・一部の患者(左心室肥大、心筋梗塞後)または不整脈を伴う場合、この薬理学的刺激と投与方法は、多くの異なる種類の心臓の問題への適切な手段とされる心筋の抵抗を使い果たす。
・ 外来患者へのカテーテル検査が、局所冠動脈血管造影を可能にする放射線状の経路を通して可能である。
・ マイクロカテーテルは、全身への流出をもたらす大動脈への注入の代わりに入口部への注入を確保する。
・ 2つのカテーテルではなく1つのカテーテルであることは、時間の短縮となる。
・血管けいれんやカテーテルのよじれなど、施術時間の延長につながる、手続き的な問題のリスクを削減する。
・遠位の蛇行血管への到達を可能にし、注入ルーメンは圧力変換器に近接しているため狭窄への交差を高める。
・使用しやすい血流予備量比の向上と副作用からの開放、これは血流予備量比測定を多くの患者にとって標準的かつ日常的なケアとして確立するのに役立つ。
(1)ガイドワイヤの遠位部分に搭載された注入カテーテル、または患者の冠動脈内の遠位にあるその他のもの、とを結合させた圧力センサを配置する
(2)注入カテーテルの遠位末端が遠位圧力センサの近位(上流)に配置されるように冠動脈内の複合カテーテルを配置する
(3)それぞれの冠動脈の近位かつ遠位部分の血圧を測定する
(4)アデノシン、一酸化窒素またはウリジン三リン酸といった充血剤を、注入カテーテルを用いて冠動脈に注入する
(5)遠位かつ近位の圧力を測定する
(6)既知かつ測定された量の血流予備量比を方程式によって計算する

Claims (9)
- 動脈内の狭窄の存在により患者の動脈内の血流が制限されるか否かを決定する診断方法に使用されるキットであって、
患者の動脈内または静脈内へ充血剤を注入するように構成されたカテーテルと、
前記充血剤として規定された量のウリジン三リン酸と、
を含んでなることを特徴とするキット。 - ガイドカテーテルを更に含み、当該ガイドカテーテルを通して患者に注入カテーテルを挿入するように前記ガイドカテーテルが構成されていることを特徴とする、請求項1に記載のキット。
- 前記カテーテルは、動脈内に配置され、注入ルーメンを含む一または二以上のルーメンを備える複合カテーテルであって、
前記動脈内に前記ウリジン三リン酸を放出するように遠位端に配置された開口部と、遠位端を有するガイドワイヤと、当該ガイドワイヤの遠位端に搭載された第一流量/圧力センサと、を備え、
前記ガイドワイヤは前記一または二以上のルーメンのうちのひとつの中を可動な状態で挿入されていることを特徴とする、請求項1又は2に記載のキット。 - 前記複合カテーテルが、互いに隣り合う少なくとも二つの前記ルーメンを備え、
前記第一流量/圧力センサを備える前記ガイドワイヤが前記注入ルーメンとは異なるルーメン内に配置されていることを特徴とする、請求項3に記載のキット。 - 第二圧力センサを備え、当該第二圧力センサが患者の動脈への挿入に適合された前記ガイドカテーテルに接続されていることを特徴とする、請求項2に記載のキット。
- 前記第一流量/圧力センサが第一圧力センサであり、
第二圧力センサを備え、当該第二圧力センサが、前記第一圧力センサとの関係で、前記ガイドワイヤの近位に配置されていることを特徴とする、請求項3又は4に記載のキット。 - 前記第一圧力センサの近位となるように前記カテーテルに配置されたバルーンを更に含んでなることを特徴とする、請求項6に記載のキット。
- 前記カテーテルが、当該カテーテルの長手方向に前記ウリジン三リン酸を注入するように構成されていることを特徴とする、請求項1〜7のいずれか一項に記載のキット。
- 注入ポンプを更に含んでなることを特徴とする、請求項1〜8のいずれか一項に記載のキット。
