Description

1)周辺組織(全膵臓/外分泌組織、肝臓、腸、脾臓、胃)と比較して、膵島において選択的に発現されること、
2)膵α細胞又は他の膵島非β細胞よりも膵β細胞において高度に発現すること、
3)膵β細胞における発現レベルが、膵β細胞において特異的に発現される酵素であるグルコキナーゼよりも高いか、又はそれと同程度であること、
4)膜中に位置し、且つそれ自体が画像化、標的化、及び免疫組織化学を可能にする抗体、ペプチド、又は小分子によって標的化可能であること、
5)β細胞塊の炎症過程で発現が誘導されず、且つ該タンパク質はT細胞及び樹状細胞、又は炎症過程に関与する他の細胞において富化され(enriched)ないこと。
a)試料中のβ細胞を、FXYD2−γ−a、FXYD2−γ−b、及び/又はFXYD2−γ−c等の1つ又は複数のFXYD2−γアイソフォームに特異的に結合する標識分子を用いて可視化する工程、
b)標識されたβ細胞の量を定量化する工程を含む、方法を提供する。

e)選択した遺伝子/タンパク質の発現レベルが炎症時に変化していないことに基づき選択することを含む、方法を提供する。
1. 「膵島特異的」の基準は、ヒト膵島の定量的MPSSデータと、32種のヒト組織のMPSSデータ(全膵臓を含む、公的に利用可能なデータベースであるLICRのMPSSデータセットから得た)との比較に基づく。本発明では、膵臓と比べて膵島において50倍超富化された発現を選択した。ヒト膵島に関する定量的MPSSデータセットは以前は利用可能でなかったため、これは新たな特徴である。
2. 「β細胞特異的」、すなわちα細胞よりもβ細胞において強く発現されること。この基準は、1型糖尿病(T1D)等の病的状態におけるβ細胞の画像化に重要である。T1Dの進行過程においてβ細胞は次第に消滅するが、α細胞の数は増加する。したがって、β細胞に特異的であり、α細胞において全く又はあまり発現しないバイオマーカー(複数可)を用意する必要がある。
3. 発現レベルはグルコキナーゼより高いか、又は同程度であるべきである。グルコキナーゼは、β細胞において高度且つ選択的に発現される酵素であり、グルコキナーゼとの比較によって、潜在的バイオマーカーの高度の発現に関する信頼性のある評価が提供される。
4. 本発明のバイオマーカーは細胞膜に位置する。初めに、膜貫通領域を検出するためにTMHMMプログラムを使用し、続いて徹底的に文献解析を行なった。GO解析及びIngenuityのパスウェイ解析は、この基準を補完するために使用した。
5. 発現レベルは炎症によって誘導されない。選択されたバイオマーカーの発現レベルは、ヒト膵島及び精製初代ラットβ細胞で得られたマイクロアレイデータセットにおいて、炎症状態(サイトカインへの曝露又はウイルスによって誘発された)で増大しない。このことは、早期T1Dにおいて一般的な、膵島炎におけるバイオマーカーの発現の変更が誤ったβ細胞塊の評価を導かないことを確かにする。
ウサギを配列番号1(MTGLSMDGGGS+C−KLH)の11AAペプチド(抗体SPY393及びSPY394と指定される)、又は配列番号2(MTGLSMDGGGSPKGD+C−KLH)の15AAペプチド、又は配列番号3(MTGLSMDGGGSPKGDVDPFYYDYETVRN+C−KLH)の28AAペプチドで免疫化することによる、FXYD2のγ−aスプライシング変異体
ウサギを配列番号4(MDRWYLGGS+C−KLH)のペプチド(抗体SPY341と指定される)で免疫化することによる、FXYD2のγ−bスプライシング変異体、
ウサギを配列番号5(GKPGPLRTLPEPSGPLPPSSGLSQPQVHALCPLSPLVTTGCCGQAAERDSCWERPPIPLLLPSLSG+C−KLH)のペプチドで免疫化することによる、FXYD2のγ−cスプライシング変異体。
a)上記マーカーに特異的に結合する同位体標識トレーサー分子を被験体に導入すること、
b)膵臓においてβ細胞に特異的に結合するトレーサー分子を、PET、PET−CT、又はSPECTを用いてin vivo可視化すること、
c)上記被験体におけるβ細胞塊を定量化すること、
d)工程c)において得られるβ細胞塊データと、健常被験体のβ細胞塊データとを比較すること、
d)工程c)において得られたβ細胞塊のレベルが、健常被験体のβ細胞塊のレベルと比較して低下している場合に、被験体を糖尿病であるか、又は糖尿病を患う危険性があると診断すると共に、工程c)において得られたβ細胞塊のレベルが、健常被験体のβ細胞塊のレベルと比較して上昇している場合に、被験体を高インスリン血症であるか、又は高インスリン血症を患う危険性があると診断すること。
a)処理した該幹細胞を、FXYD2−γマーカーに特異的に指向性を有する標識結合分子でタグ付けする工程、及び

c)工程a)の細胞と結合するが、工程b)の細胞とは結合しないこれらの候補トレーサー分子をβ細胞塊トレーサー分子として保持する工程を含む、方法を提供する。上記方法の好ましい実施形態では、FXYD2−γ陽性細胞型又は細胞株は、齧歯類膵島、ラットINS−1E、AR42J細胞、及びヒトのCAPAN−2細胞から成る群から選択され、FXYD2−γ陰性細胞型又は細胞株はPANC−1である。
本発明を以下の非限定的な実施例によって説明する。
これらの実施例において使用される戦略を図1及び図2に示す。膵臓は、外分泌組織中に散在する、1%〜2%の小さな内分泌組織であるランゲルハンス島から構成される。膵外分泌部は、腺房細胞及び網状の管から成る。膵島は、インスリンを分泌するβ細胞、及び非β細胞、例えばグルカゴンを分泌するα細胞、膵臓ポリペプチドを産生するPP細胞、及びソマトスタチンを分泌するδ細胞を含有する。T1Dでは膵β細胞は自己免疫攻撃によって選択的に破壊される。また、T1Dよりも低いレベルではあるが、長期T2Dでもβ細胞の喪失が起こるという証拠がある。膵島及び膵β細胞に関する遺伝子発現データは、過去8年間に得られたものである。使用した方法は、マイクロアレイ解析及び大規模並列シグネチャー配列決定(発現した配列に由来するシグネチャーの配列決定に基づき、遺伝子発現を検出する手順)である。MPSSによって得られたデータは定量的であり(100万個当たりの転写産物)、アレイ解析(1つのセル当たり転写産物を3コピーまでしか検出することができない)よりも高感度である。β細胞の遺伝子発現プロファイルを得るために、2つの独立したヒト膵島試料に対してMPSSを行い、一方でFACSにより精製ラット初代β細胞及び非β細胞に対してマイクロアレイデータを得た。MPSSに使用したヒト膵島は高品質であった(2つの調製物のβ細胞の割合は53%及び59%であった)。得られたMPSSデータは、少なくとも5tpmのレベルで、試料1において5662個の遺伝子、及び試料2において5929個の遺伝子を示していた。遺伝子は、膵島におけるそのシグネチャー数が、最低レベル20tpmで、検査した他の32種の組織全体(膵臓を含む)の全シグネチャー数の80%以上である場合、膵島に対して80%以上特異的であると見なした。インスリンは平均レベルが126753tpmであり、そのリストの一番上にある。これは、ヒト膵島試料中のmRNA集団全体の13%である。上記の基準に従う膵島特異的な転写産物は、膵島において観察される発現レベルを、32種のヒト組織の公開されたMPSSデータセット(LICRのMPSSデータセット:http://mpss.licr.org/)と比較することによって得た。本発明では、膵島に対して比較的特異的な合計940個の遺伝子を検出したが、これらの遺伝子のうち324個は、ラット膵臓のβ細胞においてα細胞と比較してより高いレベルで発現される。TMHMMプログラムを用いて、膜貫通領域を有するタンパク質を特定し、44種のタンパク質を確保した。これらの候補を細胞膜位置に関して(文献解析、GO解析、及びIPA解析によって)さらに解析し、炎症状態で、すなわちサイトカイン又はウイルスへの曝露後に発現レベルを解析した(独自のマイクロアレイデータ)。GNF symatlasのウェブサイト(http://symatlas.gnf.org/SymAtlas/)に基づき、T細胞/リンパ球におけるこれらの遺伝子の発現レベルも同様に検査した。マイクロアレイ解析のために、FACS選別膵島細胞を使用した。β細胞画分における初代β細胞の純度は90%であり、非β細胞画分は主にα細胞を含有していた(75%〜85%)。結果は、6190個〜6270個の遺伝子がラットのβ細胞、α細胞、及びINS−1細胞において発現されたことを示す(閾値平均シグナル強度=250、標的強度=1500)。非β細胞に対するβ細胞で発現した遺伝子の差次的発現解析によって、β細胞において富化される983個の遺伝子が得られた(倍富化(fold enrichment)≧2、FDR:5%)。972個の遺伝子が、β細胞において非β細胞と比較してより高度に発現された(倍富化≧2)。
A. マーカーFXYD2:
LICRのMPSSデータセットに由来する32種のヒト組織における発現と比較した、ヒト膵島において得られたMPSSデータ。5tpmを超える発現レベルのみを示す。
Na,K−ATPアーゼのγ−サブユニット(FXYD2、ATP1G1、HOMG2、MGC12372)は、その力学的特性を変更することによってNa,K−ATPアーゼを調節する1回膜貫通型タンパク質である。γ−サブユニットは、組織特異的に発現される7つのFXYDタンパク質のファミリーに属する(Cornelius F et al, 2003の図1)。FXYD2は近位尿細管及び遠位尿細管、並びに腎臓のヘンレ係蹄の太い上行脚髄質において多量に発現される(Arystarkhova E et al 2002、Pihakaski-MaunsbachK et al 2006)。2つのスプライシング変異体(γ−a(配列番号6)及びγ−b(配列番号7))がラット及びヒトで検出されているが(Arystarkhova E et al 2005)、第3のスプライシング変異体γ−cはマウスでしか特性化されていない(Holstead Jones D et al, 2005)。近年、FXYD2のさらなるスプライシング変異体が、GenBankデータベースに寄託されており(NM_001127489、2747bp、145 AA=14.5kDa(配列番号8)、ヒトFXYD2−γ−cスプライシング変異体として指定されている。この新たなスプライシング変異体は、同一の21個のアミノ酸残基のアミノ末端を有するため、FXYD2−γ−aタンパク質の選択的スプライシング形態であると思われる(図2B)。興味深いことに、この新たなγ−c変異体(配列番号8)も、本発明者らによって作製された抗体SPY393により認識される。
FXYD2−γ−b:
この候補の検証を、Abnovaの全長γ−bスプライシング変異体に対する唯一の利用可能な市販のモノクローナル抗体(クローン1C3−B3)を用いて開始した。膵臓薄片及び腎臓薄片に対する免疫組織化学によって、腎臓におけるFXYD2−γ−bの存在を確認した(図3a)。膵臓におけるFXYD2の発現は、膵島、及び稀に外分泌組織の介在導管に限定される(図3b)。膵臓、より具体的には膵島におけるFXYD2の発現は全く示されなかった。
ヒトγ−aスプライシング変異体のみを解析するのに利用可能な抗体はないため、γ−a及びγ−bヒトFXYD2スプライシング変異体のN末端断片に対するウサギポリクローナル抗体を生成させた(Eurogentecとの共同研究)。

MPSSデータに基づくと、FXYD2発現は、他の組織において低い発現が検出されるのに対し、膵島、腎臓、及び唾液腺において特異的に検出される(下記表2参照)。副腎、骨髄、脳、下垂体、胎盤、前立腺、小腸、脊髄、甲状腺、気管、大腸、及び単球においては検出されなかった。MPSSデータ(表3参照)は、全膵臓と比べて膵島において500倍超のシグナル増幅を示す。膵島は全膵臓量の1%〜2%であるため、この候補は膵島特異的であるとして選択される。
免疫化に使用される配列番号3のペプチド
他の組織におけるFXYD2の発現レベルは、膵島、腎臓、及びMOLT4細胞において発現を示すFXYD2 205674_x_atプローブセットに関するマイクロアレイSymatlasデータから得た(ウェブリンク:http://symatlas.gnf.org/SymAtlas/参照、対応するMPSSデータに関しては表6を参照)。FXYD2発現がβ細胞において特異的に検出されることを検証するために、FXYD2に指向性を有するSPY393抗体、抗インスリン抗体(β細胞を検出する)、又は抗グルカゴン抗体(α細胞を検出する)を用いて解析した連続膵臓薄片(3μmの膵臓パラフィン切片)において免疫組織化学を行った(図8参照)。
β細胞遺伝子発現バンクは、実験医学研究室において行われたマイクロアレイ実験結果を含有する、本発明者らによって作製されたオープンリソースである。マイクロアレイ発現値はヒトの膵島、ラット初代β細胞、及びINS−1E細胞において、対照と炎症状態(サイトカイン又はウイルスのいずれかへの曝露によって誘発された)とを比較して得た。マイクロアレイデータは、IL1β(50U/ml)及びIFNγ(1000U/ml)又はコクサッキーウイルス5B(感染効率30〜100、Ylipaasto P et al, 2005)のいずれかに48時間曝露した単離ヒト膵島に対して得た。ヒトデータセットの解析では、サイトカインに曝露した場合、FXYD2発現の2倍の減少が示され、この減少はCBV5に曝露した場合より低かった(表6参照)。これらのデータは、48時間のサイトカインへの曝露の後、FXYD2発現の2倍〜3倍の低下を示すMPSSデータを確認するものである。ラットマイクロアレイのプローブセットはスコアが低かったが、これはおそらくプローブセットの誤った選択のためである。
組織化学による解析によって、膜及び細胞質の両方でのFXYD2発現が示される(以前の図面及び図10を参照)。細胞膜を障害することなく(パラホルムアルデヒド及びトリトンX100を使用せずに)、ラット及びマウスの分散膵島に対して行った免疫細胞化は、細胞の膜上でFXYD2の検出を示した。
B.1. 導入
XL−エキソンによってコードされる代替的遺伝子産物(Alex)及びグアニンヌクレオチド結合G(s)サブユニットαアイソフォームエキストララージ(extra large)(XLas)は、GNAS1遺伝子座によって代替的な第1エクソン及びプロモータを用いてコードされる2つのタンパク質である(Abramowitz J et al, 2004)。Gs−α及びXLasは異なるN末端ドメインを有するが、カルボキシ末端部は同一である。広範に発現されるGs−αとは異なり、XLasは限られた組織分布を有する。XLasは主に神経内分泌組織において見られ、下垂体において非常に高いレベルである(Kehlenbach RH et al, 1994、Pasolli H et al, 2000)。XLasは、クラスB副甲状腺ホルモン受容体1、CRF受容体1、b2−アドレナリン受容体、TSH受容体等の幾つかのGs共役受容体のシグナル伝達に関与するGsタンパク質である(Bastepe M et al, 2002)。
MPSSデータにより、膵島において全膵臓と比較した場合に1200倍に富化されたGNAS1遺伝子座の発現が示される。したがって、これら2つのタンパク質をタンパク質検証に選択した。

アネキシンA7(Anx7)(又はシネキシン)は、膜と融合するが、電位依存性カルシウムチャネルとして働くことのできるカルシウム依存性膜結合タンパク質である(Caohuy H et al, 1996)。Anx7は、マウスランゲルハンス島における遺伝子発現の栄養制御に必要とされる(Srivastava M et al, 1999 and 2002)。ヌル表現型(nullphenotype)は胎生期10日目に致死であった。ヘテロ接合マウスは生存可能且つ繁殖可能であったが、インスリン分泌異常及び単離した膵島内のインスリン含量増加を示した。これらのマウスは、膵島におけるイノシトール−1,4,5−三リン酸(IP3)受容体機能の著しい低下を有する。種々のスプライシング変異体が組織特異的発現によって特性化されている(Magendzo, K et al, 1991)。アネキシン7は脳の小胞体ストアからカルシウムを動員する(Watson WD. FXYD2−γ−aの発現は、1型糖尿病患者(CO及びTELF(これらのコードは2人の患者を識別するものである)、1型糖尿病の診断からそれぞれ3日後及び5年後に死亡した)に由来する膵島において、正常膵臓(CTRL)と比較して大幅に減少する。3μmの連続切片を採取して、SPY393抗体、抗インスリン抗体、又は抗グルカゴン抗体で染色した。TELF膵臓では、インスリン染色を基にβ細胞を検出することはできない。このことはFXYD2染色の消失と相関していた。CO膵臓では、インスリン染色を基にβ細胞を検出することはできないが、非常にかすかなFXYD2染色が残されていた(図12参照、倍率1000×)。
SPY393抗体は、生体内分布研究を行うために任意の放射性トレーサーで標識することができる。この特定の実施例においては124Iを使用した。FXYD2に対するモノクローナル抗体を開発することもでき、検証した後、この抗体及びその断片を同様に生体内分布研究に使用することができる。モノクローナル抗体を作製する方法は当該技術分野で既知である。
別の態様では、本発明はFXYD2−γ陽性細胞に特異的に結合する小分子を特定する方法を提供する。かかる小分子(例えばペプチド、化学物質等)は、その後β細胞塊を可視化するためのトレーサーとして使用することができる。この目的で、本発明者らはFXYD2バイオマーカーを発現するか又は発現しないヒト細胞株を特定した。
A FXYD2−γ−a及びFXYD2−γ−bスプライシング変異体がマウス膵島において検出された。FXYD2−γ−aアイソフォーム(レーン1)、FXYD2−γ−bアイソフォーム(レーン2)、及びFXYD2−γ−cアイソフォーム(レーン3)を検出するためにプライマーを使用した。マーカー(M、レーン4)。重要なことに、γ−c変異体は検出されなかった。

Claims (17)
- 膵β細胞塊を測定する方法であって、
a)膵β細胞を標識する対象としてFXYD2−γ−aアイソフォームを用いて、膵臓組織中の膵β細胞を可視化する工程、及び
b)前記膵臓組織中の標識されたFXYD2−γ−aアイソフォームから膵β細胞の量を定量化する工程を含む、方法。 - 膵β細胞関連障害のin vitro診断及びin vivo診断のための診断用組成物の調製における、FXYD2−γ−aアイソフォームの使用。
- 膵β細胞関連障害のin vivo診断の判断材料を提供する方法であって、
a)FXYD2−γ−aアイソフォームが標識された被検体を準備する工程、
b)膵臓中の標識されたFXYD2−γ−aアイソフォームが存在する膵β細胞集団を、PET、PET−CT、SPECT、又はMRIを用いてin vivoで可視化する工程、
c)前記被験体における膵β細胞の量を定量化する工程、
d)工程c)において得られた膵β細胞塊のデータを健常被験体の膵β細胞塊のデータ、又は同じ被験体の以前の分析の膵β細胞塊のデータと比較する工程、及び
e)工程c)において得られた膵β細胞塊のレベルが、健常被験体の膵β細胞塊のレベルと比較して低下している場合に、該被験体を糖尿病であるか、又は糖尿病を患う危険性があり、かつ工程c)において得られた膵β細胞塊のレベルが、健常被験体の膵β細胞塊のレベル、又は同じ被験体の以前の分析の膵β細胞塊のレベルと比較して上昇している場合に、該被験体を高インスリン血症であるか、又は高インスリン血症を患う危険性がある、と診断する判断材料を提供する工程を含む、方法。 - 膵β細胞関連障害が1型真性糖尿病、2型真性糖尿病、高インスリン血症、又は膵癌である、請求項3に記載の方法。
- 膵β細胞関連障害が1型真性糖尿病、2型真性糖尿病、高インスリン血症、又は膵癌である、請求項2に記載の使用。
- 被験体において膵β細胞塊を特異的に測定するか、及び/又は膵β細胞関連障害を診断するか、及び/又は膵β細胞を精製するためのキットであって、FXYD2−γ−aアイソフォーム及び/又はこれに結合する抗体を含む、キット。
- 被験体における膵β細胞移植の成功の判断材料を提供する方法であって、
a)FXYD2−γ−aアイソフォームを検出する請求項1に記載の方法を使用して、被験体に膵β細胞を移植した後一定期間内の該被験体における膵β細胞の量を測定する工程、及び
b)膵β細胞塊を経時的に比較することにより該移植の成功の判断材料を得る工程を含む、方法。 - 膵β細胞を膵臓組織の非膵β細胞と区別し精製又は単離するin vitro方法であって、
a)in vitroにおいて、標識する対象としてFXYD2−γ−aアイソフォームを使用して膵β細胞をタグ付けする工程、及び
b)標識された膵β細胞を、非標識細胞から膵β細胞上のタグによって単離し、それにより実質的に純粋な膵β細胞調製物を得る工程を含む、方法。 - 再生された膵β細胞を特定するin vitro方法であって、
a)in vitroにおいて、標識する対象としてFXYD2−γ−aアイソフォームを使用して膵β細胞をタグ付けする工程、
b)標識された膵β細胞を、非標識細胞から膵β細胞上のタグによって単離し、それにより実質的に純粋な再生膵β細胞調製物を得る工程、
c)免疫組織化学を実行し、それにより新たに再生された膵β細胞の数を特定すると共に、新たな膵β細胞塊を確定する工程、及び
d)治療戦略を追跡調査すると共に、膵β細胞塊の回復を検出する工程を含む、方法。 - タグが磁性ビーズ、常磁性ビーズであり、膵β細胞塊がMRI(核磁気共鳴画像法)によって測定され得る、請求項8又は9に記載の方法。
- 機能的インスリン発現細胞を誘導するために膵β細胞の幹細胞集団を特定するin vitro方法であって、
a)in vitroにおいて、標識する対象としてFXYD2−γ−aアイソフォームを使用して処理した膵β細胞の幹細胞をタグ付けする工程、及び
b)標識した該幹細胞を、非標識細胞から潜在的膵β細胞幹細胞上のタグによって単離し、それにより実質的に純粋な膵β細胞幹細胞調製物を得る工程を含む、方法。 - c)免疫組織化学を実行し、それにより膵β細胞幹細胞の数を特定すると共に、新たな膵β細胞塊を確定する工程、及び
d)治療戦略を追跡調査すると共に、新たに形成された膵β細胞の量を検出する工程をさらに含む、請求項11に記載の方法。 - FXYD2−γ−a陽性細胞に特異的に結合する新たな膵β細胞特異的トレーサー分子を特定するin vitro方法であって、
a)in vitroにおいて、候補トレーサー分子をFXYD2−γ−a陽性細胞型又は細胞株と接触させると共に、該候補トレーサー分子と該細胞との相互作用を測定する工程、
b)in vitroにおいて、該候補トレーサー分子をFXYD2−γ−a陰性細胞型又は細胞株と接触させると共に、該候補トレーサー分子と該細胞との相互作用を測定する工程、及び
c)工程a)の該細胞と結合するが、工程b)の該細胞とは結合しない候補トレーサー分子をβ細胞特異的トレーサー分子として保持する工程を含む、方法。 - FXYD2−γ−a陽性細胞型又は細胞株が、齧歯類膵島、ラットINS−1E、AR42J細胞、及びヒトのCAPAN−2細胞から成る群から選択され、並びにFXYD2−γ−a陰性細胞型又は細胞株がPANC−1である、請求項13に記載の方法。
- FXYD2−γ−a陽性細胞に特異的に結合する新たな膵β細胞特異的トレーサー分子を特定するためのキットであって、FXYD2−γ−a陽性細胞型又は細胞株が、齧歯類膵島、ラットINS−1E、AR42J細胞、及びヒトのCAPAN−2細胞から成る群から選択され、並びにFXYD2−γ−a陰性細胞型又は細胞株がPANC−1である、2つの細胞型又は細胞株を含む、キット。
- FXYD2−γ−a陽性細胞に特異的に結合する新たな膵β細胞特異的トレーサー分子を特定するin vitro方法であって、
a)in vitroにおいて、候補トレーサー分子をFXYD2−γ−a分子と接触させると共に、該候補トレーサー分子と該FXYD2−γ−a分子との相互作用を測定する工程、及び
b)工程a)の該FXYD2−γ−a分子に結合する候補トレーサー分子を、膵β細胞特異的トレーサー分子として保持する工程を含む、方法。 - 膵臓組織試料中の膵β細胞を検出する方法であって、前記組織試料中の膵β細胞の標識としてFXYD2−γ−aアイソフォームを使用することによる、方法。
