Description
リトナビルを含まない経口ドセタキセルのAUC 0.29±0.26(mg.h/L)
リトナビルを含む経口ドセタキセルのAUC 2.4±1.5(mg.h/L)
リトナビルを含まない静脈内ドセタキセルのAUC 1.9±0.4(mg.h/L)
リトナビルが経口ドセタキセルの全身曝露を同用量レベルのドセタキセルの静脈内投与後のレベルと同程度であるか、またはより高いレベルまで増加させることができるという概念が患者においてはっきりと立証された。組み合わせは、安全で、非常に好ましい薬物動態特性を示すようである。
ドセタキセル60分前にリトナビルを投与する場合とドセタキセルとリトナビルの同時投与の間には有意差はなかった。経口投与のAUCは、静脈内投与のAUCよりも大きい(図1参照)。これは、ドセタキセル排除の阻害に対するリトナビルの効果によって説明される。
この臨床試験は、比較的低用量のドセタキセルで実施されたが、高いAUC値(2.4±1.5mg.h/L、ドセタキセル100mg)が生じ、同用量の静脈内投与後のAUC値よりもさらに高かった。それによって、静脈内投与後には存在するが、経口投与後では体内循環に到達しない医薬媒体がドセタキセルの組織分布を制限するため、当然のことながら経口経路後(投与直後)の分布量は静脈内経路後よりも大きくなる。本発明者らは、これらの効果を理解するための薬物動態モデルを構築した(以下参照)。このモデルはまた、ドセタキセル排除に対するリトナビルの効果が、リトナビルが血流中にもはや存在しないときなくなることを示している。リトナビルは、ドセタキセル排除をリトナビルがない場合のレベルの35%まで阻害する。
薬物動態プロファイル
前記の試験から得られたデータの薬物動態(PK)分析は、薬物動態プロファイルを作製するためのNONMEN(非線形混合効果モデリング)プログラム(GloboMax LLC、Hanover、MD、USA)を使用して実施した。これは、様々な区画を使用した薬物の吸収、排除および分布をモデル化したものである。経口投与と静脈内投与の間の薬理学的な違いを以下に提示する。
PKモデルは、i.v.ドセタキセルのPKモデルをベースにした。このモデルは、3種類の区画を使用しており、Brunoらによって詳しく記載されている[85]。経口投与ドセタキセルから得られたデータは、胃腸管をモデル化した貯蔵区画がさらに追加されているこのモデル内で実装された。リトナビルの薬物動態モデルは、Kappelhoffら[87]によって記載された2区画モデルを使用して最も良く記載された。図3は、最終的な薬物動態モデルを図示したものである。ドセタキセルの薬物動態に対するリトナビルの影響は、2種類の異なる機構、a)リトナビル存在下でのドセタキセルの吸収の改善(リトナビル(RTV)区画とC1からC2へのドセタキセル吸収とを連結するライン)、b)リトナビルは活性型CYP3A4を阻害し(C6とC7とを連結するライン)、活性型CYP3A4はドセタキセルの排除に関与する(C7とドセタキセルの排除経路とを連結するライン)によってモデル化された。
ドセタキセルの吸収は、リトナビルと共投与すると著しく改善された。経口ドセタキセル単独のバイオアベイラビリティー計算値は14%(ドセタキセル100mgを経口投与された患者3人のデータに基づく)である。リトナビルと組み合わせた経口ドセタキセルのバイオアベイラビリティーは56%で4倍高かった。この効果は、GI管に存在するCYP3A4酵素のリトナビルによる阻害によるものと考えることができる。
ドセタキセルは、主にCYP3A4によって代謝される。リトナビルはCYP3A4を阻害する。これによって、リトナビルをドセタキセルと共投与すると排除の抑制が引き起こされる。ドセタキセルの除去は、CYP3A4量と関連しており、したがって徐々に変化する。図4は、経時的な相対的推定酵素濃度を示す。ドセタキセルの除去は、酵素濃度と1:1で相関する。したがって、ドセタキセルの除去対時間は図4と類似するだろう。
中心区画(図3のC2)の量は、i.v.投与(±6L)と経口投与(±60L)とでは著しく異なる。これはおそらく、ドセタキセル製剤の主要な賦形剤の1つ、ポリソルベート80によるものである。ポリソルベート80は、ドセタキセルを捕捉できるミセルを形成する[86]。ポリソルベート80は、i.v.投与の場合は循環に入るが、経口投与の場合は吸収されない。したがって、ポリソルベートは、吸収されないという事実によって、経口投与後、ドセタキセルの薬物動態の性質に影響を及ぼさない。
経口ドセタキセルのバイオアベイラビリティーは、リトナビルと共投与すると約4倍に増加した。全身曝露は、GI管および肝臓(すなわち、吸収および排除それぞれ)におけるCYP3A4に対するリトナビルの併用効果によって、AUCに関しては、8.2倍増加した。
3.1:固体分散物および物理的混合物
この実験では、SDSと混合したパクリタキセルおよびPVP−K17の固体分散物を含む組成物の溶解度および溶解速度を無水パクリタキセル、PVP−K17およびSDSの物理的混合物と比較した。
PVP−K17に入れたパクリタキセル20%の固体分散物は、パクリタキセル100mgをt−ブタノール10mLに溶解し、PVP−K17 400mgを水6.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K17/水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(条件は表3参照)。パクリタキセル20%/PVP−K17固体分散物25mg(=パクリタキセル5mg)を乳糖125mg、ドデシル硫酸ナトリウム30mgおよびクロスカルメロースナトリウム30mgと混合した。得られた粉末混合物をカプセル化した(表4参照)。
物理的混合物は、無水パクリタキセル5mgをPVP20mg、乳糖125mg、ドデシル硫酸ナトリウム30mgおよびクロスカルメロースナトリウム30mgと混合することによって調製した。得られた粉末混合物をカプセル化した。
両カプセル製剤をUSP2(パドル)溶解装置内において、37℃に維持した注射用水900mL中で回転速度75rpmで試験した。第1の実験では、各製剤のカプセル1個を使用した。第2の実験では、各製剤のカプセル2個を使用した。試料は、様々な時点で採取し、HPLC−UVによって分析した(表4参照)。
結果を図5に示す。溶解したパクリタキセルの量は表示量(5および10mg)に対して表す。パクリタキセルの溶解は、PVPを含む固体分散物に組み込むことによって非常に改善されることは明らかに確認できる。物理的混合物を使用すると、溶解したパクリタキセルの最大量は表示量の20%未満に維持される。固体分散物を使用すると、溶解度は約65%(パクリタキセル5mg)または70%超(パクリタキセル10mg)である。パクリタキセル10mgの実験では、これは約8μg/mlの絶対溶解度に対応し、これは約15分後に実現する。したがって、固体分散物は溶解度が著しく増加し、バイオアベイラビリティーに重要な迅速な溶解速度ももたらす。
この実験では、カプセル中の界面活性剤SDSの有無の溶解度に対する効果を測定した。
固体分散物は、パクリタキセル100mgをt−ブタノール10mLに溶解し、PVP−K17 400mgを水6.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K17/水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。

結果を表6に示す。溶解したパクリタキセルの量は表示量(この場合5mg)に対して表す。凍結乾燥したタキサンおよび担体固体分散物の空隙率は十分高く、粉末形態のとき確実に迅速に溶解する(結果は示さず)。しかし、粉末をカプセルに圧縮するとき、水和性は劇的に減少する。したがって、カプセルまたは錠剤に圧縮するとき、固体分散物を湿らせるために界面活性剤が必要である。
この実験では、固体分散物へのSDSの添加の溶解度に対する効果を測定した。
固体分散物は、パクリタキセル600mgをt−ブタノール60mLに溶解し、PVP−K17 900mgを水40mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K17/水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。
固体分散物は、パクリタキセル250mgをt−ブタノール25mLに溶解し、PVP−K17 375mgおよびドデシル硫酸ナトリウム(SDS)62.5mgは水16.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K17/ドデシル硫酸ナトリウム/水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。
パクリタキセル40%/PVP−K17固体分散物62.5mg(=パクリタキセル25mg)を乳糖160mg、ドデシル硫酸ナトリウム30mgおよびクロスカルメロースナトリウム10mgと混合した。得られた粉末混合物をカプセル化した(表9参照)。
パクリタキセル40%/PVP−K17/ドデシル硫酸ナトリウム10%固体分散物68.75mg(=パクリタキセル25mg)を乳糖160mgおよびクロスカルメロースナトリウム10mgと混合した。得られた粉末混合物をカプセル化した(表10参照)。
両カプセル製剤をUSP2(パドル)溶解装置内において、37℃に維持した注射用水500mL中で試験した。回転速度は、パクリタキセル/PVP−K17/ドデシル硫酸ナトリウム固体分散物のカプセルでは75rpmに、パクリタキセル/PVP−K17固体分散物のカプセルでは100rpmに設定した。試料は、様々な時点で採取し、HPLC−UVによって分析した(表6参照)。
結果を表7に示す。溶解したパクリタキセルの量は表示量(この場合25mg)に対して表す。固体分散物に組み込んだ、ドデシル硫酸ナトリウムを含むカプセルからのパクリタキセルの溶解は、カプセルに添加したドデシル硫酸ナトリウムのカプセルからのパクリタキセルの溶解と同程度である。さらに、固体分散物に組み込む場合は、ドデシル硫酸ナトリウムは6.25mgだけ使用し、一方、カプセル製剤に添加する場合はドデシル硫酸ナトリウムは30mgを使用した。このことは、類似の結果を実現するためには、カプセルよりも固体分散物に組み込むときの方が必要な界面活性剤が少ないことを示している。この実験の別の驚くべき結果は、両組成物はパクリタキセルの絶対溶解度が約26μg/mlであり、このレベルは20〜30分で到達されることである。この結果は、今までに実現されたものよりも溶解度が高く、溶解速度が速い。
最初の実験で薬剤負荷間の明らかな差が示さなかった後で、実施例3.4の実験で使用した固体分散物は生成された。これらの製剤は、前述の実験の20%薬剤負荷製剤と同等に作用し、1個の錠剤またはカプセルでより多くのタキサンを送達する可能性がもたらされたので、40%薬剤負荷を選択した。
固体分散物は、パクリタキセル250mgをt−ブタノール25mLに溶解し、PVP−K12 375mgを水16.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K12水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。

固体分散物は、パクリタキセル250mgをt−ブタノール25mLに溶解し、HP−シクロデキストリン375mgを水16.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらHP−シクロデキストリン水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。
パクリタキセル/担体固体分散物62.5mg(=パクリタキセル25mg)を乳糖160mg、ドデシル硫酸ナトリウム30mgおよびクロスカルメロースナトリウム10mgと混合した。得られた粉末混合物をカプセル化した(表11参照)。
全カプセル製剤をUSP2(パドル)溶解装置内において、37℃に維持した注射用水500mL中で回転速度100rpmで試験した。試料は、様々な時点で採取し、HPLC−UVによって分析した(表6参照)。
2から3の実験の平均結果を図8に示す。溶解したパクリタキセルの量は表示量(この場合25mg)に対して表す。PVP−K30固体分散物からのパクリタキセルの溶解は、PVP−K17固体分散物からのパクリタキセルの溶解の速さと同じくらいであることを明らかに認めることができる。しかし、溶解したパクリタキセルの量は、PVP−K30固体分散物の場合、4時間の実験中高く維持される。
実施例3.5の実験で使用した固体分散物は、最初の実験で担体間の明らかな差が示されなかった後で生成された。これらの最初の実験は、実施例3.4のより詳細な実験の前に実施された。結果として、PVP−K17は、さらに実験するために担体として任意に選択された。

パクリタキセル約4mgに等しい固体分散物粉末の量を50mLビーカーに入れた。磁石撹拌棒および水25mLをビーカーに添加した。溶液は7200rpmで撹拌した。試料は、様々な時点で採取し、HPLC−UVによって分析した(表6参照)。
2から3の実験の平均結果を図9に示す。溶解したパクリタキセル(PCT)の量は表示量(この場合約4mg)に対して表す。薬物/担体比の影響は、図9からすぐに明らかである。パクリタキセルのピーク濃度の値は、薬物/担体比に反比例する。最高ピーク濃度には、最低の薬物/担体比(10%)で到達し、最低ピーク濃度には最高の薬物/担体比(100%)で到達する。さらに、10%薬物/担体比の固体分散物のAUC値が最高で、25、40、75および100%薬物/担体比の固体分散物のAUC値がそれに続く。
PVP−K17およびドデシル硫酸ナトリウム10%に入れた40%パクリタキセル固体分散物
固体分散物は、パクリタキセル250mgをt−ブタノール25mLに溶解し、PVP−K17 375mgおよびドデシル硫酸ナトリウム(SDS)62.5mgは水16.67mLに溶解することによって調製した。パクリタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K17/ドデシル硫酸ナトリウム/水溶液に添加した。最終混合物を8mLバイアルに移し、最大充填レベルを2mLとした。t−ブタノールおよび水はその後凍結乾燥によって除去した(表3参照)。
パクリタキセル20%/PVP−K17/ドデシル硫酸ナトリウム10%固体分散物68.75mg(=パクリタキセル25mg)を乳糖160mgおよびクロスカルメロースナトリウム10mgと混合した。得られた粉末混合物をカプセル化した(表12参照)。
カプセルは、重複して異なる2種類の溶解試験を行った。第1の試験は、2段階の溶解試験で、ペプシンを含まない模擬胃液(SGFsp;表13)500mL中における2時間の溶解試験、その後のペプシンを含まない模擬腸液(SIFsp;表13)629mLにおける2時間の溶解試験から構成される。第2の試験は、絶食状態の模擬腸液(FaSSIF;表14)媒体500mLで4時間実施された。
結果を図10に示す。溶解したパクリタキセルの量は表示量(この場合25mg)に対して表す。絶食状態模擬腸液におけるパクリタキセルの溶解は、模擬胃液(SGFsp)よりも約20%多い。SGFspにおいて2時間後、溶液中のパクリタキセルの量は媒体を模擬腸液(SIFsp)に交換すると少しだけ増加する。
材料および方法
以下の実験で使用した製剤は、以下に概略した方法および表15に記載した組成物によって調製した。
無水ドセタキセルは、ScinoPharm、Taiwanから入手して使用した。
ドセタキセルは、無水ドセタキセル300mgをt−ブタノール30mL中に溶解することによって非晶質化した。ドセタキセル/t−ブタノール溶液を常に撹拌しながら注射用水(WfI)20mLに添加した。最終混合物をステンレススチール凍結乾燥箱(ガストロノームサイズ1/9)に移し、その後t−ブタノールおよび水を凍結乾燥によって除去した(表16参照)。
物理的混合物は、ドセタキセル150mgおよび対応する量の担体および界面活性剤(表15参照)を乳鉢および乳棒で混合することによって調製した。
固体分散物は、無水ドセタキセル300mgをt−ブタノール30mLに溶解し、対応する量の担体および界面活性剤(表15参照)を注射用水20mLに溶解することによって得られた。ドセタキセル/t−ブタノール溶液を常に撹拌しながら担体/界面活性剤/WfI溶液に添加した。最終混合物をステンレススチール凍結乾燥箱(ガストロノームサイズ1/9)に移し、その後t−ブタノールおよび水を凍結乾燥によって除去した(表16参照)。
ドセタキセル約6mgに等しい粉末の量を50mLビーカーに入れた。磁石撹拌棒および水25mLをビーカーに添加した。溶液は720rpmで撹拌し、約37℃に維持した。試料は様々な時点で採取し、0.45μmフィルターで濾過してからメタノールおよびアセトニトリル1:4v/v混合物で希釈した。濾過し、希釈した試料をその後HPLC−UVによって分析した(表17参照)。
第1の実験では、ドセタキセルの溶解度に対する製剤の種類の影響を調べた。製剤AからEで実施した溶解試験のデータを比較した。結果を図11に示す。製剤Eは、4連で試験し、製剤AからDは2連で試験した。
製剤A(純粋な無水ドセタキセル)は、5分間撹拌した後、最高濃度約12μg/mL(存在する全ドセタキセルの4.7%)に達し、15分間撹拌した後、平衡濃度約6μg/mL(2%)に達する。
製剤は全て、最初は高い溶解度を示し、45分から60分撹拌すると平衡溶解度まで減少する。溶解度の減少は、過飽和溶液の結果であるドセタキセルの結晶化によって引き起こされる。過飽和の程度は、薬物の物理的状態、すなわち、非晶質であるか、または結晶であるかによって左右される。PVP−K30が担体のとき、過飽和状態は長く維持され、したがって、ドセタキセルの溶解度はあまり急速には減少しない。さらに、この結果は、非晶質ドセタキセルを使用すると、無水ドセタキセルと比較してドセタキセルの溶解度が著しく増加することを示している。さらに、非晶質ドセタキセルは、比較的速い溶解速度を示し、約5分から7.5分でピークに達する。
第2の実験では、ドセタキセルの溶解度に対する担体の種類の影響を調べた。製剤EおよびFで実施した溶解試験のデータを比較した。結果を図12に示す。製剤Eは、4連で試験し、製剤Fは2連で試験した。
製剤E(非晶質ドセタキセル、PVP−K30およびSDSの固体分散物)の最大濃度は最高で213μg/mL(存在する全ドセタキセルの90%)であり、5分後に到達する。10分と25分の間に、溶液中のドセタキセルの量は急速に減少し、45分後に平衡濃度20μg/mL(8%)を生じる。
この実験は、PVP−K30およびHPβ−CDの両方がドセタキセルの溶解度を増加させることを示している。HPβ−CDと比較してPVP−K30を担体として使用するとき、ドセタキセル最大濃度は少し高く、過飽和状態は長く維持され、したがって、ドセタキセルの溶解度は時間と共に急速には減少しない。さらに、ドセタキセル沈殿後に達する平衡濃度はHPβ−CDと比較してPVP−K30の方が高い。
第3の実験では、ドセタキセルの溶解度に対するPVP鎖長の種類の影響を調べた。製剤EおよびGからJで実施した溶解試験のデータを比較した。結果を以下の図13に示す。製剤Eは4連で試験し、製剤GからJは2連で試験した。
製剤G(PVP−K12)は、5分後にドセタキセル最大濃度206μg/mL(存在する全ドセタキセルの77%)に達する。5分と30分の間に、溶液中のドセタキセルの量は20μg/mL(7%)の値に減少し、45分後のドセタキセル濃度は17μg/mL(6%)である。
この実験は、PVPの鎖長は、過飽和の程度および過飽和が維持される期間の両方に影響を及ぼすことを示している。使用するPVP鎖長が長ければ長いほど、ドセタキセル最大濃度はより高く、過飽和期間はより長くなり、したがって、より長い期間より高い溶解度がもたらされる。
第4の実験では、ドセタキセルの溶解度に対する薬物負荷の影響を調べた。製剤EおよびKからNで実施した溶解試験のデータを比較した。結果を図14に示す。製剤Eは4連で試験し、製剤KからNは2連で試験した。
この実験は、固体分散物中で使用したドセタキセルの量に対するPVP−K30の量は、過飽和の程度および過飽和が維持される期間の両方に影響を及ぼすことを示している。薬物負荷の使用が高ければ高いほど、ドセタキセル最大濃度は低くなり、過飽和期間は短くなり、したがって、徐々に溶解度が低くなる。
この実験では、ドセタキセル15mg、PVP−K30 135mgおよびSDS15mgの固体分散物を含有する組成物を、Chenら[95]で開示したドセタキセル5mgおよびPVP−K30の固体分散物を含む組成物の文献データと比較した。溶解度の結果は、Chenら[95]で記載された溶解試験を使用して得られ、図15および16に示す。溶解試験はまた、模擬腸液で実施され、Chenの文献データと比較した。結果を図17に示す。
図15から、Chenらの組成物は水900mlにおいて組成物中のドセタキセル5mgの最大約80%を溶解できることを認めることができる。この最大値に達するためには5時間超かかる。ドセタキセル、PVP−K30およびSDS組成物は、約60分でドセタキセル15mgの100%を溶解した。
これらの結果から、ドセタキセル、PVP−K30およびSDSの組成物はChenの組成物と比較して溶解速度が速く、溶解度が高いことを認めることができる。バイオアベイラビリティーについては、どのくらい速く薬物が溶解し、0.5hから1.5hでどの溶解度に達するかを観察することが重要である。
この実験では、ドセタキセル、PVP−K30およびSDSの固体分散物を含有するカプセルの溶解を、パンクレアチンを含まない模擬腸液(SIFsp)で試験した。このカプセルは、製剤Eにしたがってドセタキセル15mgを含有した(表15参照)。SIFspはUSP28にしたがって調製した。ドセタキセル15mgを含有するカプセルは、37℃で75rpmで撹拌しながらUSP SIFsp 500mLに溶解した。結果は、図18および19に示す。
臨床試験(以後の実施例参照)のためのカプセルで使用された、製剤E(表15参照)によるドセタキセル、PVP−K30およびSDSの固体分散物は、4〜8℃で保存したとき、少なくとも80日間、化学的(分解なし)および物理的(溶解特性に変化なし)に安定であることが分かった。
材料および方法
患者10人が進行中の第1相臨床試験に参加した。
301、302、303、304、305、306、307、308、309および310。
ドセタキセル用量:全患者に対して30mg(ドセタキセル20mgを投与された患者306以外)。30mgの用量は以下のように調製した。静脈内投与用Taxotere(登録商標)プレミックス3.0ml(ポリソルベート80(25%v/v)、エタノール(10%(w/w)および水の1ml当たりドセタキセル10mgを含有する)を水と混合して、最終体積25mLにした。この溶液を水道水100mLと共に患者に経口摂取させた。
ドセタキセル用量:30mg。カプセル当たりドセタキセル15mgのカプセル2個を摂取した。臨床試験でさらに試験するために、前の実施例の製剤E(ドセタキセル1/11、PVP−K30 9/11およびSDS1/11)を選択した。製剤Eの新しいバッチは、無水ドセタキセル1200mgをt−ブタノール120mLに溶解し、PVP−K30 10800mgおよびSDS 1200mg(表15参照)を注射用水80mLに溶解することによって作製した。ドセタキセル/t−ブタノール溶液を常に撹拌しながらPVP−K30/SDS/WfI溶液に添加した。最終混合物をステンレススチール凍結乾燥箱(ガストロノームサイズ1/3)に移し、t−ブタノールおよび水を凍結乾燥によってすぐに除去した(表16参照)。
患者301、302、303、304および305は液体製剤のみを投与された。
液体製剤(ドセタキセル30mg)
AUCinf(平均の95%信頼区間):1156(808〜1504)ng*h/ml
個体間変動:85%(n=8)
AUCinf(平均の95%信頼区間):756(568〜968)ng*h/ml
個体間変動:29%(n=4)
個体内変動:33%(n=2)
試験したドセタキセル液体製剤は、新規カプセル製剤(MODRA)で得られた同用量(30mg)よりも約1.5倍のAUC値を生じた。
図23は、ドセタキセルのi.v.投与後(i.v.による1時間注入としてドセタキセル20mg、Taxotere(登録商標))(患者n=5)および経口投与(ドセタキセル30mg、MODRAカプセル、前記参照)(患者n=4、6クール)後の薬物動態曲線を示す。i.v.および経口の両方によるドセタキセル投与は、リトナビル(カプセル、Norvir(登録商標))100mgの投与と組み合わせた。施設の指針にしたがって、経口およびi.v.ドセタキセルの両方について、患者全てを経口デキサメタゾンで治療した。デキサメタゾン用量4mgを被験薬の1時間前、その後は12時間毎(2回)に4mg投与した。ドセタキセルによる治療の1時間前に、患者はまた、悪心および吐き気を抑えるため、グラニセトロン(Kytril(登録商標))1mgを投与された。
(AUC30mg経口/AUC20mg iv)×(20/30)×100%=73%(SD18%)によって計算した。
本願発明は以下の態様を包含し得る。
[1] 実質的に非晶質のタキサン、親水性であり、好ましくは重合した担体および界面活性剤を含む経口投与用固体医薬組成物。
[2] 前記タキサンおよび前記担体が固体分散物の形態である、上記[1]に記載の組成物。
[3] 前記タキサン、前記担体および前記界面活性剤が固体分散物の形態である、上記[2]に記載の組成物。
[4] 前記タキサンが、ドセタキセル、パクリタキセル、BMS−275183、それらの機能的誘導体およびそれらの薬学的に許容される塩またはエステルから選択される、前記のいずれかに記載の組成物。
[5] 前記タキサンが、ドセタキセル、パクリタキセル、それらの機能的誘導体およびそれらの薬学的に許容される塩またはエステルから選択される、上記[4]に記載の組成物。
[6] 前記担体がPVPである、前記のいずれかに記載の組成物。
[7] 前記PVPが、PVP−K12、PVP−K15、PVP−K17、PVP−K25、PVP−K30、PVP−K60およびPVP−K90から選択される、上記[6]に記載の組成物。
[8] 前記PVPが、PVP−K30、PVP−K60およびPVP−K90から選択される、上記[7]に記載の組成物。
[9] 前記界面活性剤が、ドデシル硫酸ナトリウム(SDS)、ソルビタンエステル(ソルビタン脂肪酸エステル)、ステアリン酸ポリオキシエチレン、ポリオキシエチレンソルビタン脂肪酸エステル、ポリオキシエチレンヒマシ油誘導体、ポリオキシエチレンアルキルエーテル、ポロキサマー、モノオレイン酸グリセリル、ドキュセートナトリウム、セトリミド、安息香酸ベンジル、塩化ベンザルコニウム、塩化ベンゼトニウム、ヒプロメロース、非イオン性乳化ワックス、アニオン性乳化ワックスおよびクエン酸トリエチルから選択される、前記のいずれかに記載の組成物。
[10] 前記界面活性剤がSDSである、上記[9]に記載の組成物。
[11] 前記タキサンと担体との重量比が、約0.01:99.99w/wと約72:25w/wの間である、前記のいずれかに記載の組成物。
[12] 前記タキサンと担体との重量比が、約0.01:99.99w/wと約30:70w/wの間である、上記[11]に記載の組成物。
[13] 界面活性剤と、一緒にしたタキサンおよび担体との重量比が、約1:99w/wと約50:50w/wの間である、前記のいずれかに記載の組成物。
[14] 界面活性剤と、一緒にしたタキサンおよび担体との重量比が、約2:98w/wと約17:83w/wの間である、上記[13]に記載の組成物。
[15] 前記実質的に非晶質のタキサンが凍結乾燥によって調製される、前記のいずれかに記載の組成物。
[16] 実質的に非晶質のタキサンが、経口投与用カプセル内でのタキサン溶液の凍結乾燥によって調製される、上記[15]に記載の組成物。
[17] 1種または複数の他の薬学的に活性のある成分をさらに含む、前記のいずれかに記載の組成物。

経口投与用固体医薬組成物。 - 前記非晶質のタキサンが溶媒留去方法または凍結乾燥により調製される、請求項1に記載の組成物。
- 前記担体が、ポリビニルピロリドン(PVP)、ポリエチレングリコール(PEG)、ポリビニルアルコール(PVA)、クロスポビドン(PVP−CL)、ポリビニルピロリドン−ポリビニルアセテートコポリマー(PVP−PVA)、メチルセルロース、ヒドロキシプロピルセルロース、カルボキシメチルエチルセルロース、酢酸フタル酸セルロース、フタル酸ヒドロキシプロピルメチルセルロース、ポリアクリレート、ポリメタクリレート、糖類、ポリオール、マンニトール、スクロース、ソルビトール、デキストロース、キトサンおよびシクロデキストリンから選択される、請求項1または2に記載の組成物。
- 前記担体が、PVP−K17、PVP−K25、PVP−K30、PVP−K60、PVP−K90およびPVPK120から選択される、請求項1〜3のいずれか一項に記載の組成物。
- 前記タキサンと前記担体との重量比が、5:95w/w〜1:9w/wである、請求項1〜4のいずれか一項に記載の組成物。
