本発明に係る「インプラント装置」とは、隙間を回復又は形成するために、骨、軟骨又は軟組織間に埋め込まれる装置を意味する。「インプラント装置」は、所定の距離をおいて反対側に位置する2つの表面を有することが好ましく、前記2つの表面に平行な方向における装置の寸法(装置の幅又は直径)は、前記2つの表面に直交する方向における装置の寸法(装置の厚み)よりも大きい。「インプラント装置」の例としては、顆間インプラント又は脊椎円板インプラントが挙げられる。

請求項1に係るインプラント装置をHEMAとMMAの共重合によって製造した。装置は、HEMAとMMAの比率を変化させて複数の工程によって製造し、最小値と最大値を有する水平方向の剛性勾配を生じさせた。ポリマーの含水量は10〜20%だった。圧縮強度は0.5〜5MPaだった。装置は、必要な寸法に合わせて機械的に成形した。
請求項1に係るインプラント装置をHEMAとMMAの共重合によって製造した。装置は、HEMAとMMAの比率を変化させて複数の工程によって製造し、ヒアルロン酸含有溶媒の勾配を付加した。ポリマーの含水量は15〜30%だった。剛性は0.5〜2MPaだった。装置は、乾燥状態で機械的に成形し、再水和させ、必要な寸法に合わせて表面を仕上げた。
ウレタン誘導体、ヒドロキシブチレート及びセルロース誘導体の共重合によってインプラント装置を製造した。装置はUV照射によって一段階工程で重合させ、架橋密度を局所的に制御した。ヤング率は1〜80MPaだった。生理的荷重下における変形は5%未満だった。
ポリビニルアルコールとシリコーンの層ごとの重合によってインプラント装置を製造した。機械的特性の勾配は、重合条件をゆっくりと変化させることによって生じさせた。剛性は0.5〜5MPaだった。
シリコーンとバイオテクノロジーによるキトサンの共重合によってインプラント装置を製造した。勾配は、選択的なUV照射によって生じさせた。ヤング率は1〜20MPaであり、水平方向の寸法安定性は95%超だった。装置の形状は、プリフォームにおける重合によって得た。
ポリウレタンカーボネートを使用してインプラント装置を製造した。重合時に、10%(w/w)の無機造影剤をCT撮影のために添加した。ポリマーの剛性は、重合反応を選択的に制御することによって変化させた。ヤング率は200MPa以下だった。水平方向の寸法安定性は99%超だった。
Claims (10)
- 2つの関節面間の空間に埋め込むための顆間インプラントであって、
z軸方向に所定の距離をおいて反対側に位置する2つの表面と、
x軸及び/又はy軸に沿って少なくとも1つの極小値を有する剛性勾配と、
を有する本体を含み、
前記x軸は、前記2つの表面に実質的に平行な方向において本体の幾何学的中心を通り、
前記y軸は、前記2つの表面に実質的に平行かつ前記x軸に直交する方向において前記本体の幾何学的中心を通り、
前記z軸は、前記x軸及び前記y軸に直交する方向において前記本体の幾何学的中心を通過し、
前記x軸及び前記y軸における前記本体の寸法は、前記z軸における前記本体の寸法よりも大きく、
前記本体は、最大生理的荷重条件下において前記x軸及び/又は前記y軸方向に限られた寸法変化を有し、前記本体の前記x軸及び/又は前記y軸方向の寸法は、前記z軸方向に20MPa〜200MPaの全範囲のいずれの場合の荷重が付加されても、前記z軸方向における荷重が無荷重のときの前記x軸及び/又は前記y軸方向の寸法に対する寸法変化が10%未満であり、
前記本体の剛性は、前記z軸方向に荷重が与えられた場合における前記x軸及び/又は前記y軸に沿った方向への寸法変化のしにくさであって、少なくとも部分的に異方性を有し、
前記剛性勾配は、連続的または非連続的であり、前記x軸及び/又は前記y軸方向に沿って前記本体の周辺部から中心に向かって剛性が連続的または非連続的に低下していることを特徴とするインプラント装置。 - 請求項1において、前記本体が、前記x軸及びy軸に沿って少なくとも2つの極小値を
有する剛性勾配を有し、前記x軸及び前記y軸方向に沿って前記本体の周辺部から2つの中心に向かって剛性が低下するインプラント装置。 - 請求項1または2において、前記インプラント装置の形状が埋め込み後に生理学的状況に適合するインプラント装置。
- 請求項1〜3のいずれか1項において、前記インプラント装置が埋め込まれる生理学的空間において自己調心性を有するインプラント装置。
- 請求項1〜4のいずれか1項において、材料を線形弾性材料としてモデル化した場合に、各方向において少なくとも0.1〜100MPaのヤング率に対応する剛性を有するインプラント装置。
- 請求項1〜5のいずれか1項において、0.1未満の動的摩擦係数を有するインプラント装置。
- 請求項1〜6のいずれか1項において、前記本体がポリマー材料からなるインプラント装置。
- 請求項7において、前記ポリマー材料がヒドロゲルであるインプラント装置。
- 請求項7において、前記ポリマー材料が生体適合性を有するインプラント装置。
- 請求項1〜9のいずれか1項に記載の顆間インプラントであるインプラント装置の製造方法であって、前記本体をポリマー材料で形成し、(a)溶融又は半溶融ポリマー材料を部分的に配向させるか、(b)層ごとの硬化及び/又は架橋工程を行うか、(c)配向又は非配向繊維強化材をポリマー材料の一部に導入することによって剛性勾配及び部分的な異方性を生じさせ、前記顆間インプラントを製造する方法。
