Description
本出願は、2002年6月5日付け米国特許出願第10/162796号の一部継続であり、前記出願は2001年7月31日付け米国特許出願第09/919,732号の一部継続であり、前記出願は、2001年6月6日付け米国特許仮出願第60/295,807号、第60/295,806号、第60/295,805号、第60/295,804号、および第60/296,803号に対して優先権を主張する。同時継続の2005年3月16日付け米国特許出願第11/081884号にも言及しておく。

本発明は総じて心臓分野に関し、特に、心臓血管系疾患、例えば、但し限定的ではないが、アテローム性動脈硬化、虚血、高血圧、再狭窄、狭心症、リウマチ性心疾患、先天性心血管障害、動脈炎症、その他の動脈、細動脈、毛細血管疾患などを罹患する患者を処置する方法および細胞組成物に関する。本発明は、心臓または血管バイパス手術といった従来の侵襲的な治療処置の代わりに、またはそれと合わせて使用し得る、処置、治療、および方法を意図する。
治療有効量の体細胞性幹細胞を、単独で、または、サイトカイン、例えば幹細胞因子(SCF)、顆粒球コロニー刺激因子(G−CSF)、顆粒球マクロファージコロニー刺激因子(GM−CSF)、間質細胞由来因子−1、steel factor、血管内皮増殖因子、マクロファージコロニー刺激因子、顆粒球マクロファージ刺激因子、もしくはインターロイキン−3、または幹細胞を刺激および/もしくは可動化し得る任意のサイトカインからなる群から選択されるサイトカインと組合せて含む方法および/または医薬組成物。サイトカインは、単独で、または、幹細胞の刺激および/または可動化;早期または後期造血の維持(下記参照);単球の活性化(下記参照)、マクロファージ/単球増殖;分化、運動性、生存(下記参照)において能力を発揮する任意の他のサイトカイン、並びに薬学的に許容可能な担体、希釈剤、または賦形剤(これらの組合せを含む)と組合せて投与することができる。幹細胞は、成人幹細胞、例えば造血幹細胞もしくは心臓幹細胞もしくはこれらの組合せ、または心臓幹細胞と任意の別種の幹細胞の組合せが有利である。
幹細胞と必要に応じてサイトカインを含む、かかる処置、治療、または予防に使用される薬剤。
かかる処置、治療、または予防に使用される、処方用の幹細胞および必要に応じてサイトカインを含むキット。

心血管疾患は先進工業国全体を通して主要な健康リスクである。最もよく見られる心血管疾患であるアテローム性動脈硬化は、心臓麻痺、卒中、四肢の壊疽の主因であって、米国で主要な死因である。アテローム性動脈硬化は、多くの細胞種と分子要因が関与する複雑な疾患である(詳細は、Ross,1993,Nature 362:801−809を参照)。
視覚回復療法として使用され得る、網膜幹細胞の網膜細胞へのin vivoまたはin vitro分化に関する米国特許第6,117,675号。
ランゲルハンス幹細胞島から機能性島への発達に関わる米国特許第6,001,934号。
軟骨組織修復のための間葉幹細胞の使用、および靱帯の再生のための間葉幹細胞の使用に関わる米国特許第5,906,934号および第6,174,333号。例えば、ここでは、幹細胞をゲル基質に包埋し、それを収縮させ、移植して、所望の軟組織を置換する。
細胞移植(cell transplantation)を伴うグラフトに関わる米国特許第6,099,832号および第6,110,459号。
自己骨髄および間葉幹細胞の心筋内注入に関わるPCT出願、PCT/US00/08353号(WO00/57922)およびPCT/US99/17326号(WO00/06701)。これらの特許は本発明のような造血幹細胞(特に本発明の造血細胞は有利には単離および/または精製された成人造血幹細胞である)の投与、移植、沈着、または使用を教示も示唆もしていない。
驚くべきことに、心筋梗塞のあとに梗塞の周囲の心筋に体細胞性幹細胞を移植すると、損傷領域に移動し、筋細胞、内皮細胞および平滑筋細胞に分化し、次いで増殖し、心筋、冠状動脈、細動脈および毛細血管を含む構造体を形成し、梗塞の構造的および機能的完全性を回復することが見出された。
本発明は更に、かかる処置、治療、または予防に使用するための処方用の、1種以上のサイトカインを含むキットを提供する。
本発明は更に、上記キットおよび組成物を製造する方法を提供する。
本発明は更に、1種以上のサイトカインを、心臓疾患または血管性疾患を処置するための治療剤と組合せて含む、かかる処置、治療、または予防に使用するための組成物および/またはキットを提供する。
本発明は更に、少なくとも1種のサイトカインの投与を含む、損傷後まもない心筋および/または心筋細胞を修復および/または再生するための方法および/または組成物を提供する。
本発明は更に、体細胞性幹細胞、例えば成人幹細胞または心臓幹細胞または造血幹細胞またはこれらの組合せ、例えば成人心臓幹細胞または成人造血幹細胞またはこれらの組合せ、または心臓幹細胞と別種の幹細胞の組合せ、例えば成人心臓幹細胞と別種の成人幹細胞の組合せ、並びにサイトカインを投与することを含む、損傷後まもない心筋を修復および/または再生するための方法および/または組成物を提供する。
本発明は更に、損傷後まもない心筋および/または心筋細胞を修復および/または再生するのに使用される医薬組成物を含むキットを提供する。
故に、本発明は、幹細胞および必要に応じてサイトカインを含む、かかる処置、治療、または予防に使用される薬剤を提供する。

本発明は、治療有効量の体細胞性幹細胞を、単独で、または、サイトカイン、例えば幹細胞因子(SCF)、顆粒球コロニー刺激因子(G−CSF)、顆粒球マクロファージコロニー刺激因子(GM−CSF)、間質細胞由来因子−1、steel factor、血管内皮増殖因子、マクロファージコロニー刺激因子、顆粒球マクロファージ刺激因子、肝細胞増殖因子(HGF)、インシュリン様増殖因子(IGF−1)、もしくはインターロイキン−3、または幹細胞を刺激および/もしくは可動化し得る任意のサイトカインからなる群から選択されるサイトカインと組合せて含む方法および/または医薬組成物を提供する。サイトカインは、単独で、または、幹細胞の刺激および/または可動化;早期または後期造血の維持(下記参照);単球の活性化(下記参照)、マクロファージ/単球増殖;分化、運動性、生存(下記参照);心臓疾患または血管性疾患の処置において能力を発揮する任意の他のサイトカインまたは薬剤、並びに薬学的に許容可能な担体、希釈剤、または賦形剤(これらの組合せを含む)と組合せてまたは一緒に投与することができる。
例えば治療化合物を含む本発明の医薬組成物は、任意の相容性担体、例えば種々のビヒクル、アジュバント、添加剤、および希釈剤を含む注射可能な製剤で患者に投与することができる。または、化合物は、持続放出皮下インプラントまたは標的化送達系、例えばモノクローナル抗体、イオン導入、ポリマー基質、リポソーム、およびミクロスフィアの形態で患者に非経口投与し得る。
処置は一般に、疾患の進行期間、薬剤有効性、および処置対象患者に見合った期間となる。
緩衝剤を添加してpHを調整したり、別の溶質を添加して等張性を調整する。一般に、pHは約3から7.5とし得る。組成物は、年齢、性別、体重、患者の状態、投与される組成物の形態(例えば固体か液体か)などの要素を考慮した投与量で、および医学および獣医学当業者には周知の技法で、投与され得る。ヒトその他の動物の投与量は、本開示、本明細書引例、技術分野の知識から、過度の実験をせずとも、当業者によって決定され得る。

本発明の一実施態様においては、上記培地は、心臓の損傷または梗塞領域での心筋再生または新しい心筋生成のために投与される幹細胞の培養および増殖に使用され得る。
本明細書において、「患者」は、例えば、但し限定的ではないが、ヒト、哺乳動物、爬虫類、両生類、魚類を含む任意の脊髄動物を包含する。しかしながら、患者は、ヒトまたは哺乳動物、例えばイヌ、ネコ、馬といった飼育動物や、ウシ、ヒツジ、ブタといった生産動物であるのが有利である。
本明細書において、「成体」幹細胞は、その起源が胎児ではなく、胎児または胎生組織に由来しない幹細胞を刺す。
本明細書において、「ホーミング」とは、体細胞性幹細胞の、損傷した心筋および/または心筋細胞への誘引および移動を指す。
本発明の別の態様においては、幹細胞はLineage−negativeであるよう選択される。「Lineage−negative」なる用語は当業者には周知であり、細胞が特定の細胞系に特徴的な抗原を発現しないことを意味する。
幹細胞は、注射、特に心筋内注射によって投与されるのが有利である。当業者には承知されるように、心臓は機能筋であるので、これは好ましい幹細胞送達法である。幹細胞を心臓に注射することにより、心臓の収縮運動によって幹細胞が失われないことが保証される。
本発明の別の実施態様は、梗塞領域に移動し、筋細胞、平滑筋細胞、および内皮細胞に分化する幹細胞に関わる。これらの細胞種は、構造的および機能的完全性を回復するために存在せねばならないことが業界において知られている。
本発明の更に別の実施態様は、常在心臓幹細胞の刺激、移動、増殖、および/または分化を含む。
本発明に使用される幹細胞はLineage−negativeであるよう選択することが有利である。「Lineage−negative」なる用語は当業者には周知であり、細胞が特定の細胞系に特徴的な抗原を発現しないことを意味する。Lineage−negative幹細胞はc−kit陽性であるよう選択されるのが有利である。「c−kit」なる用語は当業者には周知であり、幹細胞の表面に存在すること知られおり、幹細胞を同定し、周辺細胞から分離する過程で常用されているレセプターである。
幹細胞は、注射、特に心筋内注射によって投与されるのが有利である。当業者には承知されるように、心臓は機能筋であるので、これは好ましい幹細胞送達法である。幹細胞を心臓に注射することにより、心臓の収縮運動によって幹細胞が失われないことが保証される。
本発明の好ましい一実施態様は、経心内膜注射液を送達するためにカテーテル方式の使用を含む。カテーテルの使用により、胸腔の切開を必要とする他のより侵襲的な方法が排除される。当業者には承知されるように、本明細書に概略を示すカテーテル方式を含む侵襲がより少ない方法によって、最適回復時間が可能となる。
本発明の別の態様は、治療有効用量のサイトカインの投与を含む。有効用量は、有益なまたは所望の臨床結果をもたらすのに十分な量である。前記用量は一回以上の投与で投与される。好ましい一実施態様においては、用量は、治療開始後約2、3日の間に与えられる。しかしながら、有効用量の正確な決定は、体格、年齢、梗塞サイズ、投与されるサイトカインまたはサイトカインの組合せ、損傷からの経過時間など、各患者に固有の要因に基づいて行われる。当業者、特に内科医または心臓内科医は、過度の実験をせずとも、特に本開示および技術分野の知識から、有効用量を構成するサイトカインの十分な量を決定し得るであろう。
心臓組織の機能的および構造的完全性の回復または一部回復−有利には既存状態を超える−は本発明の別の態様である。
本発明は更に、本発明およびその多数の利点のより良い理解を提供する以下の非限定的な実施例によって説明される。
幹細胞因子はSCF(組換えヒト、組換えマウス、およびこれらに対する抗体の複数形態)の名称でR&D Systems(614 McKinley Place N.E.,Minneapolis, MN 55413)から入手可能であり;

A.造血幹細胞の採取
強化緑色蛍光タンパク質(EGFP)を発現する雄トランスジェニックマウスの大腿骨および脛骨から骨髄を採取した。大腿骨および脛骨を摘出した後、筋肉を切除し、骨の上面および下面を切開し、回収用緩衝液を骨髄に浸潤させた。緩衝液と細胞を含む液体を、1.5mLエッペンドルフチューブなどの試験管に回収した。骨髄細胞を5%ウシ胎児血清(FCS)を含むPBS中に懸濁させ、以下の造血細胞系に特異的な抗マウスモノクローナル抗体と一緒に氷上でインキュベートした:CD4およびCD8(Tリンパ球)、B−220(Bリンパ球)、Mac−1(マクロファージ)、GR−1(顆粒球)(Caltag Laboratories)およびTER−119(赤血球)(Pharmingen)。次いで細胞をPBS中で洗浄し、ヤギ抗ラット免疫グロブリン(Polyscience Inc.)で被覆した磁気ビーズと一緒に30分間インキュベートした。バイオ磁石によって細胞系陽性(lineage positive)細胞(Lin+)を除去し、Lineage−negative細胞(Lin−)をACK−4ビオチン(抗c−kitmAb)で染色した。細胞をPBS中で洗浄し、ストレプトアビジン結合フィコエリトリン(SA−PE)(Caltag Labs.)で染色し、蛍光活性化細胞分類(FACS)をFACSVantage器(Becton Dickinson)で実施して分類した。EGFPおよびACK−4−ビオチン−SA−EPの励起が波長488nmで生じた。Lin−細胞を、染色強度の対数差1〜2によってc−kit陽性(c−kitPOS)およびc−kit陰性(c−kitNEG)に分類した(図1)。c−kitPOS細胞をPBS5μL中2×104から1×105個の濃度で懸濁させ、c−kitNEG細胞をPBS5μL中1×105個の濃度で懸濁させた。
2月齢の雌C57BL/6マウスにおいて、Liら(1997)によって記載されているように心筋梗塞を誘発した。梗塞の3〜5時間後、マウスの胸腔を再度開き、梗塞に隣接する生存心筋の前面と後面にLin−c−kitPOS細胞を含むPBS2.5μLを注射した(図2)。未注射またはLin−c−kitNEG細胞を注射した梗塞マウス、および疑似手術したマウス、即ち開胸したが梗塞を誘発しなかったマウスを対照として使用した。全動物を手術の9±2日後に解剖した。プロトコルは施設内治験審査委員会によって承認されたものであった。結果は平均±SDで表される。2つの測定値間の有意性はスチューデントのt検定によって決定し、複数比較において、ボンフェローニ法(Scholzen and Gerdes,2000)によって評価した。P<0.05を有意とみなした。
マウスを抱水クロラール(400mg/kg体重、i.p.)で麻酔し、マイクロチップ血圧トランスデューサ(モデルSPR−671、Millar)を右頚動脈にカニューレ挿入し、非開胸式処置において左心室(LV)血圧およびLV+およびdP/dtを測定し、HSC移植に由来する発生筋細胞が機能に影響するか判定した。未注射またはLin−c−kitNEG細胞を注射した梗塞マウスを統計上まとめた。疑似手術群と比較し、梗塞群は心障害の徴候を示した(図3)。Lin−c−kitPOS細胞で処置したマウスにおいては、LV拡張終期圧(LVEDP)は36%低下し、発生圧力(LVDP)、LV+およびdp/dtはそれぞれ32%、40%、および41%上昇した(図4)。
腹大動脈にカニューレを挿入し、塩化カドミウム(CdCl2)を注射して心臓拡張期に心臓を停止させ、10%緩衝ホルマリンによって心筋を逆潅流した。左心室の基部から頂部に至る3種の組織片をヘマトキシリンおよびエオシンで染色した。冠状動脈閉塞の9±2日後に、心室の梗塞部位は肉眼でも組織検査でも容易に同定できた(図2A)。梗塞領域を定める心内膜面および心外膜面の長さ、左心室全体の心内膜および心外膜を各断片で測定した。次いで、それらの比を計算して、各ケースにおける平均梗塞サイズを得た。これは、顕微鏡に接続された画像分析装置を使用して倍率4倍で行った。梗塞領域を定める心内膜周長と心外膜周長の比(Pfeffer and Braunwald,1990;Li et al.,1997)は、未処置マウス(78±18%、n=8)と、Lin−c−kitPOS(75±14%、n=12)またはLin−c−kitNEG細胞(75±15%、n=11)で処置した処置済マウスとで変わらなかった。
蛍光in situハイブリダイゼーション(FISH)アッセイのため、70%ホルムアルデヒドを含む変性溶液に断片を暴露した。エタノールで脱水した後、断片をDNAプローブCEP Y(サテライトIII)スペクトル緑(Vysis)を用いて3時間ハイブリダイズした。核をPIで染色した。
心室の残存部分由来の細胞中にはY染色体は検出されなかった。しかしながら、新たに形成された筋細胞中にはY染色体が検出され、これは、それらの起源が注入骨髄細胞に由来するものであることを示す(図9)。
ウサギポリクローナル抗MEF2(C−21;Santa Cruz)、ウサギポリクローナル抗GATA−4(H−112;Santa Cruz)、ウサギポリクローナル抗Csx/Nkx2.5(Dr.Izumoから入手)、およびウサギポリクローナル抗コネキシン43(Sigma)と一緒に断片をインキュベートした。FITC結合ヤギ抗ウサギIgG(Sigma)を二次抗体として使用した。
新たに形成された筋細胞が、機能的コンピテンスを図る成熟細胞となったことを確認するため、筋細胞エンハンサ因子2(MEF2)、心特異的転写因子GATA−4,早期筋細胞発生マーカーCsx/Nkx2.5の発現を調査した。心臓において、MEF2タンパク質はGATA−4によって用いられ、ミオシン軽鎖、トロポニンT、α−ミオシン重鎖、心房性ナトリウム利尿因子、およびα−アクチンといった幾つかの心遺伝子のプロモータを相乗的に活性化する(Durocher et al.,1997;Morinet al.,2000)。Csx/Nkx2.5は、筋細胞分化の初期段階に限定される転写因子である(Durocher et al.,1997)。再構成心臓において、心筋ミオシン標識細胞の全ての核がMEF2(図7A〜F)およびGATA−4(図10)を発現したが、40±9%のみがCsx/Nkx2.5を発現した(図7G〜I)。これら筋細胞の性質を更に特性化するため、コネキシン43の発現を判定した。このタンパク質は、筋細胞間に原形質膜を生成することで細胞間接続および電気的カップリングを担う(Beardsle et al.,1998;Musil et al.,2000)。細胞の細胞質中および密に整列する分化細胞の表面でコネキシン43が明らかであった(図11A〜D)。これらの結果は、心筋表現型の期待された機能コンピテンスと一致した。更に、種々の成熟段階にある筋細胞が、同じバンドおよび異なるバンドで検出された(図12)。
A.心筋梗塞とサイトカイン
15匹の2月齢雄C57BL/6マウスを脾摘し、2週間後に、組換えラット幹細胞因子(SCF)(200μg/kg/日)および組換えヒト顆粒コロニー刺激因子(G−CSF)(50μg/kg/日)(Amgen)を1日1回、5日間にわたり皮下注射した(Bodine et al.,1994;Orlic et al.,1993)。エーテル麻酔下、左心室(LV)を露出させ、冠状動脈を結紮した(Orlic et al.,2001;Li et al.,1997;Li et al.,1999)。SCFおよびG−CSFを更に3日間与えた。脾摘し梗塞を惹起し生理食塩水を注入して疑似手術したマウス(SO)を対照とした。50mg/kg体重のBrdUを1日1回、13日間にわたり与えてから、解剖した。マウスは27日後に解剖した。プロトコルはニューヨーク医科大学によって承認されたものであった。結果は平均±SDで示される。有意性はスチューデントのt検定およびボンフェローニ法(Li et al.,1999)で判定した。対数順位検定によって致死率を計算した。P<0.05を有意とした。
13−MHz線形トランスデューサ(15L8)を備えたSequoia256c(Acuson)を用いて覚醒マウスにおいて心エコー検査を実施した。前胸部を剪毛し、乳頭筋の位置で傍胸骨短軸から二次元(2D)画像およびMモード追跡を記録した。Mモード追跡から、心臓拡張期および収縮期における解剖学的パラメータを得た(Pollick et al.,1995)。2D短軸視像におけるLV断面積から駆出率(EF(ejection fraction))を導いた(Pollick et al.,1995):EF=[(LVDA−LVSA)/LVDA]*100〔式中、LVDAおよびLVSAは心臓拡張期および収縮期におけるLV面積である〕。マウスを抱水クロラール(400mg/kg体重、ip)で麻酔し、チャート式記録計に接続されたマイクロチップ圧力トランデューサ(SPR−671、Millar)をLVに侵入させ、非開胸処置において血圧と、+およびdP/dtを評価した(Orlic et al.,2001;Li et al.,1997;Li et al.,1999)。
血行動態測定に続き、腹大動脈にカニューレを挿入し、CdCl2で心臓を心臓拡張で停止させ、10%ホルマリンで心筋を潅流した。LV腔(chamber)に、in vivoで測定した拡張終期圧に等しい圧力で固定剤を充填した(Li et al.,1997;Li et al.,1999)。LV腔内軸を測定し、基部、中間部および頂部から得た3種の横断薄片をパラフィンに包埋した。中間部を用いてLV厚、腔径、および容積を測定した(Li et al.,1997;Li et al.,1999)。LVFWから失われた筋細胞の数によって梗塞サイズを決定した(Olivetti et al.,1991;Beltrami et al.,1994)。
3種の断片の各々において、回復組織が占める面積および断片厚を測定することにより、再生心筋の容積を決定した。これら2つの変数の積から、各断片における組織修復量を出した。3種の断片の値を加算し、形成された心筋の総容積を得た。更に、心臓ごとに400個の心筋の容積を測定した。断片をデスミンおよびラミニン抗体並びにヨウ化プロピジウム(PI)で染色した。核を中心に有し、縦方向を向いた細胞のみを含めた。各筋細胞の、核を通る長さと直径を収集し、円筒形と仮定して細胞容積を計算した(Olivetti et al.,1991;Beltrami et al.,1994)。筋細胞を分類し、筋細胞クラス総容積と平均細胞容積の比から、各クラスの筋細胞数を計算した(Kajstura et al.,1995;Reiss et al.,1996)。心筋単位面積当たりの細動脈および毛細血管プロファイルの数を前と同様に測定した(Olivetti et al.,1991;Beltrami et al.,1994)。
細胞質マーカーおよび核マーカーを使用した。筋細胞核:ウサギポリクローナルCsx/Nkx2.5、MEF2、およびGATA4抗体(Orlic et al.,2001;Lin et al.,1997;Kasahara et al.,1998);細胞質:マウスモノクローナルネスチン(Kachinsky et al.,1995)、ウサギポリクローナルデスミン(Hermann and Aebi;1998)、心筋ミオシン、マウスモノクローナルα−筋節アクチン、およびウサギポリクローナルコネキシン43抗体(Orlic et al.,2001)。EC細胞質:マウスモノクローナルflk−1、VE−カドヘリン、およびVIII因子抗体(Orlic et al.,2001;Yamaguchi et al.,1993;Breier et al.,1996)。SMC細胞質:flk−1およびα−平滑筋アクチン抗体(Orlic et al.,2001;Couper et al.,1997)。I型およびIII型コラーゲン抗体の混合物によって瘢痕を検出した。
成体心室筋中に原始細胞集団が存在するか、およびかかる細胞が移動能を有するか判定するため、3種の主要増殖因子を化学誘引物質として使用した:肝細胞増殖因子(HGF)、幹細胞因子(SCF)、および顆粒球単球コロニー刺激因子(GM−CSF)。SCFとGM−CSFは、造血幹細胞の移行(translocate)を亢進することが示されているので選択した。HGFは造血幹細胞の移動を誘発するが、この増殖因子は、心臓発生早期において心臓細胞前駆体の有糸分裂、分化、および移動に深く関与する。これに基づき、マウス心臓から酵素作用により解離させた細胞を大きさによって分けた。心臓細胞を心臓組織から解離する方法は当業者には周知であり、過度の実験は必要ない(米国特許第6,255,292号参照。当該特許は参照によりその全部が本明細書に包含されるものとする)。直径5〜7μmで高い核対細胞質比を有する、小さな未分化細胞を含む均一な解離心臓細胞集団について、5μm細孔を含むゼラチン被覆フィルターを特徴とするBoydenマイクロチャンバにおいて移動アッセイを行った(Boyden et al.,1962,J.Exptl.Med.115:453−456)。
原始c−kitPOS細胞が老齢Fischer344ラットに存在するか判定するため、解離した心臓細胞を、c−kitレセプター抗体(ACK−4−ビオチン、抗c−kitmAb)で被覆した磁気ビーズに暴露した。分離後、これら小さな未分化細胞を10%ウシ胎児血清中で培養した。数日間で細胞は付着し、一週間で増殖を開始した。7〜10日で集密に達した。継代P2およびP4で達成された倍加時間はそれぞれ30時間および40時間であった。老齢期に達することなく、細胞はP18(第90代)まで成長した。複製能力はKi67によって立証された。P2において、88±14%の細胞が核中にKi67タンパク質を含んだ。P1とP4の間に追加の測定値を得た。細胞の40%がα−筋節アクチンまたは心筋ミオシンを、13%がデスミンを、3%がα−平滑筋アクチンを、15%がVIII因子またはCD31を、18%がネスチンを発現した。上記in vitro条件下で、細胞には、サルコメアが適正に整列した明らかな筋原線維組織は見られなかったし、自発的な収縮も認められなかった。同様に、AngII、ノルエピネフリン、イソプロテレノール、機械的伸長および電界刺激でも、収縮機能を開始することはできなかった。これに基づき、筋原性の平滑筋細胞系および内皮細胞系に関わる上記細胞がそれらの生物学的特性を永久に失ったか、或いはそれらの役割がin vivoで再建されるか評価することにした。P2における細胞のBrdU標識に続き、冠状動脈閉塞の3〜5時間後に、梗塞Fischer344ラットの損傷領域にBrdU陽性細胞を注射した。2週間後、動物を解剖し、梗塞領域の特性を調査した。核のBrdU標識と共に、長軸に沿って平行に並んだ筋原線維を含む筋細胞が認められた。更に、細動脈および毛細血管プロファイルを含む血管構造体が存在し、BrdUに対して陽性を示した。結論として、原始c−kit陽性細胞は老齢心臓に存在し、損傷を受け機能低下した心筋に移植された場合、増殖し、実質細胞および冠状血管へ分化する能力を維持している。
心臓は分裂終了臓器ではなく、生理学的には細胞分裂して死滅細胞を置換する筋細胞亜集団を含む。筋細胞増殖は病理学的過負荷に際して増強され、筋量を増大し、心臓の性能を維持する。しかしながら、複製筋細胞の起源は依然として同定されていない。故に、Fischer344ラットの心筋において幹細胞/前駆細胞の特性を持つ原始細胞を探索した。若年および老年の動物を試験して、幹細胞および分裂筋細胞のサイズ集団に加齢が影響を及ぼすか判定した。4月齢のラットにおけるc−kit陽性細胞およびMDRI陽性細胞の数はそれぞれ11±3および18±6/100mm2組織であった。27月齢のラットにおける値は35±10および42±13/100mm2組織であった。新たに生成された小筋細胞が多数同定され、それらはやはりc−kit陽性またはMDRI陽性であった。循環細胞の核内で発現されるKi67タンパク質は4月齢および27月齢においてそれぞれ筋細胞の1.3±0.3%および4.1±1.5%で検出された。6回または56回の注射後のBrdU局在化は、4月齢において1.0±0.4%および4.4±1.2%、27月齢において4.0±1.5%および16±4%であった。組織片中で測定した有糸分裂指数から、有糸分裂中の筋細胞核の画分は4月齢および27月齢においてそれぞれ82±28/106および485±98/106からなることが分かった。上記判定は、細胞有糸分裂指数を得るための解離筋細胞において確認された。この方法により、多核筋細胞の全ての核が同時に有糸分裂期にあったことを立証することが可能となった。この情報は、組織片では得られなかった。収集された値は、4月齢で95±31/106の筋細胞が、27月齢で620±98/106の筋細胞が分裂中であったことを示した。両月齢区間において、紡錘体、収縮環の形成、核膜の分解、核分裂、及細胞質分裂が明らかになった。結論として、原始未分化細胞は成体心臓に存在し、加齢に伴う増加は、細胞周期に入り核分裂および細胞質分裂を起こす筋細胞数の増加と並行する。この関係は、老化心臓における筋細胞成長のレベルおよび運命を心臓幹細胞が規制し得ることを示している。
損傷心臓の再構築において常在原始細胞が果たす重要な役割は、雌性心臓を雄性レシピエントに移植したときの臓器キメラ現象を考えるとき、十分に理解される。このために、雄性ホストに移植された雌性心臓8個を分析した。グラフトされた雌性心臓への雄性細胞の移行を、Y染色体のFISHによって同定した(実施例1E参照)。この方法により、Y染色体で標識された筋細胞、冠状細動脈、毛細血管プロファイルの比率はそれぞれ9%、14%、および7%であった。同時に、移植された雌性心臓中の未分化c−kit陽性細胞および多剤耐性−1(MDR1)陽性細胞の数を測定した。更に、これらの細胞がY染色体を含む可能性を確認した。心臓移植では、レシピエントの心房の一部を保存し、そこにドナー心臓をその心房の一部で取付ける。この外科処置は、ホストおよびドナーに由来する心房が、移植心臓の複雑な再構築過程に貢献する未分化細胞を含むか理解するために重要である。定量的には、c−kit標識細胞およびMDR1標識細胞の値は対照非移植心臓では極めて低く、左心室心筋100mm2当たりc−kitが3、MDR1が5であった。反対に、レシピエントの心房におけるc−kit細胞およびMDR1細胞の数は15および42/100mm2であった。ドナーの心房における対応する値は15および52/100mm2であり、心室においては11および21/100mm2であった。移植は、心臓における原始未分化細胞の著しい増加を特徴とした。ホストの心房中の幹細胞はY染色体を含み、ドナーの心房および心室においては平均55%および63%のc−kit細胞およびMDR1細胞がY染色体を呈した。c−kit陽性細胞およびMDR1陽性細胞の全てがCD45に対して陰性を示した。これらの知見は、移植心臓への雄性細胞の移行はドナー心筋の再構成に大きな影響を及ぼすことを示している。結論として、幹細胞は成体心臓に広範に分布しており、それらの柔軟性および移動性のために、高度の分化によって筋細胞、冠状細動脈および毛細血管構造体を生成する。
正常心臓においては筋細胞のターンオーバーが起こり、心筋損傷は、筋細胞増殖および血管成長の活性化をもたらす。このような適応性は、多能性原始細胞が心臓に存在し、死滅筋細胞の生理学的置換や、損傷後の細胞成長応答に関与する可能性を惹起する。これに基づき、正常マウス心臓における未分化細胞の存在を、幹細胞因子のレセプターであるc−kit、細胞色素、毒性物質、薬物を排出する能力のあるP−糖タンパク質である多剤耐性−1(MDR1)、および細胞シグナリングおよび細胞付着に関与する幹細胞抗原−1(Sca−1)を含む表面マーカーを用いて判定した。左右心房と、心室の基部、中間部、および頂部からなる4つの別個の領域を分析した。c−kit陽性細胞数は、高い方から順に、心房、心室の頂部、基部、中間部においてそれぞれ26±11、15±5、10±7、6±3/100mm2であった。基部および中間部と比較すると、心房と頂部おけるc−kit陽性細胞の大きな画分は統計的に有意である。MDR1陽性細胞数はc−kitを発現するものより大きかったが、同様の局在化パターンを示し、心房、頂部、基部、および中間部において43±14、29±16、14±7、および12±10/100mm2であった。やはり、心房および頂部における値が他の2つの部位よりも高かった。Sca−1標識細胞は最も高い値を示し、心房において150±36/100mm2個の陽性細胞が認められた。c−kit、MDR1、およびSca−1に対して陽性の細胞は、CD45に対して、並びに、筋細胞、内皮細胞、平滑筋細胞、および線維芽細胞の細胞質タンパク質に対して陰性を示した。更に、c−kitおよびMDR1の両方に対して陽性の細胞数を測定し、2種の幹細胞マーカーを有する細胞を確認した。心臓全体で、36%のc−kit標識細胞がMDR1を発現し、19%のMDR1細胞がc−kitも有した。結論として、幹細胞はマウス心臓全体に分布しているが、心房および頂部といった低ストレスの領域に蓄積する傾向にある。
移動、侵入、および発現アッセイ
HGFレポーターであるc−Metは、造血幹細胞および肝幹細胞において同定されているが(126,90)、最も重要には星状骨格筋細胞(92)および胎児心筋細胞(127)において同定されている。この知見により、著者は、c−MetがCSC中に存在するか、そのリガンドHGFがかかる未分化細胞に生物学的影響を及ぼすか判定することにした。HGFはin vitroでCSCの移動および侵入を亢進し、in vivoで貯蔵領域から梗塞心筋部位への移行を助成するという仮説を設けた。HGFは、マトリックスメタロプロテイナーゼ−2(94、95)の発現と活性化を通して細胞移動に影響する(128)。この酵素族は細胞外マトリックス中の障壁を破壊することができ、CSCの移動、ホーミング、および組織回復を容易にする。
本試験の最初の部分で、移動および侵入アッセイを実施し、走化性HGFの存在下でのc−kitPOS細胞およびMDR1POS細胞の可動性を確認した。
マウスにおいて心筋梗塞を惹起し、5時間後、HGFおよびIGF−1を含む液を、心房から境界域にかけて4箇所に注射した。HGFは漸増濃度で投与し、貯蔵CSCと死滅組織間で走化性勾配を作出した。このプロトコルは、損傷領域へのCSCのホーミングを増進し、新しい心筋を生成するよう導入された。もしそうであったならば、動物死に関連する大規模梗塞は速やかに縮小し、梗塞サイズの限界および生存が本発明によって拡大される。
A.細胞の回収とクローニング
20〜25月齢雌Fischerラットから心臓細胞を単離した(111、112)。無傷の細胞を分離し、筋細胞を除去した。小細胞を懸濁させ、凝集体をろ過器で除去した。細胞を、膜の外面に局在するN端エピトープを認識するウサギc−kit抗体(H−300、Santa Cruz)と一緒にインキュベートした。細胞を、抗ウサギIgG(Dynal)およびc−kitPOS細胞で被覆した磁気ビーズに暴露し、磁石を使って回収した(n=13)。FACS(n=4)については、細胞をr−フィコエリトリン結合ラットモノクローナル抗c−kit(Pharmingen)で染色した。両方法において、c−kitPOS細胞は小細胞集団の6〜9%の範囲とした。
BrdU標識細胞(P2;陽性細胞=88±6%)を移植した。2月齢の雌Fischerラットに心筋梗塞を惹起した(111)。5時間後、22匹のラットに2×105個の細胞を、梗塞の両側の隣接領域に注射した。10日後に12匹を解剖し、20日後に10匹を解剖した。各期間に、8〜9匹の梗塞ラットと10匹の疑似手術ラットに生理食塩水を注射し、5匹にLin−c−kitNEG細胞を注射し、対照とした。ケタミン麻酔下、20日後に解剖したラットには、9日後と19日後に心エコー検査を実施した。Mモード追跡から、LV拡張終末期の直径と壁厚を得た。駆出率を計算した(87)。10日後と20日後に動物を麻酔し、非開胸処置においてLV圧、+およびdP/dtを評価した(111)。致死率は低かったが、術後10日目および20日目において、未処置ラットよりも処置ラットで統計的な有意性はなく、全群合わせて平均35%であった。プロトコルは施設内治験審査委員会によって承認されたものであった。
心臓を拡張期に停止させ、ホリマリンで固定した。左心室から失われた筋細胞画分によって(87)、梗塞サイズは、10日後には処置ラットおよび未処置ラットにおいて53±7%および49±10%(NS)、20日後には処置ラットおよび未処置ラットにおいて70±9%および55±10%(P<0.001)とそれぞれ決定された(87)。各心臓において400個の新たな筋細胞の容積を測定した。断片をデスミンおよびラミニンおよびPIで染色した。核が中央に位置し縦方向に向いた筋細胞において、核を通る細胞長および細胞径を収集して細胞容積を計算した(87)。
覚醒イヌにおける局部的および全体的心機能を向上する梗塞心筋の修復
下記のごとき例外はあるが、上述の非限定的な実施例の方法を使用した。
げっ歯類における幹細胞のホーミングおよび分化による梗塞後の心筋再生は、大動物において同様の心修復が起こるかの疑問には答えていない。更に、新たな心筋は、梗塞部分の機能異常に作用して収縮を回復するのかも未知である。この目的で、血行動態および局部的壁機能の測定のため、イヌに慢性的に計器を取り付けた。拍出量およびEFも測定した。左前下行冠状動脈の付近で水圧オクルダーを膨張させることにより心筋梗塞を誘導した。4時間後、HGFおよびIGF−1を境界域に注射し、幹細胞を可動化および活性化した。その後、イヌを最長30日間モニタした。増殖因子により慢性心修復が惹起され、梗塞の膨張を後進させた。セグメント短縮は−2.0±0.7%から+5.5±2.2%に、拍出力は−18±11から+53±10mm×mmHgに、拍出量は22±2から45±4mLに、駆出率は39±3から64±4%に上昇した。梗塞の8時間後の処置イヌにおいては、原始細胞数がベースラインの240±40c−kit陽性細胞から1700±400(遠隔心筋)、4400±1200(境界域)、および3100±900c−kit陽性細胞/100mm2(梗塞領域)に増加した。Ki67標識は、遠隔、境界、および梗塞心筋においてそれぞれ48%、46%、および26%のc−kit陽性細胞において検出された。即ち、これらの細胞は高度に複製している。このような効果は、梗塞未処置イヌにおいては実質的になかった。急性実験を、冠状動脈閉塞の10〜30日後に移植結晶によって規定される梗塞心筋の定量分析で補足した。新たな心筋における奇異性運動から正常収縮への変化は、大きさが400から16,000、平均容積2,000±640μm3の筋細胞の生成によって特性化された。BrdU標識された内皮および平滑筋細胞を伴う抵抗血管は87±48/mm2組織であった。毛細血管は細動脈の2〜3倍であった。合わせて、梗塞の16±9%が健康な心筋によって置換された。即ち、イヌ科常在原始細胞は、貯蔵部位から可動化され、死滅心筋に到達し得る。幹細胞の活性化および分化により梗塞心臓の修復が亢進され、局地的な壁運動および全身的な血行動態が向上する。
下記のごとき例外はあるが、上述の非限定的な実施例の方法を使用した。
心筋梗塞(MI)の主要悪化因子の2つは筋量損失と腔拡張であり、いずれもネガティブな左心室(LV)再構築と心性能の低下の原因となる。これらのMIの悪影響を阻止しようと、常在心臓幹細胞(CSC)を可動化および活性化して組織再生を亢進し、AT1レセプター遮断剤ロサルタン(Los)を20mg/kg体重/日の用量で投与して細胞肥大を緩和し、それによって腔容積の増加を抑制した。これに基づき、マウスにおいてMIを惹起し、動物を4群に再分類した:1.疑似手術(SO);2.MIのみ;3.MI−Los;4.Mi−Los−CSC。MIの一ヵ月後、動物を解剖し、LV機能、梗塞寸法、心再構築を評価した。CSC処置マウスにおいては心筋再生も測定した。LVが失った筋細胞の数に基づく梗塞サイズはMIで47%、MI−Losで51%、Mi−Los−CSCで53%であった。MIおよびMI−Losと比較し、LosおよびCSCで処置したMIでは、損傷心臓の結果は次の点において好ましいものであった;腔径:MIに対して−17%およびMI−Losに対して−12%;長軸:MIに対して−26%(p<0.001)およびMI−Losに対して−8%(p<0.002);および腔容積:MIに対して−40%(p<0.01)およびMI−Losに対して−35%(p<0.04)。LV量対腔容積の比は、MIおよびMI−LosよりもMI−Los−CSCにおいてそれぞれ47%(p<0.01)および56%(p<0.01)高かった。MI−Los−CSCにおける組織修復は10×106個、900μm3の新たな筋細胞で構成された。更に、このマウス群では心筋1mm2当たり70の細動脈と200の毛細血管が存在した。9mm3の新たな心筋が生成され、MIサイズは22%減少し、LVの53%から41%となった。心エコー検査によれば、MI−Los−CSCマウスの壁の梗塞領域において収縮機能が再現された。血流動態検査によれば、MI−Los−CSCマウスはLVEDPが低く、+およびdP/dtが高かった。結論として、心室再構築に及ぼすロサルタンの好影響は、梗塞領域へのCSC移行に媒介される心修復過程によって増強される。可動化したCSCは梗塞サイズおよび心室拡張を低減し、それによって梗塞心臓の収縮作用を更に改善する。
下記のごとき例外はあるが、上述の非限定的な実施例の方法を使用した。
予備試験において、著者らは、c−kitまたはMDR−1に対して陽性を示すCSCが表面レセプターc−metを発現することを記録できた。c−metはHGFのレセプターであり、リガンド結合がマトリックスメタロプロテイナーゼの合成を介して細胞運動性を亢進した。しかしながら、c−met活性化がCSCの生態および機能に更なる影響があるかは知られていない。この目的で、NSCMにおいて50ng/mLのHGFに暴露されたCSC上のc−metが、増殖因子に細胞内内在化および移行によって応答するか試験した。驚くべきことに、原始的特性を維持した被刺激細胞においてc−metの核内局在化が共焦点顕微鏡検査によって検出された。HGFがc−metに及ぼすこの異例の影響により、可動化されたレセプターが他の核内タンパク質と反応してCSCの細胞成長および分化に関与し得る可能性が生じた。細胞系の関与における心特異的転写因子GATA−4の重要な役割の故に、免疫沈降およびウエスタンブロット法により、c−metおよびGATA−4によって作成されたタンパク質複合体を同定した。単一HGF刺激に続く時間依存性分析により、c−met−GATA−4複合体は15分から3日目まで漸増したことが分かった。時間は、筋細胞その他の心臓細胞への原始細胞の分化と対をなした。GATA−4とc−metのDNAレベルでの分子相互作用を確認するため、HGFで刺激した細胞から単離した核抽出物にゲルシフト法を1時間実施した。GATA配列を含むプローブを用い、シフトしたバンドが得られた。しかしながら、GATA−4抗体の追加により、極度にシフトしたバンドが得られた。反対に、c−met抗体の包含により、GATAバンドの光学密度は減衰した。TATAボックス上流のGATA配列はc−metプロモーターにおいて同定されたので、第二移動性シフトアッセイを実施した。今度は、HGF刺激細胞由来の核抽出物は、c−met抗体によって軽減されたバンドシフトをもたらした。反対に、GATA−4抗体は極度にシフトしたバンドを誘発した。即ち、核レベルでのHGF介在c−met移行は、c−metに転写因子機能を授与することであり得、更に試験すれば、このDNA結合がGATA−4発現を増強し、未成熟心臓細胞の分化をもたらすか示されるであろう。
作業室内無菌下で心筋組織(平均重量1g以下)を採取した。
425〜450mLのDMEM/F12(Cambrex 12−719F)、5〜10%の患者血清(心耳組織と共に得られた、100〜150mLの患者血液に由来する50〜75mLの血清)、20ng/mLのヒト組換えbFGF(Peprotech 100−18B)、20ng/mLのヒト組換えEGF(Sigma E9644)、5μg/mLのインシュリン(RayBiotech IP−01−270)、5μg/mLのトランスフェリン(RayBiotech IP−03−363)、5ng/mLの亜セレン酸ナトリウム(Sigma S5261)、1.22mg/mLのウリジン(Sigma U−3003)、および1.34mg/mLのイノシン(Sigma I−1024)を使って成長培地を調製した。
低温保存バイアルを、−70℃から−80℃に予冷したナルゲン容器内で凍結させ、−70℃から−80℃で最低3日間保存した。
組織播種から1〜2週間後、CSCの成長は明らかであった。細胞増殖の全期間にわたり、週2回、成長培地を交換した。培地は4℃で保存し、使用前に37℃で加温した。合計8mLの培地を100mmペトリ皿に使用した。培養片、即ち細胞によって生成された調整培地を保存するため、一回に培地を6mLだけ取出し、6mLの新鮮な培地を追加した。
集密前に、成長培地を除去し、一皿当たり4mLのトリプシン(0.25%)[Carnbrexカタログ番号10170;無視し得る量の内毒素]を用い5〜7分かけて細胞を分離させた。6mLの血清含有培地を用いて反応を停止させた。
上述したように心臓手術を受けた患者51人から同意のもとに不要な心筋標本を入手した。試料を細断し、肝細胞増殖因子およびインシュリン様増殖因子−1をそれぞれ200ng/mLおよび200ng/mLの濃度で補充した培地を含むコーティングしていないペトリ皿の表面に播種した。29ケースで細胞の成長に成功した。このサブセットにおいて、播種の〜4日後に細胞成長が明らかとなり、〜2週間後、クラスターをなす〜5,000から7,000個の細胞が各組織片を包囲した(図70A〜C)。組織由来の細胞成長を免疫ビーズを用いてc−kitに対して分類し、培養した(Beltrami,2003;Linke,2005)。上述のごとく細胞表現型をFACSおよび免疫細胞化学法によって定義した(Beltrami,2003;Orlic,2001,Urbanek,2005)。分類したc−kitPOS細胞を固定し、心臓、骨格筋、神経、造血の各細胞系のマーカーに対して試験し(下記表1)、Lineage−negative(Lin−)−hCSCを検出した(Beltrami,2003;Linke,2005;Urbanek,2005)。P0における心筋試料由来の細胞成長画分は細胞抗原c−kit、MDR1、およびSca−1−様を発現した(図70D〜F)、これらはそれぞれ細胞集団全体の1.8±1.7、0.5±0.7、1.3±1.9%を構成した。これらの細胞は、CD133、CD34、CD45、CD45RO、CD8、CD20、およびグリコホリンAを含む造血細胞マーカーに対して陰性を示した(下記表1)。
ヒトc−kitPOS細胞をP0で分類し、顕微鏡制御下で、個々のc−kitPOS細胞をテラサキプレートの各ウェルに0.25〜0.5細胞/ウェルの密度で播種した(図71H)(Beltrami,2003;Linke,2005)。1つ以上の細胞を含むウェルは除外した。定着すべきc−kitPOS細胞の50±10%はLin−であった。BrdU(10μM)を1日3回、5日間にわたり添加した(Beltrami,2003;Linke,2005)。〜3乃至4週間後、6,700個の単一播種細胞から53個の小クローンが産生された。即ち、c−kitPOS−hCSCは0.8%クローニング効率を有した。クローン中の細胞数は200から1,000の範囲で変動した(図71I)。53個のクローンのうち12個は更に成長しなかった。残りの41個のクローンは増殖し、免疫細胞化学法によって特性分析した。倍増時間は29±10時間であり、5日後の細胞の90±7%がBrdUPOSであった。デキサメタゾンを使用して分化を誘発し(Beltrami,2003;Linke,2005)、その結果、筋細胞、SMC、およびECを含む心臓細胞系が検出された(図71J)。筋細胞が優勢細胞集団であり、ECおよびSMCがそれに続いた(図75)。
標準免疫抑制法(Zimmermann,2002)で処置した雌の免疫不全Scidマウス(Urbanek,2005)およびFischer344ラット(Belrami,2003)において麻酔下で心筋梗塞を惹起した。上述のごとく心臓手術をした8人の患者の心筋試料(〜3標本/患者)からc−kitPOS細胞を単離し、増殖した。かかる試験において、各試料から〜200,000個のc−kitPOS細胞を入手する際に、c−kitPOS細胞をP0において回収した。このプロトコルには〜7週間を要した。冠状動脈閉塞後まもなく、〜40,000個のヒトc−kitPOS細胞を境界域の両側の2箇所に注射した(Beltrami,2003;Orlic,2001;Lanza,2004)。動物をBrdUに暴露し、梗塞および細胞移植の2〜3週間後に解剖した(Beltrami,2003;Orlic,2001;Urbanek,2005;Lanza,2004)。心エコー検査を実施し、その2〜3日後に、左心室(LV)圧とdP/dtを測定した(Beltrami,2003;Orlic,2001;Urbanek,2005;Lanza,2004)。心臓を拡張期に停止させ、ホルマリンで潅流して固定した。各心臓で、梗塞サイズ、ヒト筋細胞、細動脈、および毛細血管の形成を判定した(Anversa,2002)。
ヒト特異的Alu反復配列に対するFITC標識プローブ(Boigenex)を用いたin situハイブリダイゼーションによってヒト細胞を検出した(Just,2003)。更に、ヒトX染色体と、マウスおよびラットX染色体を同定した(Quaini,2002)。ヒト細胞で処置したラットの生存LVおよび梗塞LVの組織片からDNAを抽出した。ヒトAlu(長さ約300塩基対であり、霊長類ゲノムに特異的に認められ、ヒトゲノムの10%以上に存在し、ヒトでは平均距離4kbで位置する)、並びにラットおよびヒトミオシン軽鎖2v配列に対してPCRを実施した(下記表3参照)。
ヒトSNCおよびECによってのみ構成される冠状細動脈および毛細血管によってヒトc−kitPOS細胞の注入によって媒介される血管形成を明らかにした(図73A〜F)。マウスまたはラット冠状脈管系においてヒトSMCおよびECの目視可能な統合はなかった。ヒト細胞および非ヒト細胞によって形成された血管が見つかったケースはなかった。ヒト細動脈および毛細血管の数はラットおよびマウスに匹敵し、いずれの場合も、筋細胞8個当たり1つの毛細血管が存在した(図73G)。更に、酸素についての拡散距離は平均18μmであった。これらの毛細血管パラメーターは胎児期後半およびヒト新生児心臓におけるものと同様である(Anversa,2002)。
再生されたヒト心筋が機能的にコンピテントであり、梗塞心臓の機能を部分的に回復させるか判定するため、貫壁性梗塞および新規形成ヒト心筋の有無の組織学的資料作成に続き、心エコー図を遡及的に調査した(図74A〜C;図78)。心筋再生を、壁の梗塞域における、検出可能な収縮機能に関連付けた。これは、組織再構成がなければ決して起こらなかった。ヒト心筋の形成により、梗塞心室の駆出率が増加した(図74D)。更に、心筋再生により、腔拡張が緩和され、LV質量対腔容積の比が増加し(図74E)、梗塞後のLVEDPの上昇並びにLVDPおよび正負dP/dtの低下を制限することにより心室機能全体を向上させた(図74F)。
脈管系閉塞に及ぼす、クローン原性EGFPPOS−c−kitPOS−CSC(非活性化CSC)およびHGFおよびIGF−1によって活性化されたEGFPPOS−c−kitPOS−CSC(活性化CSC)の注入の効果を比較するため、Fischer344ラットの左冠状動脈を標準方法で閉塞した。非活性化CSCまたは活性化CSC(活性化は移植の2時間前に行なった)を閉塞した左冠状動脈の近辺に移植した。結紮部の解剖学的位置のため、細胞移植部位は、結紮により発生した心室壁の梗塞領域からは離れていた(図82)。心筋に播種された非活性化CSCのアポトーシス率は高く、送達の12および24時間後から48時間後まで漸増した(図83)。細胞死により、1〜2週間で移植細胞は完全に消失した。反対に、増殖因子で活性化したCSCの移植では著しい効能が検出された(図79a)。活性化CSCは心筋にホーミングし、そこでアポトーシスが急激に細胞複製に勝ったが、のちに細胞分裂が細胞死を上回った(図79b−d)。
2つの目的で15頭のブタを開胸した:1)心房付属組織を切除および採取すること、および2)左前下行遠位冠状動脈を90分間閉塞して心筋梗塞を誘導し、次いで潅流すること。心房付属物からCSCを採取し、上述のごとくex vivoで培養・増殖し、2〜3ヵ月(平均86日)後同じブタに冠状動脈内注射した。7頭のブタが冠状動脈内CSC注射を受け、8頭がビヒクル注射を受けた。全てのブタに、一連の心臓マーカー試験、2D心エコー検査、および(亜群において)侵襲的血行動態モニタリング、内臓の詳細組織病理学検査を実施した。心臓または組織病理学的検査した種々の臓器においてCSC処置に関連する悪影響の徴候はなく、処置されたブタは、心機能改善の傾向を示した。かかる結果から、この虚血性心筋症大動物モデルにおけるCSCの冠状動脈内送達の安全性および実現可能性が確認された。

Claims (16)
- 患者において大冠状血管又は動脈を形成する医薬の製造ための単離された心臓幹細胞の使用であって、前記心臓幹細胞は、患者に投与する前に、肝細胞増殖因子(HGF)および/またはインシュリン様増殖因子−1(IGF−1)に暴露することで活性化される、使用。
- 前記患者が、血管障害を有する請求項1に記載の使用。
- 形成される血管又は動脈の少なくとも1つは、閉塞した動脈または血管のバイパスを提供する、請求項1または2に記載の使用。
- 前記血管または動脈は、少なくとも150μmの直径を有する、請求項1から3のいずれか1項に記載の使用。
- 患者において心筋および/または心筋細胞を産生する医薬の製造ための単離された心臓幹細胞の使用であって、前記心臓幹細胞は、患者に投与する前に、肝細胞増殖因子(HGF)および/またはインシュリン様増殖因子−1(IGF−1)に暴露することで活性化される、使用。
- 前記患者は、心筋梗塞または心筋虚血を有する、請求項5に記載の使用。
- 前記心臓幹細胞は、心筋組織から単離し、培地で増殖させる、請求項1から6のいずれか1項に記載の使用。
- 前記心臓幹細胞は、自家である、請求項1から7のいずれか1項に記載の使用。
- 前記医薬は、カテーテル系で投与されるよう製剤化されている、請求項1から8のいずれか1項に記載の使用。
- 前記医薬は、心筋内、動脈内、経心内膜、又は経心外膜注射用に製剤化されている、請求項1から8のいずれか1項に記載の使用。
- 前記医薬は、冠状動脈内で投与されるよう製剤化されている、請求項1から8のいずれか1項に記載の使用。
- 前記活性化された心臓幹細胞は、活性化されていない心臓幹細胞に比較して、アポトーシス率が低い、請求項1から11のいずれか1項に記載の使用。
- 前記活性化された心臓幹細胞は、コネキシン43、N−カドヘリン、E−カドヘリン、及びL−セレクチンからなる群より選択される一以上のマーカーを発現している、請求項1から11のいずれか1項に記載の使用。
- 前記心臓幹細胞は、c−kit陽性及びLineage−negativeである、請求項1から13のいずれか1項に記載の使用。
- 前記心臓幹細胞は、flk−1陽性である、請求項1から14のいずれか1項に記載の使用。
- 前記心臓幹細胞は、ヒトの細胞である、請求項1から15のいずれか1項に記載の使用。
