本発明は、骨格筋弛緩剤として有用なメシル酸プリジノールを含有する経皮吸収性に優れたメシル酸プリジノール含有外用剤に関する。

【背景技術】
全身作用を目的とした薬物投与法としては、一般には経口投与や注射による場合がほとんどである。経口投与では肝臓での代謝(初回通過効果)が避けられず、注射による投与では生物学的半減期の短い薬物は頻繁な投与が必要となり、薬物血中濃度の維持が困難である。そこで薬物を皮膚から浸透吸収させることによって投与する治療システムが、新しい薬物投与法として注目されている。経皮投与の場合、先の欠点が克服され、また、投与回数の軽減、コンプライアンスの向上、投与の中断が容易などの利点を有している。

メシル酸プリジノールは、1,1-Diphenyl-3-piperidinopropanol methanesulfonate(分子式:C20H25NO・CH3SO3H)の化学名を有する、中枢性、特に脊髄内の介在ニューロンに主作用を持つ筋異常緊張緩解剤として慣用されている医薬物質である。
メシル酸プリジノールは、通常経口投与剤及び注射剤として多く用いられているが、経口投与剤は通常1日3回の投与が必要とされており、食欲不振、胸やけ、胃のもたれ・胃部不快感などの胃腸障害や、肝・腎機能障害等の副作用が問題となっている。
他方、注射剤では、投与時の疼痛を伴い、また血中濃度の急激な上昇によるふらつき等の副作用が発現し易いという欠点を有している。

そこで最近、水溶性重合体にメシル酸プリジノールを含有せしめた骨格筋弛緩経皮吸収剤(特開平6-336434号公報参照)が提案されているが、かかる経皮吸収剤によっても期待される有効血中濃度を十分に維持することができず、これ以上の血中濃度の上昇は期待できない。また、飽和濃度以上のメシル酸プリジノールが含有されているため、メシル酸プリジノールの結晶析出が起こり、これに伴って粘着性が低下し、使用感も悪くなるという問題点を有している。
したがって、経皮吸収性に優れ、有効血中濃度を長時間維持し、かつ使用が簡便で、副作用の少ないメシル酸プリジノール含有外用剤の開発が望まれている。

【発明の開示】
本発明者らは、このような状況において鋭意研究を重ねた結果、メシル酸プリジノールにグリセリンとベンジルアルコールとを配合せしめることにより、経皮吸収性並びに持続性に優れ、かつ使用感に優れたメシル酸プリジノール含有外用剤の開発に成功した。
すなわち、本発明は、メシル酸プリジノール、グリセリン、ベンジルアルコール及び外用基剤を含むメシル酸プリジノール含有外用剤(但し多官能性エポキシ化合物を含む貼付剤を除く)を提供するものであり、さらに本発明は、かかる外用剤の好ましい剤形のものとして貼付剤のものであるメシル酸プリジノール含有外用剤を提供するものである。

以下、本発明を詳細に説明する。
一般に外用剤には、グリセリン、プロピレングリコール、エチレングリコール、1,3ーブチレングリコール等のグリコール類が安定化剤、可塑剤、可溶化剤、湿潤剤、軟化剤、粘着剤、賦形剤、分散剤、防湿剤、保存剤、溶剤、溶解剤などの幅広い用途を目的として配合されており、製剤開発において重要な成分である。特に貼付剤ではグリコール類は必須成分とされている。
このグリコール類の中で、プロピレングリコール及び1,3−ブチレングリコールは、上記配合目的のほかそれ自身に薬物の経皮吸収促進効果があり、他の経皮吸収促進剤と組み合わせるとさらに経皮吸収促進効果を増強させる作用、すなわち併用効果を有することが報告されているが(B.W.Barry, Journal of Controlled Release, 6(1987) 85-97等参照)、同種のグリコール類であっても、グリセリンは経皮吸収促進効果及び併用効果を期待し得ないと考えられており、実際にこのような効果を得たとの報告は見当たらない。

本発明は、メシル酸プリジノールの経皮吸収促進性は、一般に経皮吸収促進剤との併用効果が高いと言われているプロピレングリコールや1,3−ブチレングリコールとの併用では格別認められないにも拘らず、併用効果が低いと考えられているグリセリンを併用することにより予想を越えて高い吸収促進作用を奏することを見出したものである。
即ち、本発明は、経皮吸収促進剤であるベンジルアルコールとグリセリンとを併用することにより、プロピレングリコールや1,3−ブチレングリコールとの併用では得られないメシル酸プリジノールの高い経皮吸収性が得られることを見出したものである。

本発明に係る外用剤におけるグリセリンの配合割合は、1〜50重量%(以下、本明細書においては単に%で表示する)、好ましくは5〜30%、更に好ましくは10〜25%で配合される。グリセリンの配合量が多いと、メシル酸プリジノールの経皮吸収性が低下する傾向がみられる。一方、配合量が少ないと、製剤化に際し粉体成分の分散性が悪く、作業性並びに均一性に欠けるという問題が生じる。また、剤形が貼付剤のものについては可塑性が悪くなるため、使用感の良好な貼付剤が得られない。また、グリセリンは他のグリコール類と比較して低刺激性であるという利点を有する物質である。

他方、ベンジルアルコールの配合割合は、0.1〜50%、好ましくは1〜20%、更に好ましくは5〜10%で配合される。ベンジルアルコールの配合量が多くなると、ベンジルアルコール自身の相分離や、浮き出しが生じ易くなる。また、配合量が少ないとメシル酸プリジノールの均一性が悪くなり、経皮吸収性を低下せしめる原因となり好ましくない。またベンジルアルコールは溶解補助剤として作用し、メシル酸プリジノールを製剤中に均一に溶解、分散させ、結晶の析出を抑える作用をなしているものと考えられる。

本発明に係る外用剤には、一般に使用されている他の吸収促進剤としてユーカリ油、モノカプリル酸プロピレングリコール、ハッカ油、アジピン酸ジイソプロピル、ラウリルアルコール、ミリスチン酸イソプロピル、オレイン酸等を配合することもできる。
メシル酸プリジノールの経皮吸収性は、pHの高いほど良好であるが、製剤のpHは、メシル酸プリジノールの経皮吸収性と製剤化の便宜さを考慮してpH5〜8の範囲内で調製することが好ましい。

本発明に係る外用剤は、一般に外用製剤として公知の剤形のものに製剤化することができる。かかる製剤の剤型としては、貼付剤、パッチ剤、テープ剤、軟膏、ゲル、クリーム剤、液剤等が挙げられる。

本発明に係る外用剤に添加される外用基剤としては、特に制限されず、通常使用されている水溶性高分子、界面活性剤、安定化剤、pH調整剤等を挙げることができる。
水溶性高分子としては、例えば、ポリアクリル酸、ポリアクリル酸ナトリウム、カルボキシビニルポリマ−、カルメロースナトリウム、ポリビニルアルコール、ポリビニルピロリドン、ヒドロキシプロピルセルロース、ヒドロキシエチルセルロース、エチルセルロース、アルギン酸、アルギン酸ナトリウム、ゼラチン、メチルビニールエーテル・無水マレイン酸共重合体等いずれも使用可能である。

実験方法:
実施例2、比較例1及び比較例2に記載した方法により調製した貼付剤をアルミラミネート包材で密封し、60℃の恒温槽中で2週間保存した後、膏体表面の外観変化を観察した。その結果を表3に示す。

比較例1及び比較例2の貼付剤についてはいずれにもメシル酸プリジノールの結晶の析出が認められたが、本発明における実施例2の貼付剤においては結晶の析出は全く見られなかった。

試験例2
実施例1、実施例2、比較例1及び比較例2に記載の方法により調製した各貼付剤につき、下記実験方法により皮膚透過性の比較試験を行った。その結果を図1に示す。
実験方法:
実験前日に7週齢、雌性ヘアレスラット(平均体重135g)の背部被毛を電気バリカンで除毛した。 前日除毛したヘアレスラットを頚椎脱臼した後、背部皮膚を摘出した。摘出皮膚をフランツ型拡散セル(直径2cm)に固定し、ドナー側にそれぞれ実施例1、2と比較例1、2の貼付剤(メシル酸プリジノールとして約1.3mg(実施例1、比較例1)と約2.7mg(実施例2、比較例2))を貼付し、レセプター側に生理食塩水を満たした。実験中レセプター側は37℃を保ち、マグネチックスターラーで撹拌した。
所定時間ごとにレセプター側から一定量を採取し、高速液体クロマトグラフィーを用いてメシル酸プリジノールの透過量を測定した。

図1から明らかなように、本発明品は各比較例のものと比較して、薬物濃度の上昇に伴い透過性の上昇が見られ、高い経皮吸収性が認められた。これに比し、比較例では薬物濃度0.5%(比較例1)で既に飽和濃度に達しているため、薬物濃度1.0%(比較例2)にしてもこれ以上の透過性の上昇は見られず、本発明品と比較して経皮吸収性は低かった。

実施例2及び比較例2に記載の方法により調製した各貼付剤につき、吸収動態を測定した。
実験方法:
実験前日に8週令、雌性ヘアレスラット(平均体重136.5g)の背部被毛を電気バリカンで除毛し、さらに絶食を行った。実施例2及び比較例2の貼付剤(4×2.5cm)1枚(メシル酸プリジノールとして約7mg)を前日除毛したラット背部皮膚に正中線を避けて右側に貼付し、その上を固定用粘着テープで固定し、さらに体幹部を包み込むようにサージカルテープ(3M)を貼付して固定した。 薬剤投与4、8、および16時間後にエーテル麻酔下において腹部下大静脈より血液4mlを採血した。血液を遠心分離(3000rpm、20分)後、血清0.75mlをとり、水酸化ナトリウムでアルカリ性とし、石油エーテルを用いてメシル酸プリジノールを抽出した後、高速液体クロマトグラフィーを用いて血清中の濃度を測定した。

薬物動態を示す各測定値を表4に示す。本発明の貼付剤は16時間後まで安定した血中メシル酸プリジノール濃度を維持し、また吸収促進剤の影響により比較例2と比較してCmaxおよびAUCが上回っており、高い経皮吸収性が認められた。

実施例3、比較例3及び比較例4に記載した方法により調製した各ゲル剤につき、試験例2に記載の方法により、皮膚透過性の比較試験を行った。また、同時に外観の観察及び塗擦感の比較を行った。皮膚透過性試験の結果を図2に示す。
結果:
図2から明らかなように、比較例4のグリセリンのみでは経皮吸収性に劣っており、比較例3のベンジルアルコール単独でも促進効果が見られたが、両者を組み合わせて配合された本発明品は高い経皮吸収性を維持することが認められた。
また、外観の観察及び塗布感の比較の結果、比較例3ではベンジルアルコールの浮き出しが見られ、塗擦感が悪いが、本発明品(実施例3)は、グリセリンを配合することによりベンジルアルコールの分散性が良くなり、またベンジルアルコールの浮き出しも認められず、塗擦感の改善が認められた。

図3から明らかなように、本発明品のベンジルアルコールとグリセリンとを組み合わせて配合された液剤には、比較例で示した一般に経皮吸収促進作用があると言われているプロピレングリコールや1,3−ブチレングリコールとの組み合わせによる液剤と比較して極めて高い経皮吸収性が認められた。
