Claims (14)
- ウォータジェットを使用して治療対象組織を特に任意選択的に切開・切除し、そこに噴流を噴射し、そこに高周波電流を流してそれを凝固させるためのHF外科治療用の多機能器械(1)において、
−流体を受ける少なくとも1つの流体チャンバ(3)と、
−前記少なくとも1つの流体チャンバ(3)と連通し、前記流体チャンバ(3)内にある流体を温め、高温にし、又は気化させる温度制御装置(9)と、
−前記流体チャンバ(3)内にある前記流体の温度を制御する温度コントローラと、
−前記多機能器械の遠位端に配置され、前記少なくとも1つの流体チャンバ(3)と連通する少なくとも1つの流体出口ポート(31、39)と、
を有しており、
前記少なくとも1つの流体出口ポート(31)が、シース(7)の遠位端(21)より先まで突出するHF発生器からの高周波電流のための中空の導体の形態で構成された活性電極(29)内に配置され、前記流体チャンバ(3)の長手軸方向に延びる貫通穴として構成され、
前記多機能器械(1)が、前記少なくとも1つの流体出口ポート(31)を通して組織を切開・切除するためにウォータジェットを放出するよう構成されていることを特徴とする多機能器械(1)。 - 請求項1に記載の多機能器械において、
前記温度制御装置(9)が、相互に離間して配置され、少なくとも特定の領域において前記流体チャンバ(3)の壁を形成する少なくとも2つの温度制御電極(11、11’)を含むことを特徴とする多機能器械。 - 請求項2に記載の多機能器械において、
前記温度制御電極(11、11’)が、それらの間に、特に少なくとも1つの好ましくは環状のスペーサ(19)により決定される半径方向のギャップ(a)を有することを特徴とする多機能器械。 - 請求項2又は3に記載の多機能器械において、
前記温度制御電極(11、11’)が、特に円筒形で柔軟なシース(7)の内面に取り付けられ、好ましくはコーティング、薄膜又はその他の薄い材料層の形態で構成されることを特徴とする多機能器械。 - 請求項1乃至4の何れか1項に記載の多機能器械において、
流体温度を制御/調整するために、前記温度コントローラが、前記温度制御装置(9)に供給されるワット数を、指定の流体流速に応じて調整することを特徴とする多機能器械。 - 請求項1に記載の多機能器械において、
前記活性電極(29)が前記少なくとも1つの流体チャンバ(3)の中まで延びるHF発生器からの高周波電流のための中空の導体の形態で構成され、特に前記少なくとも1つの流体チャンバ(3)の直径が前記活性電極(29)の直径より大きいことを特徴とする多機能器械。 - 請求項1又は6に記載の多機能器械において、
前記活性電極(29)と前記温度制御電極(11、11’)に、高周波電流の供給ケーブルへの別々のコネクタ(15、15’、35)が設けられていることを特徴とする多機能器械。 - 請求項1、6又は7の何れか1項に記載の多機能器械において、
前記活性電極(29)の前記遠位端(31b)が、組織を機械的に切開・切除するための切開・切除ツール、特に切刃として構成されることを特徴とする多機能器械。 - 請求項1乃至8の何れか1項に記載の多機能器械において、
少なくとも1つの他の流体出口ポート(39)が前記器械の遠位端(21)に設けられ、前記ポートが前記少なくとも1つの流体チャンバ(3)又は少なくとも1つの他の流体チャンバと連通することを特徴とする多機能器械。 - 請求項9に記載の多機能器械において、
前記少なくとも1つの他の流体出口ポート(39)が、前記器械の前記遠位端において前記シース(7)に接続される器械アタッチメント(23)の中のノズル型の軸方向の貫通穴として構成されることを特徴とする多機能器械。 - 請求項10に記載の多機能器械において、
前記活性電極(29)が、基本的に軸方向において前記器械アタッチメント(23)の中の中央に配置され、特にその中に移動可能に支持されることを特徴とする多機能器械。 - 請求項10又は11に記載の多機能器械において、
1つ又は複数の前記流体出口ポート(39)に、特にパッシブボールバルブ又はアクティブピエゾ式バルブとして構成されるバルブユニット(41)が設けられることを特徴とする多機能器械。 - 請求項12に記載の多機能器械において、
前記バルブユニット(41)が前記流体出口ポート(39)の近位端、特に前記器械アタッチメント(23)の近位端に配置されることを特徴とする多機能器械。 - 高周波治療電流を生成するため、及び温度制御電流を生成するための少なくとも1つのHF発生器(17)を有し、請求項1乃至14の何れか1項に記載の多機能器械(1)を有するHF手術システム。
