Description
時間系列の発射機構を備えるエネルギー源(420)に接続可能である。言い換えれば、個別の薬物送達装置(410)の各々がエネルギー源(420)に接続され、設定された時間間隔及び一定期間に薬物を送達するために、コントローラ(マイクロプロセッサ)は、一定期間にわたって所定の数の薬物送達装置(410)を実質的に発射させるという患者の必要性に応じてプログラム化可能である。さらに、当然のことながら、その薬物送達装置(410)のいくつかが1つの薬物を含み、一方でその他の装置が別の薬物を含むことができるため、このアレイは1以上の薬物を備えることが可能である。各々の薬物伝達装置(410)をエネルギー源に接続することによって、異なる薬物を異なる回数、互いに関連した適切な順序で送達可能である。
薬物送達装置の1つの応用例は人間の耳である。より詳細には、図5乃至7のかかり付きインプラント設計は、中耳炎における予防的使用のための抗感染インプラント製剤として、又は、硬化剤として構成可能である。かかり付きインプラントは局所薬剤と結合可能であり(例えば、スワブ(Qtip)又は被膜と類似する)、かかりを基盤とする製剤を鼓膜の外部表面に塗布することによって、マイクロ/ナノのかかりが膜を貫通して、中耳領域に到達することが可能となる。この中耳領域で抗感染薬(構成物質)を備えるかかりが、事前処理のために、又は、すでに感染した中耳空間を治療するために配される。アプリケーターはゲル、又は多層被膜の形状をとってもかまわない。この多層被膜は、神経が感作した領域への塗布を容易にするための局所麻酔薬を含むこともある。
図5乃至7のかかり付きインプラント設計の別の実施例は、鼻の感染症又は鼻炎における予防的使用のための抗感染又は抗アレルギーインプラント製剤として、又は、硬化剤として構成可能である。かかり付きインプラントは局所麻酔薬(例えば、スワブ(Qtip)又は被膜、又はスプレーと類似する)と結合し、かかり付き製剤が鼻粘膜に塗布されることによって、マイクロ/ナノのかかりが膜を貫通して、中耳領域に到達することが可能となる。この中耳領域で抗感染薬(抗生物質)、抗アレルギー薬(抗ヒスタミン剤など)を備えるかかりが、事前処理のために、又は、すでに感染した鼻空間を治療するために配される。アプリケーターはゲル、スプレー、又は多層被膜の形状をとってもかまわない。この多層被膜は、神経が感作した領域へとの塗布を容易にするための局所麻酔薬を含むこともある。
別の応用例は徐放薬を直接点滴するために構成された腫瘍/器官用外皮(wrap)である。外皮は「かかり」が開いた入口を駆動させるとともに標的組織への薬剤の能動輸送を可能にする、「繊維性」又は収縮性重合体製の皮膚である。外皮はスプレー又は回転塗布によって腹腔鏡下で塗布可能である。
以下に記載するのは、図14乃至22の装置の1つが薬物送達適用例としてどのように使用可能かどうかに関する一般的な記載である。第1段階では、背圧が増加するか、又は、背圧が極微針の動作と位相がずれて振動する。この結果、デューティーサイクル(周波数、振幅、及び継続期間によって定義される)の間、角質層は穿たれ、角質層に複数の孔が形成される。大きな薬物分子が(周期的に振動する)背圧動作によって角質層を通って押し出される。次の工程においては、「穿つ動作」が停止し、角質層の孔が閉じる/癒えるまで(静的な)背圧が維持される。
Claims (18)
- 皮内送達システムであって
角質層下に薬物を皮内送達するための少なくとも1つのマイクロ/ナノサイズの薬物送達装置を備え、
前記薬物は前記装置内部の容器に貯蔵され、
前記装置は前記角質層下に前記薬物を送達するために前記装置の少なくとも一部の運動を選択的に引き起こすアクチュエータを有し、
前記システムは、
内部空間を画成する上部(804)と側壁(805)を有し、底部が開放された固定ケーシング(802)を備え、その結果、側壁(805)の底表面(807)は患者の皮膚に取り付けることができ、
前記システムは、前記固定ケーシング(802)の内部空間に、前記側壁(805)と側壁(805)の間に延びるように配された移動可能な基盤(830)を備え、前記基盤(830)は前記固定ケーシング(802)に対して移動可能であり、前記基盤(830)は基盤(830)と一体的であり、かつ基盤(830)の底面から外に延びる少なくとも1つの極微針を備え、これによって該極微針は、前記基盤(805)の一部として、角質層と接触し、角質層を貫通する距離を移動することができ、
前記システムはさらに、
前記アクチュエータの作動を制御するための少なくとも1つの装置と連通するプログラム化可能なコントローラと、
前記プログラム化可能なコントローラと連通するとともに少なくとも1つの患者の生体特性を測定する少なくとも1つのバイオセンサを有するバイオフィードバック装置を備え、
前記コントローラは、規定の時間に前記薬物を送達する患者の必要性に基づいて、又は、前記バイオフィードバック装置から受信した信号に基づいてプログラム化され、
前記アクチュエータが動作可能なように移動可能な基盤と接続されることによってその移動が制御され、これによって薬物送達の先端が前記固定ケーシング(802)の底部表面(807)を超え、前記角質層下の皮膚内までの延長した位置まで前進し、引っ込められた位置において、薬物送達の先端は前記側壁(805)の底表面(807)の上の前記固定ケーシングの内部空間内に位置づけられ、前記固定ケーシングの底表面が皮膚上で静止したときに、薬物送達の先端が皮膚から離間される
ることを特徴とするシステム。 - 前記アクチュエータが前記移動可能な基盤を振動させることによって、0kHz以上乃至約3MHzの周波数、及び、0ミクロン以上乃至約250ミクロンの振幅で前記極微針が振動するようになることを特徴とする請求項12記載のシステム。
- 前記極微針が少なくとも1つのチャネルを備え、該チャネルが前記薬物容器と選択的に連通することによって、前記薬物が前記チャネルを通って開口している遠位端部に流れることを特徴とする請求項1記載のシステム。
- 前記少なくとも1つのチャネルが、前記チャネルを通る前記薬物の流れを制御する流れ制御部材と連通することを特徴とする請求項3記載のシステム。
- 前記極微針に形成された少なくとも2つのチャネルが存在し、各々のチャネルが夫々の流れ制御部材を有することを特徴とする請求項4載のシステム。
- 前記チャネルの1つが背圧チャネルであることを特徴とする請求項3記載のシステム。
- 前記流れ制御部材が一方通行バルブ/ポンプを備えることを特徴とする請求項4記載のシステム。
- 各々が互いに独立して作動及び移動可能な複数の極微針が存在することを特徴とする請求項1記載のシステム。
- 前記アクチュエータが第1及び第2の圧電ストリップを備え、該圧電ストリップは前記容器がその間に位置するように配され、前記ストリップはエネルギー源と動作可能なように接続されることを特徴とする請求項1記載のシステム。
- 前記アクチュエータは、圧力アクチュエータ及び動作アクチュエータからなる群から選択された少なくとも1つのアクチュエータであることを特徴とする請求項1記載のシステム。
- 前記バイオセンサはチャネルを介して前記容器と選択的に連通し、該チャネルは前記センサと前記容器との間の流れを制限するために前記チャネルに沿って配された流れ制御装置を有することを特徴とする請求項1記載のシステム。
- 前記センサが前記少なくとも1つの薬物送達装置の前記本体と統合されることを特徴とする請求項1記載のシステム。
- 前記バイオフィードバック装置が前記プログラム化可能なコントローラと無線通信することを特徴とする請求項2記載のシステム。
- 前記バイオフィードバック装置及び前記センサの少なくとも1つが、生体特性を測定し、前記測定された生体特性に関する情報を含む信号を前記プログラム化可能なコントローラに送ることを特徴とする請求項1記載のシステム。
- 前記プログラム化可能なコントローラが少なくとも1つのディスプレイを含む外部装置と通信し、該ディスプレイは、前記測定された生体特性に関する前記情報、及び、前記情報を蓄積可能なメモリを表示可能であることを特徴とする請求項14記載のシステム。
- 前記外部装置は携帯用装置であり、前記通信は前記携帯用装置と前記コントローラとの間の無線通信を備えることを特徴とする請求項15記載のシステム。
- 前記アクチュエータが前記極微針内に背圧を形成することによって、流体が前記極微針、前記容器に引き込まれ、さらに前記センサと接触することを特徴とする請求項1記載のシステム。
- 前記流体が血液又は間質液のひとつであることを特徴とする請求項17記載のシステム。
