炎症性腸疾患およびがんを含む種々の疾患の処置のための、患者に経口投与できる改変された片利共生細菌。
炎症性腸疾患(IBD)の病原性の顕著な特徴は、リンパ球、マクロファージ、上皮細胞および樹状細胞を含む、腸細胞によるサイトカイン産生の調節不全である。これらの細胞型の各々が、炎症促進性サイトカインの産生を引き起こす不適切なNF-κB活性化を示す。活性化NF-κBの量は腸炎の重症度に対応し、IBD処置における大きな進展は、恐らくNF-κBの活性化をもたらすシグナル伝達の多くを制限することによる、TNFαに対する抗体の使用であったという証拠が存在する。この治療介入は、IBDを有する患者の症状、疾患経過および生活の質を劇的に改善させた(1)。しかしながら、これらの薬剤の使用に関連した全身免疫抑制は、潜伏結核の再活性化、脳膿瘍および播種性真菌疾患の発生などの重篤な感染性疾患の発生に対するこれらの患者の感受性を大いに高める。本明細書において記述されるこの方法および組成物は、IBDを処置するための微生物に基づく手法を提供し、これによって免疫抑制が疾患の主要部位、つまり腸に限定される。具体的には、NF-κB活性化を阻害する細菌タンパク質を直接的に腸細胞へ送達できる片利共生の大腸菌株の作出およびこれらの菌株が自発性大腸炎の実験モデルにおいて腸炎を、少なくとも一時的に、抑制するように作用しうるかどうかの試験が本明細書において記述される。本明細書において記述される方法および組成物は、全身免疫抑制に関連した問題を回避しうるIBD処置のための強力な新しい治療パラダイムである。
先天性免疫システムは、感染の始まりをシグナル伝達する、外来分子を認識する際の宿主の防御の最前線として働く監視システムである。この応答は、宿主がリポ多糖類およびペプチドグリカンのような微生物に関連した分子パターン(MAMP)を感知する場合にトリガーされる。これらのMAMPは、TLR (Toll様受容体)およびNOD受容体により認識され、これが今度は、サイトカインの発現をもたらす下流のシグナル伝達カスケードを活性化する。これらのサイトカインは、感染部位への好中球およびマクロファージの動員を含めて炎症応答を樹立するように作用する。先天性免疫応答は、体液性および細胞性免疫システムの活性化を引き起こす。IBDとの関連で、これらの細胞型の各々が転写因子NF-κBの不適切な活性化を示す。興味深いことに、活性化NF-κBの量は、腸炎の重症度に関連付けられている(2)。NF-κBのサブユニットの1つp65に対するアンチセンス・オリゴヌクレオチドの局所投与は、IBDの実験マウスモデルにおいて炎症を著しく減少させることが実証されている(3)。これらの所見から、NF-κBを遮断する標的治療がIBDに対する有効な処置であると判明する可能性が高いことが示唆される。
細菌性病原体は、宿主応答を抑制するためにまたは宿主応答に対抗するために多数の機構を進化させてきた。例えば、サルモネラ、シゲラ、腸管病原性大腸菌およびエルシニアの種を含む多くのグラム陰性腸内病原菌は、数十種のタンパク質(エフェクター)を真核生物宿主細胞へ直接的に注入するために特殊化された3型分泌システムを利用する。これらの分泌システムは、宿主細胞への細菌タンパク質の送達のためのチャネルを本質的に形成する、宿主細胞膜へ挿入する細孔形成タンパク質で覆われた針部を形成する細菌内膜および細菌外膜の両方に及ぶ20〜25種のタンパク質から構成される複雑な機構である。T3SSの構成成分は高度に保存されているが、各細菌はそれ自体の独特のエフェクターセットを宿主細胞へ注入する。興味深いことに、ここ数年で、NF-κB活性化の阻害につながるシグナル伝達経路を阻害する複数のエフェクターが特定された。表1にまとめているように、これらのエフェクターは、NF-κBの活性化に必要とされる種々の段階を標的とする。
シゲラの場合、機能的T3SSを形成するために必要とされる20〜25種のタンパク質をコードする遺伝子の全てが、病毒性プラスミド上に位置する2つの大きな隣接オペロン内にコードされる[26]。一連の相同組み換えに基づく手法を通じて[33, 34]、この31 kbの領域全体がもっと小さな、自己複製プラスミド上へ捕捉された。大腸菌実験室株(DH10B)およびBL21へのこのプラスミドの導入は、シゲラエフェクターを培地へ分泌可能な、および宿主細胞へ直接的にエフェクターを送達可能な菌株を作出するのに十分である。顕著には、3型分泌機構をコードする病毒性プラスミドのこの領域を特異的に捕捉することにより、20種超のエフェクターをコードする遺伝子が、大腸菌株へ導入されないシゲラ病毒性プラスミド上の他の場所にコードされる。したがって、これらの3型分泌コンピテント大腸菌株は、宿主細胞に侵入する数少ない生物が複製することなく、ファゴソームに閉じ込められるので、非病原性である。加えて、これらの菌株に感染したヒト細胞は、IL-8分泌を示さず、非免疫原性の送達菌株が作出されたことを示唆している。
シゲラT3SSがサルモネラ、エルシニアおよび腸管病原性大腸菌由来のエフェクターを認識かつ分泌することが、実験によって実証された。
抗炎症性エフェクターを分泌できるNISSLE 1917株を作出することができる。現在、大々的な配列決定の努力にもかかわらず、哺乳動物の微生物叢に存在する細菌種が、T3SS (または任意の他のさらに高次の細菌分泌システム、すなわち宿主細胞へ複数のタンパク質を送達できるもの)をコードすることは特定されていない。治療用分子をマウス腸へ送達するための安全かつ有効な媒体であることが以前に実証されている菌株である片利共生ヒト大腸菌NISSLE 1917株(EcN) (4)へ、シゲラT3SSを保有するプラスミドを、導入することによって新たにそのような菌株を作出することが、本明細書において記述される。表1に記述されている9種の抗炎症性エフェクターの各々を培地へ、および直接的に宿主細胞へ分泌するこの菌株の能力を、定量的免疫ブロットアッセイ法を用いることにより試験して、培地へ分泌される各エフェクターの量を、および宿主細胞へ直接的に送達された各エフェクターの量をFLAGでタグ付けされた場合、判定することができる。異種エフェクターの1つがシゲラ3型分泌システム(これはEcNへ導入されうる)によって認識されない、あり得そうにない場合には、そのN末端をシゲラエフェクターのN末端と交換することができる。
表1に記載されている9種のエフェクターの各々を分泌する3型分泌コンピテントEcN株がNF-κB活性化を阻害する能力を、2つの補完的な手法を用いて比較することができる。先天性免疫応答を抑制する際の単一エフェクターの個々の役割が研究されるのは、これが初めてである。第一に、TNFαへの曝露後の、9種の3型コンピテントEcN株を感染させた極性をもった腸(Caco-2)上皮細胞の上清中に存在するIL-8のレベルを比較することができる。感染細胞および非感染細胞の上清を24時間、回収することができ、IL-8レベルを、市販のIL-8 ELISAキット(BD Scientific)を用いて判定することができる。これらの結果は、遺伝子特異的なプライマーを用いてqPCRによりIL-8 mRNAレベルを測定することによって確認することができる。第二には、NF-κB活性化を阻害する9種の3型コンピテントEcN株の能力は、蛍ルシフェラーゼレポーターアッセイ法を用いて比較することができる。Caco-2細胞へのプラスミドの導入に関連した技術的困難を考慮すると、この場合には、高度にトランスフェクション可能なHEK293細胞が用いられうる。NF-κBルシフェラーゼおよび構成的ウミシイタケレポーター・プラスミドをHEK293細胞へ導入してから24時間後に、9種の抗炎症促進性EcN株の各々を細胞に感染させることができる。1時間後に、TNFαを培地に加えて、NF-κB活性化につながるシグナル伝達経路を誘導することができる。NF-κB活性化を阻害する各菌株の能力は、市販のDual-Glo Luciferase Reporter Assayキット(Promega)を用いて経時的にルシフェラーゼ活性をモニタリングすることにより比較することができる。ウミシイタケ・レベルを測定し、トランスフェクション効率および/または細胞生存性に関する潜在的な問題の説明とする。
上記の実験は、NF-κB活性化を阻害する単一の3型エフェクターの能力の最初の対照比較を提供しうる。しかしながら、IBDの処置のため、複数のエフェクターを宿主細胞へ送達する菌株を作出することができる。というのは、恐らく、エフェクターは相加的にまたは多分、相乗的にでさえ働いて、NF-κB活性化を阻害するからである。そのようなエフェクターを特定するため、潜在的な72種のエフェクター対の各々を宿主細胞に注入する3型コンピテントEcN株を感染させたCaco-2細胞がTNFαに曝露される場合に分泌されるIL-8のレベルを、比較することができる。これらの研究の場合、2種のエフェクターを同じEcN株へ導入するのではなく、Caco-2細胞に、実験的に判定された感染多重度(MOI)で2種の菌株の1:1混合物を感染させて、両方のエフェクターが類似のレベルで細胞へ注入されることを確実にすることができる。これらの結果および各エフェクターの作用部位に関する情報(表1)に基づき、より複雑な組み合わせのエフェクターを調べ、相乗作用について試験することができる。3,000超の潜在的な組み合わせを伴う、可能な全ての組み合わせを試験することとは対照的に合理的基準が使用される。菌株が最低の全感染多重度で感染される場合にIL-8産生の最も顕著かつ一貫した阻害をもたらす最大4種までのエフェクターの組み合わせを、特定することができる。
初期のインビトロ用途の場合、抗炎症促進性エフェクターをコードする遺伝子を、IPTG調節可能なプロモーターを介して発現される低コピー数のプラスミドにて行うことができる。しかしながら、マウス研究の場合、抗炎症性エフェクターをコードする遺伝子が、少なくとも12種のシゲラエフェクターの発現を調節する転写因子MxiEの制御下でEcN染色体上に存在するような菌株を作出することができる。8種のMxiE調節遺伝子の転写開始部位がマッピングされた(5)。この情報は、抗炎症性エフェクターの上流に配置する配列の選択を手助けし、それらのエフェクターが的確な時間で協調的に転写されることを確実にすることができる。
本明細書において上述される抗炎症促進性EcN株がIBDのマウスモデルにおいて腸炎を抑制できるかどうかを判定する実験が、本明細書において記述される。野生型EcNは、ヒトにおいて安全であることが既に確立されており、臨床試験においてIBDを有する患者に毎日投与された場合には寛解の状態の維持で5-ASA (経口抗炎症性腸薬剤)と同じくらい有効である(6)。EcNは、3日間経口的に投与された後に少なくとも2週間(7)、およびアンピシリンの単回用量とともに単回の経口用量後に少なくとも40日間(4)マウスの腸に持続的に定着できるという証拠も存在している。しかしながら、EcNはヒトまたはマウスにおいて結腸炎の再発を抑制することは未だ実証されていない。IBDを研究するための多くの動物モデルが存在する。本明細書において記述される実験においては、TRUC (T-bet-/-×Rag2-/-潰瘍性結腸炎)モデルが用いられる。これらのマウスは自発性、高浸透性、かつ伝染性の結腸炎を発症し、この結腸炎は、1) 3.5週齢までに組織学的に検出可能であり、2) ヒト潰瘍性結腸炎に似ており、かつ3) 結腸障壁機能の変化およびTNFαの上昇と関連している(8)。
「野生型」EcNおよびT3分泌コンピテント抗炎症促進性EcNが正常な腸および炎症を起こした腸に定着する能力を比較することができる。糞便中のEcNの排菌の定量化によりおよび殺処理時の組織検査により、定着を測定することができる。EcNの可視化を促進するため、eGFPを安定的に発現する各菌株の変形体(version)を作出することができる。出生後21日目に、野生型マウスおよびTRUCマウスに3日連続で経口接種材料[1〜2×109個のコロニー形成単位(CFU)]を投与することができる。次の30日の期間にわたり、個々のマウス(n=5/株)を粘膜に沿っておよび腸壁全体にわたってeGFPを発現する大腸菌の証拠がないか、毎日の糞便サンプリングによっておよび殺処理時の組織検査(接種後d7、d14、d21、d28の1マウス/菌株遺伝子型)によってともに評価することができる。糞便ペレット中の片利共生大腸菌の検出に有利に働くように、腸内細菌科(Enterobacteriaceae)を濃縮し、かつUV光への曝露による蛍光細菌の可視化および/またはqPCRによるeGFPの直接測定を促すマッコンキー培地上に、細菌をプレーティングすることができる。新鮮マウント小腸および大腸組織サンプルを用いeGFPシグナルについて組織を評価することができる。組織を固定し、包埋し、定着パターンのさらなる分析のため抗eGFP抗体で染色することもできる。腸間膜リンパ節をサンプリングし、EcN特異的プライマーを用い菌株の検出のためにqPCR分析を行うこともできる(9)。連日投与によって菌株が腸の領域に接着することが可能になるかどうかを判定することもできる。
3型コンピテントEcN株によって標的とされる細胞型を特定するために、エルシニアおよびサルモネラ3型分泌システムによって標的とされる腸細胞を特定すべく成功裏に用いられたインビボに基づくアッセイ法(10)を用いることができる。このアッセイ法においては、TEM-1の3型分泌形態を発現する細菌株が用いられる。TEM-1は、蛍光に基づくアッセイ法によって哺乳動物細胞内の活性をモニターできるβ-ラクタマーゼである。野生型マウスおよびTRUCマウスに上述のように、抗炎症促進性エフェクターのほかにIII型分泌基質として認識されるTEM-1変形体を発現する抗炎症促進性EcN株を、経口的に接種することができる。次の4週間にわたり、週1回の間隔でマウス2〜3匹を殺処理して、EcN株により標的とされる細胞を特定することができる。免疫細胞について濃縮された上皮細胞調製物および単個細胞浮遊物を、十分に確立されたプロトコルを用いて小腸および結腸から作出することができる。細胞を次いで、TEM-1活性のFRETレポーターであるCCF2-AMとともにインキュベートすることができる。3型分泌TEM-1タンパク質が細胞へ送達されるなら、CCF2-AMは、その蛍光が緑色(520 nm)から青色(450 nm)へ変化するように切断される。細胞を次に、フローサイトメトリーにより分析して、緑色(520 nm) vs. 青色(450 nm)発光スペクトルを発現するサブセットを特定することができる。不変の大腸菌株を接種されたマウスの組織由来の細胞を、陰性対照として用いることができる。
本明細書において上述される実験で判定された接種条件に基づき、3週齢TRUCマウスに「野生型」および抗炎症促進性EcN株を接種することができる。腸の炎症環境は、3つの補完的アッセイ法を用いて2週および4週後に判定することができる。第一に、炎症に対する全体的効果は、組織学に基づく腸炎症評価のために固定されパラフィン包埋された腸組織を直接検査することによって判定することができる(13)。第二に、処置マウスの炎症環境中に存在するサイトカインの特異的変化についてモニターするために、T-bet-/-×Rag2-/-マウスの遠位結腸を単離することができ(14)、Luminexプラットフォームを用い外植片上清を分析して、IL-1α、IL-1β IL-2、IL-4、IL-6、KC、TNF-α、IFNγ、IL-10、IL-12p40、IL-12p70、IL-13、IL-17A、IL-21、およびIL-23レベルを分析することができる。そして、第三に、フローサイトメトリーに基づく実験をマウスに関して実施し、2種の異なるEcN株の存在下において腸内に存在する異なる細胞型を定量化することができる。
細菌を介したMyodタンパク質送達による分化転換
外因性遺伝子強制発現は、細胞分化および脱分化の両方を指令するための有効な方法として十分に特徴付けられている。しかしながら、導入遺伝子発現は、挿入突然変異の可能性があるため治療用途に適していない。タンパク質に基づく技法は、安全な代替案となるが、しかし現行のタンパク質送達方法は、労働集約的な精製プロセス、低いタンパク質収量および非効率的な細胞内ターゲティングによってかなり制限される。そのような制限は、本明細書において記述される天然の細菌タンパク質注入システムを用いることによって克服されうる。
胚性幹(ES)細胞は、体内でありとあらゆる細胞型を生じうる胚由来の多能性細胞株である。したがって、これらの細胞は、組織形成の機構および疾患の発症の研究のための非常に貴重なツールであり、組織修復のための「補充細胞」の有望な供給源となる。しかしながら、胚由来ES細胞による研究は、とりわけ疾患モデルまたは移植可能な補充細胞としてのその使用に関して、新しい系統の派生を妨げる調節障害によりおよび「患者特異的な」組織適合細胞を得る際の難しさにより妨げられている。成体の体細胞のES様「人工多能性(iPS)細胞」への直接的な「リプログラミング」を可能にする、エキサイティングな最近の発見は、これらの障害を減じたように思われており、研究および治療用の患者特異的な多能性細胞の産生のための簡易な機構となる。iPS技術の革新的開発は、多くの新しい機会を切り開いた; 多分、最もエキサイティングな潜在的用途は、移植用の患者特異的組織の使用である。
TTSSは内部共生体において見出されるが、研究の大部分は、病原性細菌、特にシゲラ、サルモネラおよびエルシニア属の種内にコードされるこれらのシステムを理解することに焦点を向けてきた。これらの細菌は、「エフェクター」といわれる、およそ数十種のタンパク質を、直接的に宿主細胞へ送達する。エフェクターではなく哺乳動物TFを宿主細胞へ特異的に送達するインタクトで、完全に機能的なTTSSを有する非病原性の細菌株を作出するためのいくつかの手法が、本明細書において記述される。そのような菌株を作出するために、2つの基本的な手法: (1) 大部分のエフェクターをもはやコードかつ産生しないよう病原性細菌を遺伝的に操作する手法、および(2) K12またはDH5aなどの一般的な非病原性の実験室大腸菌株へ機能的TTSSを導入する手法を取ることができる。インビボおよびインビトロの両方の条件下でネズミチフス菌SPI1 TTSSおよびフレクスナー赤痢菌mxi-spa TTSSの活性化を誘導する条件が知られている。機能的サルモネラSPI1またはシゲラmxi-spa TTSSを形成させるのに必要とされる20〜25種のタンパク質をコードする遺伝子の全てが、シゲラ病毒性プラスミドおよびサルモネラ染色体上に互いに隣接して位置する2つの大きなオペロン内にコードされる。これらの分泌システムの各々は、これらのオペロンが他の細菌へ導入される場合に機能的であり、かつ発現される[7], [8]。シゲラTTSSが大腸菌において発現され、サルモネラTTSSがシゲラにおいて発現されることを考慮すると、サルモネラTTSSは大腸菌において発現され、同様に発現時に機能的である可能性が高い。
宿主細胞への細菌TTSSからの機能的な哺乳動物TFの分泌を最大化するためには、最適化される必要のある変数がいくつかある: (1) タンパク質は、III型分泌基質として認識されるように修飾される必要があり、(2) 修飾されたTFは依然として、リプログラミングを媒介できる必要があり、(3) 細菌中のこれらの哺乳動物タンパク質の発現は、最適化される必要があり、かつ(4) TFはTTSSと協調的に発現される必要がある。真核生物タンパク質を含む、多数の異種タンパク質は、細菌TTSSによって認識かつ分泌されうる[11]。それらの分泌は、そのアミノ末端への分泌シグナルの付加に依る。コンセンサスIII型分泌シグナルが存在しない場合には、これらのシグナルの基本的な構成成分が知られている[12]。全てのエフェクターは、その最初の20アミノ酸のなかに「分泌シグナル」をコードする。この配列は、場合によっては、宿主細胞へのエフェクターの送達を媒介するのに十分である。しかしながら、他の場合には、それらの分泌はIII型分泌シャペロンの存在に依る。これらの場合には、エフェクターはその最初の60〜70残基のなかにシャペロン結合ドメインをコードする。本発明者らは体系的に、25種のうちのどのシゲラエフェクターが各範疇に入るのかについて判定し、全25種のエフェクターの分泌の相対的効率について判定した[13]。種々の分泌シグナルをTFのアミノ末端に体系的に融合させることにより、どの配列が宿主細胞へのTFの最大の分泌/移行(translocation)をもたらすかを判定することができる。恐らく、最少の残基を付加する修飾は、機能を妨害する可能性が最も少ない。感染細胞の核への修飾TFの最大の送達をもたらす修飾を、スクリーニングすることができる。これらが特定されれば、修飾TFを発現する遺伝子を、レトロウイルスベクターへ導入することができ、その上、これらの変形体がMEFをiPS細胞へリプログラミングできるかを確認することができる。
リプログラミングをともに媒介するTFの1つをそれぞれが分泌する4種の細菌株が構築された後に、マウス胚線維芽細胞(MEF)のiPS細胞へのリプログラミングのための細菌「感染」アッセイ法を開発することができる。アッセイ法の基本設計は、MEFに4種の細菌株(異なる細菌株中にコードされた各TF)を感染させ、iPS細胞の作出についてスクリーニングすることでありうる。これらの実験では、MEFのiPS細胞への転換後にのみ発現されるOct4プロモーターの制御下のGFPを保有しているMEFを用い、それによって簡単な、可視的アッセイ法を提供し、iPS細胞への成功裏のリプログラミングのスコアをつけることができる[15]。下記のように、この直接的システムを体系的に最適化して、細菌タンパク質発現および宿主細胞相互作用における重要な局面に対処することにより効率を増加させる。
DH10bおよびNissle大腸菌1917の染色体における、3型分泌システム・オペロンをコードする領域の組み込み
細菌への導入時に3型分泌プラスミドを保有する大きなプラスミドの相対的不安定性を考慮して、相同組み換え技術を用いてオペロンを染色体へ導入し、不安定要因を取り除いた。具体的には、Kuhlman and Cox (Nucleic Acids Research 2010 38:e92)により開発された「ランディングパッド」技術の変法を利用した。この改変は、機能的3型分泌システムを安定的に発現する菌株を生じさせ、重要なさらなる改変をもたらす。というのは、菌株がインビボの生物剤として使われる場合には安定性が所望の状態でありうるからである。
非侵入性3型分泌コンピテント大腸菌株の開発
機能的な機構を作出するために必要とされる遺伝子を含んだシゲラプラスミドDNAの領域上に存在する4種の3型内因性分泌シゲラタンパク質は、宿主細胞の細胞質ゾルへのminT3細菌の取り込みを媒介するのに十分であることが本明細書において実証される。「MinT3」は、最小の3型分泌システムを発現する大腸菌をいう。最小の3型分泌システムの遺伝子は、染色体中におよび/または1つもしくは複数のプラスミド上に存在しうる。この知見を考慮して、いずれの内因性分泌タンパク質も、もはやコードしないminT3細菌の変形体が開発された。この大腸菌株は、細胞の外表面に結合し、ヒトへタンパク質を送達するが、しかしそれ自体は細胞に侵入しない(図1)。菌株は異なる治療的使用を有しうることが本明細書において特に企図される。非限定的な例として、侵入し、かつ細胞死を含む先天性免疫応答をトリガーする菌株は、固形腫瘍を標的とするのに有利であるが、細胞外にとどまる菌株は、例えば、リプログラミングにまたは宿主細胞への抗炎症性タンパク質の送達に有益である。
