Description

本明細書は、一酸化窒素発生のためのシステム及び装置に関する。
フリーラジカルであるニトロシルラジカルとしても公知の一酸化窒素(NO)は、重要なシグナル伝達分子である。例えば、NOは血管の平滑筋を弛緩させ、これにより血管拡張がなされ、該血管を通る血流を増加させる。これらの効果は、NOが生存期間数秒の高反応性であり、体内において迅速に代謝されるために、小規模な生物学的領域に限定される。
簡潔に、かつ一般的用語で、様々な実施態様は、一酸化窒素(NO)発生のためのシステム及び装置に関する。一実施態様により、該装置は、入口、出口を有する本体、及び該本体内に位置する多孔質固体マトリクスを含む。一実施態様において、該多孔質固体マトリクスは、シリカゲル及び熱可塑性樹脂から作製される。該多孔質固体マトリクスは、抗酸化剤の水溶液で被覆されており、ここで、該入口は、ガス流を受け取り、該ガス流が該多孔質固体マトリクスを通過し、該ガス流中の二酸化窒素を一酸化窒素に変換させて、該出口まで流体連通するように構成されている。該多孔質固体マトリクスは、該装置を任意の方向で使用させるようにする。また該多孔質固体マトリクスは、輸送及び操作と関連した振動及び誤用に耐えるのに適した剛性構造をなしている。
一酸化窒素(NO)を発生するための様々なシステム及び装置が、本明細書中に開示される。一般に、NOは吸引するか又は別の方法で患者の肺まで送達される。NOが吸引されると、身体の他の血管を付随して拡張することなく、非常に高い局所的用量を達成することができる。したがって、およそ10〜およそ1000ppm(例えば、10を上回り、10、40、80、100、150、200、250、300、350、400、450、500、550、600、650、700、750、800、850、900、950又は1000ppm)の濃度を有するNOガスを、患者に送達することができる。したがって、高用量のNOを使用して、下記疾患の進行を防止する、逆行させる又は抑制することができる;該疾患には、急性肺血管狭窄、外傷、誤嚥又は気道熱傷、肺の脂肪塞栓症、アシドーシス、肺の炎症、成人呼吸促迫症候群、急性肺水腫、急性高山病、心臓手術後の急性肺高血圧症、新生児の遷延性肺高血圧症、周生期吸引症候群、硝子膜症、急性肺動脈血栓塞栓症、ヘパリン-プロタミン反応、敗血症、喘息、気管支喘息重積発作又は低酸素症を含むことができるが、これに限定されない。またNOを使用して、慢性肺高血圧症、気管支肺異形成症、慢性肺血栓塞栓症、特発性肺高血圧症、原発性肺高血圧症又は慢性低酸素症を治療することができる。
Claims (14)
- 二酸化窒素から一酸化窒素を発生するための装置であって、
入口、出口及び分流器を含む本体;
該本体内に配置された多孔質固体マトリクス;並びに
該本体と該多孔質固体マトリクスとの間の空間;を含み、
前記多孔質固体マトリクスが抗酸化剤で被覆され、かつ前記入口がガス流を受け取るように構成され、前記分流器が該ガス流を前記本体と多孔質固体マトリクスとの間の空間へ向かわせ、かつ該ガス流が前記多孔質固体マトリクスを通過し前記出口まで流体連通されて、該ガス流中の二酸化窒素を一酸化窒素に変換する、前記一酸化窒素発生装置。 - 前記本体の入口に連通している加湿器を更に含む、請求項1記載の装置。
- 前記抗酸化剤が、アスコルビン酸、αトコフェロール又はγトコフェロールである、請求項1記載の装置。
- 前記固体マトリクスが、熱可塑性樹脂と混合されたシリカゲルを更に含む、請求項1記載の装置。
- 前記シリカゲルの密度が、前記熱可塑性樹脂の密度と同等である、請求項4記載の装置。
- 前記多孔質固体マトリクスが、少なくとも20%のシリカゲルを含む、請求項4記載の装置。
- 前記多孔質固体マトリクスが、およそ20%〜およそ60%のシリカゲルを含む、請求項4記載の装置。
- 一酸化窒素を患者に送達するためのシステムであって、
二酸化窒素、四酸化二窒素又は一酸化窒素のガス供給源;
入口、出口及び分流器を含む本体、該本体内に配置された多孔質固体マトリクス、並びに該本体と該多孔質固体マトリクスとの間の空間を有する第1の装置であって、前記多孔質固体マトリクスが抗酸化剤の水溶液で被覆され、かつ前記入口が前記供給源からのガス流を受け取るように構成され、前記分流器が該ガス流を前記本体と多孔質固体マトリクスとの間の空間へ向かわせ、かつ該ガス流が前記多孔質固体マトリクスを通過し前記出口まで流体連通されて、該ガス流中の二酸化窒素を一酸化窒素に変換する、前記第1の装置;並びに
一酸化窒素を患者に送達する、前記第1の装置の出口と連結した患者用インターフェイス;を含む、前記一酸化窒素送達システム。 - 前記ガス供給源と前記第1の装置との間に配置された加湿器を更に含む、請求項8記載のシステム。
- 前記加湿器が、前記第1の装置と一体となっている、請求項9記載のシステム。
- 前記患者用インターフェイスより前に配置された加湿器を更に含む、請求項8記載のシステム。
- 前記第1の装置の後に位置する第2の加湿器;及び
第2の加湿器の後に配置された第2の装置;を更に含み、
該第2の装置は、入口、出口及び抗酸化剤の水溶液で被覆された多孔質固体マトリクスを含み、該入口は、第1の装置からのガス流を受け取り、該ガス流を該固体マトリクスに通過させ、該ガス流中の二酸化窒素を一酸化窒素に変換させて、該出口まで流体連通するように構成されている、請求項8記載のシステム。 - 前記抗酸化剤が、アスコルビン酸、αトコフェロール又はγトコフェロールである、請求項8記載のシステム。
- 前記多孔質固体マトリクスが、ポリマーと混合されたシリカゲルを更に含む、請求項8記載のシステム。
