この発明は、人手を借りずに患者を寝返りさせて褥瘡を防止できるベッドに関する。

【従来の技術】
寝床で同一体位を長時間保持したまま患者を放置しておくと、持続的に圧迫を受ける部位に褥瘡が発生し易い。褥瘡の最も有効な予防方法は体位の変換であり、これを少くとも2時間ごとに、或は最低1日に15回行なう必要がある。通常この体位の変換は、複数人で患者を持ち上げて患者に苦痛を与えないよう細心の注意を払いながら行なわなければならず、大変な労力を要している。

【発明の課題】
そこで、マットレスと共に床が縦に折れて傾くようにした所謂ローリングベッドが現れている。しかしながら、このベッドは患者がその上に寝たままの状態でベッドを傾けるだけであって、人体がベッドに圧着した状態は変らない。しかしながら褥瘡の予防や治療には、褥瘡が発生する部分に空気を流通させることが大切である。

従ってこの発明の課題は、患者の体位を変換することができると同時に身体のベッドへの圧着を解いて空気を流通させることができるようにすることである。

【課題の解決手段】
上記の課題を解決するために、この発明においては、一対のサイドフレームに昇降可能のガードフレームを取り付け、それぞれのガードフレームを昇降させる手段を設け、前記ガードフレームの上端に、患者を寝かせるシーツの側縁部を係止する手段を設け、一方のガードフレームの上昇によって前記シーツの片側を持ち上げ患者の体位を変換可能としたのである。また、患者の体位を変換する期間とその期間内の体位変換回数または時間間隔を入力して前記ガードフレームの昇降手段を駆動させる制御部を設けておくのがよい。

前記ガードフレームの昇降手段を、リンクで形成されたパンタグラフ機構と、このパンタグラフ機構のリンクの下端を接近又は離反させる駆動手段によって構成することができる。

また、前記ガードフレームの昇降手段を、ガードフレームにシリンダロッドが連結されたシリンダによって構成してもよい。

【実施の形態】
以下、この発明の実施形態を添付図面に基づいて説明する。図1及び図2に示すように、端板1、2の下縁には、一対のキャスタ1a、2aが取り付けられてベッドが移動可能になっており、これらの端板1と2は、チャンネル形のサイドフレーム3、3によって連結されている。そして、サイドフレーム3、3の間に床板4が支持されている。

上記それぞれのサイドフレーム3の上面中央部には、サイドフレーム3の長さ方向に支持バー5が取り付けられ、この支持バー5には、ベッドの両側から人体が落下するのを防止するガードフレーム5aが固定されている。勿論、このガードフレーム5aは、支持バー5に着脱自在に取り付けることができる。

前記サイドフレーム3には、その長さ方向に沿って回転軸6が取り付けられ、この軸6はモータ7によって回転駆動される。この回転軸6には、両端部に正ねじ6aと逆ねじ6bが形成され、このねじ6a、6bにパンタグラフ機構8、8が取り付けられている。それぞれのパンタグラフ機構8を形成するリンクの下端には、ねじ6a、6bと係合するナット8a、8bが取り付けられ、前記リンクは、これらのナット8a、8bに、ベッドの側面と並行な垂直面間で回転自在に連結され、リンクの上端は、ベッドの側面と並行な垂直面間で回転自在に支持バー5に連結されている。従ってモータ7で軸6を回転駆動すると、ナット8a、8bは互に接近し、パンタグラフ機構8によって支軸バーと共にガードフレーム5aが鎖線のように上昇し、モータ7を逆回転すると、ナット8a、8bは離反し、ガードフレーム5aが実線の位置まで下降する。

上記のようなガードフレーム5a、5aの昇降機構の使用方法を図3に基づいて説明する。まずマットレスMの上に敷いたシーツSの両側をクリップなどの係止具Cでガードフレーム5a、5aの上端部にしっかりと止着して、図の実線で示すように、シーツSをガードフレーム5a、5aの上端から円弧状に垂れ下った状態に支持する。そして、患者HはシーツSの上に寝かせられ、体位変換が必要なときに、前述のようにして一方のモータ7を駆動し、一方のガードフレーム5aを上昇させると、それにつれてシーツSが鎖線のように引張られるため、人体Hも鎖線のように片側がシーツSと共に持ち上げれて角度が変る。同様に、他方のモータ7を駆動し、他方のガードフレーム5aを上昇させると、人体Hの他側が持ち上げられて臥位が変化する。このように、シーツSを持ち上げて体位を変換させるため、シーツSとマットレスMとの空隙に空気が流通し、汗や湿気を除くと同時に皮膚呼吸を可能にして褥瘡を有効に防止する。なお、図3の場合は、仰臥位から側臥位への体位変換を示しているが、当初の体位が側臥位であっても、勿論同様に体位変換が可能である。

上述の体位変換を自動的に制御することが可能である。例えば、図4に示すようなマイクロコンピュータから成る制御部10を設け、体位変換を行なう期間(例えば午後10時から翌朝6時まで)、その期間内の体位変換回数又は時間間隔等を操作キー11で制御部10に入力し、制御部10は、組込まれたソフトウェアにより入力値に応じてモータ7、7を駆動して体位変換を行なう。勿論操作キー11には、任意の時にモータ7、7を作動できるキーを設けておくことができる。

なお、ガードフレーム5a、5aの昇降機構は、前記実施形態に限らず、例えば小形の油圧ポンプで作動する油圧シリンダのシリンダロッドをガードフレーム5a、5aに連結して昇降させるようにすることもできる。

図5に示すように、床板4を複数の屈曲部4a、4b、4cで折り曲げ可能とし、適当なリンク手段12a、12bを駆動手段(図示せず)によって図の矢印方向に押すと、床板4は、鎖線のように起立し、逆にリンク手段12a、12bを引張ると床板4は平坦になる。このような起伏機構を前記ガードフレーム5a、5aの昇降機構と共にベッドに装備しておくことができる。

この発明によれば、以上のように、ベッドのシーツを持ち上げる昇降機構を設け、このシーツの持ち上げと共に体位を変換するようにしたので、人体とマットレスとの間に隙間が生じて、湿気や汗などが散逸し易く、さらに皮膚呼吸も可能となって褥瘡の予防及び治療に極めて有効である。
