Description
この出願は「鏡視下切除装置」という名称で、2002年2月12日に出願された米国特許出願第60/356,612号からの優先権を主張し、この出願の全内容は引用によりここに取り込まれる。
この発明は、生物学的な組織のような有機材料に熱的および/又は電気的エネルギを与えて組織の特性を治療目的のために変化させる外科用装置と方法に関する。特に、開示される装置は、手当てや治療のための医療処置を行う間に、高周波(RF)エネルギを用いて組織を切断し、凝結し、および/又は切除する電気外科用装置に関する。

Claims (20)
- 縦方向軸線を有し、かつ先端部と、前記縦方向軸線に沿った長手方向に延在した第1側面とを備えた細長いシャフトと、
シャフトの前記先端部に結合されて、前記第1側面の方に向いた第1電極と、
シャフトに結合され、第1電極から電気的に絶縁された第2電極であって、第1電極から基端方向に離れて露出部分を有し、かつ該露出部分が前記第1側面の方を向いている第2電極と、
を備えた外科用装置であって、
前記第2電極を備え、細長いシャフトの周囲の一部のみに設けられる導体と、
導体の少なくとも一部分にわたって設けられ、第2電極の露出部分を区画する窓を備える絶縁層とをさらに備え、
前記絶縁層も導体が露出していない細長いシャフトの少なくとも所定部分にわたって設けられ、その所定部分が電気外科手術中に隣接組織を保護するように形成される絶縁領域を備えることを特徴とする外科用装置。 - 絶縁層が熱的および電気的絶縁を与える請求項1の外科用装置。
- 第2電極を備え、細長いシャフトの周囲の一部分のみに設けられた導体と、
導体の少なくとも一部にわたって設けられ、第2電極の露出部分を区画する窓を備える絶縁層と、
を備える請求項1の外科用装置。 - 導体がクリップを備える請求項3の外科用装置。
- 第1電極に電気的に結合された伝導性チューブと、
伝導性チューブの少なくとも一部分にわたって設けられた第1絶縁層と、
クリップの少なくとも一部分にわたって設けられた第2絶縁層と、
をさらに備える請求項4の外科用装置。 - 第1絶縁層と第2絶縁層の各々がテフロン(登録商標)からなる請求項5の外科用装置。
- シャフトが管を形成し、装置が先端部分を備え、先端部分に管開口を形成し、その管開口は管に連通して吸引を行うように形成された請求項1の外科用装置。
- 第2電極の露出部分の表面積は、第1電極の表面積の少なくともほぼ6倍である請求項1の外科用装置。
- 第1電極は組織を削るように形成された表面を備える請求項1の外科用装置。
- 第1電極は、吸引形態に形成され、表面が吸引形態におけるエッジを備える請求項9の外科用装置。
- 細長いシャフトが縦軸を規定して先端部を備え、
第1電極がシャフトの先端部分に結合され、縦軸に沿ってまっすぐに延びて組織に接触するように形づくられている請求項1の外科用装置。 - 細長いシャフトは、ユーザがシャフトを湾曲できる柔軟な先端部分を備える請求項1の外科用装置。
- 第1電極は組織を除去するように形づくられている請求項1の外科用装置。
- 第2電極の全露出部分は第1電極から基端方向に離れている請求項1の外科用装置。
- シャフトは第1電極に近い先端に湾曲を備えると共に先端チップを備え、側面を向く第1電極は先端チップに設けられている請求項1の外科用装置。
- 第2電極が細長いシャフトの周囲の一部のみに設けられた露出部分を有し、
シャフトに結合してシャフトの全側面に沿って縦方向に延びる絶縁表面を備える手段を備え、絶縁表面に隣接する非目標組織は、目標組織へ電気エネルギを印加中に保護される請求項1記載の外科用装置。 - 手段は、シャフトに結合し第1電極から電気的に絶縁された導体からなり、その導体は細長いシャフトの周囲の一部のみに設けられ第2電極として働くように形づくられた部分を備える請求項16の外科用装置。
- 絶縁表面を備える手段は、熱的および電気的に絶縁された表面を備える手段である請求項16の外科用装置。
- 第1電極は、細長いシャフトの全周に設けられない露出部分を備え、かつ、第2電極は、シャフトに結合し、第1電極から電気的に絶縁され、第1側面の方を向き、細長いシャフトの全周に設けられない露出部分を備える請求項1記載の外科用装置。
- 側面を向く第1電極と、側面を向く第2電極が、第1側面上で同じ方向を向く請求項19の外科用装置。
