Description
本出願は、2010年11月4日にファイルされた米国仮特許出願第61/410,114号の優先権の利益を主張するものであり、その内容全体をあらゆる目的で本明細書に援用する。
初期の装置設計は、レーザーダイオードモジュールを追加し、既存のオープンタイプのオトスコープ(Welch Allyn, Skaneateles Falls, NY)に実装することを中心としていた。本発明の目的で、オープンタイプのオトスコープは、厳密に焦点を合わせた光線の位置決めと調節を容易に可能にする。レーザーダイオードモジュールを固定するために、単純なマウントを構築し、光源付近でオトスコープに取り付ける。
許可を得て、耳、中耳、側頭骨、中頭蓋窩底、耳管、周辺の構造体(Cleveland Clinic Anatomy Lab)を含むex vivoでの死体標本を入手した。Cleveland Clinic Head and Neck Institute Resident Training Temporal Bone Lab(Crile Building)で実験を行った。側頭骨切開装置に標本を固定し、経中頭蓋窩法で中耳腔にアクセスした。このアクセスは、合成の滲出液代用物を出し入れするのに必要であった。
(2)緑色(532nm)レーザーを使った可視化では、仮定したとおりの視覚上の効果が得られる。鼓膜が光っていることから滲出液の存在が証明され、滲出液が存在しないと、レーザーは点のように見える。この効果は、どちらのタイプの滲出液でも証明された。
(3)赤色(605nm)レーザーでは、緑色レーザーと似たような結果が得られた。
(4)青色(405nm)レーザーでは、透明の滲出液の状態に対しては予想した結果が得られたが、黄色の滲出液では何も光らなかった(おそらく、デジタル(およびフィルム)カメラのほうが、人間の目よりもUV領域に対する感度がかなり高いためであろう)。
(5)カメラによって結果を目で確認できる形の資料にすることができるが、デジタルカメラよりも人間の目のほうが感受性が高いことで、実際には図8に示す画像よりもはっきりとした結果が得られる。
Claims (4)
- 治療対象体の中耳内に異常な流動体が存在するかどうかを決定するための装置であって、
耳を検査するための遠位端を有する細長いプローブと、
前記治療対象体の前記中耳と連関した鼓膜に穴をあけることなく、光線を、前記鼓膜を通り抜けて伝えるように構成され、前記細長いプローブに内蔵された、5mW未満の電力で動作する第1の光源と、
光を、前記細長いプローブの前記遠位端を通り抜けて伝え、前記鼓膜に照射させるように構成され、前記細長いプローブに内蔵された第2の光源と、
を備え、
前記光線は、前記治療対象体の中耳内の異常な流動体の有無に基づいて、前記鼓膜から反射した光及び前記鼓膜を通り抜けて放射される光の拡散反射を含む、鼓膜に関連した反射率のパターンを作り出すように構成され、拡散反射率パターンは、前記治療対象体の中耳内に異常な流動体が存在していることを示す、
装置。 - 前記細長いプローブは、オトスコープを含み、前記第2の光源は白熱電灯源を含む、請求項1に記載の装置。
- 前記第1の光源は、発光ダイオード(LED)、低出力レーザー、及び低出力レーザーダイオードからなる群から選択される、請求項1に記載の装置。
- 前記光線は、平行光線及び収束光線の少なくとも1つを含む、請求項1に記載の装置。
