Description

本発明は、質量分析、最も適切にはエレクトロスプレー質量分析−質量分析(MSMS)など適当な手法による、臨床診断に使える代謝産物及び/又は酵素活性及び/又はタンパク質の多重解析(例えば単一の血液スポット上での)に関する。
このシステムには、疾患又は障害、とりわけ遺伝性病(inherited disorders)の可能性について対象を評価する上での用途がある。本発明には、遺伝性疾患についての新生児スクリーニングにおいて特定の用途がある。臨床症状が現れていない時点で、信頼できる読取値を用いてスクリーニングが行われることが、本発明の利点である。
本発明の特徴は、単一の出発試料に対して複数の種類のアッセイが実施されることである。典型的には、これらのアッセイは、単一の分析の一部として実施される。これは、複数のアッセイを、並行して、連続して、又は同時に実施することを意味する場合がある。本発明の好ましい態様の特徴は、複数のアッセイが単一操作において実施されることである。これは、試料調製の予備ステップを連続的に(例えば、血液スポットの再水和に次いで基質の添加、それに次いでペプチダーゼの添加というように)実施することを意味する場合がある。しかし、複数アッセイを同時に実施することの原則は、単一の分析を実施して、単一の同一試料のインテロゲーション(複数可)から情報を収集できることである。試料を1回のステップで再水和させてから、実施する特定のアッセイの多様な必須要素を別々の連続的なステップで試料に加えるという事実だけでは、アッセイを別々な、又は異なるものとみなすには十分でない。キーポイントは、本発明により、同一試料から多様なアッセイが実施されることにより、試料の数(及び/又は必要な試料物質の量)が有利に減少し、単一の操作において有利にデータを収集する複数の異なるアッセイの実施が単純化されることである。
代謝産物スクリーニングは、試料中の特定の化学化合物の有無の定量を指す。これを代謝産物スクリーニングと呼ぶ理由は、当該化合物は、当該試料を特徴付けるものとして典型的に存在するか存在しないかのいずれかだからである。言い換えれば、アッセイされる化合物は、テストされる個々の対象の正常な代謝を特徴付けるものとして生成される(又は生成されない)。したがって、特定の代謝産物の有無の分析は、典型的には、特定の基質化合物を、又は任意の同族酵素活性を試料に加えることを含まない。当然ながら、実施される代謝産物アッセイの特定の形式によっては、代謝産物の存在が一定の状態を示すことがあり、又は、代謝産物の不在が一定の状態を示すことがある。特定の化合物の有無についての代謝産物スクリーニングには、化合物が検出されるか、又は検出されないかいずれかの場合に、非常に明確な読取値が典型的に示されるという利点がある。しかし、他の実施形態では、特定の代謝産物のレベルについてスクリーニングすることが望ましい場合がある。そのような実施形態は、アッセイされる特定の代謝産物のレベル又は濃度を正確に評価するために、計測手段の、より詳細な較正を必要とする可能性があり、或いは、試料(又は、外部基準試料、若しくは、公知の濃度の添加物質を添加された試料も)における内部標準の使用が必要な可能性があることは明らかである。そのような実施形態には、代謝産物の有無に加えて追加的な情報をもたらす、すなわち、試料中の当該代謝産物のレベル又は濃度についての情報をもたらす利点があることは明らかである。

酵素活性のスクリーニングは、試料が特定の触媒活性についてスクリーニングされる状況を指す。適切には、このスクリーニングは、触媒活性が作用できる基質を試料中に供給することにより行われる。活性が存在する可能性の有無を評価するためには、読取値は、典型的には、アッセイされる触媒活性(酵素活性など)の存在下で基質が変換されることになる予想生成物の有無である。触媒活性は、典型的には酵素活性である。そのような活性としては、以下を挙げることができる(酵素−基質;適応症の形式で記載):

本発明により試料に対して適切に実施されるさらなる種類の分析は、ポリペプチド分析である。この分析は、特定のポリペプチドの単純な有無のためのものであってもよい。或いは、この分析は、定義されたポリペプチドの特定の変異形の有無を特徴付け又は検出するためのものであってもよい。この分析は、当業者に公知の適当な任意の様式によって実施してもよい。
原理上は、読取値は、検査する試料内の個々の化学実体の存在の検出に適した当技術分野で公知の任意の手段により(例えば、その質量を定量することにより)収集できると考えられる。適切には、分析は、中間に衝突ステップを有する2つの異なる質量選別装置を備えた装置により実施できると考えられる。最も適切には、読取値は、質量分析(MS)を用いて収集する。イオンを分析する能力のある任意のMS分析機器を使用できると考えられる。例えば、MALDI/TOF/線形トラップ又は他の任意の型のMS分析機器を用いることができると考えられる。最も適切には、分析は、エレクトロスプレー質量分析−質量分析(MSMS)による。
ビオチニダーゼ欠損症の主要な懸念及び臨床効果は、両耳性難聴である。英国保健省は、聴覚障害のスクリーニングに多くの資金を注ぎ込んでいる。したがって、ビオチニダーゼ欠損症は稀な遺伝性欠損症ではあるものの、障害の主要な表現型は両耳性難聴であり、これに取り組むには終生、高額の費用がかかるという事実により、新生児においてビオチニダーゼ欠損症をスクリーニングすることは経済効果が非常に高いことを意味する。先行技術においては、ビオチニダーゼ欠損症のスクリーニングは、比色アッセイを用いて実施する。例えば、ニュージーランド、米国及び他の地域は、比色スクリーンを採用している。この先行技術の手法は、生じる色が弱いという意味で不十分なアッセイであるが、これは、先行技術の比色アッセイが非感受性だからである。この結果、現在のガイドラインによりテストを実施した場合でも、確実性に欠けることになる。加えて、血液スポットについてこのテストを実施することは技術的に非常に難しい。先行技術のアプローチには限界があることから、この領域における主要な焦点はアッセイの最適化に合わされている。対照的に、本発明により、本発明者らは、満足なアッセイは、最適化しなくても実際に作製できることを開示する。本発明者らのアプローチは、いずれかの比色酵素アッセイに焦点を合わせるのではなく、ビオシチン及びその生成物をモニターすることを含む。さらに、本発明者らの単純化したアッセイの主要な特徴は、緩衝化を用いないことである。まず、これには、同一試料において多重化することになる、可能性のある他の酵素アッセイを阻害せずに済むという利点がある。第2に、イオン抑制の問題は、緩衝液ではなく水中で基質を供給することにより有利に減少又は回避される。緩衝化は感受性を低下させることがあるため、その使用を回避することにより、本発明者らは、感受性を有利に高める。さらに、先行技術手法の場合と同様に緩衝液を使用した場合、これを除去するには手間も時間もかかるクロマトグラフィーが必要になる可能性がある。本発明者らのアプローチは、このステップを有利に排除する。
ビオシチンは、ビオチニダーゼ酵素の天然の基質である。ビオシチンは、ビオチンがリシンに結合してなる。本発明者らは、市販のビオシチンは相当量のビオチンを含有する傾向があることに注目した。したがって、本発明によれば、ビオシチン基質は、ビオチンを有利に除去したものである。言い換えれば、ビオシチン基質を精製して、混入ビオチンを除去する。これは、当技術分野で公知の適当な任意の手段により達成できる簡単な精製ステップである。適切には、ビオチンはクロマトグラフィーにより除去する。
本発明の利点は、触媒分析のための条件の標準化が不要なことである。さらに、過剰な基質を供給する必要もない。過剰な基質の必要性を取り除くことにより、本発明の利点は、基質濃度を自由に選んでテストの時間帯にわたり良好な結果をもたらすことができることである。加えて、過剰な基質を供給する必要性を省くことにより、半定量的な分析が可能になる。例えば、公知の濃度の基質を供給することにより、且つ、インキュベーションステップ後の基質及び生成物の量を分析することにより、特定の試料における活性量を良好に推定する基質:生成物比を導くことができる。これにより、有利なことに、この実施形態においては、同位体標識された安定な基質又は定量化用の生成物が必要なくなる。
先行技術では、チロシン血症1型は、血中のチロシンレベルの増加により検出されてきた。しかし、これは二次的又はさらには三次的な効果である。チロシン血症1型における酵素欠損は、実際はチロシンと直接隣合せの関係にはない。事実、多くの新生児は、とりわけ未熟児において、血流中のチロシンが上昇していた。このことは、先行技術による分析を複雑にする。例えば、オペレーターがチロシンレベルについて高いカットオフ値を設定した場合、多くの陽性結果が見落とされる可能性がある。逆に、オペレーターがチロシンレベルについて低すぎるカットオフ値を設定した場合、陰性が多く出すぎる可能性があり、これにより、追跡テスト又は再テスト時の資源が無駄になる。代替的なアプローチでは、スクシニルアセトンレベルが測定されている。しかし、これは、酸及び他の化学処理を必要とし、技術が要求される測定である。チロシン血症1型における実際の酵素欠損は、スクシニルアセトンの蓄積を引き起こすフマリルアセトアセターゼ欠損である。スクシニルアセトンは、ポルフォビリノーゲン合成酵素(PBS,porphobilinogen synthase)活性を阻害する。PBSアッセイは、スクシニルアセトン濃度の代わりとして用いられている。潜在的な酵素であるポルフォビリノーゲン合成酵素のアッセイは、先行技術における困難を伴う可能性がある。まず、アッセイに必要な濃度で基質を使用した場合、基質が酵素を阻害する可能性がある。或いは、ポルフォビリノーゲン自体の測定を試みるようにアッセイを設定した場合、このアッセイには問題がある可能性があるが、その理由は、ポルフォビリノーゲンは、典型的に、非常に速やかに代謝され続けるからである。この2つの要素があることにより、先行技術による長時間のアッセイでは一貫して低い結果が出そうだというさらなる困難が生じる。本発明者らは、基質として5−アミノレブリン酸を供給し、生成されるポルフォビリノーゲンを測定するというアプローチを提案する。加えて、並行処理においては、このアッセイはスクシニルアセトンの存在下で反復できる。スクシニルアセトンは、ポルフォビリノーゲン合成酵素を阻害する。したがって、スクシニルアセトンがある場合とない場合との結果を比較することで、ポルフォビリノーゲン合成酵素の活性を直接アッセイすることによりチロシン血症1型を検出することが可能であることを明確に実証できる。
試料は、テストされる対象に由来する適当な任意の材料であってもよい。適切には、試料は血液を含む。いくつかの実施形態では、そのような血液は、より早期の診断につながるという利点を有する可能性がある臍帯血(臍の緒の血液)であってもよい。より適切には、試料は毛細管血である。適切には、試料は、踵を針で刺して得る毛細管血である。より適切には、試料は、生後0日〜10日の新生児から採取する、踵を針で刺して得る毛細管血を含む。適切には、血液は、生後1日〜8日目、適切には生後3日〜8日目、適切には生後5日〜8日目に採取する。
適切には、プロテアーゼ消化ステップは、試料を分析する前の最終ステップ(すなわち、読取り前の最終ステップ)として実施する。典型的には、試料をまず再水和させてから、任意の基質を試料に加え(適切には水のみの中に加え、一切の緩衝系の添加を回避する)、次に、試料をインキュベートして当該基質の触媒作用を行わせ、ここで初めてプロテアーゼを加えてから適切にインキュベーションし、プロテアーゼを作用させることになる。プロテアーゼは、試料中の酵素が、外因的に加えられた基質(あれば)に対して作用する機会を得た後で加えることが好ましいのは明らかである。プロテアーゼを基質と同時に加えると、酵素自体がプロテアーゼの作用により分解されることになり、それにより当該酵素の活性について偽陰性の結果がもたらされる危険が生じるリスクがある。したがって、適切には、プロテアーゼ添加及びプロテアーゼインキュベーションのステップは、最後、又は試料分析(読取り)の直前に実施する。
タンパク質分析についての先行技術では、プロテアーゼ消化の前にタンパク質(複数可)を変性させることが必要と考えられている。先行技術では、例えば、異常ヘモグロビン症に関して、試料は、アセトニトリル及びギ酸を別々に加えてはっきり変性させてから放置する。先行技術の方法によれば、次に、トリプシンを加える前に炭酸アンモニウムを使用してpHをトリプシンの最適pHに近付けることで、試料中のギ酸を中和させる。乾燥血液スポット中のタンパク質は既に少なくとも部分的に変性しているという洞察に基づくこうしたステップの排除は、本発明の方法の強力な単純化である。したがって、適切には、本発明の方法は、有機溶媒又は有機酸をベースにした試料調製ステップを具体的に省く。適切には、本発明の方法は、化学変性に基づく試料調製ステップを具体的に省く。適切には、本発明の方法は、有機溶媒の介入ステップ(複数可)/変性を伴わない試料調製を含む。広範な態様では、本発明は、異常ヘモグロビン症の診断法(複数可)などのタンパク質分析法(複数可)であって、本明細書中で教示する要領で当該方法が多重化されない場合であってもそのようなステップを省く方法に関する。
定量化は、同位体標識された安定な基質を使用して達成できる。適切には、前記基質は、少なくとも1つの安定同位体、適切には少なくとも2つの安定同位体(複数可)で標識される。適切には、前記同位体(複数可)は、重水素、炭素13、窒素15及び酸素18からなる群から選択される。適切には、前記同位体は炭素13である。2つの同位体を使用する場合、適切にはそれらは炭素13及び窒素15である。適切には、基質がポリペプチドである場合、安定同位体は、標識がエンドペプチダーゼによる切断(あれば)後のペプチドにより保持されるように、所望のエンドペプチダーゼ切断部位(あれば)についてはN末端に組み込まれる。標準的な同位体−希釈手順を用いて代謝産物/ペプチド(例えばペプチド安定同位体)を測定するための安定同位体内部標準は、酵素基質の最初の添加と共に、又はプロテアーゼ添加と共に加えてもよい。酵素生成物の安定同位体は、適切には、酵素インキュベーション後に加えるが、これには、安定同位体が酵素活性を阻害しないという利点がある。特異性を求めて安定同位体標識された基質を加える場合、これは適切には最初の添加時点、すなわち、酵素(複数可)が、そのような任意の基質(複数可)に対して作用する機会を有するような時点においてである。酵素活性又は相対的な酵素活性の有無が、典型的には定量化を必要とせずにアッセイされることは、本発明の利点である。典型的には、こうした活性の有無は、生成された生成物又は基質:生成物比を用いて測定できる。定量化をさらに特徴とする実施形態の主要な利点は、準最適な基質濃度(複数可)を用いるときである。
本発明のシステム及び方法によれば、MSMSによる全血代謝産物、酵素活性及びトリプシンペプチドの多重測定に基づき、3.2mmの単一の血液スポット上で、且つ、試料の移動を伴わずに、遺伝性代謝疾患及び異常ヘモグロビン症の同時診断が可能になる。
[実施例]
深いウェルプレートに入った血液スポットに、フェニルアラニン、チロシン及びオクタノイルカルニチン用の安定同位体を含んだ基質100μlを加えて37℃で30分間インキュベートする。
2μlを注入。75μl/分。移動相はアセトニトリル:水 1:1に0.025%ギ酸を加えたもの。臨床的に診断される異常ヘモグロビン症用に合計1分のMSMS MRM取得、5−アミノレブリン酸、ポルフォビリノーゲン、ビオシチン、ビオチン、リシン、d5−リシン、ビオチンPABA、PABA、d4−PABA、フェニルアラニン、チロシン、オクタノイルカルニチン、d5−フェニルアラニン、d4−チロシン及びd3−オクタノイルカルニチン。
1)Schleicher & Schuell社製の濾紙(新生児スクリーニングに使用する)上に載せた全血から調製した3.2mmの全血スポットをインキュベートしておき、ポルフォビリノーゲン合成酵素の天然の基質である5−アミノレブリン酸を加えて37℃で1時間30分インキュベートし、MSMSによりポルフォビリノーゲンを測定した。
2)1の要領で、但し、100μmol/lのスクシニルアセトンを含ませ、アッセイの阻害(ポルフォビリノーゲンは全く(実質的に)検出されない)、ひいては、チロシン血症1型を検出することが可能であることを実証した。
3)Schleicher & Schuell社製の濾紙(新生児スクリーニングに使用する)上に載せた全血から調製した3.2mmの全血スポットをインキュベートしておき、ビオチニダーゼの天然の基質であるビオシチンを加えて37℃で1時間30分インキュベートし、MSMSによりビオチン及びリシンを測定した。
4)3の要領で、但し、生理食塩水で洗浄した赤血球に、ビオチニダーゼ欠損症患者に由来する血漿を加えたものを使用して、ビオチニダーゼ活性の低下を実証した。この実験及び3は、基質中に相当な量のビオチンが存在することにより複雑化した。判別は依然として可能であり、これによりビオチンダーゼ(biotindase)欠損症の検出が可能になったが、ビオチンを含有しない基質又は安定同位体標識されたリシンの利点(シグナルをチェックしたところ標準条件下では存在しない)が浮き彫りになった。
5)Schleicher & Schuell社製の濾紙(新生児スクリーニングに使用する)上に載せた全血から調製した3.2mmの全血スポットをインキュベートしておき、ビオチニダーゼの人工基質であるビオチンPABAを加えて37℃で1時間30分インキュベートし、MSMSによりビオチン及びPABAを測定した。
6)5の要領で、但し、生理食塩水で洗浄した赤血球に、ビオチニダーゼ欠損症患者に由来する血漿を加えたものを使用して、ビオチニダーゼ活性の低下を実証した。この実験及び5は、基質中に相当な量のビオチンが存在することにより複雑化した。判別(とりわけPABAを使用した場合)は依然として可能であり、これにより、ビオチンダーゼ欠損症の検出が可能になったが、ビオチン/PABAを含有しない基質の利点が強調された。
7)5及び6の要領で、但し、放出されるPABAの定量化のために安定同位体PABAを加えた。
8)前記の要領で、但し、5−アミノレブリン酸及びビオシチン、又は、5−アミノレブリン酸及びビオチンPABA、又は、5−アミノレブリン酸、ビオシチン及びビオチンPABAを加えて、両方の酵素を同時に測定し、同一血液スポット上でビオチニダーゼ及び/又はチロシン血症1型のいずれの診断も行うことができることを実証した。
9)前記の要領で、但しトリプシンを加えて37℃でさらに30分間インキュベートして、正常な酵素活性、ヘモグロビンパターン、フェニルアラニン及びオクタノイルカルニチンを実証した。
10)鎌状赤血球疾患に罹患している対象に由来する試料以外は9の要領で、正常な酵素活性、フェニルアラニン、オクタノイルカルニチン及び鎌状赤血球疾患のヘモグロビンパターンを実証した。
11)9の要領で、但し、PKU患者に由来する試料を加えて、正常な酵素活性、ヘモグロビンパターン、オクタノイルカルニチン及びPKUの診断に使えるフェニルアラニンの増加を実証した。
12)9の要領で、但し、MCADD患者に由来する試料を加えて、正常な酵素活性、ヘモグロビンパターン、フェニルアラニン及びMCADDの診断に使えるオクタノイルカルニチンの増加を実証した。
この実施例では、プロテアーゼ処理を含む多重解析並びに試料中の遊離アミノ酸のアッセイを示すが、これらは、先行技術に基づく手法では不可能であった。さらに、本発明者らは、分析及び処理はそれぞれ、本発明による多重化の形で実施できること、並びに、分析の読取値はそれぞれ、代謝産物、酵素活性、プロテアーゼ分析のために単一の試料のみを調製するという事実にもかかわらず依然としてロバストで信頼できること、並びに、この試料は外因的な緩衝化を行わず、溶媒に基づく変性ステップを伴わず、短い時間枠で調製されることを実証するが、これらは先行技術では不可能であった。
深いウェルプレートに入った3.2mmの血液スポットに基質100μl(50μM、濃度の検討後、5−アミノレブリン酸+ビオシチン又はビオチニル−PABA)を加えて37℃で60分間インキュベートする。
トリプシン100μl(0.5mg/ml)を加えて37℃でさらに30分間インキュベートする。

これらの種(並びにヒドロキシプロリン)のそれぞれが132.3ピークに寄与することに注目。これが、5−アミノレブリン酸濃度などの基質濃度が有利に最小化する理由である。したがって、多重化試料中で使用される5−アミノレブリン酸が少ないほど、その存在が132.3ピークを「膨らませる」程度は低くなる。先行技術の手法は、本明細書中で教示するものの約20倍も多い基質を典型的に使用することから、そうした効果は先行技術の方法においては観察されなかったであろうし、観察できなかったであろう。適切には、50μM 5−アミノレブリン酸基質を使用する。
これにより、トリプシンでのインキュベーションに応じたバリンシグナルの増加が実証される(MSUD患者に由来する古いスポットにおけるものではない)。安定同位体ISは影響されないことに注目。

オロチン酸は、ピリミジンの合成における重要な代謝産物である。したがって、遺伝性の新規のピリミジン合成障害、具体的にはUMP合成酵素欠損症においては、尿中のオロチン酸濃度が上昇する。オロチン酸は、カルバミルリン酸及びアスパラギン酸から合成されることから、尿素回路の遺伝性障害、すなわちオルニチントランスカルバミラーゼ(OTC)欠損症、アルギニノコハク酸合成酵素欠損症及びアルギニノコハク酸リアーゼ欠損症においては、尿中のオロチン酸が上昇する(図9を参照)。
オルニチントランスカルバミラーゼ欠損は、尿素回路代謝のX連鎖性の先天異常であり、高アンモニア血症を引き起こす。この障害は、栄養補助食品としてのアルギニン及び低タンパク質食で治療可能である。
尿素回路障害は、高アンモニア血症、脳症及び呼吸性アルカローシスの3徴候を特徴とする。尿素回路の酵素の生合成における異なる欠陥を含む5つの障害についての記載がある:OTC欠損症、カルバミルリン酸合成酵素欠損症(237300)、アルギニノコハク酸合成酵素欠損症又はシトルリン血症(215700)、アルギニノコハク酸リアーゼ欠損症(207900)及びアルギナーゼ欠損症(207800)。
Russell et al. (1962)は、慢性のアンモニア中毒症及び精神機能低下に罹患している2人のいとこについて記載した。肝生検により、肝臓のOTC活性が非常に低いことが示された。尿素合成においてオルニチンからシトルリンへの変換レベルで障害が存在すると推定された。
Rowe et al. (1986)は、13名の無症候女性のヘテロ接合体について概説した。彼女たちは、早ければ生後1週間、又は遅ければ6歳で発症した。診断前の症状は、非特異的であった:突発性の極度の易興奮性(100%)、突発性の嘔吐及び嗜眠(100%)、タンパク質回避(92%)、運動失調(77%)、段階IIの昏睡(46%)、成長遅滞(38%)、発育遅滞(38%)及び発作(23%)。母乳からの離乳時に発症することが多かった。発端者を含めると、13家族の女性の42%が症状を有した。
Scott et al. (1972)は、OTC欠損のX連鎖劣性遺伝を維持した2家系について発表した。Short et al. (1973)は、4家族を研究したが、全てX連鎖遺伝と一致した。OTC欠損についてヘテロ接合の女性の肝臓において、Ricciuti et al. (1976)は、2種類の細胞(1つは酵素活性が欠損しており、もう1つは正常)を実証した。細胞モザイク現象の知見により、OTC遺伝子がX連鎖性であることが確認された。したがって、X連鎖性優性遺伝の証拠は以下に基づく:(1)ほとんどの場合、ほぼ完全に酵素が存在しない男性における障害の重篤な性質、(2)ヘテロ接合の女性における臨床的重篤性及び酵素レベルの広範なばらつき、(3)ヘテロ接合の女性の肝臓におけるリオン(Lyon)現象の実証及び(4)マウスにおけるX連鎖の実証(DeMars et al., 1976を参照)。
Burdakin and Norum (1981)は、OTC欠損症とX染色体上のG6PD(305900)との連鎖について、3つの機会における少なくとも1つの組換え体を観察した。遺伝子座は、X染色体の反対側の末端にあることが後に見出された。
Rowe et al. (1986)は、家族歴、食事歴、突発性非特異的症状、タンパク質の枯渇(withdrawal of protein)への応答及び他の特徴により、早期診断が可能になると提唱した。テストした5名の患者においては、診断の時点ではIQは70を下回っていた。
OTCはX連鎖疾患であるが、キャリアー女性が罹患することがあり、重篤度のばらつきが非常に大きい。
男女とも、ほぼ全般的に致死性である非常に重篤な疾患を新生児期間において発症することがある。
男女とも、残存酵素活性及び低タンパク質食の自己選択によっては、小児から成人まで人生のあらゆる段階で、反社会的行動から死まで、さまざまな重篤度で発症する可能性もある。
新生児スクリーニングは、適切なテストが存在しないという理由から、考慮されてこなかった。
乾燥血液スポット上でオロチン酸の正常値を検出できるか?
予測される生理学的範囲内で、乾燥血液中のオロチン酸における変化を検出できるか?
成人の志願者が、ヘパリンリチウム処理した血液試料5mlを提供し、これを−80℃で保存した。次いで、この試料を解凍、混合し、オロチン酸標準物質を以下のとおり加えた:
全血90μl+脱イオン水10μl
(最終濃度、基本)
全血100μl+100μmol/lオロチン酸1μl
(最終濃度、基本+1μmol/l)
全血100μl+500μmol/lオロチン酸1μl
(最終濃度、基本+5μmol/l)
全血100μl+2.5mmol/lオロチン酸1μl

全血100μl+10mmol/lオロチン酸1μl
(最終濃度、基本+100μmol/l)
DBS ND対照、DBS対照+25μmol/lオロチン酸及びDBS MMAについてのクロマトグラム(図10、11及び12)を参照:
いずれの場合も、上のパネルはオロチン酸(青)及びオロチン酸の安定同位体内部標準(赤)、下のパネルはメチルマロン酸(青)及びメチルマロン酸の安定同位体標準(赤)。
現在のクロマトグラフィーシステムを用いて乾燥血液スポット上でオロチン酸の正常値を検出することは、干渉が生じるため問題である。
本発明者らは、予測される診断範囲内での乾燥血液スポット中のオロチン酸における変化を正確に検出及び測定できる。
他の診断用有機酸(例えばメチルマロン酸)を同時に測定できる、すなわち、このマーカーは、本発明による多重化が可能である。
芳香族アミノ酸デカルボキシラーゼ(AADC,aromatic amino acid decarboxylase)は、フェニルアラニン及びチロシンからの神経伝達物質(とりわけドパミン及びセロトニン)の合成に関与する代謝経路における酵素である。遺伝性AADC欠損症に罹患すると、CSF中のドパミン及びセロトニンが減少して、基質代謝産物であるL−ジヒドロキシフェニルアラニン(L−DOPA,L-dihydroxy-phenylalanine)及び5−ヒドロキシトリプトファン(5−HT,5-hydroxytryptophan)が増加する結果となる。3OMDOPAは、L−DOPAの3−O−メチル化により生成され、増加もする。
AADC欠損は、神経伝達物質代謝における先天異常であり、セロトニン及びカテコールアミンの欠損症が組み合わさった状態を引き起こす(Abeling et al., 2000)。
Hyland and Clayton (1990)及びHyland et al. (1992)は、いとこ同士の両親のもとに生まれた男性の一卵性双生児について報告したが、この双子は、生後2カ月で、重篤な低血圧症、及び、泣いた後の手足の伸展、注視痙攣及びチアノーゼからなる発作性運動を発症した。時折生じる末端の舞踏アテトーゼ様運動も示した。後に、体温調節障害及び起立性低血圧が観察された。検査分析においては、CSF中のホモバニリン酸(HVA,homovanillic acid)及び5−ヒドロキシインドール酢酸(5−HIAA,5-hydroxyindoleacetic acid)の濃度の大規模な低下、並びに、全血中セロトニン及び血漿中カテコールアミンの減少が示された。尿中のL−DOPA、5−ヒドロキシトリプトファン(5−HTP,5-hydroxytryptophan)及び3−メトキシチロシンの排泄値が顕著に上昇しており、その全てが生化学経路におけるAADC段階に先行する。この知見から、セロトニン及びドパミンの合成は、中枢神経系及び末梢神経系の両方において影響を受けたことが実証され、AADC欠損症と一致した。AADC酵素活性は血漿中及び肝組織中において激しく低下した(対照の1%)。モノアミン酸化酵素阻害薬、ドパミンアゴニスト及びピリドキシンでの治療の結果、緊張及び運動が著しく改善した。両親は無症候であったが、AADC欠損についてヘテロ接合であることと一致する生化学的プロファイルを有していた。
Verbeek et al. (2007)は、血漿中のAADC酵素活性を調べるための、その基質である5−ヒドロキシトリプトファン(5−HTP)及び3,4−ジヒドロキシフェニルアラニン(L−DOPA)を両方とも使用したアッセイについて記載した。Verbeek et al.は、対照血漿におけるAADC酵素活性は、平均すると、基質としてL−DOPAを使用した場合のほうが5−HTPを用いた場合より8〜12高い因子であることを見出した。AADCの両方の基質が、酵素の同じ活性部位をめぐって競合する結果、AADC欠損症患者における残存酵素活性が同等に低下する。AADC欠損症患者においては、HVA、5−HIAA及びMHPGのCSF濃度がそうであるように、両方の基質に対する酵素活性は同等に低下するが、ヘテロ接合体は中間のAADC活性レベルを有する。これらの酵素及びアッセイは、血液上で実施できる。
Pons et al. (2004)は、AADC欠損症の臨床管理は、モノアミン作動性伝達を増強するために、ビタミンB6、ドパミンアゴニスト及びMAO阻害薬を含むことが普通であると述べた。AADC患者群における治療応答の評価において、著者らは、2つの主群を検出した:1つは治療に応答し発育上の前進を遂げた男性5名を有する群、2つ目は、治療への応答が乏しく薬物誘発性の運動障害をしばしば発症した女性5名及び男性1名の群。この知見から、女性のほうがドパミン系に左右されるという、モノアミン作動系における性差が示唆された。
3OMDOPA(3−メトキシチロシンは3OMDOPAの別名にすぎないことに注意)についての唯一の言及は、尿に関するものである。
診断は、未だにその初期段階にあり、CSF中の神経伝達物質の減少の測定に主に頼っている。
3障害のいずれについての新生児スクリーニングの可能性も、全てが新生児スクリーニングについての国際基準に適合するにもかかわらず、先行技術においては考慮することすら行われてこなかった。
乾燥血液スポット上で3OMDOPAの正常値を検出できるか?
予測される生理学的範囲内で、乾燥血液中の3OMDOPAにおける変化を検出できるか?

DBS AADC患者、DBSグルタリルCoAデヒドロゲナーゼ欠損症患者及びDBS VLCAD欠損症患者についての、追って記載のクロマトグラムを参照。
いずれの場合も、上のパネルは3OMDOPA(およそ2.7分)定量化イオン(青)及び3OMDOPA確認イオン(赤)、中央のパネルはグルタリルカルニチン(およそ3.0分)(青)及びグルタリルカルニチンの安定同位体内部標準(赤)、下のパネルは、テトラデセノイルカルニチン(およそ2.5分)(青)、及び、内部標準としての安定同位体テトラデカノイルカルニチン(赤)である。
現在のシステムを用いて乾燥血液スポット上で3OMDOPA酸の正常値を検出することは、濃度が低すぎるため問題である。異なる計測手段であれば、この問題に対処できる。
本発明者らは、予測される診断範囲内で乾燥血液スポット中の3OMDOPAにおける変化を正確に検出及び測定できる(実際の患者の試料は確実である)。
他の診断用化合物(例えば、グルタリルカルニチン、テトラデセノイルカルニチン及びADMA)を同時に(すなわち、本発明による多重化の形で)測定できる。
Claims (14)
- 対象における疾患の診断を補助する方法であって、
前記対象に由来する、血液を含む試料を供給するステップと、
前記試料の少なくとも2つの特徴をアッセイするステップであり、前記特徴が、
(i)前記試料に含まれるポリペプチドの構造組成、
(ii)前記試料に含まれる代謝産物、及び
(iii)前記試料に含まれる触媒活性
から選択され、
前記少なくとも2つの特徴が、
(i)前記試料に含まれるポリペプチドの構造組成と、
(ii)及び(iii)から選択される少なくとも1つのさらなる特徴とを含み、
前記少なくとも2つの特徴のそれぞれが同一試料の多重解析により決定されるステップとを含み、
前記試料に含まれるポリペプチドの構造組成をアッセイするステップが、
(a)前記試料にペプチダーゼを加えるステップ、
(b)ペプチダーゼ処理後に前記試料中の前記ポリペプチドを分析するステップ、及び
(c)前記ポリペプチドの構造組成に関して(b)の情報から推測するステップ
を含む方法。 - 試料が乾燥血液スポットを含む、請求項1に記載の方法。
- 試料が、天然に存在するその成分によってのみ緩衝化される、請求項1又は2に記載の方法。
- ペプチダーゼがトリプシンである、請求項1に記載の方法。
- 試料に含まれるポリペプチドが、ヘモグロビン又はミオグロビンのうち1又は複数である、請求項1〜4のいずれかに記載の方法。
- 試料に含まれる代謝産物をアッセイするステップが、フェニルアラニン、オクタノイルカルニチン又はアシルカルニチンの有無についてアッセイするステップを含む、請求項1〜5のいずれかに記載の方法。
- 試料に含まれる触媒活性をアッセイするステップが、
(a)前記触媒活性の作用を受けやすい基質を前記試料に加えるステップ、及び
(b)前記基質の有無及び/又は前記基質に作用する前記触媒活性の作用による生成物の有無について前記試料を分析するステップ
を含む、請求項1〜6のいずれかに記載の方法。 - 触媒活性の作用に感受性がある1つを超える基質を加えて分析する、請求項1〜7のいずれかに記載の方法。
- 1又は複数の基質が水溶性である、請求項1〜8のいずれかに記載の方法。
- 各基質が水中にのみ加えられる、請求項9に記載の方法。
- 特徴がMS分析により決定される、請求項1〜10のいずれかに記載の方法。
- MSがエレクトロスプレー質量分析−質量分析(MSMS)である、請求項11に記載の方法。
- 3つの特徴(i)、(ii)及び(iii)のそれぞれをアッセイする、請求項1〜12のいずれかに記載の方法。
- 試料がインビトロ試料である、請求項1〜13のいずれかに記載の方法。
