多数の自己免疫疾患は、特定組織の細胞がTリンパ球の標的になる時に発症する(総説についてはSantamaria、2001を参照)。いくつかの事例では、Tリンパ球は、Fas、パーフォリン、またはその両方を含む細胞性細胞傷害性過程を介して組織損傷に直接作用する。パーフォリンが仲介する溶解は、Tリンパ球上の抗原特異的T細胞受容体と、標的細胞の形質膜上のMHC分子に提示される特定抗原(通常、ペプチド)との間の同種相互作用を必要とする。Fasが仲介する細胞傷害性は、標的細胞上のFasとT細胞上のFasリガンド(FasL)とのライゲーションを含むが、エフェクターリンパ球とその標的との間の同種相互作用を必要としないので、潔白な周辺細胞に障害を与える可能性がある。他の事例では、Tリンパ球は標的細胞上のプロアポトーシス受容体と結合しうるサイトカインを分泌することによりその標的を死滅させる。他の事例では、自己反応性リンパ球は、非リンパ球が仲介する死-エフェクター機構に対する感受性を高めることにより、標的組織中へのさらなる炎症性細胞(すなわち細胞傷害性マクロファージ)の補充を促進することにより、または、自己反応性B細胞の、自己抗体分泌性形質細胞への分化を駆動することにより、その標的を間接的に死滅させる。
移植片対宿主病は多段階プロセスである。エフェクターT細胞がこの疾患の誘導に中心的な役割を果たすことが示されている。「誘導期」に、エフェクターT細胞はアロ抗原不同性を確認し、次いで活性化され、そしてクローン増殖が起こる(「増殖期」)。これらのT細胞は次いでサイトカインおよび恐らくケモカイン(例えばマクロファージ炎症性タンパク質1α)を放出し、「動員(recruitment)期」に他の細胞型(マクロファージ、顆粒球、ナチュラルキラー細胞など)を動員する。最後にT細胞と他の細胞型は、移植片対宿主病に関連する病理を仲介する(「エフェクター期」)(総説はMurphyおよびBlazar、1999を参照)。
アルツハイマー病などの変性疾患に炎症または免疫系は介在しないと古典的に考えられる一方、いくつかの事例において免疫系は退行過程で重要な役割を果たすことができる。さらに、退行性と考えられる神経系疾患において免疫系自体が有益な効果を有しうることが明らかになってきた。体液性免疫応答を誘導するように設計された免疫治療手法が最近アルツハイマー病の治療用に開発された。動物モデルにおいて、細胞免疫応答を誘導するように設計された免疫療法は中枢神経系障害に有効でありうることも示されている、しかしT細胞は誘導されるT細胞応答の型に応じて有益な効果を与える場合と有害な効果を与える場合がありうる(MonsonegoおよびWeiner、2003)。
諸研究は調節性/サプレッサーT細胞の天然集団の存在について確証を与えており、この集団はin vitroでTCRが仲介する刺激があると、エフェクターT細胞の増殖を抑制する(von HerrathおよびHarrison、2003)。これらの細胞はT細胞恒常性の制御に対しておよび自己抗原、癌細胞、病原体、およびアロ抗原に対する免疫応答のモジュレーションにおいて中心的な役割を果たす。
ある特定の急性期タンパク質血漿濃度の測定値は、特定の臨床条件下において診断または予後上の価値がある。最もよく知られる急性期タンパク質はC反応性タンパク質(CRP)である。CRPはある特定症状由来の炎症とともに血中で上昇する血漿タンパク質である。血漿中のCRPのレベルは炎症に伴って1000倍の高さまで上昇しうる。通常、著しいCRPの変化を引き起こす症状としては、細菌およびウイルス感染、外傷、手術、火傷、炎症症状、冠血管および血管の疾患ならびに進行癌が挙げられる。
本発明者らは、意外なことに、エフェクター細胞の存在により特徴付けられる病状の間、エフェクター細胞とレギュレーター細胞の両方の数が周期的変化をすることを見出した。この周期的変化は、標的抗原に対するエフェクター細胞増殖とともに規則正しく起こり、ついで該エフェクターに対してレギュレーター細胞の増殖が起こる。エフェクター細胞がレギュレーター細胞により抑制されると、両方の型の細胞数が減少し、これらは次いで抗原が継続して存在するので同じ周期的変化をする。
i) a)レギュレーター細胞の数および/または活性、
b)エフェクター細胞の数および/または活性、
c)該疾患に関連する分子、ならびに/あるいは
d)免疫系マーカー
のうちの少なくとも1つの変動について疾患をもつ患者をモニタリングすることにより、エフェクター細胞および/またはレギュレーター細胞の周期的変化を分析するステップ、ならびに
ii) 患者を該疾患を治療する薬剤に曝すステップ
を含んでなる上記方法を提供する。
従って、他の態様において、本発明はエフェクター細胞および/またはレギュレーター細胞の周期的変化を分析して、エフェクター細胞の産生により特徴付けられる疾患を診断する方法であって、患者または患者から得たサンプルを、a)エフェクター細胞数および/または活性、b)レギュレーター細胞数および/または活性、c)該疾患に関連する分子、ならびに/あるいはd)免疫系マーカーのうちの少なくとも1つの変動についてモニタリングするステップを含んでなり、その場合、a)〜d)のいずれか1つの周期的変化は疾患が存在しうることを示す上記方法を提供する。
i) a)レギュレーター細胞の数および/または活性、
b) エフェクター細胞の数および/または活性、
c) 該疾患に関連する分子、ならびに/あるいは
d) 免疫系マーカー
のうちの少なくとも1つの変動について疾患をもつ患者をモニタリングすることにより、エフェクター細胞および/またはレギュレーター細胞の周期的変化を分析するステップ、ならびに
ii) 薬剤の投与のタイミングがエフェクター細胞の活性を有意に低下させないように選んで、患者を該疾患を治療する薬剤に曝すステップ
を含んでなる上記方法を提供する。
好ましくは、免疫系マーカーはレギュレーター細胞の数および/または活性、および/またはエフェクター細胞の数および/または活性を反映する。
さらなる態様において、本発明はエフェクター細胞および/またはレギュレーター細胞の周期的変化を分析して変性疾患を診断する方法であって、患者または患者から得たサンプルを、a)エフェクター細胞数および/または活性、b)レギュレーター細胞数および/または活性、c)該疾患に関連する分子、ならびに/あるいはd)免疫系マーカーのうちの少なくとも1つの変動についてモニタリングするステップを含んでなり、その場合、a)〜d)のいずれか1つの周期的変化は疾患が存在しうることを示す上記方法を提供する。
i) a)レギュレーター細胞の数および/または活性、
b) エフェクター細胞の数および/または活性、
c) 該疾患に関連する分子、ならびに/あるいは
d) 免疫系マーカー
のうちの少なくとも1つの変動について疾患をもつ患者をモニタリングすることにより、エフェクター細胞および/またはレギュレーター細胞の周期的変化を分析するステップ、ならびに
ii) ワクチンの投与のタイミングがエフェクター細胞の活性を有意に低下させないように選んで、患者を該疾患を治療するワクチンに曝すステップ
を含んでなる上記方法を提供する。

一般的技術と定義
特に断らない限り、本明細書に用いた全ての技術および科学用語は、当業者(例えば、細胞培養、分子遺伝学、免疫学、免疫組織化学、タンパク質化学、および生化学における)が一般的に理解しているのと同じ意味を有すると解釈される。
いくつかの急性期炎症マーカーは免疫応答中に最初増加する(本明細書では以後、ポジティブ急性期炎症マーカーと呼ぶ)一方、他のものは免疫応答中に最初減少する(本明細書では以後、ネガティブ急性期炎症マーカーと呼ぶ)。急性期炎症マーカーはまた、当技術分野で、急性期反応物または急性期タンパク質とも呼ばれる。当業者は急性期炎症マーカーをモニタリングするのに利用することができる多数のアッセイを知っているであろう。
本発明は概括すると3種の異なるタイプの薬剤の使用に関する。それらの試薬は次の通りである:
1)エフェクター細胞に特異的である薬剤(CD8+特異的抗体など)であって、エフェクター細胞の産生により特徴付けられる疾患を治療するために用いることができるもの、
2)レギュレーター細胞に特異的である薬剤(CD4+特異的抗体など)であって、変性疾患を治療するために用いることができるもの、ならびに
3)エフェクター細胞およびレギュレーター細胞に影響を与える非選択的な薬剤であって、しかし投与のタイミングが標的とする細胞型を規定するもの。
薬剤は、エフェクター細胞の破壊、その機能の制限、またはその産生の阻害を選択的にまたは非選択的にもたらすいずれの因子または処置であってもよい。例えばCD8+特異的抗体を、CD8+T細胞を特異的に標的化するために用いることができる。しかし、いくつかの事例では、それぞれが分裂中の細胞を標的化する抗増殖薬、抗代謝薬または照射などの非選択的薬剤を用いることができる。特に、他の細胞型と同じように、エフェクター細胞は、分裂時そして特に有糸分裂時に、抗分裂(抗増殖)薬または紡錘体毒(例えばビンブラスチンまたはパクリタキセル)による破壊に対して脆弱である。
変性疾患を治療する際、薬剤は、レギュレーター細胞の破壊、その機能の制限、またはその産生の阻害を選択的にまたは非選択的にもたらすいずれの因子または処置であってもよい。例えばCD4+特異的抗体は、CD4+T細胞を特異的に標的化するために用いることができる。しかし、いくつかの事例では、それぞれ分裂中の細胞を標的化する抗増殖薬、抗代謝薬または照射などの非選択的薬剤を用いることができる。特に、他の細胞型と同じように、エフェクター細胞は、分裂時そして特に有糸分裂時に、抗分裂(抗増殖)薬または紡錘体毒(例えばビンブラスチンまたはパクリタキセル)による破壊に対して脆弱である。
ほとんどの事例において、免疫応答の動力学が変わりうるので、薬剤を投与すべき時点は、疾患の異なる段階の患者において実験的に決定することが必要であろう。他の因子、例えば被験者の一般的健康および/または被験者の遺伝的体質なども、薬剤を投与するのに適当な時点に影響を与えうる。
本明細書において用いるワクチンは、変性疾患の特徴であるタンパク質(抗原)に対する免疫応答をもたらす。かかるワクチンは少なくとも1つの抗原、または上記抗原をコードするポリヌクレオチドを含有しうる。ワクチンは当技術分野で公知のいずれかの形態、例えば、限定されるものでないが、DNAワクチン、抗原を発現するトランスジェニック生物の摂取、または抗原を含有する組成物の形態で提供することができる。
以下に記載するのはいくつかの急性期炎症マーカーをモニタリングするために用いる典型的なアッセイの例である。
C反応性タンパク質(CRP)は、DADE Behring Dimension RxL化学分析計を使用し、Dade Behring Diagnostics(Sydney、Australia)が供給する試薬と標準物質(試薬cat No. DF-34;標準物質cat. No. DC-34)を用いて測定した。
サイトカインインターロイキン2の受容体(IL2R)は、Diagnostic Products Corporation(Los Angeles、CA、USA)のImmulite分析計を使って市販の自動化された化学発光酵素イムノアッセイ(EIA)により測定した。
サイトカインインターロイキン6は、Diagnostic Products Corporation(Los Angeles、CA、USA)のImmulite分析計を使って市販の自動化された化学発光酵素イムノアッセイ(EIA)により測定した。

ヒトSAAに対する抗体を用いてコーティングしたポリスチレン粒子は、SAAを含有するサンプルと混合すると凝集する。比濁計の散乱光の強度は、サンプル中の分析質の濃度に依存し、従って、その濃度は既知濃度の標準液の希釈液と比較して決定することができる。
自動化した方法を利用して、Dade Behring ProSpect分析計を使う比濁分析により、Dade Behring Diagnostics(Sydney、Australia)により供給された試薬と標準物質を用いて、血清サンプル中の補体タンパク質C3濃度を測定した。

当業者は気付くように、自己免疫疾患と癌の間には類似性がある。より具体的に言えば、自己免疫疾患は自己抗原に対する免疫応答により特徴付けられる。同じ様な方式で、癌患者の免疫系は癌細胞による抗原(癌抗原)の過剰発現を認識してこれらの細胞に対する免疫応答を引き起こす。しかし、少なくともいくつかの環境において癌細胞に対する免疫応答は癌を効果的に抑制することなく、疾患は持続する。このように、癌患者における免疫系の周期的変化は、自己免疫疾患の被験者と類似の周期的変化を示す好適なモデルである。
腎臓屍体アロ(allo)移植を必要とする慢性糸球体腎炎の患者に対する好適なドナーについての研究を行う。アロ移植は標準の手順を用いて実施する。アロ移植が完了した時、患者を血清アミロイドA(SAA)レベルについてモニタリングする。SAAレベルが増加し始める時、移植片に対するエフェクター細胞の産生により特徴付けられる免疫応答が移植片に対して引き起こされたことを示す(当技術分野では拒絶とも呼ぶ)。
若年性慢性関節炎の患者をSAAレベルについてモニタリングする。SAAレベルの明白な周期的変化が確立されると、SAAレベルが上昇し始めるときにビンクリスチンを標準用量だけ投与する。この事例では、ビンクリスチンはエフェクター細胞などの分裂細胞を免疫周期中のこの時点で標的化して疾患の症候群を軽減しうる。
アルツハイマー病の患者をc反応性タンパク質レベルについてモニタリングする。c反応性タンパク質レベルに明白な周期的変化が確立されると、c反応性タンパク質がピークに達したときにビンクリスチンを標準用量だけ投与する。この事例では、ビンクリスチンは免疫周期中のこの時点においてレギュレーター細胞などの分裂細胞を標的として、疾患の症候群を軽減しうる。
本出願は2004年9月8日に出願された仮特許出願第2004905118号の優先権を主張するものであって、上記出願の内容は本明細書に参照により組み入れられる。

Claims (7)
- 免疫系の周期的変化を分析して、エフェクターT細胞の産生により特徴付けられる疾患または変性疾患をもつ患者に薬剤を投与すべき時を決定するためのデータを取得する方法であって、患者または患者から得たサンプルを、
a)エフェクターT細胞数および/または活性、b)レギュレーターT細胞数および/または活性、c)該疾患に関連するマーカー分子、ならびに/あるいはd)免疫系マーカーのうちの少なくとも1つの変動についてモニタリングすることを含んでなり、
前記疾患がエフェクターT細胞の産生により特徴付けられる疾患である場合、
(i) 該疾患は、自己免疫疾患または移植拒絶であり;
(ii) 該薬剤は、エフェクターT細胞の産生を阻害し、該細胞の機能および/または活性を制限し、かつ、抗増殖薬、抗代謝薬、照射、dsRNAおよび抗体からなる群より選択されるものであり;そして、
(iii) 該薬剤の投与のタイミングは、エフェクターT細胞の拡大が起こらないように、および/またはエフェクターT細胞の数が低下または消滅するように、エフェクターT細胞がクローン増殖し始める時、または直前であり、
そして、前記疾患が変性疾患である場合、
(i) 該疾患は、細胞の損失をもたらす症状、およびアルツハイマー病またはプリオン関連疾患であり;そして
(ii) 該薬剤は、
(a)レギュレーターT細胞の産生を阻害し、該細胞の機能および/または活性を制限し、かつ、抗増殖薬、抗代謝薬、照射、dsRNAおよび抗体からなる群より選択され;そして、該薬剤の投与のタイミングは、該薬剤がレギュレーターT細胞に対してエフェクターT細胞より比率的に大きい効果を与えるような時であり、または
(b)ワクチンであり、該ワクチンの投与のタイミングは、レギュレーターT細胞の出現前にエフェクターT細胞の数および/または活性の増加を生み出す生得の免疫応答をブーストするような時である、
上記方法。 - 免疫系マーカーが急性期炎症マーカーである、請求項1に記載の方法。
- 患者がエフェクターT細胞の産生により特徴付けられる疾患をもち、薬剤が急性期炎症マーカーのレベルがその最低点に達した時と、免疫系マーカーが次の周期にピークになる前との間に投与される、請求項1または2に記載の方法。
- エフェクターT細胞がCD8+CD4-T細胞である、請求項1〜3のいずれか1項に記載の方法。
- レギュレーターT細胞がCD4+CD8-T細胞である、請求項1〜4のいずれか1項に記載の方法。
- 患者が少なくとも7日間モニタリングされる、請求項1〜5のいずれか1項に記載の方法。
- 患者が少なくとも3日毎にモニタリングされる、請求項1〜6のいずれか1項に記載の方法。
