Description
(i)ドーパミン補充遺伝子療法は、緊張性および局所性の両方の線条体のドーパミンレベルを背側線条体間で回復させる。L−Dopa処置とは違って、ドーパミン補充遺伝子療法は、腹側(「認知」)線条体または前頭前皮質に過剰投与することなく、背側の(「運動」)線条体においてドーパミンを増加させる。このことは、腹側線条体に関連する認知機能の障害を回避する。
(ii)ドーパミン補充遺伝子療法は、線条体においてドーパミン作動性緊張を回復することによりジスキネジアを予防する一方で、幹細胞移植に付随する問題を回避する。本発明者らは、任意のドーパミン補充戦略の成功の主要な要因の一つが、線条体を神経支配する黒質のドーパミン作動性末端で発現されるドーパミン能動輸送体(DAT)の不在であることを見出した。幹細胞/胎児組織戦略は通常、ドーパミン作動性細胞を患者の黒質に移植することを伴う。DATのレベルは、移植組織において比較的高い。理論に縛られることを望むものではないが、本発明者らは、DATの存在が、移植細胞により産生される細胞外ドーパミンを隔離し、それによりドーパミン作動性緊張を回復させる治療法に対する能力を低下させるものと予測する。本発明で使用されるドーパミン補充遺伝子療法において、ベクターは、ドーパミン合成に関与する遺伝子を線条体に送達する。線条体におけるドーパミン作動性末端の数、および従ってDATレベルは、パーキンソン病の患者において低下するため、従ってドーパミン産生がより効果的である。
(iii)本発明において用いられるドーパミン補充遺伝子療法戦略は、パーキンソン病に関連する運動機能障害を矯正する。この治療法は、淡蒼球内節(GPi)などの出力核の異常な活動亢進を正常化することが見出された。GPiにおけるニューロン発火のパターンの分析により、本発明において用いられるドーパミン補充遺伝子療法戦略が、バースト当たりのスパイクの数およびバースト事象の数を非PD対照に観察されるものと同様のレベルまで低下させることが明らかとなった。本発明において用いられるドーパミン補充遺伝子療法戦略はまた、視床下核(STN)の代謝活性を正常化した。STNにおけるニューロンの活動亢進は、PD被験体において検出可能な代謝活性の増加を引き起こすPDの病態生理学的特徴である。
(iv)本発明において用いられるドーパミン補充遺伝子療法戦略は、線条体において産生される生理学的レベル以下(sub−physiological levels)のドーパミンを引き起こす。しかし、これは、ドーパミン作動性緊張を回復させるため、運動障害を矯正するため、薬物誘発ジスキネジアを防ぐため、かつ主要な基底核出力核の正常な生理機能を回復させるために十分であった。及び、
(v)本発明において用いられるドーパミン遺伝子補充戦略は、経口L−DOPA投与に関連するジスキネジアを寛解することができる。このことは、遺伝子に基づくアプローチが、長期のL−Dopa処置により既にジスキネジアを発症しているPD患者において治療効力をもたらし、かつ、ジスキネジア生理学的機構を逆転させる、両方の可能性があり、従ってL−Dopa処置法の後に続いて起こる事態を防ぐことを意味する。

ベクター系も提供する。
(i)ドーパミン輸送体(DAT)による細胞外ドーパミンの隔離を回避することによる、被験体においてパーキンソン病を治療および/または予防するためのドーパミン補充遺伝子療法の効力を増大させるための、
(ii)被験体の背側および腹側の両方の線条体において一定の生理学的ドーパミン作動性緊張を維持するかまたは回復させるために、ドーパミン補充遺伝子療法を使用することにより認知障害を引き起こすことなく、被験体においてパーキンソン病を治療および/または予防するための、

(iv)ドーパミン補充遺伝子療法のためのベクター系の被験体への投与によりパーキンソン病被験体において経口L−dopa投与に関連するジスキネジアを治療および/または予防するための、
本明細書に定義されるベクター系の使用も提供する。
(i)少なくとも一つの核酸配列が、N末端タグを欠いている。
(ii)少なくとも一つの核酸配列が、コドン最適化されている。
(iii)ベクター系が3シストロン性カセットを含む場合、3シストロン性カセット中の遺伝子の順序が、TH−AADC−CH1である。
(iv)gagの少なくとも一つの潜在的ATG開始コドンが、ATTGに変更されている。
(v)Neo発現カセットが、gagの下流に挿入されている。及び

パーキンソン病(PD)は、黒質線条体経路の喪失を特徴とする神経変性障害である。パーキンソン病の原因は不明であるが、それはドーパミン作動性(チロシンヒドロキシラーゼ(TH)陽性)中脳ニューロンの進行性の死に関連し、運動機能障害を誘発する。パーキンソン病の特徴的な症状は、TH陽性黒質線条体ニューロンの70%までが変性した時に現れる。
遺伝子治療は、予防もしくは治療遺伝子を被験体に導入、置換、変更、または補充することによる疾患の予防および/または治療である。
本発明のドーパミン補充遺伝子療法では、ドーパミン合成に関与する遺伝子は、ベクター系、例えばウイルスベクター系などにより被験体に送達される。
(i)少なくとも一つの核酸配列が、N末端タグを欠いている。
(ii)少なくとも一つの核酸配列が、コドン最適化されている。
(iii)ベクター系が3シストロン性カセットを含む場合、3シストロン性カセット中の遺伝子の順序が、TH−AADC−CH1である。
(iv)gagの少なくとも一つの潜在的ATG開始コドンが、ATTGに変更されている。

ポリヒスチジンタグまたはFLAG(商標)タグのようなタグは、タンパク質精製、またはタグ特異的抗体を用いるタンパク質の検出を助けるためにタンパク質のN末端で一般的に使用される。タグは、タンパク質をコードするDNAを、そのタグをコードする配列を含むベクターに挿入することにより付加されてもよく、タグは自動的にコード配列の中に含められる。あるいは、タグをコードする配列を開始コドンに隣接して有するプライマーを用いてPCRを行ってもよい。
コドンの最適化は、国際公開第99/41397号に既に記載されている。異なる細胞は、特定のコドンの使用頻度が異なる。このコドンの偏りは、細胞種中の特定のtRNAの相対存在量の偏りに相当する。配列中のコドンを変更して、コドンを対応するtRNAの相対存在量に一致するように作り変えることにより、発現を増加させることができる。同様に、対応するtRNAが特定の細胞種において希少であることが公知のコドンを意図的に選択することにより、発現を低下させることができる。よって、さらなる翻訳の程度の制御が可能である。
コドンの最適化は、その他の利点も多くある。プロデューサー細胞/パッケージング細胞においてウイルス粒子の構築に必要なウイルス粒子のパッケージング成分をコードするヌクレオチド配列は、配列を変更することによって、RNA不安定配列(INS)を配列から除去する。同時に、パッケージング成分のためのアミノ酸配列コード配列は保持されるので、その配列にコードされるウイルス成分は同じままであるか、またはそのパッケージング成分の機能が損なわれない程度に少なくとも十分に類似している。また、コドンの最適化により、搬出のためのRev/RREの必要性が克服され、最適化された配列をRev非依存性にする。また、コドンの最適化により、ベクター系内の異なる構築物間の(例えばgag−polおよびenvオープンリーディングフレーム中の重複部分の領域間)相同組換えが減少する。したがって、コドンの最適化の全体的な効果は、ウイルス力価の顕著な増加および安全性の向上である。
翻訳が正確な開始コドン(ATG)から始まることを確保するために、gagの上流の開始コドンを操作してもよい。便宜的には、当技術分野で公知の技法によって上流の開始コドンを置換により、例えばATGをACGに、または挿入により、ATGをATTGに変異させる。
オープンリーディングフレーム、またはその一部分を、ウイルスLTRの下流および内部プロモーターの下流に挿入することは、revの不在下でウイルス力価を高めることが示されている(国際公開第03/064665号に記載される通り)。従って好ましい実施形態では、Neo発現カセットがgagの下流に挿入される。
ウイルスゲノムは、翻訳後調節エレメントを含むことができる。例えば、ゲノムはウッドチャック肝炎ウイルス転写後調節エレメント(WPRE)などのエレメント、例えば米国特許出願公開第2005/0002907号に記載されるものなどを含むことができる。
国際公開第02/29065号には、チロシンヒドロキシラーゼ(TH)、芳香族L−アミノ酸デカルボキシラーゼ(AADC)、およびGTPシクロヒドロラーゼ1(CH1)をコードする遺伝子を宿主細胞に送達する能力のある3シストロン性レンチウイルスベクターが記載されている。培養細胞において3つの酵素の発現が、ドーパミン、L−DOPAおよびDOPACの産生を引き起こし、パーキンソン病のげっ歯類モデルに対して治療上有効であることが示される。
アデノ随伴ウイルスベクター(AAV)も、ドーパミン合成に関連する遺伝子を脳に送達するために使用されている。2または3つの重要な遺伝子を送達するために別々のAAVベクターを使用することは、PDのラットおよび非ヒト霊長類(NHP)モデルにおいていくらかの行動上の利点があることが実証された(Kirik et al(2002)PNAS 99:4708− 4713;Muramatsu et al(2002)Human Gene Therapy 13:345−354)。
本発明で使用されるベクター系のウイルスゲノムは、2つまたはそれ以上のNOIを含む。両方のNOIを発現させるために、一つのNOIごとに一つの、2つまたはそれ以上の転写単位がベクターゲノム内に存在してよい。レトロウイルスベクターは、それらが遺伝子的に単純な状態が保たれるのであれば、最大力価および最も強力な遺伝子発現特性を達成する、そのため、ポリシストロン性メッセージにおいて第2の(そしてその後の)コード配列の翻訳を開始するためには配列内リボソーム進入部位(IRES)を使用することが好ましい。

IRESおよびNOIの代替配置を利用することもできる。例えば、IRESおよびNOIを含有する転写物は、必ずしも同じプロモーターに由来する必要はない。

本発明で使用されるドーパミン補充遺伝子療法のためのベクターは、医薬組成物中に存在してもよく、その組成物は、予防的上または治療上有効な量のベクターを含む。
ジスキネジアは随意運動能力の機能障害であり、結果として断片的または不完全な運動を生じる。
本発明で使用される遺伝子治療戦略は、非処理PD線条体に関連するレベルよりも高いが、非PD線条体に関連するレベルよりも低いドーパミンのレベルが実現されるように、線条体においてドーパミン合成を誘導することができる。ベクター系は、線条体において生理学的レベル以下のドーパミンの産生を引き起こすことができる。
ドーパミン輸送体またはドーパミン能動輸送体(Dopamine Active Transporter)(DAT)は、黒質のドーパミン作動性ニューロンのシナプス終末の血漿膜に局在する内在性膜タンパク質であり、線条体に位置する。DATは、シナプス間隙からリサイクル(recycles)ドーパミンを除去し、ドーパミンシグナルを終了させる。
DATが少ないかまたはDATを含まない組織への投与への代替アプローチまたはさらなるアプローチは、一つ以上のDAT抑制剤を使用することによってDATによるドーパミンの隔離を抑制することである。
ベクター系は、siRNAまたはマイクロRNAまたはshRNAまたは調節されたマイクロもしくはshRNAを含むかまたはコードすることができる(Dickins et al.(2005)Nature Genetics 37: 1289−1295,Silva et al.(2005)Nature Genetics 37:1281−1288)。
経口L−Dopa処置は、認知障害に関連する可能性がある。
パーキンソン病に関連する運動機能障害は、運動を制御する脳深部構造である大脳基底核の機能障害から生じると考えられている。
パーキンソン症を矯正するためのTH、AADCおよびCH1の遺伝子導入の可能性を調査するため、PDのMPTP霊長類モデルで長期実験を行った。TH、AADCおよびCH1の遺伝子をコードする3シストロン性レンチウイルスベクター(Lenti−TH−AADC−CH1)を設計した。MPTP中毒状態の休止後1〜8週、18頭のMPTP処置マカクザルを3つの行動的に等しい群に割り当てた。第1群(MPTP−Lenti−TH−AADC−CH1、n=6)は、各々の運動被殻にLenti−TH−AADC−CH1を両側注入した。第2群(MPTP−Lenti−lacZ、n=6)は、LacZレポーター遺伝子をコードする対照EIAVベクターを投与した。第3群(MPTP−長期、n=6)には外科的介入は行わなかったが、MPTPモデルの安定性を評価するための追加対照として含めた。全ての動物は、実験の間中L−Dopaまたはドーパミン作用薬を用いる処置を行わずに維持された。
インビボ遺伝子導入およびレンチウイルス媒介性ドーパミン産生を調べるため、導入遺伝子発現の組織学的解析を行って実験動物の線条体において局所ドーパミンレベルを測定した。Lenti−LacZで処置した動物は、注入された被殻あたり平均54947の形質導入細胞を有し、それは大部分がニューロンであったことを実証した(NeuN−ir陽性>90%、図10)。また、組織学的解析は、TH、AADCおよびCH1陽性ニューロンがMPTP Lenti−TH−AADC−CH1処置動物において被殻の注入部位の近傍において明らかであったが、MPTP−Lenti−LacZ対照では明らかでないことも実証した(図2)。
Lenti−TH−AADC−CH1が運動機能障害を矯正した機構を決定するため、基底核系内のニューロン活動を調査した。PDにおける基底核機能障害の現行モデルは、淡蒼球内節(GPi)などの出力核の異常な活動亢進が、この疾患において観察される運動症状の主な原因となることを示唆している。Lenti−TH−AAADC−CH1が基底核出力核におけるニューロンの電気的活動を正常化することがあり得るかどうかを決定するため、正常およびMPTP処置したマカクザルにGPiでの単一記録を受けさせた。これまでの報告に一致して、対照と比較して、薬物にナイーブな非処置MPTPマカクザルにおいて、GPiニューロンの平均発火率の有意な増加(52%、MW;p<0.01)が見出された(図4a)。興味深いことに、線条体のLenti−TH−AAADC−CH1投与は、異常な高い発火率を有意に低下させ、GPiニューロンの発火率を正常な(病変のない)レベルに回復させた(図4a)。
PDにおける主な課題は、運動障害性の動作を何ら誘導することなくドーパミン作動性機能を回復させることである。線条体の組織学的異常はジスキネジアを誘導する可能性があるので、遺伝子導入の後の形態変化を調査した。全ての針トラックは被殻に位置していたが(図14)、ニューロンマーカー(Neu−N)は、Lenti−TH−AADC−CH1を注入した動物において異常を示さなかった。GFAPおよびCD68免疫活性のわずかな増加がLenti−LacZ動物およびLenti−TH−AADC−CH1動物の両方で観察された。しかし、これは、針トラックを取り囲む領域に限定された(図15)。局所的な脳病理の検出のための感受性の高い方法であるT2*強調MRIを用いて、神経画像研究を行った。被殻および残りの脳領域には何ら異常なT2*シグナルのないことが見出された(図16)。
PDにおけるジスキネジアの発症に対する主な機構的仮説は、ジスキネジア発症が、黒質の線条体路のドーパミン作動性ニューロンの広範な変性と合わせて、線条体の後シナプス受容体の間欠的な拍動性ドーパミン作動性刺激の結果であるというものである。ここに、レンチウイルスによるドーパミンの線条体への連続送達が、運動障害の長期の矯正(30ヶ月まで)を媒介し、反復L−Dopa処置とは対照的に、ジスキネジアの発生をもたらさなかったことが実証される。このことは、薬物誘発ジスキネジアを引き起こすのが、黒質の変性自体の程度ではなく、むしろ線条体における一定のドーパミン作動性緊張の不在であることを示す。
<レンチウイルスベクター技術>
TH、AADCおよびCH1の遺伝子をコードする3シストロン性レンチウイルスベクターを設計した(Lenti−TH−AADC−CH1)。ベクター媒介性のドーパミン産生を改善するため、pONY8.1TSINと呼ばれる3シストロン性カセットを発現した元のEIAVベクターゲノムにいくつかの変更を行った(Azzouz et al(2002)J.Neurosci.22:10301−10312)。これらの変更により、ヒトHEK293T細胞の形質導入後にインビトロで評価した時に、統合されたゲノム当たり少なくとも2ログのドーパミン産生の増加がもたらされた(図6)。
非ヒト霊長類において進行型PDをモデル化するため、無動症、屈曲姿勢、平衡機能障害および振戦を含む、重度の安定した両側性パーキンソン病様症状群となるまで、成体カニクイザルに選択的神経毒MPTPを全身投与した。MPTP処置の前に、全ての霊長類は臨床評定尺度(CRS)で0を得た。MPTP後であるが、レンチウイルス注入の前にマカクザルは、対照MPTP前状態と比較して(正常状態)、最大の能力障害スコア(最大値=14)に接近するCRSの大幅な増加を示した(図1a)。定量的ビデオ運動分析を用いて運動機能障害の重篤度をさらに定量化した。すると、正常状態と比較して、MPTPマカクザルは顕著な無動症(移動距離=正常状態の3.7%;図1b)および姿勢機能障害(立ち上がり行動=正常状態の5%;図S2)を示したことが見出された。MPTP誘発パーキンソン症の重篤度は、対照MPTP動物において実験全体にわたって安定していた(図1および7)。神経病理学的分析により、黒質緻密部における、構造的損失と機能的損失の両方を含む、選択的黒質線条体変性(図8)ならびに線条体におけるTHおよびAADC免疫反応性線維の劇的な減少(図2)が実証された。TH(図2)およびドーパミン輸送体(DAT)免疫反応性(図9)により評価される線条体の除神経は、PDにおいて観察されるものに似た異質なパターンの変性を示し、被殻は尾状核よりも多く罹患し、かつ、被殻の背外側部分(「運動」被殻)はその腹側部分(「認識」被殻)よりも多く罹患していた。
Claims (2)
- チロシンヒドロキシラーゼ(TH)、アミノ酸DOPAデカルボキシラーゼ(AADC)及びGTPシクロヒドロラーゼ1(CH1)をコードする核酸配列を含む単一のベクターを含むレンチウイルスベクターシステムを含んでなる、パーキンソン病被験体における経口L−Dopa投与に関連するジスキネジアの改善のための医薬組成物。
- ベクターシステムは、一以上の下記の特徴、
(i)少なくとも一つの核酸配列が、N末端タグを欠いている、
(ii)少なくとも一つの核酸配列が、コドン最適化されている、
(iii)ベクター系が3シストロン性カセットを含む場合、3シストロン性カセット中の遺伝子の順序が、TH−AADC−CH1である、
(iv)gagの少なくとも一つの潜在的ATG開始コドンが、ATTGに変更されている、
(v)Neo発現カセットが、gagの下流に挿入されている、及び
(vi)ベクター系が3シストロン性カセットを含む場合、発現を強化するためにウッドチャック肝炎ウイルス転写後調節エレメント(WPRE)が3シストロン性カセットの3’末端に挿入されている、
を有する請求項1に記載の医薬組成物。
