Description

ここで開示された実施例では、副交感神経治療と交感神経治療の展開は、患者の脈拍数を低減させるために行うこともできるし、患者の血圧を上昇させ、あるいは血圧を維持するためにも行うことができる。
同様に、心拍数(HR)と血圧(BP)を上げることが必要な場合(図1に示す右上の枠内)のように、純粋な交感神経の刺激が必要な患者の場合、本システムはそのような機能を提供するようになっている。 しかし、急性心臓代償不全の多くの場合、特に急性心臓代償不全になりつつある患者または心原性ショック状態にあったり、心腎臓疾患症候群を伴っているような患者の場合、純粋な副交感神経の刺激や純粋な交感神経の刺激は、潜在的に有害な作用を呈することがある。
治療装置10では、上大静脈の頭部部分中央の後側側方セグメントに配置された副交感神経刺激用の電極と、上大静脈の頭部部分中央の後側中央セグメントに配置された交感神経刺激用の電極とを使用し、交感神経の刺激と副交感神経の刺激を同時に実行できるように、電極14あるいはその他の治療エレメントを配置している。
図4は、神経変調療法システムの1実施例を図示したものであり、この神経変調療法システムには、ここで開示した治療を実行するのに適した制御システム100が含まれている。 この神経変調療法システムは、電極アレイ、およびオプションとして、患者とシステムの診断エレメント、圧力センサ、流量センサ、その他の血行動態センサ、その他の患者の状態センサ、および位置センサ、システム接続センサ、あるいはシステム異常状態モニタ用センサのようなシステム状態センサなどの治療エレメントを有する治療カテーテル(図中には「ニューロカテーテル」として表示してある)を備えている。
血行動態の制御を実現するために、統合された装置を使用する典型的な方法について、次に説明する。 この方法は、急性心不全症候群の治療において、心拍数を低減させ、かつ血圧を上昇または維持することに特に有効な方法である。
もし、刺激している間に、心拍数及び/又は血圧が少なくとも閾値だけ、たとえば少なくとも5%だけ、増加しなければ、交感神経用のアレイを再配置することにより、2番目の交感神経の場所を選択するか、あるいは上大静脈(SVC)の後側中央の壁に配置された別のアレイに電圧を印加することにより、2番目の交感神経の場所を選択する。 このプロセスは、血圧が少なくとも閾値(この例では5%)だけ増加することになる刺激場所が決定されるまで繰り返される。
ここで、必要とする治療が心拍数を減少させ、血圧を維持させるか上昇させる場合であっても、ターゲットとする交感神経の刺激場所を識別することには、交感神経の刺激のために配置された電極に電圧を印加している間に、心拍数が少なくとも閾値だけ増加することをモニタすることを含めるようにしても良い。
この理由は、交感神経のマッピング中に心拍数が上昇することは、交感神経の場所を刺激することによって交感神経系の心臓神経が捕捉されていることを確認することになるためである。
試験の準備において、別々のカテーテルが、上大静脈(SVC)の中の前述した位置に配置された。 各カテーテルにおいて20Hz、パルス幅=0.5ms、振幅=10mAの条件で刺激が展開された。 刺激は、垂直な面内に配置された電極(4mm間隔)から、各カテーテルにより同時に実施された。 二つのカテーテル間の距離は約1‐2cmであり、円周上45度の間隔であると見積もられた。
・ 5回の犬の実験において、副交感神経と交感神経を同時に二重刺激した結果、心拍数(HR)は減少し、血圧(BP)は上昇するか維持したままの状態であった。
・ 刺激を停止した後、5匹の犬は全て、1〜3分以内に、平常状態における血行動態パラメータに戻った。
・ 計測が行われた2回の犬の実験における2回とも、二重刺激の間中、駆出率が計測可能な程度改善した。
・ 計測が行われた2回の犬の実験における2回とも、二重刺激の間中、心拍数(HR)が減少しているにもかかわらず、心拍出量が維持されるかあるいは計測可能な程度改善した。
・ 計測が行われた4回の犬の実験における4回とも、心拍数(HR)と血圧(BP)の応答は、1時間の刺激の間維持された。 そして、刺激が中止されると、心拍数(HR)は10秒以内に平常の状態に戻り、血圧(BP)は、1〜3分以内に平常の状態に戻った。
・ 5回の犬の実験の全てにおいて、アトロピンの投与により、刺激で誘起された心拍数(HR)の低下が消え、選択的な副交感神経の変調が確認された。
・ また、計測が行われた4回の犬の実験における4回とも、プロプラノロールの投与により、刺激に誘起された血圧(BP)の応答が軽減され、選択的な交感神経の変調が確認された。
Claims (8)
- 患者を治療するための神経変調療法システムであって、
血管内に配置するように適合した副交感神経の治療エレメントと、
血管内に配置するように適合した交感神経の治療エレメントと、
心不全の治療モードにおいて副交感神経と交感神経の治療を展開することによって、患者の心拍数を低減すると同時に、患者の血圧を維持するかあるいは上昇させるように、
(a) 血管の外部にある副交感神経の線維に対して副交感神経の治療を展開するために、血管内にある副交感神経の治療エレメントに電圧を印加することと、
(b) 血管の外部にある交感神経の線維に対して交感神経の治療を展開するために、血管内にある交感神経の治療エレメントに電圧を印加すること
の両者を同時に実行する心不全の治療モード用に構成された刺激装置と、
から成ることを特徴とする神経変調療法システム。 - 請求項1に記載された神経変調療法システムであって、
検出した患者の心拍数及び/又は血圧に応じて副交感神経と交感神経の刺激を制御するための制御手段を、更に含むことを特徴とする神経変調療法システム。 - 請求項2に記載された神経変調療法システムであって、
前記制御手段は、副交感神経と交感神経の刺激を独立して制御することを特徴とする神経変調療法システム。 - 請求項1に記載された神経変調療法システムであって、
前記副交感神経の治療エレメントと交感神経の治療エレメントを支持する治療装置を更に含むことを特徴とする神経変調療法システム。 - 請求項4に記載された神経変調療法システムであって、
前記治療装置は、血管の内壁に接触させて副交感神経と交感神経の治療エレメントを配置するために、少なくとも部分的に膨張し、あるいはバイアスをかけた先端部分を備えていることを特徴とする神経変調療法システム。 - 請求項5に記載された神経変調療法システムであって、
前記先端部分は、上大静脈の後側側方の壁に対して副交感神経の治療エレメントを配置するために、並びに、上大静脈の後側中央部分に対して交感神経の治療エレメントを配置するために膨張し、あるいはバイアスをかけられることを特徴とする神経変調療法システム。 - 請求項4に記載された神経変調療法システムであって、
前記治療装置はカテーテルであることを特徴とする神経変調療法システム。 - 請求項1に記載された神経変調療法システムであって、
前記副交感神経の治療エレメントと交感神経の治療エレメントは電極であることを特徴とする神経変調療法システム。
