この発明は、病院,保育所,幼稚園,児童相談所などにおいて用いられる電子式育児支援システム、及び、これに用いるプログラムに係り、特に乳幼児の身体系統の発達度合い、知能情緒系統の発達度合い、及び、育児環境の適正度合いを定量的に評価するための技術に関する。

【従来の技術】
普通、0歳〜6歳くらいまでの間の乳幼児をもつ保護者は、小児科医院の医師あるいは保育所の保育士などの育児指導者から定期的に育児指導(助言)を受けながら育児を行っている。育児指導者は、指導対象の乳幼児のからだ(身体系統)とこころ(知能情緒系統)の発達度合い、及び、育児環境の適正度合いを評価し、得られた評価を基礎にして育児指導の内容を決めている。即ち、育児指導者は、母子手帳に記入されている育児記録の閲覧結果および実際に育児担当者としての保護者に対する問診結果に基づき、乳幼児のからだとこころの発達度合い及び育児環境の適正度合いを経験的に評価し、育児指導を行っているのである。

【発明が解決しようとする課題】
しかしながら、従来の育児指導の場合、指導者(施設)間で育児指導の内容にバラツキがあるという問題がある。育児指導の内容の基礎となる乳幼児のからだとこころの発達度合い及び育児環境の適正度合い評価が、指導者の個人的経験が強く反映される主観性の強い定性的評価であるので、指導の内容がバラツクことが、避け難いのである。

この発明は、上記の事情に鑑み、乳幼児のからだとこころの発達度合い及び育児環境の適正度合いを定量的に評価することができる電子式育児支援システム、及び、これに用いるプログラムを提供することを課題とする。

【課題を解決するための手段】
前記課題を解決するために、請求項1の発明に係る電子式育児支援システムは、乳幼児の身体系統の発達度合い、知能情緒系統の発達度合い、及び、育児環境の適正度合いをそれぞれ評価して乳幼児の育児を支援する電子式育児支援システムであって、(A)身体系統の発達度合い、知能情緒系統の発達度合い、及び、育児環境の適正度合いに関して乳幼児の所定の齢(月年齢)ごとに定められている質問を質問の属する所定の齢と対応付けて記憶する質問記憶手段と、(B)乳幼児の齢を含む乳幼児の属性データおよび各質問に対する回答を入力する入力手段と、(C)入力手段により入力された乳幼児の齢に対応する質問を質問記憶手段から読み出す質問読出手段と、(D)各質問毎に回答の状況に応じて設定された評点数を予め記憶する評点記憶手段と、(E)入力手段により入力された回答の状況と回答対象の質問毎に設定されている評点数とに基づき、知能情緒系統の発達度合い、及び、育児環境の適正度合いそれぞれの合計点数を算出するとともに、乳幼児の身長と体重から算定されたカウプ指数に応じた評点数に基づいて身体系統の発達度合いの合計点数を算出する算出過程と、前記カウプ指数に応じた評点数と前記回答の状況に応じて得られた評点数とに基づいて身体系統の発達度合いの合計点数を算出する算出過程とを前記乳幼児の齢に応じて変化させて身体系統の発達度合いの合計点数を算出する評価手段と、(F)質問と回答の状況および合計点数を表示する表示手段とを備えている。

また、請求項2の発明では、請求項1に記載の電子式育児支援システムにおいて、前記算出過程は、前記カウプ指数に応じた評点数から前記回答の状況に応じて得られた評点数を差分することによって身体系統の発達度合いの合計点数を算出する過程である。

さらに、請求項5の発明に係るプログラムは、乳幼児の身体系統の発達度合い、知能情緒系統の発達度合い、及び、育児環境の適正度合いを評価して乳幼児の育児を支援する電子式育児支援システムに用いるプログラムであって、コンピュータを、(A)身体系統の発達度合い、知能情緒系統の発達度合い、及び、育児環境の適正度合いに関して乳幼児の所定の齢(月年齢)ごとに定められている質問を質問の属する所定の齢と対応付けて記憶する質問記憶手段と、(B)乳幼児の齢を含む乳幼児の属性データおよび各質問に対する回答を入力する入力手段と、(C)入力手段により入力された乳幼児の齢に対応する質問を質問記憶手段から読み出す質問読出手段と、(D)各質問毎に回答の状況に応じて設定された評点数を予め記憶する評点記憶手段と、(E)入力手段により入力された回答の状況と回答対象の質問毎に設定されている評点数とに基づき、知能情緒系統の発達度合い、及び、育児環境の適正度合いそれぞれの合計点数を算出するとともに、乳幼児の身長と体重から算定されたカウプ指数に応じた評点数に基づいて身体系統の発達度合いの合計点数を算出する算出過程と、前記カウプ指数に応じた評点数と前記回答の状況に応じて得られた評点数とに基づいて身体系統の発達度合いの合計点数を算出する算出過程とを前記乳幼児の齢に応じて変化させて身体系統の発達度合いの合計点数を算出する評価手段と、(F)質問と回答の状況および合計点数を表示する表示手段として機能させる。

〔作用〕
次に、この発明の作用を説明する。
請求項1の発明の電子式育児支援システム(以下、適宜「システム」と略記)では、入力手段により育児指導対象の乳幼児の齢を含む属性データなど必要なものが入力された後、身体系統(からだ)の発達度合い、知能情緒系統(こころ)の発達度合い及び育児環境の適正度合いに関して乳幼児の所定の齢ごとに定められた質問を質問が属する所定の齢と対応付けて記憶している質問記憶手段から、入力された乳幼児の齢に対応する質問が質問読出手段により読み出されて表示手段に表示される。
そして、入力手段により各質問に対する回答が入力されると共に回答の状況が表示手段に表示され、質問に対する回答の入力が終われば、評価手段により、入力手段で入力された回答の状況と評点記憶手段に予め記憶されている各質問毎の評点数とに基づき、こころの発達度合い及び育児環境の適正度合いそれぞれの合計点数が算出されるとともに、乳幼児の身長と体重から算定されたカウプ指数に応じた評点数に基づいて身体系統の発達度合いの合計点数を算出する算出過程と、前記カウプ指数に応じた評点数と前記回答の状況に応じて得られた評点数とに基づいて身体系統の発達度合いの合計点数を算出する算出過程とを前記乳幼児の齢に応じて変化させて身体系統の発達度合いの合計点数が算出され、表示手段に表示される。

即ち、請求項1の発明の電子式育児支援システムでは、乳幼児のからだの発達度合い、こころの発達度合い及び育児環境の適正度合いに関する各質問毎に回答の状況に見合った評点数が予め与えられていて、各質問に対する実際の回答の状況と回答対象の質問について与えられている評点数とに基づき、指導対象の乳幼児のからだの発達度合い、こころの発達度合い、及び、育児環境の適正度合いのそれぞれが合計点数という客観的な数値として算出されるのである。

【発明の実施の形態】
続いて、この発明の一実施例を図面を参照しながら説明する。図1は実施例に係る電子式育児支援システムの全体構成を示すブロック図である。
実施例のシステムは、例えば小児科医院の医師や、保育所の保母、保健所の保健婦などの育児指導者が育児指導を行おうとする際に用いられるものであって、乳幼児のからだの発達度合い、こころの発達度合い、及び、育児環境の適正度合いを評価して乳幼児の育児を支援する電子式育児支援システムであり、以下、システムが小児科医院に配設されているものとして説明する。

実施例のシステムでは、図1に示すように、小児科医院の待合室などに設置する予診用(パーソナル)コンピュータ1と、診察室に設置する本診用(パーソナル)コンピュータ2が配備され、両コンピュータ1,2は必要なデータのやり取りが行えるようローカルネットワーク3により接続されている。通常、先ず待合室において、看護婦から出される質問に対して保護者から返ってくる回答が予診用コンピュータ1に入力された後、診察室において、医師から必要に応じて出される質問に対して保護者から返ってくる回答が本診用コンピュータ2に入力されるのに加え、予診用コンピュータ1で入力された回答もローカルネットワーク3を介して本診用コンピュータ2に取り込めるよう構成されている。さらに入力された回答に従って指導対象の乳幼児のからだの発達度合い、こころの発達度合い、及び、育児環境の適正度合いそれぞれの評価を示す数値が算出されるよう構成されている。
以下、実施例のシステムの各部構成を具体的に説明する。

予診用コンピュータ1は、からだの発達度合い、こころの発達度合い及び育児環境の適正度合いに関して乳幼児の所定の齢(月年齢)ごとに定められている質問を質問の属する所定の齢と対応付けて記憶すると共に乳幼児の属性データ、各質問に対する回答などを記憶するデータ記憶部(質問記憶手段)1Aを備えているのに加え、乳幼児の齢(月年齢)を含む乳幼児の属性データおよび各質問に対する回答を入力する(マウスやキーボード等の)入力部4と、入力部4により入力された乳幼児の齢に対応する質問をデータ記憶部1Aから読み出す質問読出部1Bと、質問や回答状況などを表示する表示用モニタ部(表示手段)5を備えている。

質問を定める際の区切りとなっている各乳幼児の所定の齢は、実施例の場合、出生時,1ケ月,3〜4ケ月,6〜7ケ月,9〜10ケ月,1歳,1歳6ケ月,2歳,3歳,4歳,5歳,6歳であるが、もちろん、これに限定されるものではない。
質問の数や内容は齢に応じて変化してゆくが、からだの発達度合いに関する質問としては、体重,身長,首が座ったか否か,歯は何本はえたか,等が一例として挙げられる。こころの発達度合いに関する質問としては、おはようが言えますか,大人の言う簡単な言葉が分かりますか,ママゴトなどの遊びをしますか,等が一例として挙げられる。育児環境に関する質問としては、子守歌を歌ってあげますか,(乳幼児が)眠くなったらテレビを消しますか,生活リズムが整っていますか,等が一例として挙げられる。

予診用コンピュータ1の場合、表示用モニタ部5の画面には、質問読出部1Bにより、読み出された質問が表示され、表示用モニタ部5に表示された質問を順番に読み上げたりする等して看護婦が保護者に質問を出す一方、保護者から返ってくる回答を看護婦が入力部4で入力すると、入力された回答がデータ記憶部1Aに記憶されると同時に表示用モニタ部5の画面に回答状況が表示されるという構成になっている。

本診用コンピュータ2も、からだの発達度合い、こころの発達度合い及び育児環境の適正度合いに関して乳幼児の所定の齢(月年齢)ごとに定められている質問を質問の属する所定の齢と対応付けて記憶すると共に乳幼児の属性データ、各質問に対する回答などを記憶するデータ記憶部(質問記憶手段)2Aを備えているのに加え、乳幼児の齢(月年齢)を含む乳幼児の属性データおよび各質問に対する回答を入力する(マウスやキーボード等の)入力部6と、入力部6により入力された乳幼児の齢に対応する質問をデータ記憶部2Aから読み出す質問読出部2Bと、質問や回答状況などを表示する表示用モニタ部(表示手段)7を備えている。

本診用コンピュータ2の側においても、質問を定める際の区切りとなる所定の齢や、質問の数や内容に関しては予診用コンピュータ1の場合と実質的に同じである。
また、本診用コンピュータ2の場合も、表示用モニタ部7の画面に質問読出部2Bで読み出された質問が表示され、必要に応じて表示用モニタ部5に表示された質問を医師が読み上げたりして保護者に質問を出しながら(問診しながら)、保護者から返ってくる回答を医師が入力部6で入力すると、入力された回答がデータ記憶部2Aに記憶されると同時に表示用モニタ部7の画面に回答の状況が表示されるという構成になっている。

なお、上述したように両コンピュータ1,2の間はネットワーク3で接続されていて、入力部4,6の操作等によって相手方のデータ記憶部に記憶されている回答などのデータをネットワーク3を介してやり取りすることができる。通常、医師が乳幼児や保護者と対面して健診する前に、待合室で先に予診用コンピュータ1を使って必要なデータ(例えば、看護婦などが計測した乳幼児の体重や身長の身体計測データ、保護者が答える回答データなど)が入力される。そして、医師が乳幼児や保護者と対面して健診するときに、予診用コンピュータ1に先に入力されているデータを本診用コンピュータ2のデータ記憶部2Aに取り込む。この取り込んだデータを医師が本診用コンピュータ2の画面に表示し、これを確認して必要な問診を行うとともに、本診用コンピュータ2に健診所見のデータなどを入力するようになっている。

そして、本診用コンピュータ2は、各質問毎に回答の状況に応じて設定された評点数(ポイント)が予めデータ記憶部(評点記憶手段)2Aに記憶されるよう構成されているのに加え、入力部6で入力された回答の状況(医師が入力した健診所見も含む)と回答対象の質問毎に設定されている評点数とに基づき、乳幼児のからだの発達度合い、こころの発達度合い及び育児環境の適正度合いそれぞれの合計点数を算出する評価部2Cを備えている。

続いて、本診用コンピュータ2の評価部2Cによる各合計点数の具体的な算出過程を説明する。
こころの発達度合いの場合は、こころの発達度合いに関する各質問毎に、回答の状況が評価を上げるものであれば何ポイント(例えば1ポイント),評価を下げるものであれば何ポイント(例えば0ポイント)とそれぞれ評点数として予め与えられており、こころの発達度合いの合計点数EH(%)は、〔実際の回答の状況に従って各質問毎に得られたポイントを全て加算した合計ポイント数Ph〕÷〔全ての回答の状況が評価を上げるものであった時のポイントを全て加算した最高ポイント数PH〕×100という式によって算出される。なお、回答の状況が評価を上げるものであった時の各評点数が全て1ポイント,回答の状況が評価を下げるものであった時の各評点数が全て0ポイントであれば、最高ポイント数PHは、こころの発達度合いに関する質問の総数と同一となる。

育児環境の適正度合いの場合は、育児環境の適正度合いに関する各質問毎に、回答の状況が評価を上げるものであれば何ポイント(例えば1ポイント),評価を下げるものであれば何ポイント(例えば0ポイント)とそれぞれ評点数として予め与えられており、育児環境の適正度合いの合計点数EI(%)は、〔実際の回答内容に従って各質問毎に得られたポイントを全て加算した合計ポイント数Pi〕÷〔全ての回答の状況が評価を上げるものであった時のポイントを全て加算した最高ポイント数PI〕×100という式によって算出される。なお、回答の状況が評価を上げるものであった時の各評点数が全て1ポイント,回答の状況が評価を下げるものであった時の各評点数が全て0ポイントであれば、最高ポイント数PIは、育児環境の適正度合いに関する質問の総数と同一となる。

からだの発達度合いの場合は、からだの発達度合いの合計点数EBの算出過程が乳幼児の齢に応じて変化する。からだの発達度合いの場合、乳幼児の体重W(kg)及び身長L(m)2 の質問が必ずあって、合計点数EBの算出過程では、先ず乳幼児の体重・身長の回答からカウプ指数KP=体重W(kg)÷身長〔L(m)〕2 が算出された後、さらに指導対象の乳幼児の齢に応じて以下の・〜・のいずれかの演算が行われる。

・〔乳幼児の齢が1歳以下の場合〕
カウプ指数KPに応じて下記のようにポイントkp%が算定されると共に、からだの発達度合いに関する各質問毎に回答の状況が評価を下げるものであれば、減点用評点数として何ポイント(M%)かが予め与えられており、合計点数EB=〔ポイントkp%〕−〔実際の回答の状況に従って各質問毎に得られたポイント(M%)を全て加算した合計ポイント数Σ(M%)〕という式によって算出される。

なお、カウプ指数KPとポイントkp%との間の関係は次の通りである。
KP≧22の場合(太りすぎ):・・kp%=95%
22>KP≧15の場合(太っている、標準):・・kp%=100%
15>KP≧10の場合(やせている):・・kp%=85%
10>KPの場合(やせすぎ):・・kp%=80%
例えば、カウプ指数KPが23であって、「好き嫌いがありますか」という質問(M%=3%)に対して「はい」という評価を下げる回答があった場合は、ポイントkp%=95%,合計ポイント数Σ(M%)=3%となるので、合計点数EB=95%−3%=92%と算出される。

上記・の1歳以下の場合と全く同様にカウプ指数KPに応じてポイントkp%が算定され、そして、算定されたポイントkp%が直ちに合計点数EBとして算出される。

カウプ指数KPに応じて以下のようにして算定されるポイントkp%が、直ちに合計点数EBとして算出される。
カウプ指数KP≧18であれば、KP=18の場合をポイントkp%=100%とし、KP=28以上の場合をポイントkp%=0%として、KP=18〜28の間は補間演算によりポイントkp%が算定される。
カウプ指数KP<18であれば、KP=18の場合をポイントkp%=100%とし、KP=9以下の場合をポイントkp%=0%として、KP=9〜18の間は補間演算によりポイントkp%が算定される。

このように、実施例の電子式育児支援システムの場合、評価部2Cにより、指導対象の乳幼児のからだの発達度合い、こころの発達度合い及び育児環境の適正度合いのそれぞれが合計点数EH,EI,EBという数値として算出されるので、育児指導の基礎データとなる評価は、客観性の強い定量的評価となり、その結果、指導者(施設)間で育児指導の内容にバラツキが生じる事態が回避できることになる。

さらに、本診用コンピュータ2は、評価部2Cにより算出される合計点数EH,EI,EBを下げること(減点原因)になった回答があった質問を表示用モニタ部7に表示する減点原因チェック部2Dを備えている。医師は、減点原因チェック部2Dによって表示用モニタ部7に表示された減点原因に関連する質問を見て、育児上の問題点を容易に把握することが出来るので、育児指導をより適切に行うことができる。

加えて、本診用コンピュータ2は、質問に関連する注意事項がその質問と対応付けられてデータ記憶部(注意事項記憶手段)2Aに記憶される構成となっていると共に、評価部2Cにより算出される合計点数EH,EI,EBを下げること(減点原因)になった回答があった質問についての注意事項をデータ記憶部2Aから読み出して表示用モニタ部7に表示する注意事項チェック部2Eを備えている。医師は、注意事項チェック部2Eによって表示用モニタ部7に表示された減点原因に関係する質問についての注意事項を見て、育児上の問題点を容易に把握することが出来るので、育児指導をより適切に行うことができる。

また、本診用コンピュータ2は、各質問毎に回答の状況に応じて設定された評点数を変更する評点変更部2Fも備えている。本診用コンピュータ2の評点変更部2Fの場合、表示用モニタ部7に変更対象となる既設定の評点数を表示しておき、入力部6によって数値を変える操作を行うことで評点数を変更することができるという構成になっている。

一方、予診用コンピュータ1は、本診用コンピュータ2の評価部2Cと同様の構成で合計点数EH,EI,EBを算出する評価部1C、及び、本診用コンピュータ2の減点原因チェック部2Dと同様の構成で合計点数EH,EI,EBの減点原因になった回答があった質問を表示用モニタ部5に表示する減点原因チェック部1Dを備えている。
但し、予診用コンピュータ1には、医師以外の者が用いるのに不適当な注意事項チェック部2Eおよび評点変更部2Fに相当するものは設けられていない。さらに、予診用コンピュータ1の場合、閲覧可能な既入力データが前回の齢の分だけに制限される構成となっているが、本診用コンピュータ2の場合、閲覧可能な既入力データに制限がない構成になっている。

また、実施例の電子式育児支援システムの場合、ドライバ1aに装填されたCD−ROMなどの記録媒体1bに記録されたプログラムにより、予診用コンピュータ1に備えられた図示しないCPU(中央演算処理装置)を、質問読出部1B,評価部1C,減点原因チェック部1D,として機能させる構成となっている。また、予診用コンピュータ1に備えられた図示しない内部メモリや光磁気ディスクは、データ記憶部1Aとして機能する。同じく、ドライバ2aに装填されたCD−ROMなどの記録媒体2bに記録されたプログラムにより、本診用コンピュータ2に備えられた図示しないCPU(中央演算処理装置)を、質問読出部2B,評価部2C,減点原因チェック部2D,注意事項チェック部2E,評点変更部2Fとして機能させる構成になっている。また、本診用コンピュータ2に備えられた図示しない内部メモリや光磁気ディスクは、データ記憶部2Aとして機能する。なお、上述した各プログラムはインターネットを介して各コンピュータ1,2にダウンロードするようにしてもよい。

続いて、以上に述べた構成の実施例システムにより、乳幼児のからだの発達度合い、こころの発達度合い、及び、育児環境の適正度合いの各合計点数EH,EI,EBを算出する典型的な場合に即して、実施例のシステムをさらに具体的に説明する。以下の説明では、育児指導対象の乳幼児の齢は1歳であるとする。図2は、予診用コンピュータ1の作動プロセスを示すフローチャートである。なお、入力データは予診用コンピュータ1または本診用コンピュータ2のいずれから入力されてもよいが、以下では特に断らない限り、予診用コンピュータ1から入力されるものとして説明する。

〔ステップS1〕入力部4を使って予診用コンピュータ1にカルテナンバーやパスワードを入力してログインすると、図3に示す入力月年齢設定画面Q1が表示用モニタ部5の画面に表示される。この画面から健診コースを選択する。ここでは、1歳健診ボタンQ1aがクリックされたとする。
