Description
1.発明の分野
本発明は一般に、患者の体温の選択的な修正及び制御に関する。特に、本発明は、患者の血管の中へカテーテルを挿入し、血管の中を流れる血液から又は血液に選択的に熱を伝達することによって、低体温症又は高体温症を治療するための方法及び装置を提供する。
2.背景技術の説明
通常の環境のもとでは、人体の温度調節システムは、約37℃(98.6°F)のほぼ一定な温度を維持する。環境へ失われた熱は、体内で生じた熱によって正確にバランスされる。
低体温症は、異常に低い体温の状態である。低体温症は臨床上35℃又はそれ以下の核心体温として定義されている。低体温症は、更にその重さによって時に特徴づけられる。32℃から35℃の範囲の核心体温は、軽度の低体温症と評され、30℃から32℃は中度と呼ばれ、24℃から30℃は重度と評され、そして、24℃以下の体温は超低体温症を構成する。上記の範囲は議論に有用な基礎を与えるが、それらは絶対的なものではなく、かつ、定義は医学文献で大きく変わる。
偶発性低体温症は、環境へ失われた熱が内部で熱を生じさせる身体の能力を越えたときに起こる。多くの場合、温度調節及び熱の発生は正常であるが、患者は圧倒的な寒冷環境のストレスのために低体温症になる。これは比較的普通の状態で、しばしば風雨に晒すことから起こる。低体温症は、外傷又は疾病により温度調節能力が低下している患者が、軽い寒冷ストレスに晒されても起こる。例えば、この種の低体温症は外傷を負った患者又は、手術を受けている患者に合併症として時々起こる。
いずれの型の低体温症も、重大な医学的結果をもつことがある危険な状態である。特に、低体温症は心臓の血液を送り出す能力を阻害する。低体温症はこの理由のみで致命的であるかもしれない。加えて、低体温は血液凝固に必要な酵素反応を強く阻害する。これは、正常な凝固因子レベルがあるときでも、時々、抑制することが非常に難しい出血に至る。低体温症のこれらの影響及びその他の不利な結果は、外傷の被害者及び手術中の患者の死亡率を劇的に増加させる。
低体温症を治療するための簡単な方法が、非常に古くから知られてきた。その方法は、患者を毛布で包み、口から温かい液体を飲ませ、そして患者を湯水浴につけことからなる。これらの方法は軽度の低体温症には非常に効果的だが、重度及び超低体温症を治療するためのより侵入的な方法が開発されてきた。特に、患者の血液の直接加熱を行う方法が考案されてきた。最も一般的には、血液を患者の血液循環から引き出し、外部の加温装置に通し、患者に再び戻す。別のやり方として、加熱されたカテーテルの使用が提案されており、遠位端の近くに加熱素子を有するカテーテルを患者の脈管に挿入し、熱を患者の循環する血液へ直接伝達する。
患者の血液の直接加熱は、重度及び超低体温症の場合においても、極めて効果的ではあるが、熱の過剰な伝達により、患者の温度を正常な体温以上に上昇させ、高体温になることが、発明者により観察されている。高体温は、例えば、血液に熱が直接伝達されるのと同時に、低体温症の患者の代謝作用が相当な量の熱を生み出し始めたときに起こる。
従って、偶発的又は付随的な高体温症の治療にも備える、低体温症を治療するための方法を提供することが望しい。特に、患者の血液温度又は体温が目標レベルを越えたとき、選択的に熱を奪うことができる血液に熱を伝達するための装置及び方法を開発することが望ましい。そのような方法及び装置は、循環する血液への直接的な熱伝達のためのカテーテルを使用することが好ましいが、血液を患者の身体の外部で加熱する方法も有用である。かかる装置及び方法は初め高体温症である患者治療に有用であり、その方法及び装置は、血液の初期の冷却に備え、そして、患者の血液温度又は体温が目標温度より下に下降すれば、付加的に血液の加熱に備える。
発明の概要
本発明は、初めに低体温症又は高体温症を患っている患者の正常な体温を回復させるための装置及び方法を提供する。その装置はカテーテル及び制御ユニットからなり、それらは、患者の循環する血液の選択的な加熱及び冷却を一緒になって可能にする。その方法は、低体温症の患者に対しては、目標の血液温度又は体温が回復するまで、初めに血液を加熱する。目標温度に達した後、加熱を停止する。しかしながら、加熱を停止した後でも、患者の血液温度及び/又は体温はモニターされ続けて、目標温度を行過ぎないようにする。上で論じたように、患者の代謝作用及び外部加熱の両方から受けた熱の全量が、平熱を回復するのに必要な量を越えたならば、初め低体温症の患者は高体温になることがある。高体温に入る患者の場合には、本発明の方法は通常、同じ脈管内カテーテル又は加熱用に使用された他の装置を使用して患者の血液を冷却する。
初め高体温症の患者の場合には、本発明の方法は、血液温度及び体温を下げるために患者の血液を冷却する。冷却は、目標温度に達した後、停止する。しかしながら、患者の血液温度及び/又は体温をモニターし続け、患者が低体温に入ったならば、適切な量の熱を循環する血液に導入することによって正常な体温を回復することができる。
本発明の第一の観点によれば、患者に正常な体温を回復させるための装置は、少なくとも一つの熱伝達面を有する脈管内カテーテル、温度センサー及び、温度センサー及びカテーテルに接続可能な制御ユニットからなる。制御ユニットは、所望の目標血液温度又は体温を達成するために、少なくとも一つの熱伝達面に、又は熱伝達面から選択的に熱を伝達する。脈管内カテーテルは、熱の発生と熱の吸収両方のための単一の熱伝達面を備えるのが良いが、通常は、熱発生面と、別の熱吸収面の両方を有する。熱発生面は、典型的には、ワイヤーコイルのような抵抗加熱器からなり、熱吸収面は典型的にはカテーテルの周りに巻かれた金属箔からなり、典型的には、少なくとも2cm2の露出面積を有する。そのような場合、制御ユニットは、抵抗加熱器に接続可能な電流源及び、金属箔に接続可能な熱電冷却器からなる。別の構成では、カテーテルは少なくとも一つの流れ内腔を有し、その内腔は熱伝達面を通り越してカテーテル内の熱交換媒体の流れを可能にする。制御ユニットは、加熱器、冷却器、及び制御器からなり、制御器は患者に正常体温を回復させるために、熱交換媒体に熱を伝達するために加熱器又は冷却器を選択的に作動させるためのものである。加熱器は、電気抵抗加熱器であり、冷却器は熱電冷却器であるのが良い。
温度センサーは、典型的にはカテーテルにあり、血液温度を測定する。別の例として又は、これに加えて、温度センサーは体温を測定するために、患者に別々に取り付けできてもよい。
本発明の第二の観点では、患者の血流に又は、血流から熱を選択的に伝達することによって、患者に正常体温を回復させるためのカテーテルは、近位端及び遠位端を有するカテーテル本体を有する。遠位端は血管に挿入可能であり、熱発生熱交換面及び熱吸収熱交換面は両方ともカテーテル本体の遠位端付近に配置される。典型的には、カテーテル本体は約15cm乃至50cmの範囲の長さ及び、1mm乃至5mmの範囲の直径を有する。熱発生熱交換面は通常、電気抵抗加熱器からなり、カテーテルは抵抗加熱器を外部電流源に接続するためのコネクターを更に有する。熱吸収熱交換面は典型的には、カテーテル本体の周りに巻かれた金属箔からなり、熱吸収箔を離れた制御ユニットの冷却器に接続させるために、熱伝導素子は近位端近くまでカテーテル本体の中を通って延びる。金属箔熱吸収面は、典型的には少なくとも2cm2の面積を有し、通常は、4cm2乃至80cm2である。熱伝導素子は、一続きの金属箔面(好ましくは血液循環内に置かれない部分は断熱される)でも良いし、或いは別の例として熱伝導材料で作られた金属コアでもよい。
本発明の方法によれば、低体温症患者については患者の血流に熱を選択的に導入することによって、或いは高体温症の患者については血流から熱を除去することによって、患者に正常体温を回復させる。通常、外部制御ユニットに接続されている脈管内カテーテルを経て熱が導入され或いは除去される。別の例として、本発明の方法は又、血液の直接的な体外加熱及び冷却からなる。患者の温度特性、典型的には血液温度及び/又は体温がモニターされる。温度特性が、初期低体温症患者が高体温を有し、又は高体温になろうとしていることを指示すれば、正常体温を回復させるために循環する血液から熱が取り除かれる。同様に、モニターされた温度特性が、初期低体温症患者が高体温になろうとしていることを指示すれば、正常体温が回復されるまで患者の血液から熱が取り除かれる。
好ましい脈管内カテーテルは血管、通常は大腿動脈又は大腿静脈、或いは頸動脈又は頸静脈の中に挿入される。熱を導入する段階は、10W乃至500W、通常は50W乃至250Wの割合で熱を導入することからなり、熱を取り除く段階は1W乃至100Wの割合で熱を取り除く。好ましくは、上述したカテーテル及び装置が用いられる。
初期低体温症患者については、温度特性は通常、血液温度であり、目標血液温度、即ち加熱を停止する温度は36.9℃である。血液温度が39℃を越えるならば、冷却が始まる。初期高体温症の患者については、好ましい温度特性は血液温度であり、冷却を停止する目標温度は約36.9℃である。血液が典型的には、36℃又はそれ以下の温度に冷え続ければ、血液の加熱が始まる。しかしながら、これらの温度目標は名目上の目標であり、本発明の方法は、ここに記載した温度と幾分異なる目標温度で実行できることは認識されるべきである。

好ましい実施形態の説明
本発明は、通常、脈管内カテーテルを使用して、患者の血液を加温又は冷却することによって、患者の体温を選択的に修正及び制御するための方法及び装置を提供する。本発明の好ましい方法によれば、カテーテルは穿刺又は切開を介して患者の身体の血管の中へ挿入される。カテーテルの一部を加温又は冷却することによって、熱は血管内を流れる血液へ又は血液から伝達され、これによって患者の体温は希望通りに上昇又は下降する。処置中、患者の血液温度及び/又は核心体温は独立にモニターされ、治療は患者の血液温度及び/又は核心体温が所望のレベル、普通は約37℃の正常体温に近づくまで続けられる。そのような方法は、低体温症及び高体温症の望ましくない状態を治療する際に使用され、希望するとき、低体温の人工状態を引き起こすのに、例えば、患者の酸素需要を一時的に減らすために使用できる。そのような場合、患者の体温は正常な体温より、摂氏数度下げられる。
低体温症及び高体温症の状態を治療するには、患者の核心体温が目標体温を行過ぎる可能性がある。付与される外部加熱又は冷却に対する身体の代謝反応は、上で記載した通り、初期状態の過剰補償をもたらすことがある。特に低体温症を治療するために患者の身体を加熱したとき、内部代謝過程から起こる身体自身の熱の発生は、予測不能な挙動で体温を上昇させ、体温を正常体温以上に上昇させることがある。このような場合、本発明は、患者の血液から又は血液へ熱の伝達の逆転を行なう。抑制されていない温度上昇の場合、本発明の装置は、循環する血液から熱が取り出されるように切り替えられる。逆に、患者の身体が過冷却の場合には、装置は患者に熱が導入されるように切り替えられる。
図1は本発明によるカテーテル10の遠位端15を示す。カテーテルは患者の皮膚を通って血管BVの中に挿入される。血管を通る血液の流れを一組の流れ矢印Fによって指示する。好ましくは、カテーテルは、比較的太い血管、例えば、大腿動脈又は静脈、若しくは頸静脈に挿入される。これらの血管の使用は、これらの血管が容易に接近しやすく、安全で便利な挿入場所を提供し、かつ、比較的大量の血液がその中を流れている点で有利である。一般に、大きな血流量は患者の中へ又は患者の外へ急速な熱伝達を容易にする。
例えば、頸静脈は約22フレンチの直径、又は7mm以上のビット(bit)を有する(1フレンチ=0.013インチ=0.33mm)。このサイズの血管への挿入に適したカテーテルは、脈管系の他の領域への挿入に向けられたカテーテルに対してかなり大きく作ることが出来る。じゅく状物除去カテーテル又は血管形成バルーンカテーテルは、時々、冠状動脈及び類似の血管から障害物を取り除くのに使用される。これらのカテーテルは普通、2乃至8フレンチの範囲の外径を有する。
対照的に、本発明によるカテーテルは、典型的には、約10フレンチ又はそれ以上の外径を有することが予想されるが、この寸法形状は、請求された発明の基本原理から逸脱することなく、大きく変更されても良いことは明らかである。カテーテルは、開業医によく知られた技術である経皮セルディンガー(Seldinger)法を使って穿刺部位に入れることができるほど小さいことが望ましい。血管の外傷を回避するために、カテーテルは通常、挿入時直径が12フレンチ以下である。しかしながら、一度、血管の中に入れば、カテーテルの遠位端又は作動端は、血流が不当に妨げられない限り、任意の大きさに膨張することができる。
加えて、大腿動脈及び大腿静脈、並びに頸静脈は、全て比較的長くて真直ぐな血管である。これは、かなりの長さの温度制御領域を有するカテーテルの挿入に便利である。これは、熱伝達領域の長さを増大させれば、所定直径のカテーテルが所定温度で、より多くの熱を伝達する点で有利なことは勿論である。
カテーテルを上記の血管の中に挿入するための技術は開業医の間では良く知られている。本発明による方法は、おそらく病院で最も普通に採用されているが、その処置を手術室の中で行う必要はない。装置及び処置は非常に簡単であるから、ある場合には、救急車の中や屋外でも、カテーテルを挿入し、そして治療を始めることができる。
カテーテルの遠位端15は、希望に応じて加熱され或いは冷却され、患者の体温の幾分上又は幾分下のどちらかの温度に保たれる。それによって、血管を流れる血液は温められ又は冷やされる。その血液は患者の循環系を全体にわたってすばやく循環する。これによって、カテーテルの近くの患者の血液を温め又は冷やす有益な効果は、患者の身体全体にわたってすばやく広がる。
図2及び図3は、患者の血液の温度を増すことによって低体温症を治療するのに適したカテーテルを示す。図2に示すように、カテーテルは好ましくは可撓性カテーテル本体20を有する。カテーテル本体の中には、一対の電気伝導リード線22及び23、温度測定リード線25が配置されている。
図3に示すように、電気伝導リード線22及び23は抵抗加熱素子28に接続される。電源(図示せず)によって供給される電流は加熱コイルの中で熱に変換される。その熱はカテーテルの遠位端15を温め、それによって、血管を流れる血液に伝達される。
温度測定リード線25は温度センサー30に接続される。温度センサーは加熱コイルを流れる電流の制御を容易にする。カテーテルの遠位端の温度、かくして患者の血液への熱の流れを厳密にモニターすることは重要である。患者への適切な熱伝達率を依然として与えながら、血液を過熱しないように注意が払われなければならない。カテーテルの遠位端に高感度温度センサーを設けることは、この目的を達成するのを助ける。
図4は、外部電源からカテーテル本体20の遠位端15にエネルギーを伝達するための装置を有するカテーテルのもう一つの実施形態を示す。この実施形態において、レーザー光源(図示せず)からのレーザーエネルギーは、光波ガイド35に沿って送られる。波ガイドはレーザーエネルギーを光学拡散チップ37に差し向け、それはレーザーエネルギーを熱に変換する。拡散チップ37から、熱はカテーテルの遠位端15へ、外方に放射し、そしてそこから患者の血流へ放射される。
図5は本発明の実施するのに適した別のカテーテルを示す。この実施形態は、カテーテル本体20の長さを走る熱伝導性シャフト40を有する。シャフト40は、高い熱伝導率を有する金属又は他の材料で作られる。シャフト40の近位端42を外部加熱装置又は冷却装置45で加熱又は冷却することにより、シャフトの遠位端47或いは遠位端47の外へ熱を流れさせる。図示した実施形態では、シャフトの遠位端には熱伝達翼50が設けられ、熱伝達翼50はシャフトの表面積を増し、それにより、カテーテルと患者の血流との間のより効果的な熱伝達を促進する。
図6は熱をカテーテルの遠位端へ又は遠位端から伝達するための、更に他の手段を示す。この実施形態では、カテーテル本体20は、その中を走る2つの内腔を有する。流体はカテーテルの近位端から、流入内腔60を通り、熱伝達領域62を通って流れ、流出内腔64を通って戻る。流入内腔60を通して、加温又は冷却した流体のどちらかを供給することにより、熱が患者の血流へ又は血流から伝達される。
図示した実施形態において、熱伝達領域62はバルーン70の形態をなしている。バルーンの使用は或る実施形態では、熱伝達が起こる表面積を大きくするのに有利である。バルーンの膨張は、流体が流入内腔60及び流出内腔64を流れるとき、流体の圧力差によって維持される。バルーンは、血管の中を通る血液の流れを不当に妨げないように、血管の内径よりいくぶん小さな直径に膨張されるべきである。
図7は内部抵抗加熱素子28、及び膨張状態で示されているバルーン70を有するカテーテルを示す。この実施形態では、膨張したバルーンによって得られた大きな表面積は、バルーンの表面に設けられた一組の長手方向のフィン75の存在によって更に増大される。図8A、8B、8Cに示すように、択一的に長手方向のフィン75、半径方向のリブ77、又は一又はそれ以上の螺旋状フィン79をカテーテルの本体20上に直接配置してもよい。普通、長手方向リブは、これが他の形態よりも血管の中を通る血液の流れを制限することが少ないため、最も有利である。実際、それらのリブは、バルーンがどれだけ膨張したかに係わらず、流れの経路が常に(リブの間に)維持されるため、バルーンが血管の中の血液の流れをさえぎらないようにする。
抵抗加熱を用いるカテーテルにバルーンを含めることにより、バルーンを満たす流体を介して電流を伝える設計を可能にする。図9に示すカテーテルは、膨張可能なバルーン70を周りに配置したカテーテル本体20を有する。バルーンは、適当な流体を中央バルーン膨張内腔80を通してバルーンの中へ注入することによって膨張させられる。この実施形態では、電流は外部電力源(図示せず)から導線82及び84を通って、電極86及び88に流れる。
適当な流体により、電極86及び88の間に電流を流す。例えば、一般的な塩水は、電荷導体として役立つ溶解イオンを含有する。流体内の電気抵抗により、流体を加熱させ、かくして、カテーテルの所望の加温を行う。加温の量は電極間の電圧、電極間の距離及び、流体の抵抗率に依存する。それらの量の間の関係はかなり単純で、当業者が適切な値を選択することに困難はない。
図3、7及び9に示すカテーテルと同様の抵抗加熱カテーテルは、直流又は低周波の交流電源を使用できる。しかしながら、高周波電源を使用することが望ましいかもしれない。例えば、有害な生理学的反応又は感電死反応の危険を、約100キロヘルツから約1メガヘルツの範囲内の周波数で、少なくすることができることが知られている。これらの周波数で動作する電源は、一般に無線周波数電源又はRF電源と呼ばれる。
本発明によるカテーテルは、カテーテルと血管の中を流れる血液との間の熱伝達率を最適にするように設計されるべきである。大きな表面積が熱伝達を最大にするために、望ましいが、カテーテルが血管の中の血流を不当に制限しないように注意が払われなければならない。その上、カテーテルの温度は、血液内の望ましくない化学変化を防止するように入念に制御されるべきである。このことは、血液はほどほどに高い温度によって容易に変性されるので、熱を血液に加えるときに特に重要である。血液を温めるためのカテーテルの外部温度は、一般に約42℃から43℃を越えるべきではない。
表面温度が37℃乃至42℃に制御されたカテーテルは、患者が重い低体温症から始めて、1時間当り約1℃乃至2℃の身体核心加温率をもたらすことが見積もられる。この見積もりは、血管の中の血の流量、患者の初期体温、熱を伝達するカテーテルの外表面積等を含む、多数の要因に高く依存する。実際に達成される割合は上記の見積もりから実質的に変化するかもしれない。
上の見積もりは、カテーテルから患者の身体に伝達されるパワーのレベル、従って、装置によって要求される電源の大きさのレベルに関する、荒い推定の出発点を与える。選択された適確な電力伝送手段、抵抗加熱コイル、レーザー及び拡散チップ、直接伝導又は流体循環にも係わらず、装置に熱を与える適切な電源が要求される。
患者の身体に入ったり出たりする熱の合計は次のように書くことが出来る。

ここで、ΔHは伝達されたすべての熱の合計であり、HCはカテーテルから患者に伝達された熱であり、Hiは患者によって内部で生じた熱であり、Heは患者から外界に失われた熱である。普通、健康な患者がそうであるように、身体の内部温度調節システムが、外界に失われた熱を相殺するちょうど充分な熱を生じさせるものと仮定すれば、式は簡単になる。

上の式は時間で除した患者の身体内部温度の変化の項で、次のように書くことが出来る。

ここで、mは患者の身体の質量、cは患者の身体の比熱、(ΔT/Δt)は患者の身体内部温度の変化の時間割合、(ΔHC/Δt)はカテーテルから患者への熱送出の時間割合である。
もし患者が、質量75kg、比熱4186J/℃−kg(人間の身体の比熱を水の比熱と等しいと仮定して、実際の値は幾分異なる。)の身体を持つとすると、1時間(3600秒)当り1℃の加温割合は、約87W(lW=1J/sec)の割合で熱を患者に伝達することがカテーテルに要求される。
しかしながら、本発明のカテーテルについて使用されるべき電源の望ましい大きさの見積もりとして、この見積もりはほぼ確実に低すぎる。これは多くの理由から真実である。第一に、便宜上、患者の内部システムが、外界に失われた熱と等しい熱の量を生じさせると仮定した。低体温症の患者では、そうではないことは明らかである。ほとんど定義により、低体温症は、内部で熱を作り出す身体の能力が、外界に失われる熱によって圧倒されるときに起こる。カテーテルはその差異を補わなければならず、その理由だけで、要求される電力レベルはより大きくなる。
加えて、上の見積もりは、電源と利用されるどんな加温手段との間の電力損失をも考慮していない。そのような損失には、電源と抵抗加熱素子の間の電気伝導線中の抵抗損失、レーザー及び拡散チップを有する装置の中に内在する非効率及び他の損失、熱伝導性シャフト又は流体循環内腔に沿った熱損失、等のようなものが含まれる。そのような発生した如何なる損失も、増大した電源容量によって補償されることが必要である。
その上、使用する電源の大きさの制限によって、本発明によるカテーテルの性能を制限することは望ましくない。その代わりに、実際に必要とされる電力よりかなり大きな電力を供給することの出来る、そしてカテーテル自身の測定された温度による電力の供給を制御することのできる電源を使用することが望ましい。先に記載した通り、これはカテーテルの本体に高感度温度センサーを設けることによって容易に達成される。それにも係わらず、上の計算は、本発明によるカテーテルに使用する電源の大きさを決定するために、適当な下限の有用な見積もりとして使用できる。
現在知られている外部血液加温装置の電力要件とここに記載した種々の実施形態の適当な性能とを比較することによって、別の見積もりをすることが出来る。そのような外部加温装置は、一般的には1000乃至1500ワット程度、そして時にはそれ以上の電力の供給を概ね必要とする。おそらく本発明による装置は、それより相当に小さい電力を要求する。第一に本発明は血液を循環させる外部ポンプを必要としない、即ち、この機能は患者自身の心臓によって得られる。従って、そのようなポンプを駆動するのに電力を必要としない。第二に、本発明は外部血液加温装置より相当複雑ではない。公知の装置は、患者から加温装置を通って患者の中に戻る、比較的長い経路にわたって血液を循環させる。本発明による如何なる装置で失われるよりも多くの熱がこの長い経路にわたって失われることが予想される。
かくして、既知のタイプの外部血液循環及び加温装置によって要求される電力は、本発明による装置によって要求される電力の上限の大まかな見積もりとして使用することが出来る。そのような装置は、上に記載した2つの大まかな見積もりの間の容量を有する電源を準備するのが、おそらく最適である。従って、適した電源は、100乃至1500ワットの範囲、おそらく300乃至1000ワット範囲のピーク電力を供給できると考えられる。記載した範囲は、適したピーク電力性能の見積もりである。電源は、カテーテルの本体中の温度センサーに応答して、一般に自動的に温度制御される。従って、患者に伝達される実際の実効電力は、一般に電源装置のピーク電力容量より非常に小さい。
冷却用のカテーテルについては、温度及び電力の制約は、血液加温用のカテーテルの場合と同様の制限はない。血液を凍結させ或いは、あまりにも急な冷却により患者にショックを誘発させないことに、もっぱら注意を払うべきである。
血液は本質的に、多数の浮遊物及び溶解物を含む水である。従って、その凝固点は、0℃より幾分下である。しかしながら、高体温症の患者の血液を冷却し、或いは人工的に低体温を引き起こすようになったカテーテルは、通常は、そのような低い温度で動作しない。実際の温度は約0℃から患者の現在の体温(37℃より幾分上)の間で変化するが、現在そのようなカテーテルの外部表面は、20℃乃至24℃の間の範囲に保持されるものと考えられている。
本発明による方法を実施するのに適した装置の様々な実施形態を説明してきた。他の実施形態及び変更が当業者には思い付くだろう。例えば、本発明による装置の機能を最大に活用するために、種々の熱伝達手段、即ち、電気抵抗加熱、無線周波数加熱、レーザーエネルギー、圧送流体等を含む、これらと組み合わせられる実効熱伝達面積を増加させるための様々な手段、例えば、バルーン、フィン、リブ等がある。又、温度センサーが典型的には使用されるが、図を簡単にするためにそのようなセンサーは、記述した全ての実施形態において示されていない。さらに、図のほとんどはカテーテルの限られた部分だけを温度制御する実施形態を示しているが、実質的にカテーテルの全長を加温又は冷却するのを妨げる理由はない。
広く言えば、本発明は、患者の血管に挿入されたカテーテルの温度を制御することによって、患者の体温を修正する方法を提供する。本発明を実施するための手段のいくつかの例を上で説明したが、これらの例は、本発明を実施するための全ての可能な手段を網羅したものではない。従って、本発明の範囲は、特許請求の範囲に権利を与える均等物の全ての範囲と一緒に、特許請求の範囲を参照して決定されるべきである。
かくして、本発明は、初期低体温症患者の体温を上昇させ、初期高体温症である患者の体温を下げる方法又は、他の目的のために体温を正常な体温より下げる患者の、体温を下げる方法を提供する。全ての場合において、目標の体温を、患者の制御できない生理学的反応のために、うっかり越えることが可能である、即ち、初期低体温症の患者は高体温になり、初期高体温症の患者は低体温になる。そのような場合には、高体温になった患者を直ちに冷すことが出来、低体温になった患者を直ちに温めることが出来るように、特に、本発明は熱伝達過程の逆転に備えている。初期低体温症の患者を温め、かつ、初期高体温症の患者を冷すための制御スキームを図10に示す。初期低体温症の患者及び、初期高体温症の患者に対する、初期の温度、目標温度、行過ぎ温度を下記の表1に示す。
患者の選択的な加温及び冷却用の好ましい装置を、図11に示す。その装置は、近位端102、遠位端104、遠位端に近い熱発生面106及び、遠位端に近い熱吸収面108を有するカテーテル100からなる。熱発生面106は、上に記載した如何なる熱伝達部品でも良いが、典型的には0.1mm乃至1mm間隔を隔てた50回乃至1000回の巻線を有するコイル抵抗加熱器であることが好ましい。カテーテルの全長は、典型的には15cm乃至50cmであり、直径は1mm乃至5mmである。普通、巻線は遠位端に近い10cm乃至20cmの範囲で全距離にわたって延びる。
代表的な熱吸収面は熱伝導金属箔であり、典型的には、金、銀、アルミニウム等のような生物的適合性熱伝導金属である。銅も有用であるが、生物的適合性を増すために処理されるか又は、さやに入れられなければならない。箔はカテーテル本体の可撓性を増すために典型的には薄く、典型的には、0.001mm乃至0.01mmの範囲の厚さを一般に有する。
熱吸収面108は、カテーテルの外部、典型的には以下に説明するように制御ユニット120の中に配置された冷却器に伝導的に接続される。図示された実施形態では、熱吸収面108は、熱伝導性コア部材110によって結合され、コア部材110は、上記の熱伝導金属の中の一つから作られた可撓性ロッド又はワイヤーで構成されている。あるいは、熱吸収面の近位端を冷却器に結合することができるように、熱結合は熱吸収面108を近位方向に延長することによって達成できる。後者の場合、熱吸収面108の近位部分は、血液の循環の外側の冷却を防ぐために、断熱されることが好ましい。
装置は制御ユニット120を更に含み、制御ユニット120は、典型的にはカテーテル100に結合するための熱発生器と冷却器の両方を有する。普通、熱発生器は、カテーテルの抵抗加熱器に結合するための直流電流源を有する。通常、直流電流源は、直流10V乃至直流60Vの範囲の電圧、1A乃至2.5Aの範囲の電流出力で動作する商業的に入手できる温度制御式直流電源である。通常、電源は、加熱表面106の表面温度を40℃乃至42℃の範囲に維持するように制御される。上で論じたように、表面温度は、血液成分の損傷を防止するために、42℃を越えるべきではない。熱交換面の他の望ましい特性は上に記載した。
選択的に、熱交換面の温度は又、測定された血液温度及び/又は測定された体温に基づいて制御することが出来る。血液温度は、カテーテルの温度センサーによって測定することができる。例えば、温度センサー112は、熱交換面106及び108から間隔を隔てたカテーテル上に配置することもできる。温度センサー112は、血流の方向に基づいてカテーテルを患者に導入する方法に応じて熱交換面の上流か下流のどちらかに配置することができる。任意的に、一対の温度センサーを設け、それを上流と下流の両方の血液温度を測定するために、熱交換面の両側に配置する。カテーテルは又、表面の温度を直接制御することができるように、熱発生面106に直接結合された温度センサー(図示せず)を有する。他の温度センサー(図示せず)が、患者の核心体温を直接測定するために設けられ、核心体温は制御ユニット120にフィードバックされる。
制御ユニット120の冷却器は、血液を所望の速さで冷却するのに十分な速さで熱吸収面108から熱を奪うことができる任意の型の冷却ユニットで良い。典型的には冷却器は1W乃至100Wの定格である。好ましくは、冷却器は、ニュージャージー州、トレントン08648のメルコアサーモエレクトリクス(Melcor Thermoelectrics)から入手できるような、熱電式冷却器である。熱吸収面108からの直接的な熱伝導が、制御ユニット120の冷却器に行われるように、冷却器はコア部材110に結合される。冷却面108の温度は、加熱面106の温度よりも重要度が低いが、通常0℃乃至35℃の範囲、好ましくは30℃以下に維持される。冷却面の温度はこの範囲内で直接制御され、あるいは、装置は、冷却温度が装置全体の特性に基づいてほぼこの範囲内で動作するように設計されるのがよい。
制御ユニット120は、血液温度及び/又は体温に基づいて、熱発生面106及び熱吸収面108の温度を制御するための一又はそれ以上の温度制御器を更に有する。少なくとも、制御ユニット120は、上に述べた範囲内に熱発生面106の温度制御に備えるばかりでなく、上に記載した加熱又は冷却モードを逆転するために、患者の血液温度及び患者の体温の少なくとも一つをモニターすることに備える。図10に示した例示的な実施形態では、制御スキームはオン−オフモードで動作し、例えば、低体温症の患者を、目標温度に達するまで、初めは一定表面温度で血液を温めることによって治療する。目標温度に達したとき、熱発生面106への電力を切る。しかしながら、血液温度及び/又は患者体温のモニターは、患者の温度が目標温度より高い最高温を越えないようにするように維持される。最高温を越えたならば、超過した体温が下がるまで、装置を冷却モードで動作する。通常、再び患者を温める必要はないが、本装置は、必要ならば更なる加温及び冷却のサイクルを行なう。初期高体温症の患者に対しては、冷却及び加温のモードを逆にする。
例えば、本発明の温度制御スキームを、実質的にもっと精巧にすることが出来ることがわかる。例えば、患者を温めるための電力の入力は、比例、微分或いは積分制御スキームに基づいて制御することができ、スキームは、一般に患者体温が所望の目標レベルに近づくにつれて、熱伝達率の漸減を行なう。更に、患者の血液温度及び患者の体温の両方に基づくカスケード制御スキームも考案できる。そのような制御スキームは、患者を温めるようにしても良いし又、患者を冷やようにしても良く、制御スキームを準備するに当たって典型的な患者の生理学的特性の数学モデルを考慮する。しかしながら、今のところ、目標温度が安全な量より大きく越えたならば、熱伝達モードを逆転する単純なオフ−オン制御スキームは、十分能力があると考えられる。
Claims (5)
- 患者の血液を血管の中で加温し又は冷却することにより、患者の体温を選択的に修正及び制御する装置であって、
少なくとも一つの熱伝達面を有する脈管内カテーテルと、
患者の血液温度又は体温を測定する温度センサーと、
所望の目標血液温度又は体温を達成するために、温度センサーからのフィードバックを受け、熱伝達面の温度を42℃を越えないように維持しながら、血管の中の前記少なくとも一つの熱伝達面に能動的に熱を伝達し、又は血管の中の前記少なくとも一つの熱伝達面から熱を吸収するように作動する、温度センサーとカテーテルに接続可能な制御ユニットと、
を有することを特徴とする装置。 - 前記カテーテルが、少なくとも熱発生面及び別々の熱吸収面を有する、請求項1に記載の装置。
- 前記熱発生面が抵抗加熱器からなり、前記熱吸収面が前記カテーテルの周りに巻かれた金属箔からなる、請求項2に記載の装置。
- 前記抵抗加熱器がコイルからなり、前記金属箔が少なくとも2cm2の露出面積を有する、請求項3に記載の装置。
- 前記制御ユニットが抵抗加熱器に連結可能な電流源と、前記金属箔に接続可能な熱電式冷却器とを有する、請求項4に記載の装置。
