Description

【0001】

BACKGROUND OF THE INVENTION
The present invention relates to a distal end portion of an ultrasonic endoscope that can be inserted into a body and obtain an ultrasonic tomographic image while performing optical observation.
[0002]
[Prior art]
An ultrasonic endoscope generally includes an ultrasonic probe for transmitting and receiving an ultrasonic signal from the distal end portion of an insertion portion and performing sector scanning, and an optical observation window for optically observing a direction scanned by the ultrasonic signal. A treatment instrument protrusion for projecting the treatment instrument in the direction scanned by the ultrasonic signal is disposed (for example, Patent Document 1).
[0003]
[Patent Document 1]
JP-A-3-131243
[Problems to be solved by the invention]
In general, an ultrasonic endoscope guides an insertion portion to a target position in the body while performing optical observation, and then punctures an organ or an affected part deep inside the mucosal surface while observing an ultrasonic tomographic image. Thus, a unique treatment that cannot be performed with a normal endoscope can be performed.
[0005]
However, the treatment using the injection needle in such a manner can only inject or discharge the drug solution or body fluid, and at the same time, other treatment tools cannot be used under the observation of the ultrasonic tomographic image. For some people, the procedure was limited and it was not possible to perform the treatment as intended.
[0006]
Accordingly, the present invention provides an ultrasonic endoscope that enables a surgeon to select a procedure suitable for a case and perform a desired treatment by enabling a plurality of treatment tools to be used simultaneously while observing an ultrasonic tomogram. It aims at providing the front-end | tip part.
[0007]
[Means for Solving the Problems]
In order to achieve the above object, the distal end portion of the ultrasonic endoscope of the present invention is scanned by an ultrasonic probe for transmitting and receiving an ultrasonic signal from the distal end portion of the insertion portion and performing sector scanning, and the ultrasonic signal. In the distal end portion of the ultrasonic endoscope in which an optical observation window for optically observing the direction to be measured and a treatment instrument projection port for projecting the treatment instrument in the direction scanned by the ultrasonic signal are disposed A plurality of treatment instrument projection openings for projecting the treatment instrument along a plane in which an ultrasonic signal transmitted and received from the ultrasound probe is scanned are arranged as the instrument projection openings.
[0008]
In addition, when at least one of the plurality of treatment instrument protrusions is provided with a treatment instrument raising piece for changing the projection direction of the treatment instrument projected therefrom along the plane, one treatment instrument protrusion The protrusion angle of the treatment tool protruding from the mouth can be arbitrarily changed with respect to the treatment tool protruding from the other treatment tool protrusion port, so that a state convenient for the desired treatment can be obtained.
[0009]
DETAILED DESCRIPTION OF THE INVENTION
Embodiments of the present invention will be described with reference to the drawings.
1, 2 and 3 are a perspective view, a plan view and a side sectional view (III-III sectional view in FIG. 2) of the distal end portion of the ultrasonic endoscope according to the first embodiment of the present invention. However, the state in which the treatment tools 100 and 200 protrude from the treatment tool protrusions 6 and 7 is described only in FIG.
[0010]
Convex type that emits and receives ultrasonic signals so that the side of the tip portion main body 2 is sector-scanned (fan-shaped scan) in the front half of the tip portion main body 2 connected to the tip of the flexible insertion portion 1. An ultrasonic probe 3 is attached.
[0011]
The ultrasonic scanning range U by the ultrasonic probe 3 is on a plane along the axis of the tip end main body 2 (that is, on the plane that includes the axis of the tip end main body 2 or is close to and parallel to the axis). Yes, an alternate long and short dash line indicates a position where the plane intersects the surface of the ultrasonic probe 3 (hereinafter referred to as plane A).
[0012]
An inclined surface 20 facing the ultrasonic scanning range U of the ultrasonic probe 3 is formed on the rear half of the tip body 2, and optical observation for optical observation of the direction of the ultrasonic scanning range U is performed. A window 4 and an illumination window 5 for illuminating the observation range are arranged side by side on the slope 20.
[0013]
Further, on the inclined surface 20 of the distal end portion main body 2, a first treatment instrument projection port 6 and a second treatment instrument projection port 7 for projecting the treatment instrument in the direction of the ultrasonic scanning range U are provided with a plane A. Two are arranged side by side on the crossing line.
[0014]
The distal ends of the first treatment instrument insertion channel 60 and the second treatment instrument insertion channel 70, which are inserted and arranged in the insertion portion 1, are provided at the first treatment instrument protrusion 6 and the second treatment instrument protrusion 7, respectively. The tip end body 2 is connected at the rear end position.
[0015]
Then, the distal end portions of the first treatment tool (for example, injection needle) 100 and the second treatment tool (for example, forceps) 200 passed through the treatment tool insertion channels 60 and 70 are ultrasonically scanned along the plane A. It protrudes from each treatment instrument protrusion port 6 and 7 in the range U.
[0016]
Therefore, the surgeon can use the plurality of treatment tools 100 and 200 simultaneously while observing them in the ultrasonic tomographic image with the monitor, and can select the procedure suitable for the case and perform the desired treatment. .
[0017]
4 and 5 are a plan view and a side cross-sectional view (cross-sectional view taken along line VV in FIG. 4) of the distal end portion of the ultrasonic endoscope according to the second embodiment of the present invention. The state in which the treatment tools 100 and 200 protrude from 7 is shown only in FIG.

In this embodiment, the treatment tool raising piece 8 for changing the protruding direction of the first treatment tool 100 protruding from the first treatment tool protrusion 6 along the plane A within the ultrasonic scanning range U. However, it is arrange | positioned at the 1st treatment tool protrusion port 6 part.
[0019]
The treatment instrument raising piece 8 is rotatably supported by the distal end portion body 2 by a support shaft 9, and advances and retreats in the axial direction by an operation from an operation portion (not shown) connected to the proximal end of the insertion portion 1. The distal end of the operation wire 10 is connected to the treatment instrument raising piece 8.
[0020]
Therefore, by moving the treatment instrument raising piece 8 around the support shaft 9 by remote operation via the operation wire 10, the protrusion angle of the first treatment instrument 100 protruding from the first treatment instrument protrusion 6 is changed. The second treatment tool 200 protruding from the second treatment tool protrusion 7 can be arbitrarily changed along the plane A within the ultrasonic scanning range U. The treatment instrument raising piece 8 may be disposed on the second treatment instrument protrusion 7 side, or may be disposed on both the first treatment instrument protrusion 6 and the second treatment instrument protrusion 7. .
[0021]
In addition, this invention is not limited to said each Example, For example, you may arrange | position three or more treatment tool protrusions.
[0022]
【The invention's effect】
According to the present invention, a plurality of treatment instrument protrusions for projecting a treatment instrument along a plane in which an ultrasonic signal transmitted and received from an ultrasonic probe is scanned are arranged side by side as treatment instrument protrusions. Since a plurality of treatment tools can be used at the same time while observing an ultrasonic tomographic image, the surgeon can select a procedure suitable for the case and perform a desired treatment.
