Description
〔技術分野〕
本発明は、組織を修復及び/又は再生するための組織工学スカフォールドに関する。具体的には、本発明は、組織工学スカフォールド、並びにアニオン性多糖類/構造タンパク質コーティングを含む複合装置に関する。
創傷治癒に有用な組織工学スカフォールドは当該技術分野において既知である。これらのスカフォールドは、基材を提供して、細胞の内方成長を支持するが、これらは細胞の内方成長を誘導する最適な刺激を提供できない。
〔発明が解決しようとする課題〕
細胞の内方成長に対する刺激、並びに、細胞遊走及び増殖のための持続した構造的支持体を提供する慢性創傷療法に対する必要性が依然として存在している。
組織工学スカフォールド、並びにアニオン性多糖類/構造タンパク質コーティングを含む複合装置、並びにその使用を開示する。具体的には、組織工学スカフォールドは、生体適合性の生分解性ポリマーを含む多孔質布又は発泡体であり、該アニオン性多糖類/構造タンパク質コーティングは、55/45コラーゲン/酸化再生セルロース(コラーゲン/ORC)マトリックスである。本発明はまた、1つ又はそれ以上の滅菌複合装置を含む、キットを目的とする。
複合装置及び対照の調製
シートの形態である以下の複合装置を調製した。コラーゲン/ORCでコーティングされた不織布スカフォールドを含む複合装置、及びコラーゲン/ORCでコーティングされた発泡体スカフォールドを含む複合装置。対照は、不織布スカフォールドのみ、発泡体スカフォールドのみ、及びコラーゲン/ORC発泡体のみを含む。
67.5ミリグラム/立方センチメートルの密度及び1.86mmの厚さを有する、90/10モル%のポリ(グリコール酸−コ−乳酸)(PGA/PLA)不織布スカフォールドを用いた。不織布スカフォールドは、イソプロピルアルコール(IPA)に浸漬し、約30分間超音波処理することにより洗った。IPAをデカントで取り除き、スカフォールドを脱イオン水(DI)で3度すすいだ。不織布スカフォールドを、次いで、DI水に浸漬し、約30分間超音波処理した。不織布スカフォールドを、続いて乾燥窒素流下で乾燥させ、その後高真空下で(<150マイクロメートル)最低30分間乾燥させた。不織布スカフォールドを、1.5×1.5平方センチメートルに切断した。スカフォールドを、エチレンオキシドを用いて滅菌した。
35/65モル%のポリ(イプシロン−カプロラクトン−コ−グリコール酸)(PCL/PGA)コポリマー(エチコン社(Ethicon, Inc.)、ニュージャージー州、サマービル(Somerville))1,4−ジオキサンの5重量/体積%溶液を、溶媒9部あたり1部のポリマーを添加することにより調製した。ポリマー及び溶媒を、ポリマーが溶解するまで、攪拌しながら約60℃でおよそ4〜8時間加熱した。ポリマー溶液を濾過し、約10グラムの溶液を、およそ114mm(4.5インチ)の正方形であり、深さがおよそ12.7mmであるアルミニウムの型に注ぎ入れた。ポリマー溶液を充填した型を、約−17℃に予冷しておいたFTSデュラ(Dura)凍結乾燥機の棚に置いた。約15分後、周期を開始し、温度を約60分間−17℃に保持した。次に、真空を適用して、およそ13.3Pa(100mトール)で約60分間昇華させることにより、ジオキサンの一次乾燥を開始させた。次の二次乾燥は、約5℃で約60分間、及び約20℃で約60分間実施した。真空をおよそ2.7Pa(20mトール)で維持した。凍結乾燥機を室温にし、乾燥窒素で真空を破壊した。乾燥窒素を約30分間パージした後、発泡体を型から持ち上げた。発泡体スカフォールドを、1.5×1.5センチメートルの正方形に切断した。スカフォールドを、エチレンオキシドを用いて滅菌した。
市販の酸化再生セルロースである、サージセル(SURGICEL)(エチコン社(Ethicon, Inc.)、ニュージャージー州、サマービル(Somerville)))を、温度を約60℃未満に維持しながら、スクリーンプレートを通してロータリーナイフカッターを用いて粉砕した。水中1.39%固形分のコラーゲン懸濁液を、米国特許第6309454B1号の方法に従って、牛革からそぎ落としたウシ真皮から調製した。197.84gの懸濁液を用いて、乾燥重量2.75gのコラーゲンを提供した。2.25gの粉砕したORCを添加して、55/45比のコラーゲン/ORC固体を提供した。0.05M酢酸を最終体積が500mLになるまで添加し、混合物を3×30秒、低速でワーリングブレンダーにてブレンドした。スラリーを次いで、真空オーブン内で約15分間脱気した。
9.85gのコラーゲン/ORCスラリーをペトリ皿に注ぎ入れた。ペトリ皿を、次いで、約45分間、約−27℃の冷凍庫に入れた。別の9.85gのスラリーをペトリ皿に添加し、それを約45分間、約−27℃の冷凍庫に戻した。冷凍されたコラーゲン/ORCスラリーを収容しているペトリ皿を、次いで凍結乾燥して、約−20℃で約24時間発泡体を乾燥させた。コラーゲン/ORC発泡体を、1.5×1.5センチメートルの正方形に切断した。発泡体を、エチレンオキシドを用いて滅菌した。
9.85gのコラーゲン/ORCスラリーをペトリ皿に注ぎ入れた。正方形のスカフォールド(上記で調製した発泡体又は不織布のいずれか)を、スラリーに完全に浸漬した。スカフォールド及びコラーゲン/ORCスラリーを収容しているペトリ皿を、次いで凍結乾燥して、約45分間、約−27℃の冷凍庫に入れた。別の9.85gのスラリーをペトリ皿に添加し、それを約45分間、約−27℃の冷凍庫に戻した。冷凍されたスカフォールドコラーゲン/ORCスラリーを収容しているペトリ皿を、次いで凍結乾燥して、約−20℃で約24時間発泡体を乾燥させた。複合装置の不要な部分を切り落として、過剰なコラーゲン/ORC発泡体を除去した。複合装置を、エチレンオキシドを用いて滅菌した。
ブタ創傷治癒モデルにおける複合装置の使用
この研究では、全層切除欠損における細胞湿潤及び複合スカフォールドの創傷治癒反応を測定した。全層切除創傷(1.5×1.5センチメートルの正方形)を、4匹のブタの背部に作製した。対照群は、実施例1に記載のように調製した、コラーゲン/ORC発泡体、PCL/PGA発泡体スカフォールド、及びPGA/PLA不織布スカフォールドであった。複合装置群は、実施例1に記載のように調製した、発泡体複合装置及び不織布複合装置であった。治療群のブタをランダム化し、研究期間中所定の位置に放置した。治療群あたりのn数は6であった。
組織学的評価
肉芽組織の最大高さ、スカフォールド又は複合装置の存在、及び活性化マクロファージの存在について、ヘマトキシリン及びエオシンによる組織染色並びにマッソン三色染色を用いて、切片を分析した。
複合装置の臨床応用
複合装置で治療を受ける予定の患者は、更に前処理することなく創傷に装置を直接適用される。いくつかの慢性創傷は、複合装置の取り込むために創傷床を最適化する必要がある場合がある。これは、外科的、機械的、化学的、自己分解法又は蛆虫療法のいずれかにより達成される。創面切除では、必要に応じて局所麻酔が適用される。壊死又は生育不能な組織を、種々の方法を用いて取り除く。外科的創面切除では、鋭利器具、外科用メス、キューレット、又はレーザーを用いて、特に感染が創傷と関連するとき、大量の壊死組織を除去する。機械的創面切除は、ガーゼを用いて死んだ組織を取り除く、安価な切除手段を提供する。このアプローチは外科的医療技術を必要としないので、創傷ケア環境下で看護士にも容易に達成される。更に、自己分解、酵素的、又は生物学的前処理は、創傷床の調製に対する非外科的アプローチを提供する。
(1) 組織工学スカフォールド及びアニオン性多糖類/構造タンパク質コーティングを含む、慢性創傷の治療のための複合装置であって、前記組織工学スカフォールドが生体適合性の生分解性ポリマーであり、前記アニオン性多糖類がムコ多糖又はポリカルボキシレートである、複合装置。

(7) 前記エラストマーコポリマーが、イプシロン−カプロラクトン及びグリコリドを含む、実施態様6に記載の複合装置。

(16) 前記スカフォールドが、イプシロン−カプロラクトンのグリコリドに対するモル比が約35:65である、イプシロン−カプロラクトンとグリコリドとのエラストマーコポリマーを含む発泡体であり、前記アニオン性多糖類/構造タンパク質コーティングが、約45:55のモル比を有する酸化再生セルロース/コラーゲンである、実施態様1に記載の複合装置。
(17) 前記スカフォールドが、約90:10のモル比を有するグリコリドとラクチドとのコポリマーを含む不織布であり、前記アニオン性多糖類/構造タンパク質コーティングが、約45:55のモル比を有する酸化再生セルロース/コラーゲンである、実施態様1に記載の複合装置。
(18) a)創傷を1つ又はそれ以上の複合装置と接触させることであって、前記装置が、アニオン性多糖類/構造タンパク質コーティングを有する、生体適合性の生分解性組織工学スカフォールドを含む、創傷を接触させることと、

- 組織工学スカフォールド及びアニオン性多糖類とコラーゲンとのコーティングを含む、慢性創傷の治療のための複合装置において、前記組織工学スカフォールドが、スポンジ、発泡体、穿孔フィルム及び布地からなる群から選択され、生体適合性の生分解性脂肪族ポリエステルから形成されており、前記アニオン性多糖類が、繊維粒子又は粉末粒子の形態の酸化再生セルロースである、複合装置。

- 請求項1に記載の複合装置において、前記スカフォールドが、イプシロン−カプロラクトンのグリコリドに対するモル比が35:65〜65:35である、イプシロン−カプロラクトンとグリコリドとのエラストマーコポリマーを含む発泡体であり、酸化再生セルロースとコラーゲンとの前記コーティングが、10:90〜90:10のモル比を有する、複合装置。
- 請求項1に記載の複合装置において、前記スカフォールドが、イプシロン−カプロラクトンのグリコリドに対するモル比が30:70〜70:30である、イプシロン−カプロラクトンとグリコリドとのエラストマーコポリマーを含む発泡体であり、酸化再生セルロースとコラーゲンとの前記コーティングが、45:55のモル比を有する、複合装置。
- 請求項1に記載の複合装置において、前記スカフォールドが、90:10のモル比を有するグリコリドとラクチドとのコポリマーを含む不織布であり、酸化再生セルロースとコラーゲンとの前記コーティングが、45:55のモル比を有する、複合装置。
- 請求項1に記載の滅菌複合装置を含む、キット。
- 請求項1に記載の複合装置において、前記装置が、前記創傷内で活性化マクロファージの存在を増加させることができる、複合装置。
