Description
本発明は、ハンチントン病の処置において使用するためのベクターに関し、前記ベクターは、コレステロール−24−ヒドロキシラーゼをコードする核酸を含む。
ハンチントン病(HD)は、グルタミンリピートの伸長によって引き起こされる最も頻繁に起こる神経変性疾病である。HDの主な臨床徴候は舞踏病、認知障害および精神疾患である。HDの遺伝は、完全浸透率を伴う常染色体優性である。HDに関与する突然変異、すなわちCAGリピート配列の不安定な伸長は、ハンチントン(htt)をコードするIT15遺伝子の5’末端部分に位置する。HDの1つの重要な特徴は、他の脳領域における突然変異タンパク質の同じような発現にも関わらず、特定の脳領域である線条体の脆弱性である(Roze et al., 2008a)。さらに、全ての神経細胞における突然変異Httの早期発現(Exp−Htt)は生まれてすぐにも関わらず、最初の症状および神経病理学的な特徴は、成人期、すなわち約40〜45歳で現れる。疾病の発症年齢は、罹患した対立遺伝子におけるCAGリピートの数に反比例する。一旦最初の症状が現れると、疾病は進行し、そして進行的に死に至る。1つの現在認められている仮説は、加齢の際の特定のシグナル伝達経路の変化が、特にまたは主に線条体ニューロンにおいて、Exp−Httにより誘導される分子変化を増加させるというものである。これまで、疾病の進行を減速させることを目的とした利用可能な療法は全くなく、結果として、HDは無情にも死へと進行する。従って、HD療法のための戦略を発見することが重要である。
本発明者らは、中枢神経系においてコレステロールの分解に関与する酵素であるCYP46A1が、HDの細胞モデルにおいて神経保護的であることを観察した。さらに、本発明者らは、R6/2トランスジェニックHDマウスモデルの線条体(疾病におけるより脆弱な脳構造)におけるCYP46A1mRNAの減少を観察した。

(b)被験体の中枢神経系(CNS)にベクターを送達して、それにより、前記ベクターがCNSにおける細胞に形質導入され、これによりコレステロール−24−ヒドロキシラーゼが、治療有効レベルで、形質導入された細胞によって発現される工程
を含む。
本発明の第2の目的は、治療有効量の本発明のベクターを含むハンチントン病の処置において使用するための薬学的組成物に関する。
実施例1:in vitroにおけるCYP46A1の効果
材料および方法
マウスの畜産:
トランスジェニックR6/2マウスおよびWT同腹仔を、明/暗サイクルの維持された温度の調節された部屋で飼育した。食物および水は自由に利用できるようにした。ヒトIT15遺伝子プロモーターの制御下で115〜150個のCAGを含むヒト突然変異HD遺伝子のエキソン1を発現する、R6/2マウス[B6CBA−TgN(HDエキソン1)62]を、卵巣の移植されたヘミ接合性の雌をそのバックグラウンド株B6CBAF1/Jの雄と交雑することによって、Jackson Laboratory (Bar Harbor, ME, USA)から得た。トランスジェニックマウスの遺伝子型同定は尾DNAにおいてPCRによって実施された。マウスを、12時間の明/暗サイクルで、そして食物および水は自由にとれるようにして集団で飼育した。実験は、標準的な倫理指針に従って実施された(1985年に改訂された米国国立衛生研究所の刊行物番号85〜23、および実験動物の管理と使用に関する欧州委員会指針の指令86/609/EEC)。
線条体、皮質および海馬を、6週令のWTおよびR6/2マウスから解剖した。全mRNAを、製造業者の説明書に従ってRNeasyミニキット(Qiagen)を使用して単離した。逆転写をSuperScript III (Invitrogen)を使用して実施した。粗の線条体、皮質および海馬から抽出された400ngの全mRNAを、逆転写(RT)キット(SuperScript III -Invitrogen)を用いての逆転写のための鋳型として使用した。
一次線条体ニューロンを、妊娠スイスマウス(Janvier, Le Genest Saint Isle, France)からの14日令の胚から解剖して取り出した。細胞の解離を1mLのトリプシンEDTAを用いて15分間かけて実施し、そして300μLのウシ胎児血清および30μLのDNaseIに1mLのComplete Neurobasal (NBC)(B27、抗生物質およびグルタミンの補充されたNeurobasal)を添加することによって停止させた。機械的解離および氷上での5分間のインキュベーションの後、上清を取り出し、そしてペレットを1mLのNBCに再懸濁した。線条体細胞の溶液を5分間、4℃、900rpmで遠心分離にかけ、そしてペレットをNBCに懸濁した。
7日間培養した後、4つのウェルプレート中で増殖させたニューロンを、リポフェクタミン(Invitrogen)を使用して、GFPとin frameにある25個(Htt)または103個(ExpHtt)の連続的なCAAまたはCAGリピートのいずれかを含むヒトHttの第1エキソンをコードするpcDNA構築物(HDF Resource Bank, UCLAによって提供)を用いて一過性にトランスフェクションした。指示された場合には、線条体ニューロンを、ヘマグルチニンタグとin frameにある野生型CYP46A1タンパク質(CYP46A1−Ha)または酵素活性を欠失したミスセンス突然変異(A1309C)に対応するHaタグ化突然変異バージョン(CYP46A1−mut)(Pr Aubourgからの贈り物)をコードするpcDNA構築物を用いて同時トランスフェクションした。トランスフェクションから3時間半後に、培地を除去し、そしてNBCと交換した。
線条体ニューロンを室温で40分間かけてPBS中2%パラホルムアルデヒドで固定し、そして15分間かけてリン酸緩衝食塩水(PBS)中0.3%トリトンを用いて透過処理し、そして室温で5分間かけて1×PBS、0.1%トリトンを用いて洗浄した。PBSを用いて洗浄した後、細胞を20分間かけて遮断緩衝液(PBS、0.1%トリトン、5%NGS)と共にプレインキュベーションし、そして室温で5分間かけてPBS、0.1%トリトン、1%NGSを用いて洗浄した。一次抗体(Covance社のモノクローナルマウス抗Ha抗体、希釈率1/1000)を4℃で一晩かけてPBS、0.1%トリトン、1%NGS中でインキュベーションした。5分間かけてPBS、0.1%トリトンを用いて3回洗浄した後、細胞を二次抗体(Alexa 568抗マウス抗体、PBS、0.1%トリトン、1%NGS中の希釈率1/1000)と共にインキュベーションした。PBS、0.1%トリトンを用いての3回の洗浄を、核の標識前に、ヘキスト染色(PBS中1/20000)を使用して5分間かけて実施し、その後、5分間かけてPBSを用いて3回洗浄した。細胞を、カバーガラス下に、Vectashield medium (Vector Laboratories)を使用して積載し、そして、Leica DM4000B蛍光顕微鏡(40倍率)を用いて分析した。凝集体の形成をGFPによるExp−Httの染色により評価し、そしてトランスフェクションから24時間後に定量した。ニューロン死滅をトランスフェクションから48時間後にヘキスト染色後に評価し、そして凝縮されたまたは断片化された核を含むニューロンを、死滅細胞としてスコアリングした。提示された結果は、3回の独立した実験の結果であり、そして100を超えるトランスフェクションされたニューロンを、各実験において分析した。
全てのデータをスチューデントt検定を使用して統計学的に分析し、そして全ての統計学的分析について比較間の差異がP<0.05の場合に有意であると判断した。

HDにおいてはコレステロール生合成の機能不全が記載されている(総説についてはValenza et al 2006)。Exp−Httは転写の調節不全(特にニューロンの生存および代謝に関与する遺伝子)を誘導することが知られているので、本発明者らは、CYP46A1mRNA発現の欠損が、少なくとも部分的には、コレステロールの調節解除を、そして従ってHDにおけるニューロンの機能不全を説明し得ると考えた。R6/2マウスは、HDのより一般的に使用され、そして最も特徴付けられたマウスモデルである。これらのトランスジェニックマウスは、13週令から16週令までに起こる、死亡するまで次第に発達する数々の挙動的および調節的変化を示す。本発明者らは、定量逆転写PCRを使用して、その野生型同腹仔と共に、6週令のR6/2マウスの脳組織におけるCYP46A1mRNA発現を測定した。各マウス株について、RNAを、大脳皮質、海馬および線条体から抽出した。CYP46A1mRNA発現は、野生型マウスと比較した場合にR6/2マウスの大脳皮質において僅かではあるが有意にダウンレギュレーションされていた(図1)。海馬においては有意な変化は全く認められなかった。R6/2マウスの線条体において、本発明者らは、野生型マウスの線条体と比較した場合に、CYP46A1mRNAの発現の強力な減少を発見した(−50%;図1)。
R6/2マウスの線条体において観察されたCYP46A1mRNA発現の減少は、HDの病変発生における可能性ある原因的な役割を示唆した。これに対処するために、本発明者らは、実験室(Garcia et al., 2004; Charvin et al., 2005, Deyts et al., 2009)でセットされた、Exp−Httによって誘導された線条体ニューロン機能不全の簡単なモデルシステムを使用した。このモデルシステムは、25個のポリグルタミン(Htt)または103個のポリグルタミン(Exp−Htt)ストレッチを有するHttの第1エキソンをコードするcDNA構築物を用いて一過性にトランスフェクションされた線条体ニューロンの一次培養液からなる。この細胞モデルにおいては、Exp−Httの過剰発現により、自発的な凝集体の形成および死滅が起こる。これらの2つのパラメーターをそれぞれ24時間後および48時間後に評価し得る。CYP46A1の可能性ある防御的な役割を分析するために、その野生型の完全長タンパク質(CYP46A1)または突然変異形(mut−CYP46A1)をコードするcDNAを、Exp−Httと共に同時トランスフェクションした。HttまたはExp−Htt発現はGFP標識により検出され、一方CYP46A1およびmut−CYP46A1はHa免疫標識後に検出された。同時トランスフェクションアッセイにおいて、全てのトランスフェクションされたニューロンがGFPおよびHaの同時発現を示した(図2)。
GFP−ExpHttをコードするプラスミドを用いて線条体ニューロンをトランスフェクションしてから24時間後、トランスフェクションされたニューロンの58%がExp−Httの凝集体を示した(図3)。GFP−Httを発現するニューロンにおいては凝集体は全く観察されなかった(データは示さず)。CYP46A1とExpHttの同時発現は、凝集体形成の強力かつ有意な減少を促進した(58%vs27.5%)(図3)。凝集体の僅かであるが有意な減少(58%vs47%)が、不活性なCYP46A1突然変異体で見られた。結論すると、CYP46A1の過剰発現は、Exp−Httによる凝集体形成から線条体ニューロンを防御する。
線条体ニューロンにおけるExpHttの発現は、Htt発現ニューロンと比較した場合、生存の有意な減少を誘導した(図4)。実際に、トランスフェクションから48時間後、Htt発現ニューロンの89%がトランスフェクション後に依然として生存していたが、一方、Exp−Htt発現ニューロンの僅か55%しか生存していなかった。mut−CYP46A1とExp−Httの同時発現は、線条体の死滅に対して防御しなかった。これに対して、Exp−HttおよびCYP46A1を同時発現した線条体ニューロンにおいてはニューロン保護が認められた(図4)。従って、Exp−Htt+CYP46A1でトランスフェクションされたニューロンにおいては生存ニューロンの比率は76%まで上昇したが、これに対してExp−Htt発現ニューロンのみでは55%、またはHtt発現ニューロンにおいては89%であった。従って、CYP46A1は、ExpHttによって誘導されるニューロン死滅に対して強力な防御効果を有する。
材料および方法
マウスにおけるAAVrh.10−GFPおよびAAVrh.10−CYP46A1−Haの線条体への送達
使用した2つのAAVrh10ベクターは、AAV2の逆位末端配列によって囲まれたCMV/β−アクチンハイブリッドプロモーターによって駆動される、gfpcDNA(AAVrh.10−GFP、またはヘマグルチニンタグとin frameにあるヒトCYP46A1cDNA(AAVrh.10−CYP46A1−Ha)からなる発現カセットを含む。両方のAAVrh10ベクターが、Pr Ronald G. Crystal laboratory (Department of Genetic Medicine, Weill Medical College of Cornell University, New York, New York, USA)において作製された。4週令の時にWTまたはR6/2マウスを、ケタミン/キシラジン溶液(ケタミン100mg/kg、Merial、Lyon、Franceおよびキシラジン10mg/kg、Bayer Health Care、Germany)の腹腔内注射によって麻酔し、そして慣用的なPC2(物理的封じ込めレベル2)の実験室中の脳定位固定装置(Kopf)に配置した。マウスは、線条体の両側に(A/P+0.5mm、M/L+/−2.1mm、D/V−3.35mm)投与されたAAVrh.10GFP(103vg/ml)またはAAVrh.10CYP46A1−Ha(1.6×1012vg/ml)または食塩水溶液(NaCl0.001%)を受け、注射容量は0.2μl/分の速度で2μlであった。手術後、マウスをPC2動物施設で72時間かけて回復させ、そしてPC1動物施設を有する表現型プラットフォームに移した。
WTマウス線条体へのAAVrh.10−GFPまたはAAVrh.10−CYP46A1−Haの送達から3週間後、動物をペントバルビタールナトリウムの過剰投与250mg/kg(Sanofi, Paris, France)によって麻酔し、5分間かけて25ml/分で蠕動ポンプを用いて送達された4%パラホルムアルデヒド溶液を用いて経心的に灌流した。その後、脳を同じ溶液中で一晩かけて後固定(postfix)し、そして4℃で保存した。冠状切片または矢状切片(30μm)をビブラトーム(Leica Microsystems, Rueil-Malmaison, France)を用いて切断し、そして−20℃で30%エチレングリコール、30%グリセロール、および0.1Mリン酸緩衝液を含む溶液中に維持し、その後、組織学的分析のための処理を行なった。
脳のAAVrh.10の形質導入を、WTマウスへのAAVrh.10−GFPベクターの送達から3週間後に決定した。TBS中で3回濯いだ後、脳切片をカバーガラス下にVectashield (Vector Laboratories)を用いて積載し、そしてGFP染色を、PCコンピューターに接続されたCCDカメラの装備された落射蛍光電動顕微鏡(Zeiss)を使用して、脳の吻側尾側の方向にかけて冠状脳切片上で分析した。線条体から放出された緑色蛍光の断層画像再構成をモザイク写真(Mastronarde, D.N., 1997)から、その後、IMODソフトウェア(Kremer et al, 1996)のお陰で3次元表示によって実施した。
脳におけるAAVrh.10の指向性を、WTマウスの線条体へのAAVrh.10CYP46A1Haベクターの送達から3週間後に研究した。特異的なニューロンおよびグリアマーカーを免疫組織化学法によって評価した。自由に浮遊している切片をTBS中で濯ぎ、そして15分間かけてTBS中0.2%トリトンX−100と共にインキュベーションした。3回濯いだ後、浮遊している切片を、TBS中10%NGSを用いて室温で1時間かけて飽和させた。その後、切片をTBS中で3回濯ぎ、そしてTBS1X−NGS5%中で4℃で一晩かけて一次抗体(Ha 1/500, Covance ; NeuN 1/200, AbCys Vector ; Olig2 1/1000, Millipore; GFAP 1/1000, Dako; Isolectine 1/1000, Chemicon)と共にインキュベーションした。その後、切片をAlexa Fluor二次抗体(1/1000, Invitrogen, Carlsbad, CA, USA)と共に室温で90分間インキュベーションした。TBS中で3回濯いだ後、組織切片をカバーガラス下にVectashield (Vector Laboratories)を用いて蛍光顕微鏡検査のために積載した。
握り締め試験およびロータロッドパフォーマンスを、WTおよびR6/2マウスへのAAVrh.10−CYP46A1−Haの送達または食塩水の注射から2週間後に試験した。握り締めスコアについては、マウスを6週令から11週令まで1週間に1回試験した。マウスを尾によって30秒間吊るし、そして以下の尺度に従って握り締め表現型を特定のレベルに等級づけた:0、握り締めは全くなし;1、前肢のみの握り締め;2、1回または2回の前肢および後肢の両方の握り締め;3、3回を超えるまたは5秒間を超える前肢および後肢の両方の握り締め。ロータロッドパフォーマンスについては、6週令から11週令までのマウスを、毎週毎に3日間連続して試験した。トレーニングトライアルを5週令時に実施したが、データは最終結果に含めなかった。毎日のセッションは、4RPMでの5分間のトレーニングトライアルを含んだ。少なくとも1時間後、マウスは、30分間のトライアル間の間隔で隔てられた加速速度(5分間で0から40RPM)での3回の5分間のトライアルで試験された。ロータロッドから落ちるまでの潜伏時間(latency)を記録した。5分間を超えてロータロッド上に留まるマウスを除去し、そしてその時間を300秒間としてスコアリングした。
2元配置分散分析、その後のボンフェローニ試験を、ロータロッドパフォーマンスの比較のために使用した(*、P<0.05;**、P<0.01、***P<0.001)。全ての統計学的な分析のために、比較間の差異をP<0.05の場合に有意であると判断した。
C.AAVrh.10ベクターを使用したin vivoにおけるCYP46A1の送達
C1.形質導入の効率
マウスの線条体においてCYP46A1を発現させるために、本発明者らはAAVrh.10ベクターを使用することを決定した。第1の工程において、本発明者らは、GFPをコードするcDNAを発現するAAVrh.10を使用した。AAVrh.10−GFPベクターを、in vivoにおいて線条体にマイクロインジェクションを使用して投与した(方法の章を参照)。GFPの発現を、注射から3週間後に矢状切片および冠状切片から分析した。本発明者らのパラメーターを使用して、GFPは、線条体にシナプス的に接続された2つの脳構造である淡蒼球および大脳皮質と共に、背側線条体の全ての伸長部分内において観察することができた(図5Aおよび5B)。GFPを発現している冠状切片の3次元表示を実施し、線条体の63%における、すなわち全ての背側線条体におけるGFPの発現を示した(図6Aおよび6B)。
その後、本発明者らは、同じ実験条件に従ってAAVrh.10−CYP46A1−Haベクター(図7A)を使用した(1.6×1012vg/mlにおける2μl)。このベクターによって誘導されるCYP46A1の発現は、Ha免疫細胞検出法を使用して明らかとなり、これは予期された通り、全ての背側線条体内(尾状核被殻−CpU)に見られた(図7B)。細胞レベルにおいては、本発明者らは、CYP46A1−Ha産物の細胞質内局在を発見した(図7C)。二重免疫細胞化学検出法によって評価したところ、AAVrh10ベクターは強いニューロンへの指向性を有していた。なぜなら、Haによる標識は完全に、ニューロンマーカーNeuNと重複していたからである(図7C〜D)。さらなる実験が、グリアマーカーを使用して、アストロサイト(GFAP)、アストログリア(イソレクチン)およびオリゴデンドロサイト(Olig2)について実施された。これらのどのマーカーもHaと同時標識されなかった(図8A〜C)。従って、合わせてこれらのデータは、AAVrh10−CYP46A1−Haベクターが厳密なニューロンへの指向性を有することを示す。
D1.行動の防御
HDのトランスジェニックマウスモデルである、150個のCAGリピートを有するヒトHTT遺伝子の第1エキソンを過剰発現するR6/2マウス、およびその野生型同腹仔を、この試験部分に使用した。両方の株に、誕生から4週間後にAAVrh.10−CYP46A1−Haベクターを、線条体内の両側に注射した(野生型マウスについてはn=6、そしてR6/2マウスについてはn=12〜13)。対照群に、同じ実験条件に従って食塩水溶液を注射した。

Claims (12)
- 治療上有効量のベクターを含む、ハンチントン病の処置において使用するための薬学的組成物であって、前記ベクターは、コレステロール−24−ヒドロキシラーゼをコードする核酸の完全配列を含む、前記薬学的組成物。
- 前記ベクターは配列番号2のアミノ酸配列をコードする核酸配列を含む、請求項1の薬学的組成物。
- 前記ベクターは配列番号1の配列を含む、請求項2の薬学的組成物。
- 前記ベクターは、アデノウイルス、レトロウイルス、ヘルペスウイルスおよびアデノ随伴ウイルス(AAV)ベクターからなる群より選択される、請求項1〜3のいずれかの薬学的組成物。
- 前記ベクターはAAVベクターである、請求項1〜4のいずれか記載の薬学的組成物。
- 前記ベクターは、AAV1、AAV2、AAV3、AAV4、AA5、AAV6、AAV7、AAV8、AAV9、AAV10である、請求項5記載の薬学的組成物。
