Description
本発明は、呼吸器疾患のための抗ウイルス療法に関する。より詳細には、本発明はインターフェロン-β(IFN-β)またはIFN-βの発現を増加させる薬剤を気道送達することによって、喘息または慢性閉塞性呼吸器疾患 (COPD)のライノウイルスが誘発する増悪を治療することに関する。喘息およびCOPDは共に、感冒ウイルス(ライノウイルス)が複数の臨床的問題に関連する増悪を引き起こすことがわかっている炎症性気道疾患の例である。
ウイルスの呼吸器への感染により、多数の呼吸器疾患の増悪を生じる。実際、ウイルスの呼吸器への感染は、85%の喘息増悪の原因である(Johnstonら、BMJ, 1995;310:1225-8;Nicholsonら、BMJ, 1993;307:982-6)(入院を必要とする最も重篤なものを含む:Johnstonら、Am. J. Respir. Crit. Care Med. 1996;154:654-660)。コンプライアンスを持った患者が最適用量の吸入コルチコステロイドを摂取することによって、良好に喘息を制御していたとしても、ウイルスによる感染が重篤な喘息増悪を誘発しうることが懸念される(Reddelら、Lancet, 1999;353:364-369)。喘息増悪に関連する最も一般的な病原体はライノウイルスである。ライノウイルスによる感染により、炎症媒介物の放出 (Teranら、Am. J. Respir. Crit. Care Med. 1997;155:1362-1366)、および増大した気管支応答を生じる(Grunbergら、Am. J. Respir. Crit. Care Med. 1997;156:609-616)。
従って本発明は、喘息およびCOPDから選択される呼吸器疾患のライノウイルスが誘発する増悪を治療するための医薬の製造における、
(a) インターフェロン-β(IFN-β);
(b) IFN-βの発現を増加させる薬剤;または
(c) (a)または(b)を発現することができるポリヌクレオチド;
から選択される薬剤の使用を提供する。この治療はエアロゾル噴霧器を用いることによって、医薬を気道送達することによるものである。
(a)インターフェロン-β(IFN-β);
(b) IFN-βの発現を増加させる薬剤;または
(c) (a)または(b)を発現することができるポリヌクレオチド、
からなる群より選択した薬剤を個体に気道投与することを含む、喘息およびCOPDから選択される呼吸器疾患のライノウイルスが誘発する増悪を個体において治療する方法を提供する。このような治療は、予防的処置であっても、治療的処置であってもよい。「ライノウイルスに誘発される」とは、ライノウイルスまたは主にしかし排他的ではなくライノウイルスを含むウイルスによって、もっぱら誘導されることとして理解される。
図1は、ライノウイルス(RV)感染後の、正常および喘息の気管支上皮細胞 (BEC)における前炎症(proinflammatory)反応を示す。パネル(a)および(c):RV-16感染から8時間後のIL-8 (a)およびTNFα (c)のmRNAの誘導を、qPCRによって測定した。喘息の細胞は、IL-8の誘導倍量の中央値 (IQR)が33.2 (7.3, 208.6)であり、対して健全な細胞では101.4 (6.4, 802.9)であった(群間で有意な差異はなかった(p=0.8))。両群は、培地のみ(p=0.001)および紫外線で不活性化したRV (p=0.01)で処理した細胞と比べて、IL-8 mRNAの顕著な増加を示した。TNFα mRNAについては、喘息の細胞は、基準線に基づく誘導倍量の中央値が94.4 (5.8, 1001.4)であり、対して健全なコントロール細胞では272.9 (30, 676)であった(群間で有意な差異はなかった(p=0.8))。両群は、培地のみ (p<0.01)および紫外線で不活性化したRV (p<0.01)を用いて処理した細胞と比べて、TNF-α mRNAの顕著な増加を示した。パネル(b)および(d):RV-16感染から48時間後の上清におけるIL-8 (b)およびTNFα (d)のタンパク質産生をELISAによって測定した。IL-8の中央値 (IQR)レベルは、健全な細胞における705 pg/ml (414, 979) (p=0.6)と比べて、喘息の細胞においては922 pg/ml (868, 1065)であった。両群は、培地のみ (61.4pg/ml, p<0.001)および紫外線で不活性化したRV-16 (43.8, P<0.01)を用いて処理した細胞よりも、顕著な増加を示した。TNF-αの分泌は、RV-16に感染した喘息の細胞において10.4 pg/ml (6.9, 29.6)であり、RV-16に感染した健全なコントロール細胞(p=0.7)において24.6 pg/ml (9.2, 30.4)であった。両群は、培地のみ (1.85pg/ml, p<0.01)および紫外線で不活性化したRV-16 (4.69, P<0.01)を用いて処理した細胞よりも顕著な増加を示した。パネル(e)および(f):ICAM-1発現を、RV-16感染の直前(e)または感染から24時間後(f)に、フローサイトメトリーによって測定した。データを蛍光強度の平均値(MFI)として表す。感染前に、喘息の細胞は健全なコントロール細胞の67 (34, 83)と比べて31(12, 80)と中央値 MFIが低い傾向がみられたが、顕著ではなかった (p=0.3)。24時間後、喘息の細胞は、健全なコントロール細胞の110.4 (65, 195.3) (p=0.02)と比べて54.6 (27.6, 145.2)と顕著に低い中央値 MFIであった。グラフは箱ひげ図であり、重線(heavy line)は中央値を示し、上部箱のボーダーは第75四分位数を表し、下部は第25四分位数を表し、ひげは第5および第95百分位数である。* =培地のみおよび紫外線で不活性化したRV-16を用いて処理した細胞とは顕著に異なる。

前述のとおり、本発明はIFN-βの新規治療用途に関する。とりわけ、例えば、IFN-βを気道送達し、ライノウイルスに感染した喘息患者の気管支上皮細胞におけるアポトーシスを促すといったIFN-βの治療用途に関する。提示されるように本発明は、IFN-βを気道送達して、ライノウイルスに誘発されるCOPDの増悪を治療することにも及ぶ。
本明細書中で用いられる場合IFN-βという用語は、未変性のIFN-βの生物学的活性を保持する、好ましくは、肺、特に気管支上皮に存在するIFN-βの活性を保持するIFN-βの任意の形態または類似体をいうものと理解される。
本発明はまた、肺または、好ましくは気管支上皮におけるIFN-βの内在性の発現を増加させる薬剤を用いることを含むことができる。この薬剤は、IFN-β遺伝子のプロモーター配列または他の調節配列に直接的に作用することができる。このような薬剤は、IFN-βプロモーターの構成的なサイレンシングを減少させるように作用しうる。あるいは、この薬剤は細胞を刺激し、細胞表面の受容体における作用によって、内在性 IFN-βを産生させることができる。本発明に関して、目的のIFN-βの内在性の発現を増加させる薬剤としては、ポリ(イノシン酸)-ポリ(シチジル酸) (ポリ(IC))およびACE阻害剤であるペリンドプリルが挙げられるが、これらに限定されない。
本発明はまた、IFN-βまたは肺気道におけるIFN-βの内在性の発現を増加させる薬剤を発現できるポリヌクレオチドを使用することを含みうる。このようなポリヌクレオチドは好ましくは、IFN-βまたは気管支上皮におけるIFN-βを誘導する薬剤を直接的に発現できるベクターの形態でありうる。このようにして、得られたIFN-βまたは薬剤は治療効果を有しえる(「遺伝子治療」)。ポリヌクレオチドは上記で考察したIFN-βの任意の形態(その変異体、断片およびキメラタンパク質を含む)をコードしうる。
本発明において、アポトーシス誘導剤を用いて呼吸器疾患のウイルスによって誘発された増悪を治療する。呼吸器疾患のウイルスによって誘発された増悪は、ウイルス(ライノウイルスなど)の存在による呼吸器疾患の重篤度を高める。ウイルスは一般的に、呼吸器疾患関連の症状を悪化させ、治療および場合によっては入院に対する応答を減少させる。ウイルスは一般的に、肺組織(気管支上皮を含む)に感染する。一般的に、ウイルスは炎症媒介物の放出を生じ、気管支の反応性を高める。本明細書中前述のとおり、ライノウイルスは喘息増悪の引き起こす一般的な病原体として認識されている。同様に、ライノウイルスは別の呼吸器疾患の望ましくない増悪を促進しうる。このように、本発明に関する目的の呼吸器疾患はまた、COPDと示されうる状態も含む。
IFN-β、IFN-βの発現を増加させる薬剤または上記のポリヌクレオチドの投与は、予防目的または治療目的のいずれかでありうる。予防的に与える場合、IFN-β、薬剤またはポリヌクレオチドをいずれかの増悪の前に与える。IFN-β、薬剤またはポリヌクレオチドの予防的投与は、その後生じる増悪を防止または減少させる。治療的にIFN-βを与える場合、薬剤またはポリヌクレオチドを、増悪の症状の発症時(または直後)に与える。IFN-β、薬剤またはポリヌクレオチドの治療的投与は、実際の増悪を減少させる。治療される個体は任意の動物でありうるが、好ましくは治療される個体は、ヒト、最も好ましくは喘息のヒトである。
材料および方法
被験体
全ての被験体は非喫煙者であり、4週間前より肺疾患の増悪も呼吸器の感染暦も有さなかった。一般的なエアロアレルゲン(ハウスダストダニ抽出物、草木の花粉、樹木の花粉、猫の鱗屑、犬の鱗屑、カンジダ(Candidia)、アスペルギルス(Aspergillus)を含む)ならびにネガティブコントロール(食塩水)およびポジティブコントロール (ヒスタミン)のパネルを用いたアレルギー皮膚テスト。テストは、ネガティブコントロールよりも3mmまたはそれ以上大きな膨疹反応が見られた場合、陽性とみなした。肺機能を肺活量測定により、1秒間の強制呼気量(FEV1)および強制肺活量(FVC)を測定することによって調べた。次に、気管支の過剰反応を、8mg/ml未満のPC20 ヒスタミンにより規定されるヒスタミンで誘発して調べた。喘息を有する被験体をGINAガイドラインによる臨床的重症度(National, H., Lung and Blood Institute. Global strategy for asthma management and prevention 96-3659a, Bethseda, 1995)に基づいて分類した。
上皮細胞を、公表された標準的なガイドラインに従って、ファイバーオプティック気管支鏡を用いて得た。全ての被験体にはサルブタモールを予め投与し、(Hurd, J Allergy Clin Immunol, 1991;88:808-814)そして細胞培養をこれまでに報告されているように行った (Bucchieriら、Am. J. Respir. Cell Mol. Biol., 2001;27:179-185)。要するに、細胞をシースドナイロン(sheathed nylon)細胞学ブラシを用いて、直接視で第2〜3世代の気管支を5〜10回擦過して得た。新たに擦過した気管支上皮細胞を培養皿に播種することによって、初代培養物を確立した。細胞を50U/ml ペニシリンおよび50μg/ml ストレプトマイシンを含有するホルモン添加した気管支上皮増殖培地(BEGM;Clonetics, San Diego, USA)中において、37℃かつ5%二酸化炭素にて培養した。細胞を培養し、トリプシンを用いて組織培養フラスコに継代した。2回目の継代時に、細胞を12ウェルトレイに播種し、80%コンフルエントな状態になるまで培養した (Bucchieriら、Am J Respir Cell Mol Biol, 2001;27:179-185)。上皮細胞の純度を回収した細胞のサイトスピンにおいて異なる細胞を数えることによって調べた。
本願発明者らは、これまでに報告されているようにOhio HeLa細胞の培養物を感染させ;細胞および上清を回収し、細胞を凍結解凍することによって破壊し、細胞片を低速遠心分離によってペレット化し、そして清澄した上清を-70℃で凍結して、RV-16ストックを作製した(PapiおよびJohnston, J Biol Chem, 1999;274:9707-9720)。
細胞生存率の分析
生存率およびアポトーシスを、これまでに報告されているようにフローサイトメトリーを用いて調べた (Puddicombeら、Am J Respir Cell Mol Biol, 2003;28:61-68)。簡潔にいえば、RV 感染から8時間の後、接着性細胞をトリプシンを用いて取り出し、非接着性細胞に添加した。細胞を、フルオロクロームフィコエリトリン (PE)にコンジュゲートしたアネキシン-V とバイタル色素7-アミノ-アクチノマイシン (7-AAD)を用いて染色した。フローサイトメトリーのデータをWinMDI 2.8を用いて分析した。カスパーゼ 3/7の活性形態を、Apo-One Homogenous Caspase 3/7アッセイ(Promega, Maddison, USA)を用いて検出した。細胞を4通りの各条件で播種した。2つのウェルをメチレンブルーで染色し、細胞の現存量を測定した。その他の2つのウェルを溶解緩衝液を用いて溶解し、励起波長485nmおよび蛍光波長530nmで蛍光プレートリーダーにおいて読み取った。次にカスパーゼ活性を細胞生存量に関して補正した。取り出され、そして48時間未満の間室温にて保管されていた細胞上清中の乳酸デヒドロゲナーゼ (LDH)の活性を測定することによって、細胞溶解を測定した。LDH活性を、基質としてピルビン酸(Sigma, St Louis USA)を用いた酵素学的速度法(enzymatic rate method)により、37℃にて測定した。
IL-8、TNFα、ICAM-1、IFNβおよびRVの遺伝子発現分析を、TRIzol 試薬 (Life Technologies, Paisley, UK)を用いてBECより抽出したRNA(混入したDNAを製造者の指示に従ってRNeasy Mini Kits (Qiagen, Crawley, West Sussex, UK) を用いたデオキシリボヌクレアーゼ消化によって除去した)を用いて行った。総RNA (1μg)を、製造者のプロトコルに従ってReverse Transcription System (Promega, Southampton, UK)のランダムヘキサマーまたはオリゴ(dT)15 プライマーおよびトリ骨髄芽球症 ウイルス転写酵素を用いて逆転写した。蛍光発生プローブを、5’-レポーター色素 6-カルボキシ-フルオレセイン (FAM)および3’-クエンチャー色素 6-カルボキシ- N,N,N’,N’-テトラメチル-ローダミン (TAMRA)を用いて標識した。
細胞におけるICAM-1の発現を、感染直後、およびRV感染から24時間後までに、ICAM-1に対するモノクローナル抗体 (eBioscience 抗ヒトCD54)およびFITC標識された二次抗体(Dako, Denmark)を用いた上記フローサイトメトリーによって、基準線を用いて測定した。
インターロイキン (IL)-8、腫瘍壊死因子-α(TNF-α)(R&D systems, Abingdon, UK)およびインターフェロン-β(IFN-β) (Biosource Nivelles Belgium)の培養上清への放出を、製造者の指示に従って酵素免疫測定法(ELISA)を用いて測定した。
データをSPSS version 10.1 (SPSS Inc)を用いて分析した。試料の大きさは小さく、変数は正常に分布していなかったために、グループ間の差異を非パラメトリック試験を用いて分析した; 2つの依存変数間の差異を符号順位検定、独立変数(ウィルコクソンの順位和検定)および多重比較(Kruskal Wallis検定)を用いて分析した。0.05未満のp値を有意であるとみなした。
正常および喘息の気管支上皮細胞(BEC)の反応を比較するために、臨床的に特徴付けられた有志よりファイバーオプティック気管支鏡により得た気管支擦過検体より初代培養を増殖した。RV-16を用いたBECの感染に関する用量および時間経過を、感染細胞より得た培養上清中のIL-8の放出を測定することによって、まず最適化した。これらの実験から、推定MOIが2であるRV-16用量を詳細な研究に用いた (データは示さない)。
正常気管支上皮細胞と喘息の気管支上皮細胞との差異を調べるために、本願発明者らは14人の喘息を有する被験体と10人の正常なコントロール(表2を参照のこと)を新たに加え、ファイバーオプティック気管支鏡を受けさせた。2つの被験体群は年齢および性別に関して類似していた。全ての喘息を有する被験体は、穏やかで中程度のいつまでも続く症状を有しており、定期的に吸入コルチコステロイドを用いた。初代BEC培養物のRV-16感染に対する反応をまず、IL-8およびTNFαのmRNA発現誘導ならびにタンパク質放出を測定することによって比較した(図1a,c)。喘息または健全なコントロールのいずれかに由来するBECは、RV 感染の8時間後に、IL-8およびTNFαのmRNAの有意な誘導を示し、感染の48時間後にはIL-8およびTNFαのタンパク質放出が有意に増加した(図1b,d);2つのグループ間に有意な差異は無かった。紫外線によって不活性化したRVは前炎症反応を引き起こさなかった。
BEC培養物のRV-16感染の後、生存可能なRVの回収率を、感染の伝染および感染したBECの上清によるOhio HeLa細胞における細胞変性効果 (CPE)によって測定した。感染後8時間までであって、初代培養の感染の後ウイルス粒子を産生した後に得られた上清を用いてもCPEは観察されなかったが、その後48時間まで徐々に生じた。前炎症反応に対して、喘息のBECは、24時間および48時間で検出されるTCID50で表されるRV-16が有意に大きく増加した(図2a)。健全なコントロールと比べて喘息においてはまた、感染から8時間後にRV-16 mRNAがより多く産生された(図2b)。ICAM-1の発現レベルが同等であるとすると、感染に対する即時感受性以外の要因が、感染細胞からのウイルス産生に影響していたことを示唆している。
アポトーシスがウイルス複製に対して細胞を保護する天然の防御であるため、本願発明者らは、アネキシン-V (AxV)および核染色(7-アミノアクチノマイシンD (7AAD))を用いて、RV-16に反応した細胞死の性質を特徴づけ、アポトーシス細胞の外側表面に外在化されているホスファチジルセリンを識別した。フローサイトメトリーの分析によって、正常なBECのRV-16感染から8時間の後に、生存可能な細胞(すなわち、AxV−/7AAD−)の数が有意に減少したことが明らかとなった。これは、培地のみまたは紫外線で不活性化したRV-16で処理した細胞では見られず、感染と細胞死との直接的な関連を示している(図3a)。それに対して、喘息のBECのRV-16による感染は8時間における生存率にわずかに影響した(図3a)。AxV+/7AAD−細胞(すなわち、アポトーシス細胞)とAxV+/7AAD+細胞(すなわち、壊死細胞)とを比べることによって、正常BECと喘息のBECの全体的な生存率の差異が、正常な培養物におけるアポトーシスの有意な増加により見出された (図3b)。感染細胞におけるアポトーシス誘導を、変化したミトコンドリア膜の透過性をApoAlert Mitochondrial Membraneセンサー(Clontech, Palo Alto Ca, USA)を用いて示すことによって(データは示さない)、また活性なカスパーゼ 3/7の活性化を測定することによって確認した。後者の場合、正常なBECよりもRV-16に感染した喘息のBECにおいて、活性なカスパーゼが有意に少なかった(図4a)。
喘息のBECによる増加したウイルス粒子の産生が、喘息のBEC のアポトーシスを回避する能力に関連していたため、本願発明者らは、RV-16に感染した正常BECにおけるアポトーシスの抑制がウイルス粒子の産生を促進するのに十分であるか否かを調べた。BECをRV-16による感染の前後に、カスパーゼ 3阻害剤 (C3I)であるZVD-fmkを用いて処理した。阻害剤により健全なコントロール細胞におけるアポトーシスの著しい減少が生じたが、感染のみと比べて喘息の細胞における効果は小さかった(図4b)。健全なコントロールの細胞のC3Iによる処理もまた、48時間における伝染性感染が有意に増加し、RV-16産生に直接的に影響したが、C3Iで処理した喘息の細胞においては同様の増加は見られなかった(図4c)。これらのデータにより、初期のアポトーシス阻害と増加したウイルス産生との直接的な関連を得た。
喘息のBECによる異常な抗ウイルス反応に関連する根本的な機構を調べるために、本願発明者らは、ウイルス感染に応答するアポトーシスの重要な制御因子であると考えられているI型インターフェロン (IFN) (IFN-β)(Samuel, Clin Microbiol Rev, 2001;14:778-809;Takaokaら、Nature, 2003;424:516-523)の発現を分析した。前炎症性サイトカインで観察した場合、RV-16感染から8時間後、正常BECによるIFN-β mRNA発現において有意な増加があったが、喘息の細胞においては同様の増加は見られなかった(図5a);またRV-16感染から48時間後の喘息の細胞によるIFN-β産生も少なかった(図5b)。このIFN-β産生の差異が機能的に関連したことを確認するために、本願発明者らは、RV-16に感染した喘息のBECにおいて、外因性IFN-βがアポトーシスを誘導する能力について試験した。図5cは、IFN-β(100IU)を用いた細胞のRV-16による前処理によって、倍の数のアポトーシス細胞を生じたことを示す。IFN-βのみでは、アポトーシス指標に有意な影響を及ぼさなかったが、合成ポリ(I):ポリ(C)への暴露に応答して顕著なアポトーシス誘導を生じた。このことはIFN-βに応答してアポトーシスを行うために、二本鎖 RNAの認識を含む他のシグナルを必要とすることを示す。ウイルスに感染した喘息のBECのアポトーシスを誘導する能力と共に、IFN-βはRV-16感染性ウイルス粒子の産生の有意な減少を生じた(図5d)。
慢性閉塞性肺疾患は炎症性気道疾患の別の例であり、感冒ウイルスが増悪を引き起こし(Seemungal TA, Harper-Owen R, Bhowmik A, Jeffries DJ, Wedzicha JA.Detection of rhinovirus in induced sputum at exacerbation of chronic obstructive pulmonary disease. Eur Respir J. (2000) 16, 677-83)、しばしば入院を必要する症状を伴う(MacNee W. Acute exacerbations of COPD. Swiss Med Wkly. (2003) May 3;133 (17-18):247-57)。喘息の被験体の気管支上皮細胞が欠損したI型インターフェロン応答を有するという知見に基づいて、COPDにおける同様の欠損によってまた、この患者群の下気道症状の重篤度を説明することができると仮定された。この可能性を調べるために、培養気管支上皮細胞の保存されている試料(archival sample)をRV-16感染に対するそれらの反応について試験した。これらの細胞は、COPDを有する2人の被験体 (男性1人および女性一人、それぞれ61歳および57歳)ならびにCOPDを有さない年齢が一致したコントロール (男性、64歳)より回収した気管支擦過検体より増殖した。継代0にて細胞を、凍結保護剤として10% DMSOを含有するBEGM培地中に−170〜−180℃にて凍結保存したことを除いて、擦過検体を、喘息研究について記載したとおりに培養した。凍結保存を細胞培養物の長期保存のために慣用的に用いる。
Claims (6)
- 喘息および慢性閉塞性肺疾患(COPD)から選択される呼吸器疾患のライノウイルスが誘発する増悪を治療するための医薬の製造における、
(a) インターフェロン-β(IFN-β);または
(b) IFN-βを発現できるポリヌクレオチド;
から選択される薬剤の使用であって、該治療は該医薬の気道送達によりなされるものである、上記使用。 - 前記呼吸器疾患が喘息である、請求項1に記載の使用。
- 前記呼吸器疾患がCOPDである、請求項1に記載の使用。
- 前記薬剤がIFN-βである、請求項1〜3のいずれか1項に記載の使用。
- 前記IFN-βが、
(a)ヒト IFNβ-1a (配列番号2);または
(b)ヒト IFNβ-1b (配列番号4)
の配列を含む、請求項4に記載の使用。 - 前記呼吸器疾患が喘息であり、そして前記医薬が吸入コルチコステロイドと同時に、別々にまたは逐次的に投与されるものである、請求項2、4および5のいずれか1項に記載の使用。
