Description

本発明は、少なくとも一部は、米国立衛生研究所(National Institute of Health)によって与えられる付与番号NIH CA76550により米国政府の支援を受けて実施された。米国政府は、本発明に一定の権利を有する。
経頭蓋超音波ドップラー画像形成は、1980年代初期に開始されて以来、脳内の血流、出血および灌流を測定する能力を実証している。最近の研究は、パーキンソンおよび抑うつなどのある種の変性疾患を診断する可能性も調査している。これらの方法は、経頭蓋ドップラー超音波検査(TCD)または関連する経頭蓋カラードップラー超音波検査法(TCCS)を使用して、送信信号および後方散乱の周波数シフトを記録する。信号は、頭蓋骨を透過するために、一般に、比較的低い周波数(〜2 MHz)プローブで適用され、造影剤と併用使用されることが多い。頭蓋骨は高周波数の減衰が大きく、骨を低域通過フィルターとして作用させて低スペクトル周波数だけを戻すので、2 MHzを超える周波数を適用する利点はほとんどないと思われる。1 MHz以下の中心周波数も調査されており、より強い信号強度を示しているが、予想されるように解像度は低い。
超音波ビームは、剪断波により効果的に干渉的に骨を介して送信することができることが発見されている。これは、例えば、ビームと骨表面に対する法線の角度を、縦波の完全な反射を生じる臨界角(約20°)より大きくすることによって実施することができる。約25°〜60°の間の角度では、剪断波が発生し、骨を伝搬し、内部表面で縦波に変換され、骨の他の側の軟部組織に伝搬する。この所見はいくつかの意味を有する:第一に、剪断波は、縦波が軟部組織において有する伝搬速度とほぼ同じ伝搬速度を骨内で有する。従って、骨内での音速が大きい(2500〜3000 ms/s)ために、縦波である場合にそうであるのようには、波面は骨の種々の厚さによって大きくひずまされることはない。複雑な患者特有の収差補正を実施しないで、骨を通過する超音波ビームを集束することができる。例えば、比較的簡単な装置を用いて、脳の診断用超音波画像形成ならびに超音波脳治療および診断を実施することができる。第二の意味は、ビームの入口角を大きくすることができ、縦波を使用するより高いゲインで脳の広い容量に到達することができることである。第三に、頭蓋骨を通過する超音波集束の予測は、剪断波の伝搬モデル化を縦波伝搬モデルを組合わせることによって大きく改善することができる。骨層からの望ましくない反射は低下または排除され、診断情報の分析を容易にすることができる。
本発明の態様は、骨に剪断モードを意図的に誘導する経頭蓋骨および他の経骨伝搬の技法を提供する。経頭蓋骨伝搬では、入射波は骨層において入射縦波から剪断波へのモード変換を経験し、次いで脳内で縦波に戻る。頭蓋骨の剪断波速度は、縦波速度と比較して、すぐれた音響インピーダンス、少ない屈折および少ない位相変化を提供することができる。剪断波を使用して、超音波を脳内で集束することができる。超音波が、スネル(スネル)の臨界角を超える角度で骨に入射する場合には、縦波は骨内で誘導されない。数値解析が提供され、この現象の実証は、プラスチックファントムおよびエクスビボにおけるヒト頭蓋骨を使用して検討された。本発明の態様は、治療的および診断適用を含む種々の適用に使用することができ、以下に別個に考察されている。他の態様も本発明の範囲内である。

システム10は、診断適用に加えてまたは診断適用の代わりに別個にまたは同時に治療適用に使用することができる。システム10の要素は、一般に、上記のように作動する。しかし、治療適用のためには、別の撮像装置13が提供される場合があり、システム10は、治療用途のために調整された方法で超音波を提供するように構成されている。要素16は、好ましくは、治療適用のための高い出力レベル(適用に応じて使用されるレベル)の超音波を放出し、方向づけるように構成されている。
実験によって容易に実証することができる理想的な等方性のケースを提供するために、平行なプラスチックプレートファントムを使用してアルゴリズムを試験した。プラスチックの関連する値を表1に要約する。源は、径12 cmおよび曲率半径16 cmの1.5 MHz集束トランスデューサーであった。アルゴリズムの放射源関数は、源の後方の幾何学的焦点付近の平面から実験室で測定した圧力場を射影することによって得た。この場は、空間分解能0.5 mmで30×30領域で測定した。放射源関数は、添付資料AおよびBに数値により記載されているアルゴリズムを使用してプレートを伝播した。源から測定プレートまでの距離は121 mmであり、プラスチックの内側面から源までの距離は61 mmであった。
頭蓋骨の位置合わせおよび密度
シミュレーションのためのデータは、CT画像(Siemens, SOMATOM, AH82 Bone Kernel)を使用して得られたデジタル化したヒト頭部プロファイルから入手した。頭蓋骨表面および内部の密度変化の座標は、これらの画像から得られる。スキャンは、視野200 mm×200 mmを使用して1 mm間隔で取った。ポリカーボネート定位フレームを各試料に取り付けて、頭蓋骨のアレイへの取り付けを可能にし、機械的位置決めシステムおよびCT画像の基準を提供した。算出は、考慮中の部分のビーム幅内に存在する骨においてだけ実施した。個々の頭蓋冠の形状および構造に関する情報は、物質の密度に比例する強度を戻した画像を組み合わせることによって得た。
伝搬実験は、数値アルゴリズムを証明するために水槽内にセットした。測定は、反射を抑制するためにゴムを当てた水槽内の脱気した脱イオン水中で実施した。超音波信号は、特定の測定に個別のトランスデューサーによって発生させ、ポリビニリデンジフルオリド(PVDF)針状聴音装置(Precision Acoustic, Dorchester, UK)で受信した。強力な受信と精度の維持を保証する助けとするために、0.2 mm径の聴音装置を1.5 MHzの測定に使用し、0.5 mm径の聴音装置を0.74 MHzの測定に使用した。大きい周波数では小さい聴音装置を使用して、聴音装置の指向性を小さくする助けとし、位相の平均化を防ぐ助けとした。頭蓋骨(またはプラスチックプレート)は、回転式ステップモーター(Bloomfield, New YorkのVelmex社製)によって制御される角度で聴音装置とトランスデューサーの間に配置した。3D線形位置決めシステム(Velmex, Bloomfied, Model V P9000)により、聴音装置は、トランスデューサーの対称軸付近を中心とする測定領域をスキャンすることができる。トランスデューサーの信号は、出力増幅器(Rochester, New YorkのENI社製, Model 2100L)に供給されている任意の波形発生装置(Norwich, UKのWavetek社製, Model 305)によって発生された。聴音装置の電圧レスポンスは、Precision Acousticsプレアンプおよび増幅器(Beaverton, OregonのPreamble Instruments社製, Model 1820)を介して送信されてから、デジタルオシロスコープ(Wilsonville, OregonのTextronix社製, Model 380)によって記録された。GPIB(General Purpose Interface Bus)コントロールによって電圧波形をPCにダウンロードし、測定位置の振幅および位相を信号のFFTから算出し、トランスデューサー駆動周波数の値を取る。
プラスチックファントム
測定した場と予測した場の相関を図13に示す。参照として、プロットは、剪断波を無視した場合に得られる数値計算の振幅も含む。3つ全ての曲線の振幅および位相は、信号のスペクトルピークの縦臨界角である31°以下の入射角ではぴたりと一致している。しかし、この角度を超えると、縦波だけのシミュレーションは、試料内の弾性波だけで生じる振幅の第2の局所的な公理を予測することができない。測定データとシミュレーションの間の矛盾の主な原因は、シミュレーションにおける剪断波の吸収係数の過小評価による結果による可能性がある。この影響は、光路長が最も長い高い角度でさらに顕著になる。しかし、波の振幅が最小に近い20°〜30°の遷移領域を除いて、全ての角度のシミュレーションの超音波の位相と測定した超音波の位相の間には非常に良好な相関が見られる。この領域を無視すると、算出した残りの地点の76%は、π/6ラジアン以下だけ測定値から逸脱した。
頭蓋骨を回転することによって、縦臨界角が約30°であることに基づいて、ほぼ完全に骨の剪断波伝搬による可能性がある局所的圧力伝達の極大が32°において観察された。さらに大きい入射角において剪断波としての波の起源に関するさらに別の証拠は、波の位相の角度依存を示す図14A〜Bに示す測定データにおいて見られた。図14Bに示すように、26°以下では、位相は縦だけの波の挙動によく類似している。26°〜32°の間では、傾斜は緩やかになり、剪断波と縦波を重ね合わせた寄与領域を示している。32°を超えると、剪断波の音速が水中の音速より小さくなるので、傾斜はほぼ線形になり、負になる。
経頭蓋画像形成では、反射される信号は頭蓋骨を2回伝搬し、測定可能で、好ましくはひずみの少ない信号で戻る。信号は、1 MHz Panametrics画像形成プローブおよびパルス-受信増幅器により分析した。ナイロンナット(c=2.6×106 mm/s Z=2.9 M Rayl)が画像形成用の対象として働いた。ナットは、細いスチール針で水槽の床の上方に浮かせた。A-ライン画像は、最初頭蓋骨に垂直で、次いで約33°の伝搬方向で得た。
経頭蓋方法の適用は、脳だけでなく、感染時流体が充満している、頭蓋内の副鼻腔への適用にも使用することができる。図21を参照すると、適用を実証するために、流体(水)を充満し、廃液することができる空洞を提供するプラスチックファントム130を開発した。以下の4つの状況の1つにおいて、ファントム130を介して信号を送信するために、中心周波数1-MHzのPanametricsトランスデューサー132を送信した:(1)空気が充填されている空洞134を有するファントム130を介して剪断波伝搬を誘導するための大きい入射角の超音波(>45°);(2)水が充填されている空洞134に、大きい入射角の超音波;(3)空気が充填されている空洞134を有するファントム130を介してほぼ全ての縦波伝搬を与える小さい入射角の超音波(<5°);および(4)水が充填されている空洞134に、小さい入射角。
予備的なシミュレーションおよび実験測定は、干渉性の集束超音波ビームを剪断波だけの波として頭蓋骨を介して送信することができることを示した。0.7 MHz付近の駆動周波数では、骨内の剪断波伝搬による頭蓋骨のピーク振幅は、縦波の伝搬に似通っており、高い場合もあることが見出された。さらに、剪断波が低い全体的な位相シフトを経験したという事実は、非侵襲的な経頭蓋骨集束方法を単純化し、脳内のさらに広い領域に拡大する可能性を確認している。剪断波振幅は縦波モードより小さかったが、骨の局在化した領域を伝搬するとき、全体的なビームはひずみをあまり受けないという証拠が見られた。さらに、剪断波は、おそらく、剪断波の速度と水中の音速の類似性により、位相ひずみをあまり経験しなかった。比較可能な類似性は軟部組織において見られると思われる。シミュレーションデータと測定データのさらに正確な相関は、頭蓋骨の剪断波速度のさらに正確な測定値を使用して得ることができる。

Claims (19)
- 剪断波を使用して被験者の標的領域に超音波信号を送達するシステムであって、以下を含むシステム:
超音波エネルギーを送信するように構成されている源;
源に接続されており、源を作動して超音波エネルギーを被験者に送信するように構成されているコントローラー;および
源に接続されており、送信される超音波エネルギーの少なくとも第1の縦波を被験者の表面に入射させて、該第1の縦波が被験者の第1の骨表面で第1の剪断波に変換され、かつ第1の骨表面の反対側の被験者の第2の骨表面で第2の縦波に変換されて、第2の縦波からのエネルギーが被験者の標的領域において治療レベルで超音波エネルギーの実質的な部分を形成するエネルギーをもって該標的領域に到達する、方向づけ手段であって、被験者に対する縦臨界角と被験者に対する剪断臨界角の間の第1の角度で被験者の第1の骨表面に第1の縦波を方向づけるように構成されている、方向づけ手段。 - 方向づけ手段が、以下の少なくとも1つを含む、請求項1記載のシステム:
(1)源に対する法線方向を第1の角度で表面に対して機械的に方向づけるように構成されている保定装置;ならびに
(2)源が複数の放射要素を含み、少なくとも第1の縦波を電子的に方向づけるため複数の放射要素の位相および遅延の少なくとも1つを調節するように構成されている、位相/遅延アジャスター。 - 保定装置が、
(1)法線が第1の角度で表面方向に方向づけられるように、被験者および源を一定の方法で接続する、ならびに
(2)法線が第1の角度で表面方向に方向付けられるように源を機械的に調節する、
うちの少なくとも1つを行うように構成されている、請求項2記載のシステム。 - 源が、超音波エネルギーを放射するように構成されている複数の要素を含み、システムが、源の少なくとも一部に超音波エネルギーを放射させ、放射されるエネルギーにより戻ってくるエネルギーの徴候(indicia)を処理して源に対する表面の少なくとも一部の配向を決定し、かつ、縦波臨界角と剪断波臨界角の間で表面の一部に少なくとも部分的に方向づけされるメインビームを有する源の要素だけを作動するように構成され、接続されているコントローラーを含む、請求項2記載のシステム。
- コントローラーが、反射されるエネルギーの徴候を処理して、表面の少なくとも一部の画像を形成するように構成されている、請求項4記載のシステム。
- 源に接続されており、第3の縦波の超音波エネルギーが被験者内で標的領域に到達する第4の縦波に変換される第2の剪断波を誘導し、第2の角度が第1の角度と異なるように、被験者に対する縦臨界角と被験者に対する剪断臨界角の間の第2の角度で少なくとも一部が被験者の表面に入射すると思われる第1の縦波および第3の縦波を生じるために源を作動するように構成されているコントローラーを含む、請求項1記載のシステム。
- 源に接続されており、源を作動して、標的領域にエネルギーを送信するため第1の縦波を生じるように構成されているコントローラーを含む、請求項1記載のシステム。
- 源に接続されており、源を作動して異なる周波数の複数のパルスの第1のメインビームを生じるように構成されているコントローラーを含む、請求項1記載のシステム。
- 異なる周波数が、約0.1 MHz〜約5 MHzの周波数範囲内である、請求項8記載のシステム。
- 異なる周波数が、約0.2 MHz〜約3MHzの周波数範囲内である、請求項9記載のシステム。
- パルスが、連続波に対して約1サイクルの範囲内の持続時間を有する、請求項9記載のシステム。
- 源に接続されており、被験者に対する縦臨界角より小さい第3の角度で被験者の表面に入射する第1の縦波および第5の縦波を生じるために源を作動するように構成されているコントローラーを含む、請求項1記載のシステム。
- 以下を含む、剪断波を使用して被験者の標的領域に超音波信号を送達するシステムであって、コントローラーが源に複数のパルスのエネルギーを送信させるように構成されており、各パルスが異なる周波数を有するシステム:
超音波エネルギーを送信するように構成されている源;
源に接続されており、源を作動して超音波エネルギーを被験者に送信するように構成されているコントローラー;および
源に接続されており、第1の縦波が被験者の第1の骨表面で剪断波に変換されるとともに被験者の第1の骨表面の反対側の被験者の第2の骨表面で第2の縦波に変換され、第2の縦波からのエネルギーが標的領域に到達するように、源の少なくとも一部からの第1のメインビームの第1の縦波が、被験者に対する縦臨界角と被験者に対する剪断臨界角の間の第1の角度で被験者の第1の骨表面の一部に方向づけられることを確実にするように構成されている位置決め装置。 - 源が、超音波エネルギーを送信するように構成されている複数の要素を含み、コントローラーが、(1)縦臨界角より小さい第2の角度で少なくとも一部が被験者の第1の骨表面の一部に入射すると思われる第2のメインビームを生じるように構成されている源の一部および(2)剪断臨界角より大きい第3の角度で少なくとも一部が被験者の第1の骨表面の一部に入射すると思われる第3のメインビームを生じるように構成されている源の一部の少なくとも1つの作動を阻止するように構成されている、請求項13記載のシステム。
- 異なる周波数が、約0.1 MHz〜約5 MHzの周波数範囲内である、請求項13記載のシステム。
- 異なる周波数が、約0.2 MHz〜約3 MHzの周波数範囲内である、請求項15記載のシステム。
- パルスが、連続波に対して約1サイクルの範囲内の持続時間を有する、請求項15記載のシステム。
- 位置決め装置が、被験者に対して適宜源を機械的に配向するように被験者に接続するように構成されている、請求項13記載のシステム。
- 源が、超音波エネルギーを送信するように構成されている複数の要素を含み、位置決め装置が、第1のメインビームを電子的に方向づけるために要素の位相に影響を与えるように構成されている、請求項13記載のシステム。
