Claims (9)
- 人又は動物のような検査対象における心房性細動を検出する方法(100)であって、
所定の期間にわたる患者のプレチスモグラフ波形(300)を第1の監視装置が取得する段階(110,120)と、
前記プレチスモグラフ波形(300)から一つのパルスをプロセッサ(560)が抽出する段階(125)と、
前記プレチスモグラフ波形(300)から一組のパルス間隔を前記プロセッサ(560)が測定する段階(130)と、
前記プレチスモグラフ波形(300)から一組の特徴部を前記プロセッサ(560)が測定する段階(135)と、
前記プレチスモグラフ波形(300)からの前記一組のパルス間隔及び前記一組のパルス特徴部を前記プロセッサ(560)が分析して、前記患者の心房性細動状態を前記プロセッサ(560)が決定する段階(140)と、
前記プレチスモグラフ波形(300)からの前記パルスを前記プロセッサ(560)が分析する段階(145)と、
前記プレチスモグラフ波形(300)からの前記パルスを前記プロセッサ(560)が分析する段階が、
平均心拍数増加及び平均心拍数変動の分析(325)を前記プロセッサ(560)が行う段階と、
前記患者のNIBPパルス(335)を第2の監視装置が取得する段階と、
前記患者のNIBPパルス(335)を前記プロセッサ(560)が分析する段階と、
前記患者の一組の医療記録(345)を前記プロセッサ(560)が取得し分析する段階と、
前記平均心拍数増加及び前記平均心拍数変動の分析(325)と、前記NIBPパルス分析と、前記医療記録分析とを前記プロセッサ(560)が組み合わせ、その結果から、前記心房性細動状態を前記プロセッサ(560)が確認する、方法(100)。 - 前記第1の監視装置はパルス・オキシメータ・センサである、請求項1記載の方法。
- 前記一組の特徴部の1つは一組のパルス振幅(410)である、請求項1記載の方法。
- 更に、前記プレチスモグラフ波形(300)からの前記一組のパルス間隔及び前記一組のパルス特徴部を前記プロセッサ(560)が分析して、前記患者の心房性細動傾向を検出する段階と、前記心房性細動傾向及び状態(370,380)を前記プロセッサ(560)が報告する段階とを含んでいる請求項1記載の方法。
- 前記一組のパルス間隔を分析する段階は、前記一組のパルス間隔について隠れマルコフ・モデル分析(330)を行い且つ前記一組のパルス間隔についてコンテクスト分析(340)を行う段階を含んでおり、前記隠れマルコフ・モデル分析は、患者が不規則な心拍リズムを持つ確率を決定し、また前記コンテクスト分析(340)は心房性細動を不規則な心拍リズムの源から除外する、請求項1記載の方法。
- 前記一組のパルス特徴部を分析する段階は、一組のパルス振幅の各々の相次ぐものの間の振幅変動(360)を算出する段階を含み、前記心房性細動状態の決定がパルス振幅変動(360)の分析に依存する、請求項1記載の方法。
- 一組のパルス振幅を分析する段階が、前記一組のパルス振幅についての一組の充填時間動態を決定する段階を含み、更に、前記一組のパルス振幅を分析する段階及び前記一組の充填時間動態を決定する段階は、存在する心房増強度(350)の表示を与える、請求項1記載の方法。
- 前記パルス分析段階は、前記患者の前記心房性細動状態(370)を決定することに寄与する、請求項1記載の方法。
- 人又は動物のような検査対象における心房性細動を検出するシステムであって、
所定の期間にわたる患者のプレチスモグラフ波形(300)を取得する第1の監視装置と、
前記患者のNIBPパルス(335)を取得する第2の監視装置と、
プロセッサ(560)とを含み、
前記プロセッサ(560)が、
前記プレチスモグラフ波形(300)から一つのパルスを抽出し(125)、
前記プレチスモグラフ波形(300)から一組のパルス間隔を測定し(130)、
前記プレチスモグラフ波形(300)から一組の特徴部を測定し(135)、
前記プレチスモグラフ波形(300)からの前記一組のパルス間隔及び前記一組のパルス特徴部を分析して、前記患者の心房性細動状態を決定し(140)、
前記プレチスモグラフ波形(300)からの前記パルスを分析し(145)、
前記プレチスモグラフ波形(300)からの前記パルスを分析し、
平均心拍数増加及び平均心拍数変動の分析(325)を行い、
前記患者のNIBPパルス(335)を分析し、
前記患者の一組の医療記録(345)を取得し分析し、
前記平均心拍数増加及び前記平均心拍数変動の分析(325)と、前記NIBPパルス分析と、前記医療記録分析とを前記プロセッサ(560)が組み合わせ、その結果から、前記心房性細動状態を確認するように構成されている、システム。
