Description

R=(V2−V1)/tr
(i)適当な人工呼吸器を用いて、患者の起きている時の換気量を確認する(例えば、米国特許第6644312号明細書に従う)。
(ii)初期目標換気量を、上記換気量のある割合に設定する。
(iii)最終目標換気量を、現在のPCO2の所望のPCO2に対する比に初期目標換気量をかけて得られる値に設定する。
(iv)目標換気量の圧上昇時間を、PCO2を下げることの臨床上の緊急性に応じて、或いはPCO2の低下に好ましい量に応じて、ある適当な値に設定する(他の全ての条件が同じであれば、PCO2の低下が大きいほど時間がかかる可能性がある)。
(i)第1持続時間の間、換気量の第1レベルで、患者に換気させるステップ、及び
(ii)第1持続時間が満了した時点で、換気量のレベルを第1レベルから第2レベルに、第2持続時間にわたって、変更するステップ。
Claims (8)
- コントローラー回路と、
圧支持換気を提供するために圧搾空気を空気送出導管に沿って患者接続部分に流す、コントローラー回路の制御を受ける羽根車を有する電動モーターと、
最終目標換気量及び目標換気量圧上昇時間を設定する手段と、を含み、
前記コントローラーは、目標換気量に従った前記圧搾空気によって、前記患者接続部分を介して、患者をサーボ式に換気するように運転する、
睡眠中肺胞低換気の患者に圧支持換気を提供するためのサーボ式人工呼吸器であって、
前記コントローラーは、プログラムされたスケジュールに従って初期目標換気量から前記最終目標換気量へ、前記目標換気量を変更し、
前記目標換気量は、前記目標換気量圧上昇時間にわたって、前記最終目標換気量のレベルに至るまで決められた比率で増加し、その後は前記最終目標換気量のレベルに保持され、
前記目標換気量の圧上昇時間は、少なくとも、声門の閉鎖、または呼吸ドライブの停止を防止する長さの日数である、サーボ式人工呼吸器。 - 前記目標換気量は、所定の初期保持時間、前記初期目標換気量のレベルに保持される、請求項1記載のサーボ式人工呼吸器。
- 前記コントローラーは、更に、
前記初期目標換気量を、前記患者の起きている時の換気量のある割合に設定し、
前記最終目標換気量を、現在のPCO2値の所望のPCO2値に対する比に前記初期目標換気量をかけて得られる値に設定し、
前記目標換気量上昇時間を、PCO2を下げることの臨床上の緊急性に応じた、また、PCO2の低下に好ましい量に応じた値に設定する、
請求項1または2記載のサーボ式人工呼吸器。 - 前記コントローラーは、
圧支持レベルが高すぎて、声門の閉鎖または呼吸ドライブの停止を防止できない場合、前記目標換気量の増加の前記決められた比率を変更する、
請求項1記載のサーボ式人工呼吸器。 - 前記目標換気量の増加の前記決められた比率が下がり、前記最終目標換気量のレベルに達するまでより長くかかる、
請求項4記載のサーボ式人工呼吸器。 - 前記目標換気量の増加の前記決められた比率は、一時的にゼロに変更される、
請求項4記載のサーボ式人工呼吸器。 - 前記最終目標換気量に達しないように、前記目標換気量の増加の前記決められた比率が変更される、
請求項4記載のサーボ式人工呼吸器。 - 前記目標換気量は、前記目標換気量圧上昇時間の間、一定の比率で増加する、請求項1記載のサーボ式人工呼吸器。
