【発明の属する技術分野】
本発明は、被験者の末梢部で検出される脈波波形のうち、心臓の収縮期に相当する波形の一部または全部を解析して、当該被験者の心機能を診断・評価する心機能診断装置に関する。

【従来の技術】
脈波は、一般的に言えば、心臓から拍出されて血管を伝搬する血液の波である。このため、脈波を検出して解析することにより、種々の医学的情報を得られることが知られている。そして、脈波の研究が進むにつれ、生体から採取した脈波を種々の手法で解析することによって、血圧や心拍数などではわからないような情報が得られ、これらの情報をもとに生体の状態を診断ができるようになってきた。ここで、本願と同じ発明者は、PCT/JP96/01254(発明の名称:生体状態の診断装置及び制御装置)において、脈波波形の形状とその歪率との関係について着目した。この出願に係る発明は、被験者の脈波波形を検出して処理し、これにより当該脈波波形の歪率を算出し、この歪率から脈波波形の形状を特定することによって当該被験者の生体状態の診断を可能とするものであった。

ここで、上記出願において述べられている脈波波形の形状とその歪率との関係について簡単に説明しておく。
まず、脈波波形の分類には様々なものがあり、その形状も多岐にわたるが、ここでは、東洋伝承医学の一つである中国医学の分類による代表的な脈波波形の形状について説明する。図27(a)〜(c)は、この分類による代表的な脈波波形の形状を示す図である。
同図(a)に示す脈波波形の形状は、「平脈」といわれ、正常な健康人の脈象である。この「平脈」は、図示のように、ゆったりとして緩和であり、リズムが一定で乱れが少ないことが特徴である。
次に、同図(b)に示す脈波波形の形状は、「滑脈」といわれ、血流状態に異常を有する者の脈象であり、急に立ち上がった直後すぐに下降し、大動脈切痕が深く切れ込むと同時にその後の峰が通常よりもかなり高いのが特徴である。この「滑脈」は、浮腫や、肝腎疾患、呼吸器疾患、胃腸疾患、炎症性疾患などの病気によって、脈の往来が非常に流利・円滑になって生じると考えられる。
また、同図(c)に示す脈波波形の形状は、「弦脈」といわれ、血管壁の緊張度が上昇している者の脈象であり、急激に立ち上がってすぐに下降せず高圧の状態が一定時間持続するのが特徴である。この「弦脈」は、肝胆疾患や、皮膚疾患、高血圧、疼痛性疾患などの病気によって現れ、自律神経系の緊張によって血管壁が緊張し弾力性が減少し、拍出された血液の拍動の影響が現れにくくなったことに原因すると考えられる。
なお、同図(a)〜(c)のグラフにおいて、縦軸および横軸は、それぞれ血圧(mmHg)、時間(秒)である。

そして、このような脈波波形の形状とその歪率dとは、図28に示すような相関関係がある。ここで、脈波波形の歪率dは、次式(1)により定められる。

なお、この式(1)において、A1は脈波における基本波成分の振幅であり、A2、A3、……、Anはそれぞれ脈波の第2次、第3次、第n次調波成分における振幅である。
したがって、被験者の脈波波形を検出し、例えば、FFT(高速フーリエ変換)処理を施して振幅A1〜Anをそれぞれ求めて歪率dを算出すれば、図28に示した相関関係により脈波波形の形状を定量的に特定することが可能となり、被験者の心機能状態を診断することが可能となる。

【発明が解決しようとする課題】
しかしながら、上記従来技術においては、脈波波形と心機能状態との関係を波形の歪率を介して間接的に述べたにとどまり、脈波波形と心機能状態との関係を直接的に言及しなかった。このため、脈波波形を解析するにあたり、脈波波形の1周期分以上をFFT処理などして、基本波および各次高調波成分の振幅の大きさを求める必要があり、この結果、心機能の診断には、高い処理能力が要求されるといった問題があった。この問題は、特に、診断装置を小型化・軽量化する場合に顕著となる。

本発明は、上記問題鑑みてなされたものであり、その目的とするところは、脈波波形のうち、どの部分が心機能状態を示すかを特定することにより、簡易な構成で心機能状態を診断することが可能な心機能診断装置を提供することにある。

【課題を解決するための手段】
上記目的を達成するために、本発明にあっては、生体から脈波波形を検出する脈波検出手段と、前記脈波波形から心臓の収縮期を特定する収縮期特定手段と、前記収縮期特定手段により特定された収縮期に相当する脈波波形のうち、第2番目にピークとなる退潮波を規定する指標を算出する解析手段と、前記解析手段により算出された指標に基づいて、当該生体の心機能状態を評価する評価手段とを具備することを特徴とする。

【発明の実施の形態】
<1:第1実施形態>
はじめに、本発明の第1実施形態に係る心機能診断装置について説明する。

<1−1:第1実施形態の理論的根拠>
言うまでもなく、心臓は、収縮拡張を繰り返すことによって血液を駆出している。ここで、1サイクルの収縮拡張によって心臓から血液が流れ出る時間は、駆出時間と呼ばれる。この駆出時間は、運動等により単位時間当たりの心臓の収縮回数である拍数が高くなると、アドレナリンなどのカテコールアミンが放出される結果、短くなる傾向にある。これは、心筋の収縮力が増大していることを意味する。
また、この駆出時間が長くなるにつれて、1サイクルの収縮拡張によって心臓から流出する拍出量が大きくなる傾向にある。
さて、人が運動等すると、心筋や骨格筋などに酸素を多く供給する必要から、拍数と拍出量との積、すなわち、単位時間あたりに心臓から送り出される血液流量は増加する。ここで、拍数が増加する結果、駆出時間は短くなるので、拍出量は逆に小さくなる。ただし、拍数の増加率は拍出量の減少率を上回るため、拍数と拍出量との積は、全体的にみれば増加することになる。

次に、心臓の動きと血圧波形との関係について説明する。図15(a)は、心電波形を示すものであり、一般に、図におけるR点からT波の終点Uまでが心室収縮期と言われ、これが上記駆出時間に相当するものである。また、U点から次のR点までが心室拡張期であると言われている。ここで、心室収縮期において、心室の収縮は一様に発生するのではなく、外側から内側に収縮が進行するにつれてゆっくりとなる。このため、心臓直後における大動脈起始部での血圧波形は、同図(b)に示すように、大動脈弁解放から閉鎖までの心室収縮期において、上に凸の形状となる。

このような大動脈起始部での血圧波形が、末梢部(橈骨動脈)においてどのような波形となるかについての一例を同図(c)に示す。すなわち、同図(c)は、末梢部での脈波波形の一例を示すものである。このような形状となるのは、まず、心臓からの血液の拍出により駆出波と呼ばれる第1波が生じ、続いて、心臓に近い血管分岐部分での反射に起因して退潮波と呼ばれる第2波が生じ、この後、大動脈弁閉鎖に伴う切痕が生じて、切痕波と呼ばれる第3波が現れる、と考えられている。
したがって、脈波波形においては、最も血圧値の低い点から切痕までが心室収縮期に相当し、切痕から次のサイクルにおいて最も血圧値の低い点までが心室拡張期に相当することになる。
ここで、脈波波形において大動脈弁解放に相当する点は、血圧値の最小極小点であり、脈波波形において大動脈弁閉鎖に相当する点は、時系列的にみれば、当該最小極小点から第3番目に現れる極小点であり、また、血圧値の大小でみれば、当該最小極小点から第2番目の極小点である。
なお、同図(c)に示す脈波波形は、実際には、同図(b)に示す大動脈血圧波形に対して時間的に遅れるが、ここでは説明のため、この時間遅れを無視し、位相を揃えている。

次に、同図(c)に示した脈波波形について検討してみる。被験者の末梢部で検出される脈波波形は、いわば、拍動性のポンプたる心臓と導管たる血管系とからなる閉鎖系を経た血液の圧力波であるため、第1に、心臓のポンプ機能、すなわち、心機能状態によって規定されるほか、第2に、血管径や、血管の収縮・伸展、血液粘性抵抗などの影響を受ける。このため、脈波波形を検出して解析すれば、当該被験者の動脈系の状態のほか、心機能状態を評価することができる、と考えられる。

ここで、脈波波形のうち、どの部分を解析するかについて検討してみる。まず、上述したように、心室の収縮は一様に発生しないため、心臓からの血液は、同図(b)に示すように心室収縮期の途中で最大圧力となって拍出される形になる。一方、心臓から拍出される血液は、血管系の影響を受けて末梢部に到達する。このため、心臓に近い血管分岐部分での反射に起因して生じる退潮波は、心室の収縮によって拍出される血液の圧力特性と血液が拍出される側の血管特性との平衡により定まるはずである。

このうち、前者にかかる心室の収縮によって拍出される血液の圧力特性の意義について考察すると、一般に心臓が肥大した場合、心室が収縮してもその内容積があまり変化しなくなるので、血液は心臓から急峻に送り出されなくなると考えられる。このことは、心筋の収縮力が何らかの理由により低下した場合でも同様である。
一方、後者にかかる血管特性の意義について考察すると、血管の収縮・伸展性は、心理状態や自律神経系の疾病等によって変化し、例えば、血管が硬化傾向にあれば、弾力による反射の影響が現れにくくなるから、退潮波も現れにくくなるし、反対に血管が軟化傾向にあれば、弾力による反射の影響が現れるから、退潮波も顕著に現れると考えられる。
ここで、退潮波は、拍出される血液の圧力特性と血液が拍出される側の血管特性との影響、詳細には、血液を送り出す心臓の特性と大動脈起始部から末梢部までの動脈系の特性との双方の影響を受けて形成されると考えられるから、退潮波近傍を解析することにより、心臓の肥大や、心筋の機能、血管に影響を与える疾病・心理状態等を定量的に評価することが可能になると考えられる。なお、退潮波を解析する方法として、その退潮波を規定する指標を求めることや、退潮波を周波数解析することなどが考えられる。ここで、指標には、例えば、退潮波の振幅や、期間、さらに、これらを心室収縮期間や最小最大血圧値で正規化した値などが考えられ、また、周波数解析には、ウェーブレット変換や級数展開などが考えられる。くわえて、これらの時間的変化率も有用であると考えられる。

ところで、本願発明者は、循環(血行)の状態を非侵襲的に求めるため、大動脈起始部から末梢部までの動脈系の挙動を電気的モデルによりシミュレートして、血管の粘性抵抗やコンプライアンスなどの循環動態に関するパラメータを近似的に算出する技術を提案している(特開平6-205747号:発明の名称「脈波解析装置」や、PCT/JP96/03211:発明の名称「生体状態測定装置」などを参照)。
この技術は、端的に言えば、動脈系の挙動をシミュレートした電気的モデルに、被験者の大動脈起始部の圧力波に対応する電気信号を与えたとき、その応答波形が、実際に検出された脈波波形と一致するように、電気的モデルを構成する各素子の値を定めることで、各素子に対応する循環動態のパラメータを近似的に算出するものである。
この電気的モデルについては、図18(a)に示すような四要素集中定数モデルや、同図(b)に示すような五要素集中定数モデルがある。特に、後者の五要素集中定数モデルについては、人体の循環動態の挙動を決定する要因のうち、四要素集中定数モデルで採用される中枢部での血液による慣性、中枢部での血液粘性による血管抵抗(粘性抵抗)、末梢部での血管のコンプライアンス(粘弾性)、および、末梢部での血管抵抗(粘性抵抗)、以上4つのパラメータに対し、さらに、大動脈コンプライアンスを追加して、これらのパラメータを電気回路としてモデリングしたものである。

また、集中定数モデルに流れる電流i、ip、ic、isは、各々対応する各部を流れる血流[cm5/s]に相当する。なかでも、電流iは大動脈血流に相当し、電流isは左心室から拍出される血流に相当している。また、入力電圧eは左心室圧[dyn/cm2]に相当し、電圧v1は大動脈起始部での圧力[dyn/cm2]に相当する。さらに、静電容量Cの端子電圧vpは橈骨動脈部での圧力[dyn/cm2]に相当する。くわえて、ダイオードDは、大動脈弁に相当するものであって、心室の収縮期に相当する期間においてオン(弁が開いた状態)となる一方、拡張期に相当する期間においてオフ(弁が閉じた状態)となる。

このように、大動脈起始部から末梢部までの動脈系は、図18(a)や(b)などの電気的モデルによりシミュレートして考えることが可能である。ここで、動脈系を一種の電気的モデルとして考えた場合において、心室収縮期における脈波波形につき、再び考察してみる。
心室収縮期における大動脈起始部での血圧波形は、図15(b)に示すように、凸形状の1つのなだらかな波であるのに対し、心室収縮期における末梢部での脈波波形は、同図(c)に示すように、駆出波および退潮波の2つの波である。この現象を、上記電気的モデルに置き換えて考えてみると、同図(b)の波形に相当する電気信号をパルスとして電気的モデルの入力端に印加したならば、そのモデルの出力端に、入力パルスに応答する第1の波形と、この波形に続く第2の波形とが現れたことを意味し、さらに、第2の波形が、入力パルス印加期間中に現れたことを意味する。
したがって、第2の波形に相当する退潮波は、心臓という電源と動脈系という負荷とを接続した場合であって、かつ、心臓という電源が動作中、すなわち、大動脈弁が解放している場合において、両者インピーダンスの相違によって発生したものである。そして、この場合でも退潮波は、心臓の特性と動脈系の特性とにより定まる波形と言える。このため、退潮波近傍を解析することは、心臓の肥大や、心筋の機能、血管に影響を与える疾病・心理状態等のほかに、心臓と動脈系との不整合を評価することにもつながる、と考えられる。

次に、本願発明者は、脈波波形の形状について、その特徴を定める波形パラメータを、図16で示されるように定めた。すなわち、波形パラメータを、
▲1▼脈波波形の血圧値が最小であって1拍の立ち上がりピーク点P0(最小極小点)から、次の拍の立ち上がりピーク点P6までの時間t6、
▲2▼脈波波形において順次現れるピーク点(極大点および極小点)P1〜P5の血圧値(差)y1〜y5、および、
▲3▼ピーク点P0から、上記各ピーク点P1〜P5が現れるまでの経過時間t1〜t5
として定めた。なお、この場合、y1〜y5は、それぞれピーク点P0の血圧値を基準とした相対的な血圧値を示すことになる。

さらに、本願発明者は、22歳〜46歳までの健常な成人74名に対して脈波を検出し、これらの波形パラメータをそれぞれ求める一方、当該脈波波形から拍数を実測した。
一方、本願発明者は、実測した拍数について、各波形パラメータ単体やこれらの差等との個別的な相関関係を検討した。この結果、拍数は、心室拡張期に相当する期間(t6-t4)に対して、相関係数(R2)が0.92という高い相関関係を有することが判明した。このため、期間(t6-t4)を求めるのに必要な構成を採用すれば、これとの相関関係により拍数が求めることが可能となる。

<1−2:実施形態の機能構成>
本実施形態に係る心機能診断装置は、以上のような理論的根拠に基づいて構成されるものであり、第1に、被験者から脈波波形を検出し、第2に、検出した脈波波形を解析して、心室収縮期を特定し、第3に、特定された心室収縮期における退潮波を規定する指標を求め、第4に、その指標により被験者の心機能状態を評価するものである。ここで、本実施形態においては、退潮波を規定する指標として、その振幅(波形パラメータでいえば血圧値差(y3−y2))を取り扱うものとするが、本発明をこれに限定する趣旨ではない。
また、拍数も心機能状態を評価するのに有用な指標となることが一般に知られていることから、本実施形態に係る心機能診断装置は、拍数も参照して心機能状態を評価することとする。具体的には、人がそれほど強い運動をしていなくても、緊張やストレスなどに起因して、単位時間当たりの心臓の収縮回数である拍数が高く場合があり、このような場合に、心機能状態を評価するのは適切でないことがあるので、本実施形態にあっては、被験者の体動があまり大きくなく、かつ、被験者の拍数がしきい値以下であることを条件として心機能の状態を評価することとする。なお、拍数については、積極的に心機能評価の判断材料として用いても良いし、後述するような前提にあれば、検出しない構成としても良い。

図1は、本実施形態に係る心機能診断装置の機能構成を示すブロック図である。
この図において、脈波検出部10は、例えば、被験者の末梢部(例えば、橈骨動脈)における脈波波形を検出して、その検出信号をMHとして体動除去部30に出力するものである。
一方、体動検出部20は、例えば、加速度センサなどから構成され、被験者の体の動きを検出して、その検出信号を信号THとして波形処理部21に出力するものである。波形処理部21は、ローパスフィルタ等で構成され、体動検出部20から出力される信号THを波形整形処理して、体動成分を示す信号MHtとして出力するものである。体動除去部30は、脈波検出部10による信号MHから体動成分を示す信号MHtを減算して、脈波成分を示す信号MH’として出力するものである。

本実施形態にかかる心機能診断装置は、被験者から検出した脈波波形を処理するものであるが、被験者がなんらかの動きを伴っている場合、脈波検出部10により検出された信号MHには、脈波成分を示す信号MH’のほか、被験者の体動成分を示す信号MHtも重畳されることになる。このため、MH=MHt+MH’となり、脈波検出部10から出力される信号MHは、被験者の脈波波形を正確に示すものではない。

一方、血流は血管や組織などの影響を受けるので、信号MHに含まれる体動成分MHtは、被験者の体動を示す信号THそのものではなく、それを鈍らせたものになると考えられる。
このため、被験者の体動を直接的に示す体動検出部20による信号THを波形処理部21によって波形整形して、体動成分を示す信号MHtとして用い、これを、脈波検出部10による信号MHから減算し、これにより体動の影響を除去して、脈波成分を示す信号MH’を得ているのである。なお、波形処理部21におけるローパスフィルタの形式や、段数、定数などは、実際に測定したデータから定められる。

次に、ピーク点抽出・波形解析部40は、脈波成分を示す信号MH’について、波形パラメータに関するピーク点P0〜P6の各点に関連した「ピーク情報」と呼ばれる情報を抽出して、図16に示した波形パラメータを求めるとともに、これらの波形パラメータから、退潮波の振幅に相当する血圧値差(y3-y2)および期間(t6-t4)をそれぞれ算出するものである。なお、ピーク点抽出・波形解析部40の詳細構成およびピーク情報の内容については、後述することとする。
また、ピーク点抽出・波形解析部40は、血圧値差(y3-y2)および期間(t6-t4)を求めるにあたって、ピーク点P0およびP4を特定するが、このことは、必然的に心室収縮期および心室拡張期を特定することを意味する。すなわち、ピーク点P0は駆出波の立ち上がり開始点に相当するから、これを求めることは、心室収縮期の始点(心室拡張期の終点)を特定することを意味し、また、ピーク点P4は大動脈弁閉鎖に伴う切痕に相当するから、心室収縮期の終点(心室拡張期の始点)を特定することを意味する。したがって、ピーク点抽出・波形解析部40は、必然的に心室収縮期および心室拡張期を特定する機能も有する。

さて、拍数変換テーブル60は、期間(t6-t4)と拍数との相関関係を予め記憶して、ピーク点抽出・波形解析部40に求められた期間(t6-t4)を拍数に変換するものである。
なお、拍数を直接的あるいは正確に求めるのであれば、ピーク点抽出・波形解析部40によって時間t6を求めて、この換算値から算出すれば良い。

次に、評価許可部70は、体動検出部20による信号THがしきい値以下であって、かつ、拍数変換テーブル60により変換された拍数がしきい値以下である場合に、ピーク点抽出・波形解析部40により求められた血圧値差(y3-y2)を、現在の時刻とともに出力するものである。データ記憶部71は、評価許可部70によって出力された血圧値差(y3-y2)と時刻とを組にして記憶するものである。

また、評価部72は、次の処理を実行して、血圧値差(y3-y2)の変化率を求めるものである。すなわち、評価部72は、第1に、データ記憶部71に最近記憶された血圧値差(y3-y2)と時刻とを読み出し、第2に、評価許可部70によって出力された現時点の血圧値差(y3-y2)から、読み出した血圧値差(y3-y2)を差し引いて、血圧値差の変化分を求め、第3に、現時点の時刻から読み出した時刻を差し引いて経過時間を求め、第4に、血圧値差の変化分を経過時間で割って、その変化率を算出する。
評価内容記憶部73は、血圧値差の変化率に対応して予め複数の評価内容を記憶する一方、評価部72によって算出された変化率に対応する評価内容を読み出して出力するものである。
告知部74は、診断内容記憶部73によって読み出された診断内容および時間的推移作成部75により作成された時間推移を、表示や音声等により外部に出力するものである。
また、時間的推移作成部75は、評価許可部70から出力された血圧値差(y3-y2)および時刻、あるいは、データ記憶部71に時系列に記憶された血圧値差(y3-y2)および時刻から、血圧値差(y3-y2)の時間的推移を作成して出力するものである。

<1−2−1:ピーク点抽出・波形解析部の詳細構成>
ここで、ピーク点抽出・波形解析部40の詳細について説明する。図2は、その詳細構成を示すブロック図である。
図において、マイクロコンピュータ401は、各構成部分を制御するものであり、内部に図示しないレジスタを有する。
波形メモリ402は、RAM等によって構成され、A/D変換器403およびローパスフィルタ404を介して供給される信号MH’の値、すなわち、脈波成分を示す信号の波形値Wを順次記憶する。
波形値アドレスカウンタ405は、マイクロコンピュータ401から波形採取指示STARTが出力されている期間、サンプリングクロックφをカウントし、そのカウント結果を、波形値Wの波形値アドレスADR1(書込アドレス)として出力するものである。この波形値アドレスADR1は、マイクロコンピュータ401により監視される。
また、セレクタ406は、波形メモリ402へのアドレスを選択するものであり、マイクロコンピュータ401によってセレクト信号S1が出力されていない場合、波形値アドレスカウンタ405によって出力される波形値アドレスADR1を選択する一方、セレクト信号S1が出力されている場合、マイクロコンピュータ401によって出力される読出アドレスADR4を選択する。

一方、微分回路411は、ローパスフィルタ404から順次出力される波形値Wを時間微分して出力する。
零クロス検出回路412は、波形値Wが極大値または極小値となることによって波形値Wの時間微分がゼロとなった場合に、零クロス検出パルスZを出力するものである。さらに詳述すると、零クロス検出回路412は、図16に示した波形パラメータのピーク点P0〜P6を検出するために設けられた回路であり、これらのピーク点に対応した波形値Wが入力された場合に零クロス検出パルスZを出力する。

次に、ピークアドレスカウンタ413は、マイクロコンピュータ401によって波形採取指示STARTが出力されている期間、零クロス検出パルスZをカウントし、そのカウント結果をピークアドレスADR2として出力するものである。
移動平均算出回路414は、現時点までに微分回路411によって出力された波形値Wの時間微分値を過去所定個数分だけ蓄積して、その平均値を算出し、その結果を現時点に至るまでの脈波の傾斜を表す傾斜情報SLPとして出力するものである。

この心機能診断装置は、一定時間毎に次の測定を行うものである。このため、まず、初回に行われる測定動作について説明する。
脈波検出部10により出力される信号MHには、被験者の体動に伴う体動成分が重畳されるが、体動成分除去部30により当該体動成分が除去されて、脈波成分のみを示す信号MH’となって、ピーク点抽出・波形解析部40に供給される。
ピーク点抽出・波形解析部40においては、後述するように信号MH’のデータが蓄積・解析され、これにより脈波波形の波形パラメータが特定されて、このうち、退潮波の振幅に相当する血圧値差(y3-y2)と心室拡張期に相当する期間(t6-t4)とがそれぞれ求められる。このうち、期間(t6-t4)は、拍数変換テーブル60によって拍数に変換される。

評価許可部70により出力された血圧値差(y3-y2)および現在時刻は、データ記憶断71において時系列的に順次記憶される。なお、ここで記憶された血圧値差(y3-y2)および時刻は、次回の測定において読み出され、退潮波の振幅の変化率を算出する際の基礎とされることになる。

<1−3−1−1:ピーク点抽出・波形解析部の動作>
ここで、図2に示したピーク点抽出・波形解析部40の動作について説明する。
ピーク点抽出・波形解析部40は、脈波波形を取得してから、波形パラメータを求め、さらに、血圧値差(y3-y2)および期間(t6-t4)を求めるまでを、複数の段階で行う。そこで、ピーク点抽出・波形解析部40の動作については、各段階に分けて説明することとする。

<1−3−1−1−1:脈波波形の蓄積およびそのピーク情報の採取>
まず、ピーク点抽出・波形解析部40においては、信号MH’が図2における波形メモリ402に蓄積されるとともに、当該信号MH’で示される脈波波形のピーク情報が採取される。この動作は、詳細には次のようにして実行される。
まず、脈波波形の採取開始を指示する旨の信号STARTがマイクロコンピュータ401によって出力されると、波形値アドレスカウンタ405およびピークアドレスカウンタ413のリセットが解除される。
この結果、サンプリングクロックφのカウントが波形値アドレスカウンタ405によって開始され、そのカウント値たる波形値アドレスADR1が、セレクタ406を介して波形メモリ402に書込アドレスとして供給される。そして、体動信号除去部30から出力された脈波成分を示す信号MH’がA/D変換器403に入力され、サンプリングクロックφにしたがってディジタル信号に順次変換され、ローパスフィルタ404を介し波形値Wとして順次出力される。このようにして出力された波形値Wは、波形メモリ402に順次供給され、その時点において波形値アドレスADR1で指定される記憶領域に書き込まれる。以上の動作により、例えば、図4に例示する脈波波形における一連の波形値Wが波形メモリ402に蓄積されることとなる。

一方、この蓄積動作と並行して、ピーク情報の採取およびピーク情報メモリ205への書き込みが、以下のようにして実行される。
まず、脈波成分を示す信号MH’の波形値Wが微分回路411によって時間微分され、この結果が零クロス検出回路412および移動平均算出回路414にそれぞれ供給される。このようにして波形値Wの時間微分値が供給される毎に、移動平均算出回路414は、過去所定個数の時間微分値の平均値(すなわち、移動平均値)を演算し、演算結果を傾斜情報SLPとして出力する。ここで、波形値Wが上昇中もしくは上昇を終えて極大状態となっている場合は傾斜情報SLPとして正の値が出力され、下降中もしくは下降を終えて極小状態となっている場合は傾斜情報SLPとして負の値が出力される。

例えば、図4に示す脈波波形がローパスフィルタ404から出力されると、ピーク点P’1は極大点であるから、時間微分としてゼロが微分回路411から出力される。このため、零クロス検出パルスZが、零クロス検出回路412によって出力される。
この結果、マイクロコンピュータ401により、その時点における波形値アドレスカウンタ405のカウント値たる波形値アドレスADR1、波形値W、ピークアドレスカウンタのカウント値であるピークアドレスADR2(この場合、ADR2=0)および傾斜情報SLPが取り込まれる。また、零クロス検出パルスZが出力されることによってピークアドレスカウンタ203のカウント値たるピークアドレスADR2が「1」になる。

一方、マイクロコンピュータ401は、取り込んだ傾斜情報SLPの符号に基づいてピーク種別B/Tを作成する。このようにピーク点P’1の波形値Wが出力されている場合、その時点における傾斜情報SLPが正の値となるので、マイクロコンピュータ401は、ピーク情報B/Tの値を極大点に対応した「T」とする。そして、マイクロコンピュータ401は、ピークアドレスカウンタ413から取り込んだピークアドレスADR2(この場合、ADR2=0)をそのまま書込アドレスADR3として指定し、波形値W、この波形値Wに対応した波形値アドレスADR1、ピーク種別B/T、傾斜情報SLPを第1回目のピーク情報としてピーク情報メモリ415に書き込む。
なお、ピーク情報の書き込みが第1回目の場合、直前のピーク情報がないためストローク情報STRKの作成および書き込みは行わない。

その後、図4に示す脈波波形において、ピーク点P’2に対応した波形値Wがローパスフィルタ404から出力されると、上述と同様に零クロス検出パルスZが出力され、波形値アドレスADR1、波形値W、ピークアドレスADR2(=1)、傾斜情報SLP(<0)がマイクロコンピュータ401により取り込まれる。
そして、上記と同様、マイクロコンピュータ401により、傾斜情報SLPに基づいてピーク種別B/Tが決定される。ここでは、ピーク点P’2は極小点なので、その時点における傾斜情報SLPは負の値となり、ピーク情報B/Tの値は極小点に対応した「B」となる。また、マイクロコンピュータ401によって、ピークアドレスADR2よりも「1」だけ小さいアドレスが読出アドレスADR3としてピーク情報メモリ415に供給される。これにより、第1回目に書き込まれた波形値Wが読み出される。そして、マイクロコンピュータ401によって、ローパスフィルタ404から今回取り込んだ波形値Wと、ピーク情報メモリ415から読み出した第1回目の波形値Wとの差分が演算され、ストローク情報STRKが求められる。このようにして求められたピーク種別B/T、ストローク情報STRKが、波形値アドレスADR1、波形値W、傾斜情報SLPとともに、第2回目のピーク情報としてピーク情報メモリ415のピークアドレスADR3=1に対応した記憶領域に書き込まれる。以後、ピーク点P’3、P’4、…、が検出された場合も同様の動作が実行される。
そして、所定のタイミングで、マイクロコンピュータ401により波形採取指示STARTの出力が停止され、波形値Wおよびピーク情報の採取が終了する。

<1−3−1−1−2:1拍分の脈波波形を特定>
このようにして図4に例示した脈波波形のピーク点P’1〜のピーク情報を採取しても、それだけでは、図16に定めた波形パラメータを求めたことにはならない。すなわち、図4に示した脈波波形1拍分を特定してはじめて、図16で定めた波形パラメータと対応するのである。このため採取したピーク点P’1〜のピーク情報から脈波波形を1拍分特定する必要がある。この特定処理は、次のようにして実行される。

まず、この特定にあたっては、脈波波形の特徴、すなわち、脈波波形の血圧値が心室収縮期の開始点に相当するピーク点P0において最低となり、その直後の駆出波に相当するピーク点P1において最高となる、という特徴を利用する。
そのため、マイクロコンピュータ401は、ピーク情報メモリ415から各ピーク点P’1、P’2、…、に対応した傾斜情報SLPおよびストローク情報STRKを順次読み出す。次いで、マイクロコンピュータ401は、各ストローク情報STRKの中から正の傾斜に対応したストローク情報(すなわち、対応する傾斜情報SLPが正の値となっているもの)を選択し、さらに、これらのストローク情報の中から値の大きなものを所定個数だけ抽出する。
すなわち、マイクロコンピュータ401は、第1に、極大点であるピーク点をとりあえず選択し、第2に、その中から直前ピーク点との変化分が大きいピーク点を抽出する。ここで、ピーク点を所定個数抽出しているが、これは、複数周期分について検討する趣旨である。

次に、マイクロコンピュータ401は、抽出したピーク点に対応するストローク情報STRKの中から中央値に相当するものを特定する。これにより、1拍分の脈波波形の立上部分(例えば、図4において符号STRKMによって示した部分)に相当するストローク情報が特定される。なお、この特定は、複数周期分の脈波波形についてピーク情報が採取されていることを前提としているから、測定異常と考えられるものを除外する趣旨である。
そして、マイクロコンピュータ401は、当該ストローク情報のピークアドレスよりも「1」だけ前のピークアドレスに相当するピーク点を、波形パラメータのピーク点P0とし、以下のピークアドレスに相当するピーク点を、順次、波形パラメータのP1〜P6と特定する。
例えば、図4でいえば、立上部分STRKMの直前に位置するピーク点P’6が、波形パラメータの算出基準となるピーク点P0と特定され、以下に続くピーク点P’7〜P’12が順次波形パラメータのピーク点P1〜P6と特定される。
すなわち、ピーク点P0を基準として時系列にピーク点P1〜P6が特定される。
このように各ピーク点P1〜P6は、最小極小点となるピーク点P0を基準として時系列に特定される。例えば、心室収縮期および心室拡張期を定めるピーク点P4は、ピーク点P0から時系列に数えて第2番目に現れる極小点として特定される。なお、各ピーク点P1〜P6は、最小極小点となるピーク点P0を基準として、値の大小関係で特定されても良い。例えば、ピーク点P4は、値の大きさが下から2番目の極小点、すなわち、ピーク点P0に次いで小さい極小点として特定されても良い。

<1−3−1−1−3:波形パラメータの算出>
マイクロコンピュータ401は、上記1拍分の脈波波形に対応した各ピーク情報を参照して各波形パラメータを算出する。例えば、ピーク点P’6〜P’12が、波形パラメータの基準となるピーク点P0〜P6と特定された場合、次のようにして求められる。

このようにして、血圧値差(y3-y2)および期間(t6-t4)はそれぞれ算出されて、評価許可部70や評価部72における評価の基礎とされることとなる。

また、ここで記憶された血圧値差(y3-y2)および時刻は、次回の測定において読み出され、駆出波の立ち上がり時間の変化率を算出する際の基礎とされることになる。

さて、上述のように初回の測定動作が終了し、前回の測定から一定時間経過すると再び同じ測定が実行されて、被験者が安静・平静状態にあれば、駆出波の立ち上がりに相当する血圧値差(y3-y2)とその時点における時刻とがそれぞれ評価許可部70により出力されることになる。
ここで、評価部72においては、第1に、データ記憶部71から最近記憶された血圧値差(y3-y2)とその時刻とが読み出され、第2に、現時点で算出された血圧値差(y3-y2)から読み出された血圧値差(y3-y2)を差し引いて、その測定期間における変化分が求められる。さらに、第3に、現時点の時刻から読み出された時刻を差し引いて、前回測定時から今回測定時に至るまでの経過時間が求められ、第4に、変化分が経過時間で割った商が求められて、退潮波における振幅の変化率が算出されることになる。

さらに、退潮波の振幅は、上述のように、拍出される血液の圧力特性と血液が拍出される側の血管特性との双方の影響を受けて形成されると考えられるから、血圧値差(y3-y2)そのものを告知する構成としても良い。

なお、本実施形態にかかる心機能診断装置においては、体動検出部20により被験者の体動成分を検出する構成となっていたが、被験者が体動していない状態を前提として診断するならば、脈波検出部10により信号MHは、そのまま脈波成分のみを示す信号MH’となるので、体動検出部20および波形処理部21は不要である。
くわえて、本装置においては、心室拡張期に相当する期間(t6-t4)から拍数を求める構成としたが、波形パラメータの時間t6から求めても良いのはもちろんであるし、また、被験者の拍数が十分に低い状態にあることを前提として診断するならば、拍数を求めるための構成そのものも不要となる。このため、拍数を検出するための拍数変換テーブル60や拍数を考慮に入れる評価許可部70は、任意的な要件である。
また、上述した実施形態にあっては、退潮波における振幅の変化率を現時点の値とデータ記憶部71に記憶された最近の値とにより求めたが、これに限られないのはいうまでもない。例えば、現時点の値と過去所定個数分前の値とにより求めても良いし、現時点から過去所定個数分までの平均値と過去の平均値とから求めても良い。
さらに、上記実施形態においては、退潮波を規定する指標として振幅を用い、この振幅を血圧値差(y3-y2)で評価したが、血圧値差(y3-y4)によっても評価可能であり、さらに、次の応用例で述べるように、退潮波の振幅を最大血圧値と最小血圧値と差で正規化したものは有効な評価指標になると考えられる。また、退潮波を規定する指標としては、他に、退潮波のピーク点が現れる時間に相当する期間t3や、退潮波の幅に相当する期間(t4−t2)、さらに、これらの時間を、心室収縮期に相当する時間t4で正規化したものも有効な評価指標になると考えられる。

上記実施形態では、駆出波の振幅に相当する血圧値差(y3-y2)を評価の指標として用いた。ここで、血圧値差(y3-y2)の意義について再検討してみると、上述したように血圧値差(y3-y2)は、心室の収縮によって拍出される血液の圧力特性と血液が拍出される側の血管特性とによって定まると考えられる。
しかしながら、圧力特性および血管特性は、個人毎による相違するのが一般的である。このため、血圧値差(y3-y2)は、同一被験者についての評価であればともかく、複数の被験者にわたっての相互評価は適切でないと考えられる。
そこで、血圧値差(y3-y2)を、脈波波形における最大血圧値と最小血圧値と差である血圧値差y1で正規した値を、すなわち、(y3-y2)/y1を、(y3-y2)に換えて(あるいは(y3-y2)とともに)評価の指標として用いることが適当である。

心機能診断装置に小型化・軽量化が要求される場合、図1および図2に示す構成を1個の装置として集約する構成では、その評価・診断も医師等の判断が入り込む余地がないものとなる。
そこで、被験者の脈波波形を検出した後、図1における評価許可部70に供給すべき血圧値差(y3-y2)あるいは(y3-y2)/y1を外部機器にデータ転送して、医師等の第三者が詳細な診断・解析を可能とする構成が考えられる。

一方、図8は、このような外部機器の構成を示すブロック図である。この図のように、外部機器は、機器本体600や、ディスプレイ601、キーボード602、プリンタ603などから構成されて、以下の点を除いて通常のパーソナルコンピュータと同じものである。
すなわち、機器本体600は、図1における評価許可部70以降の構成を構築するとともに、送信データを光に変換して送信するためのLED604を有する一方、受信光をデータに変換するためのフォトトランジスタ605を有している。これらLED604、フォトトランジスタ605には、心機能診断装置の通信I/F76に設けられるLEDおよびフォトトランジスタの特性と同一もしくは近似しているものがそれぞれ用いられる。ここでは、近赤外線タイプ(例えば中心波長が940nmのもの)が望ましい。そして、近赤外線タイプを用いる場合には、可視光を遮断するための可視光カット用のフィルタが、機器本体600の前面に設けられ、光通信用の通信窓606となっている。

このような構成にかかる心機能診断装置は、データ転送を行わない場合には、単独でも(y3-y2)あるいは(y3-y2)/y1の変化率に基づいて上述した被験者の心機能状態を評価・診断を行う一方、データ転送を行う場合には、ピーク点抽出・波形解析部40により算出された(y3-y2)あるいは(y3-y2)/y1と拍数変換テーブル60により変換された拍数とからなる組を、所定の測定期間分だけ通信I/F76のデータバッファに蓄積する。
ここで、医師等の第三者がキーボード601を操作して、データ要求を指示すると、外部機器は、データリクエスト信号をLED604を介して送信した後、データを受信するための待機状態となる。
一方、心機能診断装置側における通信I/F76のフォトトランジスタがデータリクエスト信号を受信すると、通信I/F76のデータバッファに蓄積された組データが、通信I/F76のLEDにより送信される。この組データをフォトトランジスタ605が受信することにより、外部機器では、被験者における退潮波の振幅に相当する血圧値差(y3-y2)あるいはそれを正規化した(y3-y2)/y1、および、その測定時間のデータが取得されることになる。
この後、外部機器では、図1における評価許可部70以降の構成を構築することにより、(y3-y2)あるいは(y3-y2)/y1の変化率に基づいて被験者の心機能状態を評価・診断や、蓄積したデータにより医師等の診断も可能となる。

<2:第2実施形態>
次に、本発明の第2実施形態に係る心機能診断装置について説明する。

<2−1:第2実施形態の理論的根拠>
前述のように、退潮波は、拍出される血液の圧力特性と血液が拍出される側の血管特性との双方の影響を受けて形成されると考えられる。一方、従来より、脈波波形を周波数解析して得られる周波数成分が当該被験者の生体的特徴を表す情報となる得ることが判明している(例えば、PCT/JP96/01254参照)。
このため、脈波波形のうち退潮波の周波数成分を解析することは、それなりの意義があると考えられる。

一方、本願発明者は、健常な成人74名に対して脈波を検出し、これらの脈波波形パラメータをそれぞれ求めるとともに、前述のPCT/JP96/01254と同様に、FFT処理して上記式(1)を用いて当該脈波波形の歪率dを求めた。
そして、本願発明者は、求めた歪率dと各波形パラメータ単体やこれらの差等との相関関係を個別に検討した結果、歪率dは、切痕波の振幅に相当する血圧値差(y5-y4)に対し、相関係数(R2)が0.77という高い相関関係を有することが判明した。この相関関係を図17に示す。
このことは、脈波波形における高次調波成分が血圧値差(y5-y4)を定める切痕波近傍に集中していることを意味する。すなわち、単純に脈波波形を周波数解析する構成では、退潮波近傍の周波数成分が、ほとんど切痕波近傍での成分の中に埋もれてしまう形となってしまう。
したがって、脈波波形のうちの退潮波の成分を解析するためには、脈波波形から退潮波近傍の成分を抽出する処理が必要であると考えられる。
そこで、この第2実施形態においては、第1に、脈波波形のうち、処理対象となる退潮波近傍部分を特定し、第2に、特定した退潮波近傍部分に対し、時間と周波数との両面から同時に捉える時間周波数解析、ここでは、ウエーブレット変換を行い、第3に、この解析結果に最も近似するものを、予め典型的な症状に対応して求めた解析結果の中から特定し、第4に、特定した症状に対応する診断内容を告知する構成とした。

<2−2−1:ウェーブレット変換器の構成>
ここで、ウエーブレット変換部200の詳細構成について説明する。
かかるウエーブレット変換部200は、上記式(2)の演算処理を行うものであり、その構成は図10に示すように次の要素からなる。すなわち、ウエーブレット変換部200は、マザー・ウエーブレットψ(x)を記憶する基底関数記憶部W1、スケールパラメータaを変換するスケール変換部W2、バッファメモリW3、トランスレートを行う平行移動部W4、および、乗算部W5から構成される。なお、基底関数記憶部W1に記憶するマザー・ウエーブレットψ(x)としては、ガボールウエーブレットの他、メキシカンハット、Haarウエーブレット、Meyerウエーブレット、Shannonウエーブレット等が適用できる。
また、ウェーブレット変換部200は、退潮波近傍に相当するピーク点P1からピーク点P4までの期間Tを示す信号を入力して、当該期間Tを8分割した時間間隔で脈波解析データを出力するようになっている。

<2−3:第2実施形態の動作>
次に、図9に示した第2実施形態の動作について説明する。
脈波検出部10により出力される信号MHには、被験者の体動に伴う体動成分が重畳されるが、体動成分除去部30により当該体動成分が除去されて、脈波成分のみを示す信号MH’となって、ピーク点抽出・波形解析部40に供給される。
ピーク点抽出・波形解析部40は、第1実施形態と同様に、信号MH’のデータを蓄積・解析して、脈波波形の波形パラメータを算出し、これらの波形パラメータから脈波波形における期間(t6-t4)を求めた後、さらに次の動作を行う。すなわち、図2において、ピーク点抽出・波形解析部40のマイクロコンピュータ401は、各波形パラメータを算出後、第1に、ピーク情報メモリ415から、ピーク点P1、P4に特定したピーク点の波形値アドレスをそれぞれ読み出し、第2に、セレクト信号S1を出力し、第3に、読み出したピーク点P1に相当するピーク点の波形値アドレスを、ピーク点P4に相当するピーク点の波形値アドレスまでサンプリングクロックφで歩進させて、これを波形メモリの読出アドレスADR4として出力する。これにより、波形メモリ402からは、ピーク点P1からピーク点P4までの脈波波形MD、すなわち、心室収縮期における退潮波近傍に相当する脈波波形MDが読み出されることとなる。

退潮波近傍に相当する脈波波形MDは、後述するように、ウェーブレット変換部200によってウェーブレット変換されて、この結果、時間領域および周波数領域において各8分割された脈波解析データMKDが生成される。この脈波解析データMKDは、さらに、周波数補正部210によって周波数補正されて、補正解析データMKD’となる。
一方、拍数変換テーブル60においては、ピーク点抽出・波形解析部40により算出された期間(t6-t4)が拍数に変換される。
次に、評価部80においては、第1に、歪率変換テーブル60による拍数とサンプル記憶部220に記憶されているサンプルを求めたときの拍数との差を求め、第2に、その差がしきい値以上であるか否かを判断して、しきい値以上であれば、診断を行わない旨の信号を出力する一方、しきい値以下であれば、解析補正データMKD’とサンプル記憶部220に記憶されているサンプルとを互いに比較し、解析補正データMKD’と最も近似するサンプルを判断して被験者の生体的状態を特定し、その状態を示す信号を出力する。
そして、特定された生体的状態に対応する診断内容(あるいは診断を行わない旨の内容)が、評価内容記憶部73から読み出され、第1実施形態と同様に、告知部74によって、読み出された内容が表示されたり音声等により通知されたりして、被験者に告知される。

<2−3−1:ウェーブレット変換器の動作>
ここで、ウェーブレット変換部200の動作について説明する。まず、基底関数記憶部W1からマザー・ウエーブレットψ(x)が読み出されると、スケール変換部W2は、スケールパラメータaの変換を行う。ここで、スケールパラメータaは周期に対応するものであるから、aが大きくなると、マザー・ウエーブレットψ(x)は時間軸上で伸長される。この場合、基底関数記憶部W1に記憶されるマザー・ウエーブレットψ(x)のデータ量は一定であるので、aが大きくなると単位時間当たりのデータ量が減少してしまう。スケール変換部W2は、これを補うように補間処理を行うとともに、aが小さくなると間引き処理を行って、関数ψ(x/a)を生成する。このデータはバッファメモリW3に一旦格納される。
次に、平行移動部W4はバッファメモリW3からトランスレートパラメータbに応じたタイミングで関数ψ(x/a)を読み出すことにより、関数ψ(x/a)の平行移動を行い関数ψ(x−b/a)を生成する。

脈波解析データMKDは、図11(b)に示すように、0Hz〜0.5Hz、0.5Hz〜1.0Hz、1.0Hz〜1.5Hz、1.5Hz〜2.0Hz、2.0Hz〜2.5Hz、2.5Hz〜3.0Hz、3.0Hz〜3.5Hz、3.5Hz〜4.0Hzといった8つの周波数領域に分割され、かつ、心室収縮期における退潮波近傍の始点たるピーク点P1からその終点たるピーク点P4までの期間Tを8分割した時間間隔で得られる。結局、脈波解析データMKDは、M11からM88までの64個のデータから構成されることとなる。
このようにして、生成された脈波解析データMKDは、周波数補正部210によって周波数補正が施され、補正解析データMKD’として出力され、評価部80において評価の基礎とされる。

なお、ウエーブレット変換においては、周波数分解能と時間分解能とは互いにトレードオフの関係にあるので、周波数分解能を犠牲にすれば、より短い時間間隔で脈波解析データを得ることもできる。
また、第2実施形態にあっては、心室収縮期の近傍に相当する脈波波形をウェーブレット変換して診断を行うこととしたが、さらに限定して、ピーク点P2からP4までとしても良い。
くわえて、第2実施形態にあっても、心室拡張期に相当する期間(t6-t4)から拍数を求める構成としたが、波形パラメータの時間t6から求めても良いのはもちろんであるし、また、被験者の拍数が十分に低い状態にあることを前提として診断するならば、拍数を求めるための構成そのものも不要である。このため、拍数を検出するための拍数変換テーブル60や拍数を考慮に入れる評価部80が、任意的な要件であることは、第1実施形態と同様である。

以上説明したように、第2実施形態に係る心機能診断装置によれば、被験者から検出した脈波波形のピーク点を抽出して、心室収縮期を特定し、その期間における退潮波近傍の脈波波形をウェーブレット変換する構成によって、単純に脈波波形を周波数解析する構成では切痕波近傍での周波数成分の中に埋もれてしまう退潮波近傍の周波数成分を求めることができるので、退潮波近傍での周波数成分から当該被験者の心機能状態を評価することが可能となる。さらに、脈波波形について1周期分以上、周波数解析処理をしなくて済むので、処理の負担を低減することができ、装置の小型化や簡易化などに大いに貢献することが可能となる。

上述した第2実施形態に係る心機能診断装置は、退潮波近傍に相当する脈波波形を解析するにあたりウェーブレット変換を用いたが、この第3実施形態に係る心機能診断装置は、周波数解析処理としてFFTを用いるものである。
ただし、ウェーブレット変換とは異なり、ただ単に退潮波近傍に限ってFFT処理する構成では、処理に伴う高調波成分が解析結果に現れてしまうので、本実施形態にあっては、脈波波形に対してその退潮波近傍に対応させた窓(ウィンドウ)関数を乗じて、その期間に相当する脈波波形をスムージングさせて抽出し、これをFFT処理する構成とした。

<3−2:第3実施形態の機能的構成>
図12は、本実施形態に係る心機能診断装置の機能構成を示すブロック図である。

窓関数読出部240は、生成した読出アドレスをサンプリングクロックφに同期して窓関数記憶部230に供給することによって、そこに記憶された窓関数を読み出すものである。詳細には、窓関数読出部240は、ピーク点P1からP4までに相当する脈波波形MDが波形メモリ402(図2参照)から読み出された場合に、ピーク点P1およびP4が窓関数のx=−πおよび+πにそれぞれ一致するように、読出アドレスを供給する。

ピーク点抽出・波形解析部40は、第1実施形態と同様に、信号MH’のデータを蓄積・解析して脈波波形の波形パラメータを算出し、これらの波形パラメータから脈波波形における期間(t6-t4)を求めた後、さらに、次の動作を行う。すなわち、図2において、ピーク点抽出・波形解析部40のマイクロコンピュータ401は、各波形パラメータを算出後、第1に、ピーク情報メモリ415から、ピーク点P1、P4に相当する波形値アドレスをそれぞれ読み出し、第2に、これを窓関数読出部240に供給し、第3に、セレクト信号S1を出力し、第4に、読み出したピーク点P1に相当するピーク点の波形値アドレスを、ピーク点P4に相当するピーク点の波形値アドレスまでサンプリングクロックφで歩進させて、これを波形メモリの読出アドレスADR4として出力する。これにより、波形メモリ402からは、波形の切り出し始点に相当するピーク点P1から切り出しの終期に相当するピーク点P4までの脈波波形MD、すなわち、心室収縮期における退潮波近傍に相当する脈波波形MDが読み出されることとなる。

そして、図14(a)に示すように、ピーク点抽出・波形解析部40の波形メモリ402から読み出されたピーク点P1からP4までの脈波波形MDには、同図(b)に示すように、当該脈波波形MDに対応してスケーリングされた窓関数が乗算部250によって乗算され、同図(c)に示される乗算結果がFFT処理部260によってFFT処理されることとなる。

くわえて、第3実施形態にあっても、心室拡張期に相当する期間(t6-t4)から拍数を求める構成としたが、波形パラメータの時間t6から求めても良いのはもちろんであるし、また、被験者の拍数が十分に低い状態にあることを前提として診断するならば、拍数を求めるための構成そのものも不要である。このため、拍数を検出するための拍数変換テーブル60や拍数を考慮に入れる評価部90が、任意的な要件であることは、第1および第2実施形態と同様である。

以上説明したように、第3実施形態に係る心機能診断装置によれば、被験者から検出した脈波波形のピーク点を抽出して、心室収縮期における退潮波近傍の脈波波形と、その脈波波形に対応してスケーリングした窓関数とを乗じて、この乗算結果をFFT処理する構成によって、単純に脈波波形を周波数解析する構成では切痕波近傍での周波数成分の中に埋もれてしまう退潮波近傍の周波数成分を求めることができるので、退潮波近傍での周波数成分から当該被験者の心機能状態を評価することが可能となる。さらに、脈波波形について1周期分以上、周波数解析処理をしなくて済むので、処理の負担を低減することができ、装置の小型化や簡易化などに大いに貢献することが可能となる。

<4−1:腕時計型A>
まず、各実施形態に係る心機能診断装置1を腕時計型とした場合の構成例について、図19を参照して説明する。
同図(a)および(b)に示すように、心機能診断装置1は、主に、腕時計構造を有する装置本体100と、この装置本体100に接続されるケーブル101と、このケーブル101の先端側に設けられた脈波検出部10とから構成されている。
このうち、装置本体100には、リストバンド102が取り付けられている。詳細には、リストバンド102の12時方向から被験者の左腕に巻き付いて、その他端が装置本体100の6時方向で固定されている。
装置本体100の6時方向には、また、コネクタ部103が設けられている。このコネクタ部103には、ケーブル101の端部となっているコネクタピース104が着脱自在に取り付けられている。
なお、このコネクタピース104を取り外すと、コネクタ部103には、同図(c)に示すように、ケーブル101との接続ピン111、112のほか、上述したデータ転送を行うためのLED113、フォトトランジスタ102が設けられている。

一方、脈波検出部10は、同図(b)に示すように、センサ固定用バンド11によって遮光されながら、被験者の人差し指の根本に装着される。このように、脈波検出部10を指の根本に装着すると、ケーブル101が短くて済むので、装着しても邪魔にならない。また、掌から指先までの体温の分布を計測すると、寒いときには、指先の温度が著しく低下するのに対し、指の根本の温度は比較的低下しない。したがって、指の根本に脈波検出部10を装着すれば、寒い日に外出しても、脈波波形を正確に検出できる。
また、装置本体100の表面側には、液晶パネルからなる表示部110が設けられている。この表示部110は、セグメント表示領域や、ドット表示領域などを有し、現在時刻や診断内容など表示する。すなわち、表示部110は、各実施形態における告知部74に対応している。

一方、装置本体100の内部には、図示せぬ加速度センサが組み込まれており、被験者の腕の振りや、体の上下動によって生じる体動を検出している。すなわち、この加速度センサが、各実施形態における体動検出部20に対応している。

<4−1−1:脈波検出部の詳細構成>
次に、脈波検出部10の構成について図20を参照して説明する。
この図に示すように、脈波検出部10は、LED12、フォトトランジスタ13などから構成される。スイッチSWがon状態となり、電源電圧が印加されると、LED12から光が照射される。この照射光は、被験者の血管や組織によって反射した後に、フォトトランジスタ13によって受光される。したがって、フォトトランジスタ12の光電流を電圧に変換したものが、脈波検出部10の信号MHとして出力される。

ここで、LED12の発光波長は、血液中のヘモグロビンの吸収波長ピーク付近に選ばれる。このため、受光レベルは血流量に応じて変化する。したがって、受光レベルを検出することによって、脈波波形が検出されることとなる。
また、LED12としては、InGaN系(インジウム−ガリウム−窒素系)の青色LEDが好適である。青色LEDの発光スペクトルは、例えば450nmに発光ピークを有し、その発光波長域は、350nmから600nmまでの範囲にある。この場合、かかる発光特性を有するLEDに対応させてフォトトランジスタ13として、GaAsP系(ガリウム−砒素−リン系)を用いればよい。このフォトトランジスタ13の受光波長領域は、例えば、主要感度領域が300nmから600nmまでの範囲にあって、300nm以下にも感度領域がある。
このような青色LEDとフォトトランジスタとを組み合わせると、その重なり領域である300nmから600nmまでの波長領域において脈波が検出される結果、以下の利点がある。

まず、外光に含まれる光のうち、波長領域が700nm以下の光は、指の組織を透過しにくい傾向があるため、外光がセンサ固定用バンドで覆われていない指の部分に照射されても、指の組織を介してフォトトランジスタ33まで到達せず、検出に影響を与えない波長領域の光のみがフォトトランジスタ33に達する。一方、300nmより低波長領域の光は、皮膚表面でほとんど吸収されるので、受光波長領域を700nm以下としても、実質的な受光波長領域は、300nm〜700nmとなる。したがって、指を大掛かりに覆わなくとも、外光の影響を抑圧することができる。また、血液中のヘモグロビンは、波長が300nmから700nmまでの光に対する吸光係数が大きく、波長が880nmの光に対する吸光係数に比して数倍〜約100倍以上大きい。したがって、この例のように、ヘモグロビンの吸光特性に合わせて、吸光特性が大きい波長領域(300nmから700nm)の光を検出光として用いると、その検出値は、血量変化に応じて感度よく変化するので、血量変化に基づく脈波波形MHのS/N比を高めることができる。
