アテローム性動脈硬化症性プラークの特性評価
大動脈狭窄のための手術または大動脈弁置換術を受けた患者から上行大動脈プラーク生検材料を得る。大動脈プラークを切片にし、Movatペンタクローム、オイルレッドO、抗CD68およびvon Kossa染色を用いて、米国心臓協会(American Heart Association:AHA)基準20、21に従ってグレード分類する。急性心筋梗塞後に死亡した患者から冠動脈を得、切片作製のためにパラフィンに包埋する。組織切片中のコラーゲンに対するMasson染色により、易損性冠動脈プラークの特徴を明らかにする。線維性キャップを、内腔と壊死性コアを隔てる高コラーゲン組織として識別する2。50μm未満の最小線維性キャップ厚を有するプラークを不安定なものとして分類し、これに対して65μmより厚い線維性キャップ厚を有するプラークを安定したものとして分類する2。
ヒト上行大動脈からの横断切片(10μm厚)および冠動脈プラークのパラフィン包埋切片(4μm厚)をスライドガラスにマウントする。CD68、フォン・ヴィレブランド因子、アルファ平滑筋アクチンまたはI型コラーゲンに対して指向された購入抗体を用いてFAPおよび細胞特異的マーカーに対して組織切片を標識し、ジアミノベンジジン用のABC染色キット(カリフォルニア州バーリンゲームのVector Labs)を使用して蛍光標識二次抗体または免疫染色用のビオチン標識二次抗体で視覚化。

95℃賦活化用バッファー(2mMクエン酸ナトリウム、pH7.6)中での20分のインキュベーション後、冠動脈プラークのパラフィン包埋切片における抗原性賦活化処理を行う。FAPの触媒インサートに対して産生されたウサギポリクローナル抗体(A246、Ab28246、マサチューセッツ州ケンブリッジのAbcam)およびウサギアイソタイプ対照(Ab37415、Abcam)を使用して、FAPを染色する。大動脈プラークの凍結切片を氷冷アセトン中で5分間固定し、FAPに対するマウスモノクローナル(F19、ニューヨーク州ニューヨークのSloan−Kettering Instituteによって提供されたもの)と適切なマウスアイソタイプ対照(401401、カリフォルニア州サンディエゴのBioLegend)を染色する。一次抗体をビオチン標識ヤギ抗マウス(115−066−003、ペンシルバニア州ウエストグローヴのJackson ImmunoResearch)およびビオチン標識ヤギ抗ウサギ(111−066−003、Jackson ImmunoResearch)で標識し、ジアミノベンジジン用のABC染色キット(カリフォルニア州バーリンゲームのVector Labs)を使用して染色する。
1μm/画素より上の空間分解能での省電力撮像に、蛍光カメラ(DFC350 FX、Leica)を装着した蛍光顕微鏡(DM60000B、ドイツ、WetzlarのLeica)を使用する。
大動脈組織切片の単一チャネル蛍光画像(3隣接切片の1切片につき9画像)を、タグ付き画像ファイル形式(tagged image file format:TIFF)で0〜255の間のバイナリ画素強度での定常カメラ設定で撮る。群ごとに、追加の隣接組織切片をアイソタイプ対照抗体で染色して、バックグラウンド閾値を決定する。各チャネルについてのバックグラウンド強度閾値を、対照染色において画素の95%が出した強度に設定する。バックグラウンド強度閾値未満の画素は、定量から除外する25。Matlab(ミシガン州ノヴァイのMathworks)による画像解析ソフトウェアを使用して、残りの画素を合計し、画素の総数で割って平均画素強度を算出し、および陽性画素を合計して、陽性面積を算出する26。
ヒト大動脈内皮細胞(HAEC)は、弁修復のための手術を受けた患者から得た肉眼病変のない上行大動脈の生検材料から単離し、ヒト大動脈平滑筋細胞は購入し(HASMC、Promocell)、末梢血単球、マクロファージ、および泡沫細胞は、健常発端者(proband)の末梢血から単離する。細胞単離、増殖および純度検証の詳細な方法については、下記を参照されたい。
内皮細胞単離のために、大動脈内腔をPBSで洗浄し、2型コラゲナーゼを含有するDMEM(350U/mL、ニュージャージー州レークウッドのWorthington)中で30分間インキュベートし、穏やかに撹拌して血管壁から内皮を解離させる。CD34に対する磁気ビーズ分離(130−046−702、ドイツGladbachのMiltenyi Biotech)によって内皮細胞をさらに精製する。
大動脈狭窄のための手術を受けた患者から、上行大動脈プラーク生検材料を得る。弁欠損を修復するための手術を受けた患者から、プラークのない大動脈生検材料を得る。これらの試料を、4℃に予冷した滅菌ダルベッコ変性イーグル培地(DMEM、カリフォルニア州カールズバッドのGibco)に直接入れ、研究所の氷上に移し、組織ザイモグラフィーのためにOptimal Cutting Temperature Compound(カリフォルニア州トランスのTissue−Tek)中で凍結する。
3、5、10、20、40%マクロファージ調整SFMを添加した飢餓培地で48時間、静止期HASMCを処置する。FAP発現に対するTNFαの効果を判定するために、20%マクロファージ調整SFMおよびTNFα中和抗体(Ab6671、Abcam)またはIgGアイソタイプ対照(Ab27478、Abcam)抗体を添加した飢餓培地で静止期HASMCを処置する。組換えヒトTNFα(300−01A、Peprotech)を使用して、静止期HASMCにおいて用量および時間依存的様式でFAP発現を誘導する。下で詳述する細胞膜ELISAによって、すべてのFAPレベルを定量する。末梢血由来マクロファージを48時間、SFM(6ウエルプレートにおいて2mL/ウエル)中でインキュベートし、上清を滅菌濾過し、等分し、−80℃で凍結させる。購入したヒト大動脈平滑筋細胞(HASMC、Promocell)をFACS分析によってαSMA(純度>96%)について検証し、継代4で、96ウエル黒色細胞培養試験プレートに、10%ウシ胎児血清(3302−P250302、ドイツAidenbachのPAN Biotech)を添加したDMEM(10938、Gibco)中5×104細胞/cm2の密度でプレーティングし、24時間放置して付着させた後、1%ウシ血清アルブミンを添加したAdvanced DMEM(12491−015、カリフォルニア州カールズバッドのInvitrogen)飢餓培地中で一晩、静止状態にする。3、5、10、20、40%マクロファージ調整SFMを添加した飢餓培地で48時間、静止期HASMCを処置する。FAP発現に対するTNFαの効果を判定するために、20%マクロファージ調整SFMおよびTNFα中和抗体(Ab6671、Abcam)またはIgGアイソタイプ対照(Ab27478、Abcam)抗体を添加した飢餓培地で静止期HASMCを処置する。組換えヒトTNFα(300−01A、Peprotech)を使用して、静止期HASMCにおいて用量および時間依存的様式でFAP発現を誘導する。定量のために、細胞をPBSで洗浄し、4%ホルマリン中で30分間固定し、FAPに対してF19で標識する。ホースラディッシュペルオキシダーゼELISAキット(カリフォルニア州フリーモントのAnaspec)を使用して培養物を酵素標識し、酵素反応生成物を蛍光プレートリーダーで定量する。バックグラウンド値を引き、すべての値を未処置/未調整対照群に正規化する。
直接急冷ブタゼラチン(DQゼラチン、D12054、Invitrogen)を、黒色96ウエルプレートにおいて、反応バッファー(pH7.6の、0.5M Tris−HCl、1.5M NaCl、50mM CaCl2、および2mMアジ化ナトリウム)および0、10、20、40nMの組換えヒトFAP中、100μg/mLの最終濃度に希釈する。切断されたゼラチンを0.5、8および24時間の時点で定量し、バックグラウンド蛍光を引いた。FAPの触媒インサートに対して産生されたA246およびそれに対応するアイソタイプ対照IgG(Ab27478、Abcam)を反応バッファー中の20mM FAPおよび100μg/mL DQゼラチンに添加し、24時間後に分析して遮断効力を判定する。
一元配置ANOVAを用いて組織学的結果および細胞培養結果を比較し、関連性をピアソン相関係数によって算出する。スチューデントt検定をザイモグラフィーの比較に用いる。MatLab(バージョン、R2007b)を用いて、すべての統計解析を行う。データを平均±SDとして提示する。有意性は、p<0.05のレベルで許容される。
隣接凍結切片におけるFAPについての免疫蛍光染色は、AHAグレード分類基準によって特性評価したとき、プラークのない大動脈に対して線維性アテロームでのほうが増進された発現を示す(図1A)。FAPについての陽性染色は、健常上行大動脈には実質的にみとめられないが、タイプII−IIIおよびタイプIV−Vプラークでは段階的増加が観察される(図1B)。定量的画像解析は、FAPが、進行した大動脈プラークではプラークのない大動脈または早期プラークと比較して有意に増加することを示した(図1C)。
臨床的に意義のあるアテローム性動脈硬化症病変におけるプラークの易損度とFAPの関連性を判定するために、急性心筋梗塞後に死亡した患者から得た破裂しやすいヒト冠動脈のMasson法(青色でコラーゲンを染色)を行う。線維性キャップ厚に基づき、これらの検体を「不安定な」薄いキャップ(<50μm)または「安定した」厚いキャップ(>65μm)の線維性アテロームとして特性評価する。隣接切片における免疫組織学的および免疫蛍光染色は、「安定した」厚い線維性キャップに対して「不安定な」薄い線維性キャップでのほうが増進されたFAP発現を示すことを明らかにした(図4A)。共焦点画像解析は、ヒト冠動脈線維性アテロームの「安定した」厚い線維性キャップに対して「不安定な」薄い線維性キャップでのほうがFAP発現の有意な増加を示すことを明らかにした(図4B)。
免疫蛍光染色は、代表大動脈脂肪線条においてマクロファージに隣接した内側の細胞でのFAP発現を明らかにする(図5A)。FAPと炎症との関係の特徴を明らかにするために、ヒト大動脈プラークにおけるFAPとマクロファージ免疫蛍光シグナル強度(図5B)を比較する。多数のプラーク進行段階でマクロファージ負荷量とFAP発現との間に正の相関関係を認める(R2=0.763、n=12、図5C)。
マクロファージとFAP発現HASMCの間のシグナル伝達機序を解明するために、HASMCをマクロファージ調整培地に48時間曝露して、プラーク炎症に適用可能な条件をシミュレートする。培養HASMCは、マクロファージ調整培地に応じてFAPの有意な用量依存的増加を示し、最大効果は48時間後に20%培地濃度で観察される(図6A)。この効果は、マクロファージ調整培地をTNFα中和抗体で処置すると消える(図6B)。TNFαによるパラクリン誘導FAP発現を確認するために、組換えヒトTNFαを使用して、HASMCにおけるFAPを用量および時間依存的様式で誘導する。最大応答は、30ng/mLで48時間後に確認される(図6Cおよび6D)。
免疫蛍光分析は、大動脈線維性キャップの低I型コラーゲン領域におけるFAP発現増進を明らかにした(図7A)。組換えヒトFAPは、用量および時間依存的にゼラチンを分解する(図7B)。FAPの触媒性部位に対して指向された抗体であるA246は、組換えヒトFAPと活性化HASMCの両方のゼラチン分解活性を低減させる(図7Cおよび7D)。IgG対照抗体で処置した大動脈線維性キャップは、FAPと切断I型コラーゲンとの共局在を示すが、Ab246処置プラークは、I型コラゲナーゼ活性とのFAPの有意に低減された共局在をはっきりと示す(図7E)。共焦点画像解析は、A246処置線維性キャップが、FAP発現部位において切断I型コラーゲンの減少を示すことを明らかにする(図7F)。
8週齢で開始して12週間、高コレステロール食(総コレステロール1.25%、Research DietsからのRD12108)を給餌した雄アテローム性動脈硬化症アポリポタンパク質Eノックアウト(ApoE−/−)マウス(C57BL/6Jバックグラウンド)(n=5)の大動脈根におけるプラークを分析する。同じ期間、正常な食餌を用いた野生型(WT)雄C57BL/6Jマウス(n=4)を健常対照として使用する。イソフルランによって安楽死させたマウスの大動脈根を正常食塩水ですすぎ、切除し、直ちにOCTに包埋し、切片作製のために凍結する。ApoE−/−ノックアウトマウスのアテローム性動脈硬化性プラークにおいて確認されたFAPの存在により、FAP中和抗体の静脈内注射によるin−vivo FAP阻害研究のためのこのモデルの使用の正当性が立証される(図12)。
U型透明96ウエルマイクロタイタープレート(抗FAP抗体を被覆してFAPタンパク質を検出することができるプレート)を使用して、標準的なプロトコルを用いて、サンドイッチELISAを行うことができる。3から300ng/mLの濃度の100μLの組換えFAPタンパク質(rhuFAP)溶液をウエルに添加して、1時間、25℃でインキュベートする。試料のインキュベーション後、ウエルを2回洗浄し、1ウエルにつき100μLの第一の検出抗体(mAb FAP)を添加し、プレートをインキュベートする(1時間、25℃)。3回の洗浄段階の後、100μLの第二の蛍光検出抗体(1:3000希釈したもの)を添加し、インキュベートする(25℃で1時間)。インキュベーション後、ウエルを3回洗浄する。すべての試料を三重反復で試験し、結果を標準曲線に対して比較する。3〜300ngの精製ヒトFAPを使用して、アッセイ感度レベルおよび範囲を判定した(図8)。天然バックグラウンドをシミュレートするために、1μgの大腸菌(E.coli)全細胞抽出物を各試料に添加してもよい。この数は、ブランクシグナル標準偏差の3倍のバックグラウンドシグナル平均より上であるシグナルに相当する。FAP量と蛍光シグナルの間で観察される依存性を、現実の試料に関するFAP濃度の推定に用いることができる。結果を図8に示す。
ROTEM/NATEM(ROTEM(登録商標)、ドイツMunichのPentapharm CO)技術を、製造業者の指示書に従って用いる。末梢血試料を健常発端者からクエン酸塩のチューブに採取し、組換えヒトFAP(rhu FAP 0、0.875μg/mL、1.750μg/mLおよび3.5μg/mL)で0から5時間までの間、調整する。対照血液試料は、組換えFAPなしでインキュベートする。NATEMアッセイからの代表ROTEM読出しは、FAPがヒト血液凝固速度を加速することを明らかにした(図9)。加えて、組換えヒトFAPで処置した健常患者血液試料では、凝固時間、血餅形成時間、アルファ角および血餅硬度が増加される。20ng/mL未満の内因性血漿FAPレベルを呈示する6名の健常発端者から血液を採取し、0.175μg/mL、1.75μg/mLおよび3.5μg/mLの用量の組換えヒトFAPで調整し、0、2.5時間および5時間、インキュベートする。それによって、FAPが凝固時間および血餅形成時間を加速し、アルファ角および最大血餅硬度を増すことが、観察される(図10)。これらの結果を、NATEM技術を利用することにより、さらに確認する。上で説明したように試料を処置する。そのようにして、5時間のインキュベーションで1.75ug/mLのFAPで調整したとき、ビークル処置対照と比較して、絶対凝固時間および血餅形成時間は、5/6の発端者において減少し、アルファ角は、5/6の患者において増加し、および最大血餅厚は、4/6の発端者において増加したことが判明した(図11)。
ヒトパラフィン包埋冠動脈プラークおよび内胸動脈からの横断切片(4μm厚)をスライドガラスにマウントする。95℃賦活化用バッファー(2mMクエン酸ナトリウム、pH7.6)中での20分のインキュベーション後、パラフィン包埋切片における抗原性賦活化処理を行った。FAPの触媒インサートに対して産生されたウサギポリクローナル抗体(A246、Ab28246、マサチューセッツ州ケンブリッジのAbcam)およびウサギアイソタイプ対照(Ab37415、Abcam)を使用して、FAPを染色した。マウスモノクローナル抗体(303108、BioLegend)およびそれに対応するアイソタイプ対照(400123、BioLegend)を使用してCD31を染色した。一次抗体をビオチン標識ヤギ抗マウス(115−066−003、ペンシルバニア州ウエストグローヴのJackson ImmunoResearch)およびビオチン標識ヤギ抗ウサギ(111−066−003、Jackson ImmunoResearch)で検出し、ABC染色キット(カリフォルニア州バーリンゲームのVector Labs)を使用して染色した。

心筋梗塞に罹患している患者の冠動脈から血栓を吸引した。クエン酸塩が添加されたチューブに各患者から5mLの末梢血を血栓吸引前1分未満に採取した。50μLのActilyseを含有する1mLのAcutaseに末梢血(1mL)および冠血栓材料を入れ、37℃で1時間、穏やかに振盪した。注射器からの軟質ゴムを使用するセルストレーナー(孔径40μm)による取捨により、細胞凝集物をさらに解離させた。次いで、両方の試料を400Gで5分間回転させ、上清を除去した。次いで、1μg/mLのFc受容体用ブロッキング剤を含有するFACSバッファー(1%FCSおよび5mM EDTAを含有するPBS)に細胞ペレットを再浮遊させ、30分間4℃でインキュベートした。その後、FAPに対する蛍光タグ付き抗体(クローンF19)、顆粒球に対する蛍光タグ付き抗体(CD66b、BioLegend 555724)、単球に対する蛍光タグ付き抗体(CD45、BioLegend 345809)およびTリンパ球に対する蛍光タグ付き抗体(CD3、BioLegend 555332)で細胞を標識した。
末梢動脈閉塞性疾患に罹患している患者の大腿動脈から血栓を吸引した。クエン酸塩が添加されたチューブに各患者から5mLの末梢血を血栓吸引の数秒前に採取した。血栓および血液検体をリン酸緩衝食塩水に入れ、処理のために研究所に移した。上の実施例11で与えた説明に従って、POAD血栓検体を調製した。
冠動脈疾患(CAD)、安定狭心症、不安定狭心症およびSTEMIを有さない患者から5mLの血液をクエン酸塩のチューブに採取した。血液を10分間、400Gで回転させ、血漿層を抽出し、後の分析まで−80Cで凍結させた。可溶性FAPについてのサンドイッチELISAを次のように行った。FAPの触媒インサートに対して産生されたウサギポリクローナル抗体(A246、Ab28246、マサチューセッツ州ケンブリッジのAbcam)を捕捉抗体として使用し、mAb FAPを検出抗体として使用した。3から5000ng/mL濃度のヒト組換えFAPタンパク質(rhuFAP)溶液をウエルに添加して、標準曲線を確立する。捕捉抗体で被覆した後、プレートを血漿試料および標準物質とともに1時間、25℃でインキュベートした。試料のインキュベーション後、ウエルを1ウエルにつき100μLのPBSで2回洗浄した。検出抗体(mAb FAP)を添加し、プレートをインキュベートした(1時間、25℃)。3回の洗浄段階の後、100μLの第二のHRP標識検出抗体を添加し、インキュベートした(25℃で1時間)。インキュベーション後、ウエルを3回洗浄し、ペルオキシダーゼ基質を添加して検出用シグナルを生じさせた。結果として生じたシグナルを、プレートリーダーを使用して読み取って、血漿試料中のFAP濃度を判定した。ELISA分析は、CADを有さない患者と比較してACSを有する患者でのほうが上昇したFAP血漿レベルをあきらかにした(図17)。
アテローム性動脈硬化性プラークの線維性キャップは、内腔と血栓形成性壊死性コアの離隔に欠くことができない。線維性キャップの機械的強度はコラーゲンによってもたらされるので、コラーゲンを分解するMMPおよびシステインプロテアーゼは、プラーク不安定性および急性血栓イベントの発生と関連づけられる29−33。本研究は、構成的活性セリンプロテアーゼFAPとプラーク進行および易損性を結びつけ、(1)FAP発現が、ヒト大動脈アテロームと破裂しやすい冠動脈の線維性キャップにおいて増進されること、(2)FAPが、HASMCにおいて発現され、炎症性マクロファージ負荷量と相関すること、(3)FAPが、HASMCにおいてパラクリンシグナル伝達によりマクロファージ由来TNFαによって誘導されること、(4)FAPがヒトアテロームの線維性キャップのコラーゲンを切断すること、および(5)FAP介在コラゲナーゼ活性が、ヒト線維性キャップにおいてFAP遮断抗体により阻害されることを証明する。

- 患者における心血管の疾患または病態の存在を判定するためのアッセイ方法であって、
(i)前記患者から採取した血液の試料を線維芽細胞活性化タンパク質(FAP)の発現についてアッセイすることであって、該FAPの発現、もしくは対照試料と比較した該FAPの発現増加が、前記患者における前記疾患もしくは前記病態についての指標となるもの、および/または
(ii)前記患者から採取した血栓の試料をFAPの発現についてアッセイすることであって、前記患者から採取した血液試料におけるその発現と比較したFAPの発現増加が、前記疾患もしくは前記病態の指標となるもの、
