Description

アルツハイマー病(AD)における認知症と相関がある特徴的な神経病理学的病変および分子病変には、神経原線維変化、異栄養神経炎(dystrophic neuritis)、および神経網糸の形成をもたらす、過リン酸化およびポリユビキチン化された、tauのような微小管関連タンパク質の蓄積が含まれる。神経細胞骨格異常は、細胞および繊維の減少を伴う脳萎縮、ならびにシナプスの連絡切断に関連している。髄膜血管および皮質血管の壁、皮質神経網、ならびに神経細胞核周部の周囲および内部のアミロイドβ(Aβ)沈着の増加は、ADおよび正常な加齢の両方の特徴である。遺伝的要因により、個体は、AD型認知症におけるAβの時期尚早かつ過剰な脳沈着を発現しやすくなり得るが、大半の症例は孤発性であり、明らかな家族性のクラスタリングも遺伝的クラスタリングも提示しない。古典的ADに先行するか、または付随する生化学的異常、分子異常、および細胞異常の最近の検査によって、細胞脱落が、細胞死促進(pro-death)遺伝子およびシグナル伝達経路の活性化の増大、エネルギー代謝の障害、ミトコンドリア機能不全、長期的な酸化ストレス、ならびに脳血管疾患/脳の低灌流に関連していることが実証された。しかしながら、単一の主要な発病メカニズムのもとでこれらの現象を結び付けることができなかったため、大いに討論された様々な理論が発生および普及した。これらはそれぞれ、ADの1つの特定の構成要素が、他の公知の異常すべての発現をもたらすカスケードの引き金となり得る方法に焦点を合わせていた。しかしながら、以前の文献のいくつかを再評価することにより、脳のグルコース利用およびエネルギー代謝の機能障害が、認知機能障害の初期段階に先行するか、または付随する、極めて早期の異常に相当することが明らかになった。さらに、インスリンシグナル伝達の障害がADの病因において重要な役割を有し得るという証拠も出現しつつある。
ADとインスリン/インスリン様成長因子(IGF)シグナル伝達経路の関係は、AD患者の脳におけるインスリンおよびIGF発現の障害の発見によって実証された。インスリンおよびIGFシグナル伝達の下流の介在物質がAD患者において障害されていることも見出された。これらの調査結果により、診断目的および治療目的の両方に利用され得る、ADとインスリン/IGFシグナル伝達経路の関連が明確になる。

一般的方法
組織の供給源
Massachusetts General Hospital Alzheimer Disease Research Center brain bank、the Brown University Brain Bank、およびDuke University Medical Centerのthe Kathleen Price Bryan Brain Bankから死後脳を入手した。AD(BraakおよびBraak病期5〜6)ならびに正常な加齢(BraakおよびBraak病期0〜1)の診断は、臨床歴、ならびに、前頭前皮質、側頭皮質、扁桃体、および海馬のビールショースキー染色切片、ならびにリン酸化Tau、ユビキチン、およびアミロイドβによる免疫染色切片を含む、死後脳の組織病理学的切片を再検討することによって、確認した。(Braak et al., Neurobiol. Aging 18:S85 (1997); Nagy et al., Dement. Geriatr. Cogn. 製造業者のプロトコールに従ってTRIzol試薬(Invitrogen, Carlsbad, CA)を用いて、全RNAを脳組織から単離した。260nmおよび280nmで測定した吸光度からRNA濃度を決定した。AMV First Strand cDNA合成キット(Roche Diagnostics Corporation, Indianapolis, IN)およびランダムオリゴデオキシヌクレオチドプライマーを用いて、RNA(2μg)を逆転写した。リアルタイム定量RT-PCR増幅を用いて、インスリン、IGF-I成長因子、およびIGF-II成長因子、対応するそれらの受容体、インスリン受容体基質(IRSサブタイプ1、2、および4、Tau、アミロイド前駆タンパク質(APP)、グルコース輸送担体4(GLUT4)、ならびにインスリン分解酵素(IDE)のmRNAレベルを測定した。並行反応において測定した18SリボソームのRNAレベルを用いて、各mRNA転写物の相対存在量を算出した。(Xu et al., J. Biol. Chem. 278:26929 (2003); Yeon et al., Hepatology 38:703 (2003))。
ウェスタンブロット解析を用いて、Akt、ホスホ-Akt、GSK-3β、ホスホ-GSK-3β、Tau、およびβ-アクチンのレベルを評価した。プロテアーゼ阻害剤(1mM PMSF、0.1mM TPCK、1μg/mlアプロチニン、1μg/mlペプスタチンA、0.5μg/mlロイペプチン、1mM NaF、1mM Na4P2O7)およびホスファターゼ阻害剤(2mM Na3VO4)を含む、5体積量の放射性免疫沈降アッセイ法(RIPA)緩衝液(50mM Tris-HCl、pH7.5、1%NP-40、0.25%デオキシコール酸Na、150mM NaCl、1mM EDTA、2mM EGTA)中で、新鮮な凍結組織(約100mg)をホモジナイズした。ビシンコニン酸(BCA)アッセイ法(Pierce, Rockford, IL)を用いて、タンパク質濃度を決定した。タンパク質100μgを含む試料をドデシル硫酸ナトリウム、ポリアクリルアミドゲル電気泳動法(SDS-PAGE)によって分画した。(Ausubel et al., Current Protocols in Molecular Biology (2000))。タンパク質をImmobilon-P (Millipore Corporation, Bedford, MA)PVDFメンブレンに移し、かつ非特異的結合部位をSuperBlock-TBS (Pierce, Rockford, IL)に吸着させた。1%ウシ血清アルブミンおよび0.05%Tween-20(TBST-BSA)を含むTris緩衝生理食塩水(TBS;50mM Tris、150mM NaCl、pH7.4)中に希釈した1次抗体(0.5〜1μg/ml)と共に、4℃で一晩、メンブレンをインキュベートした。西洋ワサビペルオキシダーゼ(HRP)結合IgG(Pierce, Rockford, IL)、Western Lightning化学発光試薬(Perkin Elmer Life Sciences Inc., Boston, MA)、およびフィルムオートラジオグラフィーを用いて、免疫反応性を検出した。すべてのインキュベーションは、プラットホーム上で穏やかに攪拌して、実施した。Kodak Digital Science Imaging Station (NEN Life Sciences, Boston, MA)を用いて、免疫反応性を定量した。
免疫沈降法による研究を用いて、PI3キナーゼのp85サブユニットとインスリン受容体基質(IRS)1型および2型の相互作用を検査した。組織試料は、プロテアーゼ阻害剤およびホスファターゼ阻害剤(1μg/mlアプロチニン、0.5μg/mlロイペプチン、1mM PMSF、0.1mM TPCK、1μg/mlペプスタチンA、2mMバナジン酸ナトリウム)を含むRIPA緩衝液中でホモジナイズし、かつ、免疫沈降法アッセイ法で使用する直前に、10mM HEPES、100mM NaCl、1mM EDTA、および0.1%Triton X-100を含むHEPES溶解緩衝液中で希釈した。前処理後、タンパク質250μgを含む試料を、4℃で2時間、常時回転させながら1次抗体と共にインキュベートした。穏やかに回転させながら4℃で2時間インキュベーションすることによって、UltraLink固定化プロテインA/G (Pierce, Rockford, IL)上で免疫複合体を捕捉した。Hepes溶解緩衝液0.5ml中で3回、免疫沈降物を洗浄し、次いでキナーゼアッセイ法でそれらを使用した。(Ausubel et al., Current Protocols in Molecular Biology (2000))。
視床下部および側頭新皮質または前頭新皮質の緩衝化したホルマリン固定パラフィン包埋切片(厚さ8μM)を、アビジンビオチン西洋ワサビペルオキシダーゼ法および色素原としてのNovaRedまたはジアミノベンジジン(Vector Laboratories, Burlingame, CA)のいずれかを用いて、インスリン受容体、IGF-I受容体、インスリン、およびIGF-Iに対する抗体で免疫染色した(de la Monte et al., Lab. Invest. 80:1323 (2000))。ヘマトキシリンで切片を対比染色し、光学顕微鏡によって検査した。
インスリン受容体、IGF-I受容体、IRS-1、IRS-2、およびPI3キナーゼのp85サブユニットに対する抗体は、Cell Signaling (Beverly, MA)から入手した。GSK-3β、Aktに対する抗体、ならびにGSK-3βおよびAktに対するホスホ特異的抗体は、Upstate Biotechnology (Lake Placid, NY)から購入した。プロテインA/Gアガロースは、Pierce Chemical Company (Rockford, IL)から入手した。免疫組織化学的染色用の試薬は、Vector Laboratories, (Burlingame, CA)から購入した。他のすべてのファインケミカルは、CalBiochem (Carlsbad, CA) またはSigma-Aldrich (St. Louis, MO)のいずれかから購入した。
グラフ中に示すデータは、各群の平均値±標準誤差を表す。スチューデントのt検定または事後のTukey-Kramer有意差検定を伴う反復測定分散分析(ANOVA)を用いて、群間比較を実施した。ナンバークランチャー統計システム(Number Cruncher Statistical System)(Dr. Jerry L. Hintze, Kaysville, UT)を用いて、統計学的解析を行った。コンピュータソフトウェアによって得られたP値をグラフ中に示す。P値0.05未満を統計学的に有意とみなした。
ADにおける成長因子受容体発現の減少
リアルタイム定量RT-PCR研究により、対照脳およびAD脳の両方の大脳皮質、海馬、および視床下部中のインスリン受容体、IGF-I受容体、およびIGF-II受容体に対応するmRNA転写物が示された(図1)。インスリン受容体、IGF-I受容体、およびIGF-II受容体は、前頭皮質より、海馬および視床下部において400倍〜2000倍高いレベルで発現されていた。IGF-I受容体およびIGF-II受容体は、全般的に、インスリン受容体より豊富に発現されており、かつ、対照脳では、IGF-I受容体の方が、IGF-II受容体より豊富に発現されていた。ADにおいて、IGF-IおよびIGF-IIのmRNA転写物は、対照群でもそうであったように、IGF-I受容体がIGF-II受容体より高いレベルで発現されていた前頭皮質以外では、同様のレベルで発現されていた。インスリンおよびIGF-Iの受容体発現レベルは、対照の前頭皮質、海馬、および視床下部において、AD脳の対応する領域より有意に高かったが、IGF-II受容体mRNAの平均レベルは、対照試料およびAD試料において同様であった(図1)。領域による差異および群間差異を目立たせるためにインスリン受容体のデータを再プロットすると、海馬および視床下部におけるインスリン受容体およびIGF-I受容体のmRNA転写物の平均レベルが、ADにおいて、対照脳の対応する領域より8倍〜10倍低いことが明らかになったが、前頭皮質においては、インスリン受容体およびIGF-I受容体のmRNA転写物がADにおいて約40%減少していたため、群間差異はずっと小さかった(図1D)。
AD脳における局所的な成長因子発現の減少
リアルタイム定量RT-PCR研究により、高齢の対照脳およびAD脳における、インスリン、IGF-I、およびIGF-IIのmRNA発現を検出した。最も高レベルの成長因子発現は、海馬および視床下部において観察され、これらの平均レベルは、前頭皮質より30倍〜50倍高かった(図2)。インスリン遺伝子発現は海馬において最も高かったが、前頭皮質においては検出不可能であった。IGF-I mRNA発現は、視床下部または前頭皮質より、海馬において10倍〜30倍多かった。IGF-II mRNAは、海馬および視床下部において同様に高いレベルで発現され、両方とも、前頭皮質と比べて約40倍高かった。領域に基づくデータ再解析により、海馬において比較的高レベルのインスリンおよびIGF-II、ならびに視床下部および前頭皮質におけるIGF-II>IGF-I>>>インスリンの発現が実証された(図2)。
AD脳における神経細胞のインスリン、IGF-I、インスリン受容体、およびIGF-I受容体の免疫反応性の低下
14例のAD脳および10例の対照に由来する前頭皮質、海馬、および視床下部のホルマリン固定パラフィン包埋切片を免疫組織化学的染色することによって、インスリン、IGF-I、および対応する受容体の細胞分布を検査した。インスリンポリペプチドまたはIGF-Iポリペプチドに対応する免疫反応性が、神経細胞および神経突起において観察されたのに対し、インスリン受容体およびIGF-I受容体は、実質血管および軟膜血管の両方の神経細胞、神経網神経突起、グリア、および平滑筋細胞において発現されることが発見された。リアルタイム定量RT-PCRの結果に一致して、インスリン陽性神経細胞、IGF-I陽性神経細胞、インスリン受容体陽性神経細胞、およびIGF-I受容体陽性神経細胞は、正常な高齢の対照海馬試料と比較して、ADにおいて量が少なかった(図3)。AD症例における神経細胞標識の減少は、神経細胞の脱落、ならびに成長因子および対応する成長因子受容体の神経細胞による発現の減少に起因していた。後者は、組織学的に完全な外観の神経細胞において観察された陰性の免疫染色反応から明らかであった(図3)。一方、血管中の成長因子受容体標識の程度は、AD試料および対照試料において同様であった。
初代培養神経細胞中のインスリン、IGF-I、IGF-II、および対応する受容体のmRNA転写物の検出
CNSにおける神経細胞成長因子および成長因子受容体の発現の発見を確認するために、ラット遺伝子に特異的なプライマー(表2)を使用するリアルタイム定量RT-PCRにより、培養したCNS神経細胞中の同じmRNA転写物のレベルを測定することによって、調査を拡大した。以前に説明されているように、胎児ラットの大脳皮質、視床下部、および海馬、ならびに生後ラットの小脳顆粒神経細胞から初代培養神経細胞を作製した(de la Monte et al., Cell. Mol. Life Sci. 58:1950 (2001); de la Monte et al., Cell. Mol. Life Sci. 59:882 (2002); Xu et al., J. Biol. Chem. 278:26929 (2003); Chen et al., J Alzheimers Dis. 5:209 (2003); Nillni et al., Endocrinology 137:5651 (1996))。採取時、これらの神経細胞は有糸分裂後であり、かつ分化細胞に特徴的なプロセスを多数有していた。リアルタイム定量RT-PCR研究により、培養された神経細胞におけるインスリン、IGF-I、IGF-II、インスリン受容体、IGF-I受容体、およびIGF-II受容体のmRNA転写物の発現が実証された(図4)。インスリン受容体、IGF-I受容体、およびIGF-II受容体は、大脳に由来する神経細胞と比較すると、小脳顆粒神経細胞において著しく高いレベルで発現された(図4A〜4C)。大脳構成物のうちで、インスリン受容体およびIGF-I受容体は、皮質神経細胞において、より高いレベルで発現され、次に視床下部神経細胞が続き、海馬神経細胞のインスリン受容体およびIGF-I受容体発現レベルは最も低かった。皮質培養物、海馬培養物、および視床下部培養物において、IGF-II受容体発現は同様に低レベルであったが、皮質神経細胞において最も高レベルであった。成長因子受容体発現の領域による差異を目立たせるためにデータをさらに解析することにより、小脳においては、インスリン受容体発現が最も大量であり、続いてIGF-I受容体、次いでIGF-II受容体の順であるのに対し、皮質および視床下部の神経細胞においては、受容体の存在量の順序は、IGF-I>IGF-II>インスリンであることが実証された(図4Dおよび4E)。海馬神経細胞においては、IGF-II受容体のmRNAが最も大量であり、続いてインスリン受容体、次いでIGF-I受容体の順であった。
インスリン/IGF-I受容体の下流の重要なシグナル伝達分子の解析
インスリンおよびIGF-Iは、細胞表面受容体へのリガンド結合および付随する内因性受容体チロシンキナーゼの活性化によって開始される複雑な細胞内シグナル伝達経路を活性化することによって、それらの作用を媒介する(Ullrich et al., Nature 313:756 (1985); Myers et al., Trends Biochem. Sci. 19:289 (1994); O'Hare et al., Int J Biochem 22:315 (1990))。インスリン/IGF-I受容体チロシンキナーゼは、IRS分子をリン酸化する。(Myers et al., Trends Biochem. Biol. 13:7418 (1993))。チロシルリン酸化IRS-1(PY-IRS-1)は、IRS-1のC末端領域に位置する特異的なモチーフを介して、src-ホモロジー2(SH2)を含む下流の分子と相互作用し、それに付随してErk MAPKおよびPI3キナーゼ/Aktを活性化し、かつGSK-3βを阻害することによって、成長、代謝機能、および生存を媒介する細胞内シグナルを伝達する。(Giovannone et al., Diabetes Metab. Biol. 17:595 (1997); Dudek et al., Science 275:661 (1997); Burgering et al., Nature 376:599 (1995); Delcommenne et al., Proc. Natl. Acad. Sci. USA 95:11211 (1998); Kido et al., J. Clin. Endocrinol. Metab. 86:972 (2001))。Aktキナーゼは、GSK-3βおよびBADをリン酸化して、それらを不活性状態にすることによって、アポトーシスを抑制する。(Delcommenne et al., Proc. Natl. Acad. Sci. USA 95:11211 (1998); Datta et al., Cell 91:231 (1997); Kennedy et al., Mol. Cell. Cell. Biol. 19:5800 (1999); Brunet et al., Cell 96:857 (1999))。ミトコンドリア膜透過性の混乱により、ミトコンドリアDNAの損傷を引き起こし、ミトコンドリアの機能に障害を与え、かつプロアポトーシスのカスケードを活性化する、細胞のフリーラジカルが増加し得る。(Jaeschke et al., Toxicol. Sci. 65:166 (2002); Pastorino et al., J. Biol. Chem. 273:7770 (1998))。
ADの進行中の成長因子受容体発現の減少
AD重症度が様々な程度である死後脳組織を検査することによって、成長因子受容体発現の減少をさらに解析した。前前頭皮質から得た急速凍結組織(各約100mg)を使用して、RNAおよびタンパク質を抽出した。試料を以下の4群に分けた:対照(Braak0〜1)、Braak2〜3、Braak4〜5、およびBraak6。リアルタイム定量RT-PCR研究により、対照脳およびAD脳の両方から得た前頭皮質中のインスリン受容体、IGF-I受容体、およびIGF-II受容体に対応するmRNA転写物が示された(図X)。Braak0〜1の症例のうちでは、IGF-I受容体のmRNA転写物が最も大量であり、IGF-II受容体遺伝子よりほぼ10倍多く、インスリン受容体より500倍多かった。Braak病期/ADの神経変性の重症度が進行するにつれ、インスリンおよびIGF-Iの受容体mRNAの平均レベルは低下し、かつ、疾患重症度がBraak2〜3である脳においてさえ、対照より有意に低かった(図9Aおよび9B)。インスリンおよびIGF-Iの受容体発現の平均レベルの最低値は、ADのBraak6の脳において観察された。したがって、インスリンおよびIGF-Iの受容体mRNAの平均レベルは、Braak4〜5およびBraak6に比べてBraak2〜3群において有意に高かった。IGF-II受容体発現は、対照と比べて、AD群のいずれにおいても有意に変化していなかった(図9C)。
ADの進行中の成長因子発現の減少
リアルタイム定量RT-PCR研究により、高齢の対照脳およびAD脳中のインスリン、IGF-I、およびIGF-IIポリペプチドのmRNA転写物を検出した(図10A〜10C)。Braak0〜1の脳においては、IGF-II mRNAのレベルが最も高く、続いてインスリン、次いでIGF-Iの順であった。インスリンおよびIGF-II両方の遺伝子発現の著しく、かつ有意な減少が、Braak病期2〜3の症例において観察され、かつ、これらのレベルは、ADの重症度が進行するにつれさらに低下したが、Braak2〜3と比べて有意には低下しなかった(図10Aおよび10C)。IGF-I mRNA発現は、Braak2〜3群ではわずかしか減少しなかったが、Braak4〜5群およびBraak6群の両方においては、対照と比べて実質的かつ有意に減少した(図10B)。
ADの進行中のTauおよびアミロイド前駆タンパク質の発現変化
リアルタイムRT-PCR研究により、tau mRNA転写物が、Braak0〜1の対照症例において最も大量であり、かつ、これらのレベルは、Braak病期、すなわちAD神経変性の重症度が進行するにつれ、徐々に、かつ有意に低下し(図11A)、インスリンおよびIGF-Iポリペプチド遺伝子に関して観察された傾向と一致していることが発見された。Braak病期0〜1群のAPP mRNA平均レベルが最も低かった。Braak病期2またはそれ以降の脳において、APP mRNAレベルは同様に上昇しており、対照より約4倍高かった(図11B)。
ADの進行中の成長因子受容体へのリガンド結合の解析
インスリンおよびIGF-Iは、対応する細胞表面受容体へのリガンド結合によって開始される複雑な細胞内シグナル伝達経路を活性化することによって、それらの作用を媒介する。したがって、効果的なリガンド結合はシグナル伝達カスケードに極めて重要であり、かつ、神経細胞生存の減少、GSK-3β活性化の増大、およびtauリン酸化の増加を含む、ADに罹患した脳において既に確認されているインスリンシグナル伝達障害の下流の影響のうちの多くは、CNSにおけるインスリン結合の減少によって媒介され得る。成長因子シグナル伝達のこの特徴を検査するために、トレーサーとしての[125I]標識したインスリン、IGF-I、またはIGF-II、および受容体供給源としての死後の前頭葉組織の膜抽出物を用いて、競合的な平衡および親和性結合アッセイ法を実施した。

膜コレステロール含有量は、細胞表面受容体へのリガンド結合に影響し得る。例えば、膜中のコレステロール含有量の減少または増加は、成長因子結合およびシグナルトランスダクションの変化および障害に関連付けられている。観察された受容体結合親和力の差異が、膜コレステロール含有量と相関していたかどうかを判定するために、Amplex Redアッセイキット(Molecular Probes, Eugene, Oregon)を製造業者のプロトコールに従って用いて、前頭葉抽出物中のコレステロールレベルを測定した。手短に言えば、前述したように、RIPA緩衝液中で組織ホモジネートを調製した。これらの試料を1×反応緩衝液(キットと共に供給される)中で段階的に希釈し、かつ、150μM Amplex Red試薬、1U/ml西洋ワサビペルオキシダーゼ、1U/mlコレステロールオキシダーゼ、および0.1U/mlコレステロールエステラーゼと共に、最終反応体積100μl中でインキュベートした。反応物を37℃で30分間インキュベートし、かつ、Fluorocountマイクロプレートリーダー(Packard Instrument Co., Meriden, CT) (Ex 560nm/Em 590nm)中で蛍光測定した。キットと共に提供されるコレステロール標準物質を用いて、同時に検量線を作成した。試料中のタンパク質濃度に対してコレステロールのレベルを標準化した。予備研究により、測定されるコレステロールレベルおよび群間差異は、製造業者によって示されるように、RIPA緩衝液抽出物と比較して脂質抽出物において同じであることが実証された。したがって、脂質抽出物を用いて分析を実施する必要はなかった。これらの研究により、Braak病期0〜1または2〜3の脳に対して、Braak病期4〜5または6の脳において、コレステロールのレベルが有意に上昇していることが実証された(図14A)。平均コレステロールレベルは、ADのBraak病期4〜5およびBraak病期6の脳において同様であった。
ADの進行中のATPレベルの変化
インスリンおよびIGF-Iのシグナル伝達の障害は、ミトコンドリアの機能、エネルギー代謝、およびATP生成の低減をもたらし得る。ADにおけるインスリンおよびIGF-I機能の障害の影響を調査するために、ATPLiteアッセイシステム(Perkin Elmer, Boston, MA)を用いて、前頭皮質ホモジネート中の定常状態のATPレベルを測定した。溶解物を段階希釈し、かつ溶解物150μl当たりATP基質50μlを添加した。急速凍結した脳組織試料を、20mMグリシン、50mM MgSO4、および4mM EDTAを含む3体積量のPBS中でPolytron(Glen Mills Inc., Clifton, New Jersey)によってホモジナイズした。100μlのアリコートを96ウェルのブラックプレートに移し、かつATPLite溶解緩衝液50μlを各試料に添加した。これらのプレートを接着性のプラスチックシートで覆い、かつ、700rpm、室温で5分間、攪拌した。次いで、ATPLite基質50μlを各試料に添加し、かつ、シールしたプレートをアルミニウム箔で覆い、(700rpm、室温で)さらに5分間攪拌した。TopCount機(Packard Instrument Co., Meriden, CT)で発光を測定し、かつATP発光値をタンパク質濃度に対して標準化した。これらの研究により、AD(全群)において、対照脳に比べてATPのレベルが有意に低下(約50%)していることが実証された(図14B)。ADでは、平均ATPレベルは一貫して低く、かつ、神経変性の進行に伴う有意な低下も、神経変性の重症度に伴う有意な低下も起こらなかった。
アルツハイマー病の診断的アッセイ法
ADに起因する病変を呈しているか、またはADに起因する病変を呈するリスクがある被験者を、ADの診断のためにスクリーニングする。各被験者から脳組織の試料を採取する。次に、これらの試料からRNAを単離し、かつ上記の実施例2および3において前述したようにリアルタイム定量RT-PCRに供して、脳組織中のインスリン、IGF-I、IGF-II、インスリン受容体、IGF-I受容体、およびIGF-II受容体の発現レベルを決定する。次いで、測定した発現レベルを、年齢を一致させた健常被験者における発現レベルと比較する。測定した因子のうち2つまたはそれ以上の測定した発現レベルが、健常被験者の発現レベルより少なくとも2倍低いことが判明した場合、試験被験者は、ADに罹患しているとみなされる。
アルツハイマー病の動物モデル
以前の研究において、脳内ストレプトゾトシン(ic-STZ)処置を用いて、成体ラットにおけるAD型神経変性のモデルが作製された。Plaschke et al., Int. J. Dev. Neurosci. 11:477 (1993); Duelli et al., Int. J. Dev. Neurosci. 12:737 (1994); Hoyer et al., J. Neural Transm. Suppl. 44:259 (1994); Lannert et al., Behav. Neurosci. 112:1199 (1998)。STZの化学名は、2-デオキシ-2{[メチル-ニトロソアミノ)カルボニル]アミノ}D-グルコピラノース(C8H15N3O7)であり、分子量は265ダルトンである。STZは、代謝された場合に、膵島中のβ細胞を優先的に破壊し、かつ糖尿病を生じさせる細胞毒性生成物を生じる、グルコサミン-ニトロソ尿素化合物である。細胞毒性の正確なメカニズムは理解されていないが、STZ代謝生成物のアルキル化特性により、反応性の酸素種が生成し、かつ、酸化ストレスおよびDNA損傷が引き起こされる。このような影響があるため、神経変性のモデルを作製するために、脳室内STZが使用されるようになった。成体ラットにおいて、STZを脳室内注射すると、大脳皮質および海馬におけるグルコースおよびグリコーゲンの代謝の長期的な低下(10〜30%)が引き起こされる。Plaschke et al., Int. J. Dev. Neurosci. 11:477 (1993)。これらの影響は、脳の酸化的代謝の有意な低下(Duelli et al., Int. J. Dev. Neurosci. 12:737 (1994))、インスリン受容体機能の阻害(Hoyer et al., Ann. NY Acad. Sci. 920:256 (2000))、および学習、記憶、認知行動、脳のエネルギー平衡の進行性の欠損(Lannert et al., Behav. Neurosci. 112:1199 (1998); Hoyer et al., Ann. NY Acad. Sci. 893:301 (1999)に関連付けられている。したがって、このモデルは、ADにおいて発生する生化学的異常および生理学的異常との少なくとも部分的な一致をもたらす。しかしながら、以前の研究では、インスリンおよびIGF-1のシグナル伝達に関係する神経病理学、分子病理学、遺伝子発現の異常も、膵臓の構造および膵臓におけるインスリン発現が損なわれていないことも、明らかにされなかった。
3日齢のLong Evans仔ラットの脳の両側にSTZを脳内(ic)注射した。30ゲージの針を取り付けたハミルトンマイクロリッターシリンジを用いて、ブレグマから1.0mm後方および1.0mm外側、かつ各半球の頭蓋骨表面から深さ2.5mmに、STZを注射した。対照ラットには、無菌生理食塩水を同様に注射した。最初の研究では、糖尿病のモデルを作製するために以前に報告されているように、5mg/kg〜70mg/kgの範囲の様々な用量のSTZの影響を評価した。Andican et al., Clin. Exp. Pharmacol. Physiol. 32:663 (2005); Saad et al., Arch. Toxicol. (2005); Srinivasan et al., Pharmacol. Res. 52:313 (2005); Karabatas et al., Pancreas 30:318 (2005); Mabley et al., Pancreas 28:E39 (2004)。予備研究により、試験したすべての用量でのSTZによる神経変性が実証されたが、安定した結果は、少なくとも25mg/kgによって実現された。本明細書において示す結果は、40mg/kgのic-STZ処置した仔ラットから得られた。注射は3分以内に完了し、かつ、針は脳からゆっくりと引き抜いた。すべての仔ラットはすぐに回復し、したがって、直ちに母ラットのもとに戻され、母ラットによって100%受け入れられた。注射処置の精度は、皮質下の白質中および側脳室内に局在化して見出されるメチレンブルー色素を注射することによって確認した。すべての動物が注射後に生き残った。STZ処置または生理食塩水処置後7日目、14日目、または21日目に屠殺するまで、これらの動物を毎日モニターした。
パラフィン包埋した膵臓の組織学的切片(厚さ5μm)および脳の組織学的切片(厚さ8μm)を、ヘマトキシリンおよびエオシン(H&E)で染色し、かつ、組織病理学的病変、すなわち、炎症、ネクローシス、および膵島細胞変性について検査した。膵臓の隣接切片を免疫染色して、膵島におけるインスリン免疫反応性を検出した。ホスホtau、Aβ、p53、ユビキチン、グリア線維性酸性タンパク質(GFAP)、コリンアセチルトランスフェラーゼ(ChAT)、およびアセチルコリンエステラーゼ(AChE)に対するモノクローナル抗体またはポリクローナル抗体で脳のパラフィン切片を免疫染色して、ic-STZによって誘発されるAD型の神経変性の性質を特徴付けた。免疫染色反応に関する陰性対照として、1次抗体を除外するか、またはB型肝炎ウイルスに対する無関係なモノクローナル抗体を、関連する抗体の代わりに使用した。
前述したような、かつ表6に示すプライマーを使用するリアルタイム定量RT-PCR増幅によって、インスリン、IGF-I成長因子、およびIGF-II成長因子、対応するそれらの受容体、インスリン受容体基質(IRS)サブタイプ1、2、および4、tau、アミロイド前駆タンパク質(APP)、AChE、ならびにChATのmRNAレベルを測定した。さらに、ADの以前の研究において説明されたic-STZによる神経変性に関連した、細胞型における病理学的変化を検出するための研究も実施した。Steen et al., J Alzheimers Dis 7:63 (2005)。手短に言えば、表6に示すHu(神経細胞)、GFAP(星状細胞)、ミエリン関連糖タンパク質(MAG-1;乏突起膠細胞)、およびアログラフト炎症因子-1(AIF-1;小膠細胞)のmRNA転写物を検出するように設計された遺伝子特異的プライマー対を用いて、リアルタイム定量RT-PCRを実施した。
ic-STZ処置により、脳におけるインスリン、IGF-I、およびIGF-IIの受容体結合が障害されるかどうかを判定するために研究を実施した。プロテアーゼ(1mM PMSF、0.1 mM TPCK、1μg/mlアプロチニン、1μg/mlペプスタチンA、0.5μg/mlロイペプチン、1mM NaF、1mM Na4P2O7)およびホスファターゼ阻害剤(2mM Na3VO4)を含む、5体積量のNP-40溶解緩衝液(50mM Tris-HCl、pH7.5、150mM NaCl、1mM EDTA、2mM EGTA、1%NP-40)中でPolytron(Glen Mils Inc., Clifton, New Jersey)によってホモジナイズすることによって、新鮮な側頭葉凍結組織(約100mg)から膜タンパク質を抽出した。10,000×g、4℃で15分間、試料を遠心分離した後に得られた上清画分を、結合アッセイ法において使用した。Steen et al., J Alzheimers Dis 7:63 (2005)。ビシンコニン酸(BCA)アッセイ法(Pierce, Rockford, IL)を用いて、タンパク質濃度を測定した。
前述したように、ウェスタンブロット解析を用いて、tau、ホスホtau、ユビキチン、GSK-3β、ホスホ-GSK-3β、GFAP、およびβ-アクチンのレベルを評価した。
ic-STZ処置またはビヒクル処置後に様々な間隔で脳および膵臓を検査したが、ic-STZ処置後少なくとも7日目までは、顕著なAD型神経変性、およびインスリン/IGFシグナル伝達の障害の分子指標が一貫して検出されなかったため、提示したデータの大半は、14日目に屠殺したラットから得た。認知機能障害およびAD型神経変性の病因における1型糖尿病および2型糖尿病の役割を特徴付けることに対する関心が高まったため、ic-STZによる神経変性が、40mg/kgのSTZ非経口投与の後に特徴的に生じるような、膵島における炎症、変性、ネクローシス、およびインスリン免疫反応性の低下に関連しているかどうかを判定するために研究が実施された。Mythili et al., Microsc. Res. Tech. 63:274 (2004)。組織病理学的研究により、対照ラットおよびic-STZ処置ラットの両方において、外分泌構造および内分泌構造は完全であり、炎症、ネクローシス、または島細胞変性の徴候は無いことが実証された(図15Aおよび15B)。さらに、免疫組織化学的染色により、対照ラットおよびic-STZ処置ラットの両方において、すべての膵島における顕著なインスリン免疫反応性が実証された(図15Cおよび15D)。これに対応して、ic-STZ処置ラットのランダムな血中グルコース濃度の平均は、対照ラットに比べて高くなく(図16A)、かつ、ランダムな血中グルコース濃度が180mg/dlを上回るic-STZ処置ラットは無かった。実際は、ic-STZ処置群の平均血中グルコースレベルは、対照より有意に低く、これは、それらの平均体重も有意に減少していたため(P=0.04;図16B)、おそらくは摂食の減少に起因する。
ic-STZ処置群の平均脳重量は、対照に比べて有意に少なかった(P=0.002;図16C)。ic-STZ注射した脳は、顕著に小さく、かつ、急性くも膜下出血の多数の小さな病巣を有する傾向があり(図16D)、脳血管の脆弱性の増大が示唆された。この現象に対する1つの有望な説明は、Aβ免疫反応性の増大(下記を参照されたい)により、血管が、外傷性の損傷および血流による(flow-related)損傷の影響をより受けやすくなったとするものである。しかしながら、ic-STZ処置ラットにおいても対照ラットにおいても、脳内出血は発生しなかった。
探査的研究により、STZが、視床下部、海馬、側頭皮質、および小脳を含む、いくつかの脳領域における成長因子および成長因子受容体の発現を有意に障害することが実証された。したがって、側頭葉試料を用いて得られた結果を、全般的な傾向の代表として提示する。さらに、遺伝子発現の変化は、ic-STZ処置後の最初の7日間の間はいくらか変動したが、14日目〜21日目の間は安定なままであったため、処置後14日目に採取した脳からの結果を示し、かつ考察する。
効果的なリガンド結合はシグナル伝達カスケードに極めて重要であり、かつ、神経細胞生存の減少、GSK-3β活性化の増大、およびtauリン酸化の増加を含む、ADにおいて既に確認されているインスリンシグナル伝達障害の下流の影響のうちの多くは、CNSにおけるインスリンまたはIGF受容体結合の減少によって媒介され得る。ic-STZ注射した脳における成長因子シグナル伝達のこの特徴を検査するために、トレーサーとしての[125I]標識したインスリン、IGF-I、またはIGF-II、および受容体供給源としての側頭葉組織の膜抽出物を用いて、競合的な平衡結合アッセイ法を実施した。これらの結果により、ic-STZ処置脳に比べて、対照脳において、インスリン受容体への特異的結合のレベルが有意に高いことが実証された。ic-STZ処置脳におけるインスリン受容体への平均平衡結合は、対照脳に比べて約85%減少していた(図20A)。IGF-II結合もまた、ic-STZ処置脳において有意に減少していた(図20C)のに対し、IGF-I結合は増加していたが、平均値の標準誤差が大きかったため、その差異は統計学的には有意ではなかった(図20B)。
ic-STZによって誘発された神経変性が、GSK-3β活性化の増大(ホスホ-GSK-3β/全GSK-3βの比率の減少)およびホスホtauレベルの上昇に関連しているかどうかを判定するための研究を実施した。AD型神経変性の特徴的な特質には、tauを含むタンパク質のユビキチン化の増加(Godbolt et al., Arch.Neurol. 62:1097 (2005); Kosik et al., Biochim. Biophys. Acta 1739:298 (2005); de Vrij et al., Prog.Neurobiol. 74:249 (2004))および細胞脱落を伴う神経膠症も含まれるため、ウェスタンブロット解析を用いて、ユビキチンおよびGFAP免疫反応性も同様に測定した。β-アクチン発現を陰性対照として測定した。
ADにおける認知機能障害の主要な相関物は、大脳皮質におけるアセチルコリン欠乏である。最近、ChAT遺伝子発現が、インスリンおよびIGF-1刺激によって調節されること、および、AD脳において、インスリン/IGF-1シグナル伝達メカニズムの障害が、アセチルコリン産生の欠乏と相関していることが実証された。Steen et al., J Alzheimers Dis 7:63 (2005)。したがって、ic-STZ処置脳のChAT発現レベルが低下していたかどうかを判定することが関心対象であった。側頭葉組織のリアルタイム定量RT-PCR解析により、ic-STZ処置脳において、対照脳と比べて、ChAT mRNA転写物のレベルが有意に低く、かつ、AChE mRNA転写物のレベルが有意に高いことが実証された(図24Aおよび24B)。免疫組織化学的染色研究により、ic-STZ処置脳において、対照脳と比べたChAT免疫反応性のレベルの低下(図24Cおよび24D)ならびにAChE免疫反応性のレベルの上昇(図24Eおよび24F)が実証されたことによって、リアルタイムRT-PCRの結果が裏付けられた。ChATおよびAChE発現の同様の変化が、ic-STZ処置したラットの視床下部、海馬体、および小脳皮質において観察された。
以前の報告において、神経変性の媒介物としての酸化ストレスの役割に重点を置いて、成体げっ歯動物の孤発性ADモデルを作製するために、ic-STZが使用された。これらの研究では、インスリンおよびIGFシグナル伝達経路中の遺伝子に対するic-STZの影響を調査し、かつ、ADに関して最近報告された内容に照らして、関連する分子病理学を検討した。これに関しては、これらの結果により、ic-STZ処置は、インスリン、IGF-II、インスリン受容体、およびIGF-I受容体を発現する細胞のCNSにおける欠乏を引き起こすが、高血糖症も、膵島の変性も、膵臓におけるインスリン免疫反応性の低下も引き起こさないことが実証された。したがって、ic-STZモデルはCNS疾患をもたらすが、膵臓疾患はもたらさず、インスリンの末梢供給源およびCNS供給源が別々に、かつ区別して調節されていることが示唆される。さらに、このモデルにより、CNS中のインスリン/IGFシグナル伝達メカニズムの障害が、インスリン/IGFの生合成および機能に末梢性の異常が全く無い場合に起こり得るという優れた証拠が提供される。最後に、処置後4週間またはそれ以上、生存させたラットの最近の予備研究において、モリス水迷路試験により、ic-STZ処置群において、対照と比べて、学習および記憶に著しい障害があることが明らかになった。全体的に見て、このモデルは、孤発性ADが、インスリンおよびIGFポリペプチド遺伝子ならびに受容体発現および応答性(抵抗性)のCNSにおける内因性の欠乏によって引き起こされる、神経内分泌疾患であるという仮説を支持する。
Claims (14)
- 被験者由来の中枢神経系(CNS)組織における、インスリン、インスリン様成長因子-I(IGF-I)、IGF-II、インスリン受容体、IGF-I受容体、IGF-II受容体、チロシンリン酸化インスリン受容体、チロシンリン酸化IGF-I受容体、チロシンリン酸化IGF-II受容体、インスリン受容体基質-1(IRS-1)、チロシンリン酸化IRS-1、ホスフォチジルイノシトール(phosphotidylinositol)3-キナーゼ(PI3キナーゼ)、PI3キナーゼのp85サブユニット、ホスホ-Akt、およびホスホ-GSK-3βからなる群より選択されるインスリン/IGFシグナル伝達経路中の少なくとも1つの因子のレベルまたは機能の低下をインビトロで検出する段階を含む、被験者のアルツハイマー病(AD)を検査するための方法であって、健常被験者におけるレベルと比べた、1つまたは複数の該因子のレベルまたは機能の低下がADの検査指標である方法。
- 被験者が、ADに起因する病変を呈する、請求項1記載の方法。
- 被験者が、ADに起因する病変を呈する疑いがある、請求項1記載の方法。
- 被験者が、ADに起因する病変を呈するリスクがある、請求項1記載の方法。
- インスリン/IGFシグナル伝達経路中の少なくとも1つの因子のレベルが、因子ポリペプチドのレベルを測定することによって決定される、請求項1記載の方法。
- 因子ポリペプチドのレベルが、イムノアッセイ法、免疫沈降法、またはウェスタンブロット法によってエクスビボで決定される、請求項5記載の方法。
- インスリン/IGFシグナル伝達経路中の少なくとも1つの因子のレベルが、CNS組織中の該因子をコードするRNAのレベルを測定することによって決定される、請求項1記載の方法。

- 被験者由来の中枢神経系(CNS)組織における、インスリン、インスリン様成長因子-I(IGF-I)、IGF-II、インスリン受容体、IGF-I受容体、IGF-II受容体、チロシンリン酸化インスリン受容体、チロシンリン酸化IGF-I受容体、チロシンリン酸化IGF-II受容体、インスリン受容体基質-1(IRS-1)、チロシンリン酸化IRS-1、ホスフォチジルイノシトール(phosphotidylinositol)3-キナーゼ(PI3キナーゼ)、PI3キナーゼのp85サブユニット、ホスホ-Akt、およびホスホ-GSK-3βからなる群より選択されるインスリン/IGFシグナル伝達経路中の少なくとも1つの因子のレベルまたは機能を決定するのに使用され得る少なくとも1つの検出物質を含む1つまたは複数の容器を含む、ADを診断するための診断キット。
- インスリン/IGFシグナル伝達経路中の因子に特異的に結合する1つまたは複数の抗体を含む、請求項9記載の診断キット。
