本開示は、多部品外科手術用ステープルアセンブリに関する。より特定すると、本開示は、組織のステープル線にわたって均一な圧縮を提供するための、増大した圧縮領域を有する多部品外科手術用ステープルアセンブリに関する。
管状組織セクションの手術を伴う外科手術処置の間、この管状組織セクションを切断または区分して、これらの組織セクションをステープル留めして閉じることがしばしば必要である。この型の外科手術の１つの例は、管状組織セクションの疾患セクション（例えば、結腸のセクション）を切り出すこと、およびこれらの管状組織セクションの健常な自由端を再度付着させる前に、これらの自由端を一時的にステープル留めして閉じることを包含する。ステープルの線または列は、代表的に、この管状組織セクションを横切って配置される。いくつかの例において、これらの管状組織セクション内に流体圧が残り、そしてこれらの管状組織セクションを横切って配置されたステープル線に応力または力を付与する。

本開示の外科手術用ステープルアセンブリ、およびこの外科手術用ステープルアセンブリを形成するための関連するアンビルの実施形態が、ここで図面を参照しながら詳細に記載される。図において、同じ番号は、数枚の図の各々において、同じかまたは対応する要素を表す。当該分野において通常であるように、用語「近位」とは、使用者または操作者（すなわち、外科医または医師）に近い方の部品または構成要素をいい、一方で、用語「遠位」とは、使用者から遠い方の部品または構成要素をいう。
最初に図１を参照すると、外科手術用ステープラー１２と共に使用するためのアンビル１０が開示されている。外科手術用ステープラー１２は、開腹外科手術処置のために代表的に使用される型のものであり、そしてピストルグリップハンドル１４を有し、このハンドルは、ハンドル１４から遠位に延びる細長部材１６を有する。細長部材１６は、ほぼＵ字型のアンビル支持体１８で終結する。アンビル１０は、アンビル支持体１８に設置される。外科手術用ステープラー１２はまた、ステープル収容ヘッドまたはステープルカートリッジ２０を備え、このカートリッジは、本明細書中以下でより詳細に記載されるように、複数のステープルを収容する。調節ノブ２２が、ハンドル１４に提供され、このノブは、ステープルカートリッジとアンビルとを接近させて、ステープル留めされるべき組織をこれらの間に捕捉する目的で、公知の様式で、ステープルヘッド２０をアンビル１０に対して移動させるために作動する。トリガ２４が提供され、このトリガは、ステープラー１２を起動させ、そしてステープルヘッド２０からステープルを排出し、組織を通してアンビル１０に入れる。トリガロック２６が提供され、このトリガロックは、トリガ２４の移動を防止し、これによって、外科手術用ステープル１２の尚早な起動を防止する。開腹型の外科手術用ステープラーとして図示されるが、本開示は、任意の型の外科手術用ステープラー（開腹型ステープラーまたは内視鏡型ステープラーのいずれか）において容易に使用され得ることが企図される。また、ステープルヘッドを接近させてステープルを発射するための他の機構が、利用され得る。
ここで図２を参照すると、アンビル１０は、アンビル支持体１８に設置されるアンビルプレート２８を備える。アンビルプレート２８は、複数のステープル成形ポケット３０を規定し、これらのポケットは、ステープルヘッド２０と協働して、ステープルアセンブリを形成し、そしてステープルアセンブリの一部を支持する。アンビルプレートは、整列ピン３２および整列凹部３４を備え、これらが、ステープルヘッド２０の対応する凹部およびピンと協働して、ステープルヘッド２０内に収容されたステープルが、組織のステープル留めの間、ステープル成形ポケット３０と適切に整列することを確実にする。
図３を参照すると、各ステープル成形ポケット３０は、一般に、第一のステープル曲げ凹部またはポケット３６、および第一のステープル曲げポケット３６から間隔を空けた第二のステープル曲げポケット３８を備える。ポケット３６および３８は、ステープルの先端を受容し、そしてこれらの先端を、外科手術用ステープルアセンブリに関連するプラテン４０（ステープル成形ポケット３０内に配置される）の方へと方向付けるために提供される。プラテン４０は、一般的に細長いプレートとして形成され、このプレートは、プラテン４０の第一の端部４４に隣接する第一の穴４２、およびプラテン４０の第二の端部４８に隣接する第二の穴４６を有する。第一の穴４２および第二の穴４６は、これらの穴を通して外科手術用ステープルのレッグを受容し、そしてこれらのレッグを、それぞれ第一のステープル曲げポケット３６および第二のステープル曲げポケット３８内へと方向付けるために提供される。プラテン４０は、開口部５０をさらに備え、この開口部は、ほぼ楕円形の形状であり、そしてプラテン４０に沿って中心に提供される。開口部５０は、ステープルの成形の際に、外科手術用ステープルアセンブリに関連する外科手術用ステープルの組織貫入先端を、本明細書中以下でより詳細に記載される様式で受容するために提供される。
図４に示されるように、ステープル成形ポケット３０は、第一のステープル曲げポケット３６と第二のステープル曲げポケット３８との間に位置する、狭い中心領域または凹部５２を備える。こぶまたは突出部５４が、凹部５２の内部に位置し、外科手術用ステープルアセンブリのステープルを成形する際に補助する。凹部５２は、プラテン４０の開口部５０の真下に位置し、外科手術用ステープルアセンブリに関連する外科手術用ステープルの先端を、プラテン４０の開口部５０に通して方向付ける。
プラテン４０は、ステープル成形ポケット３０内に摩擦により保持される。ステープル成形ポケット３０は、第一のステープル曲げポケット３６に隣接して形成された第一の保持凹部５６、および第二のステープル曲げポケット３８に隣接して形成された第二の保持凹部５８を備える。プラテン４０の第一の端部４４および第二の端部４８は、それぞれステープル成形ポケット３０の第一の保持凹部５６および第二の保持凹部５８の内部に摩擦により保持される。
特定の実施形態において、第一の保持凹部５６は、第一の保持中心点６０を備え、第一のステープル曲げポケット３６は、第一の中心点６２を備え、第二のステープル曲げポケット３８は、中心点６４を備え、そして第二の保持凹部５８は、第二の保持中心点６６を備える。示されるように、この特定の実施形態において、第一の保持中心点６０、第一の中心点６２および第二の中心点６４は、共通の軸Ｘ−Ｘに沿って位置する。好ましい実施形態において、第二の保持中心点６６もまた、共通の軸Ｘ−Ｘに沿って位置する。
ここで図５を参照すると、プラテン４０を備える新規な多部品ステープルアセンブリ６８が開示されており、このアセンブリは、ステープル留めされた組織の両側に沿って、均一な圧縮を提供し得る。ステープルアセンブリ６８は、ほぼＵ字型のステープル７０を備える。ステープル７０は、外科手術の分野において通常使用される公知の型のものであり得、そして一般に、バックスパン７２、およびバックスパン７２の第一の端部７８および第二の端部８０からそれぞれ延びる第一のレッグ７４および第二のレッグ７６を備える。第一のレッグ７４および第二のレッグ７６は、それぞれ、第一の組織貫入先端８２および第二の組織貫入先端８４で終結する。好ましくは、レッグ７４、７６は、バックスパンに対して実質的に垂直であるが、代替例において、これらのレッグは、バックスパンに対してある角度で配置され得る。

さしあたり図６を参照すると、プラテン４０が断面で図示されている。示されるように、中心開口部５０は、プラテン４０の第一の穴４２と第二の穴４６との中間に位置する。具体的に図示される実施形態において、中心開口部５０は、プラテン４０の長さに沿って中心に位置し、これによって、組織貫入先端がアンビル１０のステープル成形ポケット３０内で成形される際に、任意の過剰の長さの組織貫入先端８２および８４を受容する。
ここで図７を参照すると、外科手術用ステープル止め装置内で成形される際に、外科手術用ステープル７０は、代表的なＢ字型に整形される。示されるように、外科手術用ステープル７０のレッグ７４は、ステープルプレート８６の第一のプレート穴８８を通過し、そしてプラテン４０の第一の穴４２を通過する。同様に、第二のレッグ７６は、ステープルプレート８６の第二のプレート穴９２を通過し、そしてプラテン４０の第二の穴４６を通過する。アンビル１０のステープル成形ポケット３０内で成形される際に、組織貫入先端７４および７６は、ステープル７０のバックスパン７２の方へと戻るように再度湾曲し、そしてステープル留めされる組織の個々の厚さに依存して、中心開口部５０を通過し得る。このことにより、外科手術用ステープルアセンブリ６８が、種々の厚さの組織に適合することが可能である。例えば、特に薄い組織セクションにおいて使用される場合、組織貫入先端７４および７６の、中心開口部５０を通る通過は、ステープルレッグ７４および７６の任意の過剰の長さを巻き上げ、そしてステープル留めされる組織セクションの完全な圧縮を可能にする。
ここで図８を参照すると、実質的に本明細書中で上に記載されたとおりであるプラテン４０および外科手術用ステープル７０を組み込む外科手術用ステープルアセンブリ９６の代替の実施形態が開示されている。外科手術用ステープルアセンブリ９６は、ステープルプレート９８の代替の実施形態を備え、このプレートは、付勢部材１００を有し、この付勢部材は、ステープルプレート９８を外科手術用ステープル７０のバックスパン７２から強制的に離すために提供される。ステープルプレート９８に付勢部材１００を提供することにより、ステープルプレート９８が一定の程度の圧力を、下にあるステープル留めされる組織に提供することが可能である。このことは、上記のように、ステープル留めされる組織が、圧縮、分解、壊死などに起因して経時的に機械的強度を失う場合に、特に有用である。すなわち、ステープル留めされる組織が経時的に弱くなる場合、外科手術用ステープルアセンブリ９６は、下にある組織セクションが適切に治癒するような時点まで、下にあるステープル留めされた組織セクションを通るあらゆる漏出を防止するために充分な圧縮を提供する。
この特定の実施形態において、付勢部材１００は、ステープルプレート９８と一体的に形成される。ステープルプレート９８は、付勢部材１００に特定の量の曲げを提供し得る任意の適切な材料から形成され得る。付勢部材１００は、スタンピング、成型などによって、ステープルプレート９８に形成され得る。付勢部材１００は、第一の端部１０２を有し、この第一の端部は、ステープルプレート９８の第一の点１０４から延び、そしてステープルプレート９８の第二の点１０８の第二の端部１０６まで延びる。付勢部材１００が、ステープルプレート９８からスタンピングされるか、打ち抜かれるか、または他の様式で切り出される場合、付勢部材１００によって占有される材料は、ステープルプレート９８に形成される開口部１１０を生じる。付勢部材１００は、第一の端部１０２および第二の端部１０６においてステープルプレート９８に接続されているように開示されるが、第一の端部１０２または第二の端部１０６のうちの一方が、ステープルプレート９８から切断されるかまたは他の様式で接続を断たれ、付勢部材１００のより大きい程度の曲げ、従って付勢力を可能にし得ることが留意されるべきである。この例において、付勢部材１００は、一般に、板ばねとして機能する。また、付勢部材は、以下に記載されるように、ステープルのバックスパンに取り付けられた別個の要素として提供され得る。
ステープルプレート９８は、ステープルプレート９８の第一の端部１１４に形成された第一の穴１１２、およびステープルプレート９８の第二の端部１１８に形成された第二の穴１１６をさらに備える。第一の穴１１２および第二の穴１１６は、ステープルプレート８６に関して本明細書中で上に記載された様式と類似の様式で、これらの穴を通してステープルレッグ７４および７６を受容するために提供される。
図９に最もよく示されるように、外科手術用ステープルアセンブリ９６が完全に成形されると、第一のレッグ７４および第二のレッグ７６は、それぞれ、ステープルプレート９８の第一の穴１１２および第二の穴１１６を通り、プレート４０の穴４２および４６を通って（そして組織の厚さに依存して、開口部５０を通って）延びる。外科手術用ステープルアセンブリ６８に関して本明細書中で上に記載されたように、図９の完全に成形されたステープル７０は、ほぼＢ字型を呈し、組織貫入先端８２および８４が、プラテンの第一の穴４２および第二の穴４６を通って延び、そして開口部５０を通って後方に湾曲している。
本明細書中で上で注目されたように、プラテン４０は、最初、アンビル１０内に保持される。本明細書中に開示される全てのステープルアセンブリの実施形態に共通して、外科手術用ステープル７０がアンビル１０のステープル整形ポケット３０内でほぼＢ字型に成形される際に、組織貫入先端８２および８４は、プラテン４０の中心開口部５０の、それぞれ第一の縁部１２０および第二の縁部１２２と係合する。組織貫入先端８２および８４と、縁部１２０および１２２との係合は、プラテン４０をステープル成形ポケット３０内での摩擦ばめから強制的に離すか、または「抜く」ように機能する。他の部品または構造体との接触を利用して、プラテン４０をステープル成形ポケットから解放し得ることもまた理解されるべきである。
ここで図１０および図１１を参照すると、実質的に本明細書中で上に記載されたようなプラテン４０および外科手術用ステープル７０を備える外科手術用ステープルアセンブリ１２４の別の実施形態が開示されている。外科手術用ステープルアセンブリ１２４は、ステープルプレート１２６を備え、このステープルプレートは、ステープルプレート１２６に固定された付勢部材１２８を有する。付勢部材１２８は、溶接、接着などによって、ステープルプレート１２６に固定され得る。本明細書中で上記のような付勢部材１００と同様に、付勢部材１２８は、一端のみでステープルプレート１２６に固定され、板ばねとして機能し得る。さらに、付勢部材１２８は、ステープルプレート１２６とは異なる材料から形成され得る。本明細書中で上に記載されたステープルプレートと同様に、ステープルプレート１２６は、ステープルプレート１２６の第一の端部１３２に形成された第一の穴１３０、およびステープルプレート１２６の第二の端部１３６に形成された第二の穴１３４を備える。本明細書中で上に記載されたステープルプレート９８と同様に、付勢部材１２８は、ステープルプレート１２６をバックスパン７２から離し、外科手術用ステープル７０の方へと推進して、下にあるステープル留めされる組織に対してステープルプレート１２６の一定の圧力を提供するために提供される。
ここで図１２〜図１５を参照し、最初に図１２を参照して、外科手術用ステープラー１２に組み込まれたステープルアセンブリ６８およびアンビル１０の、管状組織Ｔをステープル留めするための使用がここで記載される。ステープルアセンブリ６８およびアンビル１０の使用についての以下の議論は、開腹外科手術型の外科手術用ステープラー１２に関連して与えられるが、ステープルアセンブリ６８およびアンビル１０は、他の型の外科手術用ステープラー（例えば、直線ステープラー、内視鏡ステープラー、腹腔鏡ステープラー、円形ステープラーまたは吻合ステープラーなど）において使用される場合に、同等の用途を見出すことが留意されるべきである。さらに、ステープルアセンブリ６８およびアンビル１０は、管状組織セクション以外の組織をステープル留めする際に使用するためにも同等に適切であり、必ずしも互いに組み合わせて使用される必要はない。
外科手術用ステープラー１２は、本明細書中以下でより詳細に議論される様式でステープルカートリッジ２０内に装填された、複数のステープル７０およびステープルプレート８６を備える。このステープルカートリッジは、好ましくは、取り外し可能であり、そして発射後に、別の装填されたカートリッジと交換可能である。外科手術用ステープラー１２は、最初に、組織セクションＴがアンビル１０とステープルヘッド２０との間に位置するように配置される。その後、調節（接近）ノブ２２が操作されて、ステープルカートリッジ２０をアンビル１０の方へと移動させ、これらの間に組織Ｔを捕捉する。

ここで図１２および図１４を参照すると、一旦、外科手術用ステープラー１２が組織Ｔの周りに配置されると、ロックレバー２６がトリガ２４から係合を外され、そしてトリガ２４が起動され（図１２）、ドライバ（プッシャー）１４４を、ステープルポケット１４２内で遠位に移動させる（図１４）。好都合なために、図１３および図１４は、ステープル留め器具に収容された複数のステープルのうちの１つのみを発射することを示すことが、理解されるべきである。図１４を続けて参照すると、プッシャー１４４は、ステープル７０を、ステープルポケット１４２内で遠位に駆動し、その結果、組織貫入先端８２、８４が、本明細書中で上に記載されたように（図７）ステープルプレート８６の穴８８および９２を通過し、その後、組織Ｔを通る。組織Ｔに貫入した後に、組織貫入先端８２および８４は、プラテン４０の穴４２および４６（図７）を通過し、そしてアンビルプレート２８のステープル曲げポケット３６および３８内で成形される。本明細書中上で注目されたように、突出部５４との先端８２および８４の係合は、先端８２および８４をステープル７０のバックスパン７２の方へと再度湾曲させ、そしてプラテン４０に形成された中心開口部５０を通過させる。
外科手術用ステープラー１２を完全に起動させると、プッシャー１４４は、ステープル７０をステープルポケット１４２から推進し、このステープルが次に、ステープルプレート８６を押して、ステープルヘッド２０のステープルプレート凹部１４６内での摩擦係合から外し、これによって、ステープル７０およびステープルプレート８６をステープルヘッド２０から解放する。本明細書中上で議論されたように、組織貫入先端８２および８４と、プラテン４０の中心開口部５０の縁部との係合は、プラテン４０をステープル成形ポケット３０内での摩擦係合から強制的に離すか、または「抜く」。
完全に成形されて組織Ｔの管腔Ｌを閉じている外科手術用ステープルアセンブリ６８が、図１５に最もよく図示されている。プラテン４０およびステープルプレート８６を外科手術用ステープルアセンブリ６８に提供することにより、より大きくより均一な圧縮領域が組織Ｔに提供される。
さしあたり図１６および図１７を参照すると、外科手術用ステープルアセンブリ９６は、外科手術用ステープルアセンブリ６８に関して本明細書中上に記載された様式と実質的に同じ様式で、外科手術用ステープラー１２内で組み立てられ、そして管状組織セクションＴを通して駆動される。例えば、プッシャー１４４がステープル７０をステープルポケット１４２から駆動し、ステープルプレート９８および組織Ｔに通す。その後、組織貫入先端８２および８４が、プラテン４０を通過して、外科手術用ステープルアセンブリ９６を組織Ｔの周りで完全に成形する。本明細書中上で注目されたように、外科手術用ステープルアセンブリ９６は、ステープルプレート９８の一定の圧力を組織Ｔに経時的に提供するための、付勢部材１００を備える。このことは、ステープルプレート８６およびプラテン４０による、ステープル留めされた組織Ｔに対する一定のレベルの圧縮力を確実にし、これによって、経時的に組織Ｔの構造的強度が低下し、そして組織Ｔの厚さが減少する場合に、管腔Ｌを通るあらゆる漏出を防止することを補助する。ここでまた、好都合なために、複数のステープルアセンブリ９６のうちの１つのみが、図に示されている。
具体的には図示されていないが、付勢部材１２８を備える外科手術用ステープルアセンブリ１２４は、外科手術用ステープルアセンブリ９６と実質的に同じように機能することが留意されるべきである。
種々の改変が、本明細書中に開示された実施形態に対してなされ得ることが理解される。例えば、開示されたプラテンは、他の形状および大きさであり得、そして必ずしも、このプラテンと一緒に使用されるステープルのバックスパンの全体の長さより長い必要はない。さらに、開示されたプラテンは、中心開口部を備えずに形成されて、ステープル留めされた組織セクションに対する圧縮のための、より大きい表面積を提供し得る。さらに、付勢部材がまた、プラテンに提供されて、このプラテンをステープルのレッグまたは組織貫入先端から離し、組織セクションに向けて推進し得、ステープル留めされた組織セクションに対して経時的に一定の圧力源を提供し得る。従って、上記説明は、限定と解釈されるべきではなく、単に、特定の実施形態の例示であると解釈されるべきである。当業者は、添付の特許請求の範囲の精神および範囲内で、他の改変を予測する。

- ステープラーと共に使用されるように適合された外科手術用ステープルアセンブリであって、該外科手術用ステープルアセンブリは、
バックスパンと、該バックスパンから延びる１対のレッグとを有するステープルであって、該レッグの各々は、組織貫入先端で終結している、ステープルと、
