【発明の属する技術分野】
本発明は、内視鏡先端部に先端カバーが装着される内視鏡に関する。

【従来の技術】
周知のようにガストロスコープ、十二指腸スコープ等の医療用内視鏡は、挿入部を患者の体腔内に挿通して、患部の観察や処置などを行なうものである。内視鏡で患部を観察しながら鉗子などの処置具を用いて生検組織の採取や治療を行う際、所望の患部に鉗子を対向させる機構として内視鏡には処置具起上装置が設けられているものがある。

この処置具起上装置は、基本的には内視鏡先端部に設けた起上軸を中心に回動可能な処置具起上台と、この処置具起上台に連結された操作ワイヤとで構成されており、この操作ワイヤを手元側に設けた操作部で進退動作させることにより、処置具起上台の角度を適宜調整して鉗子の向きを所望の方向に定められるようになっている。

また、内視鏡使用時、内視鏡先端部及び内視鏡挿入部が体腔内に挿入されて体液などに触れるため、前記処置具起上台の起上軸や、先端構成部との摺動部、操作ワイヤの端部に接続されている操作ワイヤ固定部材などに体液が接触する。従って、内視鏡使用後、感染症などを防止するために、内視鏡の各部を洗浄しなければならなかった。しかし、処置具起上装置には操作ワイヤ固定部材や起上軸など細かな部材が多いので、洗浄には大変手間がかかっていた。

このため、実公平2−25361号公報には挿入部の先端硬質部を本体部と、この本体部に着脱可能な先端キャップとで構成した先端カバー着脱式の内視鏡が示されている。また、本出願人は特願平5−333219号に着脱式鉗子起上台付きの先端カバーを提案している。

【発明が解決しようとする課題】
しかしながら、実公平2−25361号公報に示されるような先端カバー(先端キャップ)が着脱式の内視鏡では、先端カバーを着脱式の構造にするにあたり、先端カバーの内視鏡先端部への装着性を優先すると、この先端カバーが内視鏡先端部から外れ易くなってしまうという問題があった。逆に、先端カバーを内視鏡先端部から外れ難くすると、今度は先端カバーの内視鏡先端部への装着性が悪化するという問題があった。

このため、前記特願平5−333291号では先端カバーと内視鏡先端部とを装着する際、先端カバーが接する内視鏡先端部に形成した係止部の先端側全体に傾斜部を設けて、装着性を向上させるようにしていた。しかし、図17に示すように前記内視鏡先端部101の係止部102の先端側全体に傾斜部103を設けると、前記内視鏡先端部101に先端カバー104を装着したとき、この先端カバー104と前記係止部102との間に傾斜部103の傾斜面によって形成される周状の凹部105が形成されてしまい、この凹部105が形成されることによって体腔内への挿入性が低下するという問題があった。

また、図18に示すように先端カバーが外された内視鏡110では、洗滌時、指で鉗子台111を先端構成部112より下に押し下げて露出させ、鉗子台周りを洗滌するようにしていた。しかし、鉗子台111を先端構成部112より下方に押し下げるとき、操作ワイヤ113が鉗子台ガイド壁114内面と鉗子台111との間に入り込んで、操作ワイヤ113に食いつきが発生するおそれがあった。このため、鉗子台111と鉗子台ガイド壁114内壁面との間に操作ワイヤ113が入り込むことのない内視鏡が望まれていた。

本発明は上記事情に鑑みてなされたものであり、先端カバーと内視鏡先端部との装着性が良く、先端カバーを内視鏡先端部に装着したとき、先端カバーと内視鏡先端部との装着部分の外表面に段差部が無く挿入性に優れ、先端カバーの内視鏡先端部からの脱落の無い係止機構を有する内視鏡を提供することを目的としている。

【課題を解決するための手段】
本発明の内視鏡は、内視鏡先端部と、この内視鏡先端部に装着される先端カバーとを備える内視鏡において、前記内視鏡先端部の外周に設けられ、装着される前記先端カバーの基端部が突き当たる突き当て部よりも先端側で前記内視鏡先端部の周方向に形成され、均一な高さを備えて突き出した係止凸部と、前記係止凸部と前記突き当て部との間に、前記係止凸部に対して低い段差として前記内視鏡先端部の周方向に形成された係止用溝部と、前記係止凸部の周方向端部側に設けられ、前記係止凸部の先端側面より先端側に向けて形成された傾斜面を有する傾斜凸部と、前記先端カバーに設けられ、該先端カバーが前記内視鏡先端部に装着された際に、前記係止用溝部に収まる外形部分を備えるとともに前記傾斜凸部に乗り上げて前記係止用溝部に係合する係止片と、前記先端カバーに設けられ、該先端カバーが前記内視鏡先端部に装着された際に、前記係止凸部に嵌合可能となる大きさを有するスリット部と、前記傾斜凸部に対応した位置の前記先端カバーに設けられ、前記傾斜凸部を納めるための凹部と、を具備している。

【発明の実施の形態】
以下、図面を参照して本発明の実施の形態を説明する。
図1ないし図8は本発明の第1実施形態に係り、図1は内視鏡装置の概略構成を示す説明図、図2は先端カバーと先端構成部との装着性を向上させる傾斜部を設けた先端構成部を示す斜視図、図3は係止片を設けた先端カバーを示す斜視図、図4は内視鏡先端部の鉗子台を押し下げた状態を示す図、図5は鉗子台操作装置の概略構成を示す説明図、図6は図3のA部の拡大図、図7は先端カバーを先端構成部に装着したときの逃がし部を示す図、図8は先端カバーを窓側から見たときの斜視図(一部断面図)である。

図1に示すように内視鏡装置1は、固体撮像装置(不図示)を内臓した内視鏡2と、この内視鏡2の先端部に照明光を供給する光源装置3と、内視鏡2の撮像素子から伝送される電気信号を映像信号に処理する信号処理手段を有するビデオプロセッサ4と、このビデオプロセッサ4により信号処理された映像信号による被写体像を表示するモニタ5と、前記映像信号を記録再生するVTR6と、前記モニタ5に表示された被写体像を印字するビデオプリンタ7と、前記映像信号を記録する大容量の記憶装置であるビデオディスク8などから構成されている。

前記内視鏡2は、体腔内に挿通される挿入部9と、この挿入部9の後方に位置する操作部10及びこの操作部10の側部から延出するユニバーサルコード11などを備えて構成されている。

図2に示すように前記内視鏡2の挿入部9の先端側に位置する内視鏡先端部12は、患部の観察を行なう対物レンズ13と、患部など体腔部内を照明する照明レンズ14と、前記対物レンズ13及び照明レンズ14に付着した汚物や体液などを洗浄するための洗浄液や各レンズ13,14に付着した水滴などを吹き飛ばす空気を送るノズル15と、内視鏡内を挿通して先端部から突出する処置具(不図示)を支持する鉗子台16と、この鉗子台16に一端が接続され他端が内視鏡2の操作部10に設けた後述する鉗子台操作装置に連結される操作ワイヤ17と、後述する先端カバーを係止する係止機構を形成する係止リング18と、図示しない複数の湾曲駒を連接してなる湾曲部を覆うと共に前記係止リング18の後端部に被覆される湾曲ゴム19と、前記操作ワイヤ17を牽引操作して鉗子台16を回動させたとき鉗子台16の回動方向に対して直交する外周方向への位置ずれを防止する鉗子台ガイド壁20などを先端構成部21に配設して構成されている。この先端構成部21には図3に示す先端カバー51が装着されるようになっている。

なお、前記先端構成部21の各稜線部分には符号21aに示すような面取りが施されており、先端カバー非装着時に稜線部分のエッジによる操作者の手指などへの損傷を防止している。

まず、内視鏡先端部側の係止機構について説明する。
図2に示すように前記先端カバー51を係止する係止機構を形成する先端リング18は、生体適合性を有する材質である例えば、ポリスルホンなどで成型されており、生体適合性を有する接着剤で先端構成部21に一体的に接着固定されている。

前記先端リング18には内視鏡先端部側の係止機構の凹部となる係止溝26を形成するための係止凸部22が周方向一部に設けられている。

すなわち、係止凸部22と先端リング18の間に設けられた付き当て部25との間に、内視鏡先端部側の係止機構の凹部となる係止溝26が形成される。この係止溝26には、図3に示す先端カバー51の後述する被係止機構の凸部となる係止片51が係入するようになっている。

前記係止凸部22には前記先端カバー51の係止片52の係止溝26への装着性を向上させるため、先端カバー51の係止片52を係止溝26に案内する傾斜部となる傾斜凸部27が設けられている。この傾斜凸部27は、前記係止溝26に前記係止片52を係入する際、この係止片52と最初に当接するように、係止凸部22より先端側に突出して形成されている。

また、前記傾斜凸部27は、前記係止片52が当接して滑らかに係止凸部22を乗り越えて係止溝26に係入するようにするため、先端側下面から係止凸部22の上面に向かって上昇する傾斜面として形成されている。なお、この傾斜凸部22の幅寸法を5mm以下に設定することが望ましい。

前記係止凸部22の外形寸法は、先端カバー51の外形寸法と略同寸法、あるいはやや大きな外形寸法に形成されている。また、係止リング18には、操作ワイヤ17の出口近傍23での肉厚を確保するための異形部24が設けられている。

なお、前記係止リング18の最小肉厚は、先端カバー51の繰り返しの装着によって剥離することの無い耐性を備える約0.3mmに抑えられているので、薄肉でありながら十分な強度を有する。また、前記係止リング18は、先端カバー51を先端構成部21に装着したとき、先端カバー51と係止リング18との識別を容易に行うことが可能なように先端カバー51と異なる色に着色されている。さらに、前記湾曲ゴム19に配設されている付き当て部25の外形寸法は、前記係止リング18の外形寸法と略同寸法で形成されている。

一方、図4に示すように鉗子台16は、洗浄の際、指で押し下げることにより先端構成部21より下側に押し下げられていくが、操作ワイヤ17が鉗子台ガイド壁20内面と鉗子台16との間に入り込んで操作ワイヤ17に損傷を与えることを防止するため、鉗子台16と鉗子台ガイド壁20との隙間xが操作ワイヤ17の直径寸法以上に開くことなく鉗子台16が停止するようになっている。

前記鉗子台16を、隙間xが、操作ワイヤ17の直径寸法以上に開かない位置で停止させるため、鉗子台操作装置30に図5に示すようなワイヤ侵入防止手段を設けている。

すなわち、図に示すように鉗子台16を回動操作する鉗子台操作装置30は、軸31を中心に回動する操作部10に設けた操作ノブ32と、この操作ノブ32に連結された連結棒33と、この連結棒33に接続された前記操作ワイヤ17を進退させることで鉗子台16を回動させる押引部34と、この押引部34を内視鏡の長手方向に動かすように規制するガイド35などによって主に構成されている。

このガイド35は、前記押引部34の周りを囲むように内視鏡の長手方向に平行に配設されており、前記押引部34が先端方向に移動することによってこの押引部34に設けられている段部34aがガイド35の先端側壁35aに当接する。すなわち、ワイヤ侵入防止手段として前記押引部34がガイド35の先端側壁35aに当接するまでの操作ワイヤ17の移動距離を設定することによって、前記隙間xが操作ワイヤ17の直径寸法以上に開かないようにしている。

次に、先端カバー51側の被係止機構について説明する。
図3に示すように前記係止リング18の係止凸部22と付き当て部25とで形成された係止溝26を備えた先端構成部21に装着される先端カバー51には、前記先端構成部21の係止凸部22を乗り越えて係止溝26に係入する弾性を有する係止片52が2箇所設けられている。

この係止片52の外形形状は、前記係止溝26に収まるように、この係止片52の長さ・幅寸法を係止溝26の長さ・幅寸法に対して略同寸法か、やや小さな寸法に設定して形成されている。

また、前記係止片52が係止溝26に係入されたとき、この係止片52が先端カバー51の他の部分より出っ張っていると、先端構成部21に先端カバー51を装着した内視鏡を体腔内に挿入する際、出っ張っている係止片52が何らかに引っかかったとき、引っ張られて係止溝26から外れて、先端カバー51が先端構成部21から脱落するおそれがある。このため、前記係止片52の外形寸法を、先端カバー51の他の部分の外形寸法に対して同寸法、または出っ張ることが無い寸法に設定すると共に、前記先端構成部21の係止凸部22の外形寸法に対しても同寸法、または出っ張ることの無い寸法に設定している。

前記先端カバー51に係止片52を設けるため、この先端カバー51にはスリット部53が設けられている。このスリット部53には、前記先端構成部21に設けた係止凸部22が隙間無く収まるようにするため、図6に示すように前記係止凸部22に設けた傾斜凸部27を収めるための凹部54を、前記係止凸部22に設けた傾斜凸部27に対応する位置に設けている。なお、このスリット部53の長さ・幅寸法を、前記係止凸部22の長さ・幅寸法に対して略同寸法か、やや大きな寸法に設定している。前記係止片52の根本部近傍55の内周面側にはこの係止片52に所望の曲げ性(弾性力)を持たせるため、一部分を所定の厚さに薄肉に形成したヒンジ56が設けられている。

一方、図7示すようにこの先端カバー51の内面には、この先端カバー51を先端構成部21に装着したとき、前記鉗子台16の一部と先端カバー51内面とが当たることがないように逃げ部60が形成されている。

また、図8に示すようにこの先端カバー51には、前記先端構成部21の異形部24に対応するように変形部57が設けられると共に、前記先端構成部21に設けた対物レンズ13,照明レンズ14や鉗子台16が対向する窓58が設けられている。

なお、この窓58の後端側には高周波処置具使用時、この高周波処置具(不図示)と先端構成部21との間でのショートを防止するための絶縁部59が設けられている。また、前記係止片52の係止凸部22に対しての係止強度を確保するため、この係止片52の幅寸法及び肉厚を、それぞれ幅1mm以上で肉厚0.5mm以上で形成している。さらに、前記係止片52の端部52aを、それぞれ外周面に対し45°から90°の範囲で同角度に形成している。又、本実施形態では係止片52を複数設けるようにしているが、係止片を1つだけ設ける構成であってもよい。更に、前記係止凸部22及びスリット部53の係止強度を向上させるため、前記係止凸部22及びスリット部53を周面に対してほぼ垂直に突出するように形成している。

上述のように構成した先端カバー51の先端構成部21への装着について説明する。
先端カバー51を先端構成部21すなわち内視鏡先端部12へ装着する際まず、先端構成部21の対物レンズ13、照明レンズ14が配設されている面と、先端カバー51の窓58の向きとを一致させる。そして、先端構成部21の長手方向に先端カバー51を徐々に挿入していく。すると、先端カバー51に設けられている係止片51の先端側端面が、先端構成部21の係止凸部22に設けられている傾斜凸部27の傾斜面に当接する。

ここで、先端カバー51をさらに先端構成部21に押し込んでいくと、前記係止片52が傾斜凸部27の傾斜面に沿って移動していく。傾斜部27が係止片52の根元側に位置しているため、係止片52のほぼ全体が持ち上げられ、また、係止片52の根本部近傍55にヒンジ56が設けられていることによって、前記係止片52が長手方向に直交する外周方向に持ち上げられるように曲げられていく。これにより、装着性が向上する。

そして、更に先端カバー51を押し込む。すると、前記係止片52が係止凸部22を乗り越え、前記ヒンジ56の有する復元力によって係止片52が係止溝26に収まると共に、先端構成部21の異形部24と先端カバー51の変形部57とが嵌合して先端カバー51と先端構成部21との装着が完了する。

なお、検査終了後、鉗子台周りの洗浄を行なう際は、操作ノブ32をいっぱいに動かして鉗子台16を倒置させ、その後、鉗子台16、先端構成部21などを流水中でブラシを用いて洗浄する。そして、すすぎを行った後、消毒を行う。

このように、係止凸部の一部に係止片を係止溝に案内する傾斜凸部を設けたことにより、先端カバーを先端構成部に押し込んで装着するとき、係止片を根本部近傍から持ち上げながら係止凸部を乗り越えさせて、係止溝に係入することができる。このことにより、先端カバーを先端構成部に押し込むだけで、先端カバーの内視鏡先端部への装着がスムーズに行える。

また、前記係止片の外形寸法を、先端カバーの外形寸法及び係止凸部の外形寸法に対して同寸法、または出っ張ることの無い寸法に設定する一方、係止片の長さ・幅寸法を係止溝の長さ・幅寸法に対して略同寸法か、やや小さな寸法に設定すると共に、スリット部の長さ・幅寸法を、係止凸部の長さ・幅寸法に対して略同寸法か、やや大きな寸法に設定したことにより、先端カバーを内視鏡先端部に装着したとき、図15に示すように係止片だけが突出することや外表面の凹部を無くすことができる。このことにより、先端カバーを装着した内視鏡の体腔内への挿入がスムーズに行える。

さらに、前記係止片の根元部近傍にヒンジを設けて、係止片に所望の弾性力を持たせたことにより、係止片の長手方向に直交する外周方向へスムーズに曲がるので先端カバーの装着性が大幅に向上する。

又、先端構成部に設けた異形部に対応するように先端カバーに変形部を設けたことにより、先端カバーを先端構成部に装着したとき、先端カバーが先端構成部に対して回転することを防止することができる。

更に、鉗子台と鉗子台ガイド壁との間の隙間を操作ワイヤの直径寸法より大きく開かないように、押引部がガイドの先端側壁に当接するまでの操作ワイヤの移動距離を設定しているので、操作ワイヤが鉗子台ガイド壁の内側に入り込むのを防止することができる。このことにより、操作ワイヤが鉗子台と鉗子台ガイド壁との間に入り込んで発生する操作ワイヤの食い付きがなくなる。
