【発明の属する技術分野】
本発明は、治療用器具に関し、特にアトピー性皮膚炎などの早期治癒を促進する治療用カラーおよびカラー付衣服に関する。

【従来の技術】
アトピー性皮膚炎とは大気汚染、食物汚染、ハウスダスト、ダニといった生活環境や、精神的、肉体的ストレスによる刺激、及び皮膚の抵抗力を含む体質等を原因とするアレルギー性疾患の一種である。その症状は皮膚に生ずる湿疹であり、特に顔や首周りの皮膚に生ずるものは強いむず痒さを伴う。大人だけでなく、乳幼児や子供にその症状が多く見られ近年急増している。

特に乳幼児や子供のアトピー性皮膚炎の悪化原因として、睡眠状態での掻きむしりがあげられる。これは、睡眠中の皮膚温上昇に伴って痒みが増すことにより、患者が無意識のうちに手指で患部を引っ掻いてしまうというものである。このことにより掻痒部に新たな皮膚の損傷を生じ、皮膚表面に常在する黄色ブドウ球菌などの細菌感染を引き起こすため、その炎症が更に痒みを誘発するという悪循環を招く。乳児には介護者や親が掻かないように指示しても言葉自体が理解できない。幼児の場合には言葉は理解できても、眠ってしまえば無意識に患部を掻きむしってしまうので指示しても無駄である。

この掻きむしり行為の対策用具として患者の手指を覆う手袋が知られている。この手袋は柔らかい綿などの布地で構成されており、上記の掻きむしり行為の際に患部を極力傷めないようにするものである。更に、二重の袋材によって手指を覆う構造の手袋もある(特開平9−59809参照)。これらの発明は爪や指による直接の接触をさせないため、患部の損傷を幾分か和らげることができる。しかし、これらは布地ごしであっても手指が患部に触れることを許すため、掻きむしりによる悪循環を完全に断ち切ることはできない。したがって、その治療効果は満足のいくものではなかった。

なお、掻きむしりによる悪循環のメカニズムは以下の通りである。患者が患部を掻きむしることで皮膚状態の悪化を招き、角層のバリアー機能が低下して刺激因子の侵入が容易となる。そこに侵入した抗原が免疫異常を引き起こし、同時にアレルギー炎症反応を増強させる。そして、この炎症による皮膚状態の悪化がさらに皮膚の角層のバリアー機能を低下させる原因となり、悪循環が繰り返されるのである。この悪循環を完全に断ち切ることなしに、アトピー性皮膚炎を軽快させることは不可能といってよい。

【発明が解決しようとする課題】
アトピー性皮膚炎の治療で最も重要なことは、掻きむしりによる悪循環を完全に断ち切ることにある。そのためには手指と患部とを物理的に遮断して手指が患部に届かないようにすればよい。

本発明の課題は、患者の手が直接患部に触れることを物理的に遮断することで上記悪循環を完全に断ち切り、もって患部の炎症の悪化を防止し、症状の早期改善をもたらす治療用器具を提供することにある。

【課題を解決するための手段】
その課題を解決するための手段が請求項に対応して表現される次の記載中に現れる()つきの数字は、請求項の記載事項が詳しく後述される実施の複数の形態のうちの少なくとも1つの形態の部材、工程、動作に対応することを示すが、本発明の解決手段がそれらの数字が示す実施の形態の部材に限定して解釈されるためのものではなく、その対応関係を明白にするためのものである。

本発明による治療用カラー(1)は、扇型の形状をなし、プラスチック、ポリエチレンなどの熱可塑性合成樹脂製材から形成され、その内周部には、表面が綿素材からなるへり(4)が取りつけられており、その端部(2)を他の端部(3)に接合することで患者の首の周りに笠状に取り付けることができ、後面に2つ、前面に2つの取っ手(19a,19b,19c,19d)をつけ、後面の2つの取っ手(19a,19b)に紐状の部材(20)をそれぞれ取り付け、それらを患者の脇の下を通して前面の2つの取っ手(19c,19d)とそれぞれ接合することで、治療用カラー(1)を患者の首の周りに取り付けることができるものである。

また、前記の治療用カラー(1)は、透明な部材で形成されたものであることが望ましい。また、前記の治療用カラー(1)の扇型の部材に直径3mmほどの空気穴(10)を開けたものとしてもよい。

【発明の実施の形態】
以下、本発明の治療用カラーおよびカラー付衣服の具体的な構成を図面に従って詳述する。

図1は本発明のカラー1の展開図である。図1に示すようにカラー1の形状は扇形である。カラー1の端部2には凹の止め具5が2個付加されている。カラー1のもう一方の端部3には凸の止め具6が2個ずつ3列、計6個付加されている。また、カラーの内周部にはヘリ4が接続されている。ヘリ4はカラー1を首の周囲に適度な間隔をもって保持するためのもので、その首に接する側の表面には綿100%の素材を用いている。なお、へり4がカラー1の内周部のすべてにわたって接続されておらず一部途切れているのは、患者ののど部分に対する圧迫を防ぐための工夫である。

カラー1を巻いてその端部2をもう一方の端部3と重ねあわせ、前記止め具5,6を接合することにより、カラー1は電球の笠状の形状となる。図2にカラー1を患者の首の周りに巻いた様子を示す。なお、凸の止め具6が3列あるのは、患者の頚部の太さに合わせて巻き具合を3段階に調節可能とするためである。もちろん、これらの止め具は着脱可能な部材であれば何でも良く、たとえばホック、面ファスナー、紐、ベルトなどでもよい。面ファスナーであれば着脱がより容易であると共に、無段階のきめ細かい調節が可能となる。

カラー1のサイズは、さまざまな患者の頭囲、首周りに対応するため、Sサイズ、Mサイズ、Lサイズの3通りとした。Sサイズの大きさは、外径38cmほど、内径15cmほど、幅11.5cmほどである。重さは80gほどである。また、Mサイズの大きさは、外径44cmほど、内径18cmほど、幅13cmほどである。重さは90gほどである。また、Lサイズの大きさは、外径54cmほど、内径23cmほど、幅15.5cmほどである。重さは120gほどである。

このカラー1の幅は、患者の手を患部に届かせないために必要な長さである。この幅を十分に確保することで、カラー1が患者の顔のまわりに防護壁となって手が患部に届くことを物理的に許さないため、掻きむしり行為を完全に遮断できるのである。もちろん、患者の体格によってはこれ以外のサイズのものを用いてもよい。

カラー1の材質はプラスチック、ポリエチレンなどの熱可塑性合成樹脂材からなり、その厚さは1mmほどである。この素材は柔軟性、弾力性が十分にあるため、患者の顔や首周りに優しくフィットする。また、丸めると自然に元に戻ろうとするため、常に内径が拡がる方向に力が作用することになり、へり4が首をしめつけることはない。また、軽量であるため患者の首に負担とならない。また、ヘリ4の表面を100%綿製とすることにより、患者の皮膚への刺激を軽減することができる。

図2はカラー1を患者の首周りに装着して前面より見た場合の図である。図2に示すように、カラー1の左右側がなす角度は30度ほどが望ましい。これは患者が手をのばしても顔まで至らず、顔に接触できないようにするために有効な角度である。

図3は同じくカラー1を患者の首周りに装着して側面より見た場合の図である。図3に示すように、カラー1の後面側がなす角度は60度ほど、前面側がなす角度は20度ほどが望ましい。この角度を保つことで顔とカラー1との間に適度な空間が生じるため、カラー1が顔面に接触することがなくなる。また、仰臥位に寝かせた時にも十分に呼吸の確保ができる。また、顔面付近が十分に開放されているため患者の表情がよく見え親も介護し易い。

また、カラー1を透明のものとすることにより、患者の視界を確保することができる。このことにより、患者は周りの様子が良く見えるため閉塞感や恐怖感に陥らないですみ、親の顔もよく見えるので安心できる。また、介護者も親も患者の表情や患部の状態を十分に観察できる。

図4はカラー1に装着するカバー7の平面図である。図に示す様にカバー7の形状はカラー1と同様の扇形だがその中心角はカラー1よりも小さくてもよい。カバー7は外周縁を縫い合わせた二重の綿布で形成され、カラー1をその裏表両面から包む形態になっている。カバー7の内周部はカラー1の内周部に一致している。カバー7の内周部の裏表両面には各々2cm幅の見返し部分8を持ち、各々その内部にゴム9が通してある。なお両端部は縫合せず、開いた状態となっている。なお、カバー7が覆うのはカラー1のすべてではなく、患者の髪があたる範囲だけであってもよい。

カバー7のサイズはカラー1と同様、Sサイズ、Mサイズ、Lサイズとした。これにより、カラー1のサイズに応じて適切なものを選ぶことができるため、カバー7を適度な緊張をもってカラー1上に展開することができ、シワやドレープの発生を未然に防ぐことができる。カバー7にシワやドレープが生じると顔や首周りへの摩擦が生じ、それが湿疹患部の症状悪化につながってゆく。したがって、カバー7が適度な緊張をもってカラー1上に張られていることは大切である。また、カバー7を装着する他の効果として、湿疹患部からの浸出液を吸収することがあげられる。更に、患者の後頭部や頭髪がカラー1と接する際の摩擦を和らげることができる。なお、カバー7の内周側にゴム9を通した構造としたことで、カバー7の装着操作が容易となっている。

図5はカラー1の全面に直径3mmの空気穴10を開けたことを示す図である。これにより、患者が寝返りをうって鼻や口がカラー1に直に接した時などにも十分な呼吸を確保することができる。なお、穴の直径を3mmとした理由は、患者の指が誤って空気穴に入り込まないための工夫である。これよりも大きいと穴に指が入り込み、指先を切るなどの怪我を生じる可能性があるからである。もちろん、これらの空気穴10を開けるかわりにカラー1を通気性のある素材で形成しても同様の効果が得られる。

図6はカラー1の外縁を綿製テープ11で覆い保護したことを示す図である。これにより、患者および患者に接する人が手を切傷しないで済む。また、外縁を綿製テープ11で覆うことのかわりにカラー1の外縁を丸く研磨加工しても同様の効果が得られる。

衣服12は前あき型のラグランスリーブ型であり、その前面には上下にファスナー13が取り付けられている。そしてその襟の見返し部分14には凹の止め具15が取り付けられている。一方、図8に示すように、カラー1の内周部にあるヘリ4には凸の止め具16が取り付けられている。これらの止め具をお互いに接合することにより、カラー1を患者の首の周囲に適度な間隔をもって取り付けることができる。ここでいう適度な間隔とは1〜1.5cmほどの空隙のことをいう。その詳しい様子を図9に示す。この間隔を保持することにより、患者の安全な呼吸を確保することができ、しかも患者の指の侵入を阻止することができるのである。なお、患者の年齢、体格による首の太さの個人差は、適切なサイズのカラー1を選択することで吸収する。

また、カラー1を衣服12に止め具15、16をもって接合することにより、カラー1が患者の首の周りに回転することを防止できる。このことにより、患部に対して不必要な刺激を発生しないですむ。

また、カラー1と衣服12を一体とせず着脱可能にしたことで各々の自由度が増し、衣服12の洗濯も容易となる。もちろん、これらの止め具は着脱可能な部材であれば何でも良く、たとえばホック、面ファスナー、紐、ファスナーなどでもよい。面ファスナーであればカラー1と衣服12との着脱がより容易となる。

図10は衣服12にカラー1を装着した場合の斜視図である。衣服12の両脇下部には長さ17cmほどの紐17aと17b、17cと17dとが、それぞれ15cmほどの間隔を開けて2本ずつ取り付けられている。これらの紐17aと17b、17cと17dとを両脇の下で縛ることで衣服12の首周りとの間の布地に緊張を生じ、首周り部分のたるみの発生を抑制する。これによりカラー1が移動した際の頚部への圧迫を未然に防ぐことができる。また、患者の体格差によって生じる衣服12の体幹部のたるみを調節することもできる。

なお、この治療用カラー1と衣服12の襟部分とを縫合し、衣服と一体としてもよい。この場合には、両脇の下の紐17aと17b、17cと17dを使用することなく首周りへのくい込みを防止することができるため、十分に安全な呼吸を確保できる。

衣服12の開閉手段はファスナー13以外にボタン、ホック、面ファスナー、クリップ、紐等でも良いが、ファスナーが介護操作上簡便である。また、患者は通常インナーとして肌着、寝間着を着用しているため、衣服12はそれらの上から着用することになる。したがって衣服12には大きさにゆとりのあるラグランスリーブ型が最適である。なお、衣服12の材質は患者の皮膚への刺激が少ない100%綿製のものとすることが望ましい。

衣服12にはラグランスリーブ型だけでなく、上下衣が独立したパジャマ、襟元下を重ね合わせる寝間着状のもの、頭側から被って着用する袖付きのもの、上下衣が一体となったロムパース、寝間着代わりに使用するものなど、全ての形態の衣服が対象となる。いずれにしてもカラー1を首の周囲に適度な間隔をもって取り付けることができ、患者の体にゆったりとしたものであればどのようなものであってもかまわない。

図11は衣服12の襟ぐりの外側にフード18が縫着された状態を示す外見図である。フード18の大きさは幅28cm、深さ37cmほどであり、外縁の15cmほどの折り返し部分が二重構造となっている。フード18の紐は60cmほどの長さである。フードの大きさはカラー1のSサイズ、Mサイズ、Lサイズいずれにも対応できるものとなっている。もちろん、フード18は衣服12から着脱可能なものとしても良い。

図12は衣服12にカラー1を装着し、その外側をフード18で覆った状態を示す外見図である。このようにすることにより、患者の外出時にカラー1の奇異な外見を隠すことができ、親の羞恥心を和らげることができる。

なお、カラー1は、首の周囲に適度な間隔をもって保持できるのであれば、衣服12に接合しなくてもかまわない。その詳しいようすを図13に示す。カラー1に4箇所の取っ手19a、19b、19c、19dをつけ、この後面の取っ手19a、19bに襷がけとしたゴムベルト20を取り付ける。このゴムベルト20を患者の後背面より脇の下を通して、前面の取っ手19c、19dに取り付ける。このようにすることによって、カラー1を首の周囲に適度な間隔をもって取り付けることができる。これにより、患者の衣服を加工することなしに、かきむしり防止という当初の目的を達成することができる。

なお、カラー1は一つの部材ではなく、図14に示すように、大小2つの扇型の部材から形成されたものであってもよい。そのようすを図14に従って詳しく説明する。大きな部材21の内周部にはへり22が取り付けられている。そしてへり22には衣服12の襟部14にある凹の止め具15に接合するための凸の止め具23が取り付けられている。また、大きな部材21の片側の端部には凸の止め具24が2個、もう片方の端部には2個ずつ3列計6個取り付けられている。

一方、小さな部材25の両方の端部には、大きな部材21の凸の止め具24に対応する位置に凹の止め具26が2個ずつ取り付けられている。もちろん、これらの止め具は着脱可能な部材であれば何でも良く、たとえばホック、面ファスナー、紐、ベルトなどでもよい。面ファスナーであれば各部材の着脱がより容易であると共に、無段階のきめ細かい調節が可能となる。

【臨床実験】
本発明のカラー付衣服を昼夜着用した症例において臨床実験を行なった。

【対象被験者】
皮膚科の医師により「アトピー性皮膚炎」と診断され、かつ、加療の有無に拘わらず、掻痒を伴う皮膚症状を有する患者6名(年齢生後3カ月〜生後12カ月)を被験者とした。

【調査期間】
使用開始日から4週間。ただし、副作用的な弊害や急激な悪化がみられた者は直ちに使用を中止する事とした。

【皮膚症状の改善度について】
調査開始にあたり、保護者には調査報告用紙を渡し、アトピー性皮膚炎に伴う皮膚症状の情況について記入してもらった。

調査開始段階での皮膚症状と、調査開始4週間後の皮膚症状について「極めて改善」、「改善」、「不変」、「悪化」、「極めて悪化」の5段階で評定してもらった。以上については、本調査の趣旨を保護者に十分説明した上で、保護者の同意のもとに行われた。

上記臨床実験の結果、6名の被験者すべての症例において、「極めて改善」との報告がなされた。

【発明の効果】
本発明により、患者の手が直接患部に触れることを物理的に遮断できるため、患部への掻きむしり行為が完全に阻止される。その結果、アトピーの症状を早期に改善できるようになる。

また、本発明を複数の部材からなるものとすることにより、大きな部材を衣服に取り付けたまま患者の顔の正面部分を開放することができる。これにより、食事などの介護作業が容易となるという特別の効果を奏することができる。
