Description
本発明は、2011年2月10日に出願した米国仮特許出願番号第61/441,315号の利益を主張し、参照によりその全体が援用される。
本明細書において、心血管系疾患(CVD)の治療および/または予防のための組成物、ならびにそれらの応用および使用の方法を提供する。特に、本発明は、3,3−ジメチル−1−ブタノール(DMB)および関連化合物、ならびにその製剤処方を用いる心血管系疾患の治療および/または予防を提供する。他の実施形態において、本発明は、血栓症、心臓発作および/または血小板過剰反応性を引き起こすリスクに関連する治療の前に、腸内細菌叢を標的とする抗生物質を対象に投与する方法を提供する。
心血管系疾患(CVD)とは、アテローム性動脈硬化症、冠状動脈性心疾患、脳血管疾患、大動脈腸骨疾患および末梢血管疾患を含む、心臓および血管の疾患に対する一般的な用語である。CVDを有する対象は、心筋梗塞、脳卒中、狭心症、一過性脳虚血発作、うっ血性心臓疾患、大動脈瘤および死亡を含むがこれらに限定されない、多くの合併症を発生する可能性がある。CVDは、アメリカ合衆国における死亡原因の50%を占めており、ナンバーワンの致死性疾患である。ゆえに、心血管系疾患の予防は、住民の健康にとって重要な位置を占めている。
一部の実施形態において、本発明は、心血管系疾患および/または血栓症の治療および/または予防に対する方法であって、a)対象の血小板凝集が増加している、および/またはTMAOレベルが上昇していると識別すること、b)当該対象に、ジメチルブタノールおよび/もしくはその誘導体、ならびに/または消化管標的抗生物質ならびに/またはプレバイオティック(たとえば、腸内細菌叢組成を変化させるファイバー含有食物)ならびに/またはプロバイオティック(たとえば、ヨーグルト等のプロバイオティック含有食物)を含有する組成物を投与することが含まれる、方法を提供する。ある実施形態において、組成物は、ジメチルブタノールを含有する。さらなる実施形態において、識別することには、血小板凝集の増加を示す対象由来の試料で実施された血小板凝集分析の結果を見る(たとえば、紙上で、またはコンピュータースクリーン上で)ことが含まれる。さらなる実施形態において、識別することには、TMAOレベルの上昇を示す対象由来の試料で実施されたTMAO分析の結果を見ることが含まれる。ある実施形態において、識別することには、TMAまたはTMAOレベルの上昇を示す当該対象由来の試料または呼気試料で実施されたTMAまたはTMAO分析の結果を見ることが含まれる。

本明細書において、「心血管系疾患」(CVD)または「心血管系障害」という用語は、身体の心臓、心臓弁および血管系(たとえば、動脈および静脈)に影響を与える多くの状態を分類するために用いられる用語であり、限定されないが、動脈硬化症、アテローム性動脈硬化症、心筋梗塞、急性冠症候群、狭心症、うっ血性心不全、大動脈瘤、大動脈解離、腸骨動脈瘤または大腿動脈瘤、肺塞栓症、原発性高血圧症、心房細動、脳卒中、一過性脳虚血発作、収縮機能障害、拡張機能障害、心筋炎、心房性頻脈、心室細動、心内膜炎、動脈症、血管炎、アテローム動脈硬化性プラーク、不安定プラーク、急性冠症候群、急性虚血性発作、突然心臓死、末梢血管障害、冠動脈疾患(CAD)、末梢動脈疾患(PAD)および脳血管疾患を含む疾患および状態を包含する。
本発明は、心血管系疾患を発症するリスクのあるまたは有する対象を治療するための組成物および方法を提供する。一部の実施形態において、3,3−ジメチル−1−ブタノール(ジメチルブタノール、DMBとしてもまた知られる。コリンの窒素原子が炭素に置換されているコリンのアナログ(図1を参照のこと))が対象に投与される。一部の実施形態において、本発明は、心血管系疾患を発症するリスクのあるまたは有する対象に、DMBを投与することを提供する。一部の実施形態において、治療効果を有する量のDMB、DMBを含有する化合物、DMB関連化合物および/またはそれらの誘導体をCVDの治療および/または予防のために対象に投与する。一部の実施形態において、DMB、DMBを含有する化合物、DMB関連化合物および/またはそれらの誘導体を、CVDに対して受容されている治療の一つ以上と併せて、対象に共に投与する。ある実施形態において、腸内細菌叢を標的とする抗生物質が、CVDおよび/または血栓症を治療または予防するために対象に投与される。
〔実施例1〕 ファオファチジルコリン代謝の構造特異性
実験は、本発明の実施形態の開発中に行われ、内在性泡沫細胞形成により観察される、アテローム生成促進マクロファージ表現型の促進における、ファオファチジルコリン代謝の構造特異性を検証した。離乳期のC57BL/6J.Apoe−/−マウスを通常の固形飼料(対照)、またはコリン、TMAOもしくはコリンアナログDMB(コリンの第四級アミン窒素が炭素に置換されている(図2右を参照のこと))のいずれかを補給された通常の固形飼料を与えた。DMBは、ゆえに、潜在的な開裂に対するC−N結合が無く、腸内細菌叢によるTMA形成が無いことを除き、コリンと構造的に同一である。トリメチルアミン類(コリンまたはTMAO)のいずれかが補給された食事を与えられたマウスは、マクロファージコレステロール含有量の増加およびTMAOの血漿レベルの上昇の両方を示した。反対に、食品中のDMB補給は、TMAOの増加をもたらさず、内在性マクロファージ中のコレステロール蓄積の増加ももたらさなかった。
実験は、本発明の実施形態の開発中に行われ、高コリンの食事(たとえば、洋風の食事)により、アテローム性動脈硬化の増加がもたらされ、DMB化合物の添加により食事誘導性のアテローム性動脈硬化の増加が阻害されることが示された。離乳期(4週齢)のC57BL/6J.Apoe−/−のオスのマウスに、1.3%コリンを補給した固形飼料、1.3%DMBを補給した固形飼料、その両方を補給した固形飼料、またはいずれも補給しない固形飼料を与えた。大動脈基部の切片を、オイルレッドO/ヘマトキシンで染色した。大動脈内側でレッドオイルに染色される領域は、病変プラークを示す(図3および4を参照のこと)。DMBを添加した通常の固形飼料により、apoE−/−マウスにおける大動脈プラークの形成が有意に減少した(約90%)(図4を参照のこと)。さらに、コリンの添加により、高コリン食品により誘導されたアテローム性プラークの増加が完全に阻害された(図4を参照のこと)。これらのデータは、DMBが、大動脈プラーク形成を減少する能力を有すること、および高コリン食品により誘導されたプラーク形成を食い止める能力を有することを示す。これらのデータはさらに、DMAがプラーク形成およびアテローム性動脈硬化に関する心臓疾患の予防および/または治療に対する療法を提供しうることを示している。
実験は、本発明の実施形態の開発中に行われ、固形飼料または1.3%コリン、1.3%DMBもしくはその両方を補給された固形飼料を与えられたマウスにおける、(1)コレステロール、(2)トリグリセリド、低密度リポタンパク質−コレステロール、およびHDLコレステロールの血漿中レベルを比較した(図5を参照のこと)。マウスの食事にDMBを添加すると、マウスのアテローム性リポタンパク質プロファイルが改善され、アテローム性コレステロールレベル(総コレステロール、低密度リポタンパク質およびトリグリセリド)が有意に減少し、高密度リポタンパク質が有意に増加した。
実験は、本発明の実施形態の開発中に行われ、コレステロール蓄積および泡沫細胞形成におけるDMB添加の効果を検証した。離乳期(4週齢)のC57BL/6J.Apoe−/−のオスのマウスに、1.3%コリン、1.3%DMB、もしくはその両方を補給した固形飼料またはいずれも補給しない固形飼料を与えた。腹膜マクロファージを回収し、4%パラホルムアルデヒドで固定し、オイルレッドO/ヘマトキシンで染色した。DMBの添加により、高コリン食品により誘導されたコレステロール蓄積および泡沫細胞の形成が食い止められた(図6を参照のこと)。
実験は、本発明の実施形態の開発中に行われ、腹膜マウスマクロファージにおける総コレステロールレベルにおけるDMBの効果を測定した(図7を参照のこと)。離乳期(4週齢)のC57BL/6J.Apoe−/−のオスのマウスに、1.3%コリン、1.3%DMB、もしくはその両方を補給した固形飼料またはいずれも補給しない固形飼料を与えた。細胞の総コレステロールを安定同位体希釈LC/MS/MSにより定量化した。細胞数は、細胞溶解物中のタンパク質含量により定量化した。高コリン食品により誘導されたマクロファージコレステロール含有量の顕著な増加は、DMBの添加により阻害された(図7を参照のこと)。これらのデータにより、高コリン食品により誘導される泡沫細胞の形成が減少することを示す。
実験は、本発明の実施対応の開発中に行われ、腸内微生物叢が触媒する、コリンからトリメチルアミンの産生におけるDMBの効果を検証した。マウスの盲腸を、腸内微生物叢の原料としてホモジナイズした。D9‐コリンを基質として用いて、d0−TMAの産生を、安定同位体希釈LC/MS/MSにより定量化した。DMBの添加により、腸内細菌叢が介在するコリンのTMAへの異化が著しく阻害された(図8を参照のこと)。
研究患者および研究デザイン
この実施例において、Cleveland Clinic Institutional Review Boardにより承認された2つのプロスペクティブ研究を記述する。全ての参加者は、書面でインフォームドコンセントを提出した。最初の研究は、慢性疾患または末端器官の機能不全(心不全、腎不全、肺疾患または血液病の既往歴を含む)が無い健康で、活性化された感染症または抗生物質の摂取が無く、プロバイオティクスを使用していない18歳以上のボランティアのコホート(N=40)を説明する。Visit1の間、対象は食品中の「ホスファチジルコリン(PC)のチャレンジ」(下記参照)を受けた。彼らのうち、6人がメトロニダゾール500mgを一日2回とシプロフロキサシン500mgを1日1回、1週間与えられ、抗生物質の後、繰り返しPCチャレンジを受けた(Visit2)。三番目および最後のPCチャレンジは、抗生物質の投与停止および腸内細菌叢の再獲得の1か月以上後に実施された(Visit3)。
簡単な食品中のPC/コリンのチャレンジテストが、標準医療用アイソトープとして、摂取可能な重水素で標識された、トレーサレベルのPC(d9−トリメチル−塩化ジパルミトイルホスファチジルコリン[d9−PC])とともに、既存のPC源の形態で対象に提供された(治験薬適用免除のもと)。「PCチャレンジ」のそれぞれは、一晩(12時間)の空腹後のベースラインでの採血および無作為のランダム尿採取から構成される。ベースラインでは、対象は、ゼラチンカプセルに入った50mgのd9−PCと一緒に、黄身を含む2つの固ゆで卵(大型サイズ)(それぞれ、約250mgの総コリンと推定される)を提供され、10分以内に食べた。24時間の尿採取とともに、静脈血の連続サンプリングが、1,2,3,4,6および8時間の時点で実施された。提供された高精製d9(トリメチル)−PC(>98%アイソトープ濃縮)は、d3−ヨウ化メチル(Cambridge Isotopes Laboratories Inc, Andover MA)で完全メチル化後、1−パルミトイル、2−パルミトイル、sn−グリセロ−3−ホスホエタノールアミンから合成された。d9−PCは、分取薄層クロマトグラフィーおよび高速液体クロマトグラフィーにより単離され、結晶化および真空乾燥され、その精製度(>99%)は多核NMRおよび質量分析により確認された。
分析された血漿アリコートは、採取された全血からエチレンジアミン四酢酸チューブへ単離され、処理されるまで4時間以内、0〜4℃に維持され、−80℃に保存された。24時間尿採取からのアリコートは、回転させて任意の潜在的な細胞残渣を沈殿させ、上清を分析まで−80℃で保存した。TMAO、トリメチルアミン(TMA)、コリン、ベタインおよびそれらのd9−同位体物は、最近に記述されたように、内部標準としてd4(1,1,2,2)−コリン、d3(メチル)‐TMAOおよびd3(メチル)−TMAを用いて、オンラインエレクトロスプレーイオン化タンデム質量分析(LC/ESI/MS/MS)法で安定同位体希釈HPLCを用いて定量化された10。血漿中のTMAの測定のために、−80℃で保存する前に、試料を酸性化した(最終濃度10mMの塩酸)。尿中のTMAO濃度は、尿クレアチニン濃度の分析により、尿希釈に応じて調整された。
群間の差異を検証するために、連続変数に対するスチューデントt検定およびWilcoxon順位和検定、ならびにカテゴリー変数に対するカイ二乗検定が用いられた。血漿TMAOレベルは、分析のために四分位に分けられた。イベントまでの時間分析のために、Cox比例危険回帰分析とカプランマイヤー分析を用いて、MACEに対する危険率(HR)および95%信頼区間(95%CI)が決定された。年齢、性別、収縮期血圧、糖尿病の既往歴、低密度および高密度リポタンパク質コレステロール、トリグリセリド、喫煙歴、プラスBMI、薬物治療、推定クレアチニンクレアランスならびに血漿hsCRPレベルを含む、従来の心臓リスクファクターに対する調整により、ロジスティック回帰分析を実施した。TMAOの含有により導入されたモデルパフォーマンスの改善が、正味再分類改善(NRI)インデックスを用いて評価された。c−統計が、ROC曲線下の領域を用いて算出された。MACE事象の3年予測確率が、Coxモデルから推定された。すべての分析は、R version 2.8.0(Vienna, Austria)を用いて実施された。P値<0.05(両側)は統計的有意差があるとみなした。
TMAOは、ヒトにおける食品中のPCの代謝物であり、腸内細菌叢は、TMAO形成において必須の役割を果たす。
1856年に発見されて以来、コリンおよびTMAO代謝は、動物およびヒトの両方において広い範囲で研究されてきた4,11,18,19,20。無菌マウスを用いた細菌の動物モデル研究により、PC/コリンに富んだ食事状況での、代謝産物TMAの形成およびTMAOへの変換を介した、アテローム性動脈硬化症における腸内細菌叢の役割が示されている10(参照により、その全体が本明細書に援用される)。動物において、腸内細菌叢がTMA/TMAOの産生に貢献していることは示されているが、ヒトにおいて、食品中のPC由来のTMAO作製における腸内細菌叢の関与は立証されていなかった。本実施例は、安定同位体トレーサ摂取研究の使用を通じ、ヒトにおける、食品中のPC由来のアテローム生成促進代謝物TMAOの生成を実証する。本実施例はさらに、経口広域スペクトラム抗生物質での腸内細菌叢の抑制および、抗生物質の中止後のTMAOの再獲得、腸内での再コロニー形成を介した、ヒトにおける、TMAO産生での腸内細菌叢の役割を実証する。最後に、本実施例は、この腸内細菌叢が産生した代謝産物の潜在的な臨床予後的意義を、第一予防対象および積極的なLDLコレステロールまたはアポリポタンパク質B目標を有する対象の両方を含む、複合的に低いリスクの亜群内で、空腹時血漿TMAOレベルが、従来的な心血管系リスクファクターに依存しないMACEの将来的な発症を予測することを示すことにより、実証する。本発見は、ヒトにおけるアテローム性動脈硬化CADの病態生理学における腸内細菌叢依存性経路(複数含む)の重要な貢献を指摘し、抗生物質治療がCADおよび関連病態(たとえば、血栓症)の治療または予防に有用であることを示す。

対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記識別が、血小板凝集の増加を示す前記対象由来の試料で実施された血小板凝集分析の結果を見ることを含むことを特徴とする
対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記識別が、TMAOレベルの上昇を示す前記対象由来の試料または呼気で実施されたTMAO分析の結果を見ることを含むことを特徴とする
対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記ジメチルブタノールは、コリン輸送を損うことを特徴とする
対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記ジメチルブタノールは、対象に投与された場合に、以下:
トリメチルアミンレベルの減少、TMAOレベルの減少、総コレステロールレベルの減少、LDLレベルの減少、HDLレベルの増加、トリグリセリドレベルの減少、グルコー
スレベルの減少、およびヘモグロビンA1Cレベルの減少、
のうち1つ以上を誘導することを特徴とする
対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記ジメチルブタノールは、対象に投与された場合に心血管系疾患のリスクを減少させることを特徴とする
対象由来の試料で実施された血小板凝集分析および/またはトリメチルアミンN−オキシド(TMAO)分析に基づき、心血管系疾患および/または血栓症を患っており、血小板凝集が増加しているおよび/またはTMAOレベルが上昇していると識別された前記対象に、心血管系疾患および/または血栓症の治療のために投与される請求項1に記載の医薬組成物。 - 前記ジメチルブタノールは、対象に投与された場合に血栓症のリスクを減少させることを特徴とする

心血管系疾患および/または血栓症を患っており、血小板凝集レベルおよび/またはトリメチルアミンN−オキシド(TMAO)レベルを検出する試験を実施する対象由来の試料を得て、前記試料の血小板凝集および/またはTMAOの上昇したレベルの識別に基づき、心血管系疾患および/または血栓症の治療のために投与される請求項9に記載の医薬組成物。 - 前記投与が、前記心血管系疾患および/または前記血栓症の少なくとも一つの症状が減少または除去されるような条件下で行われることを特徴とする
心血管系疾患および/または血栓症を患っており、血小板凝集レベルおよび/またはトリメチルアミンN−オキシド(TMAO)レベルを検出する試験を実施する対象由来の試料を得て、前記試料の血小板凝集および/またはTMAOの上昇したレベルの識別に基づき、心血管系疾患および/または血栓症の治療のために投与される請求項11に記載の医薬組成物。 - プレバイオティック、プロバイオティックおよび/またはファイバーと共に投与されるものであることを特徴とする
心血管系疾患および/または血栓症を患っており、血小板凝集レベルおよび/またはトリメチルアミンN−オキシド(TMAO)レベルを検出する試験を実施する対象由来の試料を得て、前記試料の血小板凝集および/またはTMAOの上昇したレベルの識別に基づき、心血管系疾患および/または血栓症の治療のために投与される請求項9に記載の医薬組成物。 - 心血管系疾患の治療を提供する一つ以上の薬剤と共に投与されるものであることを特徴とする
心血管系疾患および/または血栓症を患っており、血小板凝集レベルおよび/またはトリメチルアミンN−オキシド(TMAO)レベルを検出する試験を実施する対象由来の試料を得て、前記試料の血小板凝集および/またはTMAOの上昇したレベルの識別に基づき、心血管系疾患および/または血栓症の治療のために投与される請求項9に記載の医薬組成物。 - 前記一つ以上の薬剤が、腸内細菌叢を標的とする一つ以上の抗生物質であることを特徴とする
心血管系疾患および/または血栓症を患っており、血小板凝集レベルおよび/またはトリメチルアミンN−オキシド(TMAO)レベルを検出する試験を実施する対象由来の試料を得て、前記試料の血小板凝集および/またはTMAOの上昇したレベルの識別に基づき、心血管系疾患および/または血栓症の治療のために投与される請求項14に記載の医薬組成物。
