Description
(1.発明の分野)
本発明は、一般に、体内の診断要素および治療要素を位置決めするための構造物に関し、さらに特には、心臓の状態の処置に特に良く適したデバイスに関する。
診断要素および治療要素が体内に挿入しなければならない事例の多くがある。1つの事例は、心臓の状態(例えば、不整脈と呼ばれる、不快な、不規則な心拍の原因となる心房細動および心房粗動)の処置に関連する。
本発明に従う組織凝固デバイスは、シャフト、少なくとも1つのエネルギー伝達デバイス、およびエネルギー伝達デバイス冷却装置を備える。このようなデバイスは、従来の損傷生成デバイスを超える多くの利点を提供する。例えば、このエネルギー伝達デバイス冷却能力は、本デバイスが、従来のデバイスよりもより広くより深い損傷を形成することを可能にする。
以下は、本発明を実施する最良の現在公知の様式の詳細な説明である。この説明は、意味を限定して理解されるべきではなく、ただ、本発明の一般的な原理を例証する目的で成される。

この詳細な説明の節の表題および全体の組織は、ただ便宜上の目的のためであって、本発明を制限することを意図しない。
図1〜図5に例として示すように、本発明の好ましい実施形態に従う外科的プローブ100は、比較的短いシャフト102、ハンドル104、およびこのシャフトの末端部分に1つ以上の電極106または他のエネルギー伝達デバイスを備える[この電極106は、以下の節IIIにおいて、より詳細に記載される]。張力緩和要素(strain relief element)108がまた、提供され得る。シャフト102は、好ましくは、必ずしも必要ではないが、比較的硬く約12.7cm〜50.8cmの長さであって、最も好ましくは、約20.3cm〜30.5cmの長さである。図1〜図5に示された実施形態において、シャフト102は、マンドレル(mandrel)110および電極106が支持されている非導電性の外部構造物112からなる。この外部構造物112の外径は、好ましくは約1.7mm〜約4mmの間であるが、特定の用途に合わせて変動し得る。マンドレル110の近位端はハンドル104に固定され、一方、遠位端はチップ部材114を部分的に通過し、そしてこれに固定される。次に、このチップ部材114は、接着剤113または他の適切な手段を用いて外部構造物112の遠位端に固定される。このチップ部材114は、好ましくは、意図されない組織外傷を防ぐために遠位端が比較的柔軟であり、そしてそこに縫合材が装着されるのを可能にする窪み部(indentation)115を備える。電極または他のエネルギー伝達デバイスを冷却するためのシステムはまた、以下の節IIでより詳細に記載され、提供される。

WとLは等しいと仮定して異なるシャフトを比較する場合、それぞれのE値およびI値は、どの程度シャフトが屈曲するかを決定する。言い換えれば、シャフトの硬さは、EとIの積の関数である。この積は、本明細書中で「屈曲率」といわれる。Eはシャフトを形成する材料の特性であり、一方、Iはシャフト形状の関数である。従って、より軟らかいシャフトの慣性モーメントが、より硬いシャフトの慣性モーメントよりも十分に大きい場合、比較的軟らかい材料から形成されたシャフトは、比較的硬い材料から形成されたシャフトと同じ屈曲率を有し得る。
本明細書中に開示される電極冷却装置は、凝固手順の間、電極(または他のエネルギー伝達デバイス)を冷却するために流体を使用する。ガス冷却流体が使用され得るが、液体が好ましい。以下により詳細に記載されるように、エネルギーが電極から組織に伝達される一方で、電極の熱は、この電極を冷却するために流体に伝達される。凝固手順の間の電極の冷却は、損傷の形成を促進する(すなわち、本冷却装置を欠く別の同じデバイスで実現され得る損傷の形成よりも、より広く深い)。
流体注入口管腔116および放出口管腔118が、外部構造物112において可能な限り大きな横断面積および周囲面積を占めることを可能にするために、出力ワイヤー管腔120および信号ワイヤー管腔122は、出力ワイヤー154および信号ワイヤー160を接続するのにちょうど十分な大きさであるべきである。流体管腔116および118の幅(すなわち、外壁126と内壁128との間の距離)は、外壁126の厚さの少なくとも2倍であるべきであって、好ましくは外壁の厚さの4倍であるべきである。外部構造物112の外径が約2.67mmであって、外壁126の厚さが約0.10mm〜約0.25mmである場合の実施において、流体管腔116および118の幅は、好ましくは約0.51mm〜約1.02mmである。
示された実施形態の各々において、RFエネルギーを伝達するために適合された間隔を空けた複数の電極が使用される。しかしながら、超音波振動子およびマイクロ波電極のようなデバイスが、この電極の代わりになり得る。
上で示されたような外科的なデバイスは、例えば、限局性の(または異所性の)心房細動の原因を隔離するための貫壁性の心外膜損傷の生成に使用され得る。心臓へのアクセスは、開胸術、胸部造痩術または胸骨正中切開術によって達成され得る。ポート(port)はまた、カメラおよび他の機器のために設けられ得る。開心術の間に形成された損傷に関して、1つの例示的な損傷は、心臓へのアクセスを得るために使用される切開から、僧帽弁の弁輪(mitral valve annulus)または他の解剖学上の障壁に向かって延び、切開周囲の再入可能な伝播(reentrant propagation)のための電位を下げる。肺静脈の周囲の損傷もまた生成され得る。例示的な非心臓性の使用としては、腫瘍の処置が挙げられる。ここで、プローブの遠位部分は、腫瘍に挿入され、次いで、炭化することなく腫瘍全体を均一に凝固するために使用される。
Claims (27)
- 軟組織凝固デバイスであって、以下:
遠位端を規定するシャフトであって、熱伝導率が少なくとも0.8W/m・Kでありかつ実質的に非導電性の材料から形成される外部構造物を備える、シャフト;
該シャフトの遠位端に対して間隔をあけた関係で該外部構造物に支持される、少なくとも1つのエネルギー伝達デバイス;ならびに
該外部構造によって規定される少なくとも1つの流体管腔であって、該少なくとも1つの流体管腔の一部分が該少なくとも1つのエネルギー伝達デバイスと整列するように設置される、少なくとも1つの流体管腔;
を備える、軟組織凝固デバイス。 - 請求項1に記載のデバイスであって、前記シャフトは比較的短い、デバイス。
- 請求項1に記載のデバイスであって、前記シャフトの少なくとも一部は比較的硬い、デバイス。
- 請求項3に記載のデバイスであって、前記シャフトは、順応性のマンドレルを備え、そして前記外部構造物は、該順応性のマンドレルに取付けられる、デバイス。
- 請求項3に記載のデバイスであって、ここで、前記シャフトは、遠位端を規定する管状部材を備え、そして前記外部構造物は、該管状部材の遠位端から遠位に延びる、デバイス。
- 請求項1に記載のデバイスであって、ここで、前記シャフトは、近位部分および遠位部分を備え、該デバイスは、さらに以下:
該近位部分に対して該遠位部分を変形させるステアリング装置、
を備える、デバイス。 - 請求項1に記載のデバイスであって、前記シャフトは予め屈曲している(pre−bent)部分を備える、デバイス。
- 請求項1に記載のデバイスであって、前記少なくとも1つの流体管腔は、注入口管腔および放出口管腔を備える、デバイス。
- 請求項8に記載のデバイスであって、前記注入口管腔および放出口管腔は、各遠位端を規定し、該デバイスは、以下:
該注入口管腔の遠位端と該放出口管腔の遠位端とを接続する管腔を規定する非導電性のチップ部材、
をさらに備える、デバイス。 - 請求項1に記載のデバイスであって、前記少なくとも1つの流体管腔は、内部管腔表面および外部管腔表面を備え、該内部管腔表面および外部管腔表面は、それらの間に一定の距離を規定し、前記外部構造物は、壁を備え、該壁は、前記少なくとも1つのエネルギー伝達デバイスと該少なくとも1つの流体管腔との間の壁の厚さを規定し、該内部管腔表面と該外部管腔表面との間の該距離は、該壁の厚さより大きい、デバイス。

- 請求項1に記載のデバイスであって、前記少なくとも1つのエネルギー伝達デバイスは、電極を備える、デバイス。
- 請求項1に記載のデバイスであって、ここで、前記外部構造物は周辺部を規定し、前記少なくとも1つのエネルギー伝達デバイスは、完全な周辺部に満たない周囲に延び、前記少なくとも1つの流体管腔は、注入口管腔および放出口管腔を備え、そして該注入口管腔は、少なくとも1つの該エネルギー伝達デバイスの実質的な部分と該放出口管腔との間にある、デバイス。
- 請求項13に記載のデバイスであって、前記放出口管腔は、断熱材を備える、デバイス。
- 軟組織凝固デバイスであって、以下:
遠位端を規定するシャフトであって、実質的に非導電性の材料から形成される外部構造物を備える、シャフト;
該シャフトの遠位端に対して間隔をあけた関係で該外部構造物に支持される、少なくとも1つのエネルギー伝達デバイス;
流体注入口管腔であって、ある厚さを有する壁が該流体注入口管腔と該少なくとも1つのエネルギー伝達デバイスとの間にあるように外部構造物によって規定され、該流体注入口管腔の一部分が該少なくとも1つのエネルギー伝達デバイスと整列するように設置され、そして内部管腔表面および外部管腔表面を備え、該内部管腔表面および外部管腔表面は、該壁の厚さよりも大きい距離をそれらの表面の間に規定する、流体注入口管腔;ならびに
該外部構造物により規定される流体放出口管腔であって、該流体放出口管腔は、該注入口管腔中と該放出口管腔とで流体が反対方向に移動するように、該流体注入口管腔と作動可能に接続されている、流体放出口管腔;
を備える、軟組織凝固デバイス。 - 請求項15に記載のデバイスであって、前記シャフトは比較的短い、デバイス。
- 請求項15に記載のデバイスであって、前記シャフトの少なくとも一部は比較的硬い、デバイス。
- 請求項15に記載のデバイスであって、ここで、前記シャフトは、順応性のマンドレルを備え、そして前記外部構造物は、該順応性のマンドレルに取付けられる、デバイス。
- 請求項15に記載のデバイスであって、ここで、前記シャフトは、遠位端を規定する管状部材を備え、そして前記外部構造物は、該管状部材の遠位端から遠位に延びる、デバイス。

- 請求項15に記載のデバイスであって、ここで、前記外部構造物は周辺部を規定し、前記少なくとも1つのエネルギー伝達デバイスは、完全な周辺部に満たない周囲に延び、前記注入口管腔は、少なくとも1つの該エネルギー伝達デバイスの実質的な部分と前記放出口管腔との間にある、デバイス。
- 請求項25に記載のデバイスであって、前記放出口管腔は、断熱材を備える、デバイス。
- 請求項15に記載のデバイスであって、前記内部管腔表面と外部管腔表面との間の距離は、前記壁の厚さの少なくとも2倍の大きさである、デバイス。
