Description
本出願は、米国仮出願第60/416,382号(表題「ELECTROSURGICAL INSTRUMENT FOR SEALING VESSELS」、Johnsonらによって2002年、10月4日に出願された)に対する優先権および利益を主張する。
本発明は、ヒトもしくは動物において、管を永久的に閉鎖するための電気外科手術用器具に関し、より詳細には、圧力と電気外科手術用電流との組合せを適用することによって管および血管を封鎖し、そして封鎖プロセスの間、血管の封鎖表面の間で適切な間隔距離を維持する、双極電気外科手術用器具に関する。
本発明の一般的な目的は、縫合糸や外科手術用クリップを必要とすることなく組織を癒合し得る電気外科手術用器具を提供することである。この器具は、対向可能なあごに配置された二つの封鎖表面間に電気外科手術用電流を伝導する。この電気外科手術用電流は、あごの間にクランプされた組織を通過し、コラーゲンを再成形させて、組織を癒合させ、永久的封鎖部を形成する。
本発明は、縫合糸も外科手術用クリップも必要とすることなく、組織を癒合し得る電気外科手術用器具を提供する。図1〜4を参照して、器具10は、固定内側部材11および外側クランプ用部材12を有する。内側部材11および外側クランプ用部材12は、フランジ33との間に間隔を有するオープンロックボックス13を通して接続される。用語「内側」および「外側」は、オープンロックボックス13におけるそれらのそれぞれの位置に従って、内側部材11および外側クランプ用部材12、ならびにそれらの部品を区別するために使用される。内側部材11は、一般的に、オープンロックボックス13の内側表面内に取付けられ、そしてフランジ33によって捕らえられる。外側クランプ用部材12は、一般的に、オープンロックボックス13の外側表面を形成する。
Claims (9)
- 組織を封鎖するための電気外科手術用器具であって、該電気外科手術用器具は、以下:
内側部材および外側クランプ用部材であって、該内側部材および該外側クランプ用部材のうちの少なくとも一つは、その近位端の近くに環状ハンドルを備え、該内側部材および該外側クランプ用部材の各々は、その遠位端に近接する封鎖表面を備え、該内側部材および該外側クランプ用部材のうちの少なくとも一つは、該封鎖表面が互いに対して間隔をあけられた第一の位置から、該封鎖表面が、それらの間の組織をつかむために互いに対して対向する関係で配置される第二の位置へ移動可能である、内側部材および外側クランプ用部材;
該外側クランプ用部材に、近位端で、操作可能に接続され、かつ該外側クランプ用部材の該封鎖表面に、遠位端で、操作可能に接続される押棒であって、該押棒は、該外側クランプ用部材の該封鎖表面を該内側部材の該封鎖表面に対して回転させるために移動可能である、押棒;
止めであって、該止めは、該押棒の遠位への動きを制限するために該内側部材の該近位端の近くに配置されて、該封鎖表面が該第二の位置に配置される場合に、該対向する封鎖表面の間の間隔を維持する、止め;
該組織の封鎖部の形成後に組織を切断するために可動性切断作動器を有する切断機構;および、
該封鎖表面にエネルギーを与えるための手段であって、該手段によって、該封鎖表面が該第二の位置に配置されて組織封鎖部を形成する場合、それらの間に保持された組織を通じて電気外科手術用エネルギーが伝達される、手段、
を備える、電気外科手術用器具。 - 請求項1に記載の、組織を封鎖するための電気外科手術用器具であって、前記内側部材が柄部分を備え、そして前記外側クランプ用部材が柄部分を備え、該柄部分が、組織をクランプする間、機械的に有利なばね荷重を提供する、電気外科手術用器具。
- 請求項1または2に記載の、組織を封鎖するための電気外科手術用器具であって、前記押棒が、回転性アームに操作可能に接続され、次いで該回転性アームが、前記外側クランプ用部材に操作可能に接続されて、該外側クランプ用部材の動きが、該回転性アームを回転させて該押棒を動かす、電気外科手術用器具。
- 請求項1〜3のいずれか1項に記載の、組織を封鎖するための電気外科手術用器具であって、前記止めが、前記封鎖表面の間の分離距離を約0.03mm〜約0.15mmの範囲に維持する、電気外科手術用器具。
- 請求項1〜4のいずれか1項に記載の、組織を封鎖するための電気外科手術用器具であって、前記止めが、前記封鎖表面の間の前記分離距離を変動させるため、選択的に配置可能である、電気外科手術用器具。
- 請求項1〜5のいずれか1項に記載の、組織を封鎖するための電気外科手術用器具であって、前記止めが、前記封鎖表面の間の前記分離距離を変動させるため、選択的に再配置可能である、電気外科手術用器具。
- 請求項1〜6のいずれか1項に記載の、組織を封鎖するための電気外科手術用器具であって、前記内側部材および前記外側クランプ用部材の各々が、離れた位置において、約30,000Kg/m 2 〜約160,000Kg/m 2 (約3Kg/cm2〜約16Kg/cm2)の範囲で閉鎖力を保持するように協働する、接合するラチェットを備える、電気外科手術用器具。
- 組織を封鎖するための電気外科手術用器具であって、該電気外科手術用器具は、以下:
内側部材および外側クランプ用部材であって、該内側部材および該外側クランプ用部材のうちの少なくとも一つは、その近位端の近くに環状ハンドルを備え、該内側部材および該外側クランプ用部材の各々は、その遠位端に近接する封鎖表面を備え、該内側部材および該外側クランプ用部材のうちの少なくとも一つは、該封鎖表面が互いに対して間隔をあけられた第一の位置から、該封鎖表面が、それらの間の組織をつかむために互いに対して対向する関係で配置される第二の位置へ移動するために、オープンロックボックス内で回転可能である、内側部材および外側クランプ用部材;
