的ストレスの予防的処置のための、請求項7に記載のキット。
- 患者において骨形成を増大させるための、請求項7に記載のキット。
