本実施形態においては、図2に示すように、通気層10aが透湿性シート3aと不織布3dとの積層体であり、通気層10aの通気度及び透湿度に支配的な部材は透湿性シート3aとなる。そこで、該透湿性シート3aの通気度及び透湿度を前記の範囲に設定することにより、通気層10aそのものの値と実質的に同じになる。透湿性シート3aの通気度及び透湿度を前記の範囲内とするためには、例えば(a)全面が通気性(及び透湿性)を有するシートとして前記の範囲内の通気度及び透湿度を有するものを用いるか、又は(b)全面が通気性(及び透湿性)を有するシートとして前記範囲よりも通気度が小さく、透湿度が大きいシートを用い、該シートを部分的に、例えばホットメルト接着剤等を用いて目止めすることで、通気部位を閉塞し、全体の通気性を前記範囲内にすればよい。(b)の場合には、通気層10aの一部が通気性を有することになり、通気度及び透湿度の測定は、目止めを行った部位を含めた測定試料を用いて行う。さらに、(c)全面が非通気(及び非透湿)のシートを用い、該シートの一部を針やレーザーなどの手段により適度な大きさに穿孔し、前記範囲内の通気度及び透湿度を有するように加工したものを用いても良い。いずれの場合であっても、通気層10a全体の通気度及び透湿度が、本発明の前記範囲内にあるものであれば使用可能である。

改善具1の発熱温度は前述のとおりであるところ、該改善具1は、38℃以上の温度の持続時間が3時間以上、特に5時間以上であることが好ましい。この程度の時間にわたって発熱が持続することで、高齢者の尿失禁及び頻尿の改善に顕著な効果が奏される。ここで、持続時間は、JIS S4100に準じて測定される。前記持続時間を達成するため手法としては、例えば、(イ)1枚の通気性シートの通気度を調整する、(ロ)複数枚の通気性シートを積層する、(ハ)後述するように、蒸気温熱発生部2を構成する各成分の配合量を調整する、(ニ)蒸気温熱発生部2に酸化反応抑制剤を添加することで蒸気温熱発生部2自体の発熱を制御する等が挙げられる。

改善具1における蒸気温熱発生部2は被酸化性金属を含んでいる。蒸気温熱発生部2は被酸化性金属が酸素と接触することによる酸化反応で生じた熱を利用して、所定温度に加熱された水蒸気を発生する部位である。詳細には、蒸気温熱発生部2は、被酸化性金属、反応促進剤、電解質及び水を含む。そのような蒸気温熱発生部2は、例えば発熱シート及び発熱粉体からなる。特に発熱シートからなることが好ましい。本発明では、発熱シートは被酸化性金属、反応促進剤、電解質及び水に、更に繊維状物を加えて成形した繊維シートから構成されていることが好ましい。つまり、発熱シートは、被酸化性金属、反応促進剤、繊維状物及び電解質を含む繊維シートが含水状態となっているものであることが好ましい。具体的には、発熱シートは、被酸化性金属、反応促進剤及び繊維状物を含有する成形シートに、電解質水溶液を含有させて構成されていることが好ましい。発熱シートとしては、湿式抄造により得られたシート状物や、発熱粉体を紙等で挟持してなる積層体、繊維シートの構成繊維間に被酸化性金属等が担持されたシート等が挙げられる。そのような発熱シートは、例えば本出願人の先の出願に係る特開2003−102761号公報に記載の湿式抄造法や、ダイコーターを用いたエクストルージョン法を用いて製造することができる。発熱シートを用いることで、使用者がどのような姿勢になっている場合でも、使用者へ蒸気温熱を均一に適用し得る点から好ましい。また発熱シートは、発熱粉体に比較して、被酸化性金属の担持能力が優れている点からも有利である。

発熱シートからなる蒸気温熱発生部2は、60〜90重量%、特に70〜85重量%の被酸化性金属、5〜25重量%、特に8〜15重量%の反応促進剤及び5〜35重量%、特に8〜20重量%の繊維状物を含む繊維シートに、該繊維シート100重量部に対して、1〜15重量%、特に2〜10重量%の電解質を含む電解質水溶液が25〜80重量部、特に30〜70重量部含有されて構成されていることが好ましい。発熱シートを構成する各種材料としては、当該技術分野において通常用いられているものと同様のものを用いることができる。また、特開2003−102761号公報に記載の材料を用いることもできる。

上述のとおり、改善具1は使用者の腰部に取り付けて使用されることが、尿失禁及び頻尿の一層効果的な改善の観点から好ましい。この場合、改善具1を、図3に示すような、着用者の身体に固定可能な固定具20に装着して、図4に示す形態で使用することができる。また、改善具1における非通気層10bの表面に固定手段(図示せず)を設け、該固定手段によって非通気層10bの表面を使用者の肌着に固定して使用することもできる。

通気層10aは、坪量45g/m2の透湿性ポリエチレンシートと、坪量30g/m2のポリエチレンスパンボンド不織布の積層体から構成した。非通気層10bは、坪量35g/m2の非透湿性ポリエチレンフィルムと、坪量30g/m2のポリエチレンスパンボンド不織布の積層体から構成した。通気層10aの通気度は16000秒、透湿度は400g/(m2・24hr)であった。これらの材料を用いて図1及び図2に示す形状の収容体3を製造した。収容体3における2つの収容部内に、2枚重ねにした発熱シートをそれぞれ収容した。これによって改善具1を得た。この改善具1の平均発熱温度は40℃(発熱開始から1時間後から6時間後まで)であり、38℃以上の発熱温度の持続時間は11時間であった。

得られた改善具1について尿失禁及び頻尿の改善効果を調べた。失禁改善教室に通う70〜85歳の女性高齢者(平均年齢76歳)を被験者とした。事前に被験者に対し聞き取り調査を行い、排尿及び失禁の回数を記録した。失禁頻度及び排尿頻度に有意差が発生しないように、無作為振り分け試験によって34名の温熱群と33名のコントロール群とに被験者を振り分けた。試験期間は3ヶ月とした。温熱群の被験者には、図3に示す固定具20を用いて図4に示すように改善具1を1日1回5時間にわたって腰部に適用させた。試験期間中は、各群の被験者に対して失禁回数及び排尿回数を日記に自記させた。各評価値の統計解析は、Wilcoxon符号付順位和検定及びkruskal-Wallis検定を用い、有意水準はp<0.05とした。その結果を以下の表1(失禁回数変化)及び図5(就寝中の排尿回数変化)に示す。

表1に示す結果から明らかなように、改善具1の使用によって失禁回数は有意に改善した。就寝中の排尿回数は、7週目から有意に改善した。これらの結果から本発明の改善具は失禁及び頻尿の改善に効果があることが確認された。また、表1及び図5には示していないが、試験終了後の温熱群の被験者には改善具1の使用に起因する肌トラブルは発生していなかった。
