【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、脊柱ケージアッセ
ンブリに関する。
【０００２】
【従来の技術】隣接した椎骨間を効果的に結合するた
め、隣接した椎骨間の領域に個々に又は対をなして挿入
される、中空部材として形成された脊柱インプラントが
知られている。このようなインプラントは、例えば、欧
州特許第０ ３０７ ７４１ Ｂ１号、ドイツ国特許第
４４ １６ ６０５ Ｃ１号及びドイツ国特許第０ ６
３７ ４４０ Ａ１号、ＰＣＴ出願第ＷＯ９５／２６１
６４号、米国特許第５，０１５，２４７号、及びドイツ
国実用新案第Ｇ２９６ ００ ８７９号に示されてい
る。インプラントが中空構造であるため、骨の成長及び
結合を促すための骨材料等を受け入れることができる。
【０００３】更に、例えば椎骨の除去後、除去された椎
骨に代えて埋め込まれるいわゆる脊柱ケージアッセンブ
リが知られている。このような脊柱ケージアッセンブリ
は、例えば、ドイツ国実用新案第９１ ０７ ４９４号
及び欧州特許出願第０ １８８ ０５４号及び欧州特許
出願第０ ５３５ ２１５号から知られている。このよ
うな従来の脊柱ケージアッセンブリは、組織が比較的柔
らかいほぼ中央の領域で椎骨と係合する。従って、脊柱
ケージアッセンブリが椎骨に「食い込む」危険がある。
【０００４】比較的薄い金属網でできた脊柱ケージアッ
センブリにも同様の危険がある。
【０００５】
【発明が解決しようとする課題】本発明の目的は、効果
的な支持を提供し且つ隣接した椎骨間で新たな骨質（bo
ne substance）が成長できる脊柱ケージアッセンブリを
提供することである。
【０００６】
【課題を解決するための手段】この目的のため、本発明
は、人体と親和性の金属でできたスリーブ状部材を含む
脊柱ケージアッセンブリを提供する。スリーブ状部材
は、凹凸のある縁部を構成する端面、及び穴が設けられ
た壁を有する。
【０００７】本発明の脊柱ケージアッセンブリは、人体
と親和性の金属でできたスリーブ状部材からなる。この
スリーブ状部材は、大きな力を受け取って伝達するのに
適しており且つ比較的小さな表面圧力で隣接した椎骨と
係合できる比較的厚い壁を有する。これによって与えら
れた支持は、壁が椎骨とその皮質領域で係合するように
スリーブ状部材の直径が定められている場合、特に有効
である。周知のように、皮質は骨の最も硬質の領域であ
る。
【０００８】本発明によれば、スリーブ状部材の端面即
ち面縁部は、凹凸のある形状を有する。これにより、ス
リーブ状部材が回転移動しないようにする。更に、スリ
ーブ状部材の壁には窓が設けられており、この窓を通し
て骨材料をスリーブ状部材の内部に挿入できる。
【０００９】好ましくは、スリーブ状部材の凹凸のある
縁部は、スリーブ状部材の横方向軸線に対して例えば３
°乃至６°の所定角度で傾斜した平面内を延びている。
これによって、様々な解剖学的構造に適合させることが
できる。
【００１０】スリーブ状部材の端面には、歯及びこれら
の歯の間の隙間が設けられているのがよい。歯の先端即
ちチップは、脊柱ケージアッセンブリが椎骨に食い込ま
ないように平らな表面をなして形成されている。好まし
くは、隙間は、丸味のついた輪郭を有し、隙間と歯の頂
面とが比較的鋭い縁部を形成する。
【００１１】スリーブ状部材の組み立てを行うため、比
較的長いシャフトを有し且つねじ山を備えた延長部を自
由端に備えた工具を使用するのがよい。ねじ山を備えた
延長部は、取り外した椎骨の空間にスリーブ状部材を二
つの隣接した椎骨間の所定位置に挿入するため、スリー
ブ状部材のねじ穴と係合できる。
【００１２】脊柱ケージアッセンブリの高さは、取り外
した椎骨の高さ又は残りの隣接した椎骨間の間隔に応じ
て、及び脊柱での椎骨の位置に応じて変化する。従っ
て、従来技術の脊柱ケージアッセンブリは、一般的に
は、高さが調節自在である。本発明の脊柱ケージアッセ
ンブリでも調節を可能にするため、第１スリーブ状部材
を入れ子式に受け入れる第２スリーブ状部材を設けるの
が好ましい。第２スリーブ状部材は、スリーブ状内部材
と同じ形状を有する。更に、両スリーブ状部材には、互
いに整合するようになっており且つ二つのスリーブ状部
材を互いに関して動かないように保持するためにねじ山
を備えたボルトを受け入れるようになった穴が設けられ
ている。この位置にあるとき、スリーブ状内部材は、ス
リーブ状外部材から幾分突出している。一対のねじ穴が
設けられている場合には、二つの異なる相対位置で固定
できる。スリーブ状内部材の高さと相まって、実際に起
こる多くの場合に対して完全に満足がいく四つの異なる
高さが得られる。脊柱ケージアッセンブリの高さは、イ
ンプラントの埋め込み前に調節され、そのため、高さを
現場で調節するための困難な作業が必要でない。
【００１３】本発明の別の特徴によれば、高さを自動的
に調節できるようにするため、互いに入れ子をなして受
け入れられたスリーブ状部材間に力部材（force membe
r）等を設けることができる。例えば、夫々の固定手段
を解放したときにスリーブ状部材を互いから離すように
移動する押圧ばねを設けることができる。ばね以外の力
手段を使用してもよい。
【００１４】スリーブ状部材は、正方形又は多角形の断
面でもよいが、椎骨の隣接と一致する円形の断面が好ま
しい。
【００１５】上文中に説明したように、スリーブ状部材
を埋め込むための工具は、比較的長いシャフトを有す
る。このようなシャフトは、スリーブをその自由端に回
転自在に受け入れることができる。このスリーブはシャ
フトの肩部と係合し、スリーブ状部材の外形形状と一致
する所定形状の凹んだ凹所をその自由端に有する。この
ようにして、スリーブ状部材を埋め込むときに工具によ
って加えられる横方向力は、比較的薄いねじ山を備えた
延長部だけでなく、スリーブも介して伝達される。
【００１６】即ち、本発明は、人体と親和性の金属でで
きたスリーブ状部材を含み、上記スリーブ状部材は、凹
凸のある縁部を構成する端面及び穴が設けられた壁を有
する、脊柱ケージアッセンブリを提供する。
【００１７】上記端面のうちの少なくとも一方の上記凹
凸のある縁部は、上記スリーブ状部材の横方向軸線に対
して所定の角度で傾斜している。
【００１８】上記所定の角度は、３°乃至６°である。
【００１９】上記凹凸のある縁部には、歯及びこれらの
歯の間の隙間が設けられている。
【００２０】上記歯は、平らな表面として形成されたチ
ップを有する。
【００２１】上記隙間は、円弧形状をなしており、上記
隙間及び上記平らな表面はその間に比較的鋭い縁部を構

隔てられた少なくとも二つの半径方向穴を有し、上記ス
リーブ状内部材は、上記スリーブ状内部材が上記スリー
ブ状外部材から部分的に突出するように上記スリーブ状
外部材の上記半径方向穴と整合するようになった周方向
に間隔が隔てられた少なくとも二つのねじ穴を有し、上
記スリーブ状外部材は、上記凹凸のある縁部として形成
された端面を有し、上記半径方向穴及びねじ穴は、上記
スリーブ状内部材及び上記スリーブ状外部材を互いに関
して動かないように保持するため、ねじ山を備えたボル
トを受け入れるようになっている。

けられており、対をなした半径方向穴及びねじ穴の夫々
は、上記スリーブ状外部材又は上記スリーブ状内部材の
長さ方向軸線と平行な軸線に沿って縦に並んでいる。
【００２７】力部材が上記スリーブ状外部材と上記スリ
ーブ状内部材との間で作用して、その高さ調整を可能に
し、又、上記スリーブ状部材を夫々の位置に固定するた
めの固定部材が設けられている。

た延長部を自由端に備えた位置決め工具を受け入れるた
めの少なくとも一つの半径方向ねじ穴が設けられてい

ッセンブリ１０は、スリーブ状外部材１２及びスリーブ
状内部材１４からなる。両スリーブ状部材１２、１４は
壁が比較的厚く、人体と親和性の金属、好ましくはチタ
ニウムでできており、円形断面を有する。スリーブ状内
部材１４がスリーブ状外部材１２に入れ子式に受け入れ
られるような寸法になっている。更に、スリーブ状外部
材１２の直径及びスリーブ状内部材１４の直径は、脊柱
ケージアッセンブリを隣接した椎骨間に位置決めしたと
きに、脊柱ケージアッセンブリが実質的に皮質に対して
支持されるように選択される。図示の脊柱ケージアッセ
ンブリは、主に腰椎に適している。
【００３７】スリーブ状外部材１２には、図１及び図３
において参照番号１６を附した一対の細長い窓が設けら
れている。これらの窓は、骨材料を挿入するのに使用で
きる。
【００３８】スリーブ状外部材は、更に、縦に対をなし
て位置決めされた８個の半径方向穴１８及び２０を有す
る。一方の穴対は、スリーブ状外部材１２の長さ方向軸
線と平行な軸線に沿って、他方の穴対よりも上に位置決
めされている。隣接した穴対１８、２０は、図２及び図
３で最もよくわかるように、互いに関してずれている。
【００３９】最後に、スリーブ状外部材１２は、一対の
ねじ穴を有し、これらのねじ穴の一方に参照番号２２が
附してある。スリーブ状外部材１２の壁の様々な穴の目
的を以下に更に詳細に説明する。
【００４０】スリーブ状外部材１２の上側及び下側に
は、周方向に均等に間隔が隔てられた歯２４が設けられ
ている。これらの歯は、平らな上面２６を有する。歯の
間には円弧状の隙間２８が設けられ、これらの隙間は、
歯とともに、上面２６の領域に比較的鋭い縁部を構成す
る。従って、スリーブ状部材の上側及び下側はスリーブ
状部材の回転を阻止し、スリーブ状部材の埋め込み時に
回転を阻止する手段を提供する。
【００４１】スリーブ状内部材１４は部材１２と同様の
形状を持ち、上側及び下側に歯３２及び丸味を付けた隙
間３４が設けられている。スリーブ状内部材１４は、更
に、細長い形状を持つ対をなした窓３６を有し、これら
の窓は、窓１６と整合させることができる。更に、スリ
ーブ状内部材１４は、縦に対をなして位置決めされた４
個のねじ穴を有する。これらのねじ穴対のうちの二つの
対３８及び４０が図５に示してある。各対の穴は、互い
に垂直方向に位置決めされている。各対の穴は、スリー
ブ状外部材１２の対をなした穴１８、２０と整合する。
二つのねじ又はボルト４２、４４を穴１８及び２０の夫
々に挿入できる。スリーブ状外部材１２及びスリーブ状
内部材１４を第４図に示すように互いに関して動かない
ように保持するため、ねじ又はボルトの平滑な部分４６
が穴１８又は２０内に位置決めされ、ねじ山部分４８が
ねじ穴３８又は４０と螺合する。穴対１８、２０及びね
じ穴対３８、４０の夫々が高さに関して互いにずれてい
るため、スリーブ状外部材１２及びスリーブ状内部材１
４を様々な相対位置で固定できる。例えば、下側のねじ
穴対３８を下側の穴対１８と整合させた場合には、スリ
ーブ状内部材１４は、スリーブ状外部材１２からほんの
僅かしか突出しない。しかしながら、上側のねじ穴対４
０を上側の穴対２０と整合させた場合には、スリーブ状
内部材１４は、スリーブ状外部材１２から最大に突出す
る。
【００４２】スリーブ状内部材１４は、更に、ねじ穴５
０を有する。これらのねじ穴５０の一方を図１に示し、
図２に二つ示す。ねじ穴５０は、図６及び図８に示す工
具に以下に更に詳細に説明するように連結するため、ね
じ穴２２と同様の役割りを果たす。
【００４３】スリーブ状内部材１４は、図３に示すよう
に、上面が傾斜している。図５は、スリーブ状内部材１
４を別に示す。図示のように、上面は、横方向軸線に対
して６°の角度ａで傾斜しており、下面は３°の角度ｂ
で傾斜している。スリーブ状内部材１４のいずれの端部
を最初にスリーブ状外部材１２に挿入するのかで上面の
傾斜が決まる。これにより、夫々の解剖学的形状に適合
させることができる。
【００４４】スリーブ状内部材１４又はスリーブ状外部
材１２を単独で使用してもよいということは理解されよ
う。
【００４５】図６、図７、及び図８は、ハンドル５４、
長さが例えば２０cmのシャフト５６、及び前端に設けら
れた縮径延長部５８を持つ適当な組み立て工具５２を示
す。図７から明らかなように、延長部５８は、その自由
端にねじ山部分６０を有する。スリーブ６２が延長部に
取り付けられる。スリーブは、左端まで延びる雌ねじ穴
部分６４を持つ穴を有する。雌ねじ穴部分は、右端から
間隔が隔てられている。スリーブ６２の右端即ち自由端
には、スリーブ状内部材１４又はスリーブ状外部材１２
の丸味の付いた輪郭と一致する形状を有する凹んだ凹所
６６が設けられている。組み立て中、スリーブ６２を延
長部５８のねじ山部分６０と螺合させ、次いで一端をシ
ャフト５６の肩部６８と係合させる。その結果、スリー
ブ６２は、回転自在に取り付けられるが、ねじ山により
軸線方向に移動しないように固定され、捩じって外さな
ければ取り外すことができない。工具を図１乃至図５の
脊柱ケージアッセンブリに連結するとき、延長部５８の
ねじ山部分６０をねじ穴２２又はねじ穴５０にねじ込
み、スリーブ６２をスリーブ状部材壁の外側と噛み合う
ように回転させる。これにより、力をシャフト５６から
脊柱ケージアッセンブリに伝えることができる。

Claims (17)

    (57)【特許請求の範囲】
  1. 【請求項１】 スリーブ状外部材（１２）と、そのいず
    れかの端部から入れ子式に受け入れられるスリーブ状内
    部材（１４）とを備え、前記スリーブ状外部材とスリー
    ブ状内部材とが人体と親和性の金属でできた部品であ
    り、また前記スリーブ状外部材とスリーブ状内部材とに
    は複数の穴（１６，３６）が設けられ、互いに対して様
    々な高さ位置で固定されるようになっており、 前記スリーブ状外部材とスリーブ状内部材の少なくとも
    一方が、これら部材の横方向軸線に関して約３度〜６度
    の範囲で異なる角度に形成された２つの縁部を有し、該
    異なる角度を有する縁部は、前記スリーブ状内部材のい
    ずれの縁部を前記スリーブ状外部材の中に挿入するか
    で、上面の傾斜が決まることを特徴とする、脊柱ケージ
    アッセンブリ。
  2. 【請求項２】 前記端面のうちの少なくとも一方の前記
    凹凸のある縁部は、前記スリーブ状内部材（１４）の横
    方向軸線に対して所定の角度で傾斜している、請求項１
    に記載の脊柱ケージアッセンブリ。
  3. 【請求項３】 前記所定の角度は、３°乃至６°であ
    る、請求項２に記載の脊柱ケージアッセンブリ。
  4. 【請求項４】 前記凹凸のある縁部には、歯（２４、３
    ２）及びこれらの歯の間の隙間（２８、３４）が設けら
    れている、請求項１〜３のいずれかに記載の脊柱ケージ
    アッセンブリ。
  5. 【請求項５】 前記歯（２４、３２）は、平らな表面
    （２６）を有するチップを有する、請求項４に記載の脊
    柱ケージアッセンブリ。
  6. 【請求項６】 前記隙間（２８、３４）は、円弧形状を
    なしており、前記隙間（２８、３４）及び前記平らな表
    面（２６）はその間に比較的鋭い縁部を構成する、請求
    項５に記載の脊柱ケージアッセンブリ。
  7. 【請求項７】 前記スリーブ状外部材とスリーブ状内部
    材の壁に細長い穴（１６、３６）が設けられており、こ
    れらの穴の長さ方向軸線は、前記スリーブ状外部材（１
    ２）とスリーブ状内部材（１４）の長さ方向軸線と平行
    である請求項６に記載の脊柱ケージアッセンブリ。
  8. 【請求項８】 前記スリーブ状外部材（１２）の壁に
    は、ねじ山（６０）を備えた延長部（６０）を有する位
    置決め工具（５２）のねじ山（６０）を受け入れるため
    の少なくとも一つのねじ穴が設けられている請求項１〜
    ７のいずれかに記載の脊柱ケージアッセンブリ。
  9. 【請求項９】 前記スリーブ状外部材（１２）とスリー
    ブ状内部材（１４）とを互いに対して所定位置に一緒に
    固定する固定装置が設けられることを特徴とする請求項
    １〜８のいずれかに記載の脊柱ケージアッセンブリ。
  10. 【請求項１０】 前記スリーブ状外部材（１２）は、周
    方向に間隔が隔てられた少なくとも二つの半径方向穴
    （１８、２０）を有し、前記スリーブ状内部材（１４）
    は、前記スリーブ状内部材（１４）が前記スリーブ状外
    部材（１２）から部分的に突出するように前記スリーブ
    状外部材（１２）の前記半径方向穴（１８、２０）と整
    合するようになった周方向に間隔が隔てられた少なくと
    も二つのねじ穴（３８、４０）を有し、前記半径方向穴
    （１８、２０）及びねじ穴（３８、４０）には、ねじ山
    を備えたボルト（４２、４４）を挿入でき、それにより
    前記スリーブ状内部材（１４）及び前記スリーブ状外部
    材（１２）を互いに関して動かないように保持すること
    ができる、請求項９に記載の脊柱ケージアッセンブリ。
  11. 【請求項１１】 少なくとも２対の半径方向穴（１８、
    ２０）及びねじ穴（３８、４０）が設けられており、対
    をなした半径方向穴及びねじ穴の夫々は、前記スリーブ
    状外部材又は前記スリーブ状内部材の長さ方向軸線と平
    行な軸線に沿って半径方向穴及びねじ穴の一方が他方に
    重なって配置されている、請求項１０に記載の脊柱ケー
    ジアッセンブリ。
  12. 【請求項１２】 前記スリーブ状内部材（１４）及び前
    記スリーブ状外部材（１２）は、円形断面をする、請求
    項１〜１１のいずれかに記載の脊柱ケージアッセンブ
    リ。
  13. 【請求項１３】 前記ボルト（４２、４４）は、ヘッド
    及びステムを有し、前記ステムは、ねじ山部分（４８）
    及びこのねじ山部分と前記ヘッドとの間に位置する円筒
    形部分（４６）からなり、前記ステムは、前記スリーブ
    状外部材（１２）の前記半径方向穴（１８、２０）の直
    径と実質的に一致する所定の直径を有する、請求項１０
    〜１２のいずれかに記載の脊柱ケージアッセンブリ。
  14. 【請求項１４】 前記各縁部の角度は、互いに異なる、
    請求項２〜１３のいずれかに記載の脊柱ケージアッセン
    ブリ。
  15. 【請求項１５】 前記４つの凹凸のある縁部が、前記ス
    リーブ状内部材（１４）及び前記スリーブ状外部材（１
    ２）の横方向軸線に対して３°乃至６°である、請求項
    ２〜１４のいずれかに記載の脊柱ケージアッセンブリ。
  16. 【請求項１６】 前記スリーブ状内部材（１４）及び前
    記スリーブ状外部材（１２）の高さが実質的に同じであ
    る、請求項１〜１５のいずれかに記載の脊柱ケージアッ
    センブリ。
  17. 【請求項１７】 スリーブ状外部材（１２）と、該スリ
    ーブ状外部材（１２）に入れ子式に受け入れられるスリ
    ーブ状内部材（１４）とを備え、前記スリーブ状外部材
    とスリーブ状内部材とが人体と親和性の金属でできた部
    品であり、また前記スリーブ状外部材とスリーブ状内部
    材とには複数の穴（１６，３６）が設けられ、互いに対
    して様々な高さ位置で固定されるようになっており、 前記スリーブ状外部材とスリーブ状内部材の少なくとも
    一方が、これら部材の横方向軸線に関して約３度〜６度
    の範囲で異なる角度に形成された２つの縁部を有し、該
    異なる角度を有する縁部は、前記スリーブ状内部材のい
